Description
OPPFINNELSENS OMRÅDE
Oppfinnelsen angår en kombinasjon av probiotika, der kombinasjonen omfatter laktobacillus, propionsyrebakterier og bifidobakterier i ulike kombinasjoner. Probiotika er fortrinnsvis kombinert med et egnet prebiotikum for å produsere et synbiotikum. Kombinasjonen ifølge oppfinnelsen kan konsumeres som sådan eller kombineres med et egnet næringsmiddel slik som et meieriprodukt eller en drikk, og den er terapeutisk nyttig for eksempel for stimulering av immunsystemet og for generell forbedring av helsen.
OPPFINNELSENS BAKGRUNN
Probiotika er levende mikrober som, når de administreres til mennesker eller dyr, fremmer velvære hos verten ved å forbedre den mikrobielle balansen i tarmen (Fuller, R. Probiotics in man and animals, 1989, J. Appl. Microbiol. 66:365-378). De best dokumenterte probiotika inkluderer L. rhamnosus LGG, L. johnsonii LAI, L. casei Shirota og Bifidobacterium lactis Bbl2.1 tillegg er et antall andre probiotika beskrevet i litteraturen innen feltet (se for eksempel M. Prebiotika er ikke fordøyelige matingredienser som fremmer menneskets helse ved selektivt å stimulere vekst og aktivitet til en eller flere probiotiske bakterier i tykktarmen. Et prebiotikum er vanligvis et ikke-fordøyelig karbohydrat (oligo- eller polysakkarid) eller en sukkeralkohol som ikke degraderes eller absorberes i den øvre tarmkanal. Menneskets fordøyelseskanal huser mange bakterier som lever i symbiose med verten. p-glukoronidaseenzymene produsert av tarmbakterier antas for eksempel å bidra til dannelse av karsinogene forbindelser. Steroider og andre karsinogene forbindelser metaboliseres i leveren og konjugeres deretter med glukoronsyre. Gallen avleverer den konjugerte glukoronforbindelsen til tynntarmen og derfra passerer forbindelsen videre
til tykktarmen der glykoronidaseenzymene kan hydrolysere forbindelsen, dermed frigis toksiske forbindelser i tykktarmen (Rowland, LR. 1995. Toxicology of the colon: role
of the intestinal microflora, in: Human Colonic Bacteria, Role in nutritiion, physiology, and pathology. Forfattere: Gibson, G.R. og Macfarlane, G.T. side 155-174, Boca Raton: CRC Press). Det antas at artene Eubacterium, Bacteroides og Clostridium frigir større mengder av disse skadelige enzymene til tarmen enn representanter for artene Bifidobacterium og Lactobacillus. Dette vil være en av årsakene til at det er fordelaktig at tarmfloraen er sammensatt av bifidobakterier og laktobacillus.
I tillegg absorberes for eksempel ikke glukosider som kommer fra grønnsaker og te i tynntarmen og passerer dermed til tykktarmen der de kan hydrolyseres ved virkning av P-glukosidaser for å danne toksiske eller mutagene aglykonforbindelser (Goldin, B.R. 1990. Intestinal Microflora: metabolism of drugs and carcinogens. Annals of Medicine 22: 43-48).
Videre produserer tarmfloraen ureaseenzymer som degraderer urea til ammoniakk. Høye mengder av ammoniakk kan være toksisk for epitelcellene i tarmen (Mobley, H.L.T. og Hausinger, R.P. 1989. Microbial ureases: significance, regulation and molecular characteirzation. Microbiological Reviews 53:85-108).
Den humane tarmflora dannes under de tidlige leveår og ingen store endringer finner sted i dens sammensetning deretter. Bare små endringer innen artene kan finne sted (i for eksempel bifidobaterier).
Sammen med den økende forståelsen for viktigheten av tarmfloraen har forskning bitt fokusert omkring avdekking av hvilke faktorer som kan anvendes for å påvirke sammensetningen av floraen og dens funksjonalitet (levedyktighet) på en slik måte at fordelaktige bakteriearter vil styrkes og skadelige bakteriarter reduseres. Det antas at funksjonaliteten til mikrobene kan påvirkes av prebiotika for å fremme fordelaktige bakterier. Omfattende forskning er blitt utført på galakto-oligosakkarider (GOS) som er di-, tri-, tetra-, penta- og heksasakkarider og som primært inneholder galaktoseenheter. De fremstilles enzymatisk fra laktose og innholdet av sluttprodukt avhenger av det anvendte enzym (Matsumoto, K. et al. 1993. Galacto-oligosakkarider. In: Oligosaccharides. Production, properties and applications. red. Nakakuki, T-, Japanese Technology Reviews. vol. 3. nr. 2., side 90-116, Gordon and Breach Science Publishers, Sveits, Australia). GOS er tidligere vist for eksempel å ha bifidogene egenskaper, dvs. egenskaper som fremmer vekst av bifidobakterier (Ito, M. et al. 1990. Effect of administration of galactooligosaccharides on the human faecal microflora, stool weight, and abdorminal sensation. Microb. Ecol Health Dis. 3:285-292).
