Abstract
内窥镜装置41具有具备不同功能的第1~第3内窥镜(1A~1C),将设置于第1内窥镜(1A)的软性部(10)上的粗直径部(10b)的外径,设定在相同内窥镜装置(41)中使用的第2、第3内窥镜(1B、1C)的软性部(45、46)的最粗外径与最细外径的范围内。手术者在操作第1~第3内窥镜(1A~1C)时,由于各内窥镜(1A~1C)的软性部(10、45、46)的外径在一定的范围内,因此不会感到有极端的不适感,能够得到共通的操作感。
Description
技术领域
本发明涉及一种能够提高向大肠的插入性的内窥镜装置。
背景技术
通常,在内窥镜的插入部设置具有可挠性的软性部,该软性部的前端通过弯曲部连接有前端部。在该前端部的前端面上,按规定地设置有观察窗及照明窗、处置器具插入通道开口部等。
软性部通过加大前端附近部分的可挠性而容易柔软地弯曲,另一方面,通过减小基端附近部分的可挠性使插入性良好为已知技术。
因此,例如在日本专利申请特开2001-190494号公报中,公开了一种通过使软性部的外径从前端侧开始向基端侧逐渐增大地形成,使基端附近的部分的可挠性与前端侧附近相比较小,在确保前端附近的可挠性的同时提高插入性的技术。
但是,在同一手术者在1个内窥镜装置中使用具有不同功能的多个内窥镜的情况下,设置于各内窥镜的软性部上的粗直径部的外径,对于每个内窥镜有很大的不同时,由于手术者在把持各内窥镜的大直径部时会感到非常不适,损害了插入性。
本发明鉴于上述问题而做出,其目的是提供一种内窥镜装置,手术者在把持设置于1个内窥镜装置中使用的多个内窥镜的软性部上的大直径部时不会感到非常不适。
发明内容
本发明的内窥镜装置为,具有:包括软性部的第1内窥镜,该软性部包括设置于前端侧的细直径部,和与该细直径部相比设置于靠近手一侧并且与该细直径部相比外径较粗的粗直径部;能够使用与前述第1内窥镜相同的光源装置或视频处理器,软性部全长具有大致相同的外径的第2内窥镜;以及软性部全长为大致相同的外径且该软性部的外径与上述第2内窥镜的外径大致相同或在其以下的第3内窥镜,其特征为,前述第1内窥镜的至少粗直径部的外径与前述第2内窥镜的软性部的外径大致相同或在其以下,并且与前述第3内窥镜的软性部的外径大致相同或在其以上。

具体实施方式
以下,参照附图对本发明的实施方式进行说明。
第1实施方式
图1~图7D示出了本发明的第1实施方式。
如图所示的内窥镜1具有在前端内置有CCD等固体摄像元件的插入部2;连接在该插入部2的基端上、观察者把持以进行各种操作的操作部3;从该操作部伸出的万向软线(ュニバ一サルコ一ド)4。
在万向软线4的端部设置有连接部5。在连接部5上设置有光波导连接器6及照相机连接器7等。光波导连接器6及照相机连接器7等连接在光源装置及照相机控制装置等周边设备上。
插入部2从前端侧开始,具有前端部8、可自由弯曲的弯曲部9以及具有可挠性的软性部10,该软性部10的基端连接在操作部3上。此外,在前端部8的前端面上,按规定地设置有未图示的观察窗及照明窗、处置器具插入通道开口部、送气送水喷嘴等。
另一方面,在操作部3上设置有遥控弯曲部9的弯曲动作的弯曲操作杆11、插入钳子等处置器具的处置器具插入口12、进行图像的抓取保存(フリ一ズ)和解除等的多个开关13等。
如图2所示,软性部10具有设置在前端上、在整个长度上外径大致相同的细直径部10a;设置在该细直径部10a的手前侧上、比细直径部10a的直径粗的粗直径部10b;圆滑地连接设置在细直径部10a与粗直径部10b之间的锥部10c。此外,在图2中,细直径部10a与锥部10c的边界由箭头A表示,锥部10c与粗直径部的边界由箭头B表示。
从作为插入部2的前端的前端部8的前端面开始到箭头A所示的细直径部10a的后端的位置为止的长度为30cm、40cm、50cm等,根据使用的内窥镜1的用途存在各种不同的长度,但通常不存在达到70cm的长度。因而,由箭头A所出的细直径部10a的后端比距离前端部8的前端面70cm处更靠前方地设置。
图3大致地示出了软性部10的内部结构的一部分。
