Abstract
Description
本発明は、制御冷却による脂肪性細胞の選択的破壊に用いる方法に関する。本発明はさらに、制御冷却により脂肪性細胞を選択的に破壊するための方法において使用するための、ならびに、選択的破壊方法の前、その間およびその後に生じる、あるいは他の状態で存在する状態についてモニタリングし、検出し、影響を及ぼしまたは作用するための、装置に関する。本発明の他の局面は、以下の開示に記載されており、または以下の開示から明らかである(また、本発明の範囲内である)。
新生児の皮下脂肪組織は、寒さに対して非常に敏感である。新生児では、皮下脂肪細胞、すなわち「脂肪細胞」の細胞内脂質含量は、高い割合の高飽和トリグリセリドを含む。中程度の寒い気温ですら、高飽和脂質を含む細胞には悪影響を及ぼすことがあり、寒さに曝されると新生児皮下脂肪組織は脂肪細胞壊死を起こしやすくなる。皮下脂肪組織の低体温は、真皮および/または表皮の関連炎症を引き起こす場合がある。例えば、新生児における寒冷脂肪組織炎障害は、有痛性皮膚病変をもたらすことが分かっている。
現在では、脂肪性細胞を含む脂肪組織は、その周囲の非脂肪性組織(例えば、真皮および上皮組織)を傷つけずに、その各々の組織に適用する温度および/または圧力を制御することによって選択的に破壊することができることが分かっている。

本発明の方法を、白色、6ヶ月齢、雌、ハンフォードミニチュアブタ(「ブタI」)および黒色、6ヶ月齢、雌 ユカタンミニチュアブタ(「ブタII」)で実施した。テラゾール/キシラジン(4.4mg/kg im+2.2mg/kg im)を用いてブタに麻酔をかけた。注射用麻酔薬によって身体に十分な鎮痛作用が得られない場合に限り、吸入麻酔薬(ハロタンまたはイソフラン(1.5〜3.0%))を酸素(3.0L/分)と共にマスクによって送り、F−エアーキャニスターで濾過した。いくつかの試験部位を各試験部位の隅に墨を入れることによる、マイクロタトゥー(micro tattoos)を用いてマークした。試験部位のマッピング後、図1Aに記載の冷却装置を用いて寒冷曝露を実施した。処置インターフェース領域は、内蔵温度センサーを備えたサイズ2×4cm2の平坦領域であった。インターフェースは、インターフェースの表面温度を事前設定した温度に一定にするように、制御装置により電子的に調節される熱電冷却装置と熱的接触させた。寒冷曝露中、血流の重大な機械的圧迫を起こさない低〜中位の圧力で、冷却装置を皮膚に当てた。表面プロフィールの操作をしないで、冷却素子を皮膚に適用した。
種々の組織深度における温度プロフィール測定
本研究を、6ヶ月齢、雌、黒色、無毛ユカタンミニブタ(Sinclair Research Center, Columbia, MO)を用いて実施した。テラゾール/キシラジン(4.4mg/kg im+2.2mg/kg im)を用いてブタに麻酔をかけた。注射用麻酔薬によって身体に十分な鎮痛作用が得られない場合に限り、酸素を合わせた吸入麻酔薬(ハロタンまたはイソフラン(1.5〜3.0%)をマスクによって送り(3.0L/分)、F−エアーキャニスターで濾過した。試験部位をマイクロタトゥーを用いて各試験部位の隅に墨を入れ、皮下注射針をその試験部位の隅に挿入することによりマークした。寒冷曝露は、−7℃に調節された循環冷却剤により冷却される、熱交換器に取り付けられた凹面円銅板を用いて実施した。曝露時間は、600〜1200秒間であった。表3では、冷却適用のパラメーターとブタIIIの様々な部位で得られた結果を示している。冷却板は、直径およそ1mmの3つの中央開口部を有し、それを通じて、寒冷曝露中の異なる組織深度における温度プロフィールをモニタリングするために、熱電対を取り付けた。図9に示す寒冷曝露装置は、寒冷曝露中、試験部位でしっかりと維持された。寒冷曝露は、1週間あけた異なる2試験日に実施した。