Abstract

Description
Estructura que contiene material de tratamiento
de heridas.
La presente exposición se refiere a instrumentos
y métodos quirúrgicos para mejorar las propiedades de tejido
reparado o unido por grapas quirúrgicas y, más particularmente a
instrumentos quirúrgicos y a una estructura configurada para
aplicar sujetadores mecánicos quirúrgicos de forma concomitante con
un material de tratamiento de heridas biocompatible no mecánico
para mejorar las propiedades del tejido reparado o contiguo en una
zona quirúrgica objetivo. Los métodos no forman parte del invento
reivindicado en este momento.
A lo largo de los años el campo médico ha
utilizado distintas técnicas en un esfuerzo por unir o adherir
tejidos corporales juntos. Históricamente, la sutura fue la técnica
aceptada para volver a unir tejidos cortados y cerrar heridas. La
sutura fue históricamente conseguida con una aguja quirúrgica y un
hilo quirúrgico, y más recientemente, con una variedad de grapas
poliméricas o metálicas, como se describirá a continuación. La
función pretendida de las suturas es sujetar los bordes de una
herida o tejido uno contra otro durante el proceso de curación de
modo que se reduzca la incomodidad, el dolor, la cicatrización y el
tiempo requerido para la curación.
Recientemente, muchos procedimientos que en el
pasado requerían la sutura tradicional han sido reemplazados por la
sutura con grapas que implica la aplicación de las grapas a los
bordes de la herida o tejido con el uso de una grapadora
quirúrgica. Las grapadoras quirúrgicas han sido desarrolladas para
unir tejido adyacente, para proporcionar hemostasis de tejido
adyacente y para proporcionar hemostasis en unión con el corte de
tejido adyacente. Tales grapadoras quirúrgicas incluyen
configuraciones tanto de tipo lineal como anular. Una grapadora y
cortadora lineal típica incluye filas paralelas de grapas con una
ranura para que unos medios de corte se desplacen entre las filas
de grapas.
Grapadoras de tipo lineal típicas están
descritas en la patente norteamericana nº 6.045.560 de McKean y
col., en la patente norteamericana nº 6.032.849 de Mastri y col., y
en la patente norteamericana nº 5.964.394 de Robertson de
cesionaria común. Una grapadora y cortadora anular típica, que
incluye una pluralidad de filas de grapas anulares, típicamente
dos, y una cuchilla anular dispuesta en el interior de las filas de
grapas, está descrita en la patente norteamericana nº 5.799.857 de
Robertson y col. y en la nº 5.915.616 de Viola y col, de cesionaria
común.
Estos tipos de grapadoras quirúrgicas aseguran
el tejido corporal contiguo para cortes mejorados, unen capas de
tejido entre sí y proporcionan hemostasis aplicando filas de grapas
paralelas o anulares para rodear el tejido cuando los medios de
corte cortan entre las filas paralelas o anulares. Por consiguiente,
permitiendo que un cirujano realice todas estas tareas
simultáneamente, las grapadoras quirúrgicas han sido efectivas para
disminuir la cantidad de tiempo que requiere sujetar tejido
uniéndolo. Para mejorar incluso más la unión y hemostasis en casos
en los que la grapadora es usada en tejido muy vascularizado, se han
usado grapadoras quirúrgicas con múltiples filas de grapas con un
alto grado de éxito.
Otros procedimientos quirúrgicos utilizan
compresas, contrafuertes u otros tipos de materiales y tejidos de
refuerzo. Estos contrafuertes son situados típicamente sobre la
superficie del yunque que hace contacto con el tejido y/o sobre la
superficie del cartucho del instrumento de grapado quirúrgico que
hace contacto con el tejido y son asegurados contra el tejido
objetivo durante el disparo del instrumento grapador quirúrgico.
Puede hacerse referencia a la patente norteamericana nº 5.542.594
para una descripción más detallada del uso de contrafuertes en
cooperación con el instrumento grapador quirúrgico.
