醫藥遞送封裝及吸入器 【相關申請案之交叉引用】
本申請案主張對提交於2010年1月5日之美國臨時申請案第61/292,401號,提交於2010年1月5日之美國臨時申請案第61/292,403號及提交於2010年1月5日之美國臨時申請案第61/292,404號之優先權。該等臨時申請案之全文內容以引用之方式併入本文。
本發明大體而言係關於吸入裝置領域。本發明具有與使用一乾粉吸入器對患者遞送粉狀藥品相關之特別效用,且儘管可預期其他效用,但本發明將結合該效用來進行描述。
已知藉由直接應用治療藥劑對某些呼吸道疾病有效。由於該等藥劑最易於以乾粉形式獲得,故藉由經鼻或口吸入粉末材料最方便實現對其應用。由於藥物準確地沉積於所要之位點及要求生效之處,該粉狀形式導致藥劑之更好利用,因此,該藥物之非常微小之劑量經常如藉由其他方式給藥之較大劑量同樣有效,隨之而顯著減少非所欲之副作用之發生率及藥劑費用。或者,該形式之藥物可用於除彼等呼吸系統外之疾病之治療。當藥物沉積於肺的非常大的表面區域時,其可快速地經吸收進入至血流中,因此,該應用方法可代替藉由注射、錠劑或其他傳統方法之給藥。
制藥工業之觀點為當遞送至呼吸道之藥物粒子尺寸為1微米與5微米之間時,該藥物之生物利用率為最佳。當該等藥物粒子需要在該尺寸範圍中時,乾粉遞送系統需要處理一些問題:
(1)小型粒子在製造及存儲期間可於其自身上產生靜電電荷。此現象可導致該等粒子結塊或集聚,產生有效粒徑大於5微米之粒子凝塊。於是,該等大凝塊進入至肺之深處的機率便減少。此相應地造成患者可吸收利用之封裝藥物的百分比更低。
(2)需要遞送至患者的活性藥物的數量可為10微克級。舉例而言,在使用沙丁胺醇(albuterol,一種在氣喘中使用的藥物)的情況下,該數量通常為25微克至50微克。現時之製造設備可用滿意之精確度在毫克劑量範圍中有效地遞送藥物小劑量。因此標準操作規程為將活性藥物與諸如乳糖之填料或填充劑混合。該添加劑亦使得該藥物「易於流動」。由於該等藥物粒子亦經由靜電或化學鍵黏著於該等粒子上,故亦將該填充劑稱為載體。該等載體粒子在尺寸上比藥物粒子大很多。在本設計之可行性中,乾粉吸入器使藥物自載體分離之能力為一重要效能參數。
(3)具有大於5微米尺寸之活性藥物粒子將可能沉積於口中或咽喉中。由於藥物在該等位置中的生物利用率及吸收與在肺中不同,故此引入另一不確定水平。乾粉吸入器需最小化沉積於該等位置之藥物以減少與藥物生物利用率相關之不確定性。
先前技術之乾粉吸入器(DPIs)通常具有一將藥物(活性藥物加載體)引入至一高速空氣流之構件。使用高速空氣流作為將微粒化粒子凝塊破裂或將藥物粒子自載體分離的主要機制。在先前技術中已知若干具有分配藥劑粉狀形式用途之裝置。舉例而言,在美國專利第3,507,277號、第3,518,992號、第3,635,219號、第3,795,244號及第3,807,400號中揭露了具有用於將含納粉狀藥劑之膠囊穿孔之構件的吸入裝置,該粉狀藥劑依靠吸入自穿孔的膠囊中吸出並進入至使用者口中。該等專利中之若干專利揭露了螺旋槳構件,該構件依靠吸入幫助分配自膠囊劑中出來之粉劑,因此並非必需完全依靠吸入空氣來自膠囊中吸取粉劑。舉例而言,在美國專利第2,517,482號中揭露了一裝置,其具有一在吸入之前置於下氣室中之粉劑含納膠囊,其中該膠囊藉由使用者手動按下穿孔銷而穿孔。穿孔後,吸入開始並將該膠囊拉入至該裝置之上氣室,該膠囊在該上氣室中沿各個方向運動以導致經由穿孔洞之粉劑分配並進入吸入氣流中。美國專利第3,831,606號揭露了一吸入裝置,其具有多孔穿孔銷、螺旋槳構件及一用於經由外部手動操作來操作該螺旋槳構件之自備電源,以便該螺旋槳構件依靠吸入幫助將粉劑分配進入所吸入之氣流中。亦可參見美國專利第3,948,264號及第5,458,135號。

