Abstract
본 발명은 무균 점안약, 특히 액체 수상 및 액체 소수성 상을 포함하는 2상 캐리어 또는 겔 기제를 포함하는, 겔 안약에 관한 것이다.
Description
본 발명은 점안약 형태로 사용할 수 있는 무균성 안과용 겔제제에 관한 것으로, 특히 겔의 제조방법 및 인공누액으로 사용되어 "건조안"을 치료하기 위한 겔제제에 관한 것이다.
천연눈물을 대체하여 "건조안"을 치료하기 위한 수용성 안약이 사용되고 있는데, 이 수용성 안약은 예를 들면, 폴리비닐알콜(PVOH), 폴리비닐피롤리돈(PVP), 셀룰로우즈유도체 또는 덱스트란에 기초한 필름-형성재료(film builing materials)를 포함한다. 이들에 관한 개략적인 것이 예를 들면, 수커(Sucher), 푸크스(Fuchs) 및 스파이저(Speiser)의, 1978년의 "파마조이티쉐 테크놀로지(Pharmazeutische Technolog e)"에 기재되어 있다. 이들 제제는 일반적으로 유체 또는 액체의 단일 상 계로서, 첨가 성분을 함유하는 수용액이다.
또한, 그레젤(Gressel) 등의 미국특허 제 5,209,927호에는 비닐폴리머를 포함하는 건조안 치료용의 안약 및 치료방법이 개시되어 있는데, 이 그레젤 등의 미국특허는 2상(two-phase) 시스템으로 이루어져 있지 않으며, 이로 인해 비타민 A 및 그 유도체가 비교적 빨리 분해되는 문제점이 있어서 개선이 요구된다.
난용성 또는 불용성 성분이 존재할 경우, 이 성분들은 분리된 고체 상태로 액상중에 현탁된다.

Goodrich Company)에서 얻을 수 있음)이 독일 특허 34 40 352 및 독일 특허 43 03 818, 또한 미국 특허 5,252,318에 이미 공지되어 있다. 본 출원의 우선일 후에 발행된 미국 특허 5,441,732호는 두 개의 특정 겔 성분의 조합이 개시되어 있는 바, 그 중에 하나는 열에 의해 겔화되고, 다른 하나는 pH의 변화로 겔화된다. 다양한 셀룰로오즈유도체는 열 반응성 겔 중합체로 알려져 있는데 반해, 폴리아크릴레이트, 특히 가교결합하는 폴리아크릴레이트는 pH 값에 따라 겔화되는 중합체로서 언급된다. 점안 후, pH 변화에 의해 눈에서 겔이 형성되는 것만을 의도하는 것으로 생각된다. 이 중합체의 혼합물은 유기오일과 함께 사용되어야 하고, 여기에 여러 활성제가 용해될 수 있지만, 비타민A는 언급하지 않고 있다.
막 형성성 제제나 윤활성 제제로 사용되는 안약에는 유연성, 무자극성, 보존수명 등에 있어서 매우 높은 기준이 마련되어야 한다. 특히, 건조안을 정기적으로 치료할 경우와 같이 안약제제를 장기간 사용해야만 할 경우, 눈이 타는 듯한 일시적인 자극은 극심한 불쾌감을 유발하여 장기적인 치료에는 유해하다. 또한 안약의 사용시 시력감퇴, 침침함 또는 통증이 있어서는 안된다.
건조안의 치료시, 비타민 A를, 일반적으로는 비타민A 팔미테이트의 형태로 사용하는 것이 효과적이라는 것이 증명되었다. 따라서, (본 출원의 우선일 이후에)상품화된 카르보폴계 겔점제제에는 통상 사용되는 성분 외에 비타민A 팔미테이트가 포함되어 있다. 이들 제제는 연속적인 수용성 액상의 단상 겔이고, 소수성의 2액상을 포함하지 않는다. 비타민A 성분이 40% 이상일때 이들 제제의 수명은 약 1년이다. 실험에서 알 수 있는 것과 같이, 이들 제제의 비타민A 성분은 6개월 후 약 20% 까지 감소하므로, 함량이 40% 이상일 경우에는 1년 후에도 보증 최소 함유량은 계속 존재한다.
천연눈물은 트리글리세리드 및 인지질을 포함하는 지방산을 포함한다. 인공 눈물에 이미 트리글리세리드를 사용(WO 94/05298)하고 있지만, 인공눈물은 유화제를 첨가하여 제제 전체를 에멀젼 형태로 할 경우에만 국소 점안용으로 사용할 수있다. 이것은 바람직하게 눈에 잔류해야 하는 천연눈물이 유화제 성분에 의해 파괴된다는 단점을 갖는다.
