本发明还提供另一用于医学方法的植入物,它包括许多穿过其厚度形成的孔的网状结构,该网状结构至少具有第一连接区和第二连接区;以及第一和第二生物相容的固定元件,它们各自连接到第一和第二连接区。生物相容的固定元件具有基本上平面的矩形的构造,并且其刚性大于网状结构的刚性。

具体实施方式
虽然详细描述了关于本发明作为下尿道带子治疗应激性尿失禁的用途,然而应当理解的是本发明不受此限制,因为这样的植入物将会有很多其它的合适应用。例如结合此处描述的新特点的植入物可以被用来修复骨盆底缺陷,比如但不局限于膀胱突出症和直肠膨出,以及脱肠修复或其它的脱垂,或用于支撑或者相反修复其它类型的组织。
先翻到附图的图1-3,特别适合于治疗应激性尿失禁的下尿道带子的形式的植入物2的一个实施方案包括可植入的、伸长的带子4。主要的带子部分4具有穿过其厚度形成的许多孔,并且包括第一末端6和第二末端8,第二末端8与第一末端6纵向相对。
优选,以网或结网的形式形成带子4,网或结网带有穿过其厚度形成的约1毫米量级的许多孔以允许成纤维细胞长到带子里去以确保带子处于患者的周围组织中。带子的合适材料是PROLENE,它是厚度大约为0.7毫米的编织的或机织的聚丙烯网,并且它是Ethicon公司(Somerville,新泽西州)制造的。这种材料在美国被FDA批准可以植入人体。
上述的PROLENE带子是不可吸收的网格。然而,可以想象在本发明的范围之内可以由随着时间的推移可生物吸收的编织的或机织的材料或结网所形成的带子4,或者可能在孔径大小、纤维粗度、构造、尺寸和/或性能方面变化。
伸长的带子4可以被罩在含有成纤维细胞刺激性物质例如珐琅基底衍生物的一个或多个面上。
在图5和6中显示的另一个实施方案中,大量的间隔开来的合成纤丝缠结10可以被连接到带子4的顶面12和/或反面的底面14,如此将从用于刺激伤疤组织生长的顶面和底面的表面和连接点向外延伸到带子4。
看得见的网状带子4按照所描述的其应用选择合适的尺寸。例如,下文详细描述的一个特别适合于治疗女性应激性尿失禁的实施方案具有大约10-12毫米的宽度和约15毫米-约150毫米,优选40毫米的长度L1。包括固定元件16的植入物的总长度L2可以为从约35毫米直到约250毫米,并且优选为约80毫米。正如下文将更详细解释的,植入物2的长度可以变化,这是由于固定元件16可以被纵切或横切,这取决于插入方法和在外科植入带子期间医师的需要。而且,根据期望的用途和/或植入物的放置该长度可以变化。
再回到图1-3,植入物还可包括生物相容的,优选可生物吸收的聚合物固定元件16,该元件被连接到所述可见的主要带子部分4的第一和第二末端区6、8的每一个上。在一个实施方案中,每个可生物吸收的聚合物固定元件16具有组织粘附性能,用来将植入物活体固定在其中植入了所述植入物的患者的周围组织而不需要任何其他的用于物理连接的手段比如锚定机构等。“组织粘附性能”的含义是植入物具有相对快速和可定位的安置(即使是通过临时的手段)进入强健的组织而不需要快速地组织向内生长的能力。在另一实施方案中,固定元件仅仅具有大于主要带子部分的刚性,它们能够使固定元件被牢固地安置在插入装置上,并且当被植入到合适的强健组织比如结缔组织中时可以被坚固地保持在组织内。也可以使用其它的加固手段。
根据一个优选实施方案,将由Codman,一家Johnson&Johnson公司售出的商品名为ethisorbDura Patch,其主要是由VICRYL(polyglactin)合成外科复合材料(用于外科手术的组织增强)的材料或产品的元件组装而成可生物吸收的固定元件16。VICRYL是Ethicon,of Somerville公司,NJ制造的材料。ETHISORBDura贴包括由VICRYL(polyglactin 910)和PDS(聚-对二氧杂环己酮)未染色的细纱制成的羊毛,它是三明治结构,在一面带有一块染色的聚-p二氧杂环己酮薄膜。薄膜和羊毛在加热过程中粘合在一起,留下完整的薄膜作为片材。将该薄膜用D&C紫色2(颜色指数号60725)染成紫色。根据一个实施方案,PROLENE带子的末端可被夹在两片ETHISORBDura贴之间,其中组分通过加热被粘合在一起。然而根据一个优选实施方案,ETHISORBDura VICRYL和PDS组分被用来制造羊毛部分,以及同样的染色的聚对-二氧杂环己酮薄膜。将分离的部件、羊毛垫和染色的聚对-二氧杂环己酮薄膜薄片放在PROLENE带子的一个面上,将第二个羊毛垫和染色的聚对二氧杂环己酮薄膜薄片放在另一个面上。