本發明之另一態樣提供一吸入器,其利用如上所述之匣體遞送醫藥至人或動物患者之氣道。該吸入器包含一用於一振動裝置及一馬達的外殼。該匣體應成環狀,氣泡條帶含納在該環之內。該外殼包括一進氣口及一壓力感測器,該進氣口與該匣體之氣流道接合或配合。一O型環或類似物可用來密封該進氣口與該氣流道之間的連接。該振動裝置(其可為一壓電裝置)與該匣體之氣霧室接合。該氣霧室可形成一共振室。該馬達亦與該匣體接合,提供動力以驅動該裝置從而推動該氣泡條帶與該裝置以去除箔。
本發明之又一態樣提供一用於自動遞送醫藥劑量至人或動物患者之氣道之吸入器。該吸入器包含一含納醫藥之至少一劑量的外殼,一壓力感測器,一振動裝置及一氣霧室。該外殼與一接口連接,用於遞送醫藥至患者,且患者經由該接口呼吸。
在下述描述中,參閱作為本文一部分之隨附圖式,藉由圖示說明之方法圖示了本發明之各種實施例。應理解,在不脫離本發明之範疇的情況下,可利用其他實施例並做出改動。
本發明提供一用於遞送醫藥至患者氣道之改良的吸入裝置及方法。該所期望之患者可為人或動物且該吸入裝置應據此設計。該吸入器將結合一乾粉吸入器討論,但可預見本發明對其他類型之吸入器亦有作用。
在第一態樣中,本發明提供一用於遞送醫藥之分散劑量的封裝,其中該個別劑量經分隔至以一樣式佈置於圓柱形劑量筒上之個別分隔間中。該個別劑量可沉積在諸如在以引用之方式併入本文之共同擁有美國申請案第11/425,097號中描述之個別氣泡中。該個別劑量亦可在薄膜之間或在薄膜與基材之間封裝,該等薄膜或薄膜與基材可能或可能不與預先形成之凹部或其他凹穴而形成以成為該等分隔間之部分。
該個別分隔間以一種樣式佈置於該圓柱形劑量筒上,該劑量筒包含一成圓柱形形狀的基材,具有一內面與一外面。該等含納個別劑量醫藥之分隔間可在該基材之內面或外面上形成。或者,該等分隔間可自該基材突出而形成,至少部分地在基材之內表面與外表面上。該基材可由任何諸如(例如)塑料、陶瓷、紙張或金屬材料之適當材料組成,且在外觀上可為透明至不透明。
在本發明之本態樣的一實例中,該劑量筒以一基材材料形成,經設置至一佈置有凹部的圓筒中,各個包含一凹槽體積之凹部經設置朝向該劑量筒之中心自該基材突出。該個別劑量分隔間由該凹部形成且藉由一外鞘限制。參閱第1A-D圖,根據一緊密容差該外鞘20可成型套在該劑量筒10上以充分地分隔該個別劑量分隔間12。就此而言,該外鞘可包含一緊密接頭套管,其環繞該劑量筒以在該劑量筒之各個劑量分隔間中含納各個劑量並提供一用於藥物保存或穩定性的防潮層。該筒外鞘可設置一裝填存取艙口21於其表面以允許以選擇之醫藥裝填該分隔間,並設置一劑量艙口22用於卸載該劑量分隔間中之物至劑量室30中。
該外鞘亦可由諸如帶、箔或成膜材料之薄膜形成,該薄膜可附著於該劑量筒以便分隔該等個別劑量。該薄膜應足夠結實以承受該醫藥材料,但是就單一劑量分隔間而論,該薄膜亦可設計為可穿孔或去除的以方便裝載個別劑量至該吸入器之劑量室。該可選之設計之實例在第2圖中圖示,其中凹部(不可見)在該劑量筒上成螺旋狀樣式,且該外鞘包含一條可剝離之膜、箔或帶15。
