Abstract
본 발명은 상피하 영역에서의 AT1 수용체 증가 또는 상피에서의 AT2 수용체 증가와 관련된 증상 또는 질환의 치료용 제약 제제의 제조에 있어서 AT1 수용체 길항제 또는 AT2 수용체 조절제의 용도에 관한 것이다.

Description
효소 레닌은 α2-거대당단백질인 안지오텐시노겐을 그 자체로는 생물학적 활성이 매우 낮은 데카펩티드인 안지오텐신 I로 자른다. 이 연속과정의 다음 단계로, 주로 내피에 결합되어 있는 효소인 안지오텐신 전환 효소(ACE)의 작용에 의해 아미노산 2개가 추가로 잘라지면 안지오텐신 II가 형성된다. 안지오텐신 II는 자연에 존재하는 가장 강력한 혈관수축제의 하나인 것으로 생각된다.
안지오텐신 II는 표적 세포 표면의 특정 수용체와 상호작용한다. 현재까지, 예를 들어 AT1 수용체 및 AT2 수용체로 명명되는 수용체 서브타입들이 확인되었다. 지난 10년간 구체적으로 고혈압과 관련된 레닌-안지오텐신계에 대한 연구는 가히 폭발적으로 증가하였다. 그 결과, 현재 많은 안지오텐신 II 수용체가 확인되어 이들 중 일부가 클로닝되어 분석되었다. 최근, AT1 수용체와 결합하는 물질(이러한 특징을 지닌 활성 화합물은 흔히 안지오텐신 II 길항제라고 함)을 찾기 위해 상당한 노력이 기울여지고 있다. AT1 수용체를 저해하는 효과로 인해, 이러한 길항제를 예를 들어 항고혈압제로 또는 울혈성 심부전증의 치료에 사용할 수 있다.
또한, AT1 수용체와 AT2 수용체의 분포 및 생물학적 특성에 대하여 연구한 결과, 30 % 상동성이지만, 매우 상이한 분포 및 활성을 나타내는 것으로 밝혀졌다.
혈압 조절에 중요한 역할을 하는 AT1 수용체는 부신 피질, 신장 및 자궁 등에 존재한다. 세포 수준에서, AT1 수용체는 섬유아세포, 마크로파지 및 평활근 세포(SMC)에 존재한다.
대조적으로, AT2 수용체는 주로 태아의 조직뿐 아니라 성인의 조직, 특히 허혈성 심장병과 같은 질병에 걸린 조직에도 존재한다. 이러한 조직에서, AT2 수용체는 섬유아세포 및 내피 세포에 위치한다.
하기 기재된 연구의 목표는 본질적으로 면역세포화학법 및 in situ 혼성화 방법을 이용하여 인간 폐 내의 AT1 수용체 및 AT2 수용체 분포를 알아내려는 것이다.
기존에, AT1 수용체의 에피토프에 대항하는 특이적 항체가 제조된 바 있지만, AT2 수용체에 대항하는 특이적 항체는 없었다. 따라서, 방사성-표지된 수용체 길항제와 함께 자가방사기록법의 이용에 의존한 조직학적 연구로는 비교적 조직내 배치에 대한 대충의 결과만을 얻을 수 있을 뿐이었다. 그러나, 최근 2년 동안 인간 수용체에 대한 특성이 잘 밝혀진 특이적 항체가 만들어져서 이후의 연구에 사용하게 되었다.
방법 및 재료
1. 항체 및 in situ 혼성화(ISH) 프로브의 특이성 및 역가 측정
면역세포화학법(ICC) 및 ISH 연구의 특이성을 확인하기 위해, 파라핀을 씌운 정상 인간의 부신(피질 및 수질) 부분을 벨기에 겐트 소재의 유니버시티 하스피탈(University Hospital)의 병리학부내 보관소로부터 입수하여 시험 재료로 사용하였다. AT1 수용체는 주로 부신 피질내에 위치하며 AT2 수용체는 부신 수질내에 위치하는 것으로 알려져 있다.
인간의 폐에 대한 연구를 위해, 폐 질환 이외의 원인, 예를 들어 치명적인 사고로 사망한 환자의 부검물로부터 대조 재료를 얻었다. 이러한 재료의 일부는 상기한 바와 같이 겐트로부터 입수하였으며 다른 일부는 미국 필라델피아 소재의 펜닐바니아 하스피탈(The Pennsylvania Hospital)의 병리학부내 보관소로부터 입수하였다. 손상에 대한 감도가 더 크므로 소기도(small airway)를 포함하는 조직을 선택하였다.
부검후에 모든 시료를 가능한 곧바로 10 % 완충 포르말린 중에 넣어 고정하고 탈수하여 파라핀 왁스에 매입하였다. 3 내지 5 ㎛ 크기의 절편을 잘라내어 실란으로 코팅된 유리 슬라이드 위에 올려놓았다.
이 과정 동안의 변형을 최소화하기 위해, AT1 항체 노출용 하나와 AT2 항체 노출용 하나로 된 쌍의 여러 절편을 매 슬라이드 마다 올려놓았다. 20개에 이르는 슬라이드를 동시에 처리한 결과, 일차 항체를 제외하고는 색소원을 포함한 모든 시 약이 동일하다. 따라서, 절편 두께만이 완전히 조절할 수 없었던 유일한 변수였다.
2. 항체
미국 캘리포니아주 샌디에고 소재의 산타 크루즈 인크.(Santa Cruz Inc.)사로부터 구입한 두가지 AT1 수용체 항체(클론 N10 및 306)를 시험한 결과, 동일한 부신 피질 수용체 분포를 나타내는 것으로 밝혀졌다.
