Claims (1)
- 【特許請求の範囲】 1 腹壁を腹膜の手前で持ち上げる装置は以下のものからなる。即ち、 (a)相互に関連し合って可動可能な一対のアングル形状部位、アングル形状部 位は以下のものを備える。 即ち、一般的に平行関係を保つように配置した第1脚部で、第1脚部には方位角 度があり、且つ(1)第1脚部から横方向に伸びる第2脚部は以下の通りでる。 即ち、 (i))相互に関連し合って第1脚部の回転に合わせて扇形状に展開し、 (ii))腹壁と接するように適合し、且つ(iii))第1脚部から末梢部の 方向に減少する第1脚部の方向に有効な肉厚を有し、及び (b)アングル形状部位に揚力を伝える部位。 2 請求項1記載による装置に於いて、第2脚部の長さと断面についてはそれぞ れ以下のごとし。即ち、第1脚部に近接する第2脚部の長手方向にある第1領域 の断面は、事実上半円形であり、この半円形断面は第1脚部と事実上平行な口径 を有し、第1領域に近接する第2脚部の長手方向にある第2領域の断面は、次第 に事実上長方形に変化し、第1脚部と事実上平行な長軸を持つ長方形断面、及び 第2領域に近接する第3脚部の断面は、事実上長方形を保持し、且つ第2脚部の 長手方向にある第3領域に隣接する第4領域では、第1脚部と事実上平行な部分 から第3領域に向けて第1脚部と事実上垂直になるように徐々に変化する長軸を 持つ長方形断面、及び有効な肉厚を与えるために第4領域に於いて第1脚部と事 実上垂直方向を保持しているが、この有効厚さ寸法は第1脚部から末梢部に向け て減少する。 3 請求項2記載による装置に於いて、アングル形状部位の第2脚部の一方の長 手方向にある第1領域の断面は、アングル形状部位のもう一方の第2脚部の長手 方向にある第1領域の断面に対して相似形である。 4 請求項2記載による装置に於いて、アングル形状部位の第2脚部の一方の長 方形断面の長軸は、第1脚部と事実上平行な状態から第1方位角をとる第1脚部 に対して事実上垂直になるよう変化し、且つ、アングル形状部位のもう一方の第 2脚部の長方形断面の長軸は、第1脚部と事実上平行な状態から第1方位角をと る第1脚部と事実上垂直になるよう変化する。 5 請求項2記載による装置に於いて、アングル形状部位の第2脚部が閉の状態 にある場合、当該第2脚部は相互に事実上平行であり、 アングル形状部位の第2脚部の長手方向にある第1及び第2領域は、第1面にお けるアングル形状部位の第2脚部のもう一方の脚部の長手方向にある第1及び第 2領域に対して事実上平行であり、且つ、アングル形状部位の第2脚部の一方の 脚部の長手方向にある第4領域は、第1面と事実上垂直な面におけるアングル形 状部位の第2脚部のもう一方の脚部の長手方向にある第4領域に対して事実上平 行である。 6 請求項2記載による装置に於いて、第2脚部の長手方向にある第1領域、第 2領域、第3領域、及び第4領域の各々は、当該第2脚部の全長の概ね1/4に 等しい長さを有する。 7 請求項1記載による装置に於いて、第2脚部が持つ長さと断面について、 第1脚部に隣接する第2脚部の長手方向にある第1領域の断面は、事実上半円形 であり、この半円形断面は第1脚部と事実上平行な口径を有し、第1脚部に隣接 する第2脚部の長手方向にある第2領域の断面は、次第に事実上卵形に変化し、 当該卵形断面は第1脚部と事実上平行な主軸を有し、且つ、第2領域に隣接する 第3領域の断面は事実上卵形を保持し、第2脚部の長手方向にある第3領域に隣 接する第4領域の卵形断面は、第1脚部と事実上平行な状態から第3領域にかけ て第1脚部と事実上垂直になるように徐々に変化する主軸を有し、且つ、有効な 肉厚を与えるために第4領域に於いては第1脚部に対し事実上垂直を保持するが 、この有効な厚さ寸法は第1脚部から抹消部にかけて減少する。 