Description
本出願は、全内容が本明細書に全体として組み込まれる2013年3月13日出願の米国仮特許出願第61/779974号明細書に対する優先権を主張するものである。
配列表
本出願は、ASCIIフォーマットで電子的に提出されている、これにより全体として参照により本明細書に組み込まれる配列表を含有する。2014年3月12日に作成された上記のASCIIのコピーは、D2046−7050WO_SL.txtと名付けられており、サイズは28,329バイトである。
本発明は、例えばIL−1阻害剤のための治療用組成物及び製剤に関する。
実施形態1. 8〜12mMの濃度のクエン酸ナトリウム若しくはリン酸ナトリウム、4%〜6%(w/v)のソルビトール、0.08%〜0.12%(w/v)の濃度のポロキサマー188を含み、及びカルボキシメチルセルロースナトリウムを含んでいてもよい水性製剤であって、5.5〜7.5のpHを有し、眼障害を治療するために有効である水性製剤。
局所眼科薬は、一般に患者により自己投与される。患者は薬物を相当に長期間にわたり保管している場合があるので、製剤は、典型的には投与前には医師若しくは薬剤師によってのみ保管される製剤より高温及び高レベルの撹拌ストレスに曝される可能性がある。当分野において公知であるように、タンパク質は低分子よりも撹拌及び温度に対してより感受性である。撹拌ストレスは沈降を引き起こすことがあり、熱ストレスは沈降及び化学的劣化をもたらすことがある。さらに、送達デバイス内への化合物の装填中、熱ストレスに曝露させられる可能性がある。本出願人らは、撹拌ストレス及び熱に曝露させられた場合に素晴らしい安定性を上首尾で提供する製剤を実現した。
生物製剤を投与する際の又別の問題は、十分な濃度の生物製剤を提供することである。これは、最少投与回数で治療効果を達成できるように相当に高濃度の生物製剤を提供することが望ましい眼科用途では特別な課題である。本出願人らは、例えば上述した、及び本明細書内のどこかに記載した条件などの条件下で保管した場合に目に見えて凝集しない、沈殿しない、又は化学的純度を消失しない高濃度又は治療的に有効な濃度のポリペプチドを含有する有効用量のキメラサイトカイン製剤を送達できる製剤を実現できている。さらに、本出願人らは、等張化剤、界面活性剤及び緩衝剤を含む製剤中に80mg/mLまでの、例えば50mg/mLのタンパク質濃度にあるサイトカイン製剤の安定性を証明した。このため、1つの態様では、本発明において特徴付けた製剤は、0.1mg/mL〜100mg/mL、0.1〜80mg/mL、0.1〜50mg/mL、0.1mg/mL〜20mg/mL、0.1mg/mL〜5mg/mL、0.1mg/mL〜1mg/mL、1mg/mL〜100mg/mL、5mg/mL〜100mg/mL、5mg/mL〜30mg/mL、10mg/mL〜100mg/mL、10mg/mL〜30mg/mL、20mg/mL〜100mg/mL、30mg/mL〜100mg/mL、40mg/mL〜100mg/mL、50mg/mL〜100mg/mL、60mg/mL〜100mg/mL、1mg/mL〜80mg/mL、5mg/mL〜80mg/mL、10mg/mL〜80mg/mL、20mg/mL〜80mg/mL、40mg/mL〜80mg/mL、50mg/mL〜80mg/mL、60mg/mL〜80mg/mL、1mg/mL〜60mg/mL、5mg/mL〜60mg/mL、10mg/mL〜60mg/mL、20mg/mL〜60mg/mL、30mg/mL〜60mg/mL、40mg/mL〜60mg/mL若しくは50mg/L〜60mg/mLの濃度で安定性で存在するキメラサイトカインポリペプチドを含有する。例えば、製剤は、0.1mg/mL、1mg/mL、2mg/mL、5mg/mL〜20mg/mL、例えば5mg/mL若しくは20mg/mLを含有する。
