Description
Foreliggende oppfinnelse angår anvendelse av en kombinasjon av to typer forbindelser for fremstilling av et farmasøy-tisk preparat for gastrointestinale forstyrrelser. Mer spesielt angår den anvendelse av en kombinasjon av et salt dannet mellom ranitidin og et kompleks av vismut med en kar-boksylsyre, med antibiotika.
I vår publiserte GB-patentsøknad med publikasjonsnr. 2220937A har vi beskrevet og krevet beskyttelse for salter dannet mellom ranitidin og et kompleks av vismut med en kar-boksylsyre, særlig vinsyre, og mer spesielt, sitronsyre. Et salt av denne type er N-[2-[[[5-[(dimetylamino)metyl]-2-furanyl]metyl]tio]etyl]-N ' -metyl-2-nitro-l,l-etendiamin-2-hydroksy-1,2,3-propantrikarboksylat-vismut-(3<+>)-kompleks, også kjent som ranitidin-vismut-citrat. Saltene omtalt i GB-patent-søknad med publikasjonsnr. 2220937A har de H2-antagonist-antisekretoriske egenskaper som forbindes med ranitidin, sammen med antibakteriell aktivitet mot Helicobacter pylori (tidligere Campylobacter pylori). Dessuten har slike salter cellebeskyttende egenskaper og oppviser aktivitet overfor humant gastrisk pepsin med hemmende preferanse overfor pepsin 1, et pepsin-isozym som forbindes med mavesår. Saltene omtalt i GB-patentsøknad med publikasjonsnr. 2220937A har således en særlig fordelaktig egenskapskombinasjon for behandling av gastrointestinale forstyrrelser, spesielt mavesår og andre gastroduodenale tilstander, for eksempel gastritt og ikke-ulcerøs dyspepsi.
Vi har nå vist at den antibakterielle aktivitet av ranitidin-vismut-citrat mot Helicobacter-organismer kan for-bedres betydelig ved kombinasjons-administrering av forbindelsen med ett eller flere antibiotika.
Foreliggende oppfinnelse tilveiebringer således i henhold til ett aspekt, anvendelse av (i) ranitidin-vismut-citrat og (ii) ett eller flere Helicobacter pylori-hemmende antibiotika ved fremstillingen av et farmasøytisk preparat for bruk ved behandling eller forebyggelse av gastrointestinale forstyrrelser.
Uttrykket "gastrointestinal forstyrrelse" skal her omfatte en sykdom eller annen forstyrrelse av den gastrointestinale trakt, inkludert for eksempel en forstyrrelse som ikke har manifestert seg ved forekomst av ulcerasjoner i den gastriske slimhinne (f.eks. gastritt, ikke-ulcerøs dyspepsi, refluksøsofagitt og gastriske motilitetsforstyrrelser), samt mavesår (f.eks. gastrisk- og duodenalsår).
Ranitidin-vismut-citratet kombinasjons-administreres fortrinnsvis med ett eller to antibiotika, men i spesielt vanskelige tilfeller kan det være nødvendig med tre antibiotika.
Et bredt utvalg av antibiotika kan benyttes i henhold til oppfinnelsen, innbefattet for eksempel nitroimidazol-antibiotika (f.eks. tinidazol eller metronidazol), tetra-cykliner (f.eks. tetracyklin, doxycyklin og minocyklin), penicilliner (f.eks. amoxicillin, ampicillin og mezlocillin), cefalosporiner (f.eks. cefachlor, cefadroxil, cephradine, cefuroxim, cefuroxim axetil, cefalexin, cefpodoxime proxetil, ceftazidim og ceftriaxone), karbopenemer (f.eks. imipenem og meropenem), aminoglykosider (f.eks. paromonycin), makrolid-antibiotika (f.eks. erytromycin, clarithromycin og azithromycin), lincosamid-antibiotika (f.eks. clindamycin), 4-kinoloner (f.eks. ofloxacin, ciprofloxacin, pefloxacin og norfloxacin), rifamyciner (f.eks. rifampicin), nitrofurantoin og derivater av 10-(1-hydroksyetyl)-11-okso-l-azatricyklo-[7.2.0.0.3.8]undek-2-en-2-karboksylsyre beskrevet i publisert europeisk patentsøknad med publikasjonsnr. 0416953 og publisert internasjonal patentsøknad med publikasjonsnr. W092/03437.
Antibiotika som kan administreres oralt er i alminnelighet foretrukket. Eksempler er metronidazol, tetracyklin (spesielt som tetracyklin-hydroklorid), amoxicillin, cefuroxim axetil og clarithromycin.
