（背景）
１．技術分野
本開示は、外科用ステープル留めデバイスおよび縫合糸、そして特に、ステープルおよび縫合糸の最適形成のために最適量の組織圧縮が組織に付与されることを確実にするための外科用器具と連結して用いられるかまたは組み合わせて用いられる支持デバイスに関する。なおより特定すれば、本開示は、最適量の組織圧縮が到達されたこと、および外科用要素を付与する適正な時間である視覚指標を外科医に提供する支持デバイスに関する。
２．関連技術の説明
吻合は、別個の中空器官セクションの外科的接続である。代表的には、吻合手順は、中空組織の疾患または欠陥セクションが除去される手術で行われる。その後、この手順は、残存する端部組織セクションが接続されるステップを有する。所望の吻合手順に依存して、この端部セクションは、円形、端と端とを接した、端と側面とを接した、または側面と側面とを接した器官再構築方法のいずれかによって接続され得る。

移動可能プラテンおよび静止プラテンは、多くの形態を有し得、組織に圧縮を付与するが、例示の目的のために、組織Ｔに対する圧縮は、図３〜４に示される外科用ステープラから付与される。ここで、図３を参照して、この外科用ステープラ１００は、アンビル駆動アセンブリ１６を備えて、アンビル１２およびステープルカートリッジ１４を有して示される。このアンビル駆動アセンブリ１６は遠位方向に進行されて、図４に示されるようにアンビル１２を接触し、そしてステープラ１０を閉じ、そしてこのアンビル１２と下部顎２４との間のギャップ２０内に配置された組織Ｔへの圧縮を提供する。多くのその他のステープラアセンブリが本開示とともに用いられ得ることが想定され、先に参考として援用された米国特許第６，９５９，８５１号に記載されている器具のような吻合デバイスが挙げられる。
いく人かの外科医は、虚血組織を処置するとき、ステープルの代わりに縫合糸を用いる。従って、本開示は、適切な外科的結果を確実にするために外科用要素の適正な形成によって、組織を通って外科用器具を付与するために、この組織を支持および補強するための材料を提供する。
図３は、アンビル１２と第２の静止顎２４との間の組織Ｔを、それらの間に配置された支持材料１０とともに示す。この支持材料１０は、外科医が組織Ｔを圧縮し得、そしてステープルのような外科的要素を組織Ｔを通じて付与し、この組織を別の第２の組織またはセクションに接続し、そしてこの外科用要素の適正かつ反復する形成を支援するように、この組織Ｔに対する支持および補強を提供する。
従って、組織を圧縮することにより、外科医は、この組織Ｔに対して相補的に圧縮する性質を有する所定量の支持材料１０を有する。この支持材料１０は、上記組織に圧縮を付与するとき組織Ｔを補強する。図４、および図５中の断面図を参照して、一旦、適正な量の圧縮が到達されると、外科医は器具を発射し得る。外科医は、ステープルのような外科用要素を組織Ｔを通じて付与し得、この組織を損傷することなくこの外科用要素の適正な形成を確実にする。換言すれば、この支持材料１０は、低圧縮値の下で低ストレスを示し得、そしてこの支持材料１０は、より高い圧縮値の下で高いストレスを示して組織Ｔに一致し、そしてあたかも外科医が薄い虚血組織Ｔを通じて単にステープラを発射しているのではなく、しかしその代わりに通常の厚みおよび良好に規定された組織Ｔを通じてステープラを発射しているように、この組織に対する支持を提供する。この実施形態における特定の支持材料１０に対する圧縮範囲は、図２に示されるように個々の組織タイプのストレス−ひずみプロフィールに相補的であることが認識されるべきである。この圧縮範囲は、組織に対する所定の圧力に等しい上記ストレス−ひずみ曲線に依存し得る。理想的には、この支持材料１０の弾性率は、図２に示される特定組織の弾性率に一致し得る。

