Description
Foreliggende oppfinnelse angår nye anvendelser av fenylcykloheksylkarboksylsyreamider, nye stoffer, fremgangsmåte for deres fremstilling, legemiddel inneholdende disse, samt deres anvendelse for fremstilling av legemiddel for behandling av restenose.
NO 300810 beskriver heterocykliske-substituerte fenyl-cykloheksan-karboksylsyrederivater og deres anvendelse i legemidler, spesielt som blodtrykkssenkende og antiaero-sklerotiske midler.
Det er funnet at fenylcykloheksylkarboksylsyreamid med formelen (Ia),
og dens farmasøytisk akseptable salter overraskende bevirker en sterk hemming av proliferasjon av glatte muskeceller og således er egnet for anvendelse av bekjemping av restenose.
Fenylcykloheksylkarboksylsyreamidene kan ifølge oppfinnelsen også foreligge i form av deres farmasøytisk akseptable salter. Generelt kan her nevnes salter med organiske eller uorganiske baser eller syrer.
Innen rammen av foreliggende oppfinnelse kan de fysiologisk akseptable salter av oppfinnelsens forbindelser være salter av oppfinnelsens stoffer med mineralsyrer, karboksylsyrer eller sulfonsyrer. Spesielt foretrukket er f.eks. salter med saltsyre, hydrogenbromid, svovelsyre, fosforsyre, metan-sulfonsyre, etansulfonsyre, toluensulfonsyre, benzensulfon-syre, naftalindisulfonsyre, eddiksyre, propionsyre, melke-syre, vinsyre, sltronsyre, fumarsyre, malelnsyre eller benzosyre.
Fysiologisk akseptable salter kan også være metall- eller ammoniumsalter av oppfinnelsens forbindelser som har en fri karboksylgruppe. Spesielt foretrukket er f.eks. natrium-, kalium-, magnesium- eller kalsiumsalter, såvel som ammoniumsalter, som har avledet ammoniakk eller organiske aminer, som eksempelvis etylamin, di- hhv. trietylamin, di- hhv. trietanolamin, dicykloheksylamin, dimetylaminoetanol, arginin, lysin, etylendiamin eller 2-fenyletylamin.
Forbindelsene kan ifølge oppfinnelsen foreligge i stereo-isomere former, som enten forholder seg som bilde og speilbilde (enantiomerer), eller ikke forholder seg som bilde og speilbilde (diastereomerer). Oppfinnelsen angår såvel enantiomerene eller diastereomerene som også deres blandinger. Racematformene lar seg også på kjent måte skille i de diastereomere enhetlige bestanddelene.
Oppfinnelsen angår dessuten nye forbindelser med den generelle formel (Ia) kjennetegnet ved at de er diastereomere med en Rf-verdi på 0,35 (diklormetanrmetanol:ammoniakk-9:1:0,1) og deres salter.
Forbindelsene med den generelle formel (Ia) kan bli fremstilt ved at man forsåper forbindelsene med den generelle formel

hvor
R<3> står for rettkjedet eller forgrenet C^-C^alkoksy,
og omsetter syrene, eventuelt under foregående aktivering, i inerte oppløsningsmidler, eventuelt i nærvær av en base og/eller et dehydratiseringsmiddel med fenylglycinderivater med den generelle formel (III)
og etterfølgende gjennomfører en diastereomer adskillelse.
Egnet som oppløsningsmiddel for fremgangsmåten er vanlige organiske oppløsningsmidler som ikke forandrer seg under reaksjonsbetingelsene. Til disse hører fortrinnsvis eter som dietyleter, dioksan, tetrahydrofuran, glykoldimetyleter, eller hydrokarboner som benzen, toluen, xylen, heksan, cykloheksan eller jordoljefraksjoner, eller halogenerte hydrokarboner som diklormetan, triklormetan, tetraklormetan, dikloretylen, trikloretylen eller klorbenzen, eller eddikester, trietylamin, pyridin, dimetylsulfoksyd, dimetylforma-mid, heksametylfosforsyretriamid, acetonitril, aceton eller nitrometan. Det er likeledes mulig å benytte blandinger i nevnte oppløsningmidler. Foretrukket er diklormetan og tetrahydrofuran.
