Abstract
디포스포네이트, 또는 디포스폰산 및 선형, 분지형 치환 또는 비치환, 환, 이종환 및 방향족 아미노-알코올 사이의 이들의 염, 및 골다공증 치료에서의 이들의 용도, 및 골다공증을 치료하기 위한 유효 성분으로서 적어도 하나의 디포스포네이트를 함유하는 제약학적 조성물.
Description
잘 알려져 있는 바와 같이, 골다공증은 뼈의 부서짐 및 골절 경향이 증가하는, 부적합 및 뼈 조직의 손상되는 것을 특징으로 하는 뼈 질환이다.
예로서, 미국에서는 25만명 이상의 사람들이 이 질병으로 고통받고 있으며, 이 질병은 매년 1.300.000의 골절을 일으키는 것으로 보고된다.
이것의 사회 비용은 결과적으로 매년 미국 달러로 10억 이상의 매우 높은 비율을 차지한다. 동일한 양상이 유럽 및 일본에서도 보고되어 있다.

최근, 치료상의 관점에서, Melmon & Morrelli, Clinical Pharmacology: Basic Principles in Therapeutics, 497(1992), N. Engl. J. Med. 306, 446(1982)에 의해 보고되어 있는 바와 같이, 플루오리드를 이용하는 단일 치료를 사용하고, 또는 칼슘, 플루오리드 및 에스트로겐을 이용하는 결합 치료를 추가로 사용하는 것이 단일 치료와 비교하여 몇몇 장점을 갖는 것으로 보고된다.
대안으로, 골다공증의 치료를 위해 처방되는 또 다른 종류의 화합물은 디스포스포네이트를 함유한다. 이 약제는 대부분 경구 경로로 투여되지만, 빈약한 흡수성(투여량의 1 내지 5%)의 해결되지 않은 불편함을 갖는 외에도, 이들은 또한 일차적인 불리한 결과로서, 꽤 중증의 식도 및 위장 장애를 일으킨다.

예를 들면, 요석증의 치료에서, 뼈 재흡수를 저해하기 위하여, 디포스폰산 및 그의 염이 미국특허 제 4,621,077호에 기재되어 있고, 즉 상기 포스폰산 또는 그의 해당 나트륨 염의 생물학적 활성이 보고되어 있다. 그러나 상기에서 이들 화합물들의 흡수 정도, 즉 생물학적 이용능력은 매우 낮았고, 이러한 이유 때문에 유효 성분이 체내 활성 농도에 도달하기 위하여, 결국 더욱 바람직하지 않은 내성을 갖는 위장 역반응을 일으키는 높은 투여량으로 투여되어야만 하였다.
본 발명은 목적은 디포스폰산 염 또는 디포스포네이트 및 이들의 제조 방법이다.
본 발명의 목적은 또한 골다공증 치료를 위한 상기 디포스폰산 염 또는 디포스포네이트의 용도이다.
본 발명은 목적은 다시 유효 성분으로서 상기 디포스폰산 염 또는 디포스포네이트를 함유하는 제약학적 조성물이고, 골다공증 치료에서의 이들 조성물의 용도이다.
본 발명의 목적은 경구 경로에 의해 투여되는 경우 증가된 생물학적 이용능력을 갖는 이용 가능한 디포스포네이트를 제조하는 것이다.

다시, 본 발명의 목적은 의학 치료에서 현재 사용되고 있는 알려진 약품과 비교하여 골다공증 치료에서 향상된 치료 지표를 나타내는 이용 가능한 디포스포네이트를 제조하는 것이다.
본 발명의 목적은 또한 디포스포네이트 제조를 위한 이용 가능한 방법을 제공하는 것이다.
본 발명의 목적은 또한 유효 성분으로서 적어도 하나의 디포스포네이트를 함유하고, 쉽게 투여될 수 있으며, 충분한 내성이 있고, 높은 치료 지표를 허용할 수 있는, 이용 가능한 제약학적 제제를 제조하는 것이다.