又別の態様では、本明細書で特徴付けた製剤は、1つ以上の緩衝剤を含有する。適切な緩衝剤の例には、リン酸塩、クエン酸塩、酢酸塩、ホウ酸塩、コハク酸塩及びTRISが含まれるがそれらに限定されない。一部の場合には、緩衝剤の塩は、ナトリウム塩若しくはカリウム塩である。本発明において特徴付けた所定の実施形態では、緩衝剤は、弱緩衝作用を提供するために約10mM〜約50mM、例えば約20mMから約40mMの量で存在する。これは、チクチクする痛み若しくは不快感が生じる場合、製剤が投与部位、例えば眼の表面上で急速に中和されることを可能にする。一部の実施形態では、緩衝剤は、約10mM、約15mM、約20mM、約25mM、約30mM、約35mM、約40mM、約45mM若しくは約50mMの量で存在する。一部の製剤では、緩衝剤はクエン酸塩、例えばクエン酸ナトリウムである。他の製剤では、緩衝剤は、10mMで存在するクエン酸塩である。一般に、緩衝剤は弱緩衝剤である。
又別の態様では、本発明において特徴付けた製剤は、1つ以上のアミノ酸を含有する。適切なアミノ酸には、アルギニン、グルタミン酸、ヒスチジン若しくはメチオニンが含まれるがそれらに限定されない。アミノ酸は、典型的にはタンパク質の安定性及び/又は溶解度を強化するために選択される。そのようなアミノ酸を同定する方法は、当分野において公知である。一部の実施形態では、例えばP05製剤などの製剤は、ヒスチジン若しくはメチオニンを含有する。
又別の態様では、本発明において特徴付けた製剤は、1つ以上の増粘剤を含有する。適切な増粘剤には、カルボキシメチルセルロースナトリウム(本明細書ではカルボキシメチルセルロース若しくはCMCとも呼ぶ)を含むメチルセルロース;エチルセルロースを含むヒドロキシセルロース;ヒドロキシプロピルメチルセルロース(ヒプロメロース);カルボマー、例えば934P、971P及び974P;ポリビニルアルコール;キサンタンガム;グアールガム;ゲランガム;及びグリセリンが含まれるがそれらに限定されない。
本明細書に記載した製剤の特徴は、それらが保存料を含有しないことである。一般に、保存料は、キメラサイトカインポリペプチドに影響を及ぼす可能性がある、例えばポリペプチドの構造に変化を誘発する可能性がある。さらに、保存料は、被験者において、所望の治療作用にとって正反対である例えば炎症応答を誘発する可能性がある。製剤は、無菌条件下で無菌で保管され、それらの最終容器内に充填される。
本出願人らは、さらに予想外にも、本明細書に記載した治療用タンパク質を含有していない製剤(例えば、ビヒクル単独製剤)が眼疾患、例えばドライアイ疾患の内の1つ以上の徴候若しくは症状、例えば本明細書に記載したドライアイ疾患の内の1つ以上の徴候若しくは症状を治療するために有用であることも発見した。一部の実施形態では、ビヒクル製剤は、界面活性剤、等張化剤及び緩衝剤を含む。一部のそのような実施形態では、製剤は、疼痛(例えば、疼痛質問スコア若しくは視覚的アナログスケールを使用して評価される疼痛)、OSDI又はOSDIのサブスケールを減少させるために効果的である。一部のそのような実施形態では、製剤は、角膜フルオレセイン染色(CFS)を減少させる。本明細書で使用する用語「ビヒクル単独製剤」は、特別には、タンパク質若しくはペプチド成分を実質的に含んでいない、例えば治療用タンパク質を含有していない製剤を意味する。本明細書に記載したビヒクル製剤は、何らかの治療用タンパク質、例えば治療用ポリペプチドを含有できることを理解すべきである。