이 연구의 주요 부분을 산타 크루즈사로부터 구입한 시험한 AT2 수용체 항체(클론 C18)를 사용하여 수행한 결과, 부신 수질 및 폐에서 상기 첫번째 시험과 동일한 염색 패턴을 나타내는 것으로 밝혀졌다.
상기 모든 항체는 각 판매 업자들이 특이성 및 교차반응성을 엄격하게 시험한 것들이다.
3. ISH 프로브
PCR(중합효소 연쇄 반응) 산물을 다음과 같이 제조하여 사용하였다.
올리고(Oligo) 5.0 소프트웨어 프로그램을 이용하여 인간 안지오텐신 II 수용체 타입 I(젠뱅크(GenBank) 기탁번호 제M93394호) 및 안지오텐신 II 수용체 타입 II(젠뱅크 기탁번호 제U15592호)에 특이적인 올리고뉴클레오티드를 두 서열로부터의 상동성 영역에 대해 설계하였다(표 1). 하기의 표준 방법에 따라 인간 골 조직으로부터의 cDNA를 조제하였다[샘브룩(Sambrook J), 프리츠쉬(Fritsch EF) 및 마니아티스(Maniatis T.)의 문헌[Molecular Cloning: a Laboratory Manual, 2nd Ed., Cold Spring Harbor Laboratory Press, Cold Spring Harbor, NY, 1989]]. PCR에 다음의 올리고뉴클레오티드 프라이머 쌍, 즉 안지오텐신 II 수용체 타입 I의 경우 5'-CTggCTgACTTATgCTTTTTACTgACT-3' 및 5'-gATgCAggTgACTTTggCTACA-3'(PCR 산물의 크기는 염기쌍 236개 길이임), 안지오텐신 II 수용체 타입 II의 경우 5'-ATTTACTCCTTTTggCTACTCTTCCTC-3' 및 5'-ggTCACgggTTATCCTgTTCTTC-3'(PCR 산물의 크기는 염기쌍 489개 길이임)을 사용하였다. MJ 리써치 PCR 사이클 기기를 사용하여 주형 cDNA 10 ng으로 다음의 PCR 사이클에 따라 PCR 증폭을 수행하였다. 제조업자의 킷트에 제공된 성분과 함께 고 충실성(High Fidelity) Taq 중합효소(베링거 만하임, Boehringer Mannheim)를 사용하여 1) 94 ℃/2분, 2) 94 ℃/10초, 60 ℃/30초, 72 ℃/15초의 사이클 35회 수행. PCR 증폭의 산물은 0.8 % 아가로스/TBE 겔을 통한 전기영동에 의해 확인하였다[샘브룩(Sambrook J), 프리츠쉬(Fritsch EF) 및 마니아티스(Maniatis T.)의 문헌[Molecular Cloning: a Laboratory Manual, 2nd Ed., Cold Spring Harbor Laboratory Press, Cold Spring Harbor, NY, 1989]]. PCR 증폭 산물의 동일성을 확인하기 위해, 겔로부터 DNA를 용출시켜 A/T 클로닝 벡터 pMOSBlue(아머샴, Amersham)내에 클로닝하였다. 정확한 크기의 DNA 삽입체(표 1)를 포함하는 콜로니의 DNA 양 가닥을 완전히 서열 결정하여 이들의 동일성을 확인하였다.
상기 기재된 바와 같은 AT1에 대한 센스 및 안티센스 둘 다의 프로브 그리고 안티센스만의 프로브 각각을 플루오레신(FITC)으로 표지하고, 세포에 mRNA가 존재 하는지 여부를 상기 프로브와 혼성화하고 마우스 항-FITC 프로브 및 알칼리 포스파타제 항-알칼리 포스파타제(APAAP) 검출계를 사용하여 검출하였다. 이러한 표지 기술은 매우 낮은 카피수에 대한 개선된 검출법이다.
4. 면역세포화학법
방법은 모든 항체에 대해 다음과 같다. 5 ㎛ 크기의 절편을 먼저 마이크로파를 함께 사용하는 항원 재생 기술에 의해 시트르산염 완충액(pH 6.0) 중에서 처리하였다.
20분 동안 노출시키고 슬라이드를 상기 완충액 중에 냉각되게 두었다. 항체가 염소 폴리클로날 항체인 경우, 색소원으로 디아미노벤지딘(DAB)을 사용한 퍼옥시다제 항-퍼옥시다제(PAP) 방법을 이용하였고, 그 이외에 토끼 폴리클로날 항체의 경우 새로운 푸크신(Fuchsin)을 사용한 APAAP계를 이용하였다. 먼저 1 % 우 혈청 알부민(BSA)을 사용하여 절편을 30 분동안 차폐함으로써 비-특이적 수용체를 차폐시킨 다음, 일차 항체와 함께 실온에서 30분 동안 인큐베이션하였다. 각 항체의 역가를 측정하였고 최적 희석률은 하기와 같다.

ISH
파라핀을 벗겨내고 5 ㎛ 크기의 절편을 공지의 기술에 따라 'in situ' 혼성화하였다. 절편을 먼저 예비-혼성화 용액으로 55 ℃에서 20분 동안 처리하였다. 이어서, 절편을 프로브에 노출시키기 전에 55 ℃에서 밤새 세척하였다. 더 세척한 다음, 특이적 메시지의 위치 결정을 위해 마우스 항-FITC 항체 및 APAAP 검출계를 사용하여 절편을 처리하였다. AT1 수용체의 경우 센스 프로브는 음성 대조군이고, AT2의 경우는 이 프로브를 생략하였다.
이미지 분석
염색 강도를 정량적으로 측정하기 위해, 슬라이드를 레이카(Leica) MR500으로 관찰하고 레이카의 지침에 따라 조직의 단위 면적 당 염색량을 화소 단위로 기록하였다. 이렇게 하여 여러 영역에서의 AT1 수용체와 AT2 수용체의 비율을 정하였다. 영역은 a) 기도 상피, b) 상피하 간질, c) 혈관 주위의 SMC 및 d) 점액선이었다.