보다 상세하게는, 본 발명에 따라서, 상기 아미노-알코올은 선형 및 분지형 아미노-알코올에서 선택되고, 특히 이들은 다음 식에서 선택된다.
본 발명에 따라서 항상 상기 아미노-알코올은 환 및 이종환 아미노-알코올이고, 이들은 다음에서 선택된다.

본 발명에 따라서, 화학식 I의 디포스폰산 또는 4-아미노-1-히드록시부탄-1,1-비포스폰산(알렌드론산), 및 다음 식의 하나의 분지 아미노-알코올
또는 2-아미노-2-히드록시메틸-1,3-프로판디올(트로메타민) 사이에서 얻어지는 염에 특히 흥미를 나타내었고, 상기 화학식 I에서, R≠R1인 경우 R은 OH와 동일하게 선택되고, R1은 화학식 (X)의 치환 선형 알킬리덴쇄로서 선택되고, 여기서 n=3인 경우 R2및 R3는 다음 식에 따라서 H와 동일하다.
상기 염은 본 발명에 따라서 항상 제조 방법에 의존하여, 여러 가지 몰 비로 디포스폰산 및 아미노-알코올에 해당하는 잔재에 의해 형성되어, 이들은 골다공증 치료에 유리하게 사용될 수 있다. 상기 몰 비는 1:1 내지 1:2로 변화될 수 있다(디포스폰산:아미노-알코올).
선행기술에 따른 디포스포네이트는 전신 생물학적 이용능력이 경구 투여량의 1 내지 5% 범위로, 위장계로부터 매우 빈약한 흡수성을 특징으로 하여, 만족할만한 약동학을 갖지 않는다. 이어서, 흡수된 투여량의 약 50%는 24시간 내에 소변으로 제거되고, 남은 투여량은 뼈 조직에 보유되어, 후에 매우 천천히 제거된다. 대신에 흡수되지 않은 디포스포네이트는 배설물로 제거된다.
이러한 행동은 물에 대한 용해도가 너무 높아서 지질막을 거의 통과할 수 없는 이 생성물의 물리화학적 성질에 기인된다. 이어서 이들의 생물학적 이용능력 이상의 경구 투여량이 목표 위치(뼈)에서의 치료학적으로 유효한 농도를 달성하고 유지하기 위해 필요하지만, 메스꺼움 및 설사 등의 결과적으로 국소적으로 위장에 불리한 결과를 갖는다.
본 발명에 따른 디포스포네이트는, 알려진 디포스포네이트(나트륨 염)와 비교하여, 염의 보다 낮은 분리도 및 아미노-알코올의 물리화학적 특성에 기인하여 보다 낮은 물에 대한 용해도 및 증가된 지질 용해도를 특징으로 한다.
이러한 특징은 생물학적 멤브레인을 통한 흡수 공정에서의 주목할만한 이득을 확신하고, 따라서 생물학적 이용능력을 향상시킨다.
또한, 본 발명의 디포스포네이트는 골다공증 치료에서 예상되는 용도와 관련하여 유망한 약물-독소 프로파일을 나타냈다.
본 발명의 디포스포네이트는 뼈성장을 자극하는 좋은 효능을 나타내고, 이것은 아마도 반응 종에 의해 생산되는 독성-라디칼을 제거하는 아미노-알코올의 알려진 특성에 기인하는 것이다.
본 발명의 디포스포네이트는 정맥 내 및 경구 경로 모두에 의해 쉽게 투여될 수 있다. 후자의 경우에서, 유효 성분의 방출을 조절할 수 있는 제약 시스템의 사용이 특히 유리하다. 이러한 시스템은 쉽게 사용되고, 유효 성분의 지속적인 방출을 허용하는 적합한 담체가 첨가되는 것이 가능한, 리포솜, 니오솜 및 유사한 매트릭스를 포함한다.