一部の実施形態では、本明細書で特徴付けた製剤、例えばキメラIL−1阻害剤などの治療用タンパク質を含有する製剤若しくはビヒクル製剤は、患者、例えばヒト若しくは他の哺乳動物、例えばイヌ、ネコ、若しくはウマなどに局所投与される、及び例えば眼に投与される。一般に、本明細書に記載した製剤は、被験者に、任意の適切な方法、例えばボーラスとしての静脈内投与若しくは長期間にわたる持続注入によって、筋肉内、筋肉内、動脈内、髄腔内、嚢内、眼窩内、心臓内、皮内、腹腔内、滑液包内、経気管、皮下、表皮下、関節腔内、皮膜下、クモ膜下、脊髄内、硬膜外注射、胸骨内注射及び注入によって投与することができる。その他の適切な投与様式には、局所(例えば、皮膚若しくは粘膜)又は吸入(例えば、鼻腔内若しくは肺内)経路が含まれる。所定の用途のためには、投与経路は、静脈内注射若しくは注入、皮下注射又は筋肉内注射の内の1つである。眼に投与するために、一部の実施形態では、本明細書で特徴付けた製剤(例えば、本明細書に記載したキメラサイトカイン製剤)のための投与様式は、例えば滴剤の形態での眼への局所投与である。製剤を含有することのできる、及び/又は製剤を投与するために使用できるデバイスの例には、単純なアイドロッパー、計量機能を備える、若しくは備えないスクイーズボトル及び例えばCatalent社(ニュージャージー州サマセット)によって製造されるデバイスのようなブローフィルシール(BFS)デバイス、例えばチップ・シールテクノロジー、銀/微量作用テクノロジー、無菌フィルター、圧潰型一次容器などを使用する複数回使用容器が含まれる。
本明細書で特徴付けた一部の製剤は、治療用タンパク質を含む。実施形態では、製剤は、例えばIL−1Rに結合することができ、及びIL−1シグナル伝達に拮抗できる、このため、(i)少なくとも一部にはIL−1作動性によって誘発される、(ii)IL−1シグナル伝達成分(例えばIL−1α、IL−1β若しくはIL−1RI)の上昇したレベル若しくは活性又は上昇したIL−1シグナル伝達と関連する、及び/又は(iii)IL−1活性を低下させることによって緩和される何らかの疾患若しくは医学的状態を含む、「IL−1関連障害」を治療するために使用できるようなキメラ受容体結合剤(例えば、キメラサイトカイン)を含む。IL−1関連障害には、自己免疫障害及び炎症性障害を含む、急性及び慢性障害が含まれる。IL−1関連障害には、全身性及び非全身性障害が含まれる。IL−1α及びIL−1βは感染性応答並びに例えば関節リウマチを含む炎症性疾患に関係する強力な前炎症性サイトカインであることは明確に確定されている。増加したIL−1産生は、所定の自己免疫障害、虚血症及び様々な癌を有する患者において観察されており、このため、IL−1をこれらの疾患や関連疾患に関係付けている(例えば、Sims and Smith,Nature Rev Immunol,10:89−102(2010)を参照されたい)。
本発明において特徴付けた眼科用製剤は、局所投与のために、例えば点眼液、軟膏剤若しくはゲル剤として投与するため、又は例えば眼の前房若しくは結膜嚢内に埋め込むために送達することができる。滴剤、例えば点眼液は、アイドロッパーを使用して送達できる。ゲル剤及び軟膏剤も又ドロッパーを使用して投与できる。眼送達のために調製された場合、活性物質(例えば、キメラサイトカインタンパク質若しくは受容体結合剤)は、0.0001%〜0.1%、0.001%〜5%、例えば、0.005%〜0.5%、0.05%〜0.5%、0.01%〜5%、0.1%〜2%若しくは1%〜5%の濃度で存在してよい。一部の実施形態では、濃度は、例えばP05の2%である。他の実施形態では、濃度は、例えばP05の0.5%である。
ドライアイ及び他の表面疾患に関して、被験者は、当分野において公知のアプローチ、例えば前眼部障害指数(OSDI)、角膜及び結膜染色並びにシルマー試験の内の1つ以上を使用して評価できる。OSDIを使用する場合、ベースラインからの負の変化は視覚関連機能及び眼炎症性障害の改善を示す。
全ての技術的特徴は、そのような特徴のあらゆる可能な組み合わせで個別に組み合わせることができる。
[実施例1]
治療用タンパク質、例えばキメラタンパク質の実施例
