Description
Dispositivos y sistemas desmontables para
reducción de pulmón.
La presente invención se dirige generalmente a
un tratamiento de Enfermedad Pulmonar Obstructiva Crónica (EPOC).
La presente invención se dirige más en particular a dispositivos
desmontables de obstrucción de pasos de aire, y sistemas y métodos
para retirar los dispositivos.
La Enfermedad Pulmonar Obstructiva Crónica
(EPOC) se ha convertido en la principal causa de morbilidad y
mortalidad en los Estados Unidos en las tres últimas décadas. EPOC
se caracteriza por la presencia de obstrucción de flujo de aire
debido a bronquitis crónica o enfisema. La obstrucción de flujo de
aire en EPOC se debe en gran medida a anormalidades estructurales
en vías respiratorias más pequeñas. Causas importantes son
inflamación, fibrosis, metaplasia de células caliciforme, e
hipertrofia muscular lisa en bronquiolos terminales.
La incidencia, prevalencia y costos relacionados
con EPOC están en aumento. La mortalidad a causa de EPOC también va
en aumento. En 1991, EPOC fue la cuarta causa principal de muerte
en los Estados Unidos y han aumentado un 33% desde 1979.
EPOC afecta a la vida completa del paciente.
Tiene tres síntomas principales: tos; dificultad para respirar; y
respiración sibilante. En primer lugar, la dificultad para respirar
puede notarse al correr para coger el autobús, cavando en el
jardín, o ascendiendo una colina. Más tarde, puede notarse
simplemente andando por la cocina. Con el tiempo, puede darse cada
vez con menos esfuerzo hasta que está presente en todo momento.
EPOC es una enfermedad progresiva y actualmente
no tiene cura. Los tratamientos actuales para EPOC incluyen la
prevención de daños respiratorios adicionales, farmacoterapia y
cirugía. Cada uno de ellos se detalla a continuación.
La prevención de daños respiratorios adicionales
implica la adopción de un estilo de vida sano. Se cree que dejar de
fumar es la única y más importante intervención terapéutica. Sin
embargo, el ejercicio regular y el control de peso son también
importantes. Los pacientes cuyos síntomas limitan sus actividades
diarias o que tienen una peor calidad de vida pueden necesitar un
programa de rehabilitación pulmonar que incluye entrenamiento
muscular para ventilación y recapacitación respiratoria. La terapia
con oxígeno a largo plazo también puede resultar necesaria.
La farmacoterapia puede incluirterapia
broncodilatadora para abrir las vías respiratorias lo máximo
posible o los agonistas \beta inhalados. Para aquellos pacientes
que responden mal a la técnica citada o que tienen síntomas
persistentes, bromuro de ipratropio puede ser indicado. Además,
cursos de esteroides, como corticosteroides, también pueden
resultar necesarios. Por último, pueden requerirse antibióticos
para prevenir infecciones y gripe y pueden administrarse vacunas
neumococales de manera rutinaria. Desafortunadamente, no existe
evidencia de que el uso temprano y regular de farmacoterapia altere
la progresión de EPOC.
Aproximadamente hace 40 años, se postuló por
primera vez que la fuerza de tensión que tiende a mantener las vías
respiratorias intratorácicas abiertas se perdió en el enfisema y
que retirando quirúrgicamente las partes más afectadas de los
pulmones, la fuerza podía restablecerse parcialmente. A pesar de
que se consideró que la cirugía sería prometedora, el procedimiento
fue abandonado.
La cirugía de reducción de volumen pulmonar
(CRVP) se recuperó más tarde. A inicios de la década de 1990,
cientos de pacientes fueron sometidos al procedimiento. Sin
embargo, el procedimiento no ha resultado favorable debido a que el
Organismo de Asistencia Sanitaria dejó de reembolsar para CRVP.