Som baser for oppfinnelsens fremgangsmåter kan generelt uorganiske eller organiske baser bli anvendt. Til disse hører fortrinnsvis alkalihydroksyder som for eksempel natrium-hydroksyd eller kaliumhydroksyd, jordalkalihydroksyd som for eksempel bariumhydroksyd, alkalikarbonat som natriumkarbonat, kaliumkarbonat eller cåsiumkarbonat, jordalkalikarbonat som kalsiumkarbonat eller alkali- eller jordalkalialkoholater som natrium- eller kaliummetanolat, natrium- eller kaliumetanolat eller kalium-tert.butylat, eller organiske aminer (trial-kyl (C^-C^j )amin) som trietylamin, eller heterocykler som 1,4-diazabicyklo[2.2.2]oktan (DABCO), 1,8-diazabicyklo[5.4.0]-undec-7-en (DBU), pyridin, diaminopyridin, metylpiperidin eller morfolin. Det er også mulig som baser å tilsette alkalimetaller som natrium eller deres hydrider som natriumhydrid. Foretrukket er natriumhydrid, kaliumkarbonat, trietylamin, pyridin og kalium-tert.butylat.
Generelt tilsetter man basene i en mengde fra 0,05 mol til 10 mol, fortrinnsvis fra 1 mol til 2 mol for hvert mol av forbindelsen med formel (III).
Fremgangsmåtene blir generelt gjennomført i et temperatur-område fra -50°C til +100°C, foretrukket fra -30°C til +60°C.
Fremgangsmåtene blir generelt gjennomført ved normaltrykk. Det er også mulig å gjennomføre fremgangsmåtene ved overtrykk eller ved undertrykk (f.eks. i et området fra 0,5 til 5 bar).
Amideringen kan eventuelt forløpe over det aktiverte syrehalogenid- eller blandede anhydridtrinnet, som kan bli fremstilt med den tilsvarende syren ved omsetning med tionylklorid, fosfortriklorId, fosforpentaklorid , fosfor-tribromid eller oksalylklorid eller metansulfonsyreklorid.
Egnet som baser ved siden av de ovenfor anvendte basene er fortrinnsvis trietylamin og/eller dimetylaminopyridin, DBU eller DABCO.
Basen blir tilsatt i en mengde fra 0,5 mol til 10 mol, fortrinnsvis fra 1 mol til 5 mol, for hvert mol av forbindelsene med den generelle formel (III).
Som syrebindende middel for amideringen kan alkali- eller jordalkalikarbonater som natriumkarbonat, kaliumkarbonat, alkali- eller jordalkalihydroksyder som eksempelvis natrium-eller kaliumhydroksyd, eller organiske baser som pyridin, trietylamin, N-metylpiperidin, eller bicykliske amidiner som 1,5-diazabicyklo-[4.3.0]-nonen-5 (DBN) eller 1,5-diazabicyk-lo[5.4.0]undecen-7 (DBU), bli tilsatt. Foretrukket er trietylamin.
Egnede dehydratiseringsreagenser er karbodiimider som eksempelvis di-isopropylkarbodiimim, dicykloheksylkarbodiimid eller N-(3-dimetylaminopropyl).N'-etylkarbodiimid-hydroklor eller karbonylforbindelser som karbonyldiimiazol eller 1,2-oksazoliumforbindelser som 2-etyl-5-fenyl-l,2-oksazolium-3-sulfonat eller propanfosfonsyreanhydrid eller isobutylkloro-format eller benzotriazolyloksy-tris-(dimetylamino)fosfonium-heksafluorfosfat eller fosforsyredifenylesteramid eller metansulfonsyreklorid, eventuelt i nærvær av baser som trietylamin eller N-etylmorfolin eller N-metylpiperidin eller dicykloheksylkarbodiimid og N-hydroksysuksinimid.
