Abstract
Un dispositivo (100) de grapado quirúrgico accionable para comprimir un tejido antes de la inserción de una grapa quirúrgica en el tejido y para insertar la grapa quirúrgica en el tejido, en combinación con: un sustrato (10) para soportar el tejido, caracterizado porque el sustrato está hecho de un material predeterminado, en el que el material predeterminado tiene un color inicial cuando no es aplicado un esfuerzo al sustrato, y en el que el material predeterminado tiene un segundo color cuando es aplicado un esfuerzo de compresión predeterminado al sustrato, indicando el segundo color la compresión de tejido apropiada a fin de disparar la grapa quirúrgica al tejido.
Description
Material de apoyo regulado mecánicamente para

tejido afectado.
La presente exposición está dirigida a
dispositivos de grapado quirúrgico y, en particular, a un
dispositivo de apoyo usado en conexión o en combinación con un
instrumento quirúrgico para asegurar que se aplica una cantidad
óptima de compresión del tejido al tejido para una formación óptima
de grapas y suturas. Incluso más particularmente, la presente
exposición está dirigida a un dispositivo de apoyo que proporciona
una indicación visual al cirujano de que la cantidad óptima de
compresión del tejido ha sido alcanzada, y de que es el instante
apropiado para aplicar el elemento quirúrgico.
La anastomosis es la unión quirúrgica de
secciones de órganos huecos separados. Típicamente, un procedimiento
anastomótico sigue a la cirugía en la que se ha retirado una
sección afectada o defectuosa de tejido. Después de ello, el
procedimiento tiene las operaciones de que las secciones del tejido
extremas restantes han de ser unidas. Dependiendo del procedimiento
anastomótico deseado, las secciones de extremidad pueden ser unidas
bien por métodos de reconstrucción de órganos circular, de extremo a
extremo, de extremo a lado, o de lado a lado.
En un procedimiento de anastomosis circular
conocido, dos extremos de secciones de órgano son unidos por un
dispositivo de grapado. El dispositivo de grapado puede lanzar una
agrupación circular de grapas a través del extremo de cada sección
de órgano. El dispositivo puede simultáneamente retirar cualquier
tejido interior de la agrupación circular de grapas lanzadas para
liberar un paso tubular. Muchos ejemplos para realizar anastomosis
circulares de órganos huecos están descritas en las patentes
norteamericanas nº 6.959.851, nº 6.053.390, nº 5.588.579, nº
5.119.983, nº 5.005.749, nº 4.646.745, nº 4.576.167, y nº
4.473.077.
Típicamente, estos dispositivos incluyen un
árbol alargado que tiene una parte de empuñadura en un extremo
proximal del mismo para efectuar el accionamiento del dispositivo.
El dispositivo tiene también un componente de contención de grapas
dispuesto en un extremo distal del mismo. Un conjunto de yunque que
incluye un vástago de yunque con una cabeza de yunque unida está
montado en el extremo distal del dispositivo. El yunque es adyacente
a un componente de contención de grapas. Partes de extremidad
opuestas de tejido del órgano u órganos huecos que han de ser
grapados son sujetadas entre la cabeza del yunque y el componente de
contención de grapas. El tejido sujeto es grapado lanzando una o
más grapas que tienen un tamaño predeterminado desde el componente
de contención de grapas. De esta manera, los extremos de las grapas
pasan a través del tejido y son deformados por la cabeza del
yunque. Una cuchilla anular es hecha avanzar para retirar el tejido
de dentro del órgano hueco. De esta manera, la cuchilla libera un
paso tubular dentro del órgano.
Dispositivos de grapado quirúrgicos para
realizar anastomosis circulares han sido también usados para tratar
hemorroides internas en el recto. Durante el uso de un dispositivo
de grapado circular para el tratamiento de hemorroides, la cabeza
del yunque y el componente de contención de grapas del dispositivo
quirúrgico son insertados a través del ano y al recto con la cabeza
del yunque y el componente de contención de grapas en una posición
abierta o no aproximada. Después de ello, es usada una sutura para
estirar del tejido hemorroidal interno y/o del tejido de la mucosa
hacia el vástago del yunque. A continuación, la cabeza del yunque y
el componente de contención de grapas son aproximados para sujetar
el tejido hemorroidal y/o el tejido de la mucosa entre la cabeza
del yunque y el componente de contención de grapas. El dispositivo
de grapado es disparado para retirar o eliminar el tejido
hemorroidal y/o el tejido de la mucosa y grapar el tejido cortado.
Las suturas son también conocidas en la técnica para conectar o
unir tejidos. Aunque el uso de grapadoras de anastomosis circular
para el tratamiento hemorroidal tiene muchos beneficios a menudo, un
cirujano encontrará, durante el curso de un procedimiento
quirúrgico, uno o más tipos diferentes de tejidos en el cuerpo al
que aplicar un elemento quirúrgico tal como una grapa.
Los dispositivos de grapado no circulares son
también conocidos en la técnica. En los procedimientos endoscópicos
o laparoscópicos, la cirugía es realizada a través de una pequeña
incisión o a través de una estrecha cánula insertada a través de
una pequeña entrada y practicada en la piel. A fin de tratar las
necesidades específicas de los procedimientos quirúrgicos
endoscópicos y/o laparoscópicos, se han desarrollado dispositivos de
grapado quirúrgico endoscópico y están descritos por ejemplo en la
patente norteamericana nº 5.040.715 (Green y col.); en la patente
norteamericana nº 5.307.976 (Olson y col.); en la patente
norteamericana nº 5,312.023 (Green y col.); en la patente
norteamericana nº 5.318.221 (Green y col.); en la patente
norteamericana nº 5.326.013 (Green y col.); en la patente
norteamericana nº 5.332.142 (Robinson y col.); y en la patente
norteamericana nº 6.241.139 (Milliman y col.). Estos dispositivos
de grapado no circulares o lineales son útiles para la cirugía
gastrointestinal o bariátrica e incluyen un cartucho de grapas y un
yunque para aplicar grapas quirúrgicas de titanio al cuerpo para
unir o conectar secciones de tejidos entre sí.
Algunos otros tipos de tejido incluyen el tejido
cardíaco, el tejido colorrectal, el tejido gastrointestinal, y el
tejido pulmonar. En estos diferentes tipos de tejidos, puede haber
otros varios tipos diferentes de clases de tal tejido, tales como
el tejido isquémico, o tejido afectado o enfermo, tejido grueso,
tejido tratado con medicinas o compuestos, el tejido diabético, así
como otros numerosos. Sin embargo, un experto en la técnica debería
apreciar que la presente exposición no está limitada al tejido
afectado o enfermo.
La patente norteamericana nº 5.441.193 describe
un aparato de aplicación de un sujetador quirúrgico que sujeta el
tejido entre dos miembros móviles. Una lámina de material elástico
curvada es unida a uno o a ambos miembros. El preámbulo de la
reivindicación 1 está basado en este documento.
El documento EP 1.064.883 describe un aparato de
grapado que tiene un apoyo de esponja bioabsorbible conforme unido
al menos a una mordaza del aparato de grapado quirúrgico.