Desafortunadamente, los datos son relativamente escasos y muchos
factores conspiran para que resulte difícil interpretar los datos
existentes. El procedimiento se encuentra actualmente bajo revisión
en un ensayo clínico controlado. Los datos existentes tienden a
indicar que los pacientes se beneficiaron de este procedimiento en
términos de un aumento en el volumen espiratorio forzado, un
descenso en la capacidad pulmonar total, y una mejora significativa
en la función pulmonar, dispnea, y calidad de vida. Sin embargo, la
cirugía implica complicaciones potenciales. El tejido pulmonar es
muy fino y frágil. Por ello, es difícil suturar tras hacer el
corte. Esto da lugar a un aumento de infección potencial y pérdidas
de aire. De hecho, cerca del treinta por ciento (30%) de dichas
operaciones dan como resultado pérdidas de aire.
Las mejoras en la función pulmonar tras CRVP han
sido atribuidas a al menos cuatro mecanismos posibles. Estos
incluyen un mejor retroceso elástico, corrección de desequilibrio
ventilación/perfusión, eficiencia mejorada de musculatura
respiratoria, y un mejor relleno del ventrículo derecho.
Por último, el trasplante de pulmón también es
una opción. Hoy en día, EPOC es el diagnóstico más común para el
cual se considera el trasplante de pulmón. Desafortunadamente, esta
consideración tan sólo se da para aquellos con EPOC avanzado.
Debido a la limitada disponibilidad de órganos procedentes de
donantes, el trasplante de pulmón está lejos de estar disponible
para todos los pacientes.
En vista de la necesidad en la técnica de
terapias nuevas y mejoradas par EPOC que den resultados más
permanentes que la farmacoterapia mientras sean menos invasivos y
traumáticos que CRVP, recientemente se han propuesto al menos dos
nuevas terapias.
Estas dos nuevas terapias proporcionan una
reducción en el tamaño del pulmón plegando de manera permanente al
menos una parte del pulmón.
De acuerdo con la primera de estas terapias, tal
y como se describe en la Patente de Estados Unidos Nº 6,258,100
(técnica anterior más cercana) asignada al cesionario de la
presente invención, un pulmón puede plegarse obstruyendo un paso de
aire que comunica con la parte del pulmón que sufre el pliegue. El
paso de aire puede obstruirse colocando un miembro de obstrucción
en el paso del aire. El miembro de obstrucción puede ser un
dispositivo del tipo tapón que impide el flujo de aire en ambas
direcciones o en una válvula de un único sentido que permite que el
aire se exhale desde la parte de pulmón que va a plegarse mientras
que evita que el aire se inhale en la parte del pulmón. Una vez que
se ha sellado el paso del aire, el aire residual en el interior del
pulmón se absorberá con el paso del tiempo para provocar que la
parte del pulmón sufra un pliegue.
Tal y como se describe además en la Patente de
Estados Unidos Nº 6,258,100, la parte del pulmón puede plegarse
insertando un conducto en el paso de aire que comunica con la parte
de pulmón que se plegará. Un dispositivo de obstrucción, como una
válvula de un único sentido, avanza posteriormente bajo del
conducto en el paso del aire. El dispositivo de obstrucción se
despliega a continuación en el paso de aire para sellar el paso del
aire y para provocar que la parte de pulmón sufra el pliegue.
La segunda terapia se describe completamente en
la Solicitud copendiente de Estados Unidos con Número de Serie
09/534,244, presentado el 23 de marzo del 2000, para "Aparato y
método para constricción pulmonar" y, también se le asigna al
cesionario de la presente invención. Tal y como aquí se describe,
el dispositivo de constricción pulmonar que incluye una funda de
material elástico está configurado para cubrir al menos una parte
del pulmón. La funda tiene un par de extremos abiertos para
permitir que la parte del pulmón pueda arrastrarse en la manga. Una
vez arrastrada, la parte de pulmón se estrecha por la manga para
reducir el tamaño de la parte de pulmón.
Ambas terapias mantienen grandes promesas para
tratar EPOC. Ninguna de las dos terapias requiere corte y sutura de
tejido pulmonar.
Mientras que cualquiera de las terapias sola
podría ser efectiva para proporcionar reducción de tamaño pulmonar
y tratamiento de EPOC, recientemente se ha propuesto que las
terapias pueden combinarse para conseguir un tratamiento más
efectivo. Más específicamente, se ha propuesto que las terapias
podrían administrarse en serie, con la primera terapia mencionada
aplicada plenamente en primer lugar para evaluación de la eficacia
de la reducción de tamaño pulmonar en un paciente cuyos pulmones
deberían reducirse de tamaño para obtener los mejores resultados.
