Description
Korte aanduiding: Verbeteringen in of met betrekking tot de profylaxe van borstkanker.
De uitvinding heeft betrekking op de profilaxe van borstkanker bij vrouwen.
Borstkliercarcinoom of kanker is een belangrijk medisch probleem bij vrouwen, in het bijzonder boven de leeftijd van 5 vijfendertig jaar. Momenteel wordt in de Verenigde Staten geschat dat vrouwen een kans van een op acht bezitten voor het ontwikkelen van deze aandoening tijdens hun leven. Borstcarcinoom is een belangrijke oorzaak van sterfte bij vrouwen, alsmede een oorzaak van arbeidsongeschiktheid, psychologisch 10 trauma, en verlies aan inkomen. Een groot aantal vrouwen die deze aandoening oplopen overlijden tenslotte aan de effecten daarvan, ofwel direct ofwel indirect door complicaties.
Raloxifen bezit de chemische structuur: 15 ^^ OCH2CH2-n \ ^ vO ^

(I) en tezamen met, onder andere, de farmaceutisch aanvaardbare zouten daarvan werd voor het eerst beschreven in 25 het Amerikaanse octrooischrift no. 4.418.068.
Het Amerikaanse octrooischrift no. 4.418.068 leert dat raloxifen nuttig is voor de behandeling van borsttumors en bij de profylaxe van recidiverende, goedaardige, borst-cystic fibrosis. Klinische proeven van raloxifen voor de behandeling 30 van borstkanker ondersteunen echter niet de doelmatigheid bij mensen. Dat negatieve resultaat ontmoedigt de verdere evaluatie van raloxifen als therapeutisch middel tegen borstkanker bij mensen.