然后对5片组件进行热处理以使各组分粘合在一起。控制加热过程以保持在这样的温度,使得它只含熔融PDS细纱和染色的聚对-二氧杂环己酮薄膜。使用分离的部件提供非加压的羊毛,这样有利于两薄膜薄片随后通过网粘合,这是由于在加压过程中两个蓬松的羊毛层会整合成PROLENE网状编织物。在热压制过程之后,染色的聚对-二氧杂环己酮薄膜薄片不再存在,因为它们被熔融形成网格和羊毛层之间的许许多多粘结点。上述的羊毛组分由可吸收的材料制成,但是可以想象到可以由不可吸收的材料或它们的组合制成。制备羊毛的方法以类似于其他纺织品的方式开始,其中按优选的比率拿取相对直的细纱,在优选的实施方案中VICRYL与PDS的比率大约是8∶1。使用通常的纺织品技术将单纱股纺在一起,并且将新的股织成短袜状的结构,即直径为大约1-2英寸的连续长度。机织短袜或管状结构使得铅线可以牵引而解开该管,这样扭结成其他相对直的8∶1的股。
将短袜或管供料到织布机,机织成大约8英寸宽的一个疏松的围巾状材料薄片。因为使用了扭结的8∶1的股,所得到的围巾结构是蓬松的。围巾然后被切成大约12-18英寸的长度。然后将切好的长度放置在非加热的板上,然后将加热板落在非加热板上来捕获处于确定间隙间的围巾。当围巾的温度增加到一个低于PDS细纱的熔点的预先设定的温度时,产生较小的熔点,它保持新的蓬松羊毛结构的形状。因为单个的PDS细纱股收缩和轻微地熔融,它们牵引着VICRYL细纱串股与其在一起。而且,当最初的围巾收缩时,它同时变厚和变小,因为围巾的孔编织闭合了。所得到的新材料是上面引用的例证的“羊毛状材料”。
尽管上面描述了一个优选实施方案,应当认识到对它的改进也落在本发明的范围内。例如,你可以改变构成材料的可吸收的聚合物的组成比率。也就是说,使用较多的聚(L(-)-丙交酯-共-乙交酯)组分和较少的聚(对二氧杂环己酮)组分,或相反。还应当认识到在丙交酯/乙交酯共聚物的情形中,可以改变共聚单体的相对数量。因此可以稍微升高共聚物中丙交酯的水平来减少结晶性和升高被吸收组分的比率。还应当认识到羊毛的纤维性的组分的几何学属性可被改变以提供对组织表面增强的握持。可以提供更多的纤维性组分的非圆形横截面,比如扁平的横截面。星形的横截面也可能被用来增强握持。而且,在实施本发明时可以有利地使用其它可吸收的聚合物。特别有用的是从选自下述内酯单体制备得到的可吸收的或可生物吸收的聚合物:对二氧杂环己酮、乙交酯、L(-)-丙交酯、D(+)-丙交酯、内消旋-丙交酯、ε-己内酯和碳酸三亚甲基酯或它们的组合。在那些其中组织修复受到损害的外科手术情形中,比如在糖尿病或年长的患者中,可以相应地调节固定元件的吸收轮廓。因此,可能有利的是使用高-丙交酯(共)聚合物,比如95/5的聚(L(-)-丙交酯-共-乙交酯)。
除了从上述的内酯单体制备的可吸收的聚合物之外,可以使用氧化的再生的纤维素,又名ORC。已知这种材料,特别是在处于未中和的状态时,能连接到身体组织。氧化的程度和酸度可以被调节以增强粘附度并提供抗细菌活性。
在本发明的另一方面中,可以将可吸收的聚合物与杀菌剂相结合以提供额外的益处。应当认识到可以加入其它的活性成分来提供增强的特征,比如疼痛减轻剂等等。可以在形成最终的材料之前将这些试剂加入到聚合物中、一旦最终材料被制备好就将试剂加入最终的材料中或者一旦固定元件已经被制备好就将试剂加入到固定元件中。
假定会出现某些外科情形:其中优选不可吸收的永久性固定元件,这种假定不是不合理的。在这种情况下固定元件可从许多材料制造得到,这些材料包括但不局限于不可吸收的聚合物、金属或陶瓷。不可吸收的聚合物包括聚烯烃,比如聚乙烯或聚丙烯;聚氨酯;聚酯,比如聚对苯二甲酸乙二醇酯或聚对苯二甲酸丁二醇酯;和聚酰胺,又名尼龙,比如尼龙-6,尼龙-66或尼龙-12。
如图3的实施方案所示和如上所述,每个固定元件16可以包括第一顶面部分18和第二底面部分20。第一部分18连接到带子的顶面并且第二部分20连接到带子的底面。带子可以只沿着固定元件长度的一部分延伸,或者在另一种可以选择的实施方案中可以全部地沿着长度延伸或仅仅固定到带子的末端。固定元件部分18、20可以根据其他的已知方法另外连接到带子4,比如通过使用生物相容的粘合剂等等。
图1-3的实施方案的聚合物固定元件16包括中心体部分22和大量的纵向地间隔开来的手指部分24,这些部分沿着与中心体部分22相反的方向横向地向外延伸。优选,每个手指部分24一般地呈三角形,聚合物固定元件16呈现一般的箭形,在一个末端带有宽的底面26并且在另一末端带有三角形的或箭头状的尖端部分28。大量的三角形的手指部分24在底面26和三角形的尖端部分28之间横向地向外延伸。