Se desea asegurar la formación adecuada del
elemento quirúrgico respectivo (tal como la agrupación de grapas)
en tal tejido. Se ha observado que con ciertos tipos de tejidos
tales como el tejido isquémico, o tejido diabético puede aplicarse
una cantidad de compresión al tejido durante un período de tiempo
óptimo para ayudar al instrumento.
Sin embargo, una compresión adicional durante un
periodo de tiempo (después de un periodo de tiempo óptimo) no es
beneficiosa. En el entorno quirúrgico, es difícil apreciar
visualmente la cantidad óptima de compresión que debería aplicarse
a los distintos tipos de tejido, y también es difícil apreciar
visualmente el periodo de tiempo óptimo para la compresión del
tejido. Debería apreciarse que la compresión del tejido no está
limitada al tejido afectado o enfermo y se ha considerado que la
compresión pueda ser aplicada a otros casos en los que no se ha
encontrado tejido no afectado o enfermo en el procedimiento
quirúrgico para asegurar un resultado quirúrgico positivo.
Consiguientemente, existe una necesidad
continuada en la técnica para un dispositivo que proporcione una
indicación al cirujano de que se ha alcanzado una compresión de
umbral y que debería aplicarse el elemento quirúrgico al tejido
(tal como una grapa). Existe una necesidad continuada en la técnica
para un dispositivo que es usado en conexión con un instrumento
quirúrgico que puede actuar como un intermediario entre el
instrumento y el tejido y así el tejido es comprimido de manera
óptima por el intermediario para una formación apropiada del
elemento quirúrgico tal como una grapa sin dañar el tejido. Hay
también una necesidad para un material que pueda soportar el tejido
enfermo delgado durante el grapado y que así la compresión no pueda
dañar el tejido. Hay también una necesidad en la técnica para un
dispositivo que pueda reforzar una línea de grapas y redistribuir
el gradiente de presión que forma la línea de grapas más
uniformemente sobre un área del tejido sano o enfermo.
El presente invento proporciona un dispositivo
de grapado quirúrgico accionable para comprimir el tejido antes de
la inserción de una grapa quirúrgica en el tejido y para insertar la
grapa quirúrgica en el tejido en combinación con un sustrato para
soportar el tejido, caracterizado porque el sustrato está hecho de
un material predeterminado, en que el material predeterminado tiene
un color inicial cuando no se aplica esfuerzo al sustrato, y en que
el material predeterminado tiene un segundo color cuando se aplica
un esfuerzo de compresión predeterminado al sustrato, indicando
segundo color la compresión del tejido apropiada con el fin de
disparar la grapa quirúrgica al tejido.
La indicación no está limitada a una indicación
previa al disparo y puede también ser una indicación de una
condición posterior al disparo. El sustrato puede permanecer
conectado al lugar de la operación a la conclusión del disparo del
instrumento quirúrgico. Así, el sustrato que tiene la indicación
permanece en el sustrato, y el cirujano puede inspeccionar la
indicación en cualquier momento después del disparo. El sustrato
puede así también proporcionar una indicación de la compresión que
se ha aplicado al tejido en el disparo en un periodo de tiempo
después del disparo del instrumento.
El material predeterminado puede tener un
espesor y un perfil de esfuerzo-tensión
predeterminado. El perfil de esfuerzo-tensión
predeterminado es complementario de un perfil de
esfuerzo-tensión del tejido para permitir que el
sustrato soporte el tejido cuando el elemento quirúrgico es
entregado al tejido. El sustrato proporciona control de la
hemostasis del tejido cuando el elemento quirúrgico es entregado al
tejido. El sustrato puede estar hecho de colágeno, polímeros
degradables, o polisacáridos. En una realización, el sustrato puede
tener múltiples capas o ser material compuesto que tiene una capa
de soporte y una capa de banda. El sustrato puede tener una capa de
soporte de colágeno y una banda que proporciona soporte y
resiliencia a la capa de soporte de colágeno.
Descrito aquí, pero no reivindicado, hay un
método para determinar una cantidad óptima de compresión de tejido.
El método proporciona una indicación al cirujano de la compresión
óptima y un instante para disparar un elemento quirúrgico al
tejido. El método tiene las operaciones de aplicar una película
sensible a la presión al tejido. En otra realización alternativa,
el método puede tener las operaciones de aplicar una espuma sensible
a la presión, una película que tiene partículas, un gel, un
adhesivo o una combinación de ellos al tejido como un dispositivo
de diagnóstico para proporcionar la indicación. La película sensible
a la presión está configurada para proporcionar una indicación
cuando es comprimida a un nivel de esfuerzo de compresión
predeterminado. La película está regulada de tal manera que el
nivel de esfuerzo de compresión predeterminado indica la compresión
de tejido óptima del tipo de tejido. El método tiene las operaciones
de comprimir la película sensible a la presión y el tejido y el
disparo del instrumento cuando la película sensible a la presión
proporciona una indicación visual de la compresión óptima.
También descrito aquí, pero no reivindicado, hay
un método de determinación de una cantidad óptima de compresión del
tejido para proporcionar una indicación al cirujano de la compresión
óptima y un instante para disparar un elemento quirúrgico al
tejido. El método tiene la operación de aplicar un sustrato que
tiene un saco lleno de tinte que tiene un depósito. El saco está
configurado para reventarse o romperse cuando es comprimido a un
nivel de esfuerzo de compresión predeterminado. El saco está
configurado para dar esto como resultado cuando el nivel de
esfuerzo predeterminado es la compresión de tejido óptima del tipo
de tejido. El método tiene también la operación de comprimir el
sustrato junto con el tejido y disparar el instrumento cuando el
tinte es liberado del saco. El tinte puede ser un colorante, o un
pigmento, o incluir además una sonda bioquímica la totalidad de los
cuales pueden ser coloreados o emitir color al producirse la
excitación por radiación de baja energía.
En una realización, el sustrato puede tener un
saco lleno de tinte que tiene un cierto color. El tinte coloreado
será liberado del saco una vez que el saco se rompa y permite que el
cirujano aprecie visualmente que se ha alcanzado la compresión
óptima. En otra realización, el saco puede tener una sustancia
química predeterminada en el saco. Una vez que se ha alcanzado la
compresión predeterminada, la sustancia química predeterminada es
liberada y puede reaccionar con el sustrato u otro material para
cambiar de color una vez que se rompe el saco. Esta reacción
química permite al cirujano apreciar visualmente de manera
instantánea que se ha alcanzado la compresión óptima.
Aún en otra realización de la presente
exposición, el sustrato puede estar formado con una pluralidad de
cristales líquidos. Los cristales líquidos están hechos con el
sustrato y pueden ser colados o moldeados in situ cuando el
sustrato es fabricado. En una realización, los cristales líquidos
son colados en un material polímero y el sustrato polímero es
extrudido desde una matriz. Los cristales líquidos tienen un estado
inicial predeterminado y un segundo estado cuando son comprimidos
con la cantidad de compresión óptima. Una vez que se ha alcanzado
la compresión predeterminada en el procedimiento y aplicado al
sustrato que tiene la pluralidad de cristales líquidos, los
cristales líquidos pueden cambiar una propiedad óptica de al menos
uno de los cristales líquidos y el sustrato en respuesta a la
compresión para proporcionar la indicación. Esta indicación permite
que el cirujano aprecie visualmente de manera instantánea que se ha
alcanzado la compresión óptima.