Aún otros procedimientos quirúrgicos implican la
operación de aplicar (por ejemplo, por pulverización, cepillado,
etc.) un material adhesivo y/o un material sellador a la superficie
externa de la zona quirúrgica objetivo después del procedimiento de
grapado quirúrgico.
Otro procedimiento que ha sido desarrollado
incluye el uso de adhesivos de tejido biológicos que han sido
desarrollados recientemente para la reparación de tejido y la
creación de anastomosis. Generalmente, los adhesivos biológicos
unen tejidos separados uniéndolos para ayudar en el proceso de
curación y para mejorar la resistencia mecánica del tejido. Tales
adhesivos pueden ser usados en lugar de suturar y grapar por ejemplo
en procedimientos quirúrgicos para la reparación del tejido o la
creación de anastomosis.
La aplicación de un adhesivo biocompatible
apropiado ofrece muchas ventajas al paciente y al cirujano por
igual como, por ejemplo, la manera de evitar la penetración del
tejido por agujas y/o grapas, así como el inmediato sellado del
tejido que está siendo tratado. Además, el uso de un adhesivo
biocompatible tiende a minimizar la reacción a un cuerpo extraño y
la cicatrización. El documento WO 2003/094746 de Viola describe un
aparato grapador quirúrgico que incluye un conjunto de material de
cierre de heridas. El documento WU 03/105698 de Orbum y Nolen
describe un anillo circular de refuerzo configurado y adaptado para
usar en unión con un instrumento grapador endoscópico circular que
tiene un conjunto de cartucho de grapas y un conjunto de yunque. El
anillo de refuerzo comprende un depósito con un material de cierre
de heridas que puede ser liberado cuando el aparato es disparado y
las grapas son expulsadas desde el conjunto del cartucho de
grapas.
De acuerdo con la presente exposición, se ha
descrito un conjunto de yunque para un aparato grapador circular.
El conjunto de yunque incluye una cabeza de yunque configurada para
soportar una placa de yunque en él; un árbol que se extiende desde
la cabeza del yunque y configurado para aplicarse selectivamente a
un miembro de conexión del aparato grapador circular; una placa de
yunque conectada operativamente a la cabeza del yunque, definiendo
la placa del yunque una pluralidad de cavidades formadoras de grapas
en ella; y un material de tratamiento de heridas dispuesto en cada
cavidad formadora de grapa de la placa del yunque, comprendiendo el
material de tratamiento de heridas un sellador previamente curado o
un adhesivo previamente curado.
El conjunto de yunque puede además incluir un
revestimiento o forro que cubre la pluralidad de cavidades
formadoras de grapas. De modo deseable, las cavidades formadoras de
grapas están dispuestas en un par de anillos anulares concéntricos
espaciados. Las cavidades formadoras de grapas están en coincidencia
con las ranuras de retención de las grapas respectivas formadas en
un conjunto de cartucho de grapas del aparato grapador
quirúrgico.
Se ha considerado que el material de tratamiento
de heridas está contenido en una cápsula. Por consiguiente, cada
cápsula está configurada para su retención dentro de las cavidades
formadoras de grapas.
De acuerdo con otro aspecto de la presente
exposición, se ha descrito un aparato grapador quirúrgico circular
para realizar una anastomosis, que no forma parte del invento
reivindicado en este momento. El aparato grapador incluye un
conjunto de yunque que tiene una cabeza de yunque configurada para
soportar una placa de yunque en ella; un árbol que se extiende
desde la cabeza del yunque y configurado para aplicarse
selectivamente a un miembro de conexión del aparato grapador
circular; una placa de yunque conectada operativamente a la cabeza
del yunque, definiendo la placa del yunque una pluralidad de
cavidades formadoras de grapas en ella; y un material de
tratamiento de heridas dispuesto en cada cavidad formadora de grapa
de la placa de yunque.