결과
부신내의 분포 연구
AT1 분포: 두가지 항체는 모두 예상대로 부신 피질에서 혈관을 둘러싸고 있는 평활근 세포의 주변과, 섬유아세포와 그 주변의 간질 망상조직에 뚜렷이 염색이 된 분포 패턴을 나타냈다. 내피 세포는 염색되지 않았다.
AT2 분포: 부신 수질에 대한 항체 시험 결과, 부신 크롬친화성세포종 세포가 뚜렷하게 염색되었다.
ISH: 안티센스 프로브는 둘 다 유사한 형상을 나타내었다.
대조용 폐
AT1 분포: 기도 상피의 밑에 있는 간질 세포(상피하, sub-epithelial)는 매우 뚜렷하게 염색되는 것으로 나타났으며 혈관을 싸고 있는 평활근 세포(SMC)의 연부도 마찬가지였다. 그 외에, 마크로파지도 양성이었다.
AT2 분포: 이 수용체는 솔연부(brush border)가 진하게 염색된 것으로 보아 기도 상피세포와 강한 상관관계가 있는 듯하다. 일부 점액선, 일부 혈관 내피세포, 섬유아세포, 연골세포 및 마크로파지에서도 세포가 양성으로 관찰되었다. SMC는 염색되지 않았다.
ISH: 프로브도 역시 유사한 형상을 나타낸다. 특히, AT2 프로브는 내피세포 및 일부 점액선에서만 강한 신호를 나타내었다.

부신 피질과 수질에 안지오텐신 II 수용체가 존재한다는 것은 이미 생화학적 방법 및 조직학적 방법에 의해 확인된 것이다. 시판 항체 및 개인적으로 제공받은 항체를 둘 다 사용하여 데이타를 구한 결과, 둘 다 상기 사실을 확증하였고 또한 즉석 면역세포화학법 및 ISH 방법의 신뢰도도 입증되었다.
이러한 연구 결과로 보아, 특이적 AT1와 AT2의 분포 및 비율을 결정하기 위해서는 정상 폐 조직 및 병에 걸린 폐 조직내 AT1 수용체와 AT2 수용체의 분포를 비교해야만 한다.
폐에 AT1 수용체가 존재한다는 것은 생화학적 방법에 의해 이미 밝혀져있으며, 그의 정확한 세포내 위치가 본 발명에 이르러 증명되었다. 이러한 정보는 정상 조건 및 질병 조건하의 여러 폐 영역에서의 AT1 수용체와 AT2 수용체의 비율을 정하는데 있어서 매우 중요하다.
특히, AT2 수용체의 존재 또는 분포에 대해서는 기존의 데이타가 전혀 없기 때문에 이 수용체 분포에 대한 데이타는 전적으로 새로운 것이다. 이 연구에서 몇가지 중요한 사항이 대두되었다.
첫째, 소기도의 기관지 상피세포에 AT2 수용체가 존재한다. 이 수용체는 이미 알려져 있듯이, 항-섬유성, 항-증식성 및 아폽토시스유발성인 것으로 생각된다. 따라서, 상피세포에 대한 업- 또는 다운-레귤레이션은 상피세포 대체, 과다형성증의 발병 및 폐암의 발병에 큰 영향을 미친다.
둘째, 상당한 양의 이 단백질이 상피세포의 솔연부에 존재하는 것은, 점액선의 일부 상피세포 역시 상기 수용체를 보유한다고 알려져 있으므로 점액 분비량과 상당한 관련성이 있을 것이다. ISH 데이타에 의하면, 일부 분비선은 mRNA를 고농도로 포함한다.
마지막으로, 혈관 내피세포에 AT2 수용체가 존재한다는 것이 면역조직화학법 및 in situ 혼성화에 의해 본 발명에서 확인되었다. AT2 수용체는 드물게 분포하며 어느 한 특정한 혈관의 모든 세포에 분포하는 것도 아닌 것으로 밝혀졌다.
상기 연구를 통해 인간의 폐에 있어서 안지오텐신 II 타입 AT1 및 AT2의 존재와 분포를 확인하고 그에 대한 기존의 데이타를 확장하였다. 소기도 상피세포의 기능 변화로 인해 발병하는 질환을 이해하는데 있어서, 이러한 세포 상의 AT2 수용체의 존재는 상당한 중요성을 갖는다.
인간 폐에서 AT 수용체의 위치와 이들의 분포, 정상 폐 조직 및 병에 걸린 폐 조직에서 업- 또는 다운-레귤레이션 및 비율에 대하여는 거의 알려진 바가 없다. 이 연구를 위해, 정상 폐, 및 만성 폐색성 폐 질환(COPD) ±고혈압증이 있는 환자로부터 시료를 수획하였다.

환자의 폐를 종양 절제 또는 다른 이유로 적출한 직후에 폐에서 얻은 기도를 조심스럽게 절개하였다. 주의하여 암 조직이 없는 영역을 선택하였다. 이어서, 이 작은 블럭을 실온에서 2시간 동안 4 % 파라포름알데히드 중에 넣어 고정한 후에 파라핀 왁스 안에 매입하였다. 이렇게 하여 최적의 구조적 보존과 항원 활성의 보유가 가능하다.
<수용체 위치의 결정 방법>
a) 면역조직화학법(ICC). 두개의 AT1 항체[산타 크루즈(클론 N10 및 306)]를 시험하였다. 또한, 하나의 AT2 항체[산타 크루즈(클론 C18)]도 시험하였다.
