Abstract
Bisfosfonater, i særdeleshet mer potente N-bisfosfonater slik som zoledronsyre og derivater, kan med tilfredstillende resultater anvendes for forlenget inhibering av benresorpsjon ved tilstander med abnormt økt benomsetning (bone turnover), for eksempel osteoporose, ved periodevis administrering, der periodene mellom bisfosfonatadministreringene er lengre enn hva som er tidligere ble betraktet som passende, for eksempel et doseringsintervall på minst omtrent 6 måneder eller sjeldnere.
Description
Denne oppfinnelsen vedrører zoledronsyre som er et bisfosfonat, i særdeleshet den farmasøytiske anvendelsen av zoledronsyre ved behandling av tilstander av abnormt økt benomsetning ("bone turnover"), slik som osteoporose.
Bisfosfonater er benyttet i bred utstrekning for å hemme osteoklastaktivitet i et mangfold av både godartede og ondartede sykdommer der benresorpsjon økes. Således har bisfosfonater nylig blitt tilgjengelige for langtidsbehandling av pasienter med multiple myelom (MM). Mekanismene hvorved bisfosfonater hemmer benresorpsjon er fremdeles dårlig forstått og ser ut til å variere i henhold til bisfosfonatene som er studert. Bisfosfonater er vist å binde seg sterkt til hydroksyappatittkrystallene i ben, og redusere benomsetning og resorpsjon, å senke nivåene av hydroksypyrolin eller alkalisk fosfatase i blodet, og i tillegg å hemme både aktiveringen og aktiviteten av osteoklaster.
I tillegg er bisfosfonater foreslått for anvendelse ved behandling av osteoporose. Således har en fremgangsmåte for eksempel som beskrevet i US 4,812,304 (Procter & Gamble) blitt foreslått for behandling eller forebygging av osteoporose hos mennesker som omfatter å administrere en bencelleaktiverende forbindelse og et benresorpsjonshemmende polyfosfonat til en pasient som lider av eller som har risiko for å få osteoporose, i henhold til et regime som består av en eller flere sykluser, hvorved hver syklus består av: (a) en benaktiverende periode på fra omtrent 1 dag til omtrent 5 dager hvorunder en bencelleaktiverende mengde av en bencelleaktiverende forbindelse administreres til pasienten; etterfulgt av (b) en benresorpsjonshemmende periode på fra omtrent 10 dager til omtrent 20 dager hvorunder etan-l-hydroksy-l,l-difosfonsyre, eller et farmasøytisk akseptabelt salt eller ester derav, daglig administreres til pasienten i en mengde på fra omtrent 0,25 mgP/kg/dag til omtrent 3,3 mgP/kg/dag; etterfulgt av (c) en gjenværende periode på fra omtrent 70 dager til omtrent 180 dager hvor en pasienten verken mottar en bencelleaktiverende forbindelse eller et benresorpsjonshemmende polyfosfonat.

Vi har nå overraskende funnet at bisfosfonater, i særdeleshet mer potente nitrogenholdige bisfosfonater, kan anvendes for forlenget hemming av benresorpsjon ved tilstander av abnormt økt benomsetning ved periodevis tilbakevendende administrering, der periodene mellom bisfosfonatadministrasjonene er lengre enn hva som er tidligere vurdert som passende for å oppnå tilfredsstillende behandling. I særdeleshet og i motsetning til hva som var forventet har vi funnet at tilfredsstillende behandlingsresultater kan oppnås selv når doseringsintervallene i betydelig grad overskrider den naturlige benremoduleringssyklusen.
Følgelig tilveiebringer den foreliggende oppfinnelse anvendelse av zoledronsyre, eller et farmasøytisk akseptabelt salt derav, eller et hvilket som helst hydrat derav til fremstilling av et medikament for behandling av tilstander med abnormt økt benomsetning (bone turnover), der zoledronsyren, eller et farmasøytisk akseptabelt salt derav, eller et hvilket som helst hydrat derav administreres periodevis og der perioden mellom administreringene er på minst omtrent 6 måneder.
Oppfinnelsen tilveiebringer videre zoledronsyre eller et farmasøytisk akseptabelt salt derav, eller et hvilket som helst hydrat derav for behandling av tilstander med abnormt økt benomsetning (bone turnover), der zoledronsyren, eller et farmasøytisk akseptabelt salt derav, eller et hvilket som helst hydrat derav administreres periodevis og der perioden mellom administreringene er på minst omtrent ett år.