表1(下記)に列挙したアミノ酸配列を備えるタンパク質をコードする核酸は、T7プロモーター及びアンピシリン(pET31シリーズ)若しくはカナマイシン耐性遺伝子(pET28シリーズ)(EMD Chemicals社、米国ニュージャージー州ギブスタウン)を含有するpETベクター内で構築し、発現させた。発現のために使用できるコーディング配列の実施例は表2に提供した。
キメラタンパク質の発現及び精製
ヘキサ−ヒスチジンタグ(配列番号23)を含有するタンパク質は、37℃のLBブロス培地中での3時間にわたる1mMのイソプロピルβ−D−1−チオガラクトピラノシド(IPTG)を用いた誘導により大腸菌細胞BL21(DES)菌株中で発現させた。細胞は、20〜50mMのTris、0.5MのNaCl、2.5mMのEDTA、0.1%のTriton X−100、pH8.0中に溶解させた。溶解物は、0.1%のポリソルベート80を含有する1.25×PBSに対して透析し、次に0.8/0.2μmフィルターに通して滅菌ろ過し、その後にHisTrap HP(登録商標)プレパックカラム(GE Healthcare社、米国ニュージャージー州ピスカタウェイ)を使用して固定化イオンアフィニティークロマトグラフィー(IMAC)にかけた。カラムを50mMのリン酸塩中で平衡化させ、500mMのNaCl、pH7.1を装填し、同一バッファーを用いて洗浄した。カラムは、25mMのイミダゾールを用いて前溶出し、同一バッファー中の125mMのイミダゾールを用いて溶出させた。溶出タンパク質を1.25×PBS、0.1%のポリソルベート80、pH7.4に対して広範に透析した。
セルベースアッセイ
タンパク質若しくはタンパク質を含有する上清をIL−1活性についてセルベースアッセイで評価した。HEK−Blue(商標)IL−1β応答性細胞を使用してIL−1β活性(InvivoGen社、米国カリフォルニア州サンディエゴから入手できる)を監視した。これらの細胞は、5つのNF−kB及び5つのAP−1結合部位に融合したIFN−β最小プロモーターの制御下で分泌胚アルカリホスファターゼ(SEAP)レポーター遺伝子を含んでいる。細胞表面上のIL−1受容体のIL−1βの関与は、NF−kBの活性化及びSEAP産生をもたらした。SEAPレポートは、例えばQUANTI−Blue(商標)(InvivoGen社、米国カリフォルニア州サンディエゴ)及び分光測光分析を使用して検出できる。HEK−Blue IL−1β細胞懸濁液は、70〜80%のコンフルエンスまで培養した細胞から調製した。再懸濁させた細胞は、新鮮増殖培地(DMEM、4.5g/Lのグルコース、2mMのL−グルタミン、10%(v/v)熱不活化ウシ胎児血清(56℃で30分間)、50U/mLのペニシリン、50mg/mLのストレプトマイシン、100mg/mLのNormocin(登録商標)T)中で約330,000細胞/mLへ調整した。
キメラタンパク質の結合特性
可溶性組み換えヒトIL−1RI(IL−1RIの細胞外ドメインに対応する)に対するタンパク質の結合特性は、Reichert SR7000DCデュアルチャネルSPRシステムを使用する表面プラズモン共鳴法を用いて評価した。結合は、0.005%のTween 20を含むリン酸緩衝食塩液中で評価した。IL−1βは、8〜9nMのKD及び2〜3×10−3s−1の解離定数(Kd)、又別の実験では、約2nMのKD、1.3〜1.5×106M−1s−1の結合定数及び約2.9〜3.0×10−3s−1の解離定数(Kd)を有すると観察された。P01タンパク質はIL−1βと類似の結合キネティクスで結合したが、結合実験の解離期(約180秒間)中には解離しなかった。そこで、P01タンパク質は類似の条件下でIL−1βより大きなアフィニティーでIL−1RIに結合した。
製剤の実施例
本発明による代表的な製剤を以下に記載する:
25g/Lの濃度で存在するP05タンパク質を有する製剤;カルボキシメチルセルロースは0.25%(w/v)の濃度で存在する;ポロキサマー188は0.1%(w/v)の濃度で存在する;ソルビトールは5%(w/v)の濃度で存在する;リン酸ナトリウムは10mMの濃度で存在する;アルギニン及び/又はグルタミン酸は100mMの濃度で存在する。製剤は、約6.5のpHを有する。製剤は、逆相HPLC(RP−HPLC);弱カチオン交換HPLC(WCEX−HPLC);分光測光法(A280);及び視覚的アッセイの内の1つ以上を使用して測定されるように、室温で2週間後に安定性について、及び2〜8℃で少なくとも12カ月間までの保管安定性について試験する。
製剤の実施例及び安定性試験
第1相臨床試験において利用されたP05の製剤(EBI−005としても公知である)は、0.25%(w/v)の濃度のカルボキシメチルセルロースナトリウム;0.1%(w/v)の濃度のポロキサマー188;5%(w/v)の濃度のソルビトール;10mMの濃度のリン酸ナトリウム及び5若しくは20mg/mLのいずれかの濃度にあるP05を含有する水性製剤であった。製剤は、約6.5のpHを有する。これらの製剤を下記の外観、pH、オスモル濃度、分光測光法(A280)、非還元SDS−PAGE、還元SDS−PAGEによる含量;サイズ排除HPLC(SE−HPLC);逆相HPLC(RP−HPLC);WCEX−HPLC;効力;及び容器完全性(CIT)の測定を用いて安定性について試験した。これらの試験は、(1)リリース時(0カ月後);(2)5±3℃での1カ月間、2カ月間、3カ月間、4カ月間、5カ月間及び6カ月間の保管後;(3)25℃及び60%の相対湿度(室温実験)での1カ月間若しくは3カ月間の保管後に実施した。これらの尺度についての仕様及び5mg/mL及び20mg/mLの製剤の代表的なバッチからの結果は、下記の表14A〜Eに示した。これらの結果は、製剤が素晴らしい安定性を有したことを証明している。製剤は2〜8℃での6カ月間にわたる保管後及び室温での3カ月間にわたる保管後でさえ仕様を満たし続けた。
融解プロファイル
タンパク質P03、P04、P05、mIL−1Ra(メチオニルIL−1Ra)及びIL−1βはリン酸緩衝食塩液(PBS)、pH7.4中において0.5mg/mLで調製した。タンパク質をSYPRO(登録商標)オレンジ色染料(Invitrogen社、カリフォルニア州)とストック濃度の希釈率1:500で結合し、示差走査蛍光分析法にかけた。例えば、He et al.(2010)J Pharm Sciences,99 1707−1720を参照されたい。蛍光測定値は、温度が1℃/分の速度で25℃から95℃へ上昇するにしたがってAgilent Mx3005 QPCR装置を使用して監視した。融解温度(Tm)値は、蛍光遷移の一次導関数の最大値から引き出した。タンパク質P03、P04及びP05は50℃超〜59℃の高さまでの変性開始温度並びに59、60、62及び64℃超のTmを有することが観察された。結果は下記の表4並びに図5A及び図5Bに示した。
動物モデルにおけるドライアイの治療
精製P05(ヘキサ−ヒスチジンタグ(配列番号23)が欠如する)を1.25×PBS中で調製し、ドライアイ疾患のマウスモデルにおいて試験した。このモデルにおいてJackson Laboratories製の6〜10週齢の雌性C57BL/6マウス(≧30%の相対湿度、ヒドロゲル飼料の補給及びEnviro−dri(商標)環境エンリッチメントを備える動物飼育室内で1〜2週間馴化させた)を第0日にフルオレセイン染色についてプレスクリーニングした。フルオレセイン染色のために、10mg/mLでWFI H2O中で希釈した、新しく作成したフルオレセインを各眼に0.4μLで投与した。投与のおよそ8〜13分後、Olympus蛍光解剖顕微鏡を使用して眼をスコア付けした。点状染色は、角膜表面が分割されている5つの領域各々について0〜3の標準化NEI(国立眼科学研究所)スコアリングシステムを使用して記録した(スコア範囲0〜15/眼)。ティーチングブリッジを使用して、2つの遮蔽スコアは、各眼について単一集団スコアを生じさせるためにマウスを同時に評価した。