Aún en otra realización de la presente
exposición, el sustrato puede ser formado con una pluralidad de
microesferas. Las microesferas son fabricadas con el sustrato para
incluir un material. En una realización, las microesferas tienen un
tamaño que es de un orden menor con relación a los sacos de la
realización previa. Una vez que se ha alcanzado la compresión
predeterminada en el procedimiento y aplicado al sustrato que tiene
las microesferas, las microesferas pueden romperse para liberar su
contenido y proporcionar una indicación en respuesta a la
compresión. Esta indicación permite que el cirujano aprecie
visualmente de manera instantánea que se ha alcanzado la compresión
óptima.
Aún en otra realización de la presente
exposición, el sustrato puede ser formado con una pluralidad de
nanoesferas. Las nanoesferas son fabricadas con el sustrato para
incluir un material en un lumen formado en cada una de las
nanoesferas. En una realización, las nanoesferas tienen un tamaño
minúsculo con relación a los sacos de la realización previa. Una
vez que se ha alcanzado la compresión predeterminada en el
procedimiento y es aplicada al sustrato que tiene las nanoesferas,
las nanoesferas pueden romperse para liberar su contenido y
proporcionar una indicación en respuesta a la compresión. Esta
indicación permite que el cirujano aprecie visualmente de manera
instantánea que se ha alcanzado la compresión óptima. Cada una de
las nanoesferas puede romperse a diferentes de gradientes de
esfuerzo para proporcionar un número de indicaciones diferentes
dependiendo de la compresión encontrada específica en el
procedimiento.

En una realización, una platina móvil y una estacionaria (no
mostradas) pueden ser usadas con el fin de comprimir el tejido para
ayudar con la aplicación del elemento quirúrgico al tejido. La
presente exposición se pretende que sea usada con un dispositivo
grapador conocido en la técnica, y la referencia a cualquier tipo
particular de dispositivo grapador es simplemente como ilustración
solamente y no debería ser construida para limitar la presente
exposición. El tejido puede ser cualquier tejido gastrointestinal,
tejido abdominal, tejido pulmonar, tejido de colón, tejido
cardíaco, músculo o cualquier otro tejido de mamífero cuando el
grapado y las suturas pueden ser aplicadas para tratamiento
terapéutico. Las propiedades del tejido pueden ser determinadas bien
por ensayos estáticos o dinámicos. Con referencia a la fig. 2, el
estiramiento o extensión (\lambda) es definido como una relación
del espesor deformado del tejido a un espesor sin deformar como se
ha mostrado posteriormente en la que h_{f} es la altura final del
tejido y h_{i} es la altura inicial del
tejido:

El esfuerzo (\sigma) es definido como la
fuerza (f) dividida por el área de contacto (A) de la platina de
carga.

La tensión es derivada del estiramiento
(\lambda) y es igual a 1 menos el estiramiento (\lambda). Como
se ha mostrado en la fig. 2, el esfuerzo en libras por pulgada
cuadrada está trazada en el eje y, y la tensión está trazada en el
eje x. Como puede comprenderse a partir del gráfico, se han mostrado
varios perfiles de esfuerzo-tensión para cada uno
de los tipos de tejido diferentes incluyendo un primer y segundo
tejidos de estómago, un primer y segundo tejidos de colon, y un
primer y segundo tejidos del intestino delgado. Cuando el tejido
sufre esfuerzo y tensión las condiciones de operación de objetivo
óptimas para esfuerzo y tensión están situadas en la zona objetivo.
La zona objetivo indica una cantidad óptima de esfuerzo y tensión
para control de hemostasis y para una curación óptima del tejido
como se ha indicado por el rectángulo objetivo mostrado en líneas
de trazos con propósitos de ilustración en la fig. 2. Como el
esfuerzo es igual a un módulo de elasticidad multiplicado por la
tensión, se aprecia que cuando aumenta el esfuerzo para cada una de
las muestras de tejido discretas, el módulo de elasticidad
permanece constante a un cierto umbral y luego cambiará también
cuando sufra un esfuerzo que exceda del umbral. Se ha apreciado que
el módulo es fijo dentro de un intervalo o rango, pero luego el
módulo variará después de un cierto umbral predeterminado tal como
cuando el tejido sufre un esfuerzo intenso.
En una realización, el material de apoyo 10 está
hecho para tener sustancialmente el mismo perfil de
esfuerzo-tensión que el tejido con el que ha de ser
usado o tener sustancialmente el mismo módulo y perfil como se ha
mostrado en la fig. 2. El material de apoyo 10 puede estar hecho de
uno o más polímeros. El o los polímeros pueden ser un material
sintético o natural. Algunos de los materiales de los que el
material de apoyo 10 puede estar hecho incluyen polímeros
poliortoésteres, lactonas, copolímeros de lactona, polisacáridos,
detrano, celulosa, ácido hialurónico, CMC, alginato, proteínas,
colágeno, gelatina, queratina, elastina, seda, polímeros de vinilo,
siliconas, copolímeros de silicanos, poliuretanos, poliuretanos
permanentes, poliuretanos degradables, poliaminas, poliamidas,
poliésteres, ésteres de poliéter, polisulfina, óxidos de
polialquileno, PEG, peos, pey-ppgs, polímeros
amorfos, polímeros cristalinos, polímeros de cristal líquido,
polímeros conductores, polímeros fluorescentes, polímeros de
activación, polímeros antifluorescentes, y cualquier combinación de
los mismos. Algunos otros de los que pueden proporcionar una
capacidad de diagnóstico como se describirá más en detalle a
continuación, también incluyen un polímero cristalino líquido, un
polímero conductor, o un polímero de activación.
La rigidez del polímero es controlada para tener
sustancialmente el mismo perfil de esfuerzo-tensión
que el tejido. En una realización, el polímero tiene fibras
estructurales modificadas para tener sustancialmente el mismo
perfil de esfuerzo-tensión que el tejido.
Alternativamente, si el material de apoyo 10 es una estructura
compuesta o es una estructura multicapa, una de las capas del
material de apoyo 10 puede incluir una malla o un material similar
a una malla a fin de proporcionar soporte a la estructura compuesta
o multicapa. En otra realización, el polímero tiene fibras
estructurales modificadas que son controladas por un grado de
reticulación de las fibras para tener sustancialmente el mismo
perfil de esfuerzo-tensión que el tejido. Aún en
otra realización, el polímero puede ser seleccionado simplemente de
entre varios polímeros existentes para que coincida sustancialmente
el perfil de esfuerzo-tensión con el del tejido. Un
experto en la técnica debería apreciar que el cirujano aplicaría el
elemento quirúrgico a través de las secciones de tejido para unir
las secciones de tejido juntas y también uniría el elemento
quirúrgico a través del material de apoyo 10 mostrado en la fig.
1.