8 請求項7記載による装置に於いて、アングル形状部位の第2脚部の一方の長 手方向にある第1領域の断面は、アングル形状部位のもう一方の第2脚部の長手 方向にある第1領域の断面に対して相似形である。 9 請求項7記載による装置に於いて、アングル形状部位の第2脚部の一方の脚 部の卵形断面の主軸は、第1脚部と事実上平行な状態から第1方位角をとる第1 脚部と事実上垂直になるように変化し、且つ、アングル形状部位の第2脚部のも う一方の脚部の卵形断面の主軸は、第1脚部と事実上平行な状態から第1方位角 をとる第1脚部と事実上垂直になるよう変化する。 10 請求項7記載による装置に於いて、アングル形状部位の第2脚部が閉の位 置にある場合、第2脚部は事実上それぞれの脚部と平行であり、 アングル形状部位の第2脚部の一方の長手方向にある第1及び第2領域は、第1 面におけるアングル形状部位の第2脚部のもう一方の脚部の長手方向にある第1 及び第2領域に対し事実上平行であり、且つ、アングル形状部位の第2脚部の一 方の脚部の長手方向にある第4領域は、第1面と事実上垂直な面に於いてアング ル形状部位のもう一方の第2脚部の長手方向にある第4領域に対し事実上平行で ある。 11 請求項7記載による装置に於いて、第2脚部の長手方向にある第1領域、 第2領域、第3領域、及び第4領域の各々は、当該第2脚部の全長の概ね1/4 に等しい長さを有する。 12 腹壁を持ち上げための装置は以下のごときものからなる。即ち、 (a)相互に関連し合って可動可能な一対のアングル形状部位で、アングル形状 部位は以下のものを備える。即ち (1)一般的に平行関係を保つように配置した第1脚部であり、当該第1脚部は 方位角度を持ち、且つ、 (2)第1脚部から横方向に伸びる第2脚部は、(i)第1脚部の回転に合せて 相互に関連して合って扇形状に展開し、 (ii)腹壁に接するように適合し、且つ、(iii)第1脚部の方向に剛性が あるが、この剛性は第1脚部から末梢部の方向にかけて減少し、及び (b)アングル形状部位に揚力を伝える持ち上げ部位。 13 請求項12記載による装置に於いて、第2脚部の長さと断面に関して以下 のごとくである。即ち、第1脚部に近接第2脚部の長手方向にある第1領域の断 面は事実上半円形であり、この半円形断面は第1脚部に対して事実上平行な口径 を有し、第1脚部に近接する第2脚部の長手方向にある第2領域の断面は、次第 に事実上長方形に変化し、この長方形断面は第1脚部と事実上平行な長軸を有し 、且つ第2領域に隣接する第3領域の断面は、事実上長方形を保ち、第2脚部の 長手方向にある第3領域に隣接する第4領域における長方形断面は、第1脚部と 事実上平行な状態から第3領域にかけて第1脚部と事実上垂直になるように徐々 に変化し、且つ剛性を与えるために第4領域に於いては第1脚部と事実上垂直に なっているが、この剛性は第1脚部から末梢部にかけて減少する。 14 請求項13記載による装置に於いて、アングル形状部位の第2脚部の一方 の脚部の長手方向にある第1脚部の断固は、当該アングル形状部位のもう一方の 第2脚部の長手方向にある第1領域の断面とは相似形である。 15 請求項13記載による装置に於いて、アングル形状部位の第2脚部の一方 の長方形断面の長軸は、第1脚部と事実上平行な状態から第1方位角をとる第1 脚部と事実上垂直になるように変化し、且つ、アングル形状部位のもう一方の第 2脚部の長方形断面の長軸は、第1脚部と事実上平行な状態から第1方位角をと る第1脚部と事実上垂直になるように変化する。 