디지탈 영상은 솔연부를 비롯한 기관지 상피세포 안에 그리고 점액선 세포상에 AT2 수용체가 분포함을 보여주며, 부근의 절편이 AT1 수용체의 경우 염색되었음을 보여주는데, 이는 평활근 세포, 섬유아세포/스트로마 및 마크로파지 상에 매우 다르게 분포함을 의미한다.
환자의 인간 폐에 대한 분석
AT1 수용체 및 AT2 수용체의 위치를 결정하기 위해, 지금까지 얻은 모든 재료를 적합한 음성 대조군과 함께 절개하여 ICC에 의해 염색하였다. 지금까지 환자 한명 당 하나의 절편에서 얻은 판독결과에서부터 이미지 분석을 시작하였다(기준선이 엄격히 고수되기 때문에 이 과정은 더딘 과정이다). 상피 대 상피하 및 혈관에 대하여 측정하였다. 이 분석은 "맹검" 방식으로 수행되었기 때문에 몇몇 "환자"는 평균과 상당히 뚜렷한 차이를 나타낸다는 사실 이외의 설명은 할 수 없었다. 여러 절편에서 AT1 및 AT2에 대해 ICC를 동시에 수행함으로써 절편 두께의 변화 및 염색으로 인한 차이를 최소화하였다.
폐 상피에서 AT2 수용체의 위치 결정에 대한 결과는 많은 중요성을 갖는다. 이 수용체는 항-증식성, 항-섬유성 및 아폽토시스유발성인 것으로 알려져 있기 때문에, 그의 업-레귤레이션은 많은 폐 질환에 있어서 중요할 것으로 생각된다(예를 들어, 흡연만도 영향이 있다). AT2 수용체는 성인성 호흡곤란 증후군(ARDS)과 같은 섬유증 증상에 작용을 하거나 또는 폐암에서 상피의 증식 능력을 저하시키는 역할도 한다.

상기 두가지 수용체 타입이 비교적 풍부하게 존재하는 것으로 알려져 있는 정상 부신 피질 및 수질에서 항체 및 리보프로브를 모두 시험하였다.
AT1 수용체 및 AT2 수용체와 이들의 mRNA를 모두 검출할 수 있는 정상 폐 조직에 대해 상기 과정을 반복하였다. 위치 결정은 다음과 같았다.
AT1 - 평활근 세포, 섬유아세포/스트로마, 마크로파지에 위치함. 정상 폐에서는 수용체 농도 및 개수가 상당히 높다는 점을 제외하면, 이는 충분히 예상할 수 있는 것이다.
AT2 - 기관지 상피세포(특히, 솔연부), 점액선(일부)에 위치함. 또한, 혈관 내피세포, 섬유아세포, 마크로파지 및 연골 세포에도 존재함.
이 사실은 전혀 예상치 못한 새로운 결과이며 더 조사할 필요가 있다. 단백질 및 mRNA 함량 둘 다를 통해 상피세포 및 점액선의 위치를 확인하였으며, 솔연부의 위치하는 것은 점액 분비와 관련이 있다.

레이카 기기(이미지 분석기 MR500)를 이용해 염색 강도를 그레이 스케일(0- 250)(하나의 유닛은 하나의 화소임)값으로 해독하였다. 이로써 데이타를 정확하게 정량화할 수 있다.
상기 데이타는 환자의 이미지 분석 결과를 근거로 한 것이다. 데이타는 염색된 조직의 단위 면적 당의 화소와, 슬라이드 당 5개의 필드의 평균으로 나타내었다.
상기 결과로부터 알 수 있는 바로는, 정상 폐 조직내 상피하에서의 AT1 분포와 상피에서의 AT2 분포의 비율은 현저하게 1 보다 낮은 반면, 질병에 걸린 폐 조직에서는 이 비율이 1에 가깝거나 또는 1 이상이다. 상피는 기도 및 주 기관지의 내층을 형성한다. 대조군과 비교했을 때, 만성 기관지염증 환자의 폐의 기관지내 상피하 영역에서 AT1/AT2 수용체 비율이 증가하는 것은 주로 기도 상피 주변의 섬유아세포 및 마크로파지에 존재하는 AT1 수용체 수준의 증가로 인한 것이며, 이는 COPD에서 나타나는 염증 및 섬유증 수준의 증가를 반영한다.
상기 결과는 안지오텐신 II를 조절하는 AT1 수용체가 상피하 폐 조직내에 위치한다는 것, 특히 상응하는 폐 조직내의 상기 수용체 분포가 증가한다는 것을 명확히 입증한다. 따라서, AT1 수용체 길항제를 사용하여 안지오텐신 II를 억제시키면 기도 폐색이 줄어든다.
또한, 실험에 의해 상피 폐 조직, 특히 병에 걸린 상응하는 조직(예를 들어, 주로 기관지 상피세포) 및 폐포의 구조 세포(예를 들어, 폐포의 점액선)에서 AT2 분 포가 증가한다는 것을 알아냈다. 정상 유방 조직 및 유방암에 걸린 환자에서의 수용체 분포
본 연구에 포함된 유방 표본은 겐트 소재의 유니버시티 하스피탈내 병리학부의 보관소로부터 무작위로 입수하였다. 부서내 병리학자들은 모든 표본을 포르말린 중에 고정하여 파라핀 왁스로 처리하고 이들 표본의 병리학적 상태를 검사하였다. 침윤성 관암종 표본 14개, 침윤성 교양암 표본 1개 및 침윤성 소엽암 표본 1개의 16개 표본이 포함되었다.
상기 폐 연구에서 사용된 AT1 및 AT2에 대한 폴리클로날 항체를 사용하여 파라핀 왁스에 매입된 조직 절편에 대해 스트렙타비딘-비오틴-퍼옥시다제 복합체 방법과 병행하여 면역조직화학분석을 수행하였다.
