Abstract

 本発明は、レチノイド誘導体および/またはビタミンAアナログを構成成分とする、星細胞特異的薬物担体、これを利用した薬物送達方法、これを含む医薬、およびこの医薬を利用した治療方法に関する。薬物担体にビタミンAなどのレチノイド誘導体またはビタミンAアナログを結合させるかまたは包含させることにより、治療用薬物を星細胞特異的に運搬することが可能になり、星細胞関連疾患を効率的かつ効果的に、最小限の副作用で抑制または予防できる。星細胞の活性または増殖を制御する薬物として、たとえばコラーゲン特異的分子シャペロンであるHSP47に対するsiRNAを該薬物担体に封入して用いることにより、I型~IV型コラーゲンの分泌を同時に抑制し、その結果線維形成を効果的に抑制することができる。

[0001] 本発明は、星細胞へのドラッグデリバリーシステム(DDS)に用いる薬物担体、これ を含む医薬および当該医薬の調製キット、特に、有効成分が星細胞の活性または増 殖を制御する薬物、とりわけ星細胞によって分泌される細胞外マトリクス構成分子を 標的とする、または細胞外マトリクス構成分子の産生もしくは分泌に機能する分子のう ち 1つまたはそれ以上を標的とする薬物である、医薬およびその調製キットに関する 背景技術
[0002] 肝臓の線維化は、限定するものではないが、たとえば B型もしくは C型肝炎ウィルス 等に原因するウィルス性肝疾患、非アルコール性脂肪性肝炎、栄養障害糖尿病、寄 生虫、結核もしくは梅毒などの感染症、心臓病などによる肝臓内のうつ血、または胆 汁の通過障害などに伴う肝臓内の組織傷害などに対する創傷治癒機転の結果、肝 星細胞(HSC)が活性化され、過剰に産生'分泌された複数種のコラーゲン分子およ らされる。肝線維化の終末像は肝硬変であり、肝不全や肝細胞癌などを生じることか ら、これらを未然に防ぐため、および/またはこれらの進行を抑制するため、少なくと も肝線維化を抑制する薬物担体および薬物担体キットの開発が望まれる。

[0004] 肝臓または膝臓の線維化を抑制する有効な手段として、星細胞が重要な標的候補 の 1つとなる可能性がある(Fallowfield JA, Iredale JP、 Expert Opin Ther Targets. 20 04 Oct;8(5):423-35;Pinzani M, Rombouts K. Dig Liver Dis. 2004 Apr;36(4):231-42. )。線維化過程にぉレ、て、星細胞はクッパー細胞や浸潤細胞からのサイト力インにより 活性化され活性化細胞へと形質転換し細胞外マトリクス (ECM)を著明に産生する。 星細胞は、ビタミン Aの貯蔵細胞として知られており、筋線維芽細胞ファミリーに属す る。一方、星細胞は、マトリックス分解酵素(MMP)、その抑制因子(TIMP)、 TGF - β、 PDGFなどのサイト力イン、および HGFなどの増殖因子を産生し、肝線維化に おける中心的な役割を果す。活性化された星細胞は、収縮能が亢進して血流の調 節に関与する他、各種サイト力イン受容体の発現をも増加し、サイト力インに対して高 感受性になる。
[0005] 線維化に対する治療方法として、現在までのところ試みられている方法としては、コ ラーゲン代謝の制御、コラーゲン分解系の促進、または星細胞の活性化の抑制など が挙げられる。これらには、 truncated TGF β type II receptor (Qi Zら, Proc Natl Aca d Sci U S A. 1999 Mar 2;96(5):2345_9.)もしくは、 soluble TGF β type II receptor (G eorge Jら、 Proc Natl Acad Sci U S A. 1999 Oct 26;96(22): 12719-24.)、または HGF (特表平 5-503076号公報; Ueki Kら、 Nat Med. 1999 Feb;5(2):226_30.)などを用いた (星細胞を活性化し細胞外マトリクス (ECM)産生を促進する因子として知られる) TG F j3の抑制、 HGFもしくは MMP遺伝子含有ベクターによるマトリックス分解酵素(M MP)の産生促進(Iimuro Yら、 Gastroenterology 2003; 124:445-458.)、 MMP阻害 因子である TIMPのアンチセンス RNAなどによる抑制(Liu WBら、 World J Gastroent erol. 2003 Feb;9(2):316- 9)、 PPAR yリガンド(Marra Fら、 Gastroenterology. 2000 A ug;119(2):466_78)もしくは angiotensin— II type I receptor antagonist (Yoshiji Hら、 He patology. 2001 Oct;34(4 Pt 1):745_50.)による星細胞活性化の制御、 PDGFチロシ ンキナーゼ阻害剤などによる PDGF作用の抑制(Liu XJら、 World J Gastroenterol. 2 002 Aug;8(4):739-45.)およびアミ口ライドによるナトリウムチャンネル阻害(Benedetti Aら、 Gastroenterology. 2001 Feb; 120(2):545- 56)などを介した星細胞の増殖抑制、 c ompound 861 (Wang L,ら、 World J Gastroenterol 2004 October l;10(19):2831-283 5)、 gliotoxin (〇rr JGら、 Hepatology. 2004 Jul;40(l):232- 42.)などによる星細胞のァ ポトーシス誘導などが含まれる。し力、しながら、いずれの場合も作用特異性および Z または臓器特異性が低いため、効果および副作用の点に問題がある。
[0006] コラーゲンのタンパク質合成については、代謝経路が不明な点が多ぐこれを抑制 する薬剤による治療方法については、副作用の点などから効率的かつ生体に安全な 治療方法としては確立されていない。すなわちコラーゲン産生に関わる分子を標的 にする方法においては、それらの分子が有する多様な機能のために標的の特異性 を高めることができず、副作用を生ずる可能性が高い。最終産物であるコラーゲンを 直接抑制することができれば、線維化過程に対する共通の治療方法として理にかな うことになる力 そのためには、タイプ I〜IVに代表される様々なタイプのコラーゲンを 一網打尽に制御することが必要である。
[0007] コラーゲンに対する特異性を失わずに、多種類存在するコラーゲン分子の合成を 同時に抑制する有効な手段のひとつとして、 HSP47の機能を制御する方法が考え られる。 HSP47は、様々なタイプのコラーゲンの合成過程で共通する細胞内輸送お よび分子成熟化に必須であるコラーゲン特異的分子シャペロンである。したがって、 星細胞において HSP47の機能を特異的に制御することができれば、肝線維化の抑 制の可能性が考えられるが、そうした治療方法の試みについては、いまだ報告はな レ、。
[0008] 発明者らは、細胞の系において、 HSP47の機能を特異的に制御するリボザィムを 作製し、これによつてコラーゲン産生分泌を一度に抑制できることを示した(Sasaki H, et al. Journal of Immunology, 2002, 168: 5^8-83; Hagiwara S, et ai. J Gene Med. 2003,5:784-94)。 HSP47の合成を特異的に抑制するためには、リボザィムより最適 ィ匕の容易な siRNAが適用され得る。本明細書において用いられる siRNA (small int erfering RNAs)とは、 RNAi (RNA interference)で用いる二本鎖 RNAの総称である。
RNAiとは、ある遺伝子と相同な、センス RNAおよびアンチセンス RNAからなる二本 鎖 RNA (double-strand RNA;dsRNA)力 その遺伝子の転写産物(mRNA)の相同 部分を破壊する現象で、当初線虫を用いた実験に示されたが(Fire A,et al: Nature ( 1998)391:806-811)、哺乳動物細胞にも同様な誘導機構が存在することが明らかにさ れている(Ui-Tei K,et al:FEBS Lett(2000) 479:79-82) 0また、 Elbashirらによって、長 さとして 2:!〜 23bp程度の短い dsRNA力 哺乳動物細胞系でも細胞毒性を示さずに RNAiを誘導できることが示されている(Elbashir SM,et al:Nature(2001)411 :494-498) 。し力、しながら、これらの分子の効果を有効に発揮させるためには、標的臓器に特異 的な方法が必要である。