Det syrebindende midlet og dehydratiseringsreagenser blir generelt tilsatt i en mengde fra 0,5 til 3 mol, foretrukket fra 1 til 1,5 mol for hvert mol av den tilsvarende karboksyl-syren.
Overføringen til de tilsvarende fenylglycinamidoamider skjer med alkoholisk ammoniakk, fortrinnsvis med ammoniakk mettede alkoholer, som eksempelvis metanol eller etanol, fortrinnsvis metanol, ved romtemperatur.
Forbindelsene med den generelle formel (III) er kjente eller kan bli fremstilt ifølge vanlige fremgangsmåter.
Forbindelsene med den generelle formel (II) kan bli fremstilt ved omsetning av forbindelser med formel (IV)

Forbindelsene med den generelle formel (Ia) viser overraskende et ikke forutsebart, verdifullt farmakoligsk virkespektrum.
De hemmer proliferasjonen av glatte muskelceller og kan derfor benyttes for behandling av restenose.
De nye stoffene kan dessuten bli benyttet for behandling av arteriosklerose.
Undersøkelse for hemming av prollferas. ion av glatte muskelceller ved oppfinnelsens forbindelser
For bestemmelse av den antiproliferative virkningen av forbindelsene ble det anvendt glatte muskelceller som var utvunnet fra aorta fra svin ved media-eksplantat-teknikken [R. Ross, J. Cell. Biol. 50. 172, 1971]. Cellene ble utsådd i egnede kulturskåler, som regel 96-brønners plater, og dyrket i 2 til 3 dager i medium 199 med 7, 5% FCS og 7, 5% NCS, 2 mM L-glutamin og 15 mM HEPES, pH 7,4 i 5% C02 ved 37°C. Deretter ble cellene synkronisert ved serumfjerning i 2 til 3 dager og så tilsatt serum eller andre vekstfaktorer. Samtidig ble testforbindelsene tilsatt. Etter 16 til 20 timer ble <3>H-tymidin tilsatt og etter ytterligere 4 timer ble innbyggingen av dette stoffet i den TCA-fellbare DNA i cellene, bestemt. For bestemmelse av IC5Q-verdier ble den virkestoffkonsentra-sjonen beregnet som ved sekvensiell fortynning av virkestoffet ga en halvmaksimal hemming av den ved 10$ FCS fremkalte tymidininkorporasjon.
Til sammenligning har den kjente (NO 300810) forbindelsen trans-2-[4-[(6-cyklopropyl-imidazo[2,3-b]pyridin)-7-yl-metyl] f enyl] -cykloheksan-l-karbonyl- ( f enylglycinamido )amid en I<C>50-verdi på 7 nM;
Undersøkelser av hemmingen av c- fos- genekspres. 1onen i glatte muskelceller ved oppfinnelsens forbindelser
Den antiproliferative virkningen av forbindelsene ble undersøkt med hensyn til den serum- og vekstfaktorformidlende signaltransduksjonen og induksjonen av c-fos-genekspresjon i glatte muskelcelle-reporterlinjer. Som reporter tjener her luciferase, hvis ekspresjon blir styrt av den humane c-fos-promotoren. C-fos-promotor/luciferasekonstruktet er stabilt integrert i det kromosomale DNA i rotte-glattmuskelcellelinje A 10 (ATCC CRL 1476). Reportercellene ble utsådd i 96-brønners plater og dyrket i 1-2 dager i serum-holdig medium (D-MEM med 1096 FCS, 2 mM L-glutamin og 15 mM HEPES, pH 7,4) i 5$ COg ved 37°C. For undertrykkelse av c-fos-promotoraktivi-teten til basalverdien, ble cellene stanset ved serumsug i 24 timer. Deretter ble testforbindelsene tilsatt og cellene ble stimulert med FCS eller vekstfaktorer for induksjon av luciferaseaktivitet. Etter denne behandlinsperioden (4 timer) ble cellene lysert og deres ekstrakter benyttet for lucifera-sebestemmelse. IC5Q-verdien ble beregnet fra den virkekonsen-trasjonen som ved sekvensiell fortynning av virkestoffet bevirket den halvmaksimale hemmingen av den ved stimuliet fremkalte luciferaseaktiviteten.