Una platina móvil y una platina estacionaria
pueden tener numerosas configuraciones para aplicar compresión al
tejido, sin embargo con propósitos de ilustración, la compresión al
tejido T es aplicada desde una grapadora quirúrgica como se ha
mostrado en las figs. 3 a 4. Con referencia ahora a la fig. 3, la
grapadora quirúrgica 100 está mostrada con un yunque 12 y un
cartucho 14 de grapas con un conjunto 16 de accionamiento del
yunque. El conjunto 16 de accionamiento del yunque es hecho avanzar
distalmente para hacer contacto con un yunque 12 para cerrar la
grapadora 10 como se ha mostrado en la fig. 4, y para proporcionar
compresión al tejido T dispuesto dentro del espacio 20 entre el
yunque 12 y la mordaza inferior 24. Se ha considerado que pueden
usarse muchos otros conjuntos grapadores con la presente exposición
incluyendo un dispositivo de anastomosis tal como el instrumento
que está descrito en la patente norteamericana nº 6.959.851.
Algunos cirujanos usarán suturas en vez de
grapas cuando traten tejidos isquémicos. Por consiguiente, la
presente exposición proporciona un material para soportar y
reforzar el tejido con el fin de aplicar un instrumento quirúrgico
a través del tejido con una formación apropiada del elemento
quirúrgico para asegurar un resultado quirúrgico apropiado.
La fig. 3 muestra un tejido T entre el yunque 12
y una segunda mordaza estacionaria 24 con un material de apoyo 10
dispuesto entre ellos. El material de apoyo 10 proporciona soporte y
refuerzo al tejido T y así el cirujano puede comprimir el tejido T
y aplicar el elemento quirúrgico tal como una grapa a través del
tejido T para unir el tejido a otro segundo tejido o sección y para
ayudar con una formación apropiada y repetida del elemento
quirúrgico.
Así, comprimiendo el tejido, el cirujano tiene
una cantidad de material de apoyo 10 que tiene una propiedad de
compresión complementaria con relación al tejido T. El material de
apoyo 10 refuerza el tejido T cuando se aplica compresión al
tejido. Con referencia a la fig. 4, y la vista en sección
transversal de la fig. 5, una vez que se ha alcanzado la cantidad
de compresión apropiada, el cirujano puede disparar el instrumento.
El cirujano puede aplicar el elemento quirúrgico tal como una grapa
a través del tejido T para asegurar la formación apropiada del
elemento quirúrgico sin dañar el tejido. En otras palabras, el
material de apoyo 10 exhibiría un bajo esfuerzo para valores de
compresión bajos, y el material de apoyo 10 exhibiría un esfuerzo
elevado para valores de compresión más elevados para corresponder al
tejido T, y proporcionar un soporte al tejido como si el cirujano
no estuviera simplemente disparando a través de un tejido isquémico
delgado T, sino en su lugar estuviera disparando la grapadora a
través de un tejido T de espesor normal y bien definido. Debería
apreciarse que los intervalos de compresión para el material de
apoyo 10 particular en esta realización serían complementarios al
perfil esfuerzo-tensión del tipo de tejido
individual como se ha mostrado en la fig. 2. Los intervalos de
compresión dependerían de la curva de
esfuerzo-tensión que es equiparable a una presión
predeterminada sobre el tejido. Idealmente, el módulo de elasticidad
del material de apoyo 10 coincidiría con el módulo de elasticidad
del tejido específico mostrado en la fig. 2.
En otra realización, mostrada en la fig. 6, el
material de apoyo 10' tiene una propiedad diferente con relación al
tejido T. En esta realización, el material de apoyo 10' cede a una
tasa mayor con relación al tejido T cuando el esfuerzo de
compresión es aplicado por las mordazas 22, 24. De nuevo, cuando se
comprime el tejido T, el cirujano tiene una cantidad de material de
apoyo 10' para reforzar el tejido T cuando se aplica la fuerza de
compresión al tejido. En esta realización, el material de apoyo 10'
es más blando que el tejido T y se comprimiría y cedería más que el
tejido T cuando es aplicada la fuerza y por ello proporciona una
conformidad adicional. Por consiguiente, una vez que se ha
alcanzado la cantidad de compresión apropiada, el cirujano podría
aplicar el elemento quirúrgico tal como una grapa a través del
tejido T para ayudar con la formación apropiada del elemento
quirúrgico sin dañar el tejido T. Debería apreciarse que los
intervalos de compresión para el material de apoyo 10' particular
en esta realización también serían derivados del perfil
esfuerzo-tensión del tipo de tejido individual como
se ha mostrado en la fig. 2. Los intervalos de compresión
dependerían de la curva de esfuerzo-tensión que es
equiparable a una presión predeterminada sobre el tejido.
Idealmente, el módulo de elasticidad del material de apoyo 10'
sería más elástico que el módulo de elasticidad del tejido
específico mostrado en la fig. 2.
En otra realización, mostrada en la fig. 7, el
material de apoyo 10'' tiene de nuevo una propiedad diferente con
relación al tejido. En la realización mostrada en la fig. 7, el
material de apoyo 10'' no cede a una tasa mayor con relación al
tejido T cuando el esfuerzo de compresión es aplicado por las
mordazas 22, 24 y en vez de ello es más rígido que el tejido T.
De nuevo, cuando se comprime el tejido T, el
cirujano tiene una cantidad de material de apoyo 10'' para reforzar
el tejido T cuando se aplica la compresión al tejido T. En esta
realización, el material de apoyo 10'' es más rígido que el tejido
T. En oposición a la realización de la fig. 6, aquí el material de
apoyo 10'' resistiría la compresión y su cesión con relación al
tejido. El material de apoyo 10'' se comprimiría menos que el tejido
T cuando la fuerza es aplicada y por ello transferiría la fuerza de
compresión al tejido T. Esta transferencia ayuda con el control de
hemostasis del tejido T. Consiguientemente, una vez que se ha
alcanzado la cantidad de compresión apropiada, el cirujano podría
aplicar el elemento quirúrgico tal como una grapa a través del
tejido para ayudar con la formación apropiada del elemento
quirúrgico con repetitividad y sin dañar el tejido, y además
proporciona un control de hemostasis del tejido. Debería apreciarse
además que los intervalos de compresión para el material de apoyo
10'' particular en esta realización también serían derivados del
perfil esfuerzo-tensión del tipo de tejido
individual como se ha mostrado en la fig. 2 tal como tejido del
intestino delgado, tejido del colon, y tejido del estómago o
abdominal. Los intervalos de compresión dependerían de la curva de
esfuerzo-tensión que es equiparable a una presión
predeterminada sobre el tejido. Idealmente, el módulo de
elasticidad del material de apoyo 10'' sería menos elástico o más
rígido que el módulo de elasticidad del tejido específico mostrado
en la fig. 2.