[0010] 組織および/または臓器を標的とするために、ドラッグデリバリーシステム(DDS) の応用が有効な手段のひとつである(田端泰彦、ドラッグデリバリーシステム DDS技 術の新たな展開とその活用法一生物医学研究 ·先進医療のための最先端テクノロジ ーメディ力ノレドウ社、 ISBN: 4944157932、 2003 ;橋田充、ドラッグデリバリーシステム 創薬と治療への新たなる挑戦、新バイオサイエンスシリーズ、化学同人、 ISBN : 4759 803858、 1995)。ドラッグデリバリーシステム(DDS)に用いる薬物担体には、高分子ミ セル、リボソーム、マイクロエマルジヨンなどを応用したものなどが存在する。これらの 担体の標的臓器に対する特異性を高めるために、臓器および/または組織特異的 な抗原またはリセプターに対する抗体および Zまたはリガンドなどを該担体に混入ま たは結合させる技術、および担体の物理化学的性質を利用する技術などが知られて いる力 特に星細胞を標的とする技術については、レ、まだ知られていない。
課題を解決するための手段
[0011] 本発明は、診断および/または治療用薬物を星細胞へ特異的に運搬することを可 能とする薬物担体および薬物担体キットに関する。本発明における薬物担体は、高 分子ミセル、リボソーム、ェマルジヨン、微小球、ナノ小球のうちのいずれの形態でも よぐこれらにビタミン A(VA)もしくはレチノイド誘導体、たとえばトレチノイン、ァダパ レン、パルミチン酸レチノールなど、またはビタミン Aアナログ、たとえば Fenretinide (4 _HPR)などを結合させる力 たは包含させることにより、治療用薬物を肝星細胞特 異的に運搬することを可能にする。さらに、該薬物担体に truncated TGF j3 type II re c印 tor、および soluble TGF j3 type II rec印 torなどの TGF j3活性阻害斉 lj、 HGFなど の成長因子製剤、 MMP遺伝子含有アデノウイルスベクターなどの MMP産生促進 剤、 PPAR y— ligana、 angiotensin- II type I receptor antagonist^ PDwテロンンキ ナーゼ阻害剤、およびアミ口ライドなどのナトリウムチャンネル阻害剤を含む細胞活性 化抑制剤および/または増殖抑制剤、ならびに、 compound 861、および gliotoxinな どのアポトーシス誘導剤のうちから選択される 1つまたは複数の分子を包含するもの を作製し、線維化のリスクにある力もしくは線維化の症状を有する患者、または線維 ィ匕にともなう各種疾患、たとえば肝硬変、肝不全、肝臓癌、もしくは慢性膝炎を有する 患者に経口投与または非経口投与、たとえば静脈もしくは腹腔内投与することによつ て、星細胞の活性化を抑え、線維化および Zまたは線維化にともなう該各疾患状態 を予防、抑制または改善することができる。あるいは、またはこれに加えて、コラーゲ ン特異的分子シャペロンである HSP47もしくは MMP阻害因子である TIMPを特異 的に抑制するリボザィム、アンチセンス RNA、または siRNAを該薬物担体に封入し て用いることにより、それぞれ I型〜 IV型コラーゲンの分泌を同時に抑制し、その結果 線維形成を効果的に抑制することができる。
したがって、本発明は、レチノイド誘導体および Zまたはビタミン Aアナログを構成 成分とする、星細胞特異的薬物担体に関する。
本発明はまた、レチノイド誘導体がビタミン Aを含むことを特徴とする上記薬物担体 に関する。
本発明はさらに、 0. 2〜20重量%のレチノイド誘導体および/またはビタミン Aァ ナログを含むことを特徴とする上記薬物担体に関する。
本発明はさらにまた、高分子ミセル、リボソーム、ェマルジヨン、微小球、ナノ小球の うちのいずれかの形態である上記薬物担体に関する。
また、本発明は、上記の薬物担体と、星細胞の活性または増殖を制御する薬物とを 含む、星細胞に関連する疾患を処置するための医薬に関する。
さらに、本発明は、疾患が肝炎、肝線維症、肝硬変、肝癌、膝炎、勝線維症、膝癌 、声帯瘢痕形成、声帯粘膜線維症および喉頭の線維化からなる群から選択される、 上記医薬に関する。
さらにまた、本発明は、星細胞の活性または増殖を制御する薬物が、 TGF /3活性 阻害剤、 HGF活性製剤、 MMP産生促進剤、 TIMP産生阻害剤、 PPAR yリガンド 、アンジォテンシン活性阻害剤、 PDGF活性阻害剤、ナトリウムチャンネル阻害剤、 アポトーシス誘導剤、ならびに、星細胞によって産生される細胞外マトリクス構成分子 を標的とする、または該細胞外マトリクス構成分子の産生もしくは分泌に機能する分 子のうちの 1つまたはそれ以上を標的とする siRNA、リボザィム、アンチセンス核酸、 DNA/RNAキメラポリヌクレオチドおよびこれらを発現するベクターからなる群から 選択される、上記医薬に関する。
本発明はまた、細胞外マトリクス構成分子の産生もしくは分泌に機能する分子が H SP47である上記医薬に関する。
本発明はさらに、薬物と、薬物担体とを、医療の現場またはその近傍で混合してな る、上記医薬に関する。