수용체 길항제 시험용 모델을 제공하기 위해, 인간 유방 기원의 세포주도 그의 수용체 함량을 연구하였다. 이 연구로 다른 생화학적 및 세포학적 연구를 위한 유용한 시험관내 연구 모델을 제공할 수 있었다.
결과
얻어진 데이타는, 정상 인간의 유방 조직에는 안지오텐신 II 타입 1 및 2 수용체가 존재하는데 AT2는 관의 입방형 상피에 존재하고 AT1은 주로 관의 근상피세포에 존재함을 명확히 입증한다. 인큐베이션시에 일차 항체를 생략함으로써 모든 염색을 없앴다.
모든 표본에서, 결합 조직은 AT1 수용체에 대해 양성이었는데, 암세포는 16개의 표본 중 11개에서 전혀 염색되지 않았고 나머지 표본 5개에서 약하게 염색되었다. AT2 수용체의 경우, 반대의 결과가 관찰되었는데, 모든 암은 양성이며 스트로마 반응성은 대부분 나타나지 않았다(16개의 표본 중 1개).
염색 패턴이 각 표본마다 상이하게 나타났기 때문에 시험된 세포주로부터의 결과는 매우 흥미로운 것이었다. 정상 유방 상피세포를 단기간 배양시킨 결과, AT1 수용체에 대해서는 강하게 양성으로 나타났지만 AT2 수용체에 대해서는 약하게 염색될 뿐이었다.

결론
상기 결과로부터 알 수 있는 바와 같이, 정상 상피세포의 AT1 수용체가 폐와는 매우 다르게 존재하는 것으로 보인다. 그러나, 상피세포 타입은 근상피세포이되, 두 조직에서 AT2 수용체는 입방형 상피세포에 존재한다. 동시에, AT1 수용체에 대한 양성의 스트로마 염색은 두 조직에 대해 동일하다. 후자는 세포외 기질에서 섬유아세포의 존재와 분명히 관련성이 있다.
AT2 수용체가 암세포 상에 이와 같이 폭넓고 증식성인 방식으로 존재하는 것 은 놀라운 일이다. 본원에 기재된 특정 수용체를 포함하는 세포 타입은 이러한 연구에 관한 이상적인 시험관내 모델을 제공한다.
상기 실험은, 유방암 세포를 사용한 즉석 모델에서 AT1 수용체는 주로 스트로마에 분포하지만 AT2 수용체는 주로 암세포에서 확인할 수 있다는 놀라운 결과를 분명하게 보여준다.
이러한 모든 놀라운 결과는, 임의 AT1 수용체 길항제 또는 AT2 수용체 조절제를 사용하여 상피하 영역에서의 AT1 수용체 증가 또는 상피에서의 AT2 수용체 증가와 관련된 증상 또는 질환, 특히 폐색성 기도 질환을 치료할 수 있다는 것을 분명히 보여준다. 호흡성 질환의 한 종류인 폐색성 기도 질환은 기도 크기의 감소 및 기도 분비물의 증가를 특징으로 하며 폐포의 환기량이 줄어들게 된다. 폐색성 기도 질환은 가역성 및 비가역성 증상을 포함하며, 예를 들어 만성 폐색성 폐 질환, 예를 들어 기관지염(예를 들어, 만성 기관지염) 및 기종으로부터 선택되고, 마찬가지로 천식, 낭성 섬유증, 간질성 폐 질환, 침윤성 폐암, 폐 혈관 질환, 및 강제 호식(expiration) 동안 기류에 대한 내성 증가로부터 선택된다. 임의 이러한 치료는 고혈압증이 있는 비흡연자 및 흡연자를 치료하는 것과 관련될 수 있지만 꼭 그렇지는 않다.

AT1 수용체 길항제 또는 AT2 수용체 조절제란 종양 세포에 대한 숙주의 생물학적 반응을 변경시킴으로써 치료작용을 하는 물질이다. 포유동물 관상 근상피에서의 AT1 수용체와 포유동물 입방형 상피에서의 AT2 수용체 발현 증가는, 임의 AT1
수용체 길항제 또는 AT2 수용체 조절제를 사용하여 침윤성 유방암을 치료할 수 있음을 증명한다. 침윤성 유방암에는 경성암, 침윤성암, 유두상암, 관상암, 수질성암 및 소엽성 유방암뿐만 아니라 폐, 흉막, 골격 및 간으로의 전이도 포함된다. 보조제 요법을 수술 요법, 방사선치료 요법과 병용하거나 또는 완화제 요법을 호르몬 요법 또는 기타 생물학적 반응 변경제, 예를 들어 인터페론, 인터루킨, 종양 괴사 인자, 모노클로날 항체 등과 병용하여 치료하는 것을 고려할만 하다.
임상 검사 및 유방 뢴트겐선 조영법으로 유방암을 추정할 수는 있지만, 조직 생체검사법이 유방암을 진단하는 유일한 검사법이다. AT1 수용체 및 AT2 수용체의 분포 패턴을 과다형성증(AT1 수용체가 위치함)에 대한 마커 및 침윤성암(AT2 수용체가 위치함)에 대한 마커로 사용함으로써 종양의 악성 여부를 진단할 수 있다.

하기 화학식 I의 (S)-N-(1-카르복시-2-메틸프로프-1-일)-N-펜타노일-N-[2'(1H-테트라졸-5-일)비페닐-4-일메틸]아민(발사르탄, Valsartan) 및 제약상 이용가능한 그의 염이 바람직하다.

본 발명은 상피하 영역에서의 AT1 수용체 증가 또는 상피에서의 AT2 수용체 증가와 관련된 증상 또는 질환의 치료를 위한, AT1 수용체 길항제 또는 AT2 수용체 조절제, 또는 제약상 허용되는 이들의 염을 포함하는 제약 제제를 제공한다.