三角形形状的手指部分24可以与相邻的手指部分纵向地间隔开大约3-5毫米,并且位于中心部分的每一面上的手指可以移动以进一步增强可调整性。在最大横向程度地横向配置的手指部分24之间测得的每个聚合物固定元件16的宽度优选是大约11毫米,这是带子4和植入物2总的优选宽度。在一个可供选择的实施方案中,手指可被很紧密地间隔开,间距的量级为0.02-1.0毫米,并且可以是除三角形之外的任何构型,以便不类似于可见的手指。植入物可以进一步包括″手指″之间的指距中断。例如,在一个优选实施方案中,“手指”被纵向地间隔开大约0.05mm,每隔一个除去一个。作为一种选择,对于沿着表面在纵向上没有确定间距的不规则的切削面可以使用它的一个面或两个面。
手指部分24可以是尖锐的,但是优选在其最横向地向外延伸的部件30处被成形为具有约0.1-2毫米的半径,并且在它们最向内地延伸的部件30处被成形为具有约0.1-2毫米的半径,即在其相邻的横向向外延伸的部分30之间的区域并且每个固定元件16的三角形的顶点28之间的区域具有约11毫米的横向宽度和可以在3-10毫米之间变化的纵向长度。此外,如果医师需要缩短植入物2的总长度,固定元件16可以被剪短或修剪它们的横向宽度。
具有这种特别的形状,聚合物固定元件16可被设想成粘附于和/或啮合于周围组织,将在通过外科手术将植入物2植入患者之后的向后滑移或向前滑动最小化。优选每个固定元件16具有的刚性大于网状带子部分4的刚性以便提供一些硬度并保持其整体形状。这样阻止或最小化了相对于它们所粘附的周围组织的滑移,并且在外科植入期间保持固定元件的整体完整性。通过选择生产方法可以改变全部固定区域的刚性。
虽然图1-3实施方案掺入了上述的手指部分,但是固定区域还可能仅仅只具有如图所示的直角构造10,只要固定区域由具有合适的刚性和/或组织粘合剂性能以确保带子就位的的材料,比如上述的材料构成。在图10的实施方案中,固定区域16的下边缘17具有一个横截面积(沿着线a-a),其比它连接的网格的布告只稍微大一点。在植入期间典型地由于在网上的张力导致网采取如图10a所示的稍微缩减的宽度,横截面积的这个差别还可能增大。
如图9所示,特别地,每个固定元件16可以更进一步包括通过它们的正当中时形成的一个或多个孔穴34、孔、裂缝等,并且它们相互之间被纵向地间隔开来。这些孔穴34可以与插入工具36的一种实施方案协作用于将植入物的自由端适当地放置在患者的骨盆或闭孔组织中。这样的插入工具36可以包括伸长的棒条38,它可以被医师弯曲至期望的弯度,并在外科手术期间保持其设定的形状。棒条38包括最近的末端40用于被医师握紧以及与最近的末端40相对的远端42用于通过在患者顶面阴道壁中所做的切口插入。远端部分42包括一个或多个从棒条38径向地向外延伸并互相间隔开来的凸刺44。每个凸刺44的直径等于或稍微小于在固定元件16中形成的孔穴34的直径,并且相邻的凸刺44相互分开一段距离,该距离等于在固定元件中形成的相邻的孔穴34之间的间距。相应地,插入工具36的凸刺44可被在固定元件16中形成的孔穴34接收,以致植入物2的每个自由端可以被选择地用来确保插入工具的远端42被合适地放置在患者的骨盆组织内。一旦被适当地定位,医师可操纵最近的末端将植入物2和插入工具36分开,来操纵插入工具的远端42以远离植入物2,使得凸刺44从它们在固定元件中的各自的孔穴34中释放。各种其它的插入方法对于本领域技术人员是显而易见的,比如被用来定位固定区域镊子的任何改变,或者具有合适手段的其它插入工具,通过这些手段来握紧和/或定位固定区域的植入物。下文将详细描述插入装置的另一个优选实施方案。
具有通过固定元件16的厚度形成的这样的孔穴34的第二个理由是它们便利了通过固定元件的组织的内生长以便它们更进一步粘附于周围组织。
如附图的图4中所示,根据一个实施方案,在植入物2,特别是带子4的顶面和底面上覆盖可拆卸的塑料套46。每个套46可以包括切口或穿孔48,切口或穿孔48至少部分地跨过覆盖在植入物2的中间点上的中间点处的宽度延伸以方便从带子4的顶面和底面12、14移去它。它可以更进一步包括一条横向跨过套46的中部运行的线或其它的标志50,这样的线或其它的标志50位于植入物2的中间点,以便在外科植入过程期间指示医师带子的中点的位置在哪里。在植入过程期间可以使用镊子从带子4的顶面和底面12、14除去套46。套46可以防止带子4在插入患者身体和定位植入物2期间抓住周围组织。优选每个套46延伸稍微超出带子4的宽度以保证带子4的顶面和底面12、14和侧边在外科手术期间不抓住周围组织。套还允许防止污染或损伤。