16 請求項13記載による装置に於いて、アングル形状部位の第2脚部が閉の 状態にある場合、当該第2脚部は相互に事実上平行であり、 アングル形状部位の第2脚部の一方の脚部の長手方向にある第1及び第2領域は 、第1面におけるアングル形状部位の第2脚部のもう一方の脚部の長手方向にあ る第1及び第2領域に対して事実上平行であり、且つ、 アングル形状部位の一方の長手方向にある第4領域は、第1面と事実上垂直な面 に於いてアングル形状部位の第2脚部のもう一方の脚部の長手方向にある第4領 域と事実上平行である。 17 請求項13記載による装置に於いて、第2脚部の長手方向にある第1領域 、第2領域、第3領域、及び第4領域の各々は、当該第2脚部の全長の概ね1/ 4に等しい長さを有する。 18 請求項12記載による装置に於いて、第2脚部の長さと断面についてはそ れぞれ以下のごとし。即ち、第1脚部に近接する第2脚部の長手方向にある第1 領域の断面は事実上半円形であり、この半円形断面は第1脚部と事実上平行な口 径を有し、 第1脚部に隣接する第2脚部の長手方向にある第2領域の断面は次第に事実上卵 形に変化し、当該卵形断面は第1脚部と事実上平行な主軸を有し、且つ、第2領 面におけるアングル形状部位の第2脚部のもう一方の脚部の長手方向にある第1 及び第2領域と事実上平行であり、且つ、アングル形状部位の第2脚部の一方の 長手方向にある第4領域は、第1面と事実上垂直な面におけるアングル形状部位 の第2脚部のもう一方の脚部の長手方向にある第4領域と事実上平行である。 22 請求項18記載による装置に於いて、第2脚部の長手方向にある第1領域 、第2領域、第3領域、及び第4領域の各々は、当該第2脚部の全長の概ね1/ 4に等しい長さを有する。 23 腹壁を持ち上げための装置は以下のごときものからなる。即ち、 (a)相互に関連し合って可動可能な一対のアングル形状部位で、アングル形状 部位は以下のものを備える。即ち (1)一般的に平行関係を保つように配置した第1脚部、及び、 (2)第1脚部から横方向に伸びる第2脚部、相互に関連し合って第1脚部の回 転に合わせて扇形状に展開し、且つ腹壁と接するように適合する第2脚部、 (b)第1脚部に対して事実上等しく対応するアングルによって、第1アングル 形状部位のもう一方を回転させるために、アングル形状部位の一方の回転に対応 する二重反転装置、及び、 (c)アングル形状部位に揚力を伝える持ち上げ部位。 24 請求項23記載による装置に於いて、二重反転装置はアングル形状部位の もう一方の第1脚部に放射状に配置したソケットと噛み合うアングル形状部位の 第1脚部の一方から放射状に伸展するペグを備える。 25 請求項23記載による装置に於いて、二重反転装置はアングル形状部位の もう一方の第1脚部に取付けたギアと噛み合うアングル形状部位の第1脚部の一 方に取付けたギアを備える。 26 請求項23記載による装置に於いて、二重反転装置はアングル形状部位の もう一方の第1脚部の部分と摩擦接触するアングル形状部位のもう一方の第1脚 部の部分を構成する。 27 請求項23記載による装置に於いて、相互に関連するアングル形状部位を 回転させるために、アングル形状部位に補助的に取付けた操作装置を備える。 28 請求項27記載による装置に於いて、操作装置は第2脚部から末梢部にか けて各アングル形状部位の第1脚部に取付けたレバーを備え、各レバーは第1面 における第2脚部と事実上平行に取付けられ、第1面と垂直な面の第2脚部から 突き出すように離して取付けてある。 29 請求項28記載による装置に於いて、アングル形状部位の第2脚部が閉の 状態にある場合、第2脚部は相互に事実上平行で、且つ展開位置では、 レバーを一緒に合わせるように握り締めることで、第2脚部は閉の位置から開の 位置に移動する。 