Description
SNS-595는 AG-7352로서 이미 알려져 있는 신규 나프티리딘 세포독성제이다 (문헌 [Tsuzuki et al., Tetrahedron-Asymmetry 12: 1793-1799 (2001)] 및 미국 특허 제5,817,669호 참조). SNS-595은 화학명이 (+)-1,4-디히드로-7-[(3S,4S)-3-메톡시-4-(메틸아미노)-1-피롤리디닐]-4-옥소-1-(2-티아졸릴)-1,8-나프티리딘-3-카르복실산이고, 하기 구조를 갖는다.
본 발명은 SNS-595를 사용하여 암을 치료하기 위한 제약 조성물 및 방법에 관한 것이다.

본 발명의 다른 측면에서, 백혈병 및 림프종과 같은 혈액암의 치료 방법이 제공된다. 방법은 i) SNS-595를 60 ㎎/㎡ 내지 150 ㎎/㎡의 용량으로 환자에게 투여하고, ii) SNS-595를 투여하지 않는 2일 이상의 기간 동안 대기하고, iii) SNS-595를 60 ㎎/㎡ 내지 150 ㎎/㎡의 추가의 용량으로 상기 환자에게 투여하고, iv) 단계 ii) 내지 iii)를 수회 반복하는 것을 포함한다.
본 발명의 다른 측면에서, SNS-595로 치료받는 환자를 보조 치료하는 방법이 제공된다. 방법은 a) SNS-595를 10 ㎎/㎡ 내지 150 ㎎/㎡의 용량으로 환자에게 투여하고, b) 치료 유효량의 보조 치료제를 투여하는 것을 포함한다.
보조 치료제는 SNS-595 치료로부터의 부작용을 예방 또는 관리하는 임의의 물질이고, 상기 물질에 대한 적절한 투여 요법에 따라서 투여된다. 예를 들어, 오심을 치료하기 위한 상이한 보조 치료제는 상이한 투여 요법을 갖는다. 몇몇은 예방적으로 투여되는 반면, 다른 것들은 SNS-595와 함께 투여되며, 또다른 것들은 SNS-595 투여 후에 투여된다. 보조 치료제의 용량 및 투여 요법의 예시적 예는 문헌 [The Physician's Desk Reference]에서 찾아볼 수 있다.

실시예 1
주사 또는 정맥내 주입에 적합한 제약 조성물
산성 조성물 (< pH 4)은 SNS-595의 증가된 용해도 및 목적하는 제약 특성 (예를 들어, 전달 부위에서 자극을 덜 일으킴으로써 환자의 편안함을 증가시킴)의 적절한 균형을 제공하였다. 적합한 조성물의 예시적 예는 메탄술폰산에 의해 pH가 2.5로 조정된, 4.5% 소르비톨 수용액 ㎖ 당 SNS-595 10㎎을 포함한다. 상기 용액 100 ㎎/10 ㎖의 제조를 위한 하나의 프로토콜은 SNS-595 100 ㎎ 및 D-소르비톨 450 ㎎을 증류수에 첨가하고, 부피를 10 ㎖로 맞추고, 얻어진 용액의 pH를 메탄술폰산에 의해 2.5로 조정하는 것을 포함한다. 얻어진 조성물은 동결건조에도 적합하다. 그 후, 동결건조된 형태를 사용하기 전에 적절한 농도로 무균수로 재구성하였다.
실시예 2
암 환자에서의 SNS-595의 약동학
SNS-595를 등록된 환자에게 6 주기 이하 동안 투여하였다. 1 주기는 3주 기간으로 정의하고, 각 주기의 제1일째에 SNS-595를 투여한 다음 21일 이상 동안 관찰하였다. SNS-595를 3명 이상의 환자의 실험군에게 투여하고, 순차적 실험군에 용량을 단계적으로 증가시켰다. SNS-595의 용량은 AUC∞에 대하여 선형이고, 그의 약동학적 특성은 동일한 실험군에서 환자들 사이에서 현저하게 일치하였다. 도 1은 여러 환자 실험군에서의 시간에 따른 SNS-595의 혈장 농도를 나타내고, 표 1은 그로부터 유도된 약동학 파라미터를 나타낸다.
Claims (30)
- 수용액 중 (+)-1,4-디히드로-7-[(3S,4S)-3-메톡시-4-(메틸아미노)-1-피롤리디닐]-4-옥소-1-(2-티아졸릴)-1,8-나프티리딘-3-카르복실산 및 산을 포함하고, pH가 2 내지 3.5인 암 치료용 제약 조성물.
