Description
Foreliggende oppfinnelse vedrører SNS-595 for anvendelse i en metode for behandling av leukemi. SNS-595 er et hittil ukjent nafthydrin cytotoksisk forbindelse som tidligere var kjent som AG-73522 (se f.eks.Tsuzuki et al., Tetrahedron-Asymmetry 12: 2793-1799 (2001) og US patent nr. 5,817,669. Det kjemiske navnet til SNS-595 er (+)1,4-dihydro-7-[(3S,4S)-3-metoksy-4-(metylamino)-l-pyrrolidinyl]-4-okso-l-(2-tiazoyl)-l,8-nafthydrin-3-karboksylsyre og har strukturen som vist nedenfor
Den foreliggende oppfinnelse vedrører SNS-595 for anvendelse i en metode for behandling av leukemi i en pasient, hvori metoden omfatter å administrere forbindelsen til pasienten i en ukentlig dose på 60 mg/m<2->150 mg/m<2>.
Figur 1 viser plasmakonsentrasjonene til SNS-595 over tid blant de ulike pasientgrupper.
I søknaden beskrives det et farmasøytisk preparat omfattende:
a) SNS-595, og
b) en syre
i en vandig løsning hvor pH i løsningen er 2-3,5. Som anvendt heri er et nummererisk
område ment å være fullstendig. F.eks. inkluderer området med pH 2-3,5 både pH 2 og pH 3,5.1 en utførelsesform er pH i sammensetningen 2-3.1 en annen utførelsesform er pH i sammensetningen 2,3-2,7. Som anvendt heri er en vandig løsning en væske omfattende vann.
Ifølge oppfinnelsen kan SNS-595 administreres i form av en slik farmasøytisk sammensetning.
Egnede eksempler på syrer inkluderer både organiske og uorganiske syrer slik som eddiksyre, askorbinsyre, benzen sulfonsyre, etansulfonsyre, glykolsyre, saltsyre, hydrogenklorid, hydrogenbromid, hydroksyetansulfonsyre, melkesyre, maleinsyre, metansulfonsyre, proprionsyre, ravsyre, svovelsyre, trifluoreddiksyre, og toluensulfonsyre. I en utførelsesform er syren saltsyre, metansulfonsyre eller melkesyre. I en annen utførelsesform er syren metansulfonsyre.
Det farmasøytiske preparatet kan videre omfatte en tonisiterende forbindelse. Egnede eksempler på en tonisiterende forbindelse inkluderer aminosyrer, (f.eks. alanin og glysin), elektrolytter (f. eks. natriumklorid og kaliumklorid), monosakkarider (feks. glykose eller galaktose), disakkarider (feks. sukrose) og heksahydridiske alkoholer (feks. mannitol og sorbitol). I en annen utførelsesform er den tonisiterende forbindelse natriumklorid, glykose, manitol eller sorbitol. I en annen utførelsesform er den tonisiterende forbindelse en heksahydrisk alkohol. I en annen utførelsesform er den tonisiterende forbindelsen sorbitol.
SNS-595 er en cytotoksisk forbindelse for behandling av kreft. De ulike typer av kreft som kan bli behandlet ved anvendelse av SNS-595 inkluderer, men er ikke begrenset til: blærekreft, brystkreft, cervical kreft, tarmkreft (inkludert kolorektal kreft), spiserørskreft, hode og nakkekreft, leukemi, leverkreft, lungekreft (både småcelle og ikke-småcelle), lymfoma, melanoma, myeloma, neuroblastoma, eggstokkreft, bukspyttkjertelkreft, prostatakreft, renal kreft, sarkoma (inkludert osteosarkoma), hudkreft (inkludert skjelettcellekarcinoma), magekreft, testikkelkreft, tyroidkreft, og livmorkreft.
Oppfinnelsen tilveiebringer en metode for anvendelse av SNS-595 for behandling av leikemi. Metoden omfatter administrering til en pasient på basis av kroppsoverflatearealet, en dose på 60 mg/m<2->150 mg/m<2>av SNS-595. Kroppsoverflatearealkalkuleringer kan bli regnet ut feks. med Mostellerformelen hvori:

Den administrerte dose av SNS-595 kan bli levert simultant (feks. en enkelt bolusinjeksjon) eller over en 24-timers periode (feks. kontinuerlig infusjon over tid eller delte bolusdoser over tid) og er gjentatt frem til pasienten erfarer stabil sykdom eller regresjon, eller frem til pasienten erfarer sykdomsprogresjon eller ikke-akseptabel toksisitet. F. eks. betyr stabil sykdom for faste tumorer generelt at perpendikulære diameter av målbar skade ikke har økt med 25% eller mer fra sist måling. Se feks. Administrasjonsdosen av SNS-595 kan bli uttrykt i enheter andre enn mg/m<2>. F.eks. kan doser bli uttrykt som mg/kg. En fagmann vil lett vite hvordan doser skal konverteres fra mg/m<2>til mg/kg gitt enten høyden eller vekten til en person eller begge (se feks. http :/// www. f da. gov/ eder/ cancer/ animalframe. htm). F. eks. er en dose på 10 mg/m<2->150 mg/m<2>for en person på 65 kg omtrent tilsvarende til 0,26 mg/kg-3,95 mg/kg. I et annet aspekt av foreliggende oppfinnelse er SNS-595 administrert ifølge en doseringsplan. I en utførelsesform omfatter metoden: i) administrering av en ukentlig dose på 60 mg/m<2->150 mg/m2 av SNS-595 til
en pasient;
ii) awenting i en periode på minst seks dager hvor personen ikke blir
administrert noe SNS-595;
iii) administrering av nok en ukentlig dose av 60 mg/m<2->150 mg/m<2>av SNS-595
til pasienten;
og-
gjenta trinn ii)-iii) en rekke ganger.
For eksempel hvis ventetiden var 6 dager så er startdosen av SNS-595 administrert på dag 1 (trinn i); venteperioden er 6 dager (trinn ii); og den etterfølgende dose av SNS-595 er administrert på dag 8 (trinn iii). Andre eksempler på tidsperioder inkluderer 13 dager, 20 dager og 27 dager. I en annen utførelsesform er venteperioden minst 6 dager og trinn ii) til iii) er gjentatt minst tre ganger. I en annen utførelsesform er venteperioden minst 6 dager og trinn ii) til iii) er gjentatt minst fem ganger. I en annen utførelsesform omfatter doseringsmetoden administrering av en dose av SNS-595 til en person hver annen uke. I en annen utførelsesform omfatter doseringsmetoden administrering av en dose av SNS-595 til en person hver tredje uke. I en annen utførelsesform omfatter doseringsmetoden administrering av en dose av SNS-595 til en person hver fjerde uke. I en annen utførelsesform omfatter doseringsmetoden en syklus hvori syklusen omfatter administrering av en dose av SNS-595 til en person hver uke i tre uker etterfulgt av en periode på minst to uker hvor ikke noe SNS-595 er administrert til nevnte person og hvori nevnte syklus er repetert en rekke ganger. I en annen utførelsesform er perioden hvor ikke noe SNS-595 blir administrert to uker. I en annen utførelsesform er perioden hvor det ikke blir administrert SNS-595 tre uker.
Oppfinnelsen tilveiebringer således SNS-595 for anvendelse i en metode for å behandle en hematologisk kreft slik som leukemi og lymfoma. Metoden omfatter;
i) administrering av en ukentlig dose på 60 mg/m2-l 50 mg/m<2>av SNS-595 til en pasient;
ii) vente en periode på minst seks dager hvori personen ikke blir administrert noe
SNS-595;
iii) administrering av nok en dose på 60 mg/m<2->150 mg/m<2>av SNS-595 til
pasienten, og
iv) repetere trinn ii)-iii) en rekke ganger.
I et annet aspekt av foreliggende oppfinnelse er det tilveiebrakt en metode for støttende behandling av pasienter som blir behandlet med SNS-595. Metoden omfatter: a) administrering til en pasient en ukentlig dose på 60 mg/m<2->150 mg/m<2>av SNS-595, og b) administrering av en terapeutisk effektiv mengde av en støttende behandlingsforbindelse.
Den støttende behandlingsforbindelsen er enhver forbindelse som hindrer eller overkommer uheldige effekter fra SNS-595 behandling og er administrert ifølge det egnede doseringsregime for den forbindelsen. F. eks. har ulike støttende behandlingsforbindelser for behandling av kvalme ulike doseringsregimer. Mens noen er administrert profylaktisk, er andre ko-administrert med SNS-595 mens andre er administrert etter administrasjon av SNS-595. Illustrative eksempler på antiemetika inkluderer men er ikke begrenset til fenotiaziner, butyrofenoner, benzodiazipiner, kortikosteroider, serotonin antagonister, cannabinoider, og NKireseptor antagonister. Eksempler på fenotiazin antiemetika inkluderer proklorperazin og trimetobenzamid. Et eksempel på et butyrofenon antiemetika er haloperidol. Et eksempel på et benzodiazapin antiemetika er lorazepam. Et eksempel på et kortikostereoid antiemetika er deksametason. Eksempler på et serotonin antagonisk antiemetika inkluderer ondansetron, granisetron og dolasetron. Et eksempel på et cannabinoid antiemetika er dronabinol Et eksempel på NKireseptor antagonist er aprepitant.