発明の効果
[0016] 線維化および/またはそれに伴う各種疾患を予防、抑制または改善する有効な手 段として、診断および/または治療用薬物を星細胞へ特異的に運搬することを可能 とする本発明の薬物担体および薬物担体キットを用いることによって、実施例に見る ような画期的な治療効果を提供することができる。すなわち、本発明による薬物担体 および薬物担体キットは、星細胞を特異的な標的とするため、星細胞が中心となって 発現する病態、たとえば線維化などを効率的かつ効果的に、さらには最小限の副作 用で抑制することができる。

[0020] 本発明におけるレチノイド誘導体および/またはビタミン Aアナログは、ビタミン Aを 含むとともに、これを溶解または保持することができる媒体に溶解させるか混入させた 状態におけるレチノイド誘導体またはビタミン Aアナログをも含む。
本発明におけるレチノイド誘導体および/またはビタミン Aアナログは、星細胞によ つて積極的に蓄積されるものであれば何れのものでもよぐたとえばレチノイド誘導体 としては、限定するものではなレ、が、トレチノイン、ァダパレン、ノルミチン酸レチノ一 ノレ、または特にビタミン A (レチノール酸)などを用いることができ、またビタミン Aアナ ログとしては、限定するものではないが、たとえば Fenretinide (4— HPR)などを用い ること力 Sできる。本発明は、星細胞がレチノイド誘導体および/またはビタミン Aアナ ログを積極的に取り込む性質を利用するものであり、これらのレチノイド誘導体および /またはビタミン Aアナログを薬物担体として用いる力、またはこれ以外の薬物担体 構成成分に結合させるかまたは包含させることによって、所望の物質や物体を星細 胞に特異的に運搬させ得るようにしたものである。
[0021] したがって、本発明の薬物担体は、レチノイド誘導体および/またはビタミン Aアナ ログ以外の薬物担体構成成分を含んでレ、てもよレ、。かかる成分は特に限定されずに 、医薬および薬学の分野で知られる任意のものを用いることができる力 レチノイド誘 導体および/またはビタミン Aアナログを包含し得る力、、または、これと結合し得るも のが好ましい。このような成分としては、脂質、たとえば、グリセ口リン脂質などのリン脂 質、スフインゴミエリンなどのスフインゴ脂質、コレステロールなどのステロール、大豆 油、ケシ油などの植物油、鉱油、卵黄レシチンなどのレシチン類等が挙げられるが、 これらに限定されなレ、。このうち、リボソームを構成し得るもの、たとえば、レシチンな どの天然リン脂質、ジミリストイルホスファチジルコリン(DMPC)、ジパルミトイルホスフ ァチジルコリン(DPPC)、ジステアロイルホスファチジルコリン(DSPC)などの半合成 リン脂質、コレステロールなどが好ましい。
また、本発明の薬物担体は、星細胞への取り込みを改善させる物質、たとえば、レ チノール結合タンパク質 (RBP)などを含んでレ、てもよレ、。
[0022] 本発明の薬物担体へのレチノイド誘導体および/またはビタミン Aアナログの結合 または包含は、化学的および/または物理的な方法によってレチノイド誘導体およ び/またはビタミン Aアナログを薬物担体の他の構成成分に結合させる力または包 含させることによつても可能となる。または、本発明の薬物担体へのレチノイド誘導体 および/またはビタミン Aアナログの結合または包含は、該薬物担体の作製時に、基 本的な薬物担体構成成分とともに形成親和性のあるレチノイド誘導体および/また はビタミン Aアナログを薬物担体の構成成分中に混入させることによつても可能となる 。本発明の薬物担体に結合させるかまたは包含させるレチノイド誘導体および Zまた はビタミン Aアナログの量は、薬物担体構成成分中の重量比で 0. 01 %〜: 100%、好 ましくは 0. 2。/ο〜20。/ο、さらに好ましくは 1〜5。/0とすることが可能である。

