Abstract

Description
本発明は外科手術用メッシュに関し、より詳細には、細菌および他の感染性物体の存在に向上した耐性を示す、柔軟でしなやかなマルチフィラメント外科手術用メッシュに関する。
(項目1) 柔軟で曲げやすい柔軟な組織メッシュであって、糸のフィラメント間に存在する間質空隙がマトリックス内に封入されている、耐感染性マトリックス内に包み込まれた複数のフィラメント糸(yarn)からなる支持用格子を含む、メッシュ。
(項目2) 項目1に記載のメッシュであって、上記マトリックスは、少なくとも上記糸のフィラメントを構成する素材の柔軟性に匹敵する柔軟性を有する素材からなる上記糸の外周を包囲する、メッシュ。
(項目3) 項目2に記載のメッシュであって、上記マトリックスは、上記糸に浸透して存在する間質空隙を実質的に満たす、メッシュ。
(項目4) 項目3に記載のメッシュであって、上記マトリックスは、ポリエステル、ポリプロピレン、ポリエチレン、ポリウレタン、ポリ(メタ)アクリレート、およびそれらの共重合体からなるグループより選択された重合体からなる、メッシュ。
(項目5) 項目1に記載のメッシュであって、上記糸は、柔軟な芯と上記芯を包囲する融解(fuse)可能なシースを各々有する多数の2要素フィラメントからなり、シースは上記格子を加熱した際に上記間質空隙を閉塞すべく上記シースを互いに融解させる、芯の融点よりも低い融解温度を有する、メッシュ。
(項目6) 項目5に記載のメッシュであって、上記芯は、ポリエチレンテレフタレート・ポリエステルからなり、上記シースは、ポリエチレンテレフタレート/イソフタレート・コポリエステルからなる、メッシュ。
(項目7) 上記マルチフィラメント糸が上記支持用格子を形成するために規則正しく間隔を置いた接続部で互いに重なりあっており、上記重なり合う糸が上記メッシュのもつれ抵抗を高めるために、上記接合部で互いに接着している、項目1に記載のメッシュ。
(項目8) 上記マトリックスが薬品浸透性であり、上記メッシュを移植後、薬品が患者の体内に放出されるために上記マトリックスに組み込まれる、項目1に記載のメッシュ。
(項目9) 上記メッシュの厚みが約0.2ミリメートル以下である、項目1に記載のメッシュ。
(項目10) 上記マルチフィラメント糸が撚れずに分かれている、項目1に記載のメッシュ。
(項目11) 上記マルチフィラメント糸がほぼ楕円状の断面を有する、項目10に記載のメッシュ。
(項目12) マルチフィラメント糸で作られた支持用格子から入る感染物の存在に対して抵抗力の増加を示す柔軟で曲げ易い外科用のメッシュの生産方法であって、上記糸の間に置かれる間質空隙が、マトリックスで囲まれている非感染マトリックス内にある上記マルチフィラメント糸を包むことを含む、方法。
(項目13) 上記包むステップが上記格子の形成の前に、上記糸の外側に樹脂を付けるステップを含む、項目12に記載の方法。
(項目14) 上記樹脂が上記糸に浸透し、空隙を十分充満するように上記格子の外側に十分な量の樹脂を十分な時間をかけて付ける、項目13に記載の方法。
(項目15) 上記包むステップが格子の形成後、上記糸の外側に樹脂を付けるステップを含む、項目12に記載の方法。
(項目16) 上記樹脂が上記糸に浸透し、上記空隙を十分充満するように上記格子の外側に十分な量の樹脂を十分な時間をかけて付ける、項目15に記載の方法。
(項目17) 上記糸が各々弾力性のある芯と上記芯の周りに可溶性シースを持つ複数の2要素フィラメントから構成され、上記包むステップが上記シースがともに融解する上記シースの融解温度より上で、上記格子を加熱するステップを含む、項目12に記載の方法。
(項目18) 上記融解ステップが、上記格子に熱または光波放射線を適用して達成される、項目17に記載の方法。
(項目19) 患者身体への上記メッシュの移植後、そこからの放出のため、上記マトリックスへ医療薬剤を取り込むステップをさらに含む、項目12に記載の方法。
(項目20) 滅菌したパッケージ中で、上記包まれた格子を密封するステップをさらに含む、項目12に記載の方法。
(項目21) 項目12に記載の方法であって、所望の厚みのメッシュをプレスするために圧縮力を加えるステップをさらに含む、方法。