Foretrukne kombinasjoner innbefatter ranitidin-vismut-citrat og metronidazol; ranitidin-vismut-citrat og tetracyklin; ranitidin-vismut-citrat og cefuroxim axetil; ranitidin-vismut-citrat og amoxicillin; ranitidin-vismut-citrat og clarithromycin; ranitidin-vismut-citrat, metronidazol og amoxicillin; ranitidin-vismut-citrat, metronidazol og tetracyklin; og ranitidin-vismut-citrat, tetracyklin og clarithromycin.
Kombinasjoner av ranitidin-vismut-citrat og cefuroxim og ranitidin-vismut-citrat, cefuroxim og metronidazol kan også være å foretrekke.
Særlig foretrukne kombinasjoner omfatter ranitidin-vismut-citrat kombinasjons-administrert med enten metronidazol, tetracyklin, metronidazol og amoxicillin, eller metronidazol og tetracyklin. Slike kombinasjoner har vist en synergistisk antibakteriell effekt mot Helicobacter pylori in vitro og mot Helicobacter mustelae in vivo i fritte (ilder). I kombinasjonsundersøkelser in vivo har således kombinasjons-administrasjon av ranitidin-vismut-citrat med ett eller to av ovennevnte antibiotika ført til en antibakteriell aktivitet mot Helicobacter mustelae som er bedre enn aktiviteten som de enkelte virksomme ingredienser oppviser hver for seg, eller, når det er tale om kombinasjons-administrasjon med to antibiotika, den som oppvises av de to antibiotika sammen.
Ranitidin-vismut-citrat og det ene eller de flere antibiotika kan derfor administreres sammen i form av separate farmasøytiske preparater for simultan og/eller sekvensiell bruk. Alternativt kan ranitidin-vismut-citratet og antibiotikaene administreres som et enkelt farmasøytisk preparat for oral anvendelse og som omfatter effektive mengder av virkestoffene.
I henhold til et ytterligere aspekt, tilveiebringer oppfinnelsen således et farmasøytisk preparat som inneholder (i) ranitidin-vismut-citrat og (ii) ett eller flere Helicobacter pylori-hemmende antibiotika som et kombinert preparat for simultan, separat eller sekvensiell bruk ved behandling eller forebyggelse av gastrointestinale forstyrrelser .
Når ranitidin-vismut-citratet og det ene eller de flere antibiotika administreres som separate preparater, kan antibiotikaene for de angjeldende forbindelser fremstilles på kjent måte eller anskaffes kommersielt. Peroral administrasjon av både ranitidin-vismut-citratet og antibiotikaene er å foretrekke, selv om antibiotikaene, når det måtte passe, også kan bli gitt på annen måte, for eksempel parenteralt (f.eks. intravenøst, intramuskulært eller subkutant).
Ranitidin-vismut-citratet kan hensiktsmessig formuleres som tabletter (innbefattet tyggetabletter), kapsler (av hård eller myk type) eller som et flytende preparat, som beskrevet for eksempel i GB-patentsøknader med publikasjonsnr. 2220937A og 2248185A. Tabletter er i alminnelighet å foretrekke.
Som angitt ovenfor kan ranitidin-vismut-citrat og antibiotikaene administreres som et enkelt farmasøytisk preparat for peroral bruk. I henhold til et ytterligere aspekt ved oppfinnelsen, tilveiebringes således en farmasøytisk blanding for human- eller veterinærmedisinsk peroral anvendelse, som omfatter ranitidin-vismut-citrat og én eller flere Helicobacter pylori-hemmende antibiotika. Slike blandinger kan formuleres på konvensjonell måte ved å benytte ytterligere farmasøytisk akseptable bære- eller hjelpestoffer etter behov. Blandingen kan således fremstilles på konvensjonell måte med ytterligere bære- eller hjelpestoffer, så som bindemidler (f.eks. pregelatinisert maisstivelse, polyvinylpyrrolidon eller hydroksypropylmetylcellulose); fyllstoffer (f.eks. laktose, mikrokrystallinsk cellulose eller kalsiumhydrogenfosfat); glattemidler (f.eks. magnesiumstearat, talk eller silika); sprengmidler (f.eks. stivelse eller natriumstivelses-glykollat); eller fuktemidler (f.eks. natriumlaurylsulfat). Et alkalisalt av typen beskrevet i GB-patentsøknad med publikasjonsnr. 2248185A kan tilsettes for å bedre disintegrasjonshastigheten og/eller oppløsningen av blandingen. Tabletter kan overtrekkes etter velkjente metoder. Preparatene kan også eventuelt inneholde aroma-, farve- og/eller søtningsmidler.
Blandingene kan fremstilles etter konvensjonell teknikk som er velkjent innen den farmasøytiske industri. For eksempel kan ranitidin-vismut-citratet og antibiotikaene blandes sammen, eventuelt med passende hjelpestoffer. Tabletter kan for eksempel fremstilles ved direkte pressing av en slik blanding. Kapsler kan fremstilles ved å fylle blandingen sammen med passende hjelpestoffer over i gelatinkapsler ved bruk av en passende fyllemaskin.
