Abstract

(57)【要約】 【目的】 爪を通しての抗眞菌薬剤の浸透を改善する。 【構成】 皮フ病学的組成物において、特にアリルアミ
ン族例えば塩酸テルビナフィンと塩酸ナフチフィンとか
ら選ばれる少くとも１つの抗眞菌薬剤と、特にグリコー
ルとグリコールモノエーテルとグリコールジエーテルと
ジメチルスルホキシドとカプロラクタムとジメチルイソ
ソルビドとイソプロピリデングリセロールとジメチルイ
ミダゾリジノンと酪酸エチルとポリオキシエチレン化Ｃ
8−Ｃ10グリセリドとポリエチレングリコール２０グリ
セリルラウラートとジメチルアセタミドとから選ばれる
少くとも１つの親水性浸透剤と、抗眞菌薬剤と、前記浸
透剤が少くとも一部分混合できる水−アルコール溶剤媒
質とを溶解含有させる。

Description

【発明の詳細な説明】
【０００１】
【産業上の利用分野】本発明は、主として皮フ糸状菌ま
たはＣａｎｄｉｄａによる、人および動物双方における
爪糸状菌症処置用皮フ病学的組成物中への親水性浸透剤
の使用と、そのような浸透剤を含有する皮フ病学的組成
物とに関する。
【０００２】Ｚａｉａｓら［Ｃｌｉｎｉｃａｌ Ｅｘｐ
ｅｒｉｍｅｎｔａｌ Ｄｅｒｍａｔｏｌｏｇｙ １９８
９，１４，１２０−１２３］はテルビナフィンの経口投
与によるＴｒｉｃｈｏｐｈｙｔｏｎ ｒｕｂｒｕｍに起
因する爪糸状菌症の処置を記載している。構造および皮
フ糸状菌に対する活性とが新奇なこの抗眞菌薬剤は経口
的に低い毒性を示し爪糸状菌症の長期間処置に用いて成
功している。最も効果的な処置は６ケ月間のテルビナフ
ィン２５０ｍｇ／日の経口投与にある。これは既知の医
薬品例えばケトコナゾールとグリセオフルビンと比較し
て改良されている。しかし、テルビナフィンの投与は不
利益さ、即ち一方では処置の期間が非常に長びきその処
置を承諾しないと云う高い可能性を持つこと、他方では
その期間と毎日テルビナフィン２５０ｍｇを想定される
ことに結びついて処置費が非常に高いと云う不利益さを
示す。
【０００３】これらの理由のため、爪透過経路による爪
糸状菌症の処置が望まれてきた。そこで持ち上がる問題
は抗眞菌薬剤が爪の中および爪の下に浸透することを保
証することにある。そのような抗眞菌薬剤は有利にはア
リルアミン族に属するものよりなるものであって、テル
ビナフィンはその１つでそれは、現在では経口経路で用
いることができるその族の中の唯一つのものである。
【０００４】これに関連して、ヨーロッパ特許公告第Ａ
−００６４ ８３０号で、イミダゾール族に属する脂溶
性抗眞菌薬剤のＣ8〜Ｃ20脂肪酸例えばウンデチレン酸
に溶解した、濃厚溶液で更に低級アルカノール例えばエ
タノールとイソプロパノールと、エステル例えば酢酸エ
チルと、ケトン例えばアセトンとブタノン−２とから選
ばれる溶剤に溶解されている濃厚溶液を爪透過経過で投
与することが提案された。ウンデチレン酸はまたイミダ
ゾールの抗眞菌性を増強するのに役立つとは云え、前記
のＣ8〜Ｃ20脂肪酸は爪透過浸透剤の役を演じている。
この型の組成物は“ＴＲＯＳＹＤ溶液”の名で市販され
ていてチオコナゾール２８％を含有し、それはグリセオ
フルビンによる経口処置を補うのに用いられている。
【０００５】又フランス特許公告第Ｂ−２ ５２３ ４
４７号より、製剤の全重量に対し尿素５〜２０ｗｔ％と
共に、１つまたはそれ以上の抗眞菌性活性成分を含有す
るクリームまたは軟膏に基く抗眞菌性治療製剤が知られ
ている。
【０００６】技術の状態から、今でも、自分では爪の中
に容易には拡散しない抗眞菌薬剤の浸透を増大させよう
とする試みの中で、“マニキュア”型の組成物が提案さ
れたことは明らかである。これらは重合成分を含有し、
