Description
本願は、「ガイドワイヤをナビゲートするシステムと方法」との名称の、2015年11月19日出願の米国特許出願第14/946,117号、及び2014年12月5日出願の米国仮出願第62/087,890号の利益を主張する。
令セットに関連づけられた経皮的装置の特定の動きを許容することができる。特定の処置を行うのに必要な動き特性は、治療される状態のタイプ(例えば慢性的完全閉塞(CTO)、部分閉塞等)、治療される病状の場所(例えば冠動脈、抹消動脈等)、治療される領域の幾何学的形状、使用される経皮的装置の特定タイプ(例えば製造元、モデル、サイズ等)のような因子に依存し得る。

Claims (17)
- カテーテル処置システムの作動方法であって、
コントローラが、
ガイドワイヤ前進/後退アクチュエータを使用してガイドワイヤを、正しい枝と側枝の接合点を通過して解剖学的構造の所望の経路を通るように前進させることと、
前記ガイドワイヤの遠位端が前記接合点を通過して前記所望の経路の正しい枝に存在するか否かを、関心領域の少なくとも一つの画像を処理することによって決定することと、
前記ガイドワイヤの遠位端が前記正しい枝に存在しない場合、前記ガイドワイヤ前進/後退アクチュエータを使用して前記ガイドワイヤを前記接合点の前の位置に後退させることと、
前記ガイドワイヤを後退させるとき、ガイドワイヤ回転アクチュエータを使用して前記ガイドワイヤの近位端を所定量で自動的に回転させることと、
前記ガイドワイヤ前進/後退アクチュエータを使用して前記ガイドワイヤを所望の位置まで前進させることと、
を行う、
方法。 - 前記ガイドワイヤを所定量だけ後退させる請求項1に記載の方法。
- 前記少なくとも一つの画像は蛍光画像である請求項1に記載の方法。
- 前記ガイドワイヤを前進させるステップと前記ガイドワイヤを後退及び回転させるステップとは、前記ガイドワイヤの遠位端が前記所望の経路に存在するようになるまで繰り返される請求項1に記載の方法。
- 前記ガイドワイヤを、前記ガイドワイヤの近位端を後退させる制御信号により回転及び後退させる請求項1に記載の方法。
- カテーテル処置システムの作動方法であって、
コントローラが、
正しい第1の枝および側枝における少なくとも1つの接合点を通過して解剖学的構造の所定経路を画定する一セットのパラメータを受信することと、
ガイドワイヤ前進/後退アクチュエータを使用してガイドワイヤを、前記所定経路を通るように前進させることと、
前記ガイドワイヤの遠位端が前記所定経路に存在するか否かを、前記接合点を通過して少なくとも関心領域の少なくとも一つの画像を処理することによって決定することと、
前記ガイドワイヤの遠位端が前記接合点を通過して前記所定経路に存在しない場合、前記ガイドワイヤ前進/後退アクチュエータを使用して前記ガイドワイヤを前記接合点の前の位置に後退させることと、
前記ガイドワイヤを後退させるとき、ガイドワイヤ回転アクチュエータを使用して前記ガイドワイヤの長手方向軸を中心に前記ガイドワイヤの近位端を回転させることであって、前記ガイドワイヤの近位端を所定量だけ回転させることと、
前記ガイドワイヤを前進させることと、前記ガイドワイヤの遠位端が前記所定経路内にあるかどうかを決定することと、前記ガイドワイヤを後退させることと、を前記ガイドワイヤの遠位端が前記所定経路に存在するようになるまで繰り返すことであって、前記ガイドワイヤの近位端を最初の後退時にのみ回転させることと、
を行う、
方法。 - 前記ガイドワイヤを後退させることは、前記ガイドワイヤの近位端を回転させる間に行われる請求項6に記載の方法。
- 前記ガイドワイヤを所定量だけ後退させる請求項5に記載の方法。
- 前記少なくとも一つの画像は蛍光画像である請求項5に記載の方法。
- カテーテル処置システムであって、
ガイドワイヤ、前記ガイドワイヤに結合されたガイドワイヤ前進/後退アクチュエータ、及び前記ガイドワイヤに結合されたガイドワイヤ回転アクチュエータを含むベッドサイドシステムと、