La primera terapia es ideal para esto ya que no es invasiva y podría
administrarse en la consulta del doctor. Una vez que se ha
confirmado la eficacia de la reducción de tamaño pulmonar y la ha
determinado la identidad de las partes del pulmón que se plegará,
puede administrarse la segunda terapia que es más invasiva.
Con el fin de combinar estas terapias, o
simplemente administrar la primera terapia para evaluación, será
necesario que al menos algunos de los dispositivos de obstrucción
de pasos de aire desplegados sean desmontables. Desafortunadamente,
dichos dispositivos tal y como se conocen actualmente no son
adecuados para su retirada. Mientras que dichos dispositivos son
ampliables para despliegue permanente, dichos dispositivos no se
configuran o adaptan para volver a plegarse tras haberse desplegado
una vez en el paso de aire para facilitar la retirada. Por ello,
existe una necesidad en la técnica de dispositivos de obstrucción
de pasos de aire que se puedan retirar tras haberse desplegado y
sistemas y métodos para retirarlos.

La invención proporciona un dispositivo para
reducir el tamaño de un pulmón que comprende una estructura
obstructora dimensionada para su inserción en el paso de aire que
comunica con una parte del pulmón que va a reducirse en tamaño,
teniendo la estructura obstructora una dimensión externa que se
dimensiona de tal manera cuando se emplea en el paso de aire para
evitar que el aire fluya en la parte del pulmón, para plegar la
parte del pulmón y para reducir el tamaño del pulmón, siendo la
estructura obstructora plegable para permitir la retirada del
dispositivo de obstrucción del paso de aire.
La invención proporciona además un montaje que
comprende un dispositivo para reducir el tamaño de un pulmón,
estando dimensionado el dispositivo para su inserción en el paso de
aire que comunica con una parte del pulmón que va a reducirse en
tamaño, teniendo el dispositivo una dimensión externa que se
dimensiona de tal manera cuando se emplea en el paso de aire para
evitar que el aire fluya en la parte del pulmón que va a plegarse
la parte del pulmón para reducir el tamaño del pulmón, un catéter
que tiene un lumen interno y que está configurado para pasar por la
tráquea, en un paso de aire, y un retractor dimensionado para que
pase por el lumen interno del catéter, agarrando el dispositivo, y
tirando del dispositivo de obstrucción próximamente al lumen
interno para retirar el dispositivo del paso de aire. El
dispositivo puede plegarse tras haberse desplegado para permitir
que el dispositivo sea tirado próximamente en el lumen interno del
catéter por la acción del retractor.
La invención proporciona además un método para
retirar un dispositivo de obstrucción de paso de aire de un paso de
aire en el que el dispositivo se despliega. El método incluye los
pasos de pasar un catéter, que tiene un lumen interno, bajo la
tráquea y en el paso del aire, avanzar un retractor bajo el lumen
interno del catéter hasta el dispositivo, agarrar el dispositivo
con el retractor, plegando el dispositivo para liberar el
dispositivo del despliegue en el paso del aire, y tirando del
dispositivo con el retractor próximamente en el lumen interno del
catéter.
La invención proporciona además un dispositivo
para obstrucción de paso de aire que comprende una estructura de
marco, y una membrana flexible que recubre la estructura de marco.
La estructura de marco es plegable tras el avance del dispositivo
en el paso de aire, extensible en una estructura rígida tras el
despliegue en el paso de aire por medio del cual la membrana
flexible obstruye el flujo de aire inhalado en una parte del pulmón
que comunica con el paso de aire, y se vuelve a plegar tras la
retirada del paso de aire.
La invención proporciona además un dispositivo
para obstrucción de paso de aire que comprende medios de armazón
para formar una estructura de soporte, y medios de membrana
flexible que cubren la estructura de soporte. Los medios de armazón
son extensibles hasta un estado expandido en un paso de aire para
posicionar los medios de membrana para obstruir el flujo de aire en
el interior del paso de aire y son plegables para retirar el
dispositivo del paso de aire.