BESKRIVELSE AV TEKNIKKENS BAKGRUNN
Både produkter inneholdende en enkelt probiotisk stamme og kombinasjoner av flere ulike probiotika er rikelig beskrevet i litteraturen. Synbiotika er også beskrevet i litteraturen.
Publikasjonen EP 904 784, N. V. Nutricia, beskriver for eksempel et probiotisk produkt inneholdene Bifidobacterium, Enterococcus faecium og Lactobacillus. I tillegg kan produktet inneholde prebiotika slik som polysakkarid eller ikke-degraderbar stivelse, og immunglobuliner, vitaminer etc. Ifølge publikasjonen har produktet en helsefremmende effekt ved at det stimulerer for eksempel immunsystemet. Effekten er imidlertid ikke vist i kliniske tester, og andre biologiske aktiviteter er heller ikke studert.
WO 00/33854, N. V. Nutricia, beskriver et produkt omfattende et probiotikum og oligosakkarider. Probiotiske bakterier som spesielt refereres er Lactobacillus og Bifidobacterium, selv om Pediococcus, Propionibacterium, Leuconostoc og Saccharomyces også nevnes. De involverte prebiotika inkluderer transgalakto-oligosakkarider (TOS) og frukto-oligosakkarider (FOS). Ifølge publikasjonen omfatter en særlig fordelaktig kombinasjon Lactobacillus rhamnosus og transgalakto-oligosakkarid eller et hydrolyseprodukt av potetgalaktan, der denne kombinasjonen og dens fremstilling også illustreres i eksemplene. Ifølge publikasjonen har produktet helsefremmende effekter og det er særlig nyttig ved behandling av tarmforstyrrelser. Dens aktivitet er imidlertid ikke på noen måte vist.
WO 97/34615, University of New South Wales, beskriver en probiotisk sammensetning inneholdende, i tillegg til en eller flere probiotika, resistent (ikke-degraderbar) stivelse og oligosakkarid, der en synergistisk effekt oppnås mellom de tre komponentene. De nevnte probiotika er Saccharomyces, Bifidobacterium, Bacteroider, Clostridium, Fusobacterium, Propionibacterium, Lactococcus, Streptococcus, Enterococcus, Staphylooccus, Peptostreptococcus og Lactobacillus, og oligosakkaridene inkluderer, blant mange andre, frukto- og galakto-oligosakkarider. Eksemplene viser den synergistiske effekt av bifidokabterier, maisstivelse og frukto-oligosakkarider på mengden bifidobakterier. Noen terapeutisk effekt er imidlertid ikke beskrevet.
US 5 895 648, Sitia-Yomo S.p.A., beskriver en probiotisk sammensetning omfattende, i form av lyofiliserte, levende bakterier, minst to arter bifidobakterier og minst to arter lactobacilli eller streptococcus, kombinert med en eller flere oligosakkarider. Sammensetningen omfatter til sammen 4 til 20 vektdeler probiotika og 5 til 22 vektdeler oligosakkarider, der for eksempel glakto- og frukto-oligosakkarider nevnes, i særdeleshet inulin. De nevnte probiotika inkluderer Bifidobacterium breve,
Bifidobacterium infantis, Bifidobacterium longum, Bifidobacterium bifidum,
Lactobacillus acidophilus, Lactobacillus bulgaricus, Lactobacillus casei, Lactobacillus plantarum, Streptococcus thermophilus og Streptococcus faecium. Ifølge publikasjonen kan en blanding inneholdende probiotika og prebiotika tilsettes melkebaserte desserter, melk eller juice for å balansere tarmfunksjonen. Noen biologisk aktivitet av kombinasjonen er imidlertid ikke beskrevet.
Publikasojonen Milchwissenschaft (1988) vol. 53, nr. 11, side 603-605, beskriver PAB-melk som omfatter tre bakteriestammer: Propionibacterium freudenreichii subsp. shermanii MTCC 1371, Lactobacillus acidophilus. og Bifidobacterium bifidum NDRI. Prebiotika er ikke tilsatt produktet. Ifølge publikasjonen er PAB-melk egnet for babyer og barn, også for de som lider av laktoseintoleranse.
WO 99/10476 beskriver den stimulerende effekt av spesifikke bakteriestammer, dvs. Lactobacillus rhamndsus HN001 (NM97/09514) og HN067 (NM97/01925), og Lactobacillus acidophilus NHO 17 (NM97/09515) og Bifidobacterium lactis HN019 (NM97/09513) på immunsystemet, målt som økt fagocytose. Stammene kan anvendes enkeltvis, eller de kan tilsettes meieriprodukter eller farmasøytiske preparater.
US 5 902 578, Abbott Laboratories, angår en fremgangsmåte for å forebygge eller behandle diaré med en blanding av Lactobacillus reuterii, Lactobacillus acidophilus og Bifidobacterium infantis, der blandingen tilberedes som et pulver, væske eller piller.