作为软性部10的外壳的可挠管20从内侧开始具有卷绕方向相互反向的螺旋管21a、21b;网状管22、外皮树脂24。螺旋管21a、21b,网状管22在贯穿全长上具有大致一定的内外径,通过变化外皮树脂24的壁厚,形成细直径部10a、粗直径部10b及连接它们的锥部10c。此外,在本实施方式中,螺旋管21a、21b为2层,但螺旋管也可为1层或3层以上。
作为可挠管20的成形方法,具有例如将规定组装的螺旋管21a、21b、网状管22作为芯型,通过挤压成形将外皮树脂24成形于网状管22上的方法。此时,通过变化芯型的拉伸速度,形成细直径部10a、锥部10c及粗直径部10b。
详细地,通过加快芯型的拉伸速度形成细直径部10a,另外,通过减慢芯型的拉伸速度形成粗直径部10b,进一步,通过连续地变化从细直径部10a到粗直径部10b,或从粗直径部10b到细直径部10a之间的拉伸速度形成锥部10c。
但是,这种成形方法由于使用了相同内径的模具,难以高精度地设定10a~10c各部分的外径。
对此,如图4A~4C所示,对以单一外径制作的棒状的可挠管坯料20’,如果通过使用多个锥状研磨砂轮23进行无心研磨,形成细直径部10a及锥部10c,则能够高精度地形成10a~10c各部分的外径。
即,如图4A所示,首先,将外径具有与粗直径部10b相同尺寸的可挠性管坯料20’相对于在圆周上设置的多个锥状研磨砂轮23的轴芯相对地接近,此后,如图4B所示,将可挠管坯料20’相对于多个锥状研磨砂轮23相对地推压,将可挠管坯料20’的表面研磨成与多个锥状研磨砂轮23相仿的形状,如图4C所示,在可挠管坯料20’的前端上按规定地形成细直径部10a和锥部10c。
通过这种无心研磨,能够分别高精度地形成细直径部10a、锥部10c、粗直径部10b。
或者,也可以如图5A~5D所示地通过挤出成形分别高精度地形成细直径部10a、锥部10c、粗直径部10b。
即,在该成形方法中,如图5A所示地,首先通过挤出成形制作由螺旋管21a、21b、网状管22、第1外皮树脂24a构成的、全长的外径与细直径部10a相同尺寸的可挠性管坯料20”,此后,如图5B所示地,在与细直径部10a相当的部分上覆盖热收缩管等管体25,在此状态下,如图5C所示地,改变模具再次进行挤压成形,与管体25连续地在可挠性管坯料20”的外周上形成与粗直径部10b相同外径的第2外皮树脂24b。
此后,如图5D所示地,将管体25从可挠性管坯料20”上剥离除去,将与第2外皮树脂24b的锥部10c相当的部分如虚线所示地通过由研磨机等研磨,形成图2或图3所示的具有细直径部10a、锥部10c、粗直径部10b的可挠性管20。
接着,加热可挠性管20,通过将第1外皮树脂24a、第2外皮树脂24b和网状管22分别热熔接,使各部件可靠地紧密接触。
在此情况下,如图6所示,在下个工序中,在可挠管20的表面上印刷标记26,也可在其上涂覆薄膜的外涂层27。通过由外涂层27来涂覆标记26,能够使第1外皮树脂24a与第2外皮树脂24b的边界更圆滑。
另外,在图6中,设定为在贯穿软性部10的大致全长上等间隔标注的标记26之间具有锥部10c的状态。该标记26在本实施方式中表示从内窥镜8的前端面开始的距离,“40”意为从前端面开始40cm的位置、“50”意为从前端面开始50cm的位置。
在本实施方式中,锥部10c形成于从前端面开始约43~48cm的位置上。通过将锥部10c设置在等间隔标注的多个标记26之间,不需要在锥部10c上印刷标记26。由于向锥部10c进行印刷很麻烦,通过省略该印刷可实现制造成本的降低。
但是,细直径部10a具有在被弯曲为最小半径的情况下,如图2的虚线所示地能够弯曲180°以上的长度。在此情况下,最小半径是指在细直径部10a自然弯曲时,如图3所示的螺旋管21a、21b中任何一方的带状线部分相互冲突,不能弯曲到其以上的状态。
以下,对这种结构的本实施方式的作用进行说明。
图7A~图7D示出了将本实施方式的内窥镜1的插入部2插入大肠内的状态。大肠主要由肛门28、直肠29、S状结肠30、下行结肠31、脾弯曲32、横行结肠33、肝弯曲34、上行结肠35、盲肠36构成。