El aparato grapador quirúrgico incluye una parte
de cuerpo tubular que tiene un miembro de conexión configurado para
aplicarse selectivamente al árbol del conjunto de yunque; y un
conjunto de cartucho de grapas soportado operativamente sobre un
extremo distal de la parte de cuerpo tubular. El conjunto de
cartucho de grapas incluye una pluralidad de ranuras de retención
de las grapas en coincidencia con las cavidades formadoras de grapas
del conjunto de yunque; y una grapa dispuesta en cada ranura de
retención de las grapas.
El conjunto de yunque puede incluir un
revestimiento que cubre las cavidades formadoras de grapas. De modo
deseable, el revestimiento es capaz de ser penetrado por las grapas
cuando el aparato grapador quirúrgico es disparado.
El aparato grapador es usado en un método de
realización de un procedimiento de anastomosis quirúrgica, que no
forma parte del invento reivindicado en este momento. El método
incluye las operaciones de proporcionar un aparato grapador
quirúrgico circular que incluye un conjunto de yunque que tiene una
cabeza de yunque configurada para soportar una placa de yunque en
ella; un árbol que se extiende desde la cabeza del yunque y
configurado para aplicarse selectivamente a un miembro de conexión
del aparato grapador circular; una placa de yunque conectada
operativamente a la cabeza del yunque, definiendo la placa de yunque
una pluralidad de cavidades formadoras de grapas en ella; y un
material de tratamiento de heridas dispuesto en cada cavidad
formadora de grapa de la placa del yunque. El aparato grapador
quirúrgico incluye además una parte de cuerpo tubular que tiene una
miembro de conexión configurado para aplicarse selectivamente al
árbol del conjunto de yunque; y un conjunto de cartucho de grapas
soportado operativamente sobre un extremo distal de la parte de
cuerpo tubular. El conjunto de cartucho de grapas incluye una
pluralidad de ranuras de retención de las grapas en coincidencia
con las cavidades formadoras de grapas del conjunto de yunque; y una
grapa dispuesta en cada ranura de retención de las grapas.
El método incluye las operaciones de insertar el
conjunto de yunque en una primera sección intestinal; insertar el
conjunto de cartucho de grapas en una segunda sección intestinal;
conectar un árbol del conjunto de yunque al miembro de conexión de
la parte de cuerpo tubular; aproximar el conjunto de yunque hacia el
conjunto de cartucho de grapas; y disparar el aparato grapador
quirúrgico para accionar las grapas a través de la primera y
segunda secciones intestinales y a las cavidades formadoras de
grapas del conjunto de yunque. Por consiguiente, cuando las grapas
entran en las cavidades formadoras de grapas, las grapas liberan el
material de tratamiento de heridas desde ellas.
El conjunto de yunque puede además incluir un
revestimiento que cubre las cavidades formadoras de grapas. De modo
deseable, el revestimiento es capaz de ser penetrado por las grapas
cuando el aparato de grapado quirúrgico es disparado.
De acuerdo con otro aspecto de la presente
exposición, que no forma parte del invento reivindicado en este
momento, se ha proporcionado un aparato grapador quirúrgico que
incluye un conjunto de yunque configurado para soportar una placa
de yunque en él y una placa de yunque operativamente conectada al
conjunto de yunque y que define una pluralidad de cavidades
formadoras de grapas en ella. El perfeccionamiento incluye
proporcionar un material de tratamiento de heridas en cada cavidad
formadora de grapa de la placa del yunque.
El material de tratamiento de heridas es un
adhesivo previamente curado o un sellador previamente curado y
puede comprender un hemostato y un medicamento.
El adhesivo incluye al menos uno de varios
adhesivos que cura o se endurece al contacto con el tejido, un
adhesivo que cura al ser expuesto a luz ultravioleta (UV), y un
adhesivo que es sensible a la presión. El adhesivo puede también
incluir al menos una de entre proteínas derivadas, material adhesivo
basado en aldehído, y un material a base de cianoacrilato.
El material de sellado incluye al menos un
material sellador de fibrina, un material sellador de tejido a base
de colágeno y a base de polímero sintético, y un material sellador
de hidrogel a base de polietilen-glicol
sintético.