In- vivo- undersøkelser for hemming av den vaskulaere glatt-muskelcelle- proliferas. ionen i luftperfunderte rottecarotis-modellen
In-vivo-undersøkelser for hemming av den vaskulaere glattmuskelcelleproliferasjonen i den luftperfunderte rottecaro-tis-modellen, skjedde ifølge den lett modifiserte fremgangsmåten til Fishman et al. (Lab. Invest. 32. 339-351, 1975); operasjonen av dyrene skjedde under Nembutal<R->anestesi. Den høyre arteria carrotis communis ble frilagt og to karklemmer tatt på med en caudal til cranial avstand på ca. 1,5 cm. En kanyle ble innført i den craniale enden av dette karsegmen-tet, den caudale enden ble perforert med stikk av en nål. Etter spyling med fysiologisk koksaltoppløsning ble en luftstrøm (25 ml/min. i 4 min.) perfundert gjennom segmentet. Deretter ble klemmene fjernet, blødningen stanset under lett trykk og operasjonsfeltet ble lukket med sårklemmer. Dyrene ble avlivet åtte dager etter operasjonen, og det ble fra såvel de på forhånd luftperfunderte, som også kontrollene, tatt ut tilsvarende kontralaterale carotidsegmenter.
Applikasjonen av teststoffene (p.o., i.v., i.p., s.c.) ble startet to dager før operasjonen, behandlingen fulgte så over den totale forsøkstiden (behandlingstid totalt: 10 dager).
Den luftinduserte glattmuskelcelleproliferasjonen ble målt ved bestemmelse av DNA-innholdet i carotidsegmentet ifølge Helms et al. (DNA 43, 39-49, 1985). Deretter ble vevsstykket enzymatisk fordøyet med proteinkinase K, DNA ble isolert og bisbenzimidet ble fluorometrisk bestemt (DNA fra sildespermer som standard). DNA-innholdet i karet ble til sist angitt i jjg DNA pr. mm carotid.
For bestemmelse av den antiproliferative virkningen av oppfinnelsens forbindelser, fikk rottene et ballongkateter innført i halspulsåren, dette ble oppblåst og innersiden av blodkaret skadet ved bevegelser av kateteret [Clowes A.W., et al., Lab. Invest, Vol. 49, nr. 3, s. 327, 1983]. Denne skaden bevirket en neointimal glattmuskelproliferasjon, som forårsaker stenoser. Graden av karinnsnevring hos dyrene ble bestemt etter ca. 2 uker ved histologisk opparbeidelse av blodkaret, ved at kartverrsnittet i proliferasjonsvevets flate ble målt.
Det nye virkestoffet kan på kjent måte bli overført i vanlige formuleringer som tabletter, drageer, piller, granulater, aerosoler, siruper, emulsjoner, suspensjoner og oppløsninger under anvendelse av inerte, ikke-toksiske, farmasøytisk egnede bærestoffer eller oppløsningsmidler. Her skal den terapeutisk virksomme forbindelsen alltid være tilstede i en konsentrasjon fra omkring 0,5 til 90 vekt-# av totalblandin-gen, dvs. i mengder som er tilstrekkelig til å oppnå det angitte doseringsspillerom.
