Description
Denne oppfinnelse vedrører nye indolderivater, sammensetninger inneholdende dem og deres anvendelse for fremstilling av medisinske preparater.
Mekanismen som er involvert når migreneangrep oppstår, er ikke kjent, men det er blitt vist at de store interkraniale årer oppsvulmes under hodepinefasen. Noen forbindelser, såsom ergotamin og serotonin (5-hydroksytryptamin; 5-HT), har vasokonstriktiv virkning i det karotide vaskulære skikt ved en agonistisk virkning på de "5-HT^-lignende" reseptorer. Men mangelen på selektivitet hos disse forbindelser er opphav til uønskede og potensielt farlige bivirkninger.
I de britiske patenter 2124210A og 2162532A er det beskrevet nye forbindelser, og de synes å stimulere mer selektivt en sub-populasjon av "5-HT-lignende<11> reseptorer. Blant disse forbindelser er "Sumatriptan" med formelen:
tilgjengelig for terapeutisk behandling av migrene. Denne forbindelse har høy affinitet for 5-HT1D-reseptoren, men den har også en meget viktig affinitet for 5-HT1A-resep-toren. Denne affinitet for 5-HT1A-reseptoren forårsaker hypotensjon ved en virkning på sentralnervesystemet og ved andre bivirkninger.
Det er nå funnet at innføringen av en nitrogenring i metan-sulfonylgruppen tilveiebringer nye antimigreneforbindelser som har større affinitet for 5-HT1D-reseptoren enn for 5-HT1A-reseptoren og derfor færre bivirkninger.
Følgelig tilveiebringer foreliggende oppfinnelse en forbindelse med formel (I)
hvor R<1> og R<2> hver representerer et hydrogenatom eller en C^-C^-alkylgruppe, Z representerer en ring valgt fra: hvor n betyr 4, 5 eller 6
hvor R<3> representerer hydrogen eller en C^-C^alkylgruppe, og R<4 >representerer en Cj-Q^-alkyl-, metoksy-, benzyl- eller R<5>NHCO-gruppe hvor R<5> er en C1-C4-alkylgruppe; og
hvor R<6> representerer en Cj-C4-alkylgruppe;
og farmasøytisk akseptable salter derav.
Alkylgruppen nevnt i forbindelse med gruppen R1, R2, R<3>, R<4>, R<5>ogR<6> i forbindelsene ifølge oppfinnelsen kan være for-grenet eller rett. Forbindelsene med den generelle formel I, hvor R<1> og R<2> er Cj-C^-alkylgrupper og Z er II eller V, er foretrukket.
Indolforbindelsene med den generelle formel I kan fremstilles ved en prosess som omfatter å dekarboksylere en karbok-sylsyre med den generelle formel VI:
(hvor de forskjellige symboler er som definert ovenfor). Reaksjonen utføres fortrinnsvis i et inert løsningsmiddel, såsom kinolin, tri-n-butylamin, N,N-dimetylacetamid eller pyridin, i nærvær av en katalysator såsom kobberpulver, kobber(II)oksyd, kobber(I)oksyd eller et annet kobber-derivat, ved temperaturer mellom 100 og 200°C.
Mellomproduktene VI anvendt ved fremstillingen av forbindelsene ifølge oppfinnelsen ble fremstilt ved kjente frem-gangsmåter beskrevet i litteraturen (A. Gonzales, Synth. Commun. (1991), 21, 669; B.A. Howell, J. Chem. Ed. 176
(1984); H. Plieninger, Ber. (1950), 83., 268).
Indolderivater med den generelle formel I kan omdannes med i for seg kjente metoder til syreaddisjonssalter med syrer i egnede løsningsmidler, f.eks. aceton, alkoholer, dioksan eller ietrahydrofuran. Egnede syreaddisjonssalter er salter avledet fra uorganiske syrer, f.eks. hydroklorider eller sulfater.
Eksperimentene med vanlige forsøksdyr ble utført og eva-luert på følgende måte:
Vena saphena fra hund
Isometriske nedtegnelser ble utført hovedsakelig som beskrevet av Humphrey et al (1988). I korthet går det ut på å suspendere laterale ringpreparater fra Vena saphena (3 mm. brede) på anestiserte beaglehunder under 2 g's hvilespen-ning i 30 ml organbad inneholdende Krebs ved 37°C. Eksperimentene ble utført i nærvær av 5-HT2, Hl og muskarine antagonister, og seretonin 1/iM ble anvendt som kvantitativ referansestandard.