Biologiske og terapeutiske effekter av probiotika- og synbiotikatyper beskrevet ovenfor er også beskrevet innen teknikken. Som eksempler kan nevnes Gallagher, D. et al. Journal of Nutrition (1996) vol. 126, nr. 5, side 1362-1371 som beskriver effektene av bifidobakterier og Lactobacillus acidophilus på koloncancer hos rotter og beskriver at de beste resultater ble oppnådd ved anvendelse av både bakterier og frukto-oligosakkarid, og Kirjavainen, P. et al. Clinical and Diagnostic Laboratory Immunology (nov 1999) vol. 6, nr. 6 side 799-802 som beskriver de positive effektene av to, separatstuderte melkesyrestammer, Lactobacillus rhamnosus GG og Propionibacterium freudenreichii subsp. shermanii JS på lymfocyttnivåene, og dermed immunresponsen hos mus. Kombinasjonen av disse to bakteriene er ikke beskrevet eller studert.
Kombinasjon av probiotika med andre substanser med terapeutiske effekter er også beskrevet i litteraturen. For eksempel beskriver WO 97/29762 og WO 97/29763, Procter & Gamble Company anvendelse av laktobacilli og bifidobacterier kombinert med galakto- eller frukto-oligosakkarider sammen med en plante fra slekten Ericaceae, eller et ekstrakt fra denne for behandling av urinveisinfeksjoner og tarmforstyrrelser og WO 00/29007, Reddy, beskriver kombinasjonen av probiotika, slik som Lactococcus, Lactobacillus, Pediococcus, Streptococcus, Propionbacterium, Brevibacterium, Penicillum og Saccharomyces med plantebaserte naturlige produkter og medikamentliknende substanser.
Suomalaien, T. H., Lait, vol 79,1999 side 165-174 beskriver også en kombinasjon av prebiotiske bakterier, men kombinasjonen er forskjellig fra den som beskrives i følge foreliggende oppfinnelse.
Selv om probiotika og synbiotika er nøye studert er gode og allsidige kommersielle produkter ikke tilgjengelig i vesentlig grad. Følgelig er det fortsatt et klart behov for å tilby konsumentene nye produkter med dokumenterte prebiotiske effekter og fremstilt i en form som tillater dem å anvendes for eksempel som en hensiktsmessig del eller et supplement til den daglige dietten.
KORT BESKRIVELSE AV OPPFINNELSEN
Det er derfor et aspekt ifølge oppfinnelsen å frembringe et nytt probiotikainneholdende produkt der den prebiotiske effekten er tydelig demonstrert, som er behagelig å anvende og sunn for konsumenten.
Disse hensikter ble oppnådd med en kombinasjon ifølge oppfinnelsen som omfatter mikroorganismene Lactobacillus rhamnosus LGG (ATCC 53103), Lactobacillus rhamnosus LC705 (DSM 7061), Propionibacterium freudenreichii ssp. shermanii PJS (DSM 7067) og en bifidobakterie.
I en foretrukken utførelsesform er bifidobakterien Bifidobacterium infantis Bbi99 (DSM 13692). Imidlertid kan en hvilken som helst bifidobakterie (slik som Bbl2) inkluderes i kombinasjonen om ønskelig.
I tillegg omfatter kombinasjonen fortrinnsvis et prebiotikum som støtter veksten av de ovennevnte mikrober. Det prebiotiske middelet som anvendes kan være et oligosakkarid, fortrinnsvis galakto-oligosakkarid (GOS).
Den nye kombinasjonen kan anvendes som sådan eller som en del av et annet produkt, slik som et farmasøytisk produkt eller en matvare. Kombinasjonen ifølge oppfinnelsen har en fordelaktig effekt på den menneskelige tarmbalansen ved at den øker enterolactonproduksjonen og reduserer den ufordelaktige høye pH-verdien. Kombinasjonen påvirker også immunresponsen ved å øke mengden av lymfocytter og y-interferon (IFN) og ved å redusere dannelsen av karsinogene substanser. Kombinasjonen ifølge oppfinnelsen er således nyttig for forebyggelse og behandling av tarmforstyrrélser, allergier og cancer og for å fremme generell helse.
I henhold til foreliggende oppfinnelse er kombinasjonen følgelig også anvendbar som en terapeutisk substans og i fremstilling av terapeutiske substanser.
DETALJERT BESKRIVELSE AV OPPFINNELSEN
Av stammene anvendt i oppfinnelsen er Lactobacillus rhamnosus LGG, Lactobacillus casei ssp. rhamnosus LC705 og Propionibacterium freudenreichii ssp. shermanii PJS beskrevet innen teknikken. Bifidobacterium infantis Bbi99, som kan inkluderes i kombinasjonen, er en ny stamme og vil bli beskrevet i større detalj nedenfor.