Las características de la presente invención que
pretender ser nuevas se presentan con particularidad en las
reivindicaciones adjuntas. La invención, junto con objetos y
ventajas adicionales de la misma, pueden entenderse mejor haciendo
referencia a la siguiente descripción tomada en conjunto con los
dibujos acompañantes, en las varias figuras en las cuales los
números de referencia identifican idénticos elementos, y donde:

Ahora, en referencia a la Fig. 1, es una vista
en sección de un sistema respiratorio sano. El sistema respiratorio
20 reside en el interior del tórax 22 que ocupa un espacio definido
por la pared del pecho 24 y el diafragma 26.
El sistema respiratorio 20 incluye la tráquea
28, el bronquio izquierdo principal 30, el bronquio derecho
principal 32, las ramas bronquiales 34, 36, 38, 40, y 42 y
sub-ramas 44, 46, 48, y 50. El sistema respiratorio
20 incluye además lóbulos pulmonares izquierdos 52 y 54 y lóbulos
pulmonares derechos 56, 58, y 60. Cada rama bronquial y
sub-rama comunica con una respectiva parte
diferente de un lóbulo pulmonar, bien con el lóbulo pulmonar entero
o con una parte del mismo. Tal y como aquí se emplea, el término
"paso de aire" pretende denotar bien una rama bronquial o una
sub-rama que comunica con un lóbulo pulmonar
individual correspondiente o una parte del lóbulo pulmonar para
proporcionar aire inhalado al mismo o para conducir aire exhalado
del mismo.
Las características de un sistema respiratorio
sano es el diafragma arqueado o con un arco interior 26. Cuando el
individuo inhala, el diafragma 26 se endereza para aumentar el
volumen del tórax 22. Esto provoca una presión negativa en el
interior del tórax. La presión negativa dentro del tórax provoca a
su vez que los lóbulos pulmonares se llenen de aire. Cuando el
individuo exhala, el diafragma vuelve a su condición arqueada
original para reducir el volumen del tórax. El volumen reducido del
tórax provoca una presión positiva en el interior del tórax que a
su vez provoca la exhalación de los lóbulos pulmonares.
A diferencia del sistema respiratorio sano de la
Fig. 1, la Fig. 2 ilustra un sistema respiratorio que sufre EPOC.
Aquí puede observarse que los lóbulos pulmonares 52, 54, 56, 58, y
60 están aumentados y que el diafragma 26 no está arqueado sino
sustancialmente recto. Por ello, el individuo es incapaz de respirar
de manera normal moviendo el diafragma 28. En su lugar, con el fin
de crear la presión negativa en el tórax 22 necesaria para respirar,
este individuo debe mover la pared del pecho hacia fuera para
aumentar el volumen del tórax. Esto da como resultado una
respiración ineficiente que provoca que estos individuos respiren
rápidamente con respiraciones superficiales. Se ha descubierto que
las cúspides 62 y 66 de los lóbulos pulmonares superiores 52 y 56,
respectivamente, están más afectadas por EPOC.
De acuerdo con esta realización de la presente
invención, el tratamiento de EPOC o la evaluación se inicia
introduciendo un conducto o catéter 70 en dirección descendente por
la tráquea 28, en un bronquio central como el bronquio de raíz
izquierda 32, y en un paso de aire como la rama bronquial 42 o la
sub-rama bronquial 50. Un dispositivo de
obstrucción de paso de aire que describe la presente invención
avanza a continuación en dirección descendente en un lumen interno
71 del catéter 70 para desplegarse en el paso de aire. Una vez
desplegado, el dispositivo de obstrucción impide que el aire
inhalado entre en la parte del pulmón que va a plegarse. De acuerdo
con la presente invención, es preferible que el dispositivo de
obstrucción tome la forma de una válvula de un solo sentido. Además
de impedir que el aire entre en la parte del pulmón, el dispositivo
además permite que el aire en el interior del pulmón pueda
exhalarse. Esto da como resultado un pliegue más rápido del pulmón.
Sin embargo, los dispositivos de obstrucción que impiden que el
aire inhalado y exhalado fluya son considerados como pertenecientes
al alcance de la invención.
El catéter 70 está preferiblemente formado por
un material flexible como polietileno. Asimismo, el catéter 70 se
forma preferiblemente con una curva 72 para ayudar la introducción
del catéter desde el bronquio principal de la derecha 32 a la rama
bronquial 42.