특히 비타민A 및 그의 유도체와 같은 민감한 물질을 포함할 때 보존 기간을 개선할 수 있는 점안용 제제, 특히 겔 제제를 제공하는 것이 발명의 중요한 목적이고,
본 발명의 다른 목적은, 특히 장기간 사용하도록 조제될 때 격렬한 자극이나 시력감퇴를 유발하지 않는 제제를 제공하는데 있다.
본 발명의 또 다른 주요목적은, 장기간 반복사용할 수 있고 및/또는 이물감, 유연성 부족, 자극 등의 문제없이 매번 사용한 뒤 오래 잔류할 수 있는 안약을 제공하는 것에 있다.
본 발명은 다음과 같은 발견에 기초한 것이다.
인공누액 및 건조안 치료용 제제에서 비타민A 및 그의 유도체, 특히 비타민A 팔미테이트가 비교적 빨리 분해되던 것이, 하나의 수용성 액상 및 하나의 소수성 액상이 있는 두개의 성분시스템을 형성할 때, 크게 줄어든다는 것을 발견했다. 특히, 연속 수상 중에 소수성 액체 오일상이 매우 미세하게 나누어진 물방울의 형태로 존재하는 제제가 바람직하다. 이에 대한 설명은 쉽지 않다. 아마도, 산소 및/또는 빛이 비타민A 분해의 원인이 되지만 제제가 단일상으로 이루어지는지, 또는 각각 2개의 액체상으로 이루어지는지는 이들 양자에 대해 차이가 없다. 소수성 상 중의 비타민A 용액 내의 산소 및/또는 빛에 대한 민감성이 증가되는 것을 예상할 수 있다.
항산화제, 특히 비타민E 및 그 유도체, 보다 구체적으로는 비타민E 아세테이트를 동시에 사용하는 것은 이들 제제에 영향을 미친다. 어떤 경우에도. 비타민A는 본 발명의 제제에 항산화제로 바람직하게 사용된다.
또한, 본 발명의 더욱 놀라운 효과는, 천연 눈물의 조성물과 매우 가까운 수상 및 액체 소수성 상을 갖는 안약을 조제할 수 있고, 특히 유화제를 동시에 사용할 필요없이 이에 대응하는 트리글리세리드 함량을 갖는 제제를 조제할 수 있다는 것이다. 이는 본 발명에 의한 제제가 겔점안제 형태이기 때문이다. 겔 기제가 전혀 없는 제제와 비교하여, 유화제가 전혀 없을 때에도 겔을 충분하게 안정화시킬 수 있다. 따라서 트리글리세리드는 매우 장 기간의 보존기간에 걸쳐 매우 미세하게 나누어진 물방울형태로 본 발명의 겔 제제에 존재할 수 있다. 이들 물방울은 아무런 문제 없이 멸균이 가능하고 자극을 일으키지 않으면서 완벽히 수용될 수 있다.
겔 제제를 포함하는 본 발명의 제제는 천연 눈물과 거의 같은 굴절률을 갖고, 눈에 오래 잔류하며 영향을 준다.
본 발명의 제제는 수용성 점안용 겔의 표면장력 감소를 가져오고, 이 때문에 각막에 고르게 분포된다. 또한, 오일상에 현탁 또는 용해된 활성제는 보다 고르게 분포된다. 콘택트렌즈 또는 안과용 기구의 프론트 렌즈 등의 각막위에 놓이는 물체의 습윤성이 개선된다.
본 발명의 제제는 온화한 기후(29℃, 상대습도 45%)하에서와, 지중해성 아열대성 기후(26℃, 상대습도 60%)는 물론, 심지어 매우 덥고, 습한 기후(31℃, 70% 상대습도)하에서도 시판 중인 5g 용량의 폴리포일 튜브 포장에서 극히 안정하고,비타민A함량, pH값, 삼투압값, 점성도 및 외관의 변화가 거의 나타나지 않는다.