El material hemostático incluye al menos uno de
entre un material a base de fibrina, un material a base de
colágeno, un material a base de celulosa regenerada oxidado, un
material a base de gelatina, y un material de
fibrinógeno-trombina.
El medicamento incluye al menos uno de entre
fármacos, enzimas, factores de crecimiento, péptidos, proteínas,
colorantes, y agentes de diagnóstico.
Las características anteriores de la presente
exposición resultarán más fácilmente evidentes y pueden ser
comprendidas haciendo referencia a la siguiente descripción
detallada de una realización ilustrada de un instrumento, aparato o
estructura quirúrgico, tomada en unión con los dibujos adjuntos, en
los que:
La fig. 1 ilustra una vista en perspectiva de un
aparato grapador quirúrgico de acuerdo con una realización de la
presente exposición;
La fig. 2 es una vista en perspectiva de un
conjunto de yunque de acuerdo con una realización de la presente
exposición;
La fig. 3 es una vista en sección transversal
del conjunto de yunque como tomada a través de 3-3
de la fig. 2;
La fig. 4 es una vista en sección transversal de
un conjunto de yunque alternativo de acuerdo con la presente
exposición, como tomada a lo largo de 3-3 de la fig.
2;
La fig. 5 es una vista en perspectiva de un área
intestinal de un paciente, que ilustra un método de posicionamiento
del conjunto de yunque de las figs. 2 a 4 en la realización de una
anastomosis intestinal;
La fig. 6 es una vista en detalle agrandada del
conjunto de yunque de las figs. 2 a 4 y un conjunto de cartucho de
grapas del aparato grapador quirúrgico posicionado en la zona
quirúrgica objetivo anterior al disparo del aparato grapador

de yunque al producirse el disparo del aparato grapador quirúrgico;

grapas de un aparato grapador quirúrgico de acuerdo con otra
realización de la presente exposición, antes del disparo del
aparato grapador quirúrgico.
Las realizaciones del aparato grapador
quirúrgico descrito en este momento serán ahora descritas en detalle
con referencia a las figuras de los dibujos en las que números de
referencia similares identifican elementos similares o idénticos.
Como se ha usado aquí y como es tradicional, el término
"distal" se refiere a aquella parte que está más lejos del
usuario mientras el término "proximal" se refiere a aquella
parte que está más cerca del usuario.
Con referencia ahora en detalle específico a los
dibujos, en los que números de referencia similares identifican
elementos similares o idénticos a lo largo de las distintas vistas,
la fig. 1 muestra un aparato grapador quirúrgico 10 que emplea la
estructura para aplicar un adhesivo dispersable de acuerdo con la
presente exposición. El aparato 10 incluye un conjunto de
empuñadura 12 que tiene al menos un miembro 14 de empuñadura de
accionamiento pivotable, y además incluye medios de avance 16. Los
medios de avance 16 incluyen un miembro de agarre giratorio 18 cuya
función será descrita a continuación.
Extendiéndose desde el conjunto de empuñadura
12, hay prevista una parte 20 de cuerpo tubular que puede ser
construida de modo que tenga una forma curvada a lo largo de al
menos una parte de su longitud. La parte 20 de cuerpo tubular puede
también ser recta, o en otras realizaciones, la parte 20 de cuerpo
tubular puede ser flexible para curvarse a cualquier configuración.
La parte 20 de cuerpo termina en un conjunto 22 de cartucho de
grapas. El conjunto 22 de cartucho de grapas incluye una agrupación
anular de grapas "S". Posicionado enfrente del conjunto 22 de
cartucho de grapas hay previsto un conjunto de yunque 26 que está
conectado al aparato 10 por un árbol 28 en medios de conexión 30.
El conjunto de yunque 26 y el conjunto 22 de cartucho de grapas
están descritos en la patente norteamericana nº 5.119.983, concedida
el 9 de Junio de 1992 de cesionaria común.