Las Figs. 3 y 4 muestran un dispositivo para
obstrucción de paso de aire 80 que incorpora la presente invención.
El dispositivo 80 incluye un extremo próximo 82 y un extremo distal
84. El dispositivo 80 incluye además una estructura de armazón 86
que incluye soportes de armazón 88, 90, y 92.
Cada uno de los soportes de armazón tiene una
forma para definir una porción central generalmente cilíndrica 94 y
un par de porciones cónicas arqueadas hacia dentro que se extienden
de manera opuesta 96 y 98. La estructura del armazón incluye además
un conjunto de miembros de fijación 100, 102, y 104 que se
extienden distalmente desde el extremo próximo 82. Los miembros de
fijación tienen la forma generalmente cónica y terminan en
proyecciones o anclas de fijación 106, 108, y 110 que se extienden
radialmente hacia fuera.
Cubriendo y parcialmente cerrando la estructura
del armazón 86 se encuentra una membrana flexible 112. La membrana
flexible se extiende sobre las partes generalmente cilíndricas y
cónicas 94 y 98 definidas por la estructura del armazón. Por ello,
la membrana flexible se abre en la dirección próxima.
La membrana flexible puede estar formada por
silicona o poliuretano, por ejemplo. Puede asegurarse a la
estructura de armazón de una manera conocida en la técnica como por
medio de un pliegue, un remache o por medio de una adhesión.
La estructura de armazón 86 y el dispositivo 80
se muestran en las Figs. 3 y 4 cuando el dispositivo aparece
completamente desplegado en un paso de aire. Los soportes de la
estructura de armazón y los miembros de fijación de la estructura
de armazón están preferiblemente formados por acero inoxidable o
Nitinol u otro material adecuado que tenga memoria de una forma
original. La estructura del armazón permite que el dispositivo se
pliegue para avanzar en dirección descendente en el lumen interno
71 del catéter 70 en el paso de aire donde el dispositivo se
despliega. Una vez que se alcanza el punto de despliegue, el
dispositivo se expulsa del catéter para tomar su forma original en
el paso de aire. Al hacer esto, la porción generalmente cilíndrica
94 entra en contacto con la pared interior del paso de aire y las
proyecciones de fijación 106, 108, y 110 perforan la pared el paso
de aire para fijar o anclar el dispositivo 80 en el interior del
paso de aire.
Cuando el dispositivo 80 se despliega, la
estructura armazón 86 y la membrana flexible 112 forman una
estructura obstructora o una válvula de un único sentido. Las Figs.
5 y 6 muestran la acción de la válvula del dispositivo 80 cuando se
despliega en el paso de aire, como la rama branquial 42.
Tal y como se muestra en la Fig. 5, durante la
inhalación, la membrana flexible se llena de aire y se expande
hacia fuera para obstruir el paso de aire 42. Esto impide que el
aire entre en la parte del pulmón que va a plegarse. Sin embargo,
tal y como se muestra en la Fig. 6, durante la expiración, la
membrana flexible 112 se desvía hacia dentro para obstruir sólo
parcialmente el paso de aire 42. Esto permite que el aire, que
puede estar en la parte del pulmón que va a plegarse, se exhale
para un pliegue más rápido de la parte del pulmón. La Fig. 7 es
otra vista que muestra el dispositivo 80 durante la expiración con
una parte 114 de la membrana 112 desviada hacia dentro.
Las Figs. 8 y 9 ilustran un modo en el que el
dispositivo 80 puede retirarse del paso de aire 42 de acuerdo con
una realización de la presente invención. Tal y como se ha
mencionado previamente, puede desearse retirar el dispositivo 80 si
sólo se emplea para evaluar la eficacia de plegar una parte del
pulmón o si se descubre que el tratamiento más efectivo puede ser
con el pliegue de otras partes del pulmón.