Con referencia ahora a las figs. 8 a 10, se ha
mostrado una realización del presente invento. En esta realización,
el sistema tiene un material 200 de apoyo regulado mecánicamente. El
material 200 de apoyo regulado mecánicamente está hecho de un
material natural o sintético, y tiene un sustrato 202. El material
200 de apoyo regulado mecánicamente puede estar dispuesto en varias
construcciones reguladas mecánicamente predeterminadas siendo cada
construcción diferente adecuada para ser usada con un tejido enfermo
particular o un tipo de tejido específico. Cada material regulado
mecánicamente es adecuado para su uso con diferentes tipos de tejido
enfermos tales como tejido delgado, tejido diabético, tejido
isquémico u otros tipos de tejido tales como gastrointestinal,
tejido cardíaco, tejido pulmonar u otro tipo de tejido.
Sin embargo, en esta realización cada material
200 de apoyo regulado mecánicamente proporciona al cirujano una
indicación visual de una cantidad de compresión umbral que ha de ser
aplicada al tejido durante un período de tiempo predeterminado. Una
vez que se ha comunicado la indicación visual, el cirujano sabrá
visualmente que es el momento apropiado para implantar el elemento
quirúrgico en el tejido. El cirujano será provisto de una
indicación de que es el momento de unir las dos secciones de tejido
juntas y así liberará el tejido después de disparar el instrumento
y ya no aplicará ninguna compresión adicional al tejido. Así, el
material 200 de apoyo regulado mecánicamente proporciona una
compresión del tejido óptima antes de introducir el elemento
quirúrgico en el tejido.
Debería apreciarse que con el material de apoyo
200 fijado al tejido, el material de apoyo 200 puede ser
inspeccionado muy posteriormente después del disparo del
instrumento en el cuidado post-operatorio. El
cirujano puede inspeccionar el tejido adyacente al material de
apoyo 200 para asegurar la curación del tejido, y que se ha aplicado
la cantidad de compresión apropiada para asegurar un resultado
quirúrgico positivo.

El sustrato 202 puede estar hecho de un
polímero. El polímero puede ser el mismo o diferente de los
polímeros enunciados antes para el material de apoyo 10, y puede
incluir la capacidad de diagnóstico descrita previamente. Como se
ha indicado previamente, los polímeros pueden incluir polímeros de
cristal líquido, polímeros conductores, polímeros fluorescentes,
polímeros de activación, polímeros antifluorescentes, y cualesquiera
combinaciones de los mismos. Algunos de los polímeros que forman el
sustrato 202 pueden tener otras propiedades mecánicas que pueden
proporcionar la indicación exclusiva de cualquier esfuerzo aplicado
al polímero. El propio polímero puede incluir una o más propiedades
mecánicas que pueden contribuir a proporcionar la indicación que
incluye un peso molecular, un índice de polidispersión, una
propiedad o historia térmica, una estructura de monómero, una
selección de monómero, una densidad de reticulación, una propiedad
de viscoelasticidad del polímero, o una estabilidad del polímero.
Son posibles distintas configuraciones y dentro de la presente
exposición.
El sustrato 202 puede estar hecho de cualquier
material flexible adecuado que puede ser curvado, o enrollado
alrededor del tejido sin dañar el tejido o producir una abrasión en
el tejido. El sustrato 202 está hecho de un material biocompatible,
y puede ser aplicado y retirado fácilmente de los tejidos interiores
del cuerpo tales como el tejido gastrointestinal, tejido cardíaco,
tejido del colon, tejido del intestino delgado, tejido del
intestino grueso y tejido pulmonar sin producir abrasión en el
tejido, y/o perjudicar al tejido.
El sustrato 200 tiene una superficie distal 204
que hace contacto con el tejido y también tiene una superficie
proximal opuesta 208. En una realización, el sustrato 202 puede
estar hecho de una película sensible a la presión, en otra
realización, el sustrato 202 puede estar hecho de un gel sensible a
la presión, espuma sensible a la presión, un material que tiene
varias partículas, o adhesivos y cualesquiera combinaciones de los
mismos. Una vez que se ha aplicado una cantidad óptima de esfuerzo
al sustrato 202, el sustrato 202 se modulará para proporcionar una
indicación visual al cirujano de que se ha conseguido un cierto
umbral de compresión. La indicación visual puede ser un cambio en
una o más propiedades ópticas del sustrato 202 o un cambio en un
estado visual desde un primer estado a otro segundo estado
diferente.
El cambio en un estado visual para proporcionar
la indicación puede ser desde el estado translúcido del sustrato
200 a un estado opaco del sustrato 200. El cambio en el estado
visual es desde un primer color a un segundo color. Son posibles
distintas configuraciones. En una realización, la indicación es
desde un color inicial a un segundo color como se ha mostrado en la
fig. 8A. El segundo color proporcionará al cirujano una indicación
de que el umbral de compresión óptimo ha sido alcanzado para el
tejido específico, y que el tejido está listo para introducción de
un elemento quirúrgico tal como varias grapas.
En otra realización, el sustrato 202 no está
hecho de una película sensible a la presión y en vez de ello tiene
varios sacos 208 mostrados en la fig. 9. Los sacos 208 tienen una
resistencia a tracción o resistencia a reventar predeterminada que
está diseñada para la rotura o reventado a una presión específica
cuando el sustrato es comprimido. En una realización, los sacos 208
están dispuestos sobre la superficie distal 204 del sustrato 202.
En otra realización, los sacos 208 pueden estar colocados en el
interior del sustrato 202 y estar confinados dentro del interior
del sustrato 202.
Con referencia a la fig. 10, se ha mostrado una
vista mirando en el lado distal del sustrato 204. Los sacos 208
tienen una superficie exterior 210, y un lumen interior 212 que está
cerrado herméticamente del entorno exterior. Los sacos 208, en una
realización, pueden ser formados en una matriz de filas discretas
sobre la superficie distal del sustrato 202 tal como de diez por
diez, o veinte por veinte. En otra realización, los sacos 208
pueden estar en grupos predeterminados sobre el lado distal 204 del
sustrato 202 o alternativamente en el espacio interior del sustrato
202. Aún en otra realización, los sacos 208 pueden estar
dimensionados de modo que abarquen el lado distal 204 del sustrato
202. Distintas y posibles configuraciones dependen del elemento
quirúrgico aplicado para unir el tejido y del tipo de tejido. Los
sacos 208 del sustrato 202 pueden estar hechos de un trimetil
carbonato, o un polisacárido tal como dextrano. Alternativamente,
los sacos 208 pueden también estar hechos de ácido poliglicólico, o
ácido poliláctico.
La fig. 10 muestra un saco 208 del sustrato 202.
Los sacos 208 tienen un lumen interior 212 que está dimensionado
para cubrir un material predeterminado dentro del lumen interior
212. En una realización, el lumen interior 212 es generalmente de
forma esférica. En otra realización, el lumen interior 212 puede ser
generalmente de forma ortogonal o tubular, o aún en otra
realización, el lumen interior 212 puede tener forma irregular. Son
posibles distintas configuraciones del lumen interior y dependen del
elemento quirúrgico que es aplicado para unir el tejido, de la
cantidad de compresión, del contenido del lumen y del tipo de
tejido.