Binding til 5HTlD- reseptorer
Undersøkelser ble utført hovedsakelig som beskrevet av Bruinvels et al. Forskjellige mengder av undersøkte medikamenter ble tilsatt til 0,25 ml sluttvolum av reaksjonsblandingen som innbefattet 100 //g kaudalt kjernemembran-protein fra kalv, 100 pM (Serotonin-5-0-karboksymetyl-glycyl[<125>I]tyrosinamid (<125->GTI], 4 mM CaCl2 og 50 mM Tris-HCl-buffer, pH 7,4. Etter inkubasjon ved 37°C i 30 minutter ble prøvene filtrert under redusert trykk ved hjelp av glassfiberfiltre. Filtrene ble vasket med iskald buffer og tørket. Ikke-spesiefikk binding ble definert som den binding som ble erholdt i nærvær av 10 /xm 5HT. Bundet radioak-tivitet ble kvantifisert med en gamma-teller. Forskyvnings-kurver ble fremstilt, og konsentrasjonen som forskjøv 50% radioligand, ble beregnet for hver undersøkt forbindelse ved hjelp av ikke-lineær regresjon. Gjennomsnittet av data fra minst tre forskjellige forsøk i duplikat ble bestemt.

Binding til 5HT1A- reseptorene
Undersøkelsene ble utført hovedsakelig som beskrevet av Gozlan et al (1983) . Varierende mengder av undersøkte medikamenter ble tilsatt til 1 ml sluttvolum av reaksjons-blandinger som innbefattet 100 jug hippocampus-membran-protein fra rotte, 0,5 nM <3>H-8-OH-DPAT, 4 mM CaCl2, 0,1% askorbinsyre, 10 /iM pargylin og 50 mM Tris-HCl-buf f er, pH 7,4. Etter inkubasjon ved 25°C i 30 minutter ble prøvene filtrert under redusert trykk under anvendelse av glassfiberfiltre. Filtrene ble vasket med iskald buffer og tørket. Ikke-spesifikk binding ble definert som den binding som erholdes i nærvær av 10 /liM 5HT. Radioaktiviteten ble målt ved scintillasjonstelling, og dataene ble behandlet som beskrevet for 5HT1D-bindingsundersøkelsen. (Gozlan H.; El Mestikawy S.; Pichat L.; Glowinsky J. og Hamon M.

Resultatene av forsøkene beskrevet ovenfor med forbindelser ifølge oppfinnelsen (se eksemplene nedenfor) og med "Sumatriptan" som sammenligning, er vist i tabell I nedenfor:
Fra resultatene angitt ovenfor kan man slutte at de nye forbindelser ifølge denne oppfinnelse viser bindings-selektivitet for 5-HTlD-reseptorene og vasokonstriktorisk evne mediert av en agonisme på 5HTlD-reseptorene. I henhold til resultatene tilveiebringer denne oppfinnelse forbindelser av potensiell interesse for behandling eller forebyggelse av migrene eller annen hodepine forbundet med vaskulære forstyrrelser (f.eks. "cluster"-hodepine og kronisk paroksysmal hemikrani), med administrasjon av substanser eller frafall derav, og for behandling eller forebyggelse av tensjonal cefalalgi, smerte, bevegelsesforstyrrelser, depre&jon og angst.
Indolderivatene med formel I og farmasøytisk akseptable salter-derav, og farmasøytiske sammensetninger omfattenede slike-Tier i vater og salter derav, kan anvendes *ved behandling eller terapi av det menneskelige legeme.
Følgelig kan indolderivatene med formel I og farmasøytisk akseptable salter derav, og farmasøytiske sammensetninger omfattende slike derivater og salter derav, anvendes i en metode for behandling av forstyrrelser i det menneskelige legeme, hvilken metode omfatter å administrere til en pasient i behov av slik terapi en effektiv mengde av nevnte derivater eller salter derav eller nevnte sammensetninger.
Foreliggende oppfinnelse tilveiebringer også farmasøytiske sammensetninger som omfatter, som virkestoff, minst én forbindelse med den generelle formel I eller et farmasøy-tisk akseptabelt salt i blanding med et farmasøytisk akseptabelt bærerstoff eller fortynningsstoff. Virkestoffet kan omfatte 0,001-99 vekt%, fortrinnsvis 0,01-90 vekt%, av sammensetningen, avhengig av formuleringens natur og hvor-vidt det skal gjøres ytterligere fortynninger før bruk. Fortrinnsvis fremstilles sammensetningene i en form som er egnet for oral, topisk, perkutan eller parenteral administrasjon.
De farmasøytisk akseptable bærerstoffer eller fortyn-ningsstoffer som blandes med den aktive forbindelse, eller forbindelser eller salter av slike forbindelser, for å danne sammensetningene ifølge oppfinnelsen, er i og for seg kjent, og de aktuelle eksipienser som anvendes avhenger bl.a. av sammensetningenes tiltenkte administrasjonsmåte. Sammensetningene ifølge oppfinnelsen er fortrinnsvis gjort for administrasjon parenteralt eller per os. I dette til-felle kan sammensetningen for oral administrasjon være i form av tabletter, kapsler eller flytende preparater, såsom blandinger, eleksirer, siruper eller suspensjoner, som alle inneholder en eller flere forbindelser ifølge oppfinnelsen; slike preparater kan fremstilles ved metoder som er vel-kjent a faget.