[0024] 本発明の薬物担体は、星細胞を特異的な標的とするものであり、所望の物質や物 体、たとえば星細胞の活性または増殖を制御する薬物等を星細胞に効率的に運搬 することによって、最大の効果および最小の副作用において、所望の効果、たとえば 線維化の抑制または予防を可能にする。本薬物担体が送達する物質や物体は特に 制限されないが、投与部位から星細胞が存在する肝臓ゃ瞎臓などへ、生物の体内を 物理的に移動できるような大きさであることが好ましい。したがって、本発明の薬物担 体は、原子、分子、化合物、タンパク質、核酸等の物質はもとより、ベクター、ウィルス 粒子、細胞、 1以上の要素で構成された薬物放出システム、マイクロマシン等の物体 をも運搬することができる。前記物質または物体は、好ましくは星細胞に何らかの影 響を与える性質を有し、たとえば、星細胞を標識するものや、星細胞の活性または増 殖を制御するものを含む。
[0025] したがって、本発明の一態様においては、薬物担体が送達するものは「星細胞の 活性または増殖を制御する薬物」であり、これは、線維化促進に関係する星細胞の 物理化学的な作用を直接または間接に抑制する何れの薬物であってもよぐ限定す るものではないが、たとえば truncated TGF j3 type II receptor,および soluble TGF j3 type II receptorなどの TGF β活性阻害剤、 HGFなどの増殖因子製剤およびそれら の発現ベクター、 MMP遺伝子含有アデノウイルスベクターなどの MMP産生促進剤 、アンチセンス TIMP核酸などの TIMP産生阻害剤、 ΡΡΑΙ γリガンド、アンジォテン シン活性阻害剤、 PDGF活性阻害剤、ナトリウムチャンネル阻害剤を含む細胞活性 化抑制剤および/または細胞増殖抑制剤、ならびに compound 861、 gliotoxinなどの アポトーシス誘導剤、アディポネクチン (特開 2002-363094号公報参照)、 (+ )—トラ ンス一 4_ (1—アミノエチル) _ 1 _ (4—ピリジルカルバモイル)シクロへキサンなどの Rhoキナーゼ阻害活性を有する化合物(国際公開 WO 00/64478号パンフレット参照 )を包含する。また、本発明における「星細胞の活性または増殖を制御する薬物」は、 線維化抑制に直接または間接に関係する星細胞の物理化学的な作用を直接または 間接に促進する何れの薬物であってもよぐ限定するものではないが、たとえばコラ 一ゲン分解系を促進する薬物、たとえば MMP発現ベクターなどの MMP産生促進 剤、および HGF、 HGFアナログ、またはこれらの発現ベクターなどの HGF様の活性 を有する薬物を包含する。
[0026] 本発明における「星細胞の活性または増殖を制御する薬物」の別の例としては、細 胞外マトリクス、たとえばコラーゲンの代謝を制御する薬物、たとえば星細胞によって 産生される細胞外マトリクス構成分子を標的とする、または該細胞外マトリクス構成分 子の産生もしくは分泌に機能する分子のうちの 1つまたはそれ以上を標的とする、 si RNA、リボザィム、アンチセンス核酸(RNA、 DNA、 PNA、またはこれらの複合物を 含む)などの標的分子の発現を抑制する効果を有する物質、もしくはドミナントネガテ イブ効果を有する物質、またはこれらを発現するベクターなどが挙げられる。
siRNAは、 mRNAなどの標的分子に特異的な配列を有する 2本鎖 RNAであり、 標的分子の分解を促進することにより、それによつて形成される物質、たとえばタンパ ク質の発現を抑制する(RNA干渉)。 Fireらによってその原理が発表されて以来(Nat ure, 391:806-811,1998)、 siRNAの最適化に関してはすでに広範な研究がなされて おり、当業者は力かる技術に精通している。また、 siRNA以外の、 RNA干渉または その他の遺伝子発現阻害反応をもたらす物質についても精力的な研究がなされて おり、現在ではこのような物質も数多く誕生している。