Når ranitidin-vismut-citrat og antibiotikaene administreres som et enkelt farmasøytisk preparat for peroral bruk, har blandingen fortrinnsvis form av en kapsel eller helst en tablett.
Perorale farmasøytiske blandinger med lav pakningstetthet som omfatter et oppløselig vismut-holdig farmasøytisk middel, inkludert ranitidin-vismut-citrat som eventuelt også omfatter et Helicobacter pylori-hemmende antibiotikum, ble nylig beskrevet i internasjonal patentsøknad med publikasjonsnr. W092/11849. I henhold til et ytterligere aspekt tilveiebringer oppfinnelsen derfor også en farmasøytisk blanding for peroral bruk som omfatter ranitidin-vismut-citrat og ett eller flere Helicobacter pylori-hemmende antibiotika, hvor pakningstettheten av preparatet ikke er mindre enn 1 g/ml.
Preparatene for bruk i henhold til oppfinnelsen kan, om ønskes, inngå i en pakning eller en dispenseranordning som kan inneholde én eller flere enhetsdoseformer inneholdende virkestoffene. Pakningen kan for eksempel være metall- eller plast-folie, så som en "blister pack". Når ranitidin-vismut-citratet og ett eller flere antibiotika er beregnet for administrasjon som separate preparater, kan disse forelegges i form av for eksempel, en fler-romspakning (f.eks. tvillingpakning).
I henhold til et ytterligere aspekt ved foreliggende oppfinnelse, tilveiebringes således en fler-romspakning for bruk ved behandling eller forebyggelse av gastrointestinale forstyrrelser, hvor ett av rommene inneholder ranitidin-vismut-citrat og de øvrige inneholder et Helicobacter pylori-hemmende ant ibiot ikum.
Aktuelle doser av ranitidin-vismut-citratet og det ene eller de flere antibiotika for human administrasjon (ca. 70 kg legemsvekt) vil avhenge av antibiotikumets administrasjonsmåte og av tilstandens natur og alvorlighetgrad. Det vil innses at det kan være nødvendig å foreta rutinemessige variasjoner i doseringen, avhengig av pasientens alder og vekt.
En foreslått dosering av ranitidin-vismut-citrat for bruk i henhold til oppfinnelsen, er 150 mg til 1,5 g, fortrinnsvis 200-800 mg per enhetsdose. Enhetsdosen kan administreres for eksempel 1 til 4 ganger per dag.
Det ene eller de flere antibiotika kan hensiktsmessig administreres i doser innenfor det normale doseringsområdet, hvor forbindelsene har terapeutisk effekt, eller om nødvendig, i høyere doser. Antibiotikaene kan tas én eller flere ganger per dag.
En metode for behandling eller forebyggelse av gastrointestinale forstyrrelser hos mennesker eller dyr, omfatter således at disse gis en effektiv mengde av ranitidin-vismut-citrat og ett eller flere Helicobacter pylori-hemmende antibiotika .
Slike metoder omfatter administrasjon, av Helicobacter pylori-hemmende antibiotika og ranitidin-vismut-citrat, samtidig eller ikke-samtidig. I denne sammenheng betyr samtidig administrasjon at midlene gis i løpet av 24 timer, mens ikke-samtidig administrasjon betyr at midlene gis med mellomrom på mer enn 24 timer. Når midlene administreres samtidig, kan det være å foretrekke at midlene administreres med et mellomrom på ca. 1 time, eller helst med et mellomrom på ca. 5 minutter.
For slike metoder vil administrasjonens varighet enten det gjelder samtidig eller ikke-samtidig dosering, variere med den spesifikke gastrointestinale forstyrrelse som behandles. Et typisk regime vil imidlertid bestå i å administrere ranitidin-vismut-citrat i 4 til 8 uker og under dette tidsrom administrere ett eller flere antibiotika i 1 til 2 uker.
Som nevnt ovenfor, har enkelte kombinasjoner av ranitidin-vismut-citrat med antibiotika vist en synergistisk effekt in vitro mot Helicobacter pylori og in vivo mot Helicobacter mustelae.