본 발명의 비제한 실시예로서, 다음 식의 디포스폰산
및 다음 식의 아미노-알코올
을 반응시키는 것에 의해 얻어지는 염의 제조에 대하여 몇몇 실시예를 다음에 기재한다.
실시예 1
4-아미노-1-히드록시부탄-1,1-비포스폰산, (알렌드론산, 1g, 4.016mmol)을 CH2Cl2/메틸 알코올(9:1) 20㎖에 용해하였다. CH2Cl2/메틸 알코올(9:1) 5㎖에 용해된 2-아미노-2-히드록시메틸-1,3-프로판디올, (트로메타민, 486㎎, 4.016mmol)을 교반하에서 알렌드론산 용액에 첨가하였다. 형성된 백색 침전을 여과하고, 진공 하에서 건조하였다.
알렌드로네이트 트로메타민 염 1.263g을 얻었다(수율 85%).
실시예 2
4-아미노-1-히드록시부탄-1,1-비포스폰산, (알렌드론산, 1g, 4.016mmol)을 물 20㎖에 현탁하였다. 2-아미노-2-히드록시메틸-1,3-프로판디올, (트로메타민, 486㎎, 4.016mmol)을 교반하에 알렌드론산 수용액에 첨가하였다. 고체를 물에 신속하게 용해하고, 이 현탁액이 맑아지면, pH=4.18을 갖는 결과 용액을 냉동하고, 동결 건조하여 백색 분말을 얻었다.
알렌드로네이트 트로메타민 염 1.480g을 얻었다(수율 99%).
실시예 3
4-아미노-1-히드록시부탄-1,1-비포스폰산, (알렌드론산, 1g, 4.016mmol)을 물 40㎖에 현탁하였다. 2-아미노-2-히드록시메틸-1,3-프로판디올, (트로메타민, 972㎎, 8.032mmol)을 교반하에 알렌드론산 수용액에 첨가하였다. 고체를 물에 신속하게 용해하고, 이 현탁액이 맑아지면, pH=7.14을 갖는 결과 용액을 냉동하고, 동결 건조하여 백색 분말을 얻었다.
알렌드로네이트 트로메타민 염 1.97g을 얻었다(수율 99%).
실시예 4
4-아미노-1-히드록시부탄-1,1-비포스폰산, (알렌드론산, 1g, 4.016mmol)을 물 5㎖에 현탁하였다. 2-아미노-2-히드록시메틸-1,3 프로판디올, (트로메타민, 486㎎, 4.016mmol)을 교반하여서 알렌드론산 수용액에 첨가하였다. 고체를 물에 신속하게 용해하고, 현탁액이 맑아지면, 결과 용액은 pH=4.18을 갖게 되었다. 이소-프로필 알코올 10㎖을 첨가하고, 이 결과 용액을 -10℃에서 철야로 냉각하였다. 형성된 침전물을 여과하고, 냉각 이소-프로필 알코올로 2회 세척한 다음 진공 하에서 건조하였다.
알렌드로네이트 트로메타민 염 743㎎을 얻었다(수율 50%).
실시예 1, 2 및 4에서 보고된 방법에 따라 얻어진 생성물을 IR 및1H-NMR 분광계로 분석하였다.
용융점은 180.53℃였다.
1H-NMR(300MHz), (D20) δ=3.615(s 6H, CH2O); 2.920(t, 2H, N+-CH2, J=6.5Hz); 1.900(m, 2H, NCH2CH2CH2); 1.890(t, 2H, NCH2CH2CH2, J=6.5Hz).
실시예로서, 비제한 실시예가 다음 식의 디포스폰산
및 다음 식의 아미노-알코올
의 반응에 의해 얻어진 염의 제약학적 제형의 제조를 참조하여 앞으로 보고된다.
실시예 5
알렌드로네이트 트로메타민 염을 리포솜 매트릭스에서 제형화하였다.