計画された速さでその活性製剤を解放しようとするもの
である。例えば日本特許公告第０１−１１０ ６２０号
はポリビニル酢酸と抗眞菌薬剤、好ましくはイミダゾー
ルと長鎖Ｃ8〜Ｃ10脂肪酸と（または）高級アルコール
と揮発性溶剤とを含有する、爪の上に使用する抗眞菌性
組成物を記載している。
【０００７】ヨーロッパ特許公告第Ａ−３１９ ９６４
号はまたトルナフタート約０．１〜１．５％とジメチル
アミノエチルメタクリル酸エステルのメタクリレート共
重合物約１０〜２０％と中間の長さの分子鎖の脂肪酸エ
ステル約０．５〜１０％とを、殆んど水を含有しないア
ルコール溶剤中に含有する、外用の被膜形成型抗眞菌製
剤を記載している。トルナフタートは重合物マトリック
スから継続的に解放され、浸透は無水媒質中の脂肪酸エ
ステルにより有利にされている。
【０００８】更に、ドイツ特許公告第Ａ−３ ５４４
９８３号は、水に不溶の被膜形成剤と、ヒドロキシ−１
−ピロリドン−２系の抗眞菌薬剤例えばシクロピロック
スまたはオクトピロックスと専ら無水の溶剤とを含有す
る抗眞菌性マニキュアを記載している。
【０００９】国際特許出願第８７／０２５８０号は活性
成分を爪に供給できる薬学的賦課形薬を公開している。
これは親水性ポリマーとその親水性ポリマーのための、
薬学的に受入れられる溶剤とよりなる。実施例２では、
エタノールと水とに基く賦形薬中での、抗眞菌薬剤と親
水性ポリマーとの連合に言及している。その抗眞菌薬剤
は好ましくは３００以下かの分子量と少くとも０．０１
ｗｔ％の水への溶解度を持つべきである。
【００１０】ドイツ特許出願第Ａ−３ ７２０ １４７
号はまた水に不溶性の重合物被膜形成剤と、イミダゾー
ル抗眞菌性物質例えばトルナフタート、塩酸ナフチフィ
ンと、１００℃以下の沸点を持ち、マニキュア製造のた
めに化粧品分野で普通に用いられている有機溶剤から選
ばれた、前記被膜形成ポリマー用溶剤とを含有する、爪
糸状菌症処置用のワニスを公開している。その特許出願
の実施例８はアリルアミン族（塩酸ナフチフィン）の抗
眞菌薬剤と水４％と被膜形成ポリマーとしてポリビニル
ブチラートとニトロセルローズと酢酸エチルとを含有す
るワニスに関する。
【００１１】それで、技術の状態は抗眞菌薬剤が溶解で
きる水−アルコール混合物中の、低分子量抗眞菌薬剤に
基き、それに国際特許出願第４７／０２５８０号の場合
のような親水性ポリマーあるいはドイツ特許出願第Ａ−
３ ７２０ １４７号の場合のような水に不溶性ポリマ
ーを加えた組成物を包含する。しかし、これらの組成物
の中１つも眞に良好な結果を与えていない。ドイツ特許
出願第Ａ−３ ７２０１４７号の実施例８に記載のよう
な処方物は爪糸状菌症処置において達成を要望されてい
る結果を可能にしない数多くの不利さを有している。即
ち、第１に、使用されるポリマーが活性成分の解放を妨
げる。溶剤が蒸発すると、ポリマーの網状組織がナフチ
フィンを保持して角質層中への浸透を妨げる。第２回目
の適用の間第１回目の層の表面にポリマー＋活性成分の
組合せの蓄積が存在し、それが活性成分の不浸透を引き
起す。ニトロセルローズは強力な酸化剤であり、ナフチ
フィンはこの媒質中では化学的に不安定で、そのために
不活性にする。酢酸エチルは爪を脱水し、抗眞菌薬剤の
浸透を遅くすることがよく知られている。実際、それは
所望の効果を妨害する親油性溶剤である。
【００１２】困惑されるこの問題解決の探索において、
本出願会社は、活性成分の経皮的浸透を促進するために
使用する親水性浸透剤が爪の中および爪の下における抗
眞菌薬剤の濃度を著しく増大させる性質を持つことを発
見した。
【００１３】この結果は、多くの科学的研究が、爪は特
