(57)【要約】 【課題】外科手術中に除去した栓骨を術後に正確かつ永
久的に元の頭蓋骨に固定するための、より簡単かつ迅速
に固定できる装置を提供し、スチールワイヤーで栓骨を
縫合する場合の前記問題を解消する。 【解決手段】金属または金属化合物製のピン１１及び２
個の盤２１及び２２からなる外科手術中に除去した栓骨
３２を術後に元の頭蓋骨３１に固定するための装置であ
って、各盤２１、２２は周縁に歯２１３及び２２３と、
中央を貫通する孔２１２および２２２を有し、この孔に
ピン軸１１２を挿入し、両盤２１，２２は歯２１３，２
２３を互いに対向させて該ピン軸１１２に取着し頭蓋骨
３１及び栓骨３２に締着することを特徴とする外科手術
中に除去した栓骨３２を術後に頭蓋骨３１に固定するた
めの装置。

Description

【発明の詳細な説明】
【０００１】
【発明の属する技術分野】この発明は外科手術中に除去
した栓骨（以下プラグという）を術後に元の頭蓋骨に固
定するための装置に関する。
【０００２】
【従来の技術】外科医が脳の手術中に患部に接近可能に
するために、プラグをそれ以外の頭蓋骨から除去する必
要性がしばしば生じる。術後にプラグを鋸で削ってから
元の頭蓋骨に固定する。このようなプラグはループ状ス
チールワイヤを用いて縫合しループの突出端を捻ること
によって長く頭蓋骨に固定する。
【０００３】しかし、プラグ、頭蓋骨間の接触は比較的
不安定である。この両半体は良好に融着せず頭蓋骨が炎
症をおこす可能性もある。他の欠点として、術後のコン
ピュータ断層撮影で得られる像がワイヤにより大きく歪
められ、脳の軟構造の正確な把握を困難にする。
【０００４】なるほどワイヤの代わりに生理的拒絶反応
を引き起こさない非吸収性糸を用いれば上記欠点を解消
することができるが、プラグの元の頭蓋骨への固定は依
然として不安定である。プラグの元の頭蓋骨への固定
は、例えば、非拒絶性金属薄板またはチタン薄板を用い
て行うことも可能である（ＥＰ Ａ ０ ５１０ ３９
０）。これらの薄板は両半体の当接の橋渡しをし、ねじ
止めにより固定する。また、これらの薄板は頭蓋骨の空
洞を閉じる役割をする。
【０００５】しかし、この方法も十分に満足のいくもの
ではない。その理由は、複雑な上に時間がかかりそのた
め高価になるからである。
【０００６】
【課題を解決するための手段】本発明の目的は上記従来
技術の状況を出発点として、外科手術中に除去したプラ
グを術後に正確かつ永久的に元の頭蓋骨に固定するため
の、より簡単かつ迅速な装置を提供することである。
【０００７】本発明に従えば、上記目的は、生理的拒絶
反応を引き起こさない金属又は金属化合物製のピン及び
２個の一面が凹状で他面が凸状の円形等の盤から構成し
た上記固定装置により達成することができる。
【０００８】ピンはその一端に平坦な頭部を有する。円
盤の一方は、この頭部に凭れかかる。各円盤は、凹側の
周縁に沿い列をなして延在する歯と、中央を貫通する孔
を有する。この孔にピン軸が嵌入する。両円盤は歯を互
いに対向させて該ピン軸に取着する。第２の円盤は軸に
締着することができる。
【０００９】本発明によれば、前記固定装置において、
内側の円盤は、頭部の近傍のピンに固定する。次に、こ
の円盤（第１円盤）を接合部の下のプラグの周辺に形成
した円盤より幾分大きな凹欠部に挿入する。このとき、
ピン軸はプラグ、頭蓋骨間の裂け目（カーフ）から突出
させる。
【００１０】次に、外側の円盤（第２円盤）をピンの、
カーフから突出した部分に取着する。次に、歯がプラグ
および頭蓋骨の周縁に食い込むまで両円盤を近づける。
そして、第２の円盤を軸に固定する。
【００１１】請求項２乃至７は本発明に係る装置の実施
例を記載する。生理的拒絶反応をおこさない金属として
はチタンが特に好ましい。Ｔｉ6Ａ6Ｖａのようなチタン
合金も好適である。チタン製装置は術後のコンピュータ
断層撮影で得られる像を歪めないから有用である。ピン
の頭部、軸間の渡り部を円錐状に形成し、かつその寸法
を頭部に 凭れかかる円盤が軸の周りに強締可能に設計す