Formuleringene kan eksempelvis bli fremstilt ved blanding av virkestoffet med oppløsningsmidler og/eller bærestoffer, eventuelt under anvendelse av emulgeringsmidler og/eller dispergeringsmidler, hvorved f.eks. det ved anvendelse av vann som fortynningsmiddel eventuelt også kan bli anvendt organisk oppløsningsmiddel som hjelpeoppløsningsmiddel.
Applikasjonen skjer på vanlig måte, fortrinnsvis oralt eller parenteralt, spesielt perlingualt eller intravenøst.
For parenteral anvendelse kan oppløsninger av virkestoffet bli gitt under anvendelse av egnede flytende bærematerialer.
Generelt har det vist seg fordelaktig ved intravenøs applikasjon å gi mengder fra omkring 0,001 til 20 mg/kg, fortrinnsvis omkring 0,01 til 5 mg/kg kroppsvekt for oppnåelse av et virksomt resultat, og ved oral applikasjon er doseringen omkring 0,01 til 50 mg/kg, fortrinnsvis 1 til 10 mg/kg kroppsvekt.
Likevel kan det være nødvendig å avvike fra de nevnte mengdene, alt avhengig av kroppsvekten hhv. typen applika-sjonsvei, individuelle forhold overfor medikamentet, typen formulering og tidspunktet hhv. intervallet ved hvilken leveringen ble gitt. Det kan således i noen tilfeller være tilstrekkelig med mindre enn de ovenfor nevnte minstemeng-dene, mens det i andre tilfeller kan være nødvendig å overskride den øvre grensen. Ved applikasjon av større mengder kan det være anbefalelsesverdig å gi denne i flere enkeltdoser i løpet av dagen.

Utgangsforbindelser
De følgende eksemplene I-III angir utgangsforbindelser for fremstilling av forbindelser som ikke er en del av foreliggende oppfinnelse. Eksemplene illustrerer derfor kun en mulig fremstillingsfremgangsmåte.
EKSEMPEL I
2-isopropyl-benzimidazol
6,54 g (60 mmol) 2,3-diaminopyridin, 5,6 g (60 mmol) isosmørsyre og 45 ml polyfosforsyre ble bragt sammen og oppvarmet i 6,5 time til 130°C. Etter 6,5 timer ble det tilsatt 0,4 1 isvann, bragt til pH 7 med fast NaOH, tilsatt NagCC^ til avslutning av gassutviklingen og tre ganger ekstrahert med eddikester. De sammenslåtte organiske fasene ble tørket over NagSC^ og inndampet.
Utbytte: 8,45 g (87 av teoretisk)

EKSEMPEL II
Trans-2-[4-(2-i s opr opy 1 b enz im i dazolyl - lH-me tyl ) f enyl] - cykloheksan-1-karboksylsyre-tert.butylester
0,75 g NaH (25 mmol, 80%) ble suspendert i 50 ml DMF, avkjølt til -10°C og ved maks. 0°C dråpevls tilsatt en oppløsning av 4,03 g (25 mmol) 2-isopropylbenzimidazol i 60 ml DMF. Etter 15 min. omrøring ble det ved maks. 0°C tilsatt en oppløsning av trans-2-(p-brommetylfenyl)-cykloheksan-l-karboksylsyre-tert.butylester i 110 ml DMF. Etter 2,5 timers omrøring ved RT, ble det tilsatt 25 ml 1 N HOAc og inndampet. Resten ble skilt i eter/vann, den vandige fasen ekstrahert enda to ganger, de sammenslåtte organiske fasene tørket over Na2S04, inndampet og produktet renset over kieselgel 60 (PE/EE = 3/7).