撹拌試験
使用に適合する界面活性剤を同定するために、1mg/mL及び50mg/mLでのP05は、様々な界面活性剤を含有する(i)PBS、0.5%(w/v)のCMC、pH7.4又は(ii)10mMのクエン酸ナトリウム、pH6.0のいずれかの溶液中で調製した。撹拌は、タンパク質を4時間にわたり室温でボルテックスミキサーにかけることによって実施した。サンプルは、マイクロ流体イメージング(MGI)、SEC、A280及び視覚的検査によって分析した。0.1%(w/v)のポロキサマー188の使用は、他の界面活性剤(例えば、ポリソルベート20、ポリソルベート80を含む、又は界面活性剤なし)と比較して、タンパク質を沈降(視覚的検査を使用して評価した)及び撹拌中の有意な肉眼では見えない粒子蓄積から保護した。例えば、0.1%(w/v)のポロキサマーは、≧10μの粒子及び≧25μの粒子について粒子数を減少させた。
製剤の調製
P05の製剤を調製した。手短には、P05は、1×PBS、pH6.5中に52.8mg/mLのP05を含有する冷凍液として提供された。ポリペプチドは、10mMのクエン酸ナトリウム及び5%(w/v)のソルビトールをpH6.0で含有するバッファーに対して、2〜8℃で約24時間をかけておよそ10,000倍の交換で3,500分子量カットオフ透析カセットを使用して透析した。透析後、濃度はA280/A320を測定することによって決定した。
リン酸バッファー対クエン酸バッファー内でのP05の安定性
動的光散乱法若しくはDLS(準弾性光散乱法若しくはQELSとしても公知)は、サンプル上にレーザー光を集束させ、高速光子計数器によって測定される散乱光のゆらぎ速度を監視することによってウエルプレート内での分析物(例えば、P05)の拡散を測定する。相関関数として公知の数学的技術を使用して、拡散係数を決定するためにゆらぎ速度が定量される。拡散係数を使用して、ストークス−アインシュタイン方程式によって水和半径(Rh)が入手される。
安定性試験
実施例11において記載した製剤の安定性を試験するために、様々な濃度のP05を備える製剤を(上記の実施例11に記載したように)調製し、ベースライン測定値について分析した。調製したポリペプチドのバイアルは、25℃で0日間、3日間、1週間、2週間及び4週間並びに40℃で3日間、1週間及び2週間インキュベートした。少なくとも二本のバイアルは時点毎に調製した。インキュベーション後、サンプルは、サイズ排除HPLC(SE−HPLC)、弱カチオン交換HPLC(WCX−HPLC)、逆相HPLC(RP−HPLC)、濃度(A280−A320)、視覚的外観、写真を用いて実施する正式な検査を用いて分析した。pHは、T=0及びT=4週間に分析した。オスモル濃度は、T=0でのみ分析した。視覚的検査及び濃度は、どちらも25℃及び40℃で2週間後及び4週間後に評価した。全製剤は、2週間後に透明及び無色であり、目に見える粒子はなかった。25℃のサンプルは、4週間後に透明及び無色であった。これらの濃度試験の結果は、表5に例示した。
25℃サンプルのpHは、4週間後に決定した。全ての場合に、T=0でのpHは6.00〜6.01であり、4週間後のpHは6.03〜6.07であった。これらのデータは、25℃で少なくとも4週間及び40℃で少なくとも2週間にわたり製剤中のある範囲の濃度にわたる製剤の安定性を証明している。
純度は、吸光度及び蛍光検出を使用するサイズ排除HPLC法を使用して評価した。手短には、Sepax Zenix(登録商標)SEC−150の7.8mm×20cm(PN213150−7820)カラムを使用した。移動相は1×PBSであった。評価は、Agilent 1100 HPLCシステムを、1mL/分の流量、18分間の総ラン時間、周囲温度を含むイソクラチックモードで使用して実施した。吸光度検出は、280nmの励起波長での蛍光検出及び350nmの発光検出波長で280nmであった。1及び5mg/mLのサンプルの蛍光検出を使用する実験のためには10μgのポリペプチド製剤を装填し、20mg/mLのサンプルの吸光度検出のためには、50μgのポリペプチド製剤を装填した。
