Description
Foreliggende oppfinnelse angår fremgangsmåter ved fremstilling av farmasøytiske preparater inneholdende kalsitonin for intranasal tilførsel.
Spesielt vil slike blandinger være fri for preserverings-midler og overflateaktive midler.
Enda mer spesielt kan slike farmasøytiske blandinger for intranasal tilførsel omfattende kalsitonin - fortrinnsvis laksekalsitonin - være oppløst i fysiologisk saltvann justert til pH 3,5-4,5 ved hjelp av saltsyre.
Kalsitoniner er kjent for å være av grunnleggende viktighet ved behandling av osteoporose, Pagets lidelse, hyperkalemi og lignende patologiske tilstander. Kalsitoniner er også kjent for lett å undergå nedbrytning grunnet polypeptid-naturen av disse og som en følge derav kan de ikke tilføres via oral vei. På den annen side gir parenteral tilførsel av kalsitoniner ved injeksjon bemerkelsesverdige ulemper, spesielt smertereaksjoner hos pasienten.
Intranasal tilførsel av polypeptidlegemidler er også bredt beskrevet i litteraturen, se f.eks. Felber et al.,
Experientia, 1969, s.1195; Genser et al., Lancet, 1974, s. 865; Greenberg et al., Antimicrobial A<g>ents and Chemo-therapv, 1978, s. 596; Bergquist et al., Lancet, 1979, s. 215; Pontiroli et al., British Medical Journal, 1982, s.303, ved siden av et antall patenter eller patentsøknader som angår intranasal tilførsel av insulin (EP 94157), vaso-pressin (japansk patentsøknad 55066-517, 55055-120), polypeptider av forskjellig type (tyske patenter 2.256.445 og 2.758.483, EP 252, belgisk patent 860.717, sør-afrikansk patent 68/4241).
Intranasal tilførsel av kalsitonin er i sin tur beskrevet i forskjellige patenter. Således krever italiensk patent 1.172.324 galeniske blandinger omfattende kalsitonin som er særpreget ved at de inneholder, ved siden av den aktive be-standdel, benzalkoniumklorid eller et overflateaktivt middel, spesielt et ikke-iohisk overflateaktivt middel eller begge av ovennevnte additiver. Høyere alkanoler eller sterol polyoksyalkylenetere er spesielt krevet som overflateaktive midler.
Både overflateaktive midler og benzalkoniumklorid er angitt å være nødvendige bestanddeler for å gi en god kalsitonin biotilgjengelighet, dvs. å sikre effektiv adsorpsjon derav av nasal mucosa, idet benzalkoniumklorid også ville virke som et preserveringsmiddel mot patogener eller uønskede mikroorganismer.
Lignende blandinger som også inneholder et kvarternert ammoniumsalt (benzalkoniumklorid, cetyltrimetylammonium-bromid) er krevet i EP-A-193.372.
Japansk patentsøknad 61126-034 beskriver i sin tur nasale blandinger av kalsitonin inneholdende absorpsjonsøkende midler, såsom glukose og/eller glukosamin, idet behovet for absorpsjonshjelpemiddel også er understreket i japansk patentsøknad 61118-325, som foreslår for dette formål tilsetnng av aminosyrer, og i EP-A-183.527 som i motsetning til dette antyder bruken av benzylalkohol, etanol, salisylsyre, capronsyre, polyetylenglykol og lignende som absorp-sjonsøkende midler. Videre krever EP-A-111.841 og 115.627 intranasale blandinger som inneholder kalsitonin i blanding med overflateaktive midler blant hvilke biliære syrer og benzalkoniumklorid er nevnt.
Ikke desto mindre er tilstedeværelsen av sistnevnte - og generelt av kvarternere ammoniumsalter - ikke tilfredsstil-lende grunnet de mulige uønskede effekter derav
(Am J Ophtamol-1988, 105 (6) s 670-3; Contact Dermatitis-1987, 17 (1) s. 41-2; Cutis-1987, 39 (5) s.381-3) . Følgelig ville det være bedre å fremskaffe intranasale blandinger av kalsitonin som er fri for nevnte salter, men ifølge enkelte av de ovenfor nevnte dokumenter ville slike blandinger lide av dårlig biotilgjengelighet. Alle patentdokumentene angitt ovenfor er fullstendig enige om behovet for å øke nevnte biotilgjengelighet ved hjelp av overflateaktive midler av forskjellig natur som fremmer kalsitoninabsorpsjon gjennom nasal mucosa.
Følgelig er søkers funn svært overraskende at intranasale blandinger av kalsitonin inneholdende overflateaktive midler av forskjellig type eller også midler som har til hensikt å øke absorpsjon av den nasale mucosa, og blandinger inneholdende kalsitonin alene i en i hovedsak fysiologisk vandig oppløsning av natriumklorid, praktisk talt ikke viser noen forskjell i biotilgjengelighet eller preser-veringsegenskaper.