第3A-C圖說明本發明之另一實例。在該實例中,該形成劑量筒之圓筒直徑增大且高度降低,形成一劑量環60。該劑量環為環形帶形式,分隔間環繞其佈置,該等分隔間將接受預計量之藥物重量及/或體積。該劑量環可與一驅動器連接,該驅動器將導致該環以一旋轉方向推動。儘管第1A-1D圖之劑量筒具有大於劑量分隔間之一列的高度,但該劑量環僅為劑量分隔間之單一列之高度。
本發明之外鞘在本實例中經改造形成環外鞘70,一緊密接頭套管環繞該劑量環,以在該劑量環之各個劑量容器中含納個別劑量及提供用於保存該醫藥物質之防潮層。與上述外鞘相似,該環外鞘將具有一裝填存取艙口71及一劑量艙口72,其中後者與該劑量室連接。與上述劑量筒相似,該環外鞘可以一可剝離之膜、箔或帶替換,該膜、箔或帶可僅在暴露於計量室之前去除。
本發明之劑量筒可藉由配合一含納所要之醫藥的料斗來裝載個別劑量,其中該個別劑量分隔間之幾何結構可用來幫助計量醫藥。
可將該劑量筒製造為吸入器之可重複用部件或可棄式醫藥劑量容器。該特殊的設計可指導何種材料適於在該劑量筒之製作中使用。
本發明之另一態樣提供一吸入器,其利用如上所述之劑量筒遞送醫藥至人或動物患者之氣道。再參閱第1A-1D圖,該吸入器包含一具有圓柱形基材之劑量筒10,在該基材上配置有複數個劑量分隔間12;一劑量室30,用於在個別劑量遞送至患者之前自個別劑量分隔間接受個別醫藥劑量;以及一與該劑量室鄰接之氣流道40,其用於運送該醫藥至患者之氣道。
該劑量室30可包含一共振室,其具有一可以選擇之頻率聲學共振的體積與形狀。如共同擁有美國專利第7,318,434號、第7,334,577號及第7,779,837號(該等專利之內容以引用之方式併入本文)所描述,該共振室接著可與一諸如壓電換能器之振動裝置50耦接,以提供振動能量利用該共振室之聲學性質而產生一合成射流。該共振室經由劑量艙口22自該劑量筒接收個別劑量。在共同擁有美國專利申請案第11/060,267號中揭露了合適的振動裝置之其他實例,該申請案以引用之方式併入本文。
經由計量洞32,該醫藥材料然後藉由合成射流解凝塊並排出至氣流道40中。計量洞促進合成射流之形成並使該醫藥便於自該計量室傳遞至該氣流道中。該洞之尺寸可影響合成射流的速度並最終使粒子分佈良好。
振動裝置50與電源供應器52連接。如在以引用之方式併入本文之同在申請中的共同擁有美國申請案第12/392,686號中所描述,該振動裝置可進一步與一頻率產生器連接以達到最佳效能。
該吸入器可利用一佈局裝置以推動該劑量筒並將一特殊的劑量分隔間對準該外鞘之劑量艙口。該佈局裝置可為(例如)一螺桿傳動裝置。該佈局裝置可(例如)如第2圖所示影響該劑量分隔間在該劑量筒表面之幾何排列。
本發明之吸入器可進一步包含一腔室密封件。該腔室密封件可為一塞子形式,當該裝置閒置時,該塞子防止空氣進入該劑量室。該塞子包括於吸入器中以進一步避免個別劑量在濕氣、氧及其他污染物中之不當暴露。當該吸入器感測到患者吸入,該腔室密封件在吸入器之授權信號下打開(例如,見均轉讓至公共受讓人之美國專利第6.152,130號及美國公開申請案第2005/0183725號),並在該劑量用完之後關閉。
該腔室密封件可進一步藉由一氮管路與一加壓氮室連接,該氮管路在劑量間將以氮裝填該劑量室。該腔室密封件亦可與一真空源連接以抽空劑量間的空氣及濕氣。此可利用相同氮管路之至少一部分來達到。該腔室密封件亦可與一乾燥劑室連接以在分配劑量期間吸收傳遞自大氣的濕氣。