30 請求項27記載による装置に於いて、アングル形状部位の第2脚部が閉の 状態にある場合、第2脚部は相互に事実上平行で、且つ展開位置では、 閉の状態で第2脚部を固定するためと、展開した状態で第2脚部を固定するため に補助的に操作装置に結合する固定装置を構成する。 31 請求項27記載による装置に於いて、第1脚部は方位角度を有し、第2脚 部は第1脚部の方向に剛性を有するが、この剛性は当該第1脚部から末梢部に向 けて減少する。 32 請求項23記載による装置に於いて、第1脚部は方位角度を有し、第2脚 部は第1脚部の方向に有効な肉厚を有するが、この肉厚は当該第1脚部から末梢 部にかけて減少する。 33 腹壁を持ち上げるための装置は、以下のごときものからなる。即ち、 (a)相互に関連し合って可動可能な一対のアングル形状部位、アングル形状部 位は以下のものを備える。 即ち (1)一般的に平行関係を保つように配置した第1脚部、及び、 (2)第1脚部から横方向に伸びる第2脚部、相互に関連し合う第1脚部の回転 に合わせて扇形状に展開し 且つ、腹壁と接するように適合する第2脚部、及び、 (b)アングル形状部位に揚力を伝える持ち上げ装置、及び (c)揚力測定用の持ち上げ装置に連結した揚力測定装置。 34 請求項23記載による装置に於いて、(a)持ち上げ装置は以下のごとき ものからなる。即ち、 (1)アングル形状部位の第1脚部を受け止める内径孔に沿ってその途中にリッ プを備えた内径孔を有する管状スリーブ、及び、 (2)アームを持ち上げるための管状スリーブに取付ける装置、及び、 (b)補助的に備える揚力測定装置、即ち、(1)管状スリーブの内径孔に滑り 込ませるように取付ける取付ブロック、管状で接触面を備えた取付ブロック、及 び揚力を示すための揚力指示装置、接触面と向き合う第2脚部を備えた取付ブロ ックに回転させながら取付けたアングル形状部位の第1脚部、及び (2)取付ブロックのリップと接触面の間に位置する管状スリーブの内径孔の中 に配置したコイル・スプリング。 35 請求項34記載による装置に於いて、持ち上げ装置は補助的に管状スリー ブに取付けたドブテールを備える。 36 請求項34記載による装置に於いて、取付ブロックは揚力に対応する管状 スリーブと相関的な軸運動を有し、且つ、 取付ブロックの軸運動に対応する揚力を示す表示装置を有する。 37 請求項36記載による装置に於いて、管状部位は管状スリーブに接する接 触面を有し、且つ、揚力表示装置は管状スリーブの隣接面上に表示した目盛を備 えている。 38 請求項36記載による装置に於いて、(a)取付ブロックは以下のものを 備えている。即ち、(1)接触面と対持する第2表面、基準マークを持つ覗き窓 を備えた第2表面、 (2)覗き窓に隣接する曲面をした溝、及び、(b)揚力表示装置は以下のごと きものを含め、補助的に伸長テープを備える。即ち、 (1)管状スリーブの内径孔に取付けた端末部、(2)目盛を付けた表面、及び 、 (3)基準マークに隣接する覗き窓に表示する目盛のように曲面を持った溝と接 するエッジ部分。 39 請求項36記載による装置に於いて、(a)取付ブロックは以下のものを 備えている。即ち、(1)接触面に対峙する第2表面、目盛が付いた覗き窓を持 つ第2表面、及び、 (2)覗き窓に隣接する曲面をもった溝、及び、(b)表示装置は以下のごとき ものを含め、補助的に伸長テープを備える。即ち、 (1)管状スリーブの内径孔に取付けた端末部、(2)基準マークを付けた表面 、及び、(3)目盛に隣接する覗き窓に表示する基準マークのように曲面を持っ た溝と接するエッジ部分。 