El dispositivo 80 se muestra en la Fig. 8 en un
estado completamente desplegado. El catéter 70 que tiene el lumen
interno 71 avanza hasta el extremo próximo del dispositivo 80. En
la Fig. 8, puede observarse que los miembros de fijación 102 y 104
definen un radio cónico más grande que los soportes del armazón 88
y 90. Por ello, cuando el extremo próximo del dispositivo se
engancha por un retractor y el catéter 70 se mueve distalmente tal
y como se muestra en la Fig. 9, el lumen del catéter se engancha
con los miembros de fijación 102 y104 antes de que se enganche con
los soportes del armazón 88 y 90. Esto provoca que las proyecciones
de fijación se desenganchen en primer lugar de la pared interna del
paso de aire 42. Con el dispositivo ahora libre de la pared lateral
del paso de aire, el retractor se empleará para tirar del
dispositivo en el lumen interno 71 del catéter 70 provocando que la
estructura de soporte y por lo tanto el dispositivo se pliegue. El
dispositivo plegado puede ahora entrar completamente en el lumen
interno del catéter para retirarse.
Las Figs. 10-12 muestran otra
realización de la presente invención para retirar el dispositivo 80
del paso de aire 42. Aquí, el catéter 70 se introduce en el
broncoscopio 118 del dispositivo 80. El retractor toma la forma de
unos fórceps 120.
En la Fig. 10 puede observarse que los fórceps
se han enganchado con el extremo próximo 82 del dispositivo 80. En
la Fig. 11 los fórceps 120 se mantienen fijos mientras que el
catéter 70 avanza de modo que el lumen interno 71 del catéter 70 se
engancha con los miembros de fijación 102 y 104. Un avance
adicional del catéter 70 tal y como se observa en la Fig. 2 desvía
las proyecciones de fijación 110 y 108 hacia dentro, lejos de la
pared interna del paso de aire 42. Ahora, pueden emplearse los
fórceps para tirar del dispositivo 80 en el lumen interno 71 del
catéter 70 para retirar el dispositivo 80 del paso de aire 42.
Las Figs. 13 y 14 muestran otro dispositivo
desmontable para obstrucción de paso de aire 130 y un método para
retirarlo de un paso de aire de acuerdo con la presente invención.
El dispositivo 130 se muestra en la Fig. 13 plegado en el paso de
aire 42 y el catéter 70 está en la posición preparada para retirar
el dispositivo 130 del paso de aire 42.
El dispositivo 130 es de configuración similar a
la del dispositivo 80 previamente descrito. Sin embargo, aquí los
miembros de fijación 136 y 138 son extensiones de los soportes del
armazón 132 y 134, respectivamente. Con este fin, se observará en
la Fig. 13 que los soportes del armazón 132 y 134 se cruzan en el
punto del eje central 140 en un extremo distal 142 del dispositivo
130. Se extienden distalmente y a continuación vuelven en un ángulo
agudo para terminar en los extremos de fijación o anclaje 146 y
148. Cuando el dispositivo se despliega tal y como se muestra en la
Fig. 13, las partes cilíndricas del armazón de soporte se enganchan
con la pared interna del paso de aire 42 y los puntos de fijación
146 y 148 se proyectan en la pared interna del paso de aire 42 para
mantener el dispositivo en una posición fija. La membrana flexible
150 se extiende desde la línea discontinua 152 hasta el eje central
o punto de cruce 140 de los soportes de armazón 132 y 134 para
formar una válvula de único sentido.
Cuando es dispositivo se va a retirar, la
estructura de armazón del dispositivo 130 se mantiene estática por
un retractor en el interior del catéter 70 y el catéter avanza
distalmente. Cuando el catéter 70 se engancha con los soportes del
armazón 132 y 134, los soportes del armazón se desvían hacia dentro
desde sus posiciones de línea discontinua hasta sus posiciones de
línea continua. Esto también provoca que los miembros de fijación
136 y 138 que van a desviarse hacia dentro desde sus posiciones de
línea discontinua hasta sus posiciones de línea continua en la
dirección de las flechas 154. Estas acciones desenganchan el
dispositivo 130 desde su pared interna del paso de aire 42. Ahora,
el retractor puede tirar del dispositivo en el lumen interno 71 del
catéter 70 para retirar el dispositivo 130 desde el paso de aire
42.