图7A示出了将内窥镜1的插入部2插入S状结肠30内的状态。一般地,S状结肠30即使在大肠中也可最复杂地曲折,而且柔软并具有可动性。在插入部2以一定程度地沿该S状的结肠30的形状的状态移动并插入的情况下,希望软性部10的前端尽可能地柔软,并且较小地弯曲。因此,在对于S状结肠30等复杂地曲折的部位进行插入时,通过如本实施方式所示的内窥镜1那样在软性部10的前端形成细直径部10a,即使在象S状结肠30那样复杂地曲折的部位中,也能将插入部2流畅地导入。
并且,如图7B所示,在插入部2的前端部8通过下行结肠31并进入脾弯曲32内时,一旦将插入部2拉动进行操作时,柔软并具有可动性的S状的结肠30在被折叠并缩短的同时基本上变直。
这样,在S状的结肠30缩短并变直的状态下,将插入部2的前端部8进入到比脾弯曲更深。当插入部2的前端部8向深处进入时,由于插入部2整体向前进方向移动软性部10被弯曲,因此被缩短并变直的S状结肠30要复原为原来的曲折形状。
由于该S状结肠的复原力,手术者的靠近手一侧的操作难以传递到前端部侧8,插入部2的插入性能降低。因此,在图7B所示的状态下,软性部10进入大肠内的部位比较粗且硬的情况下,由于不容易弯曲而容易操作。从而,如果连接设置于软性部10的前端侧上的细直径部10a与设置于靠近手一侧的粗直径部10b之间的锥部10c的至少一部进入大肠内,则由于由此开始的软性部10的外径逐渐加粗,所以手术者例如在肛门28附近进行的靠近手一侧的操作容易向插入部2的前端部8侧传递,操作性提高。
通过文献“一人操作法にょるト一タルコロノスコピ一—左右結腸曲通過にっぃて—”(通过一人操作法的完整大肠内镜检查一关于通过左右结肠弯-”(消化器内视镜Vol.5 No.51 993 629~633页)等可知,在此情况下,如图7所示,内窥镜前端部8进入脾弯曲32,S状结肠30缩短并变直时,从肛门28到内窥镜前端部8的长度约为40~45cm。从而,如果锥部10c的至少一部分比从内窥镜前端部8的前端面开始45cm的位置更靠前,则能够将在肛门28附近进行的靠近手一侧的操作传递到插入部2的前端部8侧。
并且,由上述的文献等可知,如图7c所示,当内窥镜前端部8到达盲肠36时,不仅S状结肠30,横行结肠33也缩短并变直。因此,在插入部2以最短距离从肛门28插入到盲肠36时,肛门28中的软性部10从内窥镜前端部8开始约为60~70cm。
从而,在通过使软性部10的靠近手一侧加粗而不易弯曲的同时,能够由较小的力扭转必要的量,但如果为了使手术者这样的扭转操作容易地向内窥镜前端部8侧传递,锥部10c的至少一部分必须在比内窥镜前端部8开始70cm处更靠前方,否则由于患者的因素,锥部10c及粗直径部10b几乎不进入大肠内,不能得到所期待的效果。
因此,通过将锥部10c的至少一部分设定在比内窥镜前端部8的前端面开始70cm的位置靠前方,在大部分患者中,当使内窥镜前端部8进入大肠内的深处时,锥部10c及粗直径部10b能够进入缩短了的S状结肠30内,软性部10更加难以弯曲,能够阻止S状结肠返回原形状的复原动作。并且,由于手术者的手的把握位置为软性部10的锥部10c及粗直径部10b,因此容易进行扭转操作,容易将该扭转操作向内窥镜前端部8传递。
这样,通过将连接设置软性部10上所设的前端侧的细直径部10a与靠近手一侧的粗直径部10b的锥部10c的至少一部分设置在比内窥镜前端部8的前端面开始45cm或70cm更靠前等,可最适宜地确定锥部10c的形成位置,能够提高内窥镜插入时的操作性。
此外,图7D中示出了S状结肠30的一部分锐角弯曲的状态。如图7D所示,在锐角弯曲到肠壁相互接触(或粘连)程度的情况下,其弯曲部的弯曲角最大达到约180°。
为了使这样的最锐角的弯曲的部位通过内窥镜前端部8及与其连接的弯曲部9,最后弯曲部9能够180°左右弯曲操作,并且连接设置在该弯曲部9上的作为软性部10前端侧的细直径部10a也能够承受外力而自然地弯曲至180°为止。