El material predeterminado es un tinte 214 que
está en el lumen interior 212. El tinte 214 puede ser un tinte de
sutura y no es tóxico y una cantidad del tinte 214 puede entrar en
el cuerpo sin ningún efecto tóxico o perjudicial para el tejido. El
material predeterminado puede ser alternativamente un tinte
fluorescente, un tinte ultravioleta, un tinte visible, un pigmento
o un aditivo de color. En una realización, cada saco individual 208
es ajustado previamente en relación a un tipo de tejido tal como
tejido gastrointestinal, tejido cardíaco, tejido pulmonar, o
músculo y así los sacos 208 cederán cuando una cierta cantidad de
fuerza de compresión sea aplicada sobre el sustrato 202 que
corresponde con el tipo de tejido.
En una realización, los tintes 214 pueden ser
cualquier tinte visible ultravioleta (colorante alimenticio, etc.)
o sondas tales como sondas fluorescentes o infrarrojas (IR)/próximas
a IR. Estas sondas pueden requerir una cantidad predeterminada de
energía de excitación para su emisión. Además, el polímero que
compone el sustrato de las partículas podría ser preparado a partir
de, o funcionalizado con, las moléculas de "sonda" antes
mencionadas, porque el polímero proporciona tanto las propiedades
mecánicas como de visualización. Algunas sondas incluyen una sonda
fluorescente, sondas de fluoresceína FTI, de isotiocianato de
fluoresceína (FITC) y de isotiocianato de tetrametilrodamina
(TRITC), una sonda molecular de rodamina, DAPI, tal como una sonda
de "amarillo nuclear", una sonda naranja de acridina,
etiquetas nucleares de DRAQ^{TM} y SYTOX^{TM}, yoduro de
propidio y otros fluoroforos con absorción y espectros de emisión
similares.
Con referencia ahora a la fig. 11, cuando es
comprimido, el contenido o tinte 214 de los sacos 208 escapará del
sustrato 202 e irá sobre el tejido T. En otra realización, cuando es
comprimido, el contenido o tinte 214 de los sacos 208 escapará de
los sacos 208 y será confinado dentro del interior del sustrato 202
pero será visible desde el exterior del sustrato 202, y no hará
contacto con el tejido. En esta realización, el tinte 214 no puede
escapar del sustrato 202, y pueden ser usados otros tipos de tinte.
Como se ha mostrado en la fig. 8A, de esta manera, el cirujano verá
un cambio de color en la zona deseada a la compresión
predeterminada, y el cirujano sabrá entonces aplicar el elemento
quirúrgico al tejido T al ver la indicación. En otra realización,
el sustrato 202 puede ser formado sin tinte 214. En su lugar, el
lumen interior 212 estará vacío y simplemente reventará de forma
audible a la compresión óptima. Aquí, el material de apoyo 200
indicará de modo audible que hay que aplicar el elemento quirúrgico
al tejido T.
Alternativamente, si el cirujano no ve u oye la
indicación, el cirujano será provisto con una indicación alternativa
o falsa. El cirujano sabrá que el sustrato 202 no está siendo
aplicado con una fuerza de compresión, bien de manera uniforme, o
bien que se está aplicando una fuerza de compresión insuficiente al
tejido T. En este caso, el cirujano puede reorientar el
instrumento, e intentar de nuevo aplicar el elemento quirúrgico en
la zona deseada.
Además, si el cirujano después de repetidos
intentos no ve la indicación, el cirujano sabrá que el sustrato 202
no está siendo aplicado con una fuerza de compresión o bien de una
manera uniforme o bien que una fuerza de compresión insuficiente
está siendo aplicada al tejido T. En este caso, el cirujano puede a
continuación aplicar otro tipo diferente de elemento quirúrgico,
tal como una sutura en el lugar deseado.
Con referencia ahora a la fig. 11A, se ha
mostrado una realización alternativa del sustrato 202 de la presente
exposición. En esta realización, el sustrato 202 no tiene ningún
colorante como en la fig. 11 sino que en vez de ello tiene
contenidos diferentes. En esta realización, el saco 208 tiene una
sustancia química 214 dispuesta en él. Como las realizaciones
previas, la sustancia química 214 puede escapar del sustrato 202
cuando el saco 208 se rompe. Sin embargo, en esta realización,
cuando es comprimido, el contenido o sustancia química 214 de los
sacos 208 se romperá y se escapará desde los sacos 208 y hará
contacto con una superficie exterior del sustrato 202.
Con referencia ahora a la fig. 11B, cuando la
sustancia química 214 hace contacto con el sustrato 202, el
sustrato 202 reaccionará con la sustancia química 214 para
proporcionar la indicación visual de que se ha alcanzado la
compresión óptima. La reacción química entre el sustrato 202 y la
sustancia química 214 cambiará una propiedad óptica del sustrato
202 como se ha mostrado en la fig. 11B, y de esta manera, el
cirujano observará visualmente un cambio de color en la zona
apropiada a la compresión predeterminada. Una vez que es provisto
con la indicación de diagnóstico el cirujano sabrá entonces aplicar
el elemento quirúrgico al tejido T después de ver la
indicación.
Aún en otra realización de la presente
exposición, el sustrato 202 mostrado en la fig. 11C puede estar
formado con una pluralidad de cristales líquidos o sólidos 209. Los
cristales 209 están hechos con el sustrato 202 y pueden ser colados
o moldeados in situ cuando el sustrato 202 es fabricado. En una
realización, los cristales 209 son colados en material polímero y
el sustrato polímero 202 es extruído desde una matriz. Los cristales
209 tienen un estado inicial predeterminado y un segundo estado
cuando son comprimidos con la cantidad óptima de compresión. Una
vez que se ha alcanzado la compresión predeterminada en el
procedimiento y se ha aplicado al sustrato 202 como se ha mostrado
en la fig. 11D, los cristales 209 pueden romperse. De esta manera,
una propiedad óptica de al menos uno de los cristales 209 y del
sustrato 202 en respuesta a la compresión es cambiada para
proporcionar la indicación. Esta indicación permite al cirujano
apreciar visualmente de modo instantáneo que se ha alcanzado la
compresión óptima.
Debería apreciarse que con el sustrato 202
fijado al tejido, éste permite al cirujano inspeccionar el tejido
posteriormente o después del disparo o mucho tiempo después del
disparo del instrumento tal como en el cuidado
post-operatorio. El cirujano puede inspeccionar el
tejido adyacente al sustrato 202 para asegurar la curación del
tejido, y que la cantidad apropiada de compresión ha sido aplicada
para asegurar un resultado quirúrgico positivo.
Debería apreciarse que los sacos 208 pueden ser
formados con distintos tamaños. Aún en otra realización de la
presente exposición como se ha mostrado en la fig. 11E, el sustrato
202 puede ser formado con una pluralidad de microesferas 211. Las
microesferas 211 son fabricadas con el sustrato 202 para incluir el
colorante 214 u otro material como se ha descrito antes. En una
realización, las microesferas 211 tienen un tamaño que es de un
orden menor con relación a los sacos 208 de la realización previa.