활성 화합물의 투여량은 투여 방식, 항온동물의 종, 연령 및(또는) 개별 증상과 같은 다양한 인자에 따라 달라질 수 있다. 일반적으로, 경구 투여의 경우 체중이 약 75 kg인 환자에 대한 일일 투여량은 예를 들어 약 40 mg, 80 mg, 160 mg 또는 320 mg의 발사르탄의 경우 약 10 mg 내지 약 360 mg이다.
본 발명의 추가의 측면은 예를 들어 상기 기재한 바와 같은 질환 및 증상의 치료에 사용할 수 있는 고상 경구 투여 형태의 발사르탄에 관한 것이다.
국제 특허 공개 제WO97/49394호(이 문헌의 전체, 특히 청구된 대상(이에 한정되지는 않음)은 본원에 참고로 포함됨)에는 예를 들어 발사르탄(임의로 염 형태)을 임의로 히드로클로로티아지드(HCTZ)와 배합하여 압착시킴으로써 압착된 고상 경구 투여 형태가 개시되어 있다. 국제 특허 공개 공보 제WO97/49394호에서, 셀룰로스의 바람직한 범위는 발사르탄/HCTZ 조성물 및 5 % 발사르탄 단독에 대해 10 내지 30 %, 예를 들어 21 %로 주어져 있다. 가교 폴리비닐피롤리돈(크로스포비돈, Crospovidone)의 바람직한 범위는 10 내지 20 %, 예를 들어 13 %로 주어져 있다.
투약 시험 후에, 놀랍게도 미세결정질 셀룰로스의 비율을 증가시킴으로써 공지된 고상 발사르탄 제형의 생체이용성을 개선할 수 있다는 것을 알아냈다. 또한, 놀랍게도 상기 공지된 고상 발사르탄의 정제 제형의 경우 가교 PVP 크로스포비돈의 비율을 감소시킴으로써 품질, 예를 들어 우수한 무게 균일성 및 우수한 압착성을 개선시킬 수 있음을 알아냈다.
따라서, 추가의 측면으로, 본 발명은 활성 물질로 발사르탄 및 상기 고상 경 구 투여 제형의 코어 성분을 총 중량을 기준으로 미세결정질 셀룰로스 30 % 이상(예를 들어, 31 내지 65 %, 예를 들어 50 %)을 포함하는 고상 경구 투여 제형에 관한 것이다.
추가의 측면으로, 본 발명은 활성 물질로 발사르탄 및 미세결정질 셀룰로스를 포함하며, 여기서 미세결정질 셀룰로스에 대한 발사르탄의 중량비는 2.5:1 내지 3:1, 예를 들어 2:1 내지 1:1, 예를 들어 1.4:1인 고상 경구 투여 제형에 관한 것이다.
추가의 실시태양에서, 본 발명의 고상 경구 투여 제형은 고상 경구 투여 제형의 코어 성분의 총 중량을 기준으로 크로스포비돈을 13 중량% 미만으로, 예를 들어 2 내지 10 중량% 포함한다.
바람직하게는, 발사르탄과 크로스포비돈의 중량비는 7:1 내지 3:1, 예를 들어 6:1 내지 4:1, 예를 들어 5.3:1이다.
바람직하게는, 미세결정질 셀룰로스와 크로스포비돈의 중량비는 7:1 내지 1:1, 예를 들어 4:1 내지 2:1, 예를 들어 3.6:1이다.
본 발명에 따른 고상 경구 투여 제형은 발사르탄을 20 내지 360 mg, 예를 들어 40, 80, 160, 320 mg 포함할 수 있다. 이러한 투여 범위에서, 치료의 유연성 및 효과는 예를 들어 혈압 감소시에 증가할 수 있다.

통상의 조성물은 발사르탄 20 내지 65 %, 미세결정질 셀룰로스 31 내지 50 %, 크로스포비돈 2 내지 10 %, 스테아르산 마그네슘 1 내지 10 %, 및 콜로이드성 무수 실리카 0.5 내지 5 %를 포함할 수 있다.
원한다면, 코어 조성물 1 내지 10 중량%, 예를 들어 큐티나(Cutina) 5 내지 10 %, 또는 코어 조성물 1 내지 10 중량%, 예를 들어 스테아르산 5 내지 10 %를 첨가할 수 있다.
바람직하게는, 본 발명의 고상 경구 투여 제형은 압착된 정제 형태이다.
추가의 측면으로, 본 발명은 고상 경구 투여 제형, 예를 들어 활성 물질로 발사르탄 250 mg 이상 360 mg 이하, 예를 들어 320 mg을 포함하는 압착된 정제에 관한 것이다.
고상 경구 제형에 통상적으로 사용되는 윤활제 및 활탁제로 다른 부형제를 사용할 수 있으며, 적합한 물질에 대하여는 광범위한 문헌[특히, 전체 내용이 본원에 참고로 포함되는 피들러(Fiedler)의 문헌["Lexicon der Hilfstoffe", 4th Edition, ECV Aulendorf 1996] 및 문헌["Handbook of Pharmaceutical Excipients" Wade and Weller Ed.(1994)]을 참고]을 참고한다.
본 발명에 따른 고상 경구 투여 제형은, 고상 경구 투여 제형에 코팅, 통상적으로 당, 쉘락 또는 당업계에서 아주 통상적인 다른 피막코팅을 한 당제(dragee) 형태일 수 있다. 당업계에서 이용되는 이미 알려진 수많은 코팅 방법, 예를 들어 유동층내의 분무 코팅법(예를 들어, 액셀라 코타(Accela Cota) 방법에 의해 구멍이 뚫린 팬에서 아에로마틱(Aeromatic, Glatt, Wurster or Huettlin)사로부터 구입한 장치를 사용한 공지된 방법) 또는 서브머지 스워드(submerged sword) 코팅 방법이 고려된다. 당제에 사용되는 통상의 첨가제가 이러한 방법에 사용된다. 예를 들어, 사용할 수 있는 코팅으로 오파드리 등의 국제 특허 공개 제WO97/49394호에 개시된 것을 사용할 수 있다.