再び、組織Ｔを圧縮するとき、外科医は、圧縮を組織Ｔに付与するとき、組織Ｔを補強するために所定量の支持材料１０’’を有する。この実施形態では、支持材料１０’’は組織Ｔより剛直性である。図６の実施形態とは反対に、ここで、この支持材料１０’’は、圧縮に抵抗し得、そして組織に対して降伏する。この支持材料１０’’は、力が付与されるとき組織Ｔより少なく圧縮し得、そしてそれによって圧縮力を組織Ｔに移動する。この移動は、組織Ｔの止血制御を支援する。従って、一旦、適正量の圧縮に到達すると、外科医は、ステープルのような外科用要素を組織を通じて付与し得、外科用要素の適正な形成を反復性をもって、かつ組織を損傷することなく支援し、そして組織の止血制御をさらに提供する。この実施形態における特定の支持材料１０’’に対する圧力範囲はまた、小腸組織、結腸組織、および胃または腹部組織のような、図２に示されるような個々の組織タイプのストレスひずみプロフィールに由来し得ることをさらに認識すべきである。この圧縮範囲は、組織に対する所定の圧力と同等視するストレスひずみ曲線に依存し得る。理想的には、支持材料１０’’の弾性率は、図２に示されるような特定組織の弾性率より少ない弾性または剛直性であり得る。
ここで、図８〜１０を参照して、本開示の別の代替の実施形態が示される。この実施形態では、このシステムは、機械的に調整された支持材料２００を有する。機械的に調整された支持材料２００は、天然または合成材料から作製され、そして基板２０２を有する。この機械的に調整された支持材料２００は、特定の疾患組織または特定の組織タイプと用いられるために適した各々異なる構成で、多くの所定の機械的に調整された構成で配列され得る。各々の機械的に調整された材料は、薄い組織、糖尿病組織、虚血組織、または胃腸管、心臓組織、肺組織またはアンサー組織タイプのようなその他のタイプの組織のような異なる疾患または組織タイプとの使用のために適切である。
しかし、この実施形態では、各々の機械的に調整された支持材料２００は、外科医に所定の時間の期間の間組織に付与されるべき閾値量の圧縮の視覚指標を提供する。一旦、この視覚指標が送られると、外科医は、組織中に外科用要素を移植する適正な時間であることを視覚で知る。外科医は、２つの組織セクションを一緒に接続するための時間であり、そしてそれ故、器具の発射後、組織を開放し、組織に任意のさらなる圧縮をもはや付与しないことの指標が提供される。従って、この機械的に調整された支持材料２００は、組織中への外科用要素の導入前に、組織の最適圧縮を提供する。
この支持材料２００を組織に固定して、支持材料２００は、手術後ケアにおいて器具の発射後長く後に検査され得る。外科医は、この支持材料２００に隣接する組織を検査し得、組織の治癒を確実にし、しかも適正な量の圧縮が付与され正の外科的結果を確実にする。
基板２０２はポリマーから作製され得る。このポリマーは、支持材料１０について上記で列挙されたポリマーと同じか、または異なり得、そして先に論議された診断能力を含み得る。先に示されたように、これらポリマーは、液晶ポリマー、伝導性ポリマー、蛍光ポリマー、作動ポリマー、抗蛍光ポリマー、およびこれらの任意の組み合わせを含み得る。基板２０２を形成するポリマーのいくつかは、これらポリマーに付与される任意のストレスを除く指標を提供し得るその他の機械的性質を有し得る。ポリマー自体は、分子量、多分散指数、熱性質または履歴、モノマー構造、モノマー選択、架橋密度、ポリマーの粘弾性性質、またはポリマーの安定性を含む指標を提供することに寄与し得る１つ以上の機械的性質を含み得る。種々の形態が可能であり、そして本開示内である。
基板２０２は、曲がり得るか、または組織を損傷することなく、もしくは組織を擦傷することなく組織の周りに巻かれ得る任意の適切な可撓性材料から作製され得る。この基板２０２は生体適合性材料から作製され、そして胃腸管組織、心臓組織、結腸組織、小腸組織、大腸組織および肺組織のような身体の内部組織から、組織を擦傷することなく、そして／または組織を害することなく容易に付与または除去され得る。
この基板２００は、組織と接触する遠位面２０４を有し、そしてまた対向する近位面２０６を有する。１つの実施形態では、この基板２０２は感圧フィルムから作製され得る。別の実施形態では、この基板２０２は感圧ゲル、感圧発泡体、多数の粒子を有する材料、または接着剤およびそれらの任意の組み合わせから作製され得る。一旦、最適量のストレスが基板２０２に付与されると、この基板２０２は調整されて、外科医に、特定の圧縮閾値に到達したことの視覚指標を提供する。この視覚指標は、基板２０２の１つ以上の光学的性質における変化であってもよく、または第１の状態から別の第２の異なる状態への視覚状態の変化であってもよい。
１つの実施形態では、上記指標を提供するための視覚状態における変化は、基板２００の半透明状態から基板２００の不透明状態までであり得る。別の実施形態では、この視覚状態における変化は、第１の色から第２の色までであり得る。種々の形態が可能である。１つの実施形態では、この指標は、図８Ａに示されるような１つの初期色から第２の色まであり得る。この第２の色は、外科医に、特定組織について最適圧縮閾値に到達し、しかも、この組織が複数のステープルのような外科用要素の導入について準備が整っていることの指標を提供する。