La fig. 11F muestra una vista agrandada del sustrato 202 a lo largo
del círculo de referencia 11F de la fig. 11E. Una vez que se ha
alcanzado la compresión predeterminada en el procedimiento y se ha
aplicado al sustrato 202 que tiene las microesferas 211 en la
superficie distal 204, las microesferas 211 pueden romperse para
liberar su contenido y proporcionar una indicación en respuesta a
la compresión óptima. Esta indicación permite al cirujano apreciar
visualmente de modo instantáneo que se ha alcanzado la compresión
óptima.
Las microesferas 211 podrían ser preparadas a
partir de polímeros incluyendo lactonas, óxidos de polialquileno,
poliortoésteres, polifosfazenos, polímeros vinílicos incluyendo el
polimetacrilato de metilo, ácido acrílico, ácido metacrílico, ácido
estireno sulfónico, polivinil pirrolidonas, hidroxietil
metacrilatos, sulfopropil acrilatos, lípidos vinílicos y
fosfolípidos, y copolímeros de vinilo, proteínas (colágeno,
albúmina, caseína, gelatina, lactoferrina, sintético/recombinante,
etc.), polisacáridos (ácido hialurónico, carboxi metil celulosa,
sulfato de heparina, dextrano, quitosano, alginatos, metil celulosa,
derivados funcionalizados (amino, carboxi, hidroxi, sulfonatado,
fluorado), etc.), poliésteres, poliaminas, polianhidridos,
polihidroxi-alcoanatos, ésteres de polieter, drogas
de polímero, o cualesquiera combinaciones de los mismos.
Aún en otra realización de la presente
exposición mostrada como en la fig. 11G, el sustrato puede ser
formado con una pluralidad de nanoesferas 213 que pueden ser de un
orden de una décima a una milésima menores que los sacos 208. Las
nanoesferas 213 son fabricadas con el sustrato 202 para incluir un
material en un lumen (no mostrado) formado en cada una de las
nanoesferas 213. En una realización, las nanoesferas 213 tienen un
tamaño minúsculo con relación a los sacos 208 de la realización
previa. Una vez que se ha alcanzado la compresión predeterminada en
el procedimiento y se ha aplicado al sustrato 202 que tiene las
nanoesferas 213 mostradas en la fig. 11H, las nanoesferas 213
pueden romperse para liberar su contenido y proporcionar una
indicación en respuesta a la compresión óptima. Esta indicación
permite al cirujano apreciar visualmente de manera instantánea que
la compresión ha sido alcanzada. En otra realización, alguna de las
nanoesferas 213 puede romperse a diferentes gradientes de esfuerzo
para proporcionar varias indicaciones diferentes dependiendo de la
compresión deseada especifica en el procedimiento.
Aún en otras realizaciones de la presente
exposición como se ha mostrado en la fig. 12, el sustrato 202 puede
tener una marca o identificación sobre la superficie proximal 206
para indicar un tipo de tejido tal como el tejido gastrointestinal,
tejido cardíaco, tejido pulmonar, o músculo así los sacos 208
producirán un resultado todos de forma colectiva cuando una cierta
cantidad de fuerza de compresión es situada sobre el sustrato 202
que es importante para ese tipo de tejido, y el cirujano puede
intercambiar fácilmente entre varios tipos diferentes de materiales
de apoyo 200 durante el curso de un único procedimiento. El
contenido de los sacos 208 puede escaparse del sustrato 202 y al
tejido T una vez que se ha conseguido la fuerza de compresión
predeterminada. De esta manera, el cirujano apreciará visualmente
la indicación en la zona deseada, y el cirujano sabrá entonces
aplicar el elemento quirúrgico al ver la indicación.
Aún en otra realización de la presente
exposición, el sustrato 202 puede tener una marca o identificación
I en la superficie proximal 206 para indicar una condición de tejido
específica para el tipo de tejido. Por ejemplo, para el tejido
gastrointestinal puede haber varios sustratos diferentes 202 cada
uno con sacos 208 que producirán un resultado de modo diferente
cuando una cantidad predeterminada de compresión es aplicada para
tipos de tejidos enfermos tales como el tejido diabético. En una
realización, los sacos 208 pueden ceder a una fuerza de compresión
adecuada para el tejido gastrointestinal diabético. La superficie
proximal 206 puede proporcionar tal indicación I.
Los sacos 208 en otra realización pueden ceder a
una segunda fuerza de compresión que es diferente de la primera
fuerza de compresión y ser adecuados para el tejido gastrointestinal
isquémico. La superficie proximal 206 proporcionará otra indicación
I' tal como "tejido gastrointestinal isquémico" como se ha
mostrado en la fig. 13. Los sacos 208 en otra realización pueden
ceder y dejar escapar el tinte 214 a una tercera fuerza de
compresión que es diferente de la primera y segunda fuerzas de
compresión y ser adecuados para el tejido gastrointestinal cuando
el paciente ha sufrido un uso prolongado de un compuesto medicinal y
ser marcado consiguientemente. En un ejemplo, cuando el compuesto
medicinal es un esteroide, la superficie proximal 206 del sustrato
202 proporcionará otra indicación I' tal como "tejido
gastrointestinal - uso de esteroides" como se ha mostrado en la
fig. 13. Son posibles distintas configuraciones y dentro del marco
de la presente exposición.
Los sacos 208 en otras realizaciones pueden
ceder a fuerzas de compresión adecuadas para distintos tipos de
enfermedad, tales como tejido isquémico, tejido diabético, tejido
expuesto a medicina o medicinas prescritas específicas, tejido
blando, tejido grueso, o tejido enfermo específico para todos los
tipos de tejidos incluyendo pero no estando limitados a tejido
gastrointestinal, tejido pulmonar, tejido cardíaco, tejido
abdominal, tejido de colon o cualquier otro tejido animal o humano
conocido.
Debería apreciarse que los sacos 208 del
sustrato 202 no resultarán con fugas simplemente al ser comprimidos,
sino que en vez de ello reventarán y se abrirán para proporcionar
instantáneamente la indicación visual al cirujano de que se ha
alcanzado la compresión óptima desde el instrumento. Los sacos 208
pueden tener una cantidad de material en el lumen interior 212 para
ayudar al reventado de modo que resulten abiertos o rotos
súbitamente cuando la carga de compresión óptima es aplicada al
sustrato 202, especialmente a partir de una presión interna del
saco 208.
Con referencia a la fig. 14, el gas en el lumen
interior 212 del saco 208 que reventará puede ser aire, oxígeno,
nitrógeno, un gas inerte, dióxido de carbono u otra sustancia
gaseosa predeterminada específica. El gas ayudará además con el
reventado repentino de las paredes laterales del saco 208 cuando la
cierta cantidad de fuerza de compresión es aplicada sobre el
material de apoyo 200 que es complementario de ese tipo de
tejido.
Debería apreciarse además que los sacos 208
reventarán además abriéndose ampliamente a fin de que el material
que está dispuesto en los sacos 208 escape de los sacos 208 de modo
que resulte visible de una manera sencilla. El contenido de los
sacos 208 tiene una pequeña resistencia a fluir y una baja
viscosidad. El contenido se escapa de los sacos 208 y del sustrato
202 y sobre el tejido una vez que la fuerza de compresión
predeterminada es conseguida de una manera fácil. De este modo, el
cirujano inmediatamente después del reventado repentino apreciará
visualmente un cambio en el color en la zona deseada (o confinada en
sustrato 202) y el cirujano sabrá entonces aplicar el elemento
quirúrgico al ver la indicación.