Utbytte: 2,91 g (28$ av teoretisk)

EKSEMPEL III
Trans-2-[4-( 2-isopropylbenzimidazolyl-lH-metyl)-fenyl]-cykloheksankarboksylsyre
2,91 g (6,7 mmol) av forbindelsen fra eksempel II ble omrørt i 9 ml CH2C12 med 9 ml trifluoreddiksyre ved RT i 2,5 time. Etter inndampingen ble to ganger tatt opp i eter og på nytt inndampet. Resten ble skilt i eter/H20 (pH 10), den vandige fasen gjort sur, ekstrahert igjen tre ganger og de sammenslåtte organiske fasene ble to ganger ristet med H20/NH4C1, to ganger med H20, en gang med H20/NaHC03 og enda en gang med H2O/NH4CI, deretter tørket over Na2S04 og inndampet.
Utbytte: 2,2 g (86$ av teoretisk)

De følgende eksemplene 1-3 beskriver fremgangsmåten for fremstilling av forbindelser som ikke utgjør en del av foreliggende oppfinnelse. Eksemplene 4 og 5 som er forbindelser ifølge oppfinnelsen blir fremstilt analogt hermed.
EKSEMPEL 1
Trans-2-[4-( 2 - i s op r opy 1 b enz im i dazolyl -lH-metyl )fenyl] - cykloheksan-1-karbonyl-(fenylgycinmetylester)amid
0,55 g (1,46 mmol) av forbindelsen fra eksempel III, ble under argon ved -30 ° C oppløst i 12 ml DMF tilsatt 0,81 ml (5,82 mmol) trietylamin og 0,12 ml (1,6 mmol) mesylklorid og omrørt i 1 time. Deretter ble det tilsatt en oppløsning av 0,35 g (1,7 mmol) fenylglycinmetylester, 0,18 g (1,46 mmol) N,N-dimetylaminopyridin og 0,24 ml (1,72 mmol) trietylamin, oppløst i 10 ml DMF, dråpevis og over natten bragt til RT under omrøring. Deretter ble det tilsatt H20 og tre ganger ekstrahert med eddikester. De sammenslåtte organiske fasene ble tørket over Na2S04, inndampet og produktet kromatografert over kieselgel 60 (CH2Cl2/MeOH = 20/1).
Utbytte: 0,37 g (48* av teoretisk)
Rf = 0,51 (CH2Cl2/MeOH = 95/5)
EKSEMPEL 2 og 3
Trans-2-[4-(2-i sopropy 1 benz imi dazolyl -lH-metyl )f enyl ] - cykloheksan-l-karbonyl-(fenylglycinamido)amid
0,33 g (0,64 mmol) av forbindelsen fra eksempel 13 ble oppløst i 1 ml MeOH og omrørt med 4 ml NB^-mettet MeOH i 5 dager ved RT. Deretter ble bunnfallet skilt fra og oppløs-ningen inndampet. Resten ble renset på kieselgel 60 (CH2C12-/MeOH/NH3 = 100/5/0,3).
Utbytte:

Anvendelse av en forbindelse med formel (Ia)
og dens farmasøytisk akseptable salter for fremstilling av legemidler til behandling av restenose.
2.
Diastereomere fenylcykloheksylkarboksylsyreamider av formelen (Ia) ifølge krav 1,karakterisert ved at de er diastereomere med en Rf-verdi på 0,35 (diklormetan:metanol:ammoniakk - 9:1:0,1) og deres salter.
3.
Fremgangsmåte for fremstilling av fenylcykloheksylkarboksylsyreamider ifølge krav 2, karakterisert ved at man forsåper forbindelser av den generelle formel (Ila) i hvilke R<3> står for rettkjedet eller forgrenet C^-C4~alkoksy og omsetter syren, eventuelt under foregående aktivering, i inerte oppløsningsmidler, eventuelt i nærvær av en base og/eller et dehydratiseringsmiddel, med fenylglycinderivater av formelen (III)
og etterfølgende gjennomføre en diastereomer adskillelse.

Legemiddel, karakterisert ved at det inneholder fenylcykloheksylkarboksylsyreamider ifølge krav 2.