在本实施方式中,由于细直径部10a具有以最小半径弯曲时,弯曲到180°所需的充分长度,因此也能够比较容易地通过这种最锐角性的大肠弯曲部。
在此情况下,由于锥部10c及粗直径部10b比细直径部10a硬,因此如果细直径部10a过短,则软性部10不能被弯曲到180°,难以通过大肠内的锐角性的弯曲部。
因此,通过适当地确定细直径部10a、锥部10c、粗直径部10b的轴方向的长度关系,最锐角性的弯曲部也能够比较容易地通过,提高了内窥镜插入部2的插入性。
第2实施方式
图8示出了本发明的第2实施方式。如该图所示,本实施方式中所示的内窥镜装置41具有具备不同功能的多个内窥镜1A、1B、1C;可相对于各内窥镜1A、1B、1C共用地连接的光源装置42;视频处理器43及监视器44。各内窥镜1A、1B、1C通过将从它们的操作部分别延伸出的万向软线4的前端上设置的连接部5选择性地连接在光源装置42及视频处理器43上来使用。此外,图中示出了3种内窥镜1A、1B、1C,但也可具有4种以上的内窥镜。
第1内窥镜1A具有与前一实施方式中所述的内窥镜1相同的功能,在软性部10上形成细直径部10a、粗直径部10b及连接设置细直径部10a和粗直径部10b的锥部10c。并且,在第2、第3内窥镜1B、1C的软性部45、46全长上具有大致相同的外径。第2内窥镜1B的软性部45具有比较粗的外径,另一方面,第3内窥镜1C的软性部46具有比较细的外径。
在此情况下,设置于第1内窥镜1A的软性部10上的至少粗直径部10b的外径被设定为与第2内窥镜1B的软性部45的外径大致相同(±5%以内)或在其以下,或者相对于第3内窥镜1C的软性部46的外径大致相同(±5%以内)或在其以上,即,粗直径部10b的外径被设定为在第2内窥镜1B的软性部45的外径和第3内窥镜1C的软性部46的处径的范围内。
此外,在此情况下也可将设置在第1内窥镜1A的软性部10上的细直径部10a的外径设定为,与第3内窥镜1C的软性部46的外径大致相同,并且将粗直径部10b的外径设定为与第2内窥镜1B的软性部45的外径大致相同。
以下,对于这种结构的本实施方式的作用进行说明。
关于将第1内窥镜1A的插入部2向大肠插入的操作,由于与第1实施方式相同,在此省略对其说明。
为了将第1内窥镜1A的插入部2顺畅地向大肠插入,不仅依靠插入部2自身的结构,减少操作它的手术者的疲劳也是重要的。
一般地,在操作大肠内窥镜(例如第1内窥镜1A)的情况下,一面用右手握住插入部2,用左手把持操作部3,一面通过其协调操作进行插入、观察、处置。右手进行插入部2的推拉和扭转,但是一般地,在软性部10为粗外径的情况下,特别是在扭转操作中以较小的力来进行必需的扭转操作,因此疲劳较小。
另一方面,各手术者有平时习惯了的软性部10的外径。尽管软性部10的外径粗的情况下疲劳感较小,但是在此前还没有操作过就来操作过于粗的软性部10的情况下,相反不适感会加强,进而造成疲劳。
在本实施方式中,由于第1内窥镜1A的粗直径部10b的外径被设定在其他第2、第3内窥镜1B、1C的软性部45、46的最粗外径和最细外径的范围内,因此手术者在操作第1内窥镜1A时难以感到不适感。在此情况下,最好,通过将第1内窥镜1A的粗直径部10b的外径设定为与第2内窥镜1B的软性部45的外径大致相同,能够进行更加没有不适感的操作。
再者,在设置于第1内窥镜1A的软性部10上的锥部10c及粗直径部10b具有插入部全长(有效长)的一半以上的情况下,由于手术者握住的部分大多是锥部10c及粗直径部10b,因此假如,即使在细直径部10a的外径比第3内窥镜1C的软性部46的外径细的情况下,通过将至少锥部10c及粗直径部10b的外径设定在第2内窥镜1B的软性部45的外径与第3内窥镜1C的软性部46的外径的范围内,能够减轻对手术者不适感。
当然,由于在插入的初期及拔去的最后时也要握住细直径部10a,因此最好将细直径部10a的外径也设定在第2内窥镜1B的软性部45和上行结肠35(参照图7)的范围内,此外,如果将该细直径部10a的外径设定为与第3内窥镜1C的软性部46的外径大致相同,则能够进行没有不适感的操作。