Lactobacillusrhamnosus LGG er for eksempel beskrevet i US patent 5 032 399, Gorbach & Goldin. Stammen er isolert fra human feces, den gror godt ved pH 3 og overlever selv lavere pH-verdier så vel som høyt gallesaltinnhold. Stammen har utmerket adhesjon til både slimhinneceller og epitelceller. Melkesyreutbytte fra glukosen er god: når den dyrkes i MRS-næringsløsning produserer stammen 1,5 - 2% melkesyre. Stammen fermenterer ikke laktose og produserer følgelig ikke melkesyre fra laktose. Stammen anvender de følgende karbohydrater: D-arabinose, ribose, galaktose, D-glukose, D-fruktose, D-mannose, rhamnose, dulcitol, inositol, mannitol, sorbitol, N-acetylglukosamin, amygdalin, arbutin, esculin, salicin, cellobiose, maltose, sakkarose, trehalose, melezitose, gentibiose, D-tagatose, L-fucose og glukonat. Stammen vokser godt ved +15 - 45°C, der den optimale temperatur er 30 - 37°C. Lactobacillus rhamnosus LGG er deponert ved deponeringsinstitusjonen American Type Culture Collection under aksesj onsnummer ATCC 53103.
Lactobacillus rhamnosus casei ssp. rhamnosus LC705 er beskrevet i større detalj i Fl patent 92498, Valio Oy. LC705 er en gram-positiv kort stavbakterie som opptrer i kjeder; den er homofermentiv; svakt proteolytisk; vokser godt ved +15 - 45°C; produserer ikke ammoniakk fra arginin; er katalasenegativ; når stammen dyrkes i MRS-næringsmedium (LAB M) produserer den 1,5% melkesyre med en optisk aktivitet av L(+)-konfigurasjonen; stammen nedbryter sitrat (0,169%) dermed produseres diacetyl og acetoin; stammen fermenterer minst de følgende karbohydrater (sukkere, sukkeralkoholer): ribose, galaktose, D-glukose, D-fruktose, D-mannose, L-sorbose, rhamnose, mannitol, sorbitol, metyl-D-glukosid, N-acetylglukosamin, amygdalin, arbutin, esculin, salicin, cellobiose, maltose, laktose, sukrose, trehalose, melezitose, gentiobiose, D-turanose og D-tagatose. LC705 festes svakt til slimhinneceller, og moderat til epitelceller. Viabiliteten til stammen er god ved lave pH-verdier og høyt gallesaltinnhold. Stammen overlever godt ved et saltinnhold på 5% og ganske godt ved et saltinnhold på 10%. Lactobacillus casei ssp. rhamnosus LC705 er deponert hos Deutsche Sammlung von Mikroorganismen und Zellkulturen GmbH (DSM) under aksesj onsnummer DSM 7061.
Propionibacterium freudenreichii ssp. shermanii PJS er også beskrevet i detalj i Fl patent nr. 92498, Valio Oy. PJS er en gram-positiv kort stavbakterie; den fermenterer glukose, fruktose, galaktose og laktose; den fermenterer godt laktat; og dens optimale veksttemperatur er 32°C. Levedykdigheten til stammen ved lavepH-verdier og høyt galleinnhold er utmerket. Propionibacterium freudenreichii ssp. shermanii PJS er deponert ved den Deutsche Sammlung von Mikroorganismen und Zellkulturen GmbH (DSM) med aksesjonsnummer DSM 7067.
Bifidobacterium infantis Bbi99 er isolert fra feces hos friske nyfødte. B. intantis Bbi99 er en gram-positiv pleomorf stavbakterie. Stammen er katalase-negativ, fruktose-6-fosfat-fosfoketolase-positiv (F6PPK) og både a- og P-galaktosidase og a- og P-glukosidase-positiv. B. infantis Bbi fermenterer de følgende karbohydrater: ribose, galaktose, D-glukose, D-fruktose, D-mannose, metyl-D-mannose, N-acetylglukosamin, esculin, salicin, cellobiose, maltose, laktose, melibose og gentibiose. Den optimale veksttemperatur er 30 - 40°C og pH 6,5 - 7,0. Når den dyrkes i næringsløsninger inneholdende heksose produserer stammen L-melkesyre og eddiksyre (i forholdet 2:3). G+C-innholdet av DNA er 55 - 67 mol%. Bifidobacterium infantis Bbi99 ble deponert ved den Deutsche Sammlung von Mikroorganismen und Zellkulturen GmbH (DSM) ifølge budapestavtalen under aksesjonsnummer DSM 13692 den 28. august 2000.
Kombinasjonen ifølge oppfinnelsen kan også inkludere andre mikroorganismer, slik som mikroorganismer og probiotika inneholdt i såkalte startere anvendt i meieriindustrien. Det er flere vel dokumenterte stammer av startere, som er kommersielt tilgjengelig for produsenter slik som Hansen A/S, Danmark og Danisco/Wiesby GmbH, Tyskland.
For å tilberede kombinasjonen ifølge oppfinnelsen dyrkes mikroorganismene ved anvendelse av konvensjonelle prosesser innen teknikken. De dyrkes som rene kulturer eller som ulike blandingskulturer. Kulturene kan anvendes som sådan eller de kan bearbeides etter behov for eksempel ved rensing, konsentrering, lyofilisering eller ved til slutt å produsere ulike preparater.
En tilstrekkelig mengde av probiotika anvendes i kombinasjonen for å frembringe den ønskede probiotiske effekt. Mengden av hvert probiotika må følgelig variere innenfor et bredt område avhengig av for eksempel stammene som anvendes og deres antall, totalmengden av celler av probiotika, den daglige totaldosen og andre egenskaper og ingredienser i produktet. En daglig dose av kombinasjonen omfatter vanligvis ca. IO6 - 1010 cm av probiotika.