图4A-4C举例说明了保护套的其它形式。更具体地说,图4A-4C举例说明了作为包裹物形成的植入物的保护套,其可以被折叠裹在植入物外面的在植入期间至少部分地包封带子。
先翻到图4A和4B,保护套46′包括背片60,优选由尾对尾放置的两个片段61形成。相对于带子,这两个片段61优选在带子的中央相会,并且从所述背片的外表面垂直地向外延伸来一起限定接头62。医师可以使用镊子握紧形成接头62的背片60的每个片段61来在植入物2已经被适当地定位和植入患者体内以后从植入物2中除去保护套46′。
保护套46′更进一步包括可折叠的侧片64,该侧片连接到背片并从背片60的背面至少部分沿着背片的纵向长度延伸。如图4B所见,植入物2和套46′被组装在一起,使得植入物的底面停靠在背片62的内表面上,并且套的侧片64被折叠罩在植入物的顶面上。可以调节套46′的侧片64的尺寸使得它们沿着植入物的纵向中心线在其自由边缘处相会,或调节尺寸使得一个侧片延伸与另一个侧片部分重叠。
如图4A和4B所示,优选保护套46可以更进一步包括轴向的端片66,其连接到并从背片60的相反的轴向面延伸。优选将每个轴末端片66的尺寸裁减到其宽度和长度符合每个固定元件16的中心体部分22的宽度和长度,使得它们只覆盖中心体部分22的一个面,留下中心体部分22的另一面以及手指部分24的两面都暴露着。这样,在植入期间和利用附属于植入物2的保护套46,固定元件16的暴露部分将接触并能粘附到患者的周围组织。
图4C图解了用于本发明的植入物的另一种形式的保护套46″。这个套46″与图4a和4b中所示的相同,除了它是以两个纵向排列的、单个地可折叠的二等分形式形成的,即背片60和对面的侧片64每个是由两个可分离的片段68以尾对尾的方式相互纵向地放置而形成的。套46″的每个半部分片段68可以用先前描述的相同方法单独除去,即医师用镊子握紧形成接头62的套背片,使得如果需要的话,在植入物被部分植入期间套46″的二分之一片段68可被除去,或者在植入物2的两端已经被适当地定位并植入患者体内之后,可以除去两个二分之一片段68。
另外,带子4可在其顶面12形成一个标志,比如第一种颜色,并在其底面14形成另一个标志,比如不同于第一标志或颜色的第二种颜色。这将使得医师在外科植入作业期间知道带子4是否扭曲。换句话说,植入物2的顶面和底面12、14中的一个应该面向尿道,当它部分地环绕尿道成环,顶面和底面12、14中的另一个具有医师看得见的不同颜色或其它的标志,这一部分在对患者外科植入期间应该背向尿道。当然,可以设想套46、46′、46″而不是植入物2可以具有类似的颜色或包括可区别的标志。
除了提及的在固定末端上标记之外,可以标记网或套以描述网环的中心或者可以标志区域来帮助医师在植入期间相对对尿道对称地定位网环。
图7和8图解了用于在患者中外科植入植入物2以治疗应激性尿失禁(SUI)的方法。首先,在顶面阴道壁54中做一个小切口52。医师通过将凸刺44插入相应的孔穴34而将插入工具36的远端42连接在植入物2的固定元件16上,孔穴34是通过固定元件形成的。在植入手术期间医师可以弯曲插入工具至任何他感到有必要的曲度。
通过操纵插入工具36的最近的末端40,医师指导工具的远端42带着与其相连的植入物2的一端一起通过外科切口52并插到尿道56的外侧和耻骨58后面的软组织上。套46、46′、46″上的横向标志50将指示医师植入物2的中点或中间区域,这样医师可以判断组织贯穿的程度以及判断是否需要更进一步的插入。由于其固有的对周围组织的粘附倾向性,固定元件16将植入物2的自由端固定到它所接触的周围组织。医师现在操纵插入工具36使得它本身释放固定元件16并被除去,让植入物2的第一个一半保持植入患者的状态。
对于所描述的插入概念,在插入第一个固定元件以后,通过比较植入物的中央标志或中间区域和尿道的位置,医师可以认识到植入物太长。如有必要,所述的植入物的第二固定元件16可以被剪短以弥偿长度上的差别。
一旦修剪了长度,医师然后将插入工具的远端42连接在植入物2另一端的固定元件16上,再次引导工具通过阴道壁切口52,从而穿过位于尿道56的另一侧面上和耻骨58后面的软组织。医师然后在植入物2的第二末端将插入工具36与固定元件16分离,并除去工具,让带子就位并优选围绕尿道56的中央部分成环。第二固定区域16粘附于它接触的周围组织,并保持植入物2的第二个一半就位。在这个手术期间,医师可核查植入物2的顶面和底面或套46、46′、46″上的颜色或其它的标记以确保带子没有扭曲。如有必要,医师可以修剪一个或两个固定元件16来调整植入物的长度。