본 발명의 제약 조성물은 본원에 포함된 특정 활성 물질의 알려진 적응증에 유용하다.
투여되는 활성 물질 및 특정 제형의 정확한 투여량은 예를 들어 치료될 증상, 원하는 치료 기간 및 활성 물질 방출 속도와 같은 많은 인자에 따라 달라진다. 예를 들어, 필요한 활성 물질의 양 및 그의 방출 속도는, 혈장내 특정 활성 물질의 농도가 소정의 치료 효과를 위해 허용되는 수준으로 얼마나 오랫 동안 유지되는지를 결정하는 공지된 시험관내 또는 생체내 기술을 기초로 결정할 수 있다.
예를 들어, 본 발명의 조성물은 임상 실험에서 시판 제형인 디오반(Diovan, 등록상표)에 필적하는 생체이용성을 나타냈다.
본 발명의 고상 제형의 용해 속도는 바람직하게는 30분 동안 90 % 이상이다.
예를 들어, 체중이 755 kg인 포유동물, 예를 들어 인간 및 표준 동물 모델에 있어서 하루에 발사르탄 10 mg 내지 360 mg의 범위의 투여량을 사용할 수 있다. 본 발명의 조성물에 의해 제공되는 발사르탄의 우수한 관용성은 표준 동물 시험 및 임상 실험에서 관찰할 수 있다.
또다른 측면으로, 본 발명은 상기 기재된 고상 경구 투여 제형의 제조 방법을 제공한다. 국제 특허 공개 공보 제WO97/49394호(본원에 포함됨)에서와 같은 성 분, 예를 들어 상기 정의된 바와 같은 성분을 적정량으로 구성하여 단위 투여 제형을 제조함으로써 상기 고상 경구 투여 제형을 제조할 수 있다.
예를 들어, 본 발명은
i) 활성 물질 및 제약상 허용되는 첨가제를 분쇄하는 단계,
ii) 분쇄된 활성 물질 및 첨가제의 혼합물을 압착하여 코프리메이트(압착된 덩어리)를 형성하는 단계,
iii) 코프리메이트를 전환시켜 과립을 형성하는 단계 및
iv) 과립을 압착하여 고상 경구 투여 제형을 형성하는 단계
를 포함하는, 상기 기재된 고상 경구 투여 제형을 제조하는 방법을 제공한다.
상기 방법은 물의 부재시에 수행되는, 즉 건조 압착 방법이다. 상기 방법은 주변 조건의 온도 및 습도하에 수행할 수 있으며, 상기 방법을 꼭 건조 분위기하에 수행할 필요는 없다.
초기 분쇄 단계 i)은 통상의 분쇄 방법 또는 미립자화 방법에 따라 수행할 수 있다.
활성 물질 및 첨가제를 각각 또는 함께 약 0.1 ㎛ 내지 약 1500 ㎛, 예를 들어 1.0 ㎛ 내지 900 ㎛, 예를 들어 60 ㎛ 내지 600 ㎛의 입도로 분쇄할 수 있다. 활성 물질 및 첨가제의 결정의 90 % 이상은 상기 범위로 존재한다. 이러한 크기의 입자는 통상의 분쇄 방법, 예를 들어 에어 젯 밀링, 햄머 및 스크린 밀링, 미세 충격 밀링, 볼 밀링 또는 진동 밀링법으로 분쇄함으로써 얻을 수 있다.
미립자화는 바람직하게는 예를 들어 브랜슨 소니파이어(BRANSON Sonifier)형 초음파 분해기를 사용하는 공지된 방법에 의해 또는 예를 들어 호모렉스(HOMOREX)형 교반기를 장착한 고속 교반기를 사용하여 현탁액을 교반시킴으로써 수행한다.
분쇄된 입자는 임의로 이 단계에서 공지된 방법에 따라 체로 걸러내어 혼합할 수 있다.
압착을 수행하여 코프리메이트를 형성하기 위해서는 건조 분쇄 성분을 압착해야 한다. 강타(slugging)법 또는 바람직하게는 롤러 압착법을 이용하여 압착을 수행할 수 있다. 롤러 압착 장치는 통상적인 장치이며 사실상 서로 마주보는 두개의 롤러를 이용한다. 유압 램은 마주보는 롤러의 하나가 스크류 콘베이어 시스템을 통해 롤러 압착기로 이송된 분쇄된 입자에 대해 압착력을 수행하게끔 한다.
25 내지 65 kN, 예를 들어 25 내지 45 kN의 압착력을 사용할 수 있다. 이 압착력 범위에서, 놀랍게도 각 특정 제형의 경우 최소 압착력을 사용함으로써 과립이 바람직한 속도로 개별 일차 입자로 분해되는, 예를 들어 최소 압착력 이상으로 압착된 고상 경구 투여 제형의 경우 6시간 정도 더 빨리 분해되는 고상 경구 투여 제형을 수득할 수 있음을 알아내었다. 정제의 경우 이러한 빠른 분해 속도는 드문일이며 캅셀 제형의 분해 속도와 비슷하다. 구체적인 최소 압착력은 임의 주어진 제형내 활성 물질의 함량에 따라 다르며, 따라서 존재하는 첨가제의 양 및 특성에 따라 달라지기도 한다.
상기 정보대로 라면, 당업자는 과도한 실험 없이도 통상의 실험을 행하여 다른 제형에 대한 최소 압착력도 분명히 결정할 수 있음은 물론이다.