れば、内側の円盤をより安定的にピン軸に取着すること
ができる。
【００１２】貫通孔から半径方向に延びるスリットを、
軸に取着した円盤に形成してもよい。円盤の中央を押し
下げるようにしてもよい。貫通孔、周縁間で、円盤の領
域を一定の間隔で除去することにより材料を節約しても
よい。
【００１３】装置を両頭蓋骨へ固定する方法は、原則的
に、目くらリベット止めと同様である。従って、各ピン
軸に切欠きを形成し、軸に取着した第２の円盤がピンか
ら滑って逃げないようにすることができる。軸に取着し
た第２の円盤が凸方向と逆方向に変形する場合は、軸に
変形部分を切欠に押し込むだけで軸に固定することがで
きる。軸にねじを形成してナットを収容してもよい。
【００１４】ナットは第２の円盤に対して締め付けるこ
とができる。あらゆる場合において、第２の円盤はその
歯が両半体の継ぎ目（ジョイント）に係合するまで変位
してプラグの隣接する周縁を頭蓋骨に固定する。
【００１５】この新規装置は簡単かつ迅速に操作可能で
ある。その上、外科手術中に除去したプラグを術後に正
確かつ永久的に元の頭蓋骨に固定することができる。
【００１６】
【発明の実施の形態】以下、本発明を添付図面を参照し
て具体的に説明する。
【００１７】外科手術中に除去したプラグを術後に元の
頭蓋骨に固定するための装置は、ピン１１と、２個の内
面が凹形で外面が凸形の円盤２１、２２からなる。ピン
１１は軸１１２と、頭部１１１からなる。先ず、一方の
円盤２１、即ち、内側円盤（第１円盤）を軸１１２に取
着してプラグ及びプラグと接合する頭蓋骨の残存部の内
表面に凭れかける。
【００１８】次に、他方の円盤２２、即ち、外側円盤
（第２円盤）を軸１１２に取着してプラグ及びプラグと
接合する頭蓋骨の残存部の外表面に凭れかける。一方の
円盤２１の中央には貫通孔２１１を形成し、他方の円盤
２２に貫通孔２２１を形成する。
【００１９】一列の歯２１３を円盤２１の凹側の周縁に
沿って形成し、他の一列の歯２２３を円盤２２の凹側の
周縁に沿って形成する。図１および図４から明らかなよ
うに、円盤２１、２２は凹側及び歯２１３と２２３を対
向させるようにしてピン１１の軸１１２に取り付ける。
【００２０】軸１１２は円盤２１の貫通孔２１１に隙間
なく嵌入する。図２に示すように、いづれの円盤にも貫
通孔２１１の中央から半径方向外側に向かって延びるス
リット２１４を形成することができる。ピン１１の頭部
１１１、軸１１２間の渡り部を円錐状に形成すればスリ
ット２１４の存在と相俟って、円盤はピン１１に対して
正しく位置決めされ、装置の装着後はプラグおよび頭蓋
骨を両部材が支持することになる。
【００２１】両円盤の中央の貫通孔、周縁間の領域に、
図３に示すように、複数の穴２２６を形成して材料を節
約するとともに重量を軽くすることができる。各円盤は
スリット２１４および穴２２６の両方を有することもで
きる。
【００２２】図５及び図６は装置をどのように採用する
かを示す図である。図５は外科医が穴３３を介して脳に
接近可能にするために既にプラグ３２を大人の頭蓋骨３
１から鋸引きした頭蓋骨のその部分を示す図である。円
盤２１より幾分大きな凹欠部３２１がプラグの周縁部分
を除去し形成されている。手術が終了した後、プラグを
穴３３に戻す。
【００２３】次に、内側の円盤２１をピン１１の軸１１
２に取着する。内側の円盤は軸を外に突きだした状態で
凹欠部３２１を介して１つずつ挿入され、軸をカーフか
ら突出させた状態で矢印Ａで示す方向にカーフ３３１内
を所定の位置までプラグ及び頭蓋骨の内面に沿って滑ら
せる。
【００２４】外側の円盤２２はピンの軸に取着する。外
側の円盤２２は目くらリベットを締め付けるために使用
するタイプの工具を用いて最終的に軸に固定する。この
工具は外側の円盤２２を内側の円盤２１と一緒に歯がプ
ラグおよび頭蓋骨の組織に食い込むまで矢印Ｂで示す方
向に押しつけて両者を固着する。外側の円盤から外方に
延出した各軸の部分を切り取る。軸にねじ部を設け歯が
組織に食い込むまで円盤を強制的に互に合体する方向に