기본적으로, 안과용으로 사용가능한 유기 오일은 본 발명의 2상 제제, 특히 겔의 액체 소수성 성분으로서 적합하고, 유화제가 존재하지 않더라도 수상 중에 작은 물방울로 분산된다. 실시예들은 지방산 유도체, 특히 지방산 에스테르, 트리글리세리드 및 프탈산 에스테르로 형성된다. 트리글리세리드는 대부분 또는 전체적으로 특히 C8-C12지방산으로부터 형성된 바람직한 균일 또는 혼합된 트리글리세리드가 바람직하다. 특히, "도이치 아르쯔나이부흐(Deutsches Arzneibuch)"(DAB) 10(1993)에서 정의된 타입의 중간 사슬 트리글리세리드가 바람직하다. 이 트리글리세리드의 산성분은 적어도 95%의 n-옥탄산 및 n-데칸산의 혼합이고; 나머지는 사슬 1 길이가 더 짧은 지방산으로 형성된다.
이런 중간 사슬 트리글리세리드는, 코코야자 배유의 건조고형분의 오일을 이용하여, 이 배유에서 얻어진 야자오일을 가수분해하고, 이렇게 얻어진 지반산을 분류하고, 재에스테르화하여 반합성적으로 제조된다.
중간 사슬 트리글리세리드는 화장품의 기본적인 재료, 의약용 첨가제와 담체, 또한 식품으로 이미 이용되고 있다. 비록 중간 사슬 트리글리세리드가 유익한 성질을 가지고 있다는 것이 잘 알려져 있지만, 그들의 잠재적 유도 및 점안응 세제로서의 사용한계에 대해서는 알려진 것이 거의 없다.
특히 겔 형성제로서, 통상 0.1∼3중량%, 특히 약 0.2중랑%의 폴리아크릴산을 함유하는 겔 기제의 본 발명의 제제는 보통 0.5중량% ∼ 10중량%, 특히 약 1중량%의 중간사슬 트리글리세리드를 함유한다.
본 발명의 겔 제제는 바람직하게 pH값 6∼8 대략 2000 ∼ 6000mPa s의 점성도를 가진다.
본 발명의 안약은 바람직하게 센트리미드(centrimid), 벤즈알코늄클로라이드 또는 티오머살(thimersal)같은 보존제를 포함한다. 이들 겔제제는 적어도 1종의 등장제를 함유하는 것이 더 바람직하고, 이 목적으로는 특히 소르비톨이 적당하다.
비타민A 성분, 특히 비타민A 팔미테이트의 특히 바람직한 함량은 본 발명의 제제의 그램당 500국제단위이다. 비타민A 성분은 소량의 적어도 1종의 항산화제에 의해 안정화되는 것이 바람직하고, 여기에 있어서 비타민E 및 비타민E 아세테이트가 특히 유리하다.
본 발명의 무균 제제, 특히 겔의 제조는 다단계로 진행된다. 겔제조에 있어서, 무균 폴리아크릴산 서스펜션은 독일 특허 43 03 818에서 개시된 과정, 즉 약 120℃, 1바 초과 압력으로 20분 동안에 오토클레이브 처리하여 바람직하게 얻어진다. 동시에 보존제 및 등장제, 즉, 바람직하게는 센트리미드 및 소르비톨을 포함하는 수용액을 제조한다. 이 수용액을 가압가스로서 질소가스를 사용하거나, 간단히 압축공기를 이용하여 멸균여과하여 오토클레이브 처리된 폴리아크릴산 서스펜션에 첨가된다. 다음으로, 겔형성을 개시하는 무균 수산화나트륨 용액을 첨가함으로써 조심스러운 중화반응이 이루어진다. 일단 중화가 이루어지면, 형성중인 겔에는 더 이상 자유염기가 없다. 다음으로 소수성 액체 성분, 즉 바람직하게 중간 사슬 트리글리세리드 성분을 무균의 상태에서 무균 겔에 가한다. 완전한 균질화 상태가 이루어질 때까지 계속 교반한다. 본 발명에 따른 분산액에서, 얻어진 오일 방울의 크기는 최대로 약 100㎛로서, 종래의 강한 유화제를 첨가하지 않고 얻어지는 것과 같은 정도이다.
무균의 겔은 통상적인 방법으로 제조된다.
특히, 비타민A 팔미테이트와 같은 활성제를 이렇게 형성된 무균 겔에 첨가할 수 있는바, 비타민A 성분 및 극히 소량의 항산화제를 중성 오일에 용해한뒤, 멸균 여과하고 난 뒤, 무균 오일 용액을 교반하여 겔에 가한다.
겔 기제없는 제제, 즉 점안제를 제조해야 할 때는 비교공정이 이용된다.