Tilstander av abnormt økt benomsetning som kan behandles ifølge den foreliggende oppfinnelsen inkluderer: behandling av postmenopause osteoporose, for eksempel for å redusere risikoen for osteoporotiske brudd; forebygging av postmenopause osteoporose, for eksempel forebygging av postmenopause bentap; behandling eller forebygging av mannlig osteoporose; behandling eller forebygging av kortikosteroidindusert osteoporose og andre former for bentap sekundært eller på grunn av medisinering, for eksempel difenylhydantion, tyroidhorrnonerstatningsterapi; behandling eller forebygging av bentap forbundet med immobilisering og romreise; behandling eller forebygging av bentap forbundet med reumatoid artritt, ostogenese imperfekta, hypertyroidisme, anoreksia nevrosa, organtransplantasjon, leddproteseløsning, og andre medisinske tilstander. For eksempel kan slike andre medisinske tilstander inkludere behandling eller forebygging av periartikulære benarisjoner ved reumatoid artritt; behandling av osteoartritt, for eksempel forebygging/behandling av subkondral osteosklerose, subkondrale bensyster, osteopytdannelse og osteoartritt smerte, for eksempel ved reduksjon i det indre bentrykket; behandling eller forebygging av hyperklasemi som kommer av overdreven benresorpsjon sekundært til hyperparatyriodisme, tyrotoksikose, sarkoidose eller hypervitaminose D.
Således henviser betegnelsene "behandling" eller "behandle" i den foreliggende beskriv-elsen til både profylaktisk eller preventiv behandling så vel som kurerende eller sykdomsmodifiserende behandling, inkludert behandling av pasienter som er i risikogruppen for å trekke til seg sykdommen eller forventes å ha trukket til seg sykdommen så vel som pasienter som er syke eller som har fått diagnosen som er at de lider av en sykdom eller medisinsk tilstand. I særdeleshet kan foretrukne utførelsesformer anvendes for profylaktisk behandling av osteoporose og lignende sykdommer. Således kan for eksempel zoledronsyre administreres til individer som er i risikogruppen for å utvikle osteoporose på en regulær basis ved doseringsintervaller på minst omtrent 6 måneder, for eksempel zoledronsyre kan administreres rutinemessig til postmenopause kvinner ved doseringsintervaller på en gang hver 6. måned eller mindre hyppig, for eksempel årlig.
Ifølge den foreliggende oppfinnelsen er zoledronsyredoseirngsintervallet minst omtrent 6 måneder, for eksempel en gang pr. 180 dag, eller mindre hyppig, passende en gang i
året, eller ved et hvilke som helst intervall mellom omtrent for eksempel 1 gang hver 7, 8,9,10 eller 11 måned. Doseringsintervaller på mer enn 1 gang pr. år kan benyttes, for eksempel omtrent 1 gang hver 18. måned eller omtrent 1 gang hvert andre år eller enda sjeldnere, for eksempel en frekvens på opptil omtrent 1 gang pr. 3 år eller sjeldnere.
N-bisfosfonatet for anvendelse ved oppfinnelsen er 2-(imidazol-lyl)-l-hydroksyetan-1,1-difosfonsyre (zoledronsyre) eller et farmakologisk akseptabelt salt derav. Farmakologisk akseptable salter er fortrinnsvis salter med baser, passende metallsalter avledet fra gruppene Ia, Ib, Ila og Ilb i det periodiske system, inkludert alkalimetallsalter, for eksempel kalium- og særlig natriumsalter, eller jordalkalimetallsalter, fortrinnsvis kalsium- eller magnesiumsalter, og også ammoniumsalter med ammoniakk eller organiske aminer.
Særlig foretrukne farmasøytiske akseptable salter er de der en, to, tre eller fire, i særdeleshet en eller to, av de sure hydrogenene i bisfosfonsyren er erstattet av et farmasøy-tisk akseptabelt kation, i særdeleshet natrium, kalium eller ammonium, først og fremst natrium.
En svært foretrukken gruppe av farmaøsytiske akseptable salter erkarakterisert vedat de har et surt hydrogen og et farmasøytisk akseptabelt kation, særlig natrium, i hvert tilfelle av fosfonsyregruppene.