ねじ込んでも良い。
【００２５】再述すると、外科手術中に除去した栓骨を
術後に頭蓋骨に固定するための装置は以下の構成を有す
る。
【００２６】生理的拒絶反応を引き起こさない金属又は
金属化合物製のピン１１及び２個の円形等の盤２１およ
び２２からなる外科手術中に除去したプラグを術後に元
の頭蓋骨に固定するための装置であって、ピンはその一
端に平坦な頭部１１１を有し、第１の盤２１は、この頭
部に凭れかかり、各盤は、凹側の周縁に沿い列をなして
延在する歯２１３および２２３と、中央を貫通する孔２
１２および２２２を有し、この孔にピン軸１１２が嵌入
し、両盤は歯を互いに対向させて該ピン軸に取着し、第
２の盤２２は軸に締着する。
【００２７】前記ピン１１および盤２１、２２はチタン
または生理的非拒絶性チタン合金からなる。前記スリッ
ト２１４は第１の盤２１を介して孔２１１から半径方向
外方へ延びている。第１の盤２１又は第２の盤２２の中
央はその凸方向と反対の方向に変形している。穴２２６
が盤２１及び２２の全領域に均一に分布している。
【００２８】第２の盤２２の外側面に係合してピン１１
の頭部１１１の方に押しつけるピン１１の軸１１２に形
成した切欠きを有する。ピン１１の軸１１２にはねじ部
を形成し、ナットを螺合して第２の盤２２に対してねじ
止め可能にする。
【００２９】
【発明の効果】本発明によれば内側と外側の盤がピン軸
を介して締結されることにより頭蓋骨と栓骨を強固にク
ランプし、歯が組織に喰い込むことによって確実且つ安
定なる栓骨固定が行なえる。
【００３０】即ち本発明によれば外科手術中に除去した
栓骨を術後に正確かつ永久的に元の頭蓋骨に固定するた
めの、より簡単かつ迅速に固定できる装置を提供するこ
とができ、スチールワイヤーで栓骨を縫合する場合の前
記問題を解消できる。

    【特許請求の範囲】
  1. 【請求項１】金属または金属化合物製のピン及び２個の
    盤２１及び２２からなる外科手術中に除去した栓骨を術
    後に元の頭蓋骨に固定するための装置であって、各盤は
    周縁に歯２１３及び２２３と、中央を貫通する孔２１２
    及び２２２を有し、この孔にピン軸１１２を挿入し、両
    盤は歯を互いに対向させて該ピン軸１１２に取着し栓骨
    ３２及び頭蓋骨３１を締着することを特徴とする外科手
    術中に除去した栓骨を術後に頭蓋骨に固定するための装
    置。
  2. 【請求項２】前記ピン１１及び盤２１、２２はチタン又
    は生理的非拒絶性チタン合金からなることを特徴とする
    クレーム１に記載の外科手術中に除去した栓骨を術後に
    頭蓋骨に固定するための装置。
  3. 【請求項３】スリット２１４が盤２１又は２２の孔２１
    １又は２２２から半径方向外方へ延びていることを特徴
    とするクレーム１又はクレーム２に記載の外科手術中に
    除去した栓骨を術後に頭蓋骨に固定するための装置。
  4. 【請求項４】盤２１又は２２の中央はその凸方向と反対
    の方向に変形していることを特徴とするクレーム１乃至
    クレーム３のいずれかに記載の外科手術中に除去した栓
    骨を術後に頭蓋骨に固定するための装置。
  5. 【請求項５】穴２２６が盤２１又は２２の全領域に均一
    に分布していることを特徴とするクレーム１乃至クレー
    ム４のいずれかに記載の外科手術中に除去した栓骨を術
    後に頭蓋骨に固定するための装置。
  6. 【請求項６】外側の盤２２の外側面に係合するピン１１
    の軸１１２に形成した切欠きを有することを特徴とする
    クレーム１乃至クレーム５のいずれかに記載の外科手術
    中に除去した栓骨を術後に頭蓋骨に固定するための装
    置。
  7. 【請求項７】ピン１１の軸１１２にはねじ部を形成し、
    ナットを螺合して外側の盤２２に対してねじ止め可能に
    することを特徴とするクレーム１乃至クレーム５のいず
    れかに記載の外科手術中に除去した栓骨を術後に頭蓋骨
    に固定するための装置。