Las Figs. 15-17 muestran un
dispositivo desmontable adicional para obstrucción de paso de aire
160 que incorpora la presente invención. Tal y como se muestra en
el estado plegado inicial en la Fig. 15, el dispositivo 160 incluye
un conjunto de soportes de armazón 162, 164, 166 y 168. Los
soportes del armazón se extienden entre un anillo próximo 170 y un
anillo distal 172. El dispositivo 160 se corta preferiblemente con
láser a partir de una lámina de Nitinol.
Debido a que cada uno de los soportes del
armazón es idéntico, aquí solamente se describirá el soporte de
armazón 164. Tal y como se observará, el soporte 164 incluye un
punto con curva 174 con una sección relativamente larga 176 que se
extiende distalmente desde el punto con curva 174 y una sección
relativamente corta 178 que se extiende próximamente desde el punto
con curva 174. La sección corta 178 incluye una proyección o ancla
de fijación 180 que se extiende ligeramente de manera distal desde
el punto con curva 174.
Las Figs. 16 y 17 muestran el dispositivo 160 en
su configuración desplegada. Cuando el dispositivo de despliega,
avanza por el catéter hasta su punto de despliegue en su estado
plegado tal y como se muestra en la Fig. 15. Cuando se alcanza el
punto de despliegue, el dispositivo 160 se contiene fuera del
catéter y los anillos 170 y 172 se tiran entre sí. Esto provoca que
el dispositivo se curve en los puntos con curva de los soportes del
armazón. Esto forma las proyecciones de fijación 180, 182, y 184
que se extienden en la pared interna del paso de aire para fijar en
posición.
Las secciones relativamente largas de los
soportes del armazón se cubren con una membrana flexible 186 tal y
como se muestra en las Figs. 16 y 17 para formar una válvula con un
único sentido. La válvula funciona tal y como se ha descrito
previamente para obstruir flujo de aire inhalado pero para permitir
flujo de aire exhalado.
Las Figs. 18-20 ilustran una
manera de retirar el dispositivo 160 de un paso de aire. De nuevo,
un catéter 70 avanza en dirección descendente por un broncoscopio
118 hasta el dispositivo 160. A continuación, un retractor que
incluye unos fórceps 120 y un clavo 190 avanzan hasta el
dispositivo. El clavo 190, que transporta un disco de diámetro
mayor 192, se extiende en el dispositivo cuando los fórceps 120
agarran el anillo próximo 170 del dispositivo 160. El clavo 190
continúa avanzando hasta que el disco 192 se engancha con el anillo
distal 172 del dispositivo 160 tal y como se muestra en la Fig. 19.
A continuación, mientras los fórceps 120 sujetan el anillo próximo
170, el clavo 190 y el disco 192 avanzan distalmente transportando
el anillo distal 172 de manera distal. Esto provoca que el
dispositivo 160 se enderece y se pliegue tal y como se muestra en
la Fig. 20. Ahora, se puede tirar de los fórceps 120, el clavo 190,
y el dispositivo 160 en el lumen interno 71 del catéter 70 para
retirar el dispositivo. Tal y como aquellos expertos en la técnica
apreciarán, los pasos precedentes pueden invertirse para desplegar
el dispositivo 160.
Claims (13)
1. Un dispositivo (80, 130, 160) para reducir el
tamaño de un pulmón que consiste en una estructura obstructora
dimensionada para insertarse en un paso de aire (42) que comunica
con una porción del pulmón para reducirse en tamaño, teniendo la
estructura obstructora una dimensión externa que toma tal forma
cuando se despliega en el paso de aire (42) para impedir que el aire
fluya en la parte del pulmón, para plegar la parte del plumón y
para reducir el tamaño del pulmón,
donde la estructura obstructora es plegable para
permitir la retirada del dispositivo de obstrucción (80, 130, 160)
del paso de aire (42), y
donde la estructura obstructora comprende una
estructura de armazón (86) y una membrana flexible (112, 150, 186)
que cubre la estructura de armazón (86), siendo la estructura de
armazón (86) plegable tras el avance del dispositivo (80, 130, 160)
en el paso de aire (42) y extensible en una estructura rígida de
armazón tras el despliegue en el paso de aire (42) de modo que la
membrana flexible (112, 150, 186) obstruye el flujo de aire
inhalado en la parte del pulmón que reducirá su tamaño,
caracterizado porque
la estructura de armazón (86) está constituida
por soportes de armazón (88, 90, 92, 132, 134, 162, 164, 166, 168),
teniendo cada uno de los soportes de armazón (88, 90, 92, 132, 134,
162, 164, 166, 168) una forma para definir generalmente un par de
porciones finales internamente cónicas y que se extienden en
dirección opuesta (94, 98, 176, 178).