En eller flere prebiotika tilsettes fortrinnsvis til kombinasjonen for å danne et synbiotikum. Prebiotika utvelges i overensstemmelse med mikroorganismene inkludert i kombinasjonen vurdert slik at den vil støtte veksten av mikroorganismene. Egnede prebiotika kan inkludere for eksempel oligosakkarider, spesielt galakto-oligosakkarider (GOS), palatinose-oligosakkarider, søyabønne-oligosakkarider, gentio-oligosakkarid, xylo-oligomerer, ikke-degraderbar stivelse, laktosakkarose, laktulose, laktitol, maltitol, polydekstrose eller liknende. En tilstrekkelig mengde av prebiotika tilsettes til synbiotika for å produsere en prebiotisk effekt. Hva som er en tilstrekkelig mengde bestemmes for eksempel i samsvar med stammen av interesse, mengden av prebiotika inkludert og annet innhold og anvendelse av produktet. Mengden varierer derfor også innenfor et bredt område, det kan være fra 0,5 til 5 g for eksempel i en daglig dose.
Prebiotikum trenger ikke nødvendigvis være inkludert i kombinasjonen. Avhengig av sluttproduktet og den tilsiktede anvendelse kan det være bedre å konsumere prebiotikumet atskilt, allikevel omtrent på samme tidspunkt som probiotikumkombinasjonen. I noen tilfeller kan det være tilstrekkelig bare å konsumere probiotikumkombinasjonen og prebiotikumet er følgelig ikke nødvendig i det hele tatt. Et eksempel på dette er tilfellet der tilstanden i tarmen hos verten er egnet for vekst av probiotika uten at tilførsel av prebiotika er nødvendig, og når prebiotika finnes i den normale dietten (dersom den for eksempel konsumeres i grøt eller rugbrød).
Foreliggende oppfinnelse har vist at de anvendte mikroorganismene imøtekommer settet av kriterier for probiotika: de overlever godt under krevende betingelser i fordøyelseskanalen, festes godt til tarmceller og vokser godt i tarmene. Det er også vist at de innehar utmerkede biologiske effekter for eksempel øker de antallene mikroorganismer ønskelig ut fra et helsesynspunkt og reduserer antallet skadelige mikroorganismer i tarmen, de reduserer aktiviteten av skadelige enzymer og dermed dannelsen av skadelige eller også karsinogene substanser, og de har en stimulerende effekt på immunresponsen.
Kombinasjonen ifølge oppfinnelsen kan anvendes som sådan eller i form av kapsler, piller eller tabletter, for eksempel fremstilt ved konvensjonelle fremgangsmåter for fremstilling av farmasøytiske produkter. Kombinasjonen ifølge oppfinnelsen kan også tilsettes til ulike spiselige produkter, slik som matvarer, drikkeprodukter eller konfektindustrien, helsefremmende produkter, naturlige produkter etc. Innenfor rammen av foreliggende oppfinnelse er produkter inneholdende kombinasjonen ifølge oppfinnelsen slik som meieriprodukter, særlig yoghurt og andre fermenterte melkeprodukter; oster og smørepålegg, barnemat; juice og supper og kapsler. Et produkt er i form av en kapsel som vanligvis bare inneholder den probiotiske kombinasjon der det prebiotiske middel konsumeres separat.
Sluttproduktene fremstilles ved konvensjonelle prosesser, der kombinasjonen tilsettes enten i forbindelse med fremstillingen eller deretter under ferdigstillingen av sluttproduktet.
Oppfinnelsen er beskrevet i større detalj med henvisninger til de følgende eksempler, som bare skal illustrere oppfinnelsen og ikke begrense rammen på noen måte.
Eksempel 1
Tilberedning av kombinasjonen
Kombinasjonen ble tilberedt fra en bakterieblanding, tilsatt, om ønskelig, galakto-oligosakkarid (GOS) som et prebiotisk middel. Bakterieblandingen ble laget fra bakteriekulturer (konsentrater eller frysetørket pulver) av fire stammer, dvs. Lactobacillus rhamnosus LGG (ATCC 53103), Lactobacillus rhamnosus LC705 (DSM 7061), Propionibacterium freudenreichii ssp. shermanii PJS (DSM 7067) og Bifidobacterium infantis Bbi99 (DSM 13692).
Både LGG og bifidobacterium ble dyrket atskilt.
LGG ble dyrket i et medium omfattende 5,0% mysepermeat (Valio Oy), 0,5% caseinhydrolysat (Valio Oy), 0,5% teknisk gjær og 0,0015% MnS04 x H20. Komponentene i mediet ble løst i vann og mediet ble sterilisert (i 20 minutter ved 120°C). Tørkingen ble utført ved en temperatur på 37°C og en pH på 4,8 (justert med NH4OH) i ca. 18 timer og ved en omrøringshastighet på 100 rpm. Etter dyrkingen ble bakteriecellene konsentrert, vasket og frysetørket ved anvendelse av 10 volum% tilsatt beskyttelsesmiddel, slik som en sakkaroseløsning på 46%, eller et tilsvarende alternativ kjent for fagpersoner innen teknikken. Det endelige bakterieinnhold var > 1 x IO<9 >cfu/ml i kulturen, > 1 x 10<10> cfu</g> i konsentratet og >1 x 10u cfu/g i det frysetørkede pulveret.