롤 속도는 1 내지 20 rpm, 바람직하게는 9 내지 15 rpm으로 설정할 수 있다. 롤러를 통과한 다음, 압착된 덩어리(코프리메이트)는 단편의 얇은 리본 모양을 나타낸다.
코프리메이트를 스크리닝 또는 분쇄하여 과립을 제조할 수 있다. 가장 간단한 형태의 스크리닝에는 롤러로부터 나온 코리메이트를 기계적 압력하에 체를 통해 통과시키는 것이 포함된다. 보다 바람직하게는, 진동 분쇄기, 예를 들어 MGI624 Frewitt(Key International Inc.)를 사용하여 코프리메이트를 스크리닝한다.
과립의 정제 코어로의 압착은 통상의 타정기, 예를 들어 EK-0 Korsch 편심타정기 또는 회전 압착기를 사용하여 예를 들어 2 kN 이상의 압착력에서 수행할 수 있다. 정제 코어의 모양은 다를 수 있고, 예를 들어 구형, 타원형, 직사각형, 실린더형 또는 임의 다른 적합한 형태일 수 있으며, 치료 물질의 농도에 따라 크기를 달리할 수 있다. 본 발명에 따른 정제의 특징은 정제에 포함된 활성 물질의 양을 고려할 때 정제의 크기가 작다는 것이다.
본 발명의 바람직한 실시태양에서, 상기 기술한 압착법에 의해 수득한 정제는 약간 타원형이다. 정제의 연부는 베벨형이나 또는 원형일 수 있다.
본 발명의 특히 바람직한 실시태양에서, 고상 경구 투여 제형은 직사각형 형태를 갖는 정제 형태로 압착되는데, 여기서 길이:폭:높이의 치수 비율은 예를 들어 2.5 - 5.0 : 0.9 - 2.0 : 1.0이며, 바람직하게는 정제의 하면 및 상면은 서로 독립적으로 평면이거나 또는 장축에 대해 볼록하게 구부러져 있으며, 측면은 평면이고, 말단 면은 임의 형태일 수 있으며 연부는 임의로 베벨형이거나 또는 원형이다.
본 발명의 특히 바람직한 실시태양에서, 고상 경구 투여 제형은 과립으로부 터 직사각형 형태의 정제 형태로 압착되는데, 여기서 길이는 약 10.0 내지 15.0 mm이고, 폭은 약 5.0 내지 6.0 mm이며, 높이는 약 3.0 내지 4.0 mm이다.
본 발명의 특히 바람직한 다른 실시태양에서, 고상 경구 투여 제형은 과립으로부터 직사각형 형태의 정제 형태로 압착되는데, 여기서 길이는 약 15.0 내지 18.0 mm이고, 폭은 약 6.0 내지 9.0 mm이며, 높이는 약 3.5 내지 5.0 mm이다.
본 발명의 바람직한 또다른 실시태양에서, 표면이 약간 볼록한 상면 및 하면을 갖는, 본질적으로 디스크 모양의 정제가 제공된다. 바람직하게는, 정제의 직경은 약 8 내지 8.5 mm이고 높이는 약 3 내지 3.5 mm이거나 또는 직경은 약 16 mm이고 높이는 약 6 mm이다. 정제의 체적은 약 0.1 cm3 내지 약 1 cm3, 예를 들어 0.1 cm3 내지 약 0.45 cm3, 예를 들어 0.2 cm3 내지 약 0.3 cm3, 예를 들어 0.125 cm3 내지 약 0.25 cm3일 수 있다.
본 발명의 정제는 또한 투명하고, 무색 또는 유색이며, 이 생성물이 각각의 형태를 나타내도록 표시하여 이들을 즉시 알아볼 수 있게끔 할 수 있다. 염료를 사용하면 외관 특성을 증대시킬 수 있을뿐 아니라 조성을물을 확인할 수도 있다. 약제에 사용하기에 적합한 염료에는 통상적으로 카로티노이드, 산화철 또는 클로로필이 포함된다.

발사르탄, 미세결정질 셀룰로스, 크로스포비돈, 콜로이드성 무수 실리카/콜로이드성 이산화규소/에어로실 200의 일부, 이산화규소 및 스테아르산 마그네슘의 혼합물을 확산 혼합기로 예비-혼합한 다음, 스크리닝 분쇄기를 통과시켜 걸러낸다. 생성 혼합물을 다시 확산 혼합기로 예비-혼합하여 롤러 압착기로 압착한 다음, 스크리닝 분쇄기를 통과시켜 걸러내었다. 생성 혼합물에, 콜로이드성 무수 실리카/콜로이드성 이산화규소/에어로실 200의 나머지를 첨가하고 확산 혼합기로 최종 혼합하였다. 회전 타정기로 전체 혼합물을 압착하고, 구멍이 뚫린 팬에서 디올락 페일 레드를 사용하여 피막으로 정제를 코딩하였다.

과립화/건조
유동층 과립기로 발사르탄 및 미세결정질 셀룰로스를, 정제수 중에 용해된 포비돈 및 소듐 라우릴 술페이트로 이루어진 과립화 용액을 사용하여 분무-과립화한다. 수득된 과립은 유동층 건조기에서 건조한다.
분쇄/혼합
건조된 과립을 크로스포비돈 및 스테아르산 마그네슘과 함께 분쇄한다. 이어서, 덩어리를 원뿔형 스크류형 혼합기에서 약 10분 동안 혼합한다.
캅셀화
조절된 온도 및 습도 조건하에 빈 경질 젤라틴 캅셀에 혼합된 벌크 과립을 채워넣는다. 품질보증부에서 충전된 캅셀의 먼지를 제거하고 육안으로 검사하고 무게를 측정한 다음 검역한다.