El sustrato 202 puede ser curvable sin que los
sacos 208 revienten y así sea enrollado alrededor de la zona de
tejido deseada. En una realización, el sustrato 202 puede estar
dispuesto independiente del aparato para aplicar un elemento
quirúrgico. El sustrato 202 puede estar dispuesto en un dispensador
adecuado en un rollo o lateralmente en una caja cerrada
herméticamente lista para su uso quirúrgico como se ha mostrado en
la fig. 15. El sustrato 202 puede ser aplicado a un primer lado del
tejido, y a continuación las mordazas 92, 24 del instrumento
quirúrgico pueden ser ajustadas alrededor del tejido y del sustrato
202, y a continuación comprimidas.
En otra realización, dos láminas de sustrato 202
pueden ser aplicadas a un primer lado del tejido, y luego a otro
segundo lado opuesto del tejido. Las mordazas del instrumento
quirúrgico pueden ser ajustadas alrededor del tejido y dos láminas
del sustrato 202 para compresión. Aún en otra realización,
alternativamente el sustrato 202 puede ser dispuesto fijado
directamente sobre o a través de una ranura en las mordazas 22, 24
de un instrumento quirúrgico y conectado a un dispensador enrollado
adecuado adyacente al instrumento. Después de ello, las mordazas
92, 94 pueden ser comprimidas alrededor del tejido y el sustrato 202
para determinar la cantidad óptima de compresión. Después de ello,
una parte de sustrato 202 formado en un rollo puede tener una
perforación (no mostrada). La parte usada del rollo del sustrato
202 puede ser retirada después de que el elemento quirúrgico es
introducido en el tejido simplemente desgarrando en la
perforación.
Debería apreciarse que la visualización del
cambio de color puede ocurrir bajo iluminación natural, o ser
observada en condiciones de iluminación fluorescente. En una
realización, el tinte 214 puede ser visualizado bajo una luz
fluorescente o a una longitud de onda predeterminada tal como 488
nm. En una realización, el cirujano puede usar lentes o gafas
protectoras para facilitar la visión del colorante 214 y para ayudar
a situar una línea de grapado apropiada a fin de introducir las
grapas quirúrgicas. Son posibles distintas configuraciones y están
dentro de la presente exposición.
El sustrato 202 puede ser aplicado en seco a la
zona del tejido, o con un gel o agente lubricante adecuado. En una
realización, el cirujano puede aplicar lubricación tal como
VASOLINE^{TM}, o KY JELLY^{TM}, u otro gel adecuado tal como
maltodextrano que permite que el sustrato 202 se pegue hacia abajo y
permanezca sobre el tejido durante la compresión.
Aunque se han mostrado varias realizaciones de
la exposición en los dibujos, no se pretende que la exposición esté
limitada a ellas, lo que se pretende es que la exposición debería
ser construida simplemente como ejemplificaciones de realizaciones
preferidas.
Claims (15)
1. Un dispositivo (100) de grapado quirúrgico
accionable para comprimir un tejido antes de la inserción de una
grapa quirúrgica en el tejido y para insertar la grapa quirúrgica en
el tejido, en combinación con: un sustrato (10) para soportar el
tejido, caracterizado porque el sustrato está hecho de un
material predeterminado, en el que el material predeterminado tiene
un color inicial cuando no es aplicado un esfuerzo al sustrato, y
en el que el material predeterminado tiene un segundo color cuando
es aplicado un esfuerzo de compresión predeterminado al sustrato,
indicando el segundo color la compresión de tejido apropiada a fin
de disparar la grapa quirúrgica al tejido.
2. El dispositivo según la reivindicación 1, en
el que el material predeterminado es una película (202) sensible a
la presión.
3. El dispositivo según la reivindicación 2, en
el que la película sensible a la presión cambia de color para
indicar un área para una formación de grapa.
4. El dispositivo de grapado quirúrgico según la
reivindicación 1, en el que el material predeterminado del sustrato
(202) incluye un compartimiento (208) que tiene un tinte, estando
hecho el compartimiento con una resistencia mecánica a tracción que
está configurada para romperse al nivel de esfuerzo de compresión
predeterminado para liberar el tinte para proporcionar el segundo
color.
5. El dispositivo según la reivindicación 4, en
el que el tinte no es tóxico, y colorea el tejido a otro color.
6. El dispositivo según la reivindicación 3, en
el que el tinte está contenido en el sustrato y no es liberado
sobre el tejido.
7. El dispositivo según cualquiera de las
reivindicaciones 4 a 6, en el que el compartimiento forma un lumen
interior (212) con un gas en el lumen, reventando y abriéndose el
lumen para liberar el colorante y el gas en el lumen interior al
ser comprimido al nivel de esfuerzo de compresión
predeterminado.
8. El dispositivo según la reivindicación 1, en
el que el material predeterminado del sustrato (202) tiene una
pluralidad de sacos (208) teniendo cada uno de la pluralidad de
sacos un reactante, en el que el material predeterminado tiene el
color inicial cuando no hay aplicado esfuerzo al sustrato, y en el
que cuando es aplicado un esfuerzo de compresión predeterminado al
sustrato los sacos se rompen para liberar el reactante, reaccionando
el reactante con el material predeterminado para modular el
material predeterminado desde el color inicial al segundo
color.
9. El dispositivo según la reivindicación 1, en
el que el material predeterminado del sustrato (202) tiene una
pluralidad de cristales (209), en el que el material predeterminado
tiene el color inicial cuando no hay aplicado esfuerzo al sustrato,
y en el que el material predeterminado tiene el segundo color cuando
el esfuerzo de compresión predeterminado es aplicado al sustrato,
rompiendo el esfuerzo predeterminado al menos uno de la pluralidad
de cristales, modulando los cristales rotos una propiedad óptica del
material predeterminado.
10. El dispositivo según cualquiera de las
reivindicaciones precedentes, en el que el sustrato ha de permanecer
conectado al tejido a la conclusión del disparo del dispositivo y
el segundo color permanece sobre el sustrato de modo que un
cirujano puede inspeccionar el segundo color después de disparar
para indicar la compresión que ha sido aplicada al tejido al
disparar en un instante después del disparo del dispositivo.
11. El dispositivo según cualquiera de las
reivindicaciones precedentes, en el que el material predeterminado
del sustrato es biocompatible y el sustrato ha de permanecer
conectado al tejido a la conclusión del disparo del
dispositivo.
12. El dispositivo según cualquiera de las
reivindicaciones precedentes, en el que el sustrato es un
polímero.
13. El dispositivo según cualquiera de las
reivindicaciones precedentes, en el que el sustrato tiene una
pluralidad de capas.
14. El dispositivo según cualquiera de las
reivindicaciones precedentes, en el que el sustrato es un material
compuesto.
15. El dispositivo según cualquiera de las
reivindicaciones precedentes, en el que el compuesto del sustrato
incluye colágeno.