第1回目の試験日では、寒冷曝露中、熱電対を時々外したところ、熱電対深度測定で0.5mmの変動が生じた。第2回目の試験日には、熱電対の深度の変動が極微ないし無い明確な深度で、熱電対を用いてさらなる一連の曝露を実施した。第1回目の試験日での試験部位1、2、3、7、11および12についての熱電対の位置は、深度2.5、4.5および10mm(+/−0.5mm)の地点であった。第2回目の試験日では熱電対深度2、4および8mm(変位は極微〜なし)で試験部位14、15、16および18を処置した。熱電対深度の特定の変動性は、寒冷曝露中の組織の圧縮によって生じ得る。皮膚表面での良好な熱的接触を確保するために、グリコール含有溶液を用いた。犠牲にし、解析のために試験部位の組織を摘出するまで、ブタを処置後3 1/2ヶ月間観察した。表3では、冷却適用のパラメーターとブタIIIの様々な部位で得られた結果を示している:
皮下脂肪の経時的な選択的減量
処置の数ヵ月後、標準的な食餌を与えたブタIIIでは皮下脂肪の選択的減量が維持された。本明細書においてさらに詳細に記載したように、瘢痕形成または皮膚損傷は観察されなかった。
皮下脂肪の経時的な選択的減量
処置過程中の相転移を示す温度プロットを、ミニチュアユカタンブタ(minature Yuacatan pig)を用いて作成した。−5℃に調節した冷却剤を循環させる、銅板を含む冷却装置を適用部位と接触させて約600秒間置いた。この試験の目的では、表皮の温度は0℃を下回らなかった。
Claims (109)
- 非乳幼児処置対象における脂肪性細胞の選択的破壊のための冷却方法であって、
前記処置対象の適用部位の近位に少なくとも1つの冷却素子を適用し、
前記適用部位の真下の温度を低下させるために、前記冷却素子を利用し(ここで、その温度の低下が、そこにある脂肪性細胞を破壊し、それによって減少させるのに十分である)、
温度を低下させる間に、前記冷却素子に近位の非脂肪性細胞が破壊されないことを確認するための十分なフィードバック情報を提供するために、少なくとも1つのフィードバック装置を利用し、それにより、前記対象における脂肪性細胞を選択的に破壊する、
工程を含む、方法。 - フィードバック情報が真皮、表皮または皮下脂肪組織の少なくとも1つの生理的パラメーターについての情報を含み、前記情報が、冷却素子に近位の非脂肪性細胞が破壊されていないことを示す、請求項1に記載の方法。
- 温度の低下が対象の真皮、表皮および皮下脂肪組織にわたる局所的温度勾配を生じさせる、請求項1に記載の方法。
- 脂肪性細胞が皮下脂肪組織内の脂肪細胞である、請求項1に記載の方法。
- 冷却素子が適用部位と接する、請求項1に記載の方法。
- 生理的パラメーターが対象の表皮の温度である、請求項2に記載の方法。

前記少なくとも1つの非侵襲的冷却素子と接触している直径約2cm未満の変換器を備えた超音波に基づくフィードバック装置(前記フィードバック装置は、前記冷却素子に近位の非脂肪性細胞が破壊されないことを確認するための十分なフィードバック情報を提供するのに効果的である)
を含む、システム。 - 非乳幼児処置対象における脂肪性細胞の選択的破壊に関連してフィードバック情報を提供するシステムであって、
前記処置対象の適用部位に近位の少なくとも1つの冷却素子(前記冷却素子は、前記適用部位の真下の温度を低下させるのに効果的であり、その温度の低下が、前記適用部位の真下の脂肪性細胞を破壊し、それによって減少させるのに十分である);および
少なくとも1つの冷却素子と通信している少なくとも1つのフィードバック装置(前記フィードバック装置は、前記冷却素子に近位の非脂肪性細胞が破壊されないことを確認するための十分なフィードバック情報を提供する)
を含む、システム。 - 少なくとも1つのフィードバック装置が少なくとも1つの冷却素子と接続されている、請求項39に記載のシステム。