Bisfosfonatet (heretter referert til som midlene ifølge oppfinnelsen) kan benyttes på formen av en isomer eller av en blanding av isomere der det er passende, typisk som optiske isomerer slik som enantiomere eller diasteromere eller geometriske isomere, typisk cis-transisomere. De optiske isomerene oppnås på formen av de rene antipodene og/eller som racemater.
Midlene ifølge oppfinnelsen kan også benyttes på formen av deres hydrater eller inkluderer andre løsningsmidler som benyttes for deres krystallisering.
Midlene ifølge oppfinnelsen benyttes fortrinnsvis på formen av farmasøytiske sammensetninger som inneholder en terapeutisk effektiv mengde av en aktiv ingrediens eventuelt sammen med eller i blanding med uorganiske eller organiske, faste eller flytende, farmasøytiske akseptable bærere som er egnet for administrering.
Midlene ifølge oppfinnelsen kan administreres alene eller i kombinasjon med andre benaktive legemidler, enten i ordnede kombinasjoner eller separat både fysisk og i tid, inkludert hormoner, slik som steoridehormoner, for eksempel et østrogen; en delvis østrogenagonist, eller østrogen- gestagenkombinasjon; et calcitonin eller en analog eller derivat derav, for eksempel laks, ål eller humant calcitonin parathyroidhormon eller analoger derav, for eksempel PTH (1-84), PTH (1-34), PTH (1-36), PTH (1-38), PTH
(1-31)NH2eller PTS 893 ;en SERM (Selective Estrogen Receptor Modulator) for eksempel raloksifen, lasofoksifen, TSE-424, FC1271, Tibolone (Livial®); vitamin D eller analog. Slike tilleggsbeinaktive legemidler kan administreres hyppigere enn bisfosfonatet.
De farmasøytiske sammensetningene kan for eksempel være sammensetninger for enteral, slik som oral, rektal, aerosol inhalering eller nasaladministrering, sammensetninger for parenteral, slik som intravenøs eller subkutan administrering, eller sammensetninger for transdermal administrering (f. eks. passiv eller ionoforetisk).
Fortrinnsvis er de farmasøytiske sammensetningene tilpasset oral eller parenteral (særlig intravenøs, subkutan, intramuskulær eller transdermal) administrering. Intravenøs og oral, først og fremst intravenøs administrering, er ansett som å ha spesiell viktighet. Fortrinnsvis er den bisfosfonate aktive ingrediensen på formen av en parenteral, mest foretrukket en intravenøs, form.
Den særskilte administreirngsmåten og doseringen kan velges av den behandlende legen ved å ta i betraktning særtrekkene til pasienten, spesielt alder, vekt, livsstil, aktivitets-nivå, hormonstatus (for eksempel postmenopausal) og benmineraltetthet som er passende.
Dosering av midlene ifølge oppfinnelsen kan avhenge av forskjellige faktorer, slik som effektivitet og virkningsvarighet av den aktive ingrediensen, for eksempel inkludert den relative kraften av bisfosfonatet som benyttes, administreirngsmåten, den varmblodige arten, og/eller kjønn, alder, vekt og individuelt tilstand til det varmblodige dyret.
Normalt er doseringen slik at en enkeltdose av den bisfosfonat aktive ingrediensen fra 0,05 til 20 mg/kg, spesielt 0,01 til 10 mg/kg, administreres til et varmblodig dyr som veier tilnærmet 75 kg.
"mg/kg" betyr mg legemiddel pr. kg. kroppsvekt av pattedyret - inkludert mennesket - som skal behandles.
Dosen som er nevnt over administreres typisk periodevis med en periode på minst 6 måneder mellom dosene. Denne perioden mellom bisfosfonatadministreringene kan være lengre, for eksempel passende en gang pr. år, en gang pr. 18 måned eller 1 gang pr. annet år, eller til og med lengre eller en hvilken som helst tidsperiode der i mellom.
Formuleringer på enkeltdose enhetform inneholder fortrinnsvis fra omtrent 1% til omtrent 90%, og formuleringer som ikke er på enkeltdose enhetsform inneholder fortrinnsvis fra omtrent 0,1% til omtrent 20%, av den aktive ingrediensen. Enkeltdose enhetsformer slik som ampuller for infusjonsløsning eller faste for fremstilling av infusjonsløsningsdoser, kapsler, tabletter eller dragées inneholder for eksempel fra omtrent 0,5 mg til omtrent 500 mg av den aktive ingrediensen. Det vil forstås at den aktuelle enhetsdosen som benyttes vil avhenge av kraften hos bisfosfonatet, doseringsintervallet og administreirngsruten blant annet. Zoledronsyren kan benyttes som en enhetsdose på fra omtrent 1 opp til omtrent 10 mg for parenteral, for eksempel intravenøs, administrering. For eksempel kan det også benyttes en enhetsdose på fra omtrent 1 til omtrent 5 mg parenteralt for dosering en gang pr. 6 måned; mens en dose på fra omtrent 2 opp til omtrent 10 mg kan benyttes for en gang pr. år parenteral dosering.