Aunque el aparato 10 ha sido mostrado y descrito
como utilizando un conjunto de cartucho de grapas que tiene una
agrupación anular de grapas posicionada sobre la parte de cuerpo
tubular, y que tiene el conjunto de yunque posicionado enfrente del
conjunto de cartucho de grapas para movimiento de acercamiento y
alejamiento del conjunto de cartucho de grapas, se ha considerado
desde luego que el conjunto de yunque pueda ser posicionado sobre
la parte de cuerpo tubular y el conjunto de cartucho de grapas y la
agrupación de grapas ser posicionados enfrente del conjunto de
yunque para movimiento de acercamiento y alejamiento del conjunto de
yunque. Tal construcción ha de ser considerada dentro del marco de
la presente exposición, pero no forma parte del invento
reivindicado en este momento.
En funcionamiento, el aparato 10 es posicionado
dentro de un órgano tubular en el cuerpo del paciente y los
extremos del órgano que han de ser unidos son posicionados en el
espacio entre el conjunto 22 de cartucho de grapas y el conjunto de
yunque 26 de modo que el conjunto de yunque 26 esté completamente
extendido. Como es tradicional, los extremos del órgano pueden ser
asegurados sobre el conjunto de yunque 26 y el conjunto 22 de
cartucho de grapas por una sutura en bolsa de tabaco antes de la
aproximación del conjunto de yunque 26 en relación al conjunto 22
de cartucho de grapas. Con el conjunto de yunque 26 y el conjunto 22
de cartucho de grapas suturados en bolsa de tabaco, el vástago 28
del conjunto de yunque 26 es acoplado a los medios de conexión 30
dispuestos dentro del conjunto 22 de cartucho de grapas.
Con el fin de aproximar el conjunto de yunque 26
hacia el conjunto 22 de cartucho de grapas, el miembro de agarre 18
es hecho girar para desplazar un miembro de vástago interior (no
mostrado) en una dirección proximal. Esto extrae el conjunto de
yunque 26 a posición adyacente al conjunto 22 de cartucho de grapas
y sitúa los extremos del tejido entre estos dos miembros.
Una vez que la distancia apropiada es ajustada
entre el conjunto de yunque 26 y el conjunto 22 de cartucho de
grapas los medios de interbloqueo 32 pueden ser liberados y las
empuñaduras 14 de accionamiento pueden ser pivotadas para accionar
las grapas a través del tejido y contra el conjunto de yunque 26
para completar la anastomosis circular del órgano tubular. Puede
hacerse referencia a la patente norteamericana nº 5.119.983 para
una descripción y discusión más detalladas de la estructura y
funcionamiento del aparato grapador quirúrgico 10.
Como se ha visto en las figs. 1 a 4, el conjunto
de yunque 26 incluye una cabeza de yunque 34 y un vástago 28 que se
extiende desde la cabeza de yunque 34. La cabeza de yunque 34 está
configurada para soportar una placa de yunque 36 en ella. De modo
deseable, las cavidades 38 formadoras de grapas están dispuestas en
un par de anillos anulares concéntricos espaciados formados en una
placa de yunque 36.
Como se ha visto en las figs. 3 y 4, cada
cavidad 38 formadora de grapa incluye una cantidad de material de
tratamiento de heridas "W" en ella. Como se ha visto en la fig.
3, una película o revestimiento 40 puede ser colocado sobre o en la
superficie de la placa de yunque 36 cubriendo por ello las cavidades
38 formadoras de grapas y reteniendo el material de tratamiento de
heridas "W" en ella. Alternativamente, como se ha visto en la
fig. 4, el material de tratamiento de heridas "W" puede estar
contenido en una cápsula o gel líquido 42 situado en cada cavidad
38 formadora de grapa. De modo deseable, cada cápsula 42 está
adherida o contenida de forma fija de otro modo a las cavidades 38
formadoras de grapas.