医师现在使用镊子分开并除去塑料套46、46′、46″,它们覆盖着带子4的顶面和底面12、14。让暴露的带子保持下植入患者,使得随着时间的过去,成纤维细胞将繁殖并长到带子中去,用来将带子固定在周围组织中。固定元件16完成了它们用于暂时固定植入物的目的。在吸收了ETHISORB以后,全部的网孔长度(包括固定元件之间的部件)将生长到周围组织中,留下植入的带子来为尿道56提供支撑。
再一次参考图10、10a和14的实施方案,特别适合于与本公开的植入物配合使用的插入器的另一个实施方案更进一步显示在图12-14中。插入器100优选包括带有曲面等的第一个插入器装置102(参见图13),它特别适于基本上遵循期望的插入路径,这将在下面进行描述。在举例说明的实施方案中,插入器包括远端区103,远端区103包括穿透组织的远端尖端104。穿透组织的远端尖端优选包括与钝边缘相对的刃口。第一个插入器装置102包括基本上平面的部分108,和包含基本上弯曲部分110的近端区106。基本上平面的部分优选具有大约1.2英寸的长度l1和大约0.32英寸的宽度。而且,弯曲的部分110优选具有大约1.5英寸的长度l2,大于大约50度。优选,基本上平面的部分与弯曲的部分110相切,并且第一个插入器装置的基本上直的近端部分113被放置在与弯曲部分110的切线成大约40度。
在举例说明的实施方案中,插入器还包含偶联在一起的加固元件116,并优选带有与第一个插入器装置的基本上平面的部分基本上互补的外围。加固元件给固定所述植入物的区域中的插入器提供额外的刚性,下文将更详细的描述。优选该加固元件与第一个插入器元件间隔开来,通过插入器装置或者加固元件上大量的小突起130等等间隔开大约0.006英寸的量级,以便允许杀菌气体在部件间更自由地流动从而增强气体灭菌过程的效率和效果。这个加固元件是带有外刃口105的薄的硬化材料,并且宽度W1稍微大于第一个插入器装置102的宽度,优选前者比后者宽0.015英寸的数量级。
在举例说明的实施方案中,加固元件还包含第一通过元件132,它优选是弹簧型的元件。第一通过元件带有前面部分115和后面部分117(参见图13和14),两者都优选基本上垂直地从加固元件向上延伸。如下面进一步描述的那样,当植入物被固定在插入器上时,第一通过元件至少部分地通过植入物中的小孔向上延伸,以便前面部分115防止植入物相对于插入器的向后滑移和后面部分117防止植入物相对于插入器的向前滑移。而且,当植入物被固定在插入器上时,第一通过元件的弹簧属性使它偏斜而回复到更能充满插入器的位置。因此,当抓取元件118回缩时(如下所述),植入物更容易被释放。最后,后部分117的长度优选使得在植入物被释放之后它适合于插入器的厚度或剖面,以便在放置好植入物后从身体除去插入器时不咬住或者以其它方式抓住组织。
插入器100还包含植入物保持元件118,它与朝向插入器装置的近端的在一个区域内的插入器装置可移动地偶联,如此它可按照图13中所显示的箭头所指示的方向相对于第一个插入器装置进行移动。从下文的更进一步说明中可以更清晰的明了这个目的。例如,在举例说明的实施方案中,植入物保持元件118由单线和双线部件构成,它们就位多个点。在本实施方案中,这些多个点包括抓取元件122、通过元件132、114和指垫140。优选地,指垫140包含在双丝191a、191b之间延伸的凸起190,并且起着阻止这两条线在此处192会合的部分的作用以限制植入物保持元件的回缩。
植入物保持元件的远端120可移动地与第一插入器装置偶联,优选在插入器的远端处被抓取元件122接收。然而,通过由用户操纵植入物保持装置,植入物保持元件的远端可以随后被从抓取元件中释放出来。特别地,植入物保持装置按照图13中显示的箭头方向的移动,将会使植入物支持装置移动到图12a中所显示的第二个位置,其中植入物保持元件不穿过通过元件延伸,并且不被接收在抓取元件内。在这个第二位置它也不延伸通过植入物,从而从插入器装置释放植入物,这将通过下文的更进一步说明很容易被理解。
在举例说明的实施方案中,抓取元件122从加固元件的顶面表面向上突起,并且带有一个小孔124,其中尺寸被裁减以接受植入物保持元件的远端。抓取元件122还起着第二个作用,当它被插入时,它为植入物提供缓冲。虽然这是一种优选的实施方案,但是本领域技术人员将会容易地认识到抓取元件的各种形式和构造是可能的,只要起到可移动地接受植入物保持元件的远端这一目的。