従って、本発明の薬物担体が運搬する好ましい物質としては、たとえば、 HSP47を 標的とする siRNAや、 DNAZRNAハイブリッドもしくはキメラポリヌクレオチド、アン チセンス核酸などが挙げられる。
本発明の薬物担体の送達物としてはまた、線維化を抑制する薬剤、たとえば、 G- CSF (WO 2005/082402参照)、トロンボモジュリン様タンパク質(特開 2002-371006号 参照)、ケラタン硫酸オリゴ糖 (特開平 11-269076号参照)などを挙げることができる。
[0029] 本発明の薬物担体が送達する物質や物体は、標識されていてもいなくてもよい。標 識化により、運搬の成否や、星細胞の増減などをモニタリングすることが可能となり、 特に試験 ·研究レベルでは有用である。標識は、当業者に公知な任意のもの、たとえ ば、任意の放射性同位体、標識化物質に結合する物質 (たとえば抗体)、蛍光物質、 フルオロフオア、化学発光物質、および酵素などから選択することができる。
[0030] 本発明はまた、前記薬物担体と、前記星細胞の活性または増殖を制御する薬物と を含む、星細胞に関連する疾患を処置するための医薬、ならびに、前記薬物担体の 、星細胞に関連する疾患を処置するための医薬の製造への使用に関する。ここで、 星細胞に関連する疾患とは、星細胞が、疾患の過程、すなわち疾患の発症、増悪、 改善、寛解、治癒などに直接的または間接的に関与している疾患を指し、たとえば、 肝炎、特に慢性肝炎、肝線維症、肝硬変および肝癌等の肝疾患、膝炎、特に慢性陴 炎、膝線維症および陴癌等の膝疾患が含まれる。また、最近の報告によると声帯にも 星細胞が存在するため(たとえば、 Fuja TJら、 Cell Tissue Res. 2005;322(3):417-24 参照)、上記疾患には声帯瘢痕形成、声帯粘膜線維症、喉頭の線維化などの声帯- 喉頭の疾患も含まれる。 [0031] 本発明の医薬においては、薬物担体に含まれるレチノイド誘導体および/またはビ タミン Aアナログの少なくとも一部力 遅くとも星細胞に到達するまでに製剤の外部に 露出していれば、薬物担体が薬物を内部に含んでも、薬物含有体の外部に付着して 存在しても、また、薬物と混合されていてもよい。したがって、投与経路や薬物放出様 式などに応じて、上記医薬を、適切な材料、たとえば、 S募溶性のコーティングや、時 限崩壊性の材料で被覆してもよぐまた、適切な薬物放出システムに組み込んでもよ レ、。
本発明の医薬は、経口および非経口の両方を包含する種々の経路、たとえば、限 定することなぐ経口、静脈内、筋肉内、皮下、局所、直腸、動脈内、門脈内、心室内 、経粘膜、経皮、鼻内、腹腔内、肺内および子宮内等の経路で投与してもよぐ各投 与経路に適した剤形に製剤してもよい。力、かる剤形および製剤方法は任意の公知の ものを適宜採用することができる(たとえば、標準薬剤学、渡辺喜照ら編、南江堂、 20 03年などを参照)。
たとえば、経口投与に適した剤形としては、限定することなぐ散剤、顆粒剤、錠剤、 カプセル剤、液剤、懸濁剤、乳剤、ゲル剤、シロップ剤などが挙げられ、また非経口 投与に適した剤形としては、溶液性注射剤、懸濁性注射剤、乳濁性注射剤、用時調 製型注射剤などの注射剤が挙げられる。非経口投与用製剤は、水性または非水性 の等張性無菌溶液または懸濁液の形態であることができる。
[0032] 本発明の薬物担体または医薬は、いずれの形態で供給されてもよいが、保存安定 性の観点から、好ましくは用時調製可能な形態、たとえば、医療の現場あるいはその 近傍において、医師および/または薬剤師、看護士、もしくはその他のパラメディ力 ルなどによって調製され得る形態で提供される。この場合、本発明の薬物担体または 医薬は、これらに必須の構成要素の少なくとも 1つを含む 1個または 2個以上の容器 として提供され、使用の前、たとえば、 24時間前以内、好ましくは 3時間前以内、そし てより好ましくは使用の直前に調製される。調製に際しては、調製する場所において 通常入手可能な試薬、溶媒、調剤器具などを適宜使用することができる。
したがって、本発明はまた、薬物担体構成物質、およびレチノイド誘導体および Z もしくはビタミン Aアナログ、および Zまたは薬物のうちの 1つまたはそれ以上を含む 1 つまたはそれ以上の容器を含む薬物担体または医薬の調製キットを含み、そのような キットの形で提供される薬物担体または医薬の必要構成要素をも含む。本発明のキ ットは、上記のほか、本発明の薬物担体および医薬の調製方法や投与方法などが記 載された説明書等を含んでいてもよい。また、本発明のキットは、本発明の薬物担体 または医薬を完成するための構成要素の全てを含んでいてもよいが、必ずしも全て の構成要素を含んでいなくてもよい。したがって、本発明のキットは、医療現場や、実 験施設などで通常入手可能な試薬や溶媒、たとえば、無菌水や、生理食塩水、ブド ゥ糖溶液などを含んでレ、なくてもょレ、。
[0033] 本発明はさらに、前記医薬の有効量を、それを必要とする対象に投与することを含 む、星細胞に関連する疾患を処置するための方法に関する。ここで、有効量とは、対 象疾患の発症を低減し、症状を軽減し、または進行を防止する量であり、好ましくは、 対象疾患の発症を予防し、または対象疾患を治癒する量である。また、投与による利 益を超える悪影響が生じない量が好ましい。かかる量は、培養細胞などを用いた in vi tro試験や、マウス、ラット、ィヌまたはブタなどのモデル動物における試験により適宜 決定することができ、このような試験法は当業者によく知られている。
本発明の方法において投与する医薬の用量は、使用する薬物や、レチノイド誘導 体および/またはビタミン Aアナログの種類によって異なる力 たとえば、薬物として HSP47に対する siRNAを用いる場合においては、薬物の重量として、たとえば 0· 0 l~45mg/kg/曰、好ましく ίま 0·:!〜 30mg/kg/曰、より好ましく ίま:!〜 20mg/ kg/日、最も好ましくは 4〜6mg/kg/日である。また、レチノイド誘導体および/ま たはビタミン Aアナログとしてビタミン Aを用いる場合においては、ビタミン Aが、典型 的には、 10〜20mg/kg/日の用量で投与されるようにする。薬物担体に含まれる レチノイド誘導体および/またはビタミン Aアナログ、および本発明の方法に用いる 薬物の用量は当業者に公知であるか、または、上記の試験等により適宜決定するこ とができる。
[0034] 本発明の方法において投与する医薬の具体的な用量は、処置を要する対象に関 する種々の条件、たとえば、症状の重篤度、対象の一般健康状態、年齢、体重、対 象の性別、食事、投与の時期および頻度、併用している医薬、治療への反応性、お よび治療に対するコンプライアンスなどを考慮して決定され得るため、上記の典型的 用量と異なることもある力 力かる場合であっても、これらの方法はなお本発明の範囲 に含まれる。
投与経路としては、経口および非経口の両方を包含する種々の経路、たとえば、経 口、静脈内、筋肉内、皮下、局所、直腸、動脈内、門脈内、心室内、経粘膜、経皮、 鼻内、腹腔内、肺内および子宮内等の経路が含まれる。
投与頻度は、用いる医薬の性状や、上記のような対象の条件によって異なるが、た とえば、 1日多数回(すなわち 1日 2、 3、 4回または 5回以上)、 1日 1回、数日毎(すな わち 2、 3、 4、 5、 6、 7日毎など)、 1週間毎、数週間毎 (すなわち 2、 3、 4週間毎など) であってもよい。