Enhetsdoser kan administreres som en enkel eller fordelt dose, det vil si en dose der enhetsdosen er oppdelt i to eller flere like eller ulike deler og der delene administreres til pasienten samtidig, under overlappende tidsperioder eller ved separate tidspunkter. Når enhetsdosene administrerer som en oppdelt dose ved separate tidspunkter, kan intervallet mellom de separate administreringene av den oppdelte dosen være fra timer, for eksempel 1 time, opp til omtrent 1 måned (omtrent 30 dager). Ifølge oppfinnelsen er tidsintervallet mellom administrering av den siste delen av den oppdelte dosen og administrering av den første delen av den neste, påfølgende oppdelte dosen minst 6 måneder eller lengre, for eksempel omtrent et år.
Således kan for eksempel en 10 mg enhetsdose administreres som to like 5 mg deler ved et intervall på omtrent 1 uke til omtrent 1 måned, for eksempel omtrent 2 uker, mellom administrering av delene. Alternativt kan for eksempel en 5 mg enhetsdose administreres som to ulike deler med 4 mg og 1 mg (eller 3 mg og 2 mg) ved et intervall på fra 1 til 3 dager til 1 til 3 uker for eksempel omtrent 1 uke, mellom administrering av delene.
Farmasøytiske preparater for enteral og parenteral administrering er for eksempel de som er på doser i enhetsform, slik som dragées, tabletter eller kapsler og også ampuller. Det fremstilles på en i seg selv kjent måte, for eksempel ved hjelp av konvensjonelle blande-, granulering-, konfeksjonering-, oppløsning- eller lyofiliseringsprosesser. For eksempel kan farmasøytiske preparater for oral administrering oppnås ved å kombinere den aktive ingrediensen med faste bærere, der passende granulering av den oppnådde blandingen, og bearbeiding av blandingen eller granulering, om ønskelig eller nødvendig etter tilsetning av egnede hjelpemidler, til tabletter eller dragéekjerner.
Egnede bærere er i særdeleshet fyllstoffer, slik som sukker, for eksempel laktose, sakka-rose, mannitol eller sorbitol, cellulosepreparater og også bindemidler slik som stivelses-pasta, ved å benytte for eksempel mais-, hvete-, ris- eller potetstivelse, gelatin, tragant, metylcellulose og/eller polyvinylpyrrolidon og, om ønskelig oppløsende midler (dis-intergraters), slik som de ovennevnte stivelsene, også karboksymetylstivelse, kryssbundet polyvinylpyrrolidon, agar eller algeninsyre eller et salt derav, slik som natriumalginat. Hjelpemidler er i særdeleshet flytregulerende midler og smøremidler, for eksempel kiselsyre, talkum, stearinsyre eller salter derav, slik som magnesium- eller kalsiumstearat, og/eller polyetylenglykol. Dragéekjerner tilveiebringes med egnede belegg som kan være motstandsdyktige mot magesafter, det benyttes blant annet konsentrerte sukkerløsninger som eventuelt inneholder arabisk gummi, talkum, polyvinylpyrrolidon, polyetylenglykol og/eller titandioksid, eller lakkløsninger i egnede organiske løsningsmidler eller løsningsmiddelblandinger eller, for å frembringe belegg som er motstandsdyktige mot magesafter, benyttes løsninger av egnede cellulosepreparater slik som acetylcelluloseeftelat eller
hydroksypropylmetylcelluloseftalet. Fargesubstanser eller pigmenter kan tilsettes til
tablettene eller dragée belegningsmidlene, for eksempel i den hensikt å identifisere eller for å indikere forskjellige doser av aktiv ingrediens.