본 발명에 따라 사용된 겔 형성제는 바람직하게 대략 3백만 내지 5백만의 분자량을 가지는 폴리아크릴산이다. 구체적으로 바람직한 것은 비.에프. 굿리치 케미칼 주식회사 제품인 카르보폴 같은 상품이다. 카르보폴 980 NF가 특히 바람직하다. 본 제제에서 그 농도는 대략 0.2중량%이다.
겔 형성에 필요한 중화는 무균의 희석한 수산화나트륨 용액, 특히 1-N 수산화나트륨 용액을 사용해 수행된다. 그러나, 다른 무기염기 또는 탄산 알칼리, 또는 아민, 특히 트리에틸아민 및 디이소프로필아민같은 유기염기가 이용될 수도 있다.
얻어진 겔은 대략 20℃에서 2000 내지 6000mPa s의 점성도를 가진다.
센트리미드, 벤즈알코늄클로라이드 또는 티오머살 같이 본 발명에 의해 일반적으로 사용되는 보존제는 센트리미드의 경우에 일반적인 농도, 즉 대략 0.01중량%의 농도로 이용된다. 등장제, 예를 들면 만니톨, 덱스트로오즈, 글리세롤, 프로필렌 글리콜 또는 특히 바람직한 소르비톨같은 다기능성 알코을 역시 일반적인 농도로 이용된다. 소르비톨의 경우 대략 4.85중량%의 농도가 특히 유리하다.
두 가지의 실시예에 의해 본 발명을 더욱 상세히 설명한다.
(실시예 1)
약 375kg의 물(주사용)과 2000kg 폴리아크릴산(카르보폴 980 NF)의 균질한 서스펜션을 대략 25 내지 40㎛의 다공성 섬유제거 여과기를 통해 처리장치로 보낸다. 이 현탁액을 20분 동안 121℃에서 1바 초과 압력하에서 교반하면서 오토클레이브 처리하고, 이어서 무균의 공기 여과기를 이용하여 주변압력으로 실온까지 냉각시킨다.
그 동안, 약 964kg의 물(주사용)은 적당한 용기에 넣어 교반하면서 0.100kg 센트리미드 및 48.510kg 소르비톨을 그것에 용해시킨다. 이 용액을 0.2㎛ 크기의 구멍을 가지는 증기멸균 막 필터, 및 가압가스로서 질소를 이용해 고압 멸균된 폴리아크릴산 서스펜션에 첨가한다. 다음, 이 장치를 한번 이상 진공으로 해서 거품을 없앤다.
이제, 0.832kg의 수산화나트륨을 무균 상태하에서 대략 20kg의 물(주사용)에 용해한다. 수산화나트륨을 증기멸균된 막 필터를 통해 여과하여 센트리미드/소르비톨/폴리아크릴산 서스펜션에 첨가하고, 이 때 나머지 물(주사용)을 첨가한다. 이렇게 형성된 겔을 호모지나이저로 가공한다.
그뒤, 중간 사슬 트리글리세리드를 0.2㎛의 구멍을 가지는 무균 필터를 통해 무균겔에 첨가한 뒤, 완전히 균질화될 때까지 교반한다. 이렇게 형성된 겔의 pH값은 20℃에서 6 ∼ 8로 결정된다. 본 발명 제제의 삼투압은 260 ∼ 320mOsm/kg의 범위이다. 이렇게 형성된 겔을 무균상태에서 5g의 폴리포일 튜브에 채운다.
(실시예 2)
0.6g의 비타민A 팔미테이트 및 0.03g의 비타민E 아세테이트가 중성 오일(미리톨 318(Miritol 318))에 용해된다. 이 용액을 0.22㎛ 필터(밀리포어(Millipore))로 여과하여 멸균한다.
10g의 중성 오일 용액을 실시예 1에 따라 제조된 겔 990g에 첨가하고, 점차 윙교반기를 이용해 겔에 가한다. 20분의 교반작업 뒤 준비된 생성물을 10g의 폴리포일 튜브에 채운다. 이 겔은 20% 첨가로 그램 안약당 500 국제단위의 비타민 A함량에 해당한다.
(비교 실시예)
카르보폴 겔은 실시예 1에 따라 만들어지지만: 중간 사슬 트리글리세리드 성분은 제공되지 않는다.
(비교 실험)
실시예 2와 같이, 비교 실시예의 겔이 비타민A 팔미테이트와 일치하는 함유량으로 제공된다.