40 請求項33記載による装置に於いて、第1脚部は方位角度を有し、且つ、 第2脚部は第1脚部の方向に剛性を有するが、この剛性は第1脚部から抹消部に かけて減少する。 41 請求項33記載による装置に於いて、第1脚部は方位角度を有し、且つ、 第2脚部は第1脚部の方向に有効な肉厚を有するが、この有効な肉厚は第1脚部 から抹消部にかけて減少する。 42 腹壁を持ち上げ方向に持ち上げるための装置は、以下のごときものからな る。即ち、 (a)相互に関連し合って可動可能な一対のアングル形状部位、アングル形状部 位は以下のものを備える。 即ち、 (1)一般的に平行関係を保つように配置した第1脚部、及び、 (2)第1脚部から横方向に伸びる第2脚部、相互に関連し合って第1脚部の回 転に合わせて扇形状に展開し且つ、腹壁と接するように適合した第2脚部、 (b)アングル形状部位に揚力を伝える持ち上げ装置、持ち上げ方向にアングル 形状部位に揚力を伝えるための一方向性カプリングを備え、且つ、持ち上げ方向 と逆の方向にはアングル形状部位に事実上力が掛からないようにした持ち上げ装 置。 43 請求項42記載による装置に於いて、一方向性のカプリングはドブテール ・コネクターを備える。 44 腹壁を持ち上げる装置は、以下のごときものからなる。即ち、 (a)相互に関連し合って可動可能な一対のアングル形状部位、アングル形状部 位は以下のものを備える。 即ち、 (1)一般的に平行関係を保つように配置した第1脚部、及び、 (2)第1脚部から横方向に伸びる第2脚部、第2脚部は以下の通り。即ち、 (i)相互に関連し合って第1脚部の回転に合わせて扇形状に展開し、 (ii)腹壁と接するように適合し、 (iii)内部通路を備え、且つ、 (b)アングル形状部位に揚力を伝えるための持ち上げ装置。 45 請求項44記載による装置に於いて、第1脚部の少なくとも1個は内部通 路と接続する内孔を有する。 46 請求項44記載による装置に於いて、更に内部通路内にある光ファイバー を含む。 47 請求項44記載による装置に於いて、第1脚部の少なくとも1個は、内部 通路と接続する内孔を有し、且つ光ファイバーが内孔内を貫通する。 48 請求項44記載による装置に於いて、第2脚部は表面を有し、内部通路は 以下のものを備える。即ち、第2脚部の表面につながる横断通路、及び横断通路 につながる縦通路。 49 腹膜外脂肪層と腹膜を含む腹壁を持ち上げる方法論、当該方法論は以下の ごとき手順からなる。即ち、(a)第1脚部、及び当該第1脚部から端末部に向 けて横方向に伸びる第2脚部を有するアングル形状部位を備えたリトラクターを 準備し、 (b)腹壁を通して腹膜外脂肪層まで切開術を施し、(c)この切開部を通して 腹膜付近まで第2脚部の端末部を導入し、 (d)腹膜外脂肪層と腹膜の間に第2脚部を進入させた上で、 (e)アングル形状部位に揚力を伝達する。 50 請求項49記載による方法論に於いて、第1脚部は方位角度を有し、且つ リトラクターの準備段階は、第1脚部から抹消部にかけて減少する剛性が、第1 脚部の方向に発揮される第2脚部を備えたリトラクターの準備によるものである 。 51 請求項49記載による方法論に於いて、第1脚部は方位角度を有し、且つ リトラクターの準備段階は、第1脚部から抹消部にかけて減少する有効な肉厚が 、第1脚部の方向に向けて第2脚部の有効な肉厚を構成するリトラクターの準備 によるものである。 52 腹膜外脂肪層と腹膜を含む腹壁を腹膜の手前で持ち上げる方法論は、以下 のごとき段階による。即ち、(a)相互に関連し合って可動可能な一対のアング ル形状部位を備えたリトラクターを準備し、リトラクターは以下のものを備える 。