collagen (I〜IV型)の共通分子シャペロンである HSP47の塩基配列を標的とする si RNA認識至適配列のうち、配列 Aおよび Bは、株式会社 iGENEの siRNAオリゴ用プ ログラムデザインに従い作成した。また配列 Cは、インターネット上で Ambion社製の si RNA arget ΡιηαΘΓ (1ι1:ΐρ:/Α¥\¥\¥·&Γηοιοη·(:οιηΛΘοηιϋ3/Γηΐ3θ/3ίΚΝΑ_ηη(ΐΘΓ.1ι1:Γη1)を用 いて、 rat gp46 (ヒト HSP47ホモログ、 GenBank Accession No. M69246)に対する ta rgetとなる 19塩基配列を選び検索し作成した。設計においては、 1 )開始コドンから 7 5〜100塩基下流から開始すること、 2)最初の AAダイマーを位置付けること、および 3) GC含有率が 30%〜70%であること、に留意した。本実施例においては、以下の 配列を有する siRNAを作製した。

ratの gp46を持っており、なおかつ collagenを産生する線維芽細胞である Normal rat kidney細胞(NRK細胞)に、 0. 1ηΜ〜50ηΜの各 siRNAをそれぞれ transfectionし 、 12〜48時間培養した(図 1)。 gp46の発現量は、 Western blot法で確認した(図 2 〜4、上のノ ンドカ ½p46、下がコントロールの actin)。いずれの siRNAも、 vehicleに 比べて顕著に gp46タンパク質の発現を抑制した(図 2)。以下の実験では、このうち 最も効果が強かった配列 Aを有する siRNAを用いた。 siRNAによる抑制は濃度依 存的であり(図 3)、 gp46の蛋白発現は 50nMの siRNAにて 48時間後には約 90% 抑制された(図 4)。 [0040] 実施例 3 作製した siRNA による collagen合成の抑制
前述のとおりの条件(siRNAの濃度は 50nM、時間は 48時間で処理)で、コラーゲ ンの合成量を検討するためラット線維芽細胞(NRK細胞)の培養上清に3 H-prolineを 加え、 transfection後の分泌蛋白中の3 H量を検討した(図 5)。コラーゲン合成量は、 P eterkofskyらの報告(Peterkofskyら、 Biochemistry. 1971 Mar 16;10(6):988_94)を元 に、 gp46siRNA導入線維芽細胞を3 H-proline存在下に培養し、上清中に分泌され た蛋白量と collagenaseで分解された蛋白量の比から算定した。

(5.4 X 非感受性分画 + co//crgenoye感受性分画) ラット線維芽細胞におけるコラーゲン合成率は control群と比較して約 40%低下した (図 6)。
[0041] 実施例 4 核酸の肝星細胞(HSC)特異的な導入
10%VAとリボソームを混合し VAを被包化したリボソームと GFP発現 plasmidとを混 合したェマルジヨン (VA— Lip— GFP)を作製し、ラット門脈内へ投与後、肝組織を 回収固定した。ェマルジヨンは、 200gのラットの血漿量を約 10mlと想定し、門脈血 中の VAおよび GFP濃度が ΙΟ /i Mとなるように作製した。具体的には、まず al卜 trans -retinol (VA) 25mgをDMS〇87 μ lで溶解し、 lOOmMの stock solutionを作製した 。この VA stock solution 1 μ 1に lipofectaminelO μ 1 PBS 179 μ 1を加えて、さらに G FP発現 plasmidlO μ gを添加し totalで 200 μ 1とし、 3分間 voltexし VA_Lip _GFPと した。 SDラットを開腹し、 VA_Lip_GFPを末梢門脈内にゆっくり注入した。注入の 48時間後に肝組織を採取した。中間径フィラメントのデスミンは、他の肝細胞と比較 して肝星細胞(HSC)に特異的に発現しているので、固定肝組織を Alexa Fluor568 標識抗デスミン抗体で染色し、 GFPとの蛍光二重像を観察したところ、肝星細胞 (H SC)内で GFPが発現していることが確認された(図 7)。無処置対照や GFP発現 plas mid vector単独投与群ではラット肝星細胞での発現は認めなかった力 S VA-Lip - GFPを投与した群では、星細胞特異的に GFPの発現を認めた。
[0042] 実施例 5 核酸導入率の定量 GFP発現 plasmidの代わりに FITC標識 gp46siRNAを用いた以外は、実施例 4と 同様にして、 VAを被包化したリボソームと FITC標識 gp46siRNAとを含むェマルジ ヨン(VA— Lip— gp46siRNA (FITC) )を作製し、 SDラットに門脈内投与した(siRN 八の量として10