Sammensetningen av vekstmediet til bifidobakterien var som følger: mysepermeat 4% (Valio Ltd.), kaseinhydrolysat 1,0% (Valio Ltd.), teknisk gjærekstrakt 1,0% (LAB M) cystein-HCl 0,03% (Merck, Darmstadt, Tyskland). Andre ingredienser i mediet ble først løst i vann, deretter ble cystein-HCl tilsatt og mediet sterilisert (i 20 minutter ved 120°C). Dyrkingen ble utført i ca. 18 - 20 timer ved en temperatur på 37°C og en pH på 6,7 (justert med NH4OH) og en omrøringshastighet på 100 rpm. Bakterieinnholdet i kulturen var > 1 x IO<9> cfu/ml. Etter dyrkingen ble bakteriecellene konsentrert, vasket og frysetørket ved anvendelse av et tilskudd av 10 volum% beskyttelsesmiddel, slik som en sakkaroseløsning på 46% eller et tilsvarende alternativ kjent for fagpersoner innen teknikken. Bakterieinnholdet i konsentratet var > 1 x 10<10 >cfu/g og i det frysetørkede pulver > 1 x 10<11> cfu/g.
LC705 og PJS ble dyrket sammen ved å innokkulere bakterieceller til en mysebasert vekstløsning i et forhold 1:2. Det mysebaserte vekstmediet inneholdt 3,5 - 5% mysepermeat (Valio Oy), 1,0% kaseinhydrolysat og 1,0% gjærekstrakt (Valio Oy). Stammene ble dyrket i 3 dager ved 30°C ved pH opprettholdt på 4,5 ved hjelp av en automatisk pH-justering. Etter dyrkingen var innholdet av hver bakteriestamme > 1 x IO<9> cfu/ml. Eter dyrkingen ble bakteriecellene konsentrert, vasket og frysetørket ved anvendelse av et tilskudd på 10 volum% beskyttelsesmiddel, slik som en sakkaroseløsning på 46% eller et tilsvarende alternativ kjent for fagpersonen innen teknikken. Innholdet av hver av bakteriene i konsentratet var > 2 x IO<10> cfu/g og i det frysetørkede pulveret > 1 x IO<11> cfu/g.
Stammene ble også dyrket separat. I det tilfelle at LC 705 dyrkes som beskrevet ovenfor i forbindelse med den kombinerte dyrkingen er tørkingstiden bare 1 dag ved 30°C. PJS dyrkes i et mysebasert vekstmedium omfattende 2% mysepermeat, 1,0% kaseinhydrolysat og 1% gjærekstrakt. Stammen dyrkes i 3 dager ved 30°C med en pH-verdi på 6,3, hvorpå prosessen utføres som beskrevet ovenfor.
Konsentratene eller pulverne blandes i et forhold på 1:1:1. Når LC705 og PJS har vært dyrket separat er blandingsforholdet 1:1:1:1. Den oppnådde blanding av konsentrater eller frysetørket pulver anvendes som en probiotisk del i ulike produktapplikasjoner av kombinasjonen. Blandingen tilsettes produktapplikasjonen for å oppnå de følgende sluttbakteireinnhold i produktet.
GOS (Valio Oy) ble tilsatt, om ønskelig, som et separat produkt til produktapplikasjonen for å oppnå en GOS-konsentrasjon på ca. 0,5 - 5 g/dose i produktet.
Eksempel 2
Adhesive egenskaper til stammene og deres toleranse ved tarmbetingelser
Adhesjon av de prebiotiske stammene til slimhinnene ble undersøkt i henhold til Ouweland et al. (Ouwehand, A. C, Kirjavainen, P.V., Granlund, M. - M, Isolauri, E., og Salminen, S. J. 1999. Adhesion of probiotic micro-organisms to intestinal mucus. Int. Dairy J. 9:623-630). LGG og propionibakterien PJS festet seg utmerket til tarmslimhinnen, Bbi99 moderat og LC705 svakt. Adhesjon er en forutsetning for at mikroben frembringer fordelaktige effekter i tarmen. På den annen side er LC705 kjent for å adherere godt til epitelceller, tilsvarende som LGG (Lehto, E. and Salminen S. 1997. Adhesion of two lactobacillus strain and one propionibacterium strain to cultured human intestinal CaCO-2 cell lines. Bioscience Microflora 16: 13 - 17). Denne egenskap er fordelaktig når det er en ubalanse i tarmen og den beskyttende slimhinnen er svekket.
In vitro tester har vist at stammene tåler de fysiologiske gallesaltinnhold og den lave pH i magen.