Op verrassende wijze is nu vastgesteld met een toevallige uitvinding dat raloxifen bijzonder doelmatig is bij de profylaxe van borstkanker, en dat een beschermend effect is gevonden dat zich bijzonder sterk uit in een smal doserings-5 traject.
Volgens één aspect van de uitvinding wordt derhalve het gebruik van raloxifen, of een farmaceutisch aanvaardbaar zout daarvan, verschaft voor de vervaardiging van een geneesmiddel dat geschikt is voor de toediening van ongeveer 60 tot onge-10 veer 120 mg per dag van raloxifen, of het farmaceutisch aanvaardbare zout daarvan, aan een post-menopauzale vrouw, om de de novo omzetting van normale borstcellen in kankerachtige of kwaadaardige cellen bij die vrouw te remmen.
Het raloxifen, of het farmaceutisch aanvaardbare zout 15 daarvan, dient niet te worden toegediend in een hoeveelheid die groter is dan 120 mg per dag, meer bij voorkeur niet groter dan 100 mg per dag. Uitstekende beschermende effecten zijn waargenomen bij ongeveer 60 mg per dag.
Raloxifen, en de farmaceutisch aanvaardbare zouten 20 daarvan, kunnen worden vervaardigd volgens vastgestelde procedures, zoals die welke zijn beschreven in de Amerikaanse octrooischriften no. 4.133.814, 4.418.068, en 4.380.635 en de Britse octrooiaanvrage 2.293.602. Een kristalvorm die de voorkeur verdient is beschreven in de Britse octrooiaanvrage 25 no. 2.293.382, welke publicaties alle als hier ingelast worden beschouwd. In het algemeen begint de synthese met een benzo[b]thiofen dat een 6-hydroxylgroep en een 2-(4-hydroxy-fenyl)groep bezit. De hydroxylgroepen van de uitgangsverbin-ding worden beschermd, de 3-positie wordt geacyleerd, en van 30 het product de bescherming verwijderd ter vorming van de gewenste verbindingen.
Raloxifen vormt farmaceutisch aanvaardbare zuur en base additiezouten met een groot aantal organische en anorganische zuren en basen waaronder de fysiologisch aanvaardbare zouten 35 die vaak in de farmaceutische chemie worden toegepast. Dergelijke zouten maken eveneens deel uit van deze uitvinding. Kenmerkende anorganische zuren die worden toegepast om dergelijke zouten te vormen omvatten chloorwaterstofzuur, broom- 1007387 3 waterstofzuur, joodwaterstofzuur, salpeterzuur, zwavelzuur, fosforzuur, hypofosforzuur, en dergelijke. Zouten die zijn afgeleid van organische zuren, zoals alifatische mono- en dicarbonzuren, fenylgesubstitueerde alkaanzuren, hydroxy-5 alkaanzuren en hydroxyalkaandizuren, aromatische zuren, alifatische en aromatische sulfonzuren, kunnen eveneens worden toegepast. Dergelijke farmaceutisch aanvaardbaar zouten omvatten derhalve acetaat, fenylacetaat, trifluorace-taat, acrylaat, ascorbaat, benzoaat, chloorbenzoaat, dinitro-10 benzoaat, hydroxybenzoaat, methoxybenzoaat, methylbenzoaat, o-acetoxybenzoaat, naftaleen-2-benzoaat, bromide, isobuty-raat, fenylbutyraat, β-hydroxybutyraat, butyn-1,4-dioaat, hexyn-1,4-dioaat, capraat, caprylaat, chloride, cinnamaat, citraat, formiaat, fumaraat, glycollaat, heptanoaat, hippu-15 raat, lactaat, malaat, maleaat, hydroxymaleaat, malonaat, mandelaat, mesylaat, nicotinaat, isonicotinaat, nitraat, oxalaat, ftalaat, tereftalaat, fosfaat, monowaterstoffosfaat, diwaterstoffosfaat, metafosfaat, pyrofosfaat, propiolaat, propionaat, fenylpropionaat, salicylaat, sebacaat, succinaat, 20 suberaat, sulfaat, bisulfaat, pyrosulfaat, sulfiet, bisul-fiet, sulfonaat, benzeen-sulfonaat, p-broombenzeensulfonaat, chloorbenzeensulfonaat, ethaansulfonaat, 2-hydroxyethaansul-fonaat, methaansulfonaat, naftaleen-l-sulfonaat, naftaleen-2-sulfonaat, p-tolueensulfonaat, xyleensulfonaat, tartaraat, en 25 dergelijke. Het voorkeurszout voor toepassing in de uitvinding is het hydrochloridezout.
De farmaceutisch aanvaardbare zouten bezitten in het algemeen verbeterde oplosbaarheidskarakteristieken in vergelijking met de verbinding waarvan zij zijn afgeleid, en zijn 30 derhalve vaak beter geschikt voor formulering als vloeistoffen of emulsies.
Farmaceutische formuleringen kunnen worden vervaardigd volgens in de techniek bekende procedures. De verbindingen kunnen bijvoorbeeld worden geformuleerd met gebruikelijke 35 excipiëntia, verdunningsmiddelen of dragers, en gevormd tot tabletten, capsules, suspensies, poeders, en dergelijke. Voorbeelden van excipiëntia, verdunningsmiddelen, en dragers die geschikt zijn voor dergelijke formuleringen omvatten de 1007387 4 volgende: vulstoffen en toevoegsels zoals zetmeel, suikers, mannitol, en kiezelzuurderivaten; bindmiddelen zoals carboxy-methylcellulose en andere cellulosederivaten, alginaten, gelatine, en polyvinylpyrrolidon; bevochtigingsmiddelen zoals 5 glycerol; desintegratiemiddelen zoals calciumcarbonaat en natriumbicarbonaat; middelen voor het vertragen van het oplossen zoals paraffine; resorptieversnellers zoals quater-naire ammoniumverbindingen; oppervlakteactieve middelen zoals cetylalcohol, glycerolmonostearaat; absorberende dragers 10 zoals kaolien en bentoniet; en smeermiddelen zoals talk, calcium en magnesiumstearaat, en vaste polyethylgycolen.
De verbindingen kunnen eveneens worden geformuleerd als elixers of oplossingen voor geschikte orale toediening. Hoewel parenterale toediening, bijvoorbeeld op intramuscular-15 re, subcutane of intraveneuze wijze een mogelijkheid is, is dit duidelijk voor profylaxe veel minder wenselijk dan orale toediening. Voorts zijn de verbindingen goed geschikt voor formulering als doseringsvormen voor aanhoudende afgifte en dergelijke. De formuleringen kunnen eveneens zo worden samen-20 gesteld dat zij het werkzame bestanddeel alleen of bij voorkeur in een bepaald gedeelte van het darmkanaal vrijmaken, mogelijk over een tijdsperiode. De bekledingen, omhullingen, en beschermende matrices kunnen bijvoorbeeld worden gevormd uit polymeerstoffen of wassen.
25 Doelmatige dagelijkse doseringen van raloxifen, of farmaceutisch aanvaardbare zouten, zullen van ongeveer 60 tot ongeveer 120 mg/dag bedragen. Dergelijke doseringen zullen worden opgenomen door een persoon van eenmaal tot mogelijk driemaal daags.
30 Hoewel men zou verwachten dat het profylactische effect dat wordt verkregen door de uitvinding direct plaatsvindt, zijn statistisch significante gegevens, in vergelijking met de algehele bevolking, het meest duidelijk wanneer de toedie-ningsduur tenminste zes maanden bedraagt, bij voorkeur ten-35 minste één jaar, meer in het bijzonder tenminste twee jaren. Het voorkomen van borstkanker is duidelijk een zodanige belangrijke verbetering van de kwaliteit van het leven van 1007387 5 een vrouw dat een zeer langdurige, zelfs levenslange, therapie voordelig is.
Zoals hierboven is vermeld zal de voorkeurstoedienings-wijze de orale wijze zijn. Voor dergelijke doeleinden zijn de 5 volgende orale doseringsvormen beschikbaar.