性上親水性であり、即ち親油性で、水の障壁として働く
皮フの角質層と反対に、爪はヒドロゲルとして働くこと
を示している故に完全に予期されない。それ故に、以下
の実施例１０で示すごとく角質層を通すのに広く用いら
れている浸透剤が抗眞菌薬剤の爪の中への浸透に有利で
あることは驚くべきことである。
【００１４】本発明の第１の目的はそれ故、爪糸状菌症
の処置のための皮フ病学的組成物中に、爪を通しての抗
眞菌薬剤の浸透の改良のために、活性成分の至皮的浸透
のために通常に用いられる型の親水性浸透剤を用いるこ
とにある。
【００１５】本発明はまた、溶液の形で提供され、一方
で少くとも１つの抗眞菌薬剤の有効量と、他方で爪のケ
ラチン化されている爪層中およびそれを通っての前記抗
眞菌薬剤の浸透に有利である少くとも１つの薬剤の有効
量と、最後に前記抗眞菌薬剤のための溶剤媒質とを含有
する爪糸状菌症処置のための皮フ病学的組成物で、前記
抗眞菌薬剤が少くとも部分的には水に可溶性であり、前
記溶剤媒質が直鎖または分枝鎖をもつＣ2−Ｃ8アルカノ
ールの中から選ばれる少くとも１つの共溶剤と混合した
水を含有し、前記浸透剤が親水性で、少くとも部分的に
前記溶剤媒質と混合できることを特徴とする爪糸状菌症
処置のための皮フ病学的組成物に関する。
【００１６】好ましい抗眞菌薬剤はアリルアミン族に属
するものである。塩酸テルビナフィンと塩酸ナフチフィ
ンとを挙げてもよい。それらのそれぞれの構造式は次の

【００１８】抗眞菌薬剤濃度は組成物の全重量に対して
約２〜３０ｗｔ％、好ましくは５〜２０ｗｔ％である。
【００１９】親水性浸透剤は有利には次の中から選んで
もよい。グリコール例えばプロピエングリコールとブチ
レングリコールとヘキシレングリコールとエチレングリ
コールとポリエチレングリコール。グリコールモノエー
テル例えば“ＤＯＷＡＮＯＬ ＰＭ，ＤＰＭ，ＴＰＭ，
ＰｎＢ，ＰＰＨ，ＤＰｎＢ，ＴＰｎＢ，ＰＭＡ”の名称
の下で市販されているエチレングリコールモノエーテ
ル。グリコールジエーテル例えば『ＰＲＯＧＬＹＤＥＳ
(TM)』の名称の下に市販されているもので、特にプロピ
レングリコールジメチルエーテルとジプロピレングルコ
ールジメチルエーテル。ジメチルスルホキシドとカプロ
ラクタムとジメチルイソソルビドとイソプロピリデング
リセロールとジメチルイミダゾリジノンとＮ−メチル−
ピロリドン−２とピロリドン−２と酪酸エチルとポリオ
キシエチレン化Ｃ8〜Ｃ10グリセリド特に『ＬＡＢＲＡ
ＳＯＬ(TM)』の名称の下に市販されているものとポロエ
チレングリコール２０グリセリルラウラートとジメチル
アセタミド。
【００２０】浸透剤の濃度は有利には、組成物の全重量
に対して約１〜６０ｗｔ％に亘ってもよい。
【００２１】溶剤媒質は有利には、直鎖または分枝鎖を
もつＣ2−Ｃ8アルカノールを包含する群から選ばれる少
くとも１つの共溶剤と混合した水よりなる。その共溶剤
には、好ましくはエタノールとイソプロパノールとｎ−
ブタノールとを挙げてもよい。
【００２２】水は全重量に対し５〜３０ｗｔ％の割合で
存在する。共溶剤の濃度については、組成物の全重量に
対し１０〜９０ｗｔ％に亘ることができる。
【００２３】本発明に従う組成物は好ましくは如何なる
界面活性剤を含まない。しかし、防腐剤例えばフエニル
エチルアルコールとベンジルアルコールとフエノキシエ
タノールと、酸化防止剤例えばブチルヒドロキシアニソ
ール（ＢＨＡ）とブチルヒドロキシトルエン（ＢＨＴ）
とパルミチルアスコルバートとα−トコフエロールと
（または）そのエステルと、着色剤、充てん剤または顔
料例えばマニキュア製造のため化粧品分野で広く用いら
れているマイカチタン（ｍｉｃａｔｉｔａｎ）と、浸透