De modo deseable, el revestimiento 40 es
fabricado a partir de un material que puede ser penetrado o roto
por grapas quirúrgicas "S". Por ejemplo, el revestimiento 40
puede ser fabricado a partir de un material polimérico, tal como,
polietileno, poliéster, poliuretano y similares. Se ha considerado
que el revestimiento 40 está fabricado de un material
bio-absorbible. De este modo, si una parte o la
totalidad de la funda 40 permanece en el cuerpo después del
procedimiento quirúrgico, esa parte del revestimiento 40 será
absorbida dentro del cuerpo.
El material de tratamiento de heridas es un
adhesivo previamente curado o un sellador previamente curado y
puede incluir un hemostato y/u otro medicamento. De modo deseable,
en uso, el componente sellador del material de tratamiento de
heridas "W" funciona para retardar cualquier hemorragia que
pueda ocurrir desde el tejido, y el componente adhesivo del
material de tratamiento de heridas "W" funciona para asegurar
el tejido aproximado junto.
Se ha considerado que el adhesivo incluye y no
está limitado, a adhesivos que curan al hacer contacto con el
tejido, que curan al exponerlos a la luz ultravioleta (UV), que son
sensibles a la presión, que son cualquier combinación de los
mismos, o cualquier otro adhesivo apropiado conocido. En una
realización, se ha considerado que puede ser usado un adhesivo que
tiene un tiempo de curado de aproximadamente 10 a 15 segundos. En
otra realización, se ha considerado que puede ser usado un adhesivo
que tiene un tiempo de curado de aproximadamente 30 segundos.
El material de tratamiento de heridas "W"
es un adhesivo o un sellador previamente curado. El sellador o
adhesivo previamente curado reaccionará con la humedad y/o el calor
del tejido corporal para activar con ello las propiedades de
sellado y/o adhesivo del sellador o adhesivo. Se ha considerado que
el sellador o adhesivo previamente curado puede ser un hidrogel o
similar.
Ejemplos de adhesivos que pueden ser empleados
incluyen derivado de proteína, materiales adhesivos a base de
aldehído, por ejemplo, los materiales de albúmina/glutaraldehido
comercialmente disponibles vendidos bajo la marca registrada
BioGlue^{TM} por Cryolife, Inc., y materiales a base de
cianoacrilato vendidos bajo las marcas registradas Indermil^{TM}
y Derma Bond^{TM} por Tyco Healthcare Group, LP y Ethicon
Endosurgery, Inc., respectivamente. Ejemplos de selladores, que
pueden ser empleados, incluyen selladores de fibrina y selladores
de tejido a base de colágeno y a base de polímero sintético.
Ejemplos de selladores disponibles comercialmente son materiales
sintéticos a base de polietilen-glicol sintéticos,
de hidrogel vendidos bajo la marca registrada CoSeal^{TM} por
Cohesion Technologies y Baxter International, Inc. Ejemplos de
materiales hemostáticos, que pueden ser empleados incluyen
hemostatos tópicos a base de fibrina, a base de colágeno, a base de
celulosa regenerada oxidizada y a base de gelatina. Ejemplos de
materiales hemostáticos disponibles comercialmente son materiales
de combinación de fibrinógeno-trombina vendidos bajo
las marcas registradas CoStasis^{TM} por Tyco Goup Healthcare
Group, LP, y Tisseel^{TM} vendidos por Baxter International, Inc.
Los hemostatos incluyen aquí astringentes, por ejemplo, sulfato de
aluminio, y coagulantes.
El material de tratamiento de heridas "W"
puede también incluir medicamentos. Los medicamentos pueden incluir
una o más sustancias útiles médica y/o quirúrgicamente tales como
fármacos, enzimas, factores de crecimiento, péptidos, proteínas,
colorantes, agentes diagnósticos o agentes hemostáticos o cualquier
otro farmacéutico usado en la prevención de estenosis.