あるいは、外科医が指標を見ないか、または聞かないとき、外科医は、代替または偽指標を提供される。外科医は、基板２０２が、一様な様式で圧縮力が付与されていないか、または不十分な圧縮力が組織Ｔに付与されているかのいずれかであることを知る。この事例では、外科医は、器具を再配向し得、そして所望の部位に外科用要素を付与することを再び試み得る。
さらに、外科医が繰り返された試みの後に指標を観察しない場合、外科医は、基板２０２が、一様な様式で圧縮力が付与されていないか、または不十分な圧縮力が組織Ｔに付与されているかのいずれかであることを知る。この事例では、外科医は、次に、所望の部位に、縫合糸のような、別の異なるタイプの外科用要素を付与し得る。

その他の実施形態における袋２０８は、虚血組織、糖尿病組織、特定の処方された医薬に曝された組織、軟組織、厚い組織、または制限されないで胃腸管組織、肺組織、心臓組織、腹部組織、結腸組織または任意のその他の公知の動物またはヒト組織を含むすべての組織タイプについての特定の疾患組織のような、種々の疾患タイプに適切な圧縮力で降伏し得る。
基板２０２の袋２０８は、圧縮される際に単に漏れ易くなるのではなく、それに代わって破裂して開き、外科医に器具からの最適圧縮に到達したことの視覚指標をすぐに提供することを認識すべきである。これら袋２０８は所定量の物質を内部管腔２１２中に有し得、最適圧縮負荷が、突然に開くかまたは破裂するように特に袋２０８の内部圧力から基板２０２に付与されるとき、破裂することを支援する。
図１４を参照して、破裂する袋２０８の内部管腔２１２中のガスは、空気、酸素、窒素、不活性ガス、二酸化炭素または特定の所定のその他のガス状物質であり得る。このガスは、特定量の圧縮力が組織タイプに相補的である支持材料２００上に配置されるとき、袋２０８の側方壁を破壊することをさらに支援する。
袋２０８は、容易な様式で見えるようになるようにするため、袋２０８中に配置されている材料が袋２０８から漏れるために、さらに広く開いて破裂することがさらに認識されるべきである。これら袋２０８の内容物は、流れおよび低粘度に対する抵抗性をほとんどもたない。内容物は、一旦、所定の圧縮力が容易な様式で達成されたなら、袋２０８から、そして基板２０２から、組織に漏れる。このようにして、外科医は、破裂のすぐ後に所望の部位で（または基板２０２に閉じ込められて）色における変化を視覚によって認識し、そして次に外科医は、この指標を観察する際に外科用要素を付与することを知る。
基板２０２は、所望の組織部位の周りに巻かれるように、袋２０８が破裂することなく屈曲可能であり得る。１つの実施形態では、基板２０２は、外科用要素を付与するために装置とは独立に配列され得る。この基板２０２は、図１５に示されるように外科用用途の準備のため、シールされたボックス中でロール状にまたは側方に、適切なディスペンサー中に配置され得る。基板２０２は、組織の第１の側面に付与され得、そして次に、外科用器具の顎２２、２４は、組織および基板２０２の周りで形造られ得、そして次に圧縮され得る。