TESTPROCEDURE
20 De resultaten van klinische studies met raloxifen (in de vorm van het hydroxchloridezout) worden hierna gegeven om het beschermende effect volgens de uitvinding toe te lichten.
Het belangrijkste onderzoek was dubbelblind willekeurig gemaakt, en placebo-gecontroleerd bij ongeveer 7500 post-25 menopauzale vrouwen. De studies bezaten in het algemeen drie groepen (takken) van patiënten die willekeurig waren toegewezen aan placebo, 60 mg geneesmiddel per dag, of 120 mg geneesmiddel per dag, opgegeven in gelijke aantallen per groep, alle via de orale wijze.
30 De patiënten die waren gekozen voor deze studies waren post-menopauzale vrouwen (2+ jaren sinds de laatste menstrua-tieperiode) tussen de leeftijden van ongeveer vijftig tot tachtig jaar oud. Voorts verkeerden die vrouwen in in het algemeen goede gezondheid, overeenkomend met hun leeftijd.
35 Uitsluitingscriteria voor het deelnemen aan deze studie omvatten voorafgaande of aanwezige kanker. In het bijzonder werd geen vrouw toegestaan om deel te nemen aan deze studie die momenteel had of werd verondersteld een geschiedenis te 100 73 8 7 7 bezitten van borstcarcinoom of andere oestrogeen-afhankelijke neoplasie. Dit uitsluitingscriterium vormde een populatie van patiënten die die van de algemene populatie weergaf met betrekking tot het vermogen voor het ontwikkelen van borst-5 kanker.
Mogelijke patiënten werden gescreend voorafgaande aan de plaatsing in de studie. Patiënten moesten huidige medische toestanden en geschiedenissen openbaar maken. Alle mogelijke patiënten moesten ofwel een basismammogram ofwel een evalua-10 tie met ultrageluid van de borst hebben - om te zeven op de aanwezigheid van borstkanker. Wanneer er enige reden was voor een patiënt om de studie te verlaten, en borstkanker was of kon worden verwacht, dan zou deze patiënt een borstkanker-evaluatie verkrijgen. De diagnose van borstkanker werd his-15 topathologisch gedaan uit biopsie of chirurgische monsters.
De in Tabel I weergegeven resultaten zijn voor patiënten die waren gevonden via verschillende diagnostische methoden, waaronder de mammogramresultaten na één jaar, die borst-kliercarcinoom of kanker bezaten. Deze patiënten beëindigden 20 direct de deelname aan de studie en hun status werd ongeblindeerd om de therapie (tak) te onthullen, waarvoor zij waren gerandomiseerd.
Het aantal patiënten in elke tak van de studie was 5078 voor de gecombineerde twee doseringshoeveelheden van 25 raloxifen en 2539 voor placebo.

De resultaten van de gegevens geven aan dat de relatieve kans voor de patiënten in deze studie om te worden gediag-10 nostiseerd als het hebben van borstkanker was 1,0 (per definitie) voor placebo en 0,21 (95%'s betrouwbaarheidsinterval, 0,07 tot 0,59) voor die die raloxifen innamen (gegevens van patiënten voor zowel de 60 mg als de 120 mg doseringen werden bij elkaar gevoegd), in vergelijking met placebo. De dosering 15 van 60 mg was gunstiger en is daarom een voorkeursuitvoeringsvorm van de onderhavige uitvinding.
Wanneer alle relevante studies van raloxifen bij elkaar worden gevoegd, liepen in totaal 14 patiënten van de placebo-tak borstkanker op, terwijl in totaal 9 raloxifenpatiënten 20 borstkanker opliepen. Het totale aantal ingeschreven raloxifenpatiënten was 6584.
De volgende Tabel II vermeldt enkele andere resultaten van een studie waarbij verschillende raloxifendoseringshoe-veelheden werden toegepast.

Voor alle bij elkaar gevoegde studies was de ruwe 35 relatieve kansschatting (RR) voor het ontwikkelen van borstcarcinoom voor raloxifen versus placebo 0,31 (95% betrouw baarheidsinterval, 0,13 tot 0,71). De waarschijnlijkheid dat de verschillen van deze behandelingsgroep in vermeld voor- 1007387 i 9 komen waren te wijten aan willekeurige kans is ongeveer 0,6%. Benaderde voorkomingsgetallen van kankervermeldingen voor elke tak voor raloxifen en placebo waren: Placebo = 4,23,
Raloxifen 3 0 mg = 4,0, Raloxifen 60 mg = 0,6, Raloxifen 120 5 mg = 1,6, Raloxifen 150 mg = 2,6. (Voorkomingsgetallen zijn weergegeven als het aantal gemelde gevallen per 1000 patiënt-jaren blootstelling aan het geneesmiddel).