Stammene ble undersøkt ved ulike pH-verdier i MRS-næringsløsning der pH ble justert med melkesyre til verdiene pH 4, pH 3 og pH 2. Stammene som ble studert (ferske kulturer) ble innokkulert i en pH-næringsløsning på 1% og dyrket ved 37°C i 3 timer, hvorpå innholdet av levende celler ble bestemt ved anvendelse av en agar egnet for stammen (se tabell 5). Stammene opprettholdt sin levedyktighet i løpet av den 3 timers prosessen ved en pH-verdi på 3 utmerket. Propionibakterien var levedyktig selv ved pH 2.1 testen ble bakterien ikke beskyttet av komponenter forbundet med mat (slik som fet) og de antas derfor muligens å bevares enda bedre når de konsumeres in vivo sammen med mat.
Med hensyn på gallesalttoleranse ble stammene testet i MRS-næringsløsning som inneholdt 0,3% og 0,5% Oxgal (Sigma)-gallesalt ved inokkulering av 1% fersk kultur til gallesalt MRS-løsningen som skulle studeres. Stammene ble dyrket i næringsløsningen i 3 timer ved 37°C, hvorpå innholdet av levende celler ble bestemt ved anvendelse av en agar egnet for stammen (se tabell 5). Alle stammer overlevde behandlingen helt utmerket.
Eksempel 3
Utvelgelse av et prebiotikum egnet for kombinasjonen
Alternative prebiotika ble studert for ulike stammer ved å dyrke hver stamme i en sukkerfri MRS-næringsløsning til hvilket det ble tilsatt 1% av det prebiotiske middelet som skulle studeres. Hver stamme ble dyrket i 1-2 dager ved sin optimale temperatur. Bakterievekst ble observert i løpet av studien ved å bestemme turbiditet på kulturen ved spektrofotometri. Som vist i tabell 4 var den beste vekstfremmeren i alle fire stammene et galakto-oligosakkarid (GOS) tilsetning på 1%
Eksempel 4
Fremstilling av sluttproduktet
En funksjonell drikk (på finsk "tehojuoma"; Valio Oy) ble anvendt som en base for å fremstille en juice (dose 65 ml/dag) til hvilket 0,1 g av en frysetørket blanding av bakterier/dose (=65 ml) og 3,8 g 70% GOS-sirup/dose (=65 ml) ble tilsatt. Tilsvarende juice ble ikke tilsatt GOS-sirup eller bakterieblandingen ble anvendt som en kontroll.
Bakterieinnholdet i de ferdige drikkene var som følger:
Produktet ble anvendt i de følgende kliniske undersøkelser der pro = juice + probiotisk tilskudd og syn = juice + probiotisk tilskudd + prebiotisk tilskudd.
Eksempel 5
Kliniske effekter av kombinasjonen ifølge oppfinnelsen
Drikken beskrevet i eksempel 4, som inneholdt den ovenfor beskrevne probiotiske kombinasjon (Pro) eller den probiotiske kombinasjon og et prebiotikum (Syn), ble testet klinisk på 20 menn. Testpersonene drakk daglig i samsvar med studieplanen og i løpet av studien fikk de ikke lov til å konsumere andre probiotikuminneholdende produkter. Studieplanen var slik at testen startet med en innkløringsperiode som fortsatte med en probiotikumperiode på to uker og en påfølgende synbiotikumperiode, og ble avsluttet med det som er kjent som en utvaskningsperiode.
Studieskjema og -plan
På slutten av hver periode leverte forsøkspersonene en fecesprøve og en blodprøve (=N).
Mikrober og enzymer ble analysert i fecesprøvene og enterolaktoninnholdet og immunresponsen i blodet.
5.1. Mikrober
Mengde mikrober
Det totale melkesyrebakterieinnholdet, LGG, LC705, det totale propionsyrebaketerieinnholdet, PJS, og det totale bifidobakteriuminnholdet ble bestemt ved anvendelse av fremgangsmåter kjent innen teknikken og parametrene vist i tabell 5.
Effekt på tarmbakteriecelleinnholdet in vivo
Innholdet av LGG, LC705 og PJS økte betydelig i prøver fra forsøkspersoner i løpet av den perioden de anvendte det probiotikuminneholdende produkt (tabell 6). Siden bifidobalcteireinnholdet var høyt helt fra begynnelsen av testen er endringer fortrinnsvis funnet sted bare innenfor artene.
Tilsetting av synbiotikum til produktet konsumert under inntakstesten forbedret levedyktigheten av de tilførte probiotika i tarmen. Dette er vist for eksempel ved økningen i LGG-innhold under den synbiotiske perioden.
Effekt på pH-verdi
pH-nivået hos personer med et initielt pH-nivå over 7 gikk ned i gruppene som inntok den probiotiske blandingen og synbiotikumet, mens ingen nedgang ble observert hos personer med en initiel pH-verdi lavere enn 7 (tabell 7).
5.2. Enzymer
Fecesprøvene ble bearbeidet som beskrevet av Ling et al. (Ling, W-H., Korpela, R., Mykkånen, H., Salminen, S., og Hanninen, 0.1994 Lactobacillus GG supplementation decreases colonic hydrolytic and reductive activities in healthy female adults. Journal of Nutrition 124,18-24).
