SNS-595 OG ANVENDELSE AV DET SAMME
SNS-595 er et hittil ukjent nafthydrin cytotoksisk forbindelse som tidligere var kjent som AG-73522 (se f.eks.Tsuzuki et al., Tetrahedron-Asymmetry 12: 2793-1799 (2001) og US patent nr. 5,817,699. Det kjemiske navnet til SNS-595 er (+)l,4-dihydro-7-[(sS,4S)-3-metoksy-4-(metylamino)-l-pyrrolidinyl]-4-okso-l-(2-tiazoyl)-l,8-nafthydrin-3-karbolsyre og har strukturen som vist nedenfor
Den foreliggende oppfinnelse vedrører farmasøytisk preparat, pulver, vandig farmasøytisk oppløsning og anvendelse av SNS-595 til behandling av kreft.

Som anvendt heri er et nummererisk område ment å være fullstendig. Egnede eksempler på en tonoserende forbindelse inkluderer aminosyrer, (f.eks. alanin og glysin), elektrolytter (f.eks. natriumklorid og kaliumklorid), monosaccarider (f.eks. glykose eller galaktose), disaccarider (f.eks. sukrose) og heksahydridiske alkoholer (f.eks. mannitol og sorbitol). I en utførelsesform omfatter sammensetningen rundt 100 mg av (+)-l,4-dihydro-7-[(3S,4S)-3-metoksy-4(metylamino)-l-pyrrolidinyl]-4-okso-l-(2-tiasolyl)-l,8-naftyridin-3-karboksylsyre og rundt 450 mg sorbitol per 10 ml av sammensetningen.
I en utførelsesform er pH i sammensetningen rundt 2,5 når målt i en vandig løsning.

I et annet aspekt av foreliggende oppfinnelse er det tilveiebrakt et pulver, som kjennetegnes ved at det i hovedsak består av sammensetningen som omfatter (+)-l,4-dihydro-7-[(3S,4S)-3-metoksy-4(metylamino)-l-pyrrolidinyl]-4-okso-l-(2-tiasolyl)-l,8-naftyridin-3-karboksylsyre og en syre, hvori sammensetningen har en pH mellom 2 til 3,5, når målt i en vandig løsning hvori (+)-l,4-dihydro-7-[(3S,4S)-3-metoksy-4(metylamino)-1 -pyrrolidinyl] -4-okso-1 -(2-tiasolyl)-1,8-naftyridin-3 -karboksylsyre er til stede i en mengde på 10 mg/ml.

I en utførelsesform omfatter sammensetningen som angitt over for anvendelse i behandling av kreft i en pasient som har behov for kreftbehandling rundt 100 mg av (+)-l,4-dihydro-7-[(3S,4S)-3-metoksy-4(metylamino)-l-pyrrolidinyl]-4-okso-l-(2-tiasolyl)-l,8-naftyridin-3-karboksylsyre og rundt 450 mg sorbitol per 10 ml av sammensetningen.

SNS-595 er en cytotoksisk forbindelse for behandling av kreft. De ulike typer av kreft som kan bli behandlet inkluderer, men er ikke begrenset til: blærekreft, brystkreft, cervical kreft, tarmkreft (inkludert kolorektal kreft), spiserørskreft, hode og nakkekreft, leukemi, leverkreft, lungekreft (både småcelle og ikke-småcelle), lymfoma, melanoma, myeloma, neuroblastoma, eggstokkreft, bukspyttkjertelkreft, prostatakreft, renal kreft, sarkoma (inkludert osteosarkoma), hudkreft (inkludert skjelettcellekarcinoma), magekreft, testikkelkreft, tyroidkreft, og livmorkreft.
SNS-595 kan anvendes for behandling av human kreft. Anvendelsen omfatter administrering til en pasient på basis av kroppsoverflatearealet, en dose på 10 mg/m<2->150 mg/m aav SNS-595. Kroppsoverflatearealkalkuleringer kan bli regnet ut f.eks. med Mostellerformelen hvori: BSA (m ) = kvadratroten av [(høyde (cm) x vekt (kg)/3600]. Dosen kan være 10 mg/m 2 -100 mg/m 2 . Den administrerte dose av SNS-595 kan bli levert simultant (f.eks. en enkelt injeksjon) eller over en 24-timers periode (f.eks. kontinuerlig infusjon over tid eller delte doser over tid) og er gjentatt frem til pasienten erfarer stabil sykdom eller regresjon, eller frem til pasienten erfarer sykdomsprogresjon eller ikke-akseptabel toksisitet. pasienten;