2. El dispositivo (80, 130) de la reivindicación
1 donde cada uno de los soportes del armazón (88, 90, 92, 132, 134)
tiene una forma para definir una parte central generalmente
cilíndrica (94) y un par de porciones finales internamente cónicas
y que se extienden en dirección opuesta (96, 98).
3. El dispositivo de la reivindicación 2 donde
la membrana (112, 150) cubre y parcialmente cierra la estructura
del armazón (86) y se extiende sobre la parte generalmente
cilíndrica (94) y la parte final cónica (98).
4. El dispositivo (160) de la reivindicación 1
donde los soportes del armazón (162, 164, 166, 168) se extienden
entre un anillo próximo (170) y un anillo distal (172) y cada uno
de los soportes del armazón (162, 164, 166, 168) incluye un punto
con curva (174) con una sección relativamente larga (176) que se
extiende distalmente desde el punto con curva (174) y una sección
relativamente corta (178) que se extiende próximamente desde el
punto con curva (174).
5. El dispositivo de la reivindicación 4 donde
la sección relativamente larga (176) de los soportes del armazón
(162, 164, 166, 168) está cubierta por una membrana flexible
(186).
6. El dispositivo (80, 130, 160) de cualquiera
de las reivindicaciones 1 a 5 donde la estructura obstructora forma
una válvula de único sentido.
7. El dispositivo (80, 130, 160) de las
reivindicaciones 1 a 6 que incluye además al menos un enganche o
ancla (106, 108, 110, 146, 148, 180, 182, 184) que ancla el
dispositivo (80, 130, 160) en el interior del paso de aire (42)
cuando el dispositivo (80, 130, 160) se despliega y donde al menos
un enganche o ancla (106, 108, 110, 146, 148, 180, 182, 184) es
capaz de soltarse durante el pliegue de la estructura obstructora y
la retirada del dispositivo (80, 130, 160) del paso de aire
(42).
8. El dispositivo (80, 130, 160) de la
reivindicación 7 donde la estructura plegable del armazón (86)
incluye al menos un enganche o ancla (106, 108, 110, 146, 148, 180,
182, 184).
9. El dispositivo (80, 130, 160) de las
reivindicaciones 1 a 8 donde la membrana flexible (112, 150, 186)
tiene un extremo próximo abierto y un extremo distal cerrado.
10. El dispositivo (80, 130, 160) de la
reivindicación 9 donde la membrana flexible (112, 150, 186) infla
el aire inhalado para impedir que el aire fluya en la parte del
pulmón y al menos desinfla parcialmente el aire exhalado para
permitir que el aire fluya desde la parte del pulmón.
11. Un montaje que comprende:
un dispositivo (80, 130, 160) de acuerdo con las
reivindicaciones precedentes,
un catéter (70) que tiene un lumen interno (71)
y que está configurado para pasar por una tráquea (28), en el paso
de aire (42); y
un retractor dimensionado para pasar por el
lumen interno (71) del catéter (70), agarrando el dispositivo (80,
130, 160), y tirando del dispositivo de obstrucción (80, 130, 160)
próximamente en el lumen interno (71) para retirar el dispositivo
(80, 130, 160) del paso de aire (42),
el dispositivo (80, 130, 160) que es plegable
tras haber sido desplegado para permitir que se pueda tirarse del
dispositivo (80, 130, 160) de manera próxima en el lumen interno
(71) del catéter (70) por acción del retractor.
12. El montaje de la reivindicación 11 donde el
dispositivo (80, 130, 160) es plegable por el catéter (70) cuando
se tira del dispositivo (80, 130, 160) en el lumen interno (71) del
catéter (70) por acción del retractor.
13. El montaje de la reivindicación 11 donde el
retractor incluye medios de pliegue para plegar el dispositivo (80,
130, 160) antes de tirar del dispositivo (80, 130, 160) en el lumen
interno (71) del catéter (70).