- 제1항에 있어서, pH가 2 내지 3인 제약 조성물.
- 제1항에 있어서, pH가 2.3 내지 2.7인 제약 조성물.
- 제1항에 있어서, 산이 유기산 또는 무기산인 제약 조성물.
- 제4항에 있어서, 산이 아세트산, 아스코르브산, 벤젠술폰산, 에탄술폰산, 글리콜산, 히드록시에탄술폰산, 락트산, 말레산, 메탄술폰산, 프로피온산, 숙신산, 트리플루오로아세트산 및 톨루엔술폰산으로부터 선택된 유기산인 제약 조성물.
- 제5항에 있어서, 유기산이 메탄술폰산인 제약 조성물.
- 제4항에 있어서, 산이 브롬화수소 및 황산으로부터 선택된 무기산인 제약 조성물.
- 제1항에 있어서, 등장화제(tonicity agent)를 더 포함하는 제약 조성물.
- 제8항에 있어서, 등장화제가 아미노산, 전해질, 단당류, 이당류 및 6가 알콜로부터 선택되는 것인 제약 조성물.
- 제8항에 있어서, 등장화제가 소르비톨인 제약 조성물.
- 제1항에 있어서, (+)-1,4-디히드로-7-[(3S,4S)-3-메톡시-4-(메틸아미노)-1-피롤리디닐]-4-옥소-1-(2-티아졸릴)-1,8-나프티리딘-3-카르복실산이 10 ㎎/㎖의 양으로 존재하고, 소르비톨이 조성물의 총 부피를 기준으로 4.5%의 양으로 존재하며, 조성물의 pH를 2.5로 조정하기 위해 충분한 양의 메탄술폰산이 존재하는 것인 제약 조성물.
- 제1항에 있어서, 조성물 10 ㎖ 당 (+)-1,4-디히드로-7-[(3S,4S)-3-메톡시-4-(메틸아미노)-1-피롤리디닐]-4-옥소-1-(2-티아졸릴)-1,8-나프티리딘-3-카르복실산 100 ㎎ 및 소르비톨 450 ㎎, 및 조성물의 pH를 2.5로 조정하기 위한 충분한 양의 메탄술폰산을 포함하는 제약 조성물.
- 제1항 내지 제12항 중 어느 한 항에 있어서, 주사 또는 정맥내 주입을 위해 제형화된 제약 조성물.
- 제1항 내지 제12항 중 어느 한 항의 제약 조성물의 동결건조된 형태.
- 제14항에 있어서, 무균수로 재구성 시에 pH 2 내지 3.5인 조성물을 생성시키는 동결건조된 형태.
- 제1항에 있어서, 난소암, 유방암, 또는 백혈병 치료용 제약 조성물.
- (+)-1,4-디히드로-7-[(3S,4S)-3-메톡시-4-(메틸아미노)-1-피롤리디닐]-4-옥소-1-(2-티아졸릴)-1,8-나프티리딘-3-카르복실산 화합물을 포함하고, 상기 화합물은 60 ㎎/㎡ 내지 110 ㎎/㎡의 용량으로 투여되는 것인, 백혈병 치료용 제약 조성물.
- 제17항에 있어서, 화합물이 하기와 같은 단계로 투여되는 것인 제약 조성물:i) 60 ㎎/㎡ 내지 110 ㎎/㎡의 용량으로 투여하는 단계;ii) 화합물을 투여하지 않는 1, 2, 3, 6, 13, 20 또는 27일의 기간 동안 대기하는 단계;iii) 60 ㎎/㎡ 내지 110 ㎎/㎡의 추가의 용량으로 투여하는 단계; 및iv) 단계 ii) 내지 iii)를 수회 반복하는 단계.
- 제17항에 있어서, 60 ㎎/㎡ 내지 110 ㎎/㎡의 용량이 주단위 용량인 제약 조성물.
- 제17항에 있어서, 60 ㎎/㎡ 내지 110 ㎎/㎡의 용량이 3주 마다 투여되는 것인 제약 조성물.
- 제17항에 있어서, 화합물이 주기적으로 투여되며, 상기 주기는 매주 60 ㎎/㎡ 내지 110 ㎎/㎡의 용량으로 3주 동안 투여된 다음 2주의 기간 동안 투여되지 않는 것을 포함하고, 이 주기가 수회 반복되는 것인 제약 조성물.
- 제17항에 있어서, 용량이 60 ㎎/㎡ 내지 90 ㎎/㎡, 90 ㎎/㎡ 내지 100 ㎎/㎡, 또는 100 ㎎/㎡ 내지 110 ㎎/㎡인 제약 조성물.