For eksempel hvis ventetiden var 6 dager så er startdosen av SNS-595 administrert på dag 1 (trinn i); venteperioden er 6 dager (trinn ii); og den etterfølgende dose av SNS-595 er administrert på dag 8 (trinn iii). Andre eksempler på tidsperioder inkluderer 2 dager, 3 dager, 13 dager, 20 dager og 27 dager. Venteperioden kan være minst 2 dager og trinn ii) til trinn iii) er gjentatt minst tre ganger. Behandling av en fast tumor kan omfatte:
i) administrering av en dose på 10 mg/m<2->100 mg/m<2>av SNS-595 til en pasient;
ii) vente i en periode på minst seks dager hvori personen ikke blir administrert noe
SNS-595;
iii) administrering av nok en dose av 10 mg/m 2 -100 mg/m 2 av SNS-595 til
pasienten; og
iv) repetere trinn ii)-iii) en rekke ganger.
Behandling av hematologisk kreft slik som leukemi og lymfoma kan omfatte:
i) administrering av en dose på 60 mg/m2-l 50 mg/m<2>av SNS-595 til en pasient;

Støttende behandling av pasienter som blir behandlet med SNS-595 kan omfatte:
a) administrering til en pasient en dose på 10 mg/m<2->150 mg/m<2>av SNS-595, og b) administrering av en terapeutisk effektiv mengde av en støttende
behandlingsforbindelse.
Den støttende behandlingsforbindelsen er enhver forbindelse som hindrer eller overkommer uheldige effekter fra SNS-595 behandling og er administrert ifølge det egnede doseringsregime for den forbindelsen. F.eks. har ulike støttende behandlingsforbindelser for behandling av kvalme ulike doseringsregimer. Illustrative eksempler på hematopoetiske forbindelser inkluderer, men er ikke begrenset til granulocyt kolonistimulerende faktor (G-CSF), granulocyt makrofag kolonistimulerende faktor (GM-CSF), erytropoietin og erytroposestimulerende protein, og derivater derav. Eksempler på G-CSF inkluderer men er ikke begrenset til filgrastim og det derivater inkludert pegfilgrastin. Et eksempel på GM-CSF inkluderer saegramostim. Et eksempel på erytropoetin er epoetin alfa. Et eksempel på erytropoiesestimulerende protein er darbepoetin alfa.

EKSEMPEL 1
Farmasøytisk sammensetning egnet for injeksjon eller intravenøs infusjon
Sure sammensetninger (< pH 4) tilveiebrakte hensiktsmessig balanse av økt løselighet av SNS-595 og ønskede farmasøytiske egenskaper (f.eks. økt pasientvelvære ved å medføre mindre irritasjon omkring injeksjonsområdet). Et illustrativ eksempel på en egnet sammensetning omfatter: 10 mg SNS-595 per ml av vandig løsning av 4,5% sorbitol som er justert til pH 2,5 med metansulfonsyre. En protokoll for å lage en slik løsning inkluderer følgende for å lage en 100 mg/10 ml presentasjon: 100 mg av SNS-595 og 450 mg D-sorbitol er tilsatt til destillert vann; volumet er justert opp til et volum på 10 ml; pH i den resulterende løsningen er justert til 2,5 med metansulfonsyre. EKSEMPEL 2
Farmakokinetikk av SNS- 595 i kreftpasienter
SNS-595 ble administrert til innskrevne pasienter i opptil seks sykluser. En syklus er definert som en tre ukers periode hvor SNS-595 er administrert på den første dagen i hver syklus (dag 0), fulgt av minst 21 dager ved observasjon. SNS-595 ble administrert til grupper på minst 3 pasienter og doseeskalering inntraff ved sekvensiell gruppe. Doser av SNS-595 var lineær med AUC <x> og dets farmakokinetikkegenskaper var uvanlig konsekvent blant pasienter i samme gruppe. Figur 1 viser plasmakonsentrasjoner av SNS-595 over tid blant de ulike pasientgrupper og tabell 1 viser farmakokinetikkparameterne avledet derfra.

Farmasøytisk sammensetning,karakterisert vedat den omfatter (+)-l,4-dihydro-7-[(3S,4S)-3-metoksy-4(metylamino)-l-pyrrolidinyl]-4-okso-l-(2-tiasolyl)-l,8-naftyridin-3-karboksylsyre og en syre, hvori sammensetningen har en pH mellom 2 til 3,5, når målt i en vandig løsning hvori (+)-l,4-dihydro-7-[(3S,4S)-3-metoksy-4(metylamino)-l-pyrrolidinyl]-4-okso-l-(2-tiasolyl)-l,8-naftyridin-3-karboksylsyre er til stede i en mengde på 10 mg/ml.

Farmasøytisk sammensetning ifølge krav 1,karakterisertv e d at syren er valgt fra eddiksyre, askorbinsyre, benzensulfonsyre, metansulfonsyre, etansulfonsyre, glykolsyre, saltsyre, hydrogenbromid, hydroksyetansulfonsyre, melkesyre, maleinsyre, propionsyre, ravsyre, svovelsyre, trifluoreddiksyre og toluensulfonsyre.