- 레시놀 엘에스(리포솜), 20%
- 4-아미노-1-히드록시부탄-1,1-비포스폰산, (알렌드론산) 및 2-아미노-2-히드록시메틸-1,3 프로판디올, (트로메타민), 15%
(알렌드론산 및 트로메타민의 비 ≡ 1:1)
- 트로메타민 염기; 1%
- 콜레스테롤; 4%
- 프로필렌글리콜; 10%
- 에탄올; 1%
- 카르보폴 이티디 2020; 0.5%
- Na2EDTA; 0.15%
- 니파진; 0.2%
- 페노닙(방부제); 0.3%
- 구연산; 0.2%
- BHT; 0.01%
- 물; 100% 까지.
본 발명에 따른 화학식 I의 디포스폰산 및 아미노-알코올의 반응에 의해 얻어진 염의 약물-독성학 활성의 평가와 관련하여, 상기 실시예에서 기재된 바와 같이 얻어진 생성물인 알렌드로네이트 트로메타민을 실험 화합물로서 이용하여 생물학적 실험을 수행하였다.
효율
조직 배양물(A. Togari, Gen. Pharmacol. 24, 1133, 1993에 기재된 방법에 따라 제조된 신생 마우스의 머리 가죽편)에 대한 연구를 수행하였다. 배양기 종점에서, 표제 생성물의 마이크로 몰 농도로 첨가된 조직을 식염수에 트리톤 엑스-100 0.1%을 함유하는 용액 중에 균질화시켰다.
얻어진 균질물을 사용하여 알칼리 포스파타아제(조골 세포의 활성 지수로서) 및 N-아세틸글리코사미니다아제(파골 세포의 활성 지수로서)를 결정하기 위해 사용하였다.
10 마이크로 몰 농도로부터 시작하는 실험 생성물은 중요한 방식으로 조골 세포의 활성(알칼리 포스파타아제)을 활성화하고, 파골 세포의 활성(N-아세틸글리코사미니다아제)를 저해하는 것을 나타냈다.
독성
인간 치료상 투여량의 배수(×300 배)인 50㎎/㎏의 투여량으로 마우스에 경구적으로 투여되는 경우의 화합물은 명백한 독성 증후군의 발병을 일으키지 않았다.
생물학적 이용능력
이 연구는 실험 화학물인 알렌드로네이트 트로메타민을 8%와 동일한 평균 절대 생물학적 이용능력(mean absolute bioavailability)으로 계산되는 10㎎/㎏으로 경구 및 정맥 내 모두에 투여한 랫에서 수행되었다. 계산은 J. of Chromatography, 533, 183-193, 1992: "Improved determination of the biphosphonate alendronate in human plasma and urine by automated pre-column derivatisation and HPLC with fluorescent and electro-chemical detection"에 기재되어 있는 방법에 따라 0-24시간 동안 알렌드론산의 소변 제거로 수행하였다.
Claims (14)
- 다음 화학식 I을 갖는 디포스포네이트, 또는 디포스폰산 및 선형, 분지형, 치환 및 비치환, 환, 이종환, 방향족 아미노-알코올 유도체의 염.화학식 I여기서 R=R1또는 R≠R1이고, R 및 R1은 H, OH, Cl, 선형 또는 분지형, 치환 또는 비치환 알킬기, 선형 또는 분지형, 치환 또는 비치환 알킬리덴쇄이거나, 또는 이들은 다음 식에서 선택된다.
- 제 1항에 있어서, 상기 아미노-알코올은 선형 및 분지형 아미노-알코올에서 선택되고, 특히 이들은 다음 식에서 선택되는 것을 특징으로 하는 디포스포네이트.
- 제 1항에 있어서, 상기 아미노-알코올은 환 및 이종환 아미노-알코올이고, 다음 식에서 선택되는 것을 특징으로 하는 디포스포네이트.