Volviendo ahora las figs. 5 a 7, se ha ilustrado
el uso de un dispositivo grapador quirúrgico 10 y un conjunto de
yunque desmontable 100 en un procedimiento de anastomosis para
efectuar la unión de secciones intestinales adyacentes "T1 y
T2". El procedimiento de anastomosis es típicamente realizado
usando técnicas quirúrgicas mínimamente invasivas que incluyen
medios e instrumentación laparoscópicos. En el punto en el
procedimiento mostrado en la fig. 5, se ha retirado previamente una
sección intestinal enferma, el conjunto de yunque 26 ha sido
aplicado a la zona de la operación, bien a través de una incisión
quirúrgica o de manera transanal y posicionado dentro de la primera
sección intestinal "T1", y una parte 20 de cuerpo tubular del
dispositivo grapador quirúrgico 10 ha sido insertada de manera
transanal en la segunda sección intestinal "T2". Las secciones
intestinales "T1 y T2" son también mostradas temporalmente
aseguradas alrededor de sus componentes respectivos (por ejemplo,
el árbol 28 del conjunto de yunque 26, y el extremo distal de la
parte 20 de cuerpo tubular) por medios tradicionales tales como una
sutura en bolsa de tabaco (no mostrada).
El cirujano a continuación maniobra el conjunto
de yunque 26 hasta que un extremo proximal del árbol 28 es
conectado operativamente a medios de conexión 30 de la parte 20 de
cuerpo tubular. Después de ello, el conjunto de yunque 26 y la
parte 20 de cuerpo tubular son aproximados a las secciones
intestinales aproximadas "T1 y T2".
Volviendo ahora a la fig. 6, con el conjunto de
yunque 26 aproximado hacia el conjunto 22 de cartucho de grapas y
las secciones intestinales "T1 y T2" sujetadas o capturadas
entre ellos, las cavidades 38 formadoras de grapas del conjunto de
yunque 26 están en coincidencia con ranuras 24 de retención de
grapas del miembro 22 sujetador con grapas. En particular, las
grapas "S", retenidas en las ranuras 24 de retención de grapas,
están en coincidencia con cavidades 38 formadoras de grapas del
conjunto de yunque 26.
Con el conjunto de yunque 26 así posicionado en
relación al conjunto 22 de cartucho de grapas, el dispositivo
grapador quirúrgico 10 es disparado grapando y adhiriendo por ello
las secciones intestinales "T1 y T2" entre sí. Como se ha
visto en la fig. 7, al disparar el dispositivo grapador quirúrgico
10, las grapas "S" son accionadas a través de las secciones
intestinales "T1 y T2", asegurando por ello mecánicamente las
secciones intestinales "T1 y T2" entre sí. Cuando las grapas
"S" son accionadas a través de las secciones intestinales "T1
y T2", las grapas "S" penetran en el revestimiento 40 y
liberan el material de tratamiento de heridas "W" contenido en
las cavidades 38 formadoras de grapas de la placa de yunque 36 sobre
del tejido intestinal "T2". De manera deseable, el material de
tratamiento de heridas "W" se extiende a lo largo de las grapas
"S" al enlace entre los tejidos intestinales "T1 y T2".
De esto modo, si el material de tratamiento de heridas "W"
contiene un adhesivo, el material de tratamiento de heridas
"W" ayuda a adherir las secciones intestinales "T1 y T2"
entre sí.
Se ha considerado además que si el material de
tratamiento de heridas "W" contiene un sellador, la filtración
de sangre en él o sobre el lado de yunque de la grapa "S" (por
ejemplo, en el primer tejido intestinal "T1") es reducida con
relación a un aparato quirúrgico que tiene un conjunto de yunque sin
ningún material de tratamiento de heridas. En uso, cuando las
grapas "S" son accionadas a las cavidades 38 formadoras de
grapas del conjunto de yunque 26, el material de tratamiento de
heridas "W", que incluye un sellador, es desplazado al área que
rodea a las grapas "S".
Simultáneamente con esto, la cuchilla 50 corta
las partes de las secciones intestinales "T1 y T2" situadas
radialmente hacia dentro de la cuchilla 50.