本發明之該裝置容許修改。舉例而言,該劑量室可與多劑量筒連接以遞送組合醫藥產品。
參閱第4-11圖,本發明之另一態樣提供一吸入器101,其利用一成匣體120型之醫藥遞送封裝,該匣體可重複使用或為可棄式。該裝置之操作原理與在上文描述並進一步在上文提到之共同擁有專利中描述之裝置相似。詳言之,以引用之方式併入本文之美國專利公開案第2010/0294278號描述了一緊密型吸入器,其中複數個個別劑量醫藥含納在旋轉盒子中。該盒子含納成徑向排列之個別氣泡。除旋轉盒子由一含納氣泡條帶130的匣體120替代之外,在該專利公開案第2010/0294278號中描述之吸入器在許多方面與本發明之態樣相似。其他差異可自隨後之討論顯而易見。
如第4-10圖所示,該吸入器通常包含一外殼110及一匣體120。如同上述發明,該吸入器包括一吹口125及一振動裝置111。該吹口與一氣流道123連接,該氣流道123與一氣霧室121連接,位於與該振動裝置鄰接處。然而,與上述實例不同,該吹口、氣流道及氣霧室併入於該匣體120中。該佈置在減少該吸入器之尺寸,藥物保護及維持效能方面提供優點。
該匣體120含納一氣泡條帶130,該氣泡條帶130由一諸如冷成型塑料之基材133組成,在該基材133上形成有一系列凹陷134或孔穴以含納該個別劑量105於其中。一箔、膜或帶135置於該基材之上以密封該個別凹陷,其形成個別以該醫藥裝填之氣泡131。該匣體亦包括氣霧室121,根據上文及上述參考之申請案所述之方法,個別劑量在該氣霧室121中解聚集。該氣霧室如本文所描述較佳地為一共振室,且在裝配時藉由一與該振動裝置111接觸之薄膜122約束。
在所示實例中,該匣體亦包括一推動裝置140,其用於相對於該氣霧室121推動該氣泡條帶130。該用於推動氣泡條帶之裝置可包含一臂、一彈簧、一凸輪、一齒輪、一輪或其任何組合。舉例而言,第5圖圖示一導向環148,如同一彈簧與一臂之組合,其與該氣泡條帶接合以便推動該氣泡條帶。一氣泡偵測開關142感測該氣泡條帶之推動並使該氣泡條帶之推動便於控制從而相對於該氣霧室121一致地安置個別氣泡。
該匣體亦包括一箔、膜或帶去除裝置141,用於在該氣泡條帶相對於該氣霧室推動時從該氣泡條帶上去除該箔、膜或帶。該箔、膜或帶去除裝置包括一收集捲筒145(take-up spool 145)。一旦將該箔、膜帶或帶去除,即將該個別劑量105卸載至該氣霧室121以遞送至患者。
該氣泡131在氣泡條帶130上之佈置便於,或必需,在一(例如)中心具有一開放區150之環中形成該匣體120。該外殼包括一輔助突出件151。在該突出件之內,該外殼含納一電池112及馬達113。該馬達轉動齒輪143、144,並從而驅動該收集捲筒145及箔去除裝置141。該電池亦提供電力至該可能為一壓電裝置之振動裝置111。該電池亦可能攜帶於該匣體之上,且可隨同該匣體替換。或者,該氣泡條帶可藉由(例如)一指撚螺釘、槓桿或時鐘機構推動。
在所示實例中,該外殼進一步包括一進氣口116,其與該匣體之氣流道124連接。放置一O型環119以便於密封在該氣流道與該進氣口之間之接口。一藉由該電池提供電力之壓力感測器117,其位於該進氣口附近之壓力感測器閥口118中以便經由該吸入器感測患者之呼吸。
本態樣可根據本文及以引用之方式併入本文之共同擁有專利之其他實例修改。舉例而言,該吸入器可以吹口125、氣流道123、氣霧室121或其任何組合製作,以形成代替匣體120之主外殼110之部分。