(項目22) 患者の身体の損傷を受けた部分を修復する方法であって、柔軟な組織メッシュを提供すること、上記メッシュが耐感染性マトリックス内で包まれたマルチフィラメント糸から形成された支持用格子を含み、それによって、上記メッシュは柔軟でかつ曲げやすく、同時に感染性物体の存在に耐性を示すこと、上記身体の上記損傷を受けた部分に接近すること、上記損傷を受けた部分を補強するために、上記身体中に上記外科手術用メッシュを移植し、上記身体に、上記メッシュを同化させることを含む、方法。
(項目23) 項目22に記載の方法であって、上記糸はそれぞれ柔軟性のある芯および該芯を囲む可溶性シースを有する複数の2要素フィラメントから形成され、上記提供する工程が上記シースの溶融温度より上で上記格子を加熱するステップを含み、それによって上記シースがともに溶けて上記芯を包み込む、方法。
(項目24) 項目22に記載の方法であって、上記マトリックスは薬剤透過性であり、上記メッシュの移植後に患者の身体へ放出するための上記マトリックスに医療薬剤が組み入れられている、方法。
(項目25) 項目1に記載の柔軟組織メッシュであって、上記格子が50ミクロンから400ミクロンの範囲のサイズを有する細孔を規定する、メッシュ。
(項目26) 項目7に記載の柔軟組織メッシュであって、上記格子が0.05ミリメートルから0.5ミリメートルの範囲の厚さを有する、メッシュ。
(項目27) 項目7に記載の柔軟組織メッシュであって、上記格子が0.20ミリメートルより薄い厚さを有する、メッシュ。
(項目28) 項目27に記載の柔軟組織メッシュであって、上記格子が約5ミリメートルまたはそれより小さい直径を有するトロカールへの挿入が可能となるのに十分に薄い、メッシュ。
(項目29) 項目28に記載の柔軟組織メッシュであって、上記メッシュが広げられた状態で形状記憶を実現するのに十分な温度で十分な時間加熱状態にされる、メッシュ。
(項目30) 項目28に記載の柔軟組織メッシュであって、上記メッシュが形状記憶材質で作られた糸またはフィラメントを含む、メッシュ。
(発明の要旨)
本発明は、当該技術の必要性を提起し,柔軟で曲折可能な外科手術用メッシュを提供する。このメッシュは、耐感染性マトリックスに包まれた、マルチフィラメント糸(yarn)で形成される支持用格子を合み、それにより糸のフィラメント間に置かれた間質空隙は、マトリックス内に封じられる。また、このマトリックスはメッシュがほどけないための必要な抵抗を付与し、それによって糸が一ヶ所に片寄ったり、ほどけたりするのを防ぐ。このメッシュは微細なマルチフィラメント糸により編物状または織物状に構成され、これは組織の修復または補強で使用されるのに望ましい機械的強度および有孔性を有する。
図、特に図1には、従来の発明である外科手術用支持メッシュ10が図示されている。メッシュ10はモノフィラメントまたはマルチフィラメントの糸から製造できる。図のとおり、従来のメッシュ10は横に伸びているマルチフィラメント糸12と縦に伸びているマルチフィラメント糸14が編み込まれて支持用格子を構成している。
以下の実施例は本発明における外科手術用支持メッシュを説明する。
支持用格子は、特に250デニール/16フィラメント/タイプのLHCV鐘紡Bellcouple(登録商標)ポリエステル糸による2要素糸で織られている。鐘紡Bellcouple(登録商標)糸には、ポリエチレンテレフタレート・ポリエステル芯、および芯よりも融点が低いシースであるポリエチレンテレフタレート/イソフタレート・コポリエステル・シースが含まれる。
支持用格子は、特に75デニール/24フィラメント/タイプのLHCV鐘紡Bellcouple(登録商標)ポリエステル糸による2要素糸で縦編みされている。サポート・格子が構築された後、格子は対流式オーブンにかけられ、およそ210℃まで加熱することにより個々のシースを融解する。
支持用格子は、特に75デニール/24フィラメント/タイプのLHCV鐘紡Bellcouple(登録商標)ポリエステル糸による2要素糸で単純織布状に織られている。糸数は1インチにつき70端数および1インチにつきピック数70本である。長方形孔の寸法は各面50から70ミクロメートルの範囲である。
支持用格子が構築された後、格子は強制熱空気対流式オーブンにかけられ、およそ210℃まで15分間加熱することにより個々のシースを融解する。構造の厚みはおよそ0.10ミリメートル以下である。図9は本例に基づいた構造の一部を示した顕微鏡写真である。図9aは同構造の断面図を示した顕微鏡写真である。