Fortynningsstoffene som kan anvendes ved fremstilling av sammensetningene, innbefatter de væsker og faste stoffer som er kompatible med virkestoffet, sammen med farvestoffer og aromastoffer, hvis ønsket. Tabletter og kapsler kan med fordel inneholde mellom 1-200 mg virkestoff eller den ekvivalente mengde av et salt derav.
Den flytende sammensetning beregnet for oral anvendelse kan være i form av løsninger eller suspensjoner. Løsningene kan være vandige løsninger av et løselig salt eller andre derivater av virkestoffet i blanding med f.eks. sukrose for å danne en sirup. Suspensjonen kan omfatte en aktiv forbindelse ifølge oppfinnelsen eller et farmasøytisk akseptablet salt derav i blanding med vann, sammen med et suspensjons-middel eller aromastoff.
Sammensetninger for parenteral injeksjon kan fremstilles fra løselige salter, som kan eller ikke kan frysetørkes, og som kan oppløses i vann eller et egnet parenteralt injek-sjons fluidum.
Effektive doser er normalt i området 10-600 mg virkestoff pr. dag.
Følgende eksempler illustrerer fremstillingen av forbindelsene ifølge foreliggende oppfinnelse.
EKSEMPEL 1
Til en løsning av tidliger tørket l-[[2-karboksy-3-(2-dimetylaminoetyl)-5-indolyl]metansulfonyl]pyrrolidin (1,6 g; 0.0442 mol) i vandig kinolin (75 ml) og under nitro-genatmosfære, ble kobber(I)oksyd (160 g; 0,0011 mol) tilsatt. Reaksjonsblandingen ble oppvarmet til 190°C i 15 minutter, omrørt til værelsestemperatur, helt i en blanding av IN saltsyre (150 ml) og etylacetat (50 ml), rystet og dekantert. Den vandige løsning ble vasket flere ganger med etylacetat, deretter ble fast natriumbikarbonat tilsatt til pH = 7,8, og løsningen ble vasket med n-heksan-for å fjerne kinolinet. Den vandige løsning ble gjort alkalisk med fast kaliumkarbonat og ekstrahert med etylacetat. Den organiske løsning ble tørket (Na2C04) , løsningsmidlet ble fjernet under redusert trykk, og en mørk olje ble erholdt (1,3 g; utbytte 92%). Dette produkt ble renset ved kolonnekromato-grafi med kiselgel og metylenklorid:etanol:ammoniumhydr-oksyd (60:8:1) som elueringsmiddel, og et hvitt skum (0,8 g) av 1-[[3-(2-dimetylaminoetyl)-5-indolyl]metansulfonyl]-pyrrolidin ble erholdt.
Til en løsning av ovennevnte produkt (0,8 g) i aceton (30 ml) ble det tilsatt noen få dråper hydrogenkloridmettet dioksan-løsning. Det utfelt faste stoff ble oppsamlet ved filtrering, vasket med aceton og tørket, hvilket gav l-[[3-(2-dimetylaminoetyl)-5-indolyl]metansulfonyl]pyrrolidin-hydroklorid (0,75 g). Smeltepunkt 218-220°C.
Ytterligere indolderivater med den generelle formel I, som angitt i tabell 2 nedenfor, ble fremstilt i henhold til fremgangsmåten beskrevet i eksempel 1, men ved å anvende de passende substituerte reaktanter med formel VI.
EKSEMPEL 2
20.000 ampuller som hver inneholder 10 mg 1-[[3-(2-dimetyl-aminoetyl) -5-indolyl]metansulfonyl]piperidin-hydroklorid (aktiv ingrediens) ble fremstilt fra følgende formulering:
Fremgangsmåte
Virkestoffer og natriumklorid ble oppløst i 40 liter vann, deretter sendt gjennom et bakterietilbakeholdende filter og fylt under sterile betingelser på 2 ml's glassampuller på kjent måte.

hvor R<6> representerer en C1 -(^-alkylgruppe;
og farmasøytisk akseptable salter derav.
2. Forbindelse i henhold til krav l, karakter i-
sert ved at R<1> og R<2> er like eller forskjellige, og hver er C^-alkyl, og Z har formelen II.
3. Forbindelse ifølge krav lkarakterisert ved at den er l-[[3-(2-dimetylaminoetyl)-5-indolyl]-metansulfonyl]pyrrolidin;
1_[[3 ~(2-dimetylaminoety1)-5-indolyl]metansulfonyl]piperi-din; eller 1~[[3-(2-dimetylaminoetyl)-5-indolyl]metansulfonyl]-4-etoksykarbony1-piperaz in;
eller et hydrokloridsalt derav.
4. Sammensetning karakterisert ved at den omfatter en forbindelse i henhold til et av kravene 1-3 i blanding med et farmasøytisk akseptabelt bærerstoff eller fortynningsstoff.
5. Anvendelse av en forbindelse i henhold til et av kravene 1-3 eller en sammensetning i henhold til krav 4 for fremstilling av et medikament for behandling av hodepine inklusive migrene, bevegelsesforstyrrelser, depresjon eller angst.