(3-glukuronidase og P-glukosidase ble bestemt som beskrevet av Freeman (Freeman, H.J. 1986. Effects of differing purified cellulose pectin, and hemicellulose fibre on faecal enzymes in 1,2-dimethyl-hydrazine-induced rat colon carcinogenesis. Cancer Research 46: 5529 - 5532) and urease according to the instructions of the manufacturer (Boehringer Mannheim eat. nr. 542946).
I løpet av testperiodene var det en nedgang i innholdet av P-glukuronidase, urease og P-glukosidase under det probiotiske inntak og det synbiotiske inntak (tabell 8). Etter inntaket gikk enzymnivåene tilbake til det normale. Synbiotikumet hadde en sterkere reduserende effekt på enzymnivåene enn den probiotiske blandingen.
Metabolisme av glukosidase og glukuronidase frembringer karsinogene forbindelser. Den betydelige nedgang i enzymaktivitet frembrakt av den probiotiske og synbiotiske kombinasjon ifølge oppfinnelsen viste tydelig en positiv effekt med hensyn på reduksjon i dannelsen av karsinogene substanser.
5.3. Enterolaktoninnhold
Enterolaktoninnholdet ble bestemt ved anvendelse av fremgangsmåten ifølge Adlercrutz et al. (Aldercreutz, H., Fostis, T., Lampe, J., WåMla, K., MSkelå, T., Brunow, G. og Hase, T. 1993, Quantitative determination of lignans and isoflavonoids in plasma of omnivorous and vegetarian women by isotope dilution gas-chromatography mass-spectrometry. scan J. Clin Lab Invest 53: 5 - 18)
Enterolaktoninnholdet hos forsøkspersonene med et initielt enterolaktonnivå på
<10 nmol/1 økte betydelig som et resultat av det synbiotiske inntak (til et nivå på 11,2).
Ingen endringer ble observert under testen i enterolaktonnivået hos personer der serum enterolaktonnivået var normalt (10< x <30) allerede ved begynnelsen av testen. Resultatene er vist i tabell 9.
Enterolaktoninnholdet er vist å tydelig korrelere med risikoen for å få cancer: jo høyere innhold desto lavere risiko. Dette resultatet viste følgelig også de fordelaktige effekter av de probiotiske og synbiotiske kombinasjoner ifølge oppfinnelsen for en redusert cancerrisiko.
5.4. Immunologiske studier
Effekter på lymfocyttfunksjon
Lymfocyttfunksjon ble studert før inntakt av det synbiotiske produktet startet og 4 uker etter inntaket var startet.
Lymfocyttfunksjonen ble studert som følger:
Lymfocytter ble isolert fra perifert blod ved anvendelse av en Ficoll-gradient. Lymfocyttene ble stimulert med PHA-mitogen (Sigma) i RPMI-dyrkningsmedium (National Public Health Institute; department of nutrient broths) som inneholdt 5% inaktivert AB+ serum (finske røde kors) og L-glutamin. Etter 48 timer ble cellekulturmediet samlet for cytokinbestemmelse fra fire nærliggende kulturbrønner med en celletetthet på 200 000 celler per 200 ul kulturmedium i brønnen, enten med eller uten mitogen. Cellene ble høstet etter 16 timer fra tilsetting av tymidin og inkorporering i DNA (cpm) av radioaktivt tymidin ble målt. Innholdet av cytokinene IL-4, IL-5, TGG- pi og IFN- y ble målt fra cellekulturmediene ved anvendelse av ELISA-metoden.
I løpet av oppfølgingen ble ingen endringer påvist i IL-4-, IL-5- og TGF- pi-innholdet utskilt fra lymfocyttene. IFN- y-innholdet utskilt fra PHA-stimulerte lymfocytter økte betydelig under oppfølgingen (p=0,009, Wilcoxon test, se figur 1). Både spontan og PHA-stimulert proliferering av lymfocytter økte under oppfølgingen (p=0,0002 i begge tilfeller, Wilcoxon test figur 1 og 2).
Ifølge studieresultatene øker følgelig anvendelsen av den synbiotiske produkt proliferering av lymfocyttene og sekresjon av IFN y-cytokin fra forsøkspersonene. IFN-y tilhører det som er kjent som Thl—cytokiner, som styrker cytotoksisk lymfocyttfunksjon og er antagonister av IL-4- og TG- pi-cytokiner. Lav IFN- y-sekresjon er rapportert hos personer disponert for allergi. Barn med en predisposisjon for atopisk eller allergiske reaksjoner er i tillegg antatt å lide av en langsom modning av IFN- y-sekresjon. Den betydelige stimulerende effekt av kombinasjonene ifølge oppfinnelsen på IFN- y-sekresjonen beviser følgelig deres effektivitet ved forebyggelse og behandling av allergier.
Claims (11)
1.
Kombinasjon, karakterisert ved at den omfatter mikroorganismene Lactobacillus rhamnosus LGG (ATCC 53103), Lactobacillus rhamnosus LC705 (DSM 7061), Propionibacterium freudenreichii ssp. shermanii PJS (DSM 7067) og en bifidobakterie.

Kombinasjon ifølge krav 1,karakterisert ved at bifidobakterien er Bifidobacterium infantis Bbi99 (DSM 13692).