Andre oralt administrerbare farmasøytiske preparater er tørrfylte kapsler fremstilt av gelatin, og som myke, forseglete kapsler fremstilt av gelatin og en mykner slik som glyserol eller sorbitol. De tørrfylte kapslene kan inneholde den aktive ingrediensen på formen av et granulat, for eksempel i blanding med fyllstoffer, slik som laktose, bindemidler, slik som stivelser, og/eller glidemidler, slik som talkum, eller magnesiumstearat, og der det er passsende, stabiliseringsmidler. I myke kapsler er den aktive ingrediensen fortrinnsvis oppløst eller suspendert i egnede væsker, slik som fettoljer, parafinolje eller flytende polyetylenglykoler, og det er mulig å også tilsette stabilisatorer.
Parenterale formuleringer er i særdeleshet injiserbare fluider som er effektive på forskjellige måter, slik som intramuskulært, intraperitonalt, intranasalt, intradermalt, subkutant eller intravenøst. Slike fluider er fortrinnsvis isotoniske vandige løsninger eller suspensjoner som kan fremstilles før bruk, for eksempel fra lyofiliserte preparater som inneholder en aktiv ingrediensen alene eller sammen med en farmasøytisk akseptabel bærer, eller fra løsningskonsentrater. De farmasøytiske preparatene kan steriliseres og/eller inneholde hjelpemidler, for eks. konserveringsmidler, stabiliseirngsmidler, fuktemidler og/eller emulgeringsmidler, oppløsende midler salter for å regulere det osmotiske trykket og/eller buffere.
Egnede formuleringer for transdermal applikasjon inkluderer en effektiv mengde av den aktive ingrediensen med bærer. Fordelaktige bærere inkluderer absorberbare farmako-logiske akseptable løsningsmidler for å medvirke til passasje gjennom huden hos pasienten. Karakteristisk er transdermalt utstyr på form av en bandasje som omfatter en støtte, et reservoar som inneholder forbindelsen eventuelt med bærere, eventuelt en hastighetskontrollerende barriere for å tilføre den aktive ingrediensen til huden på pasienten ved en kontrollert og forhåndsbestemt hastighet over en forlenget tidsperiode, og middel for å feste utstyret til huden.
De etterfølgende eksemplene illustrerer oppfinnelsen som her er beskrevet.
I de følgende eksemplene skal betegnelsen "aktiv ingrediens" forstås som å være et hvilket som helst av zoledronsyrederivatene som er nevnt over som nyttige ifølge den foreliggende oppfinnelsen.
EKSEMPLER
Eksempel 1: Kapsler som inneholder belagte pellets av aktiv ingrediens.
En blanding av aktiv ingrediens med Avicel® PH 105 fuktes med vann og knas, ekstru-deres og formes til kuler. De ønskede pelletene blir deretter belagt i det fluidiserte sjiktet med et indre belegg, bestående av cellulose HP-M 603, polyetylenglycol (PEG)
8000 og talkum, og det vandige magesaft bestandige belegget bestående av Eudragit<®>L 30 D, trietylcitrat og Antifoam<®>AF. De belagte pelletene pudres med talkum og fylles i kapsler (kapselstørrelse 0) ved hjelp av en kommersiell kapselfyllemaskin, for eksempel Hofliger og Karg.
Eksempel 2: Monolittadhesivt transdermalsystem.
Fremstilling:
Komponentene over oppløses sammen i 150 g av en spesialkokepunktspetroleums-fraksjon 100-125 ved valsing på et valselager. Løsningen påføres en polyesterfilm (Hostaphan, Kalle) ved hjelp av en spredeinnretning slik som en 300 mm bestryknings-kniv, hvilket gir et belegg på omtrent 75 g/m<2>. Etter tørking (15 minutter ved 60°C) påføres en silikonbehandlet polyesterfilm (tykkelse 75 mm, Laufenberg) som avskrell-ingsfilmen. De ferdige systemene stanses ut i størrelser på den ønskede formen på fra 5 til 30 cm<2>ved å benytte stanseverkstøy. De fullførte systemene forsegles individuelt i poser av et aluminisert papir.
Eksempel 3: Medisinglass som inneholder 1,0 mg tørr, lyofilisert, l-hydroksy-2-(imidazol-l-yl)etan-l,l-difosfonsyre (blandede natriumsalter derav). Etter fortynning med 1 ml vann oppnås en løsning (konsentrasjon 1 mg/ml) for i.v. infusjon. 11 ml vann titreres den aktive ingrediensen med trinatriumcitrat x 2 H2O til pH 6,0. Deretter tilsettes mannitolen og løsningen lyofiliseres og lyofilisatet fylles i et medisinglass.