前記ベッドサイドシステムに結合されたワークステーションと
を含み、
前記ワークステーションは、
ユーザインタフェイスと、
少なくとも一つのディスプレイと、
前記ベッドサイドシステム、前記ユーザインタフェイス及び前記少なくとも一つのディスプレイに結合されたコントローラと
を含み、
前記コントローラは、
前記ガイドワイヤが側枝内を前進しているか否かを、関心領域の少なくとも一つの画像を処理することによって決定することと、
前記ガイドワイヤが前記側枝内を前進している場合、前記ガイドワイヤ前進/後退アクチュエータを使用して、前記ガイドワイヤを前記側枝の前の位置に後退させるとともに、前記ガイドワイヤ回転アクチュエータを使用して前記ガイドワイヤの長手方向軸まわりに前記ガイドワイヤの近位端を所定量だけ回転させることと、
前記ガイドワイヤが前記側枝の前の位置まで後退したとき、前記ガイドワイヤ前進/後退アクチュエータを使用して、前記ガイドワイヤを、前記側枝を避けて前進させることと
を行うようにプログラムされる、
カテーテル処置システム。 - 前記コントローラが、
関心領域の少なくとも1つの画像を処理することによって、前記ガイドワイヤの遠位端が所望の経路にあるかどうかを決定するようにプログラムされ、前記少なくとも1つの画像は透視画像である、請求項10に記載のカテーテル処置システム。 - 前記ガイドワイヤを前進させるステップと前記ガイドワイヤを後退させるステップと前記ガイドワイヤの近位端を回転させるステップとは、前記ガイドワイヤの遠位端が前記所望の経路に存在するようになるまで繰り返される請求項11に記載のカテーテル処置システム。
- 前記ガイドワイヤを前記接合点の前に複数回後退させるステップであって、前記ガイドワイヤの近位端を、前記ガイドワイヤの最初の後退時にのみ回転させるステップを含む請求項1に記載のカテーテル処置システムの作動方法。
- 前記ガイドワイヤの近位端を所定量だけ回転させる請求項6に記載のカテーテル処置システムの作動方法。
- 前記コントローラが、
前記ガイドワイヤを複数回後退させる場合、前記ガイドワイヤの最初の後退時にのみ、前記ガイドワイヤの近位端を自動的に回転させるようにプログラムされる請求項10に記載のカテーテル処置システム。 - カテーテル処置システムであって、
ガイドワイヤ、前記ガイドワイヤに結合されたガイドワイヤ前進/後退アクチュエータ、及び前記ガイドワイヤに結合されたガイドワイヤ回転アクチュエータを含むベッドサイドシステムと、
前記ベッドサイドシステムに結合されたワークステーションと
を含み、
前記ワークステーションは、
ユーザインタフェイスと、
少なくとも一つのディスプレイと、
前記ベッドサイドシステム、前記ユーザインタフェイス及び前記少なくとも一つのディスプレイに結合されたコントローラと
を含み、
前記コントローラは、
前記ガイドワイヤ前進/後退アクチュエータを使用して前記ガイドワイヤを後退させるとき、前記ガイドワイヤ回転アクチュエータを使用して、前記ガイドワイヤの近位端が前記ガイドワイヤの長手方向軸の周りに所定量だけ回転させるようにプログラムされ、
さらに、前記コントローラは、
正しい第1の枝および側枝の接合点を通過する関心領域の少なくとも1つの画像を処理することによって、前記ガイドワイヤの遠位端が所定経路に存在するか否かを決定することと、

前記ガイドワイヤの遠位端が前記接合点の前の位置まで後退したとき、前記ガイドワイヤ前進/後退アクチュエータを使用して前記ガイドワイヤを前進させることと、
前記ガイドワイヤの前進及び前記ガイドワイヤの後退を、前記ガイドワイヤの遠位端が、前記接合点を通過して前記所定経路に存在するようになるまで繰り返すことと、
を行うようにプログラムされる、
カテーテル処置システム。 - 前記コントローラが、
前記ガイドワイヤを複数回後退させる場合、前記ガイドワイヤの最初の後退時にのみ、前記ガイドワイヤの近位端を自動的に回転させるようにプログラムされる請求項16に記載のカテーテル処置システム。