트리글리세리드 성분이 없는 비타민A 제제샘플은 표준상태하에서 실시예 2에 의한 샘플과 함께 보관된다. 총 여섯달 보관후에 트리글리세리드 성분이 없는 비교 실시예의 비타민A의 함유량은 총 20%까지 떨어진다. 실시예 2에 따른 샘플에서는, 비타민A 함유량이 측정 정밀도 내에서 변화하지 않는다는 것을 발견했다.
Claims (21)
- 무균성 점안용 겔 제제로서, 2상 담체액 또는 겔 기재가,폴리아크릴산 또는 중합성 폴리아크릴산 유도체로 이루어지는 그룹에서 선택되는 적어도 하나의 중합성 겔 형성 성분을 겔 특성을 부여하기에 충분한 양으로 함유하는 액체 수상과,적어도 하나의 지방산 유도체, 하나의 트리글리세리드 또는 프탈산에스테르로 이루어지는 그룹으로부터 선택되고, 별도의 유화제 없이 수상 중에서 방울 형태로 분산되고, 그 분산상태가 유지될 수 있는 약학적으로 수용가능한 유기 오일로 이루어지는 액체 소수성 상을 포함하여 이루어지는 것을 특징으로 하는 무균성 점안용 겔 제제.
- 제1항에 있어서, 연속상으로서의 수상과 이것에 분산된 방울형태의 소수성 상으로 이루어진 것을 특징으로 하는 무균성 점안용 겔 제제.
- 제 1항 또는 2항에 있어서, 상기 폴리아크릴산의 함량이 0.1중량%∼3중량%인 깃을 특징으로 하는 무균성 점안용 겔 제제.
- 제1항에 있어서, 상기 오일이 트리글리세리드성분을 함유하는 것을 특징으로 하는 무균성 점안용 겔 제제.
- 제4항에 있어서, 상기 오일이 중간 사슬 트리글리세리드를 포함하는 것을 특징으로 하는 무균성 점안용 겔 제제.
- 제5항에 있어서, 상기 안약이 0.5%~ 10중량%의 중간 사슬 트리글리세리드를 포함하는 것을 특징으로 하는 무균성 점안용 겔 제제.
- 제1항 또는 제2항에 있어서, 점성이 2000 ~ 6000mPa s인 것을 특징으로 하는 무균성 점안용 겔 제제.
- 제1항 또는 제2항에 있어서, pH가 6 ~ 8인 것을 특징으로 하는 무균성 점안용 겔 제제.
- 제1항 또는 제2항에 있어서, 치료제로 적어도 1종의 안약활성제를 효과적인 양으로 함유하는 것을 특징으로 하는 무균성 점안용 겔 제제.
- 제9항에 있어서, 비타민A, 비타민 A 팔미테이트, 비타민E 또는 비타민E 아세테이트를 포함하는 그룹 중에서 선택되는 항산화제와의 결합제를 함유하는 것을 특징으로 하는 무균성 점안용 겔 제제.
- 제1항 또는 제2항에 있어서, 보존제 및 등장제를 함유하는 것을 특징으로 하는 무균성 점안용 겔 제제.
- 제11항에 있어서, 상기 보존제가 센트리미드, 벤즈알코늄클로라이드 또는 티오머살이고 상기 등장제가 소르비톨인 것을 특징으로 하는 무균성 점안용 겔 제제.
- 소수성 상이 연속적인 수상에서 균질로 분산되는 것을 특징으로 하는 제1항, 2항, 4항 내지 6항, 제10항 중 어느 한 항에 따른 무균성 점안용 겔 제제의 제조방법.
- 제13항에 있어서, 적절한 염기와 반응하여 카르복실산기의 중화 작용에 의해 겔을 형성하는 폴리아크릴산을 이용하는 것을 특징으로 하는 무균성 점안용 겔 제제의 제조방법.
- 제14항에 있어서, 활성제가 무균상태에서 무균성 제제에 첨가되고, 균일하게 혼합되는 것을 특징으로 하는 무균성 점안용 겔 제제의 제조방법.
- 제1항 또는 2항에 있어서, 상기 폴리아크릴산의 함량이 0.2중량% 인 것을 특징으로 하는 무균성 점안용 겔 제제.
- 제4항에 있어서, 상기 오일이 C8-C12지방산으로 형성된 혼합 트리글리세리드 성분을 함유하는 것을 특징으로 하는 무균성 점안용 겔 제제.
- 제5항에 있어서, 상기 오일이 1중량%의 중간사슬 트리글리세리드 성분을 함유하는 것을 특징으로 하는 무균성 점안용 겔 제제.