本發明進一步提供一方法與裝置,用於計量醫藥材料至一劑量數量並遞送彼劑量至患者之氣道。
舉例而言,本發明之一態樣提供一用於遞送醫藥劑量至患者之氣道之吸入器。該所期望之患者可為人或動物且該吸入裝置應據此設計。該吸入器將結合一乾粉吸入器討論,但可預見本發明在修改其他類型吸入器之設計中亦有作用。
該吸入器包括一含納大量形式之醫藥材料之儲器。參閱第11A-11B圖及第12A-12B圖,該儲器之有利形狀可為圓筒,一活塞211及壓力彈簧212包括於其中以輔助卸載該儲器。其他諸如球形,圓錐體等形狀亦可利用,且該等圖例並不意謂將本發明限制於使用圓柱形儲器。其他形式之背壓力可作為活塞及壓力彈簧之替代使用。同樣地,除了圖示之螺旋彈簧,許多類型之彈簧適用於本發明之吸入裝置。本發明之顯著優勢為,該儲器可以大量材料重新裝載且該裝置可重複使用。
本實例之吸入裝置亦包括一用於將大量醫藥材料量測為單一醫藥劑量之計量裝置。該計量裝置應具有一用於自該儲器接收醫藥材料的計量凹槽,該計量凹槽按尺寸製作以接收所要之劑量。
參閱第11A圖及第11B圖,該計量裝置可由一計量筒220及一外鞘230組成。在該實例中,在該計量筒外表面上形成計量凹槽作為計量凹部222。該凹部可構成該計量筒之整體且應製造為精確體積以計量該醫藥劑量。

參閱第12A圖及第12B圖,本發明之計量裝置可包含一藉由至少兩個計量門225、226封閉之計量凹槽或計量分隔間。第一計量門225位於該計量分隔間與該劑量室240之間。第二計量門226在該計量分隔間與該儲器210之間。因此,當第二計量門打開(第一計量門保持關閉)時,該計量分隔間可以該醫藥材料201裝載。然後,當第一計量門打開(第二計量門保持關閉)時,該計量之醫藥材料劑量可置於該劑量室中。
本發明亦提供一方法,其藉由以下操作而用於遞送醫藥材料至可為人或動物之患者之氣道:提供醫藥材料於一與計量裝置連接之儲器中;以該計量裝置計量該醫藥材料以形成單一醫藥劑量;移動該單一醫藥劑量至一劑量室中;在該單一醫藥劑量中解聚集該醫藥材料及經由一氣流道自該劑量室遞送該單一醫藥劑量至患者之氣道。
本發明之方法可隨同諸如本文描述之包含一計量筒及一外鞘的計量裝置一同使用。或者,該計量裝置可包含一封閉於第一計量門與第二計量門之間的計量分隔間,其中該第二計量門在計量該醫藥材料之步驟期間打開,且該第一計量門在移動該單一醫藥劑量至該劑量室中之步驟期間打開。
該劑量室可充當與一振動裝置耦接之共振室,其中該解聚集醫藥材料之步驟涉及啟動該振動裝置以產生一合成射流從而經由一氣流道自該劑量室遞送該單一醫藥劑量至患者的氣道。
本發明之另一態樣提供一裝置,其用於計量醫藥材料至挑選之劑量數量並遞送彼劑量至患者之氣道。本發明之裝置為一吸入器,其包括一含納醫藥材料之室及一用於遞送該醫藥材料至患者之氣流道。各個該等元件之基本功能在上述共同擁有發明中描述。
根據本發明,多劑量醫藥材料儲存在組合之儲器與計量室之內,該室經設計以儲存多劑量醫藥材料,並以預定之劑量計量出該醫藥材料。
參閱第13圖,圖示了一吸入器,其包含一組合之儲器與計量室,該組合之儲器與計量室經設置以接受醫藥材料之供應,解聚集該醫藥材料並遞送該材料至氣流道340。該組合之儲器與計量室330包含一共振室,該共振室具有一可以選擇之頻率聲學共振的體積與形狀。該共振室及其優點在上文已詳細討論。