即ち、(1)一般的に平行関係を保つように配置した第1脚部、及び、 (2)第1脚部から端末部に向けて横方向に伸び、相互に関連し合って第1脚部 の回転に合わせて扇形状に展開する第2脚部、 (b)腹壁を通して腹膜外脂肪層まで切開術を施し、(c)当該切開部を通して 腹膜付近まで第2脚部の端末部を導入し、 (d)腹膜外脂肪層と腹膜の間に第2脚部を進入させ、(e)第2脚部を扇形状 に展開させるために、相互に関連し合う第1脚部を回転させ、 (f)アングル形状部位に揚力を伝達する。 53 請求項52記載による方法論に於いて、第1脚部は方位角度を有し、且つ リトラクターの準備手段は、第1脚部から抹消部にかけて減少する剛性が、第1 脚部の方向に発揮される第2脚部を備えたリトラクターの準備によるものである 。 54 請求項52記載による方法論に於いて、第1脚部は方位角度を有し、且つ リトラクターの準備手段は、第1脚部から抹消部にかけて減少する有効な肉厚が 、第1脚部の方向に向けて第2脚部の有効な肉厚を構成するリトラクターの準備 によるものである。 55 請求項52記載による方法論に於いて、腹膜外脂肪層と腹膜の間にリトラ クターの第2脚部の端末部を進入させる手段によって、腹膜外脂肪層から腹膜を 引き離し、且つ、 第2脚部を扇形状に展開させるために、相互に関連し合って第1脚部を回転させ る手段を用いて、腹膜外脂肪層から腹膜の更に精密な部分を引き離す。 56 請求項52記載による方法論に於いて、リトラクターの準備手段には揚力 表示器を備えたリトラクターの準備が含まれ、且つ、 リトラクターの第1脚部に揚力を伝える手段には、当該揚力を決定する揚力表示 計による監視を含む。 57 腹壁を持ち上げる方法論は以下のごとき手段からなる。即ち、 (a)相互に関連し合って可動可能な一対のアングル形状部位を備えたりトラク ターを準備し、当該リトラクターは以下のものを含む。即ち、 (1)一般的に平行関係を保つように配置した第1脚部、及び、 (2)第1脚部から端末部に向けて横方向に伸びる第2脚部は、 (i)相互に関連し合って第1脚部の回転に合わせて扇形状に展開し、 (ii)第1脚部の方向に向けて剛性を有するがこの剛性は第1脚部から抹消部 にかけて減少する。 (b)腹壁を貫通する切開術を施し、 (c)当該切開部に第2脚部を進入させ、(d)第2脚部を扇形状にに展開させ るために、相互に関連し合う第1脚部を回転させ、 (e)第2脚部を腹膜に当てて、 (f)アングル形状部位に揚力を伝達する。 58 腹壁を持ち上げる方法論は以下のごとき手段からなる。即ち、 (a)相互に関連し合って可動可能な一対のアングル形状部位を備えたリトラク ターを準備し、当該リトラクターは以下のものを備える。即ち、 (1)一般的に平行関係を保つように配置した第1脚部、及び、第1アングルに よって第1脚部の1個の回転と連動した動きは第1アングルに等しく対峙するア ングルによって第1脚部のもう一方の脚部をを回転させ、且つ (2)第1脚部から横方向に伸びる第2脚部は、相互に関連し合う第1脚部の回 転と等しい逆回転に合わせて対称的に扇形状に展開する。 (b)腹壁を貫通する切開術を施し、 (c)当該切開部に第2脚部を進入させ、(d)第2脚部を扇形状に展開させる ために、相互に関連し合う第1脚部を回転させ、 (e)第2脚部を腹膜に当てて、 (f)アングル形状部位に揚力を伝達する。 59 腹壁を持ち上げる方法論は、以下のごとき手段からなる。即ち、 (a)相互に関連し合って可動可能な一対のアングル形状部位を備えたりトラク ターを準備し、当該リトラクターは以下のものを備えている。