插入器还优选包含一个或多个通过元件(即132、114),该通过元件从其顶面表面向上突出,并且包括一个或多个穿过其的孔(即134、119)。孔可以具有任意的构造或形状,只要它们允许植入物保持元件能通过,如图1 4所最好显示的。这个特征至少部分地保持植入物保持元件相对于插入器的位置。虽然所举例说明的通过元件是与插入器整体形成的,但是可以使用适合于至少部分地保持抓取元件的相对于插入器的位置的任何通过元件,比如缝合环等等。
如所指示的,目前描述的插入器特别适合于被用来与结合图10描述的但是带有图14中所显示的构造的固定元件140的植入物结构相似的植入物类似结构结合使用,该固定元件140包括基本上平的侧边195a、195b或者如上所述的粗切侧边。在一个优选的实施方案中,植入物的最外面宽度W2相对于插入器的最外面宽度W1的比率大约是1.4∶1。这个比率是优选的以便当植入物如下所述被插入结缔组织时达到插入力和用户控制之间的平衡。更低的和更高的比率提供不同的结果,这取决于外科需要和/或被穿透的组织类型。例如,在0.7和1.4之间的较低比率可以逐渐地降低插入力和降低植入物在周围组织上的摩擦力。然而这些低的比率可能提供较少的用户控制。较高的比率,比如在1.4和2.8之间的比率可以逐渐地升高插入力,如果用于强韧的组织时,但是用于软组织时不会增加插入力。
优选地,通过如所示的植入物保持元件将每一固定元件140固定到插入器装置的远端区。在举例说明的实施方案中,植入物保持元件118沿着顶面表面119延伸,并且基本上与第一个插入器装置的上表面119相邻,直到它到达基本上平面的区域。在沿着基本上平面的区域的一些点,它开始延伸远离第一个插入器装置以便留下如图13所示的间距。虽然与第一插入器装置间隔开来,但是植入物保持元件穿过第一通过元件132中的小孔134,其远端120随后被接收在抓取元件122中。如图14中所示,为了将植入物固定到插入器,植入物保持元件在其远端被接收在抓取元件122中之前通过植入物向上延伸。优选地,植入物包含穿过其延伸的一个或多个小孔135,以方便植入物保持元件通过它延伸,并且也允许通过元件132穿过它向上延伸。
根据一个实施方案,插入器100更进一步包含位于装置的近端附近的握持元件或指垫140等等。将握持元件牢固地固定到第一个插入器装置102,并且握持元件具有任何适合于增强握持和操纵装置的构造。在举例说明的实施方案中,握持元件被构造成用来接受用户的食指。另外它具有锥形的远端以便当插入器100被推进到身体中时允许其容易进入。指垫140还可含有通常用于卫生器材的无线电不透明材料,以帮助测定是否有遗失的或未觉察组分被意外地留在患者体内。在插入之前,插入器还可包含一个或两个保护性盖子(没有显示)覆盖着加固元件的刃口以防止被刃口损伤或伤害。
当插入此处描述的植入物时,插入器提供了几个明显的优点,因为它允许以使整个系统的轮廓最小化的方式将植入物固定到插入装置。这在例如上面描述的那些应用中是重要的,因为理想的是将组织损伤最小化而不是必需直接插入植入物。当放置植入物时目视观察同样重要,因为在植入期间插入器妨碍外科医生的视野最小化非常关键。现在描述的插入器的另一个重要的方面是它提供一种手段用来在将植入物适当放置之后释放它而不需要进一步地操作植入物。而且,插入器100与所描述的植入物结合允许植入物在最后释放之前正反方向调节植入物,并在释放之前保证外科医生优越的触觉感受。这是重要的优点,原因在于使用插入器元件可以达到适当的吊挂放置和张力,并且一旦放置合适,植入物无需进一步移动即可被释放。这与目前存在的吊挂式植入物相反,其中网在最初就被植入,随后通过用力拉吊挂的末端或中心进行调节。最后,设计插入器元件以便在放置植入物期间使它不位于植入物和尿道之间(与插入装置相比,它可能将网夹芯在两个相对的组分之间),就适当的放置植入物和目视观察而言,它具有优点。
本发明的插入器的另一个有利特点是至少第一个插入器装置的弯曲部分110的横截面具有基本上“V”形的构造151,正如可以图15b中最好地看到的那样。相对于插入器的最外层宽度w1,这个“V”形构造确保弯曲部分的宽度w3减小,同时仍然保持足够的强度。现已发现在植入物放置期间,缩减的宽度减轻了对尿道的压力。较大的宽度可以对尿道施加压力,并引起假阳性,导致在除去插入器之后不能正确地放置网。在一个优选实施方案中,宽度W1对W3的比率是2∶1。