1)。投与の 48時間後に肝組織を採取し、他の肝細胞と 比較して HSCに特異的に発現してレ、る a SMA (平滑筋ァクチン)を Alexa Fluor568 標識抗ひ SMA抗体で、細胞核を DAPIでそれぞれ染色し、共焦点レーザー走查顕 微鏡(LSM)で蛍光象を観察した。図 8左側に示すとおり、 VA-Lip - gp46siRNA (FITC)投与群では、 FITCによる緑色蛍光と Alexa Fluor568による赤色蛍光の両方 を発する細胞が多く見られ、 NIH imageによって定量したところ(xlOOOの蛍光顕微 鏡写真を任意の 10視野選び上記細胞数を検定した)、導入効率は 77. 6 % ( 10視 野平均)であった。これに対して、 VAを含まなレ、 Lip _ gp46siRNA (FITC)投与群 においては、導入効率は 14. 0%と低く、しかも、星細胞以外の細胞への導入が 3. 0 %見られた(図 8右側参照)。以上の結果から、 VAを包含させることにより、星細胞へ の導入効率が劇的に高まることが分かる。
[0043] 実施例 6 VA— Lip— gp46siRNAによる gp46の発現抑制
実施例 5で採取した組織の別の切片において、 gp46を Alexa Fluor568標識抗 HS P47抗体で、細胞核を DAPIでそれぞれ染色し、共焦点レーザー走査顕微鏡で蛍 光象を観察した。図 9に示すとおり、 VA— Lip— gp46siRNA投与群において、赤色 蛍光として認められる gp46の発現(同図右側)が、 gp46に特異的でない random si RNAを含む VA—Lip—random siRNAを投与した対照群(同図左側)に比べて 顕著に低減してレ、ることが認められた。対照群の 6視野平均に対する発現抑制率は 、実施例 7と同様に NIH imageにより xlOOOの蛍光顕微鏡写真を任意の 10視野選 び gp46陰性細胞数を検定したところ、 75%と極めて高いものであった。
[0044] 実施例 7 LC ratの治療(門脈内投与 1 )
Jezequelら(Jezequel AMら、 J Hepatol. 1987 〇ct;5(2): 174- 81)の報告に従い、 Dime thylnitrosamine (DMN)を使用して、 LC model ratを作製した(図 10)。具体的には、 5週齢の SD rat (male)に1

を1111171¾ (腹腔内投与) の分量において週 3回連日投与した。既報の如く 2週目より線維の増加を認め 4週目 には著明な線維化、肝小葉構築の破壊、再生結節形成を伴う所見を認めた (図 11) 。そこで、実施例 4と同様な方法によって gp46siRNAをリボソーム化し、これに 10% VAを混合したェマルジヨン (VA— Lip— gp46siRNA)を投与した。十分に線維化 の認められる 3週目より VA_Lip_gp46siRNAの投与を開始し、 4週目と 5週目で 評価を行った。実施例 2によりインビト口にて 48時間まで効果が認められることを確認 しているので、投与は週に 2回とした(図 11)。投与量は siRNAを direct injectionした 既報(McCafferyら、 Nature. 2002 Jul 4;418(6893):38- 9)に基づき siRNAを総量で 40 μ gとした。 siRNA投与後の肝臓の azan染色では、 4週目では生食投与群、 siRNA (random)投与群、および siRNA (gp46)投与群の間で明らかな差を認めなかった が、 5週目では gp46siRNA投与群にて線維量の低下が見られた(図 12)。線維の量 を定量ィ匕するために NIH imageを使用して被染色部分を抽出し、その面積を計測 した(図 13)ところ、 gp46siRNA投与群において有意に collagen面積の低下が認め られた(図 14)。また、別の尺度で線維化の程度を評価するために、線維化の指標と なるヒドロキシプロリンの定量を定法により行った。具体的には、凍結乾燥した肝組織 20mgを HC1で 24時間加水分解した後、反応液を遠心分離し、上清を Ehrlich' s solu tion等の試薬にて処理し、遠心分離した。上清を回収し、 560 醒における吸光度を 測定することで、肝組織中のヒドロキシプロリン量を測定した(H印 atology 1998 Nov; vol.28: 1247-1252)。図 15に示すとおり、 gp46siRNA投与群では、ヒドロキシプロリン の量が極めて少なくなつていた。
実施例 8 LC ratの治療(門脈内投与 2)
さらに、本発明の医薬の投与による生存率の変化を検討するため、 Qi Zらの方法( Proc Natl Acad Sci U S A. 1999 Mar 2;96(5):2345_9.)に基づき、通常より 20%増量 した Dimethylnitrosamine (DMN)を使用して、 LC model ratを作製した。本モデルで は、 1週目と 2週目に計 4回の門脈内投与を行った。投与内容は、 PBS、 Lip -gp46 siRNA, VA- Lip -random siRNAおよび VA_Lip _gp46siRNAとした(各群 とも n = 7)。 3週後では、 control (PBS投与群、 VA- Lip -random siRNA投与群 および Lip_gp46siRNA投与群)が全例死亡したのに対し、 VA_Lip _gp46siR NA投与群では 7匹中 6匹が生存していた(図 16)。また、 21日目の肝臓の azan染色 では、 gp46siRNA投与群にて線維量の明らかな低下が見られた(図 17)。
実施例 9 LC ratの治療(門脈内投与 3)
別な実験では、前記 Qi Zらの方法および Ueki Tらの方法(Nat Med. 1999 Feb;5(2): 226-30)に基づいて作製した LC model rat (l %DMNlmgZkgを週 3回腹腔内投与 )に、 3週目から下表に示す内容の門脈内投与を行った (各群とも n = 6)。なお、各投 与物には PBSをカ卩えて総体積が 200 μ ΐとなるようにして投与し、投与回数は週 1回と した。