Volviendo ahora a la fig. 8, en una realización
alternativa, el material de tratamiento de heridas "W", que
incluye un sellador, puede estar dispuesto dentro (por ejemplo,
cargado en, empaquetado en, etc.) de las ranuras 24 de retención de
grapas. Por consiguiente, en uso, cuando un aparato grapador
quirúrgico 10 es disparado, el material de tratamiento de heridas
"W" es dispensado o extendido de otro modo sobre el área de la
segunda sección intestinal "T2" que rodea una longitud de
grapas "S".
Aunque se han ilustrado y descrito varias formas
particulares de conjuntos de yunque, y un método particular para
usar los mismos, no formando el método parte del invento
reivindicado en este momento, será evidente también que pueden
hacerse distintas modificaciones sin salir del marco de las
reivindicaciones. Por ejemplo, se ha considerado dentro del marco
de la presente exposición que un material (sellador o adhesivo) de
tratamiento de heridas activado con luz ultravioleta sea usado en
cavidades 38 formadoras de grapas del conjunto de yunque 26. Por
consiguiente, en uso, o bien antes o bien después de disparar el
dispositivo grapador quirúrgico 10, la zona de anastomosis es
irradiada con luz UV para activar con ello el adhesivo, no formando
este uso, sin embargo, parte del invento reivindicado en este
momento.
Así, debería comprenderse que pueden hacerse
distintos cambios en forma, detalle y aplicación del conjunto de
yunque de la presente exposición sin salir del marco de la presente
exposición, estando definido el marco por las reivindicaciones.
Claims (9)
1. Un conjunto de yunque (26) para un aparato
grapador circular (10), comprendiendo el conjunto de yunque:
- \quad
- una cabeza de yunque configurada para soportar una placa de yunque en ella; un árbol (28) que se extiende desde la cabeza del yunque y configurado para aplicarse selectivamente a un miembro de conexión del aparato grapador circular; una placa de yunque (36) conectada operativamente a la cabeza del yunque, definiendo la placa del yunque una pluralidad de cavidades (38) formadoras de grapas en ella; y un material ("W") de tratamiento de heridas dispuesto en cada cavidad formadora de grapa de la placa del yunque, comprendiendo el material de tratamiento de heridas un sellador previamente curado o un adhesivo previamente curado.
2. El conjunto de yunque según la reivindicación
1, en el que el material de tratamiento de heridas es al menos uno
de entre un hemostato y un medicamento.
3. El conjunto de yunque según la reivindicación
2, en el que el adhesivo incluye al menos uno de entre un adhesivo
que cura al exponerlo a la luz ultravioleta (UV), y un adhesivo que
es sensible a la presión.
4. El conjunto de yunque según la reivindicación
2 o la reivindicación 3, en el que el material sellador incluye un
hidrogel.
5. El conjunto de yunque según cualquiera de las
reivindicaciones 2 a 4, en el que el medicamento incluye al menos
uno de entre fármacos, enzimas, factores de crecimiento, péptidos,
proteínas, colorantes, y agentes de diagnóstico.
6. El conjunto de yunque según cualquiera de las
reivindicaciones precedentes, que comprende además un revestimiento
forro (40) que cubre la pluralidad de cavidades formadoras de
grapas.
7. El conjunto de yunque según la reivindicación
6, en el que las cavidades formadoras de grapas están dispuestas en
un par de anillos anulares concéntricos espaciados.
8. El conjunto de yunque según la reivindicación
6 o la reivindicación 7, en el que las cavidades formadoras de
grapas están en coincidencia con ranuras (24) de retención de grapas
respectivas formadas en un conjunto (22) de cartucho de grapas del
aparato quirúrgico.
9. El conjunto de yunque según cualquiera de las
reivindicaciones precedentes, en el que el material de tratamiento
de heridas está contenido en una cápsula, en el que cada cápsula
está configurada para retención dentro de las cavidades formadoras
de grapas.