- 제 1항에 있어서, 화학식 I에서 정의된 상기 선형 또는 분지형 치환 또는 비치환 알킬리덴쇄는 n이 1 내지 6을 갖는 다음 식과 동일하게 선택되고,여기서, R2=R3또는 R2≠R3이고, R2및 R3는 H, 선형 또는 분지형, 환, 치환 또는 비치환 알킬기에서 선택되거나, 또는 R2및 R3는 질소 원자 N과 함께 지방족 또는 방향족 이종환을 나타내는 것을 특징으로 하는 디포스포네이트.
- 제 1항에 있어서, 화학식 I에서 정의된 상기 치환된 알킬리덴쇄는 다음 식과 동일하게 선택되고,여기서, n은 1 내지 6이고, R4는 다음 식과 같이 선택되는 것을 특징으로 하는 디포스포네이트.
- 제 1항에 있어서, 화학식 I의 디포스폰산 또는 4-아미노-1-히드록시부탄-1,1-비포스폰산(알렌드론산), 및 다음 식의 하나의 분지 아미노-알코올또는 2-아미노-2-히드록시메틸-1,3-프로판디올(트로메타민) 사이에서 얻어지는 염이고, 상기 화학식 I에서, R≠R1인 경우 R은 OH와 동일하게 선택되고, R1은 화학식 (X)의 선형 알킬리덴쇄로서 선택되고, 여기서 n=3인 경우 R2및 R3는 다음 식에 따라서 H와 동일한 것을 특징으로 하는 디포스포네이트.
- ·적절한 용매에 용해되거나 또는 현탁되는 화학식 I의 디포스폰산을, 가능하게는 적절한 용매에 용해된 아미노-알코올과 혼합하고,·얻어진 혼합물을 냉각 및/또는 적절한 용매로 가능한 처리를 하여, 디포스포네이트, 또는 상기 화학식 I의 디포스폰산 및 상기 아미노-알코올의 염을 얻고,·상기 생성물(염)을 여과, 세척, 가능한 정제 및 건조(동결 건조)하는 단계로 이루어지는, 제 1항에 따른 디포스포네이트의 제조방법.
- 제 1항에서와 같은 디포스포네이트, 또는 화학식 I의 디포스폰산 및 아미노-알코올 사이의 염으로 이루어지는 골다공증 치료제.
- 유효 성분으로서, 적어도 하나의 제 1항에서와 같은 디포스포네이트, 또는 화학식 I의 디포스폰산 및 아미노-알코올 사이의 염으로 이루어지는 골다공증 치료를 위한 제약학적 조성물.
- 제 9항에 있어서, 비경구(정맥 내) 투여를 위해 제조되는 것을 특징으로 하는 제약학적 조성물.
- 제 9항에 있어서, 유효 성분의 방출을 조절하는 리포솜, 니오솜 및 적절한 담체를 포함하는 것을 특징으로 하는 제약학적 조성물.
- 제 9항에 있어서,- 레시놀 엘에스(리포솜), 20%- 4-아미노-1-히드록시부탄-1,1-비포스폰산, (알렌드론산) 및 2-아미노-2-히드록시메틸-1,3-프로판디올, (트로메타민), 15%(알렌드론산 및 트로메타민의 비 ≡ 1:1)- 트로메타민 염기; 1%- 콜레스테롤; 4%- 프로필렌글리콜; 10%- 에탄올; 1%- 카르보폴 이티이 2020; 0.5%- Na2EDTA; 0.15%- 니파진; 0.2%- 페노닢(방부제); 0.3%- 구연산; 0.2%- BHT; 0.01%- 물; 100% 까지의 조성물로 이루어지는 것을 특징으로 하는 제약학적 조성물.
- 제 12항에 있어서, 리포솜 형태 하에 있는 것을 특징으로 하는 제약학적 조성물.
- 제 1항에 있어서, 상기 디포스폰산 및 아미노-알코올의 몰 비는 1:1 내지 1:2인 것을 특징으로 하는 디포스포네이트.