この結果、本発明の医薬を投与された群(処置群 9一 4)以外は、 DMNの投与開始 力 45日後までに 6匹すべてが死亡した力 本発明の医薬を投与された群は、 36日 目に死亡した 1例を除き、全個体が DMNの投与開始から 70日を超えて生存を続け た(図 18)。なお、死亡個体について肝の線維量を実施例 7と同様に Collagen面積に 基づいて定量したところ、 VA_Lip_gp46siRNAの投与により肝線維量の増加が 顕著に抑制されてレ、た(図 19)
実施例 10 LC ratの治療(静脈内投与)
実施例 9と同様に作製した LC model rat (l %DMNl μ g/BW(g)を週 3回腹腔内 投与)に、 3週目から下表に示す内容の静脈内投与を行った (各群とも n = 6)。なお、 各投与物には PBSをカ卩えて総体積が 200 / lとなるようにして投与した。また、投与期 間は、 10— 4群は 7週目まで、 10— 10群は 6週目までそれぞれ投与した以外は、死 亡するまで投与した。

RNA ^50^g この結果、本発明の医薬を投与された群(処置群 10— 4および 10— 10)以外は、 DMNの投与開始から 45日後までに 6匹すべてが死亡した力 本発明の医薬を投与 された群は、処置群 4において 45日目に 2匹が死亡したのを除き、全個体が DMNの 投与開始から 70日を超えて生存を続けた(図 20および 21)。なお、死亡個体につい て肝の線維量を実施例 7と同様に定量したところ、 VA_Lip_gp46siRNAの投与 により肝線維量の増加が顕著に抑制されていた(図 22)。
以上の結果は、本発明の医薬が星細胞が関与する線維化の予防および治療に極 めて有効であることを示すものである。
実施例 11 RBP (レチノール結合タンパク質)による効果の改善
ヒト肝星細胞由来の細胞系である LI90を用いて、 RBPが VA_Lip_gp46siRNA 導入効率にもたらす影響について検討した。まず、実施例 5で作製した VA_Lip_g p46siRNA(FITC)100nMを、種々の濃度(すなわち、 0、 0.1、 0.5、 1、 2、 4また は 10%)の FBS (ゥシ胎仔血清)と共に、培養中の LI90に加え、 48時間インキュべ ートした後、蛍光像を LSMで観察し、個々の細胞に取り込まれた siRNAの量を FA CSにて定量した。なお、 FBSには、 RBP力 S約 0.7mg/dl含まれている。図 23に示 すとおり、 FBS(RBP)は濃度依存的に siRNAの導入量を増加させた。次に、 100η Mの VA— Lip— gp46siRNA(FITC)と、 4%の FBSと共に、 10 (21,476 nmol) の抗 RBP抗体を、培養中の LI90に加え、同様に siRNAの導入効率を評価した。図 24に示すとおり、 RBPにより増大した導入量力 抗 RBP抗体の添加により顕著に減 少していることが分かる。以上の結果は、 RBPが本発明の医薬の導入をさらに向上さ せるのに有効であることを示すものである。

Claims

請求の範囲
[1] レチノイド誘導体および Zまたはビタミン Aアナログを構成成分とする、星細胞特異 的薬物担体。
[2] レチノイド誘導体がビタミン Aを含むことを特徴とする請求項 1記載の薬物担体。
[3] 0. 2〜20重量0 /0のレチノイド誘導体および/またはビタミン Aアナログを含むことを 特徴とする請求項 1に記載の薬物担体。
[4] 高分子ミセル、リボソーム、ェマルジヨン、微小球、ナノ小球のうちのいずれかの形 態である請求項 1〜3のいずれかに記載の薬物担体。
[5] 請求項 1〜4のいずれかに記載の薬物担体と、星細胞の活性または増殖を制御す る薬物とを含む、星細胞に起因する疾患を処置するための医薬。
[6] 疾患が、肝炎、肝線維症、肝硬変、肝癌、膝炎、勝線維症、膝癌、声帯瘢痕形成、 声帯粘膜線維症および喉頭の線維化からなる群から選択される、請求項 5に記載の 医薬。
[7] 星細胞の活性または増殖を制御する薬物が、 TGF β活性阻害剤、 HGF活性製剤 、 ΜΜΡ産生促進剤、 ΤΙΜΡ産生阻害剤、 PPAR yリガンド、アンジォテンシン活性 阻害剤、 PDGF活性阻害剤、ナトリウムチャンネル阻害剤、アポトーシス誘導剤、なら びに、星細胞によって産生される細胞外マトリクス構成分子を標的とする、または該 細胞外マトリクス構成分子の産生もしくは分泌に機能する分子のうちの 1つまたはそ れ以上を標的とする siRNA、リボザィム、アンチセンス核酸、 DNA/RNAキメラポリ ヌクレオチドおよびこれらを発現するベクターからなる群から選択される、請求項 5ま たは 6に記載の医薬。
[8] 細胞外マトリクス構成分子の産生もしくは分泌に機能する分子が HSP47である請 求項 7に記載の医薬。
[9] 薬物と、薬物担体とを、医療の現場またはその近傍で混合してなる、請求項 5〜8の いずれかに記載の医薬。
[10] 星細胞の活性または増殖を制御する薬物、薬物担体構成物質、ならびにレチノイド 誘導体および/またはビタミン Aアナログのうちの 1つまたはそれ以上を含む 1つまた はそれ以上の容器を含む、請求項 5〜9のいずれかに記載の医薬の調製キット。
[11] 請求項 5〜9のいずれかに記載の医薬の有効量を、それを必要とする対象に投与 することを含む、星細胞に起因する疾患を処置するための方法。
[12] 疾患が、肝炎、肝線維症、肝硬変、肝癌、膝炎、勝線維症、膝癌、声帯瘢痕形成、 声帯粘膜線維症および喉頭の線維化からなる群から選択される、請求項 11に記載 の方法。
[13] 医薬が非経口投与される、請求項 11または 12に記載の方法。
[14] 請求項:!〜 4のいずれかに記載の薬物担体の、星細胞に起因する疾患を処置する ための医薬の製造への使用。
[15] 請求項:!〜 4のいずれかに記載の薬物担体を利用した、星細胞への薬物送達方法