する前に組成物が流れてしまうのを防止できるポリマー
例えば使用する溶剤媒質に全く溶解し、特にメチルセル
ローズとエチルセルローズとヒドロキシアルキルセルロ
ーズ例えば“ＫＬＵＣＥＬ”の名の下に市販されている
ものの中から選ばれるアルキルセルローズとよりなる群
から選ばれる少くとも１つの添加物を含有してもよい。
【００２４】本発明に従う組成物は有利にはローション
または流体ゲルの形で提供される。
【００２５】本発明のよりよい理解のため、その幾つか
の説明を記載する。それらは何らの制限的含意を持たな
い、純粋に説明的例として意図を持っている。
【００２６】
【実施例１】次の処方物を調製する。 塩酸テルビナフィン １０ｗｔ％ エタノール ３０ｗｔ％ 精製水 ３０ｗｔ％ ジエチレングリコールモノエチルエーテル ３０ｗｔ％
【００２７】塩酸テルビナフィンを３０℃で撹拌しなが
ら水−アルコール混合物に溶解し、それからジエチレン
グリコールモノエチルエーテルを完全に溶解するまで継
続的に添加する。そうして得られたローションを爪糸状
菌症処置の目的で、小さいブラシまたはポンプ動作のス
プレーの何れかを用いて罹患している爪に適用する。
【００２８】処置第２月からその爪の状態の改善が明白
になる。このことは健康な爪の成長の形をとり、罹患し
た爪を次第に除去して行く。
【００２９】次の実施例２〜８のそれぞれで、実施例１
に記載の手順を繰返し実施例１において適用したローシ
ョンを得る。実施例９においてはゲルが得られるが適用
方法は同じのままである。処置第２月で爪糸状菌症の有
意の改善が観察された。濃度は凡て、組成物全重量に対
する重量パーセントで支えられている。

【実施例１０】親水性浸透剤による、抗眞菌薬剤の爪中
への浸透現象の説明上記の浸透現象を、人の爪を非常に
よく再現する生体外模型を用いて説明する。
【００３９】この目的のため、２００〜４００μｍの均
一の厚さを持つ、牛のひづめからの薄い角質円板をＦｒ
ａｎｚセル（静止型）の表面におく。その角膜層を抗眞
菌性組成物で処理し、下面の下の“生き残った”液を用
いて与えられた時間Ｔ後の抗眞菌薬剤の濃度を測定す
る。
【００４０】調べるべきる抗眞菌性組成物を角質円板に
適用した場合、期間Ｔの終りに、どれだけ抗眞菌薬剤が
その表面に残っていたか、どれだけが角質ひづめ材料中
に浸透したか、およびどれだけ“生き残った”液中に存
在するかを測定することができる。これらの観測値から
浸透の評価を引き出せる。
【００４１】本発明に従い次に示す組成物のローション
を調製する（重量パーセント）。 塩酸テルビナフィン １０ 精製水 ３０ エタノール ３０ ジプロピレングリコールモノエチルエーテル ３０
【００４２】このローションを、角質材料１ｃｍ2が１
００ｍｇに相当すると認めて、角質物に５μｌ／ｃｍ2
適用する、処理後７２時間の終りにおいて、適用した塩
酸テルビナフィン量の０．１２２±０．０２％（ｎ＝１
１試験）が角質材料中に、即ち約６１０ｎｇあるいは角
質材料ｍｇ当り塩酸テルビナフィン約６．１０ｎｇ存在
した。
【００４３】Ｆｉｎｌａｙら（Ｂｒｉｔｉｓｈ Ｊｏｕ
ｒｎａｌ ｏｆ Ｄｅｒｍａｔｏｌｏｇｙ １９９０，
１２３，４８１−４８６）によれば、塩酸テルビナフィ
ン２５０ｍｇ／日，８週間の経口処置−有効であったと
云はれる処置−後の塩酸テルビナフィンの爪中濃度は爪
材料１ｍｇ当り塩酸テルビナフィン僅に０．４±１５ｍ
ｇであった。塩酸テルビナフィン１０％含有する、本発
明に従うローションを用いる、７２時間の１回の局所処
置が、２５０ｍｇ／日を用いる８週間の処置より約１０
〜２５倍多い活性成分を送達したことになる。

Claims (12)