別の実施形態では、基板２０２の２枚のシートが、組織の第１の側面に、そして次に組織の別の対向する第２の側面に付与され得る。外科用器具の顎は、圧縮のために組織および基板２０２の２枚のシートの周りに形造られ得る。なお別の実施形態では、それに代わって基板２０２は、外科用器具の顎２２、２４中のスロット上、またはそれを通って直接固定されて配列され得、そしてこの器具に隣接する適切に巻かれたディスペンサーに連結される。その後、顎２２、２４は、組織および基板２０２の周りで圧縮され得、圧縮の最適量を決定する。その後、ロールに形成された基板２０２の一部分は、穿孔を有し得る（図示せず）。基板２０２のロールの用いられた部分は、外科用要素がこの穿孔で単に裂くことによって組織中に導入された後、除去され得る。
色変化を見ることは、自然の照明の下で行われ得るか、蛍光照明状態の下で観察され得ることが認識され得る。１つの実施形態では、色素２１４は、蛍光の光の下、または４８８ｎｍのような所定の波長で可視化され得る。１つの実施形態では、外科医は、レンズまたはゴーグルを着用し得、色素２１４を見ることを容易にし、そして外科用ステープルを導入するために適正なステープルラインを配置することを支援する。種々の形態が可能であり、そして本開示内である。
基板２０２は、組織部位に乾燥して、または適切なゲルもしくは潤滑剤とともに付与され得る。１つの実施形態では、外科医は、ＶＡＳＯＬＩＮＥ（登録商標）、もしくはＫＹ ＪＥＬＬＹ（登録商標）のような潤滑剤、または基板２０２が圧縮の間に組織上に留められ、そして残ることを可能にするマルトデキストリンのような別の適切なゲルを付与し得る。
本開示のいくつかの実施形態が図面中に示されているが、本開示はそれらに制限されないことが意図される。なぜなら、本開示は、当該技術分野が許容し、しかも本明細書が同様に読まれ得ると同程度に広いことが意図されるからである。従って、上記の説明は、制限するものと解釈されるべきではなく、好ましい実施形態の例示として解釈されるべきである。

外科用ステープル留めデバイスおよび縫合糸、そして特に、ステープルおよび縫合糸の最適形成のために最適量の組織圧縮が組織に付与されることを確実にするための外科用器具と連結して用いられるかまたは組み合わせて用いられる支持デバイスが提供される。

Claims (2)

  1. 外科用要素の所定の組織タイプ中への挿入の前に組織圧縮の最適量を示すための外科用インジケーターであって：
    その上にある所定の組織および非毒性色素を有する区画を有する基板であって、該区画が、ストレスが付与されないときの第１の破裂していない状態およびストレスが付与されるときの第２の破裂された状態を有し；そして
    ここで、該区画が、引っ張り強度を有して作製され、圧縮によって引き起こされる所定のストレスレベルに圧縮されるとき破裂し、該色素を放出して該所定の組織を別の色に着色し、ここで該圧縮によって引き起こされる所定のストレスレベルが該所定の組織タイプの止血および最適治癒のための最適組織圧縮を示し、該破裂が、該組織圧縮の量が該所定の組織タイプに対する該外科用要素の挿入のために最適である指標であり、該外科用要素がステープルまたは縫合糸である、インジケーター。
  2. 前記色素が前記基板の内部内に囲われ、そして前記組織上に放出されない、請求項１に記載のインジケーター。