Illutstrative eksempler på hematopoetiske forbindelser inkluderer men er ikke begrenset til granulocyt kolonistimulerende faktor (G-CSF), granulocyt makrofag kolonistimulerende faktor (GM-CSF), erytropoietin og erytroposestimulerende protein, og derivater derav. Eksempler på G-CSF inkluderer men er ikke begrenset til filgrastim og det derivater inkludert pegfilgrastin. Et eksempel på GM-CSF inkluderer saegramostim. EKSEMPEL 1
Farmasøytisk sammensetning egnet for injeksjon eller intravenøs infusjon
Sure sammensetninger (< pH 4) tilveiebrakte hensiktsmessig balanse av økt løselighet av SNS-595 og ønskede farmasøytiske egenskaper (feks. økt pasientvelvære ved å medføre mindre irritasjon omkring injeksjonsområdet). Et illustrativ eksempel på en egnet sammensetning omfatter: 10 mg SNS-595 per ml av vandig løsning av 4,5% sorbitol som er justert til pH 2,5 med metansulfonsyre. En protokoll for å lage en slik løsning inkluderer følgende for å lage en 100 mg/10 ml presentasjon: 100 mg av SNS-595 og 450 mg D-sorbitol er tilsatt til destillert vann; volumet er justert opp til et volum på 10 ml; pH i den resulterende løsningen er justert til 2,5 med metansulfonsyre. Den resulterende sammensetningen er også egnet for lyofilisering. Den lyofiliserte formen er så rekonstituert med sterilt vann til passende konsentrasjon før bruk.
EKSEMPEL 2
Farmakokinetikk av SNS- 595 i kreftpasienter
SNS-595 ble administrert til innskrevne pasienter i opptil seks sykluser. En syklus er definert som en tre ukers periode hvor SNS-595 er administrert på den første dagen i hver syklus (dag 0), fulgt av minst 21 dager ved observasjon. SNS-595 ble administrert til grupper på minst 3 pasienter og doseeskalering inntraff ved sekvensiell gruppe. Doser av SNS-595 var lineær med AUC oo og dets farmakokinetikkegenskaper var uvanlig konsekvent blant pasienter i samme gruppe. Figur 1 viser plasmakonsentrasjoner av SNS-595 over tid blant de ulike pasientgrupper og tabell 1 viser farmakokinetikkparameterne avledet derfra.
Claims (13)
1.
Forbindelse som er (+)-l,4-dihydro-7-[(3S,4S)-3-metoksy-4(metylamino)-l-pyrrolidinyl]-4-okso-l-(2-tiasolyl)-l,8-naftyridin-3-karboksylsyre for anvendelse i en metode for behandling av leukemi i en pasient, hvori metoden omfatter å administrere forbindelsen til pasienten i en ukentlig dose på 60 mg/m2-150 mg/m2.

Forbindelse for anvendelse ifølge krav 1, hvori metoden omfatter å administrere forbindelsen til pasienten i kombinasjon med en støttebehandlingsforbindelse valgt fra gruppen bestående av fenotiazin, butyrofenon, benzodiazipin, kortikosteroid, serotoninantagonist, cannabinoid, NK reseptor antagonist, granulocyt kolonistimulerende faktor, granulocyt makrofag kolonistimulerende faktor, erytropoietin, erytroposestimulerende protein, darbepoetin alfa.
3.
Forbindelse for anvendelse ifølge krav 1, hvori metoden omfatter: i) administrering av en ukentlig dose av forbindelsen på 60 mg/m<2->150 mg/m<2>; ii) vente en periode på minst seks dager hvor forbindelsen ikke blir administrert; iii) administrering av nok en ukentlig dose av forbindelsen på 60 mg/m2-l 50 mg/m<2>;
og iv) repetere trinn ii) til iii) en rekke ganger.
4.
Forbindelse for anvendelse ifølge krav 3, hvori venteperioden er minst 13 dager, minst 20 dager eller minst 27 dager.

Forbindelse for anvendelse ifølge krav 3, hvori venteperioden er minst 6 dager og trinn ii) til iii) repeteres minst tre, eller minst fem ganger.

Forbindelse for anvendelse ifølge krav 3, hvori venteperioden er minst 20 dager og trinn ii) til iii) repeteres minst tre, eller minst fem ganger.

Forbindelse for anvendelse ifølge krav 3, hvori venteperioden er minst 27 dager og trinn ii) til iii) repeteres minst tre, eller minst fem ganger.
8.
Forbindelse for anvendelse ifølge krav 1, hvori metoden omfatter å administrere den ukentlige dosen på 60 mg/m<2->150 mg/m<2>hver uke, hver andre uke, hver tredje uke eller hver fjerde uke.
9.
Forbindelse for anvendelse ifølge krav 1, hvori metoden omfatter en syklus, hvori syklusen omfatter administrering av dosen på 60 mg/m<2->150 mg/m<2>hver uke i tre uker etterfulgt av en periode på minst to uker hvor forbindelse ikke blir administrert og hvori syklusen er repetert en rekke ganger.

Forbindelse for anvendelse ifølge krav 1, hvori metoden omfatter å administrere forbindelsen i form av en farmasøytisk sammensetning omfattende forbindelsen og en syre i en vandig løsning, hvori løsningens pH er 2 - 3,5.

Forbindelse for anvendelse ifølge krav 11, hvori syren er eddiksyre, askorbinsyre, benzensulfonsyre, etansulfonsyre, glykolsyre, saltsyre, hydrogenbromid, hydroksyetansulfonsyre, melkesyre, maleinsyre, metansulfonsyre, propionsyre, ravsyre, svovelsyre, trifluoreddiksyre eller toluensulfonsyre.