    【特許請求の範囲】
  1. 【請求項１】 爪を通しての抗眞菌薬剤浸透を改善する
    ため、爪糸状菌症処置用皮フ病学的組成物への、活性成
    分の経皮的浸透に日常的に用いられる親水性浸透剤の使
    用。
  2. 【請求項２】 溶液の形で提供され、一方で少くとも１
    つの抗眞菌薬剤の有効量と、他方で爪のケラチン化され
    ている爪層中およびそれを通しての前記抗眞菌薬剤の浸
    透に有利である少くとの１つの薬剤の有効量と、最後に
    前記抗眞菌薬剤のための溶剤媒質とを含有する爪糸状菌
    症処置のための皮フ病学的組成物において、前記抗眞菌
    薬剤が少くとも部分的には水に可溶性であり、前記溶剤
    媒質が直鎖または分枝鎖をもつＣ2−Ｃ8アルカノールの
    中から選ばれる少くとも１つの共溶剤と混合した水を含
    有し、そして前記浸透剤が親水性で、少くとも部分的に
    前記溶剤媒質と混合できることを特徴とする爪糸状菌症
    処置のための皮フ病学的組成物。
  3. 【請求項３】 抗眞菌薬剤がアリルアミン族から選ばれ
    る請求項２に記載の組成物。
  4. 【請求項４】 アリルアミンが塩酸テルビナフィンと塩
    酸ナフチフィンを包含する群より採用される請求項３に
    記載の組成物。
  5. 【請求項５】 抗眞菌薬剤の濃度が組成物全量に対し２
    〜３０ｗｔ％である請求項２〜４の何れか１つに記載の
    組成物。
  6. 【請求項６】 浸透剤がグリコールとグリコールモノエ
    ーテルとグリコールジエーテルとジメチルスルホキシド
    とカプロラクタムとジメチルイソソルビドとイソプロピ
    リデングリセロールとジメチルイミダゾリジノンとＮ−
    メチルピロリドン−２とピロリドン−２と酪酸エチルと
    ポリオキシエチレン化Ｃ8−Ｃ10グリセリドとポリエチ
    レングリコール２０グリセリルラウラートとジメチルア
    セタミドとを包含する群から採用される請求項２〜５の
    何れか１つに記載の組成物。
  7. 【請求項７】 グリコールがプロピレングリコールとブ
    チレングリコールとヘキシレングリコールとエチレング
    リコールとポリエチレングリコールとを包含する群より
    採用され、グリコールモノメチルエーテルがエチレング
    リコールのモノエーテルとプロピレングリコールのモノ
    エーテルとを包含する群から採用され、そしてグリコー
    ルジエーテルがプロピレングリコールジメチルエーテル
    とジプロピレングリコールジメチルエーテルとを包含す
    る群から採用される請求項６に記載の組成物。
  8. 【請求項８】 浸透剤がジプロピレングリコールジメチ
    ルエーテルである請求項６に記載の組成物。
  9. 【請求項９】 浸透剤の濃度が組成物の全量に対し１〜
    ６０ｗｔ％である請求項２〜８の何れか１つに記載の組
    成物。
  10. 【請求項１０】 水と共溶剤とが組成物の全量に対しそ
    れぞれ５〜３０ｗｔ％と１０〜９０ｗｔ％の割合で存在
    する請求項２〜９の何れか１つに記載の組成物。
  11. 【請求項１１】 防腐剤と酸化防止剤と着色剤と充てん
    剤と顔料と浸透する前に組成物が流れることを防止する
    ためのポリマーとを包含する群から採用される少くとも
    １つの添加物を含有することを特徴とする請求項２〜１
    ０の何れか１つに記載の組成物。
  12. 【請求項１２】 ローションまたは流体ゲルの形で提供
    されることを特徴とする請求項２〜１１の何れか１つに
    記載の組成物。