Eksempel 4: Ampulle som inneholder aktiv ingrediens, oppløst i vann. Løsningen (konsentrasjon 3 mg/ml) er for i.v. infusjon etter fortynning.
Eksempel 5. Behandling av pasienter
"Et multinasjonalt, randomnisert, dobbelblind, placebo-kontrollert, parallellgruppe, doserangering-, sikkerhet- og effektivitetsforsøk med intravenøst bolusinjeksjon av zoledronat ved behandling av postmenopauseosteoporose"
Dette var et dose- og doseregimeundersøkelse -12 måneders forsøk av iv zoledronsyre i pasienter med postmenopauseosteoporose. Trehundreogfemtien pasienter ble tilfeldig fordelt (randomisert) til seks studiearmer. Pasienter som nylig var blitt utsatt for benaktive legemidler, for eksempel bisfonater, østrogen, kalsitonin, raloxifen, eller som hadde en historie med metabolistiske bensykdommer ble ekskludert. Alle pasienter ble vurdert ved grunnlinjen og ved 3 måneders besøk. Zoledronsyre eller placebo ble administrert som en bolus i ved injeksjon til en perifer vene i løpet av 5 minutter ved hvert besøk.
Effektivitet ble sikret ved å måle prosent forandring fra grunnlinje i benmineraltetthet (BMD) målt ved dobbel energi røntgen absorpsjonsmeter (DEXA) sammenlignet med placebo, ved 6,9 og 12 måneder.
Som en spesiell sikkerhetsmåling ble trans-hoftebensbiopsi oppnådd i en undergruppe av pasienter fra alle studiearmene ved 12 måneder, og røntgen av thorax- og lumbalryggraden for alle studiedeltakerne ble vurdert ved grunnlinje og ved 12 måneder med hensyn på forekomst av insidente virvelbrudd.
I tillegg ble graden og varigheten av undertrykkelse av biokjemiske markører for benomsetning-paratyroidhormon (PTH), benspesifikk alkalisk fosfatase (BSAP), serum C-telopeptid (CTX), serum osteokalsin, urin N-telopeptid (NTX)/kreatininforhold, urin deoksypypirdinolin (d-pyd)/kreatininforhold, urin pyridinolin (pyd)/kreatininforhold-oppnådd hver 3. måned og målt i et sentralt laboratorie.

12 måneders resultatene viste at alle behandlingsarmer ga en prosent forandring fra grunnlinje i BMD som var signifikant større (p<0,001) større enn placebo og ikke ulik fra hverandre.
Fotnote: Trinnvis multiple sammenligning av de aktive dosene av zoledronat mot placebo ved et ensidig multiple alfanivå av 2,5% som er regulert for multiple sammenligninger ifølge Marcus, Peritz og Gabriel (1976).
Benmineraltetthet økte sammenlignet med placebo i ryggraden, hoften, distal radius, og "hele kroppen". Undertrykkelse av biokjemiske markører for bendannelse og benresorpsjon bekreftet og underbygget BMD resultatene, hvilket demonstrerte undertrykkelse av benomsetning på pre-menopause nivået gjennom hele 6- og 12 måneders
doseringsintervallene.
BMD dataene indikerer at zoledronsyredoseadministrering så sjeldent som hver 6 eller 12 måned trygt kan resultere i en statistisk signifikant og medisinsk relevant benmasse-økning. Det antas at disse dataene videre indikerer at en vedvarende konservering av nytt ben utover 1 år, uten ytterligere doseadministrering, er sannsynlig eller at ytterligere benmasseøkning er mulig. Det antas også at gjentatt behandling i ytterligere sykluser på hver 6 måned, 12 måned eller sjeldnere doseadministrering vil føre til BMD økning. En reduksjon av risikoen for osteoporosebrudd forventes å følge med benmasseøkningene.

Anvendelse av zoledronsyre, eller et farmasøytisk akseptabelt salt derav, eller et hvilket som helst hydrat derav til fremstilling av et medikament for behandling av tilstander med abnormt økt benomsetning (bone turnover), der zoledronsyren, eller et farmasøy-tisk akseptabelt salt derav, eller et hvilket som helst hydrat derav administreres periodevis og der perioden mellom administreringene er på minst omtrent 6 måneder.

Anvendelse ifølge krav 1, der perioden mellom administreringene er på minst omtrent ett år.