现在将参考图15a-c和16详细描述用于使用图12-14的插入装置的优选方法。优选将带有固定元件16的植入物与比如两个上述的插入器一起提供给用户,每个固定到如图15b中所示的植入物的一端。通过给膀胱导液、使用局部的、区域的或全身麻醉,随后将患者置于膀胱切石的位置使患者准备本手术。使用镊子或河鲱夹钳两者之一在尿道的每个侧边抓住阴道壁。然后使用解剖刀等做一个约1.5厘米长,从离外尿道导管大约1.0厘米处开始的矢状切口。切口将盖住中尿道区域,并允许随后植入物的通过。然后,使用一小对钝的剪子,做大约1.0厘米的两个小的尿道旁的剖开。
然后使用任何标准针状起子或夹持器150等握紧一个插入器的近端106,夹在保持元件118上。然后为了进行“U”形放置,将插入器元件的远端1 08定向在向着同侧肩的大约11点钟的位置。在指垫上放食指,将第一个152插入器和连接的植入物穿过阴道切口和如图15a所示的第一个预解剖的尿道旁剖开插入,让插入元件的远端仍然与耻骨的下位-后部方面紧密接触,以便植入物被插入尿生殖膈的结缔组织中。当插入装置牢固地处于结缔组织(离固定元件的远端大约4厘米处)中时,应当停止插入。然后从第一插入器解开针起子/夹持器,并用同样的方式与第二插入器154偶联。然后在尿道156的另一边重复该方法,留下如图15b所示的插入装置/植入物。带子4应该无张力地放置在中尿道下面。可以按需要通过进一步插入或回缩第一个152或第二个154插入器来进行调节。然后如图15c中所示通过牵引相应的植入物保持元件158、160来同时从第一个152和第二个154插入装置中释放植入物,而不是如同其它已知的元件那样牵引网。除去插入装置并封闭切口,从而完成手术。
根据另一个优选方法,如图16所示可以将植入物放置在更侧卧的位置,本领域技术人员所知的“吊床”位置。该手术与上述的基本上相同,除了将第一个插入器152定向在大约9点钟位置或以与中间线成大约45度的角度平行于地板进行定位,朝向坐骨耻骨的枝骨以外。将插入装置的远端插入,与骨骼紧密的接触,但是朝着闭孔内肌肌肉的方向,在那里它被牢固地固定。使用插入器100进行调节,而不是牵引网。
此处描述的植入物可以被有利地植入患者中无需使用穿过全身的针并且无需在腹壁、大规模的预解剖管道或者患者的大腿形成切口,并且不需要骨骼锚之类的东西。比起使用延长的弯针或者钩状元件等过去常被用来咬住和拖曳网穿过身体的常规操作,本作业不仅对患者在情绪上的创伤要小,而且对患者的侵入较少,因为不需要在患者的腹壁或者大腿上产生切口。因为本发明的实际植入物小于常规的植入物,这潜在地导致较少的外科并发症、较少的网排异反应和/或者较少的网感染以及缩短了的外科手术。
使用本发明的“U”形植入物放置,可以对一个门诊病人在局部的、区域的或者全身麻醉的情形下进行外科植入手术。并且,本植入作业需要使用膀胱镜,然而通道的风险较少,就象所有外科手术那样近距离通过一个结构总会有伤害的风险。在外科植入作业期间,患者的安全性进一步得到提高,如并发症的机会变少,这是由于不必引导“U”形植入物放置,如果做得适当的话,不会从膀胱或者肠或者任何大的导管附近经过,因为目的是在靠近耻骨的结缔组织中的阴阜综合体的下边缘处进行最小限度的元件侵入放置。
可在对一个门诊病人的局部的、区域的或全身麻醉下进行类似的“吊床”放置。在外科植入作业期间患者的安全性将比“U”形进一步提高,由于如果正确完成则对膀胱或者肠或者任何大的导管没有风险,因为装置路径不进入雷丘斯空间。另外,本发明还进一步提供安全的、目前已知的″吊床″型操作,它绕过或者靠近含有神经和动脉的闭孔管束。本发明的植入物优选被连接在内部肌肉中,并且不大足以从闭孔管束附近通过。
如先前所指示的,虽然针对下尿道带子对本发明进行了详细描述,但本发明不受此限定。可以掺入上述的固定元件作为用于外科手术中的任何类型网的固定元件,这些外科作业比如骨盆底修复或者脱肠修复或者整形外科或者重新构造组织。例如图11中所显示的网70(形状仅仅用于举例说明的目,因为可以使用任何形状的网)可以掺入一个或多个可以固定固定元件16的连接区72。就骨盆底修复而言,例如,可以对网的大小与形状加以确定来治疗膀胱突出症,按照与上述类似的方式使用固定元件在合适的位置固定网。对于被用来改变组织位置的随便什么整形外科,例如元件可以看上去与本发明很相似,并且插入装置的形状插入器与植入物的任何宽度比率、盖子的使用、连接方法和尺寸可以被改变,以适应被尝试的外科应用。
虽然在此处参考附图说明了本发明的实施方案,但是应当理解的是本发明不局限于那些精确的实施方案,而且本领域技术人员只要不背离本发明的精神进行各种改变和改进可以实施。本发明仅仅由附加的权利要求限定。