Metodikk for synergibestemmelse in vitro
Synergisme ble målt avhengig av den observerte drapstid i de minste baktericide konsentrasjoner (MBC) ved å benytte en 2-dimensjonal mikrotiter sjakkbrett-metode<1.>Sjakkbrettet fremstilles ved å seriefortynne middel A før tilsetning til platene. Middel B ble deretter fortynnet i brønnene som inne-holdt A til et sluttvolum på 50 fil. Hver brønn ble deretter inokulert med 50/il buljongdyrket H. pylori og platene inkubert ved 37°C. Prøver for bestemmelse av H. pylori-vekst under bruk av en 0-6 kvantitativ skala tas deretter ut etter 24, 48 og 72 timer. Gjennomsnittlig FIC (fractional inhibitory concentrations) ble deretter bestemt fra 2-dimensjonale isobologrammer. En gjennomsnittlig FIC-indeks på mindre enn

For å bekrefte synergisme ble drapstid-undersøkelser ved bruk av de tilsynelatende effektive kombinasjoner, deretter foretatt i 3 ml buljongkulturer.
Metodikk for synergibestemmelse in vivo
Fritter (ildere) naturlig kolonisert med H. mustelae ble behandlet med enten ranitidin-vismut-citrat (24 mg/kg), amoxicillin og metronidazol (henholdsvis 10 mg/kg og 20 mg/kg) eller kombinasjonen av alle tre midlene. Forbindelsene ble gitt peroralt 3 ganger daglig i 2 8 dager med unntak av metronidazol (kun dagene 0-9). Den tilsynelatende koloniseringsfrekvens ble bestemt ved gastriske antrumbiopsier foretatt ved endoskopi, før, under og etter terapifasen. Med en kultur-positiv biopsi menes at forbindelsen fremdeles er kolonisert.
Resultater
In vitro viste kombinasjonen av ranitidin-vismut-citrat med tetracyklin, metronidazol, amoxicillin eller cefuroxim en synergistisk effekt. En kombinasjon av ranitidin-vismut-citrat med tetracyklin eller metronidazol ga faktisk en gjennomsnittlig FIC-indeks < 0,5.
In vivo viste kombinasjonen av ranitidin-vismut-citrat med metronidazol og amoxicillin seg å være bedre enn enten ranitidin-vismut-citrat for seg eller antibiotikaene for seg og førte til at 2/6 infeksjoner ble bekjempet. Grupper behandlet med ranitidin-vismut-citrat eller kun med antibiotika sviktet når det gjaldt bekjempelse, men viste god suppresjon. Seponeringen av metronidazol fra regimet etter dosering over 10 dager, resulterte i en senere fornyet opp-blussing av infeksjonene den 28. dag, særlig i metronidazol/- amoxicillin-gruppen.

Claims (14)
1. Anvendelse av (i) ranitidin-vismut-citrat og (ii) ett eller flere Helicobacter pylori-hemmende antibiotika ved fremstilling av et farmasøytisk preparat for bruk ved behandling eller forebyggelse av gastrointestinale forstyrrelser.
2. Anvendelse, ifølge krav 1, hvor nevnte ett eller flere antibiotika valgt fra metronidazol, tetracyklin, amoxicillin, cefuroxim axetil og clarithromycin.
3. Anvendelse, ifølge et av kravene 1 og 2, hvor nevnte ett eller flere antibiotika er valgt fra metronidazol og amoxicillin, metronidazol og tetracyklin, og tetracyklin og clarithromycin.
4. Anvendelse ifølge krav 1, hvor nevnte ett eller flere antibiotika er valgt fra cefuroxim, og cefuroxim og metronidazol .
5. Anvendelse ifølge et av kravene 1-4, hvor ranitidin-vismut-citrat og nevnte ett eller flere antibiotika er i en form egnet for peroral administrasjon.
6. Farmasøytisk preparat,karakterisertved at det inneholder (i) ranitidin-vismut-citrat og (ii) ett eller flere Heliobacter pylori-hemmende antibiotika som et kombinert preparat for simultan, separat eller sekvensiell anvendelse ved behandling eller forebyggelse av gastrointestinale forstyrrelser.
7. Preparat ifølge krav 6,
karakterisert vedat nevnte ett eller flere antibiotika er valgt fra metronidazol, tetracyklin, amoxicillin, cefuroxim axetil og clarithromycin.

10. Preparat ifølge et av kravene 6-9,karakterisert vedat ranitidin-vismut-citratet og nevnte ett eller flere antibiotika er i en form egnet for oral administrasjon, særlig som en tablett.
11. Preparat ifølge et av kravene 6-10,karakterisert vedat det er i form av en faramsøytisk blanding.
12. Farmasøytisk blanding ifølge krav 11,karakterisert vedat pakningstettheten av blandingen ikke er mindre enn 1 g/ml.
13. En fler-romspakning for anvendelse ved behandling eller forebyggelse av gastrointestinale forstyrrelser,karakterisert vedat ett av rommene inneholder (i) og de øvrige inneholder (ii) som definert i tidligere krav.
14. Pakning ifølge krav 13,
karakterisert vedat den er sammen med instruksjoner for bruk av både (i) og (ii) ved behandling eller forebyggelse av gastrointestinale forstyrrelser.