这样,根据本实施方式,在前述的第1实施方式的效果之外,还能够大幅度地减轻手术者在操作第1内窥镜1A的插入部2时的不适感。
第3实施方式
图9示出了本发明的第3实施方式。由于插入部的形状与第1实施方式中说明了的插入部2的结构相同,在此省略对其说明。
在本实施方式中,在插入部2的前端部8及弯曲部9之间和弯曲部9及设置于软性部10的前端侧的细直径部10a之间的至少一方中,设置有连接各部位的连接外皮48、49。此外,在图中,示出了设置两连接部外皮48、49的状态。另外,连接部外皮48、49可适用硬质的管状部及粘接剂及软性的热收缩管等各种材料。
本实施方式的内窥镜的设置于软性部10的靠近手一侧的粗直径部10b的外径被设定为与连接部外皮48、49外径大致相同(±5%以内)或比其略小的尺寸。例如,在连接部外皮48、49的外径为12.8mm的情况下,将粗直径部10b的外径设定为12.8mm,并将细直径部10a的外径设定为11.5mm。
以往,一般来说内窥镜以连接部外皮48、49的外径被设定为比软性部10的外径粗的情况为多。因此,在将内窥镜插入大肠中的过程中,首先,对于由前端部8观察的管腔,前端部8及弯曲部9能够通过的大肠部位不会造成其后接着的软性部10难以通过。
但是,在如果软性部10的外径明显比粗直径部10b及连接部外皮48、49的外径粗的情况下,大肠的某些部位即使前端部8及弯曲部9能够通过,在大肠的内壁与前端部8及弯曲部9之间的间隙极窄的情况下,也有其后接着的粗直径部10b不能通过,或难以通过的可能性。
例如在上述的图7A中S状结肠30的一部分中具有狭窄的部分,尽管其能够使前端部8及弯曲部9通过,但难以使粗直径部10b通过的情况下,在图7B的状态下,插入部2不能顺畅地前进。此时,会有手术者不能确切地判断插入部2难以前进的原因,不能实施适当的对策的情况。
这一点在本实施方式中,由于粗直径部10b的外径与连接部外皮48及49大致相同或在其以下,因此连接部外皮48及49能够通过的部位,粗直径部10b也能够通过,相应地,在插入部2难以顺畅地前进的情况下,由于预先知道其原因不是粗直径部10b太粗,所以只要用与以往的内窥镜插入部2同样的原因推测进行处理即可。
由此,根据本实施方式,由于在插入部2的前端部8及弯曲部9之间和弯曲部9及设置于软性部10的前端侧的细直径部10a之间的至少一方中,设置连接各部位的连接外皮48、49,并且将设置在软性部10的靠近手一侧的粗直径部10b的外径,设定为相对于该连接部外皮48、49的外径大致相同或略小,因此在插入插入部2时不会由于粗直径部10b粗的原因妨碍插入,能够得到良好的插入性。
以上,对本发明的实施方式进行了说明,但不用说,并不限定于上述的实施方式,在不超出本发明精神的范围内可进行各种变化。
产业上的可利用性
根据上述的本发明,在手术者把持设置于一个内窥镜装置中使用的多个内窥镜的软性部上的粗直径部时不会感到极端的不适感,能够得到良好的插入性。
Claims (3)
1、一种内窥镜装置,具有:包括软性部的第1内窥镜,该软性部具有设置于前端侧的细直径部、与该细直径部相比设置于靠近手一侧并且外径大于该细直径部的粗直径部;能够使用与所述第1内窥镜相同的光源装置或视频处理器,软性部全长具有大致相同的外径的第2内窥镜;以及软性部全长为大致相同的外径且该软性部的外径与上述第2内窥镜的外径大致相同或在其以下的第3内窥镜,其特征为:
所述第1内窥镜的至少粗直径部的外径与所述第2内窥镜的软性部的外径大致相同或在其以下,并且与所述第3内窥镜的软性部的外径大致相同或在其以上。
2、按照权利要求1所述的内窥镜装置,其特征为:
所述第1内窥镜的所述细直径部的外径与所述第3内窥镜的软性部的外径大致相同或在其以上。
3、按照权利要求1或2所述的内窥镜装置,其特征为:
所述第1内窥镜的细直径部的外径与所述第3内窥镜的所述软性部的外径大致相同,所述第1内窥镜的所述粗直径部的外径与所述第2内窥镜的软性部的外径大致相同。