Claims (36)
1.用于治疗患者的应激性尿失禁的植入物,它包括
可植入的、伸长的带子,该带子具有很多穿过带子的厚度形成的孔,该带子具有第一末端区和与第一末端区纵向相对的第二末端区;以及第一和第二生物相容的固定元件,它们各自连接到带子的第一和第二末端区,每个生物相容的固定元件的组织粘附性能大于带子的组织粘附性能。
2.根据权利要求1的植入物,其中所述植入物的长度大约是5-10厘米。
3.根据权利要求1的植入物,其中所述的带子由聚丙烯网或结网构成。
4.根据权利要求1的植入物,其中所述植入物还包括基本上覆盖所述带子的套。
5.根据权利要求1的植入物,其中所述固定元件基本上围绕各自带子的第一个和第二末端区。
6.根据权利要求1的植入物,其中每个固定元件还包括中心体部分和大量的从中心体部分横向地向外延伸的间隔开来的手指部分。
7.根据权利要求1的植入物,其中所述带子包括顶面和底面,在所述顶面上具有第一标志并且在所述底面上具有第二标志,第二标志不同于第一标志,从而区分顶面和底面。
8.根据权利要求1的植入物,其中所述带子包括大量的连接在带子上的间隔开来的纤丝缠结。
9.根据权利要求1的植入物,其中每个固定元件由从第一聚合物和第二聚合物的复合材料制造的材料制成,该第一聚合物从选自丙交酯和乙交酯的单体制备得到,并且第二聚合物是富含聚(对二氧杂环己酮)的聚(对二氧杂环己酮)聚合物。
10.根据权利要求9的植入物,其中所述的第一聚合物是10/90的聚(L(-)-丙交酯-共-乙交酯),并且第二聚合物是聚(对二氧杂环己酮)。
11.根据权利要求9的植入物,其中第一聚合物是95/5的聚(L(-)-丙交酯-共-乙交酯),并且第二聚合物是聚(对二氧杂环己酮)。
12.根据权利要求1的植入物,其中所述的固定元件由ETHISORBDura Patch制成。13.根据权利要求1的植入物,其中所述的固定元件具有基本上矩形的平面构型。
13.根据权利要求1的植入物,其中所述的固定元件的最外面的宽度基本上等于带子的宽度。
14.用于治疗患者的应激性尿失禁的植入物,它包括

23.在患者中植入植入物用于治疗应激性尿失禁的方法,该方法包括下列步骤:
提供包括可植入的、伸长的带子的植入物,该带子具有很多穿过其厚度形成的孔,并且具有第一末端区和与第一末端区纵向相对的第二末端区;以及第一和第二生物相容的固定元件,它们各自连接到带子的第一和第二末端区,每个生物相容的固定元件的组织粘附性能大于带子的组织粘附性能;
在患者的阴道壁中做一个切口;
通过该切口插入所述的第一固定元件和连接的带子,并插到患者的位于尿道一侧上的骨盆或任何闭孔组织中并且不要离开身体;
通过该切口插入所述的第二固定元件和连接的带子,并插到患者的位于尿道的相反一侧上的骨盆或任何闭孔组织中并且不要离开身体,使得所述带子形成部分地围绕尿道的环以提供对尿道的支撑;和
让所述的植入物保持植入患者的身体的状态。
24.在患者中植入植入物用于治疗应激性尿失禁的方法,该方法包括下列步骤:
提供包括可植入的、伸长的带子的植入物,该带子具有很多穿过其厚度形成的孔,并且具有第一末端区和与第一末端区纵向相对的第二末端区;以及第一和第二生物相容的固定元件,它们各自连接到带子的第一和第二末端区,每个生物相容的固定元件的刚性和/或组织粘附性能大于带子的刚性和或组织粘附性能;
在患者的阴道壁中做一个切口;
通过该切口插入所述的第一固定元件和连接的带子,并插到和耻骨相连的结缔组织中,到达患者尿道的第一侧面上;
通过该切口插入所述的第二固定元件和连接的带子并插入到和耻骨相连的位于患者尿道的相反一侧的结缔组织中,使得所述带子形成部分地围绕尿道的环以提供对尿道的支撑;和
让所述的植入物保持植入患者的身体的状态。
25.根据权利要求24的方法,其中所述的第一个和第二个插入步骤还包含分别将所述的第一个和第二固定元件插入位于阴阜综合体的下边缘的结缔组织中。
26.用于医学方法的植入物,包括:
具有许多穿过其厚度形成的孔的网状结构,该网状结构至少具有第一连接区和第二连接区;以及第一和第二生物相容的固定元件,它们各自连接到第一和第二连接区,每个生物相容的的固定元件的组织粘附性能大于所述网的组织粘附性能;
27.根据权利要求26的植入物,其中每个固定元件由从第一聚合物和第二聚合物的复合材料制造的材料制成,该第一聚合物从选自丙交酯和乙交酯的单体制备得到,并且第二聚合物是富含聚(对二氧杂环己酮)的聚(对二氧杂环己酮)聚合物。