本發明之吸入器視需要可包含一腔室密封件336。該腔室密封件可為一塞子形式,當該裝置閒置時,該塞子防止空氣進入該組合之儲器與計量室。該塞子包括於吸入器中以進一步避免該醫藥材料在濕氣、氧及其他污染物中之不當暴露。諸如當該吸入器感測到患者吸入時,該腔室密封件將在來自該吸入器之授權信號下打開(見前述參考之共同擁有專利及申請案),並在劑量遞送後關閉。

本發明允許該組合之儲器與計量室藉由合成射流排出多劑量。藉由控制該頻率及各個啟動之持續時間將可電子控制該劑量之大小。因為剩餘藥物裝載將影響共振,例如藉由遵循以引用之方式併入本文之共同擁有美國專利申請案第12/246,208號之教示而感測該振動裝置之運動及電源並反饋控制遞送至該振動裝置之電力,各個劑量啟動較佳地以電子調整以提供一致之用於每劑量的藥物排出。
利用一設立作為用於排出多劑量之共振室的氣泡測試了本發明之一實例。一個4 mg之氣泡經裝載入該測試裝置並設置參數以允許每劑量大約1 mg之醫藥排出。對其他的劑量重複該操作。在啟動間去除並稱重該等氣泡。在下列表1及表2中,展示了藉由改變「開啟時間」(on-time)或啟動該振動裝置之持續時間,即使未最佳化振動頻率及樣式,亦可以足夠之預測性遞送劑量。
本發明之裝置容許修改。兩個或兩個以上組合之儲器與計量室可併入一用於遞送組合醫藥產品之單一吸入器。
本發明之另一態樣提供一遞送醫藥材料至可為人或動物之患者之氣道之方法。該方法提供一含納在一組合之儲器與計量室中之醫藥材料,該組合之儲器與計量室亦充當共振室。然後在該組合之儲器與計量室中振動解聚集該醫藥材料,以允許經由合成射流遞送單一劑量至患者。如上所述,解聚集該醫藥材料從而產生合成射流之步驟可藉由控制持續時間來完成,在該持續時間中,藉由一電源352對振動裝置344提供電力。該電源同時可能為一頻率控制源,用於進一步控制該合成射流之效果。
藉由與振動裝置耦接,該組合之儲器與計量室亦充當共振室,其中該解聚集醫藥材料之步驟涉及啟動該振動裝置以產生一合成射流從而經由一氣流道自該組合之儲器與計量室遞送該醫藥至患者的氣道。
本發明提供了獨特的、節省空間之設計及一機構,允許產生更小之遞送裝置,保存材料,賦能在單一封裝中包括增加數量之計量劑量的醫藥封裝;該機構允許遞送乾粉吸入劑至患者,而非當前藉由商業乾粉吸入器達成之遞送。
本文中所用之術語「醫藥」意欲包括所有適合於藉由吸入器遞送之藥物形式。舉例而言,因本發明對乾粉吸入器(DPIs)尤其有用,故該技術亦可用來增強吸入器之其他實施例。因此,涉及本發明之醫藥必然包括液態藥品以及乾粉藥品。
此外,該術語「醫藥」不應嚴格解釋而將其他諸如植物醫藥、維生素、激素、類固醇及其他生理活性小分子、肽、蛋白質等等之有用物質排除。

Claims (15)

  1. 一種醫藥遞送封裝,其含納一醫藥之複數個個別劑量,形成一匣體,該匣體之內容包含:該匣體含納一條帶,以一基材材料形成,該條帶形成一環並進一步包含一基材,該基材具有成一系列佈置並成型以含納一醫藥之該等個別劑量之複數個凹陷;及一箔、膜或帶,用於封裝該等凹陷,其中該等個別凹陷藉由該箔、膜或帶密封以形成複數個氣泡;一氣流道;一氣霧室,連接至該氣流道,其中該氣霧室與一振動裝置接合,其用於解聚集一醫藥之該個別劑量;及一推動裝置,其用於相對於該氣霧室推動該氣泡條帶;及一開啟裝置,其用於開啟鄰近於該氣霧室之一個別的氣泡,據此該醫藥之一個別劑量被遞送到該氣霧室。