Følgelig er et mål for foreliggende oppfinnelse gitt ved en fremgangsmåte for fremstilling av farmasøytiske preparater for intranasal tilførsel hvor preparatet fremstilles som angitt i krav l's karakteriserende del og fortrinnsvis omfatter: a) en terapeutisk effektiv mengde av kalsitonin oppløst i b) en omkring 0,9% NaCl-opløsning i renset vann justert til pH 3,5-4,5.
Mer foretrukket omfatter oppfinnelsen fremgangsmåter ved fremstilling av blandinger omfattende: a) en terapeutisk effektiv mengde av laksekalsitonin oppløst i b) en omkring 0,9% NaCl-oppløsning i renset vann
justert til pH 3,5-4,5 ved hjelp av saltsyre.
Videre inneholder blandingene fremstilt ifølge oppfinnelsen fortrinnsvis 100-5000 I.U., fordelaktig 250-2000 I.U., kalsitonin pr. ml blanding.
Fordelen ved blandingene fremstilt ifølge oppfinnelsen sammenlignet med dem ifølge tidligere teknikk er innlysen-de, ikke bare siden det er mulig å unngå kvarternere ammoniumsalter, men også fordi tilsteningen av de opptil nå antydede kalsitoninabsorpsjonsøkende midler også kan bli unngått.
Nevnte midler utgjør faktisk en økning i kostnader av utgsangsmaterialer og arbeid.
Preparatene fremstilt ifølge oppfinnelsen ble sammenlignet med blandinger fremstilt i henhold til tidligere teknikk, spesielt den farmaco-kinetiske følgende intranasale tilfør-sel av samme dosering (100 I.U.) av laksekalsitonin (salcatonin) i blandingene A og B i 12 pasienter hver gang: Salcatonin plasmakonsent ras joner (12 pasienter gjennomsnitt ± standardavvik) ved forskjellige tilføringstider er angitt i den følgende tabell:
AUC (areal under kurve) er 1663 i tilfelle A og 2108 i tilfelle B. T/2S er henholdsvis 12,2 minutter og 11,7 minutter.
Forskjellen mellom de to kurvetyper er ikke signifikant.
Fullstendig analoge resultater erholdes ved å sammenligne blandingene fremstillt ifølge oppfinnelsen med blandinger inenholdende istedet for benzalkoniumklorid en kolesterol polyoksyetyleneter eller henholdsvis etanol, salisylsyre og natriumtaurokolat.
De følgende eksempler illustrerer oppfinnelsen i større detalje.
EKSEMPEL 1
Salcatonin (500000 I.U.) oppløses i 850 ml rent vann inneholdende 9 g analytisk rent natriumklorid, pH justeres til 4,0 ved hjelp av analytisk ren IN HC1 og oppløsningen fortynnes til 1000 ml ved å tilsette renset vann. Oppløs-ningen filtreres på et 0,2 fixa filter og fordeles i 5 mis beholdere som beskrevet i de medfølgende figurer, utstyrt med en doseringsanordning som avleverer 0,1 mis doser hver gang, lik 50 I.U. salcatonin.
EKSEMPEL 2
Fremgangsmåten fra eksempel 1 følges, men ved å bruke en dobbel mengde av salcatonin (1000000 I.U.). Oppløsningen fordeles i egnede 2,5 mis beholdere som beskrevet i de medfølgende figurer som avleverer 0,1 mis doser hver gang, lik 100 I.U. salcatonin.
Claims (7)
1. Fremgangsmåte ved fremstilling av et farmasøytisk preparat inneholdende kalsitonin for intranasal tilførsel,karakterisert vedat en farmasøytisk akseptabel mengde kalsitonin oppløses i en natriumklorid-oppløsning i rent vann, hvorpå pH av den resulterende oppløsning justeres til pH 3,5 - 4,5, eventuelt hvor pH justeres i den vandige oppløsning før tilsetning av den farmasøytiske mengde kalsitonin.
2. Fremgangsmåte ifølge krav 1,karakterisert vedat kalsitonin tilsettes til en konsentrasjon på 100 - 5.000 I.U. pr. ml. blanding.
3. Fremgangsmåte ifølge krav 2,karakterisert vedat kalsitonin tilsettes til en konsentrasjon på 250 - 2.000 I.U. pr. ml. blanding.
4. Fremgangsmåte ifølge ethvert av de foregående krav,karakterisert vedatpH justeres til pH 3,5 - 4,5 ved tilsetning av saltsyre.
5. Fremgangsmåte ifølge ethvert av de foregående krav,karakterisert vedat kalsitoninet som tilsettes er lakse-kalsitonin (salcatonin).