即ち、(1)一般 的に平行関係を保つように配置した第1脚部、 (2)第1脚部から横方向に伸びる第2脚部は、相互に関連し合って第1脚部の 回転に合わせて扇形状に展開し、且つ、 (3)揚力測定用の揚力測定装置、 (b)腹壁を貫通する切開術を施し、 (c)当該切開部に第2脚部を進入させ、(d)第2脚部を扇形状に展開させる ために、相互に関連し合う第1脚部を回転させ、 (e)第2脚部を腹膜に当て、 (f)アングル形状部位に揚力を伝達して、(g)揚力測定装置を用いて揚力を 監視する。 60 腹壁の下部にある組織を処置する方法論は以下のものからなる。即ち、 A.以下のものを備えた扇形リトラクターを準備する。即ち、 (a)相互に関連し合って可動可能な一対のアングル形状部位は以下のものを備 えている。即ち、(1)一般的に平行関係を保つように配置した第1脚部、及び 、 (2)第1脚部から横方向に伸びる第2脚部は、(i)相互に関連し合う第1脚 部の回転に合わせて扇形に展開し、 (ii)各々は面を有する。 (b)近端部及び端末部を有する光ファイバーの端末部は第2脚部の1個の脚部 の表面まで、第2脚部の1個の脚部にある内部通路を貫通し、且つ、(c)アン グル形状部位に揚力を伝える持ち上げ装置、 B.光源を準備し、 C.腹壁に扇形リトラクターの第2脚部を当て、D.扇形リトラクターの持ち上 げ装置に揚力を伝達して腹壁を持ち上げ、 E.光源に光ファイバーの近端部をつないで照明を当て、 F.光ファイバーの近端部から放射された光線による照明の下で組織を処置する 。 61 請求項60記載による装置に於いて、扇形リトラクターの少なくとも1個 の第1脚部は内部通路と接続する内孔を有し、且つ、 光ファイバーの端末部が第1脚部の1個の脚部にある内孔を補助的に貫通する。 62 腹壁の下部にある組織を処置する方法論は以下のものからなる。即ち、 A.以下のものを備えた扇形リトラクターを準備する。即ち、 (a)相互に関連し合って可動可能な一対のアングル形状部位は以下のものを備 えている。即ち、(1)一般的に平行関係を保つように配置した第1脚部、及び 、 (2)第1脚部から横方向に伸びる第2脚部は、(i)相互に関連し合う第1脚 部の回転に合わせて扇形に展開し、 (ii)各々の第2脚部は面を有し、 (iii)少なくとも第2脚部の1個の表面と接続する内部通路を備えた少なく とも1個の第2脚部、及び、 (b)アングル形状部位に揚力を伝える持ち上げ装置、 B.吸引管を準備し、 C.扇形リトラクターの第2脚部を腹壁に当て、D.扇形リトラクターの持ち上 げ装置に揚力を伝達して腹壁を持ち上げ、 E.内部通路に吸引管を接続して、 F.内部通路を通して吸引管に体液を吸引させる。 63 腹壁の下部にある組織を処置する方法論は以下のものからなる。即ち、 A.以下のものを備えた扇形リトラクターを準備する。即ち、 (a)相互に関連し合って可動可能な一対のアングル形状部位は以下のものを備 えている。即ち、(1)一般的に平行関係を保つように配置した第1脚部、及び 、 (2)第1脚部から横方向に伸びる第2脚部は、(i)相互に関連し合う第1脚 部の回転に合わせて扇形に展開する第2脚部、 (ii)各々の第2脚部は面を有し、且つ、(iii)少なくとも第2脚部の1 個の表面と接続する内部通路を備えた少なくとも1個の第2脚部、及び、 (b)アングル形状部位に揚力を伝える持ち上げ装置、B.注入液の源泉部を準 備し、 C.扇形リトラクターの第2脚部を腹壁に当て、D.扇形リトラクターの持ち上 げ装置に揚力を伝達して腹壁を持ち上げ、 E.内部通路に注入液の源泉部をつないで、F.注入液の源泉部から内部通路を 通して注入液を注入する。 64 請求項52記載による方法論に於いては、注入ガスである。