追加の試験において、弱カチオン交換HPLC法を使用して製剤を評価した。この方法では、Dionex ProPac(登録商標)WCX−10、4×250mm(PN054993)カラムを使用した。移動相Aは10mMの酢酸ナトリウム、pH5.5であり、移動相Bは移動相A+0.25MのNaClであった。アッセイは、1.2mL/分の流量、35分間の総ラン時間、周囲温度を用いてAgilent 1100 HPLCシステムを使用して実施した。検出は、214nm及び280nmでアッセイすることによって実施した。サンプルサイズは、25μg/注入であった。25℃での2週間の保管後の結果の要約は表6に、40℃での2週間の保管後の結果は表7に、及び25℃での4週間の保管後の結果は表8に示した。
製剤は、さらにRP−HPLCを使用して評価した。RP−HPLCアッセイは、疎水性に基づいて生成物不均質性を監視することによって純度を評価する又別の方法である。この方法は天然分子を酸化メチオニンを含有する生成物関連不純物から分離することができる。プレピーク2及び3はP05の酸化形であり、ポストピーク1及び2は分子のアセチル化形である。
ブローフィルシール容器内での製剤の安定性
ブローフィルシール(BFS)容器内へ製剤を包装する工程は、プラスチック押出、成型、無菌充填及び密封を順序正しく含んでいる。例えば、Liu,W.et al.2011 Biopharm International,24(7):22−29を参照されたい。押出工程では、プラスチックグラニュールを160℃を超える温度で融解させる。引き続いて、プラスチックを所望の容器形に成型し、製剤溶液を充填し、気密密封する。
ブローフィル加工処理後のP05の初期解析は、タンパク質がブローフィル加工処理にもかかわらずその化学的及び物理的安定性を保持することを証明した。BFS工程に曝露させられていないP05製剤(50mg/mLの濃度のP05、0.01%(w/v)のポロキサマー188、5%(w/v)のソルビトール、10mMのリン酸ナトリウム、pH6.5を含有する水性製剤)からの結果をブローフィル工程を受けさせたP05製剤(5mg/mLの濃度のP05、0.1%(w/v)のポロキサマー188、5%(w/v)のソルビトール、10mMのクエン酸ナトリウム、pH6.0を含有する水性製剤)と比較した。これらの製剤は、同一製造バッチからのP05を用いて作成した。
BFS容器内のP05製剤は、相対湿度60%の25℃又は2〜8℃に設定したインキュベーター内に保管した。1カ月間隔で、サンプルをSEC、RP−HPLC及びwCEX−HPLCによって分析した。4カ月後及び5カ月後に、濃度をA280により測定した。5カ月後に、A280によるオスモル濃度(蒸発を評価するため)、pH及び濃度も又測定した。
メチオニン含有製剤
一部の実施形態では、本発明は、メチオニンを含有する本明細書に記載した製剤に関する
10mMのクエン酸ナトリウム、5%(w/v)のソルビトール、0.1%(w/v)のポロキサマー188、pH6.0中で調製したP05のための酸化防止剤の使用について2種の異なるストレス条件:温度(40℃での保管)及び過酸化水素を使用した強制酸化を使用して試験した。過酸化水素は、フリーラジカルによる酸化を誘発するストレス因子であり、そこでγ線照射されていた多用量容器内での保管後に発生する可能性がある酸化作用を模倣するために使用した(下記参照)。P05は、高温で長期間にわたり保管した場合に酸化レベルを増加させる。製剤に加えられた10mMのメチオニン若しくは7mMの重硫酸塩いずれかの使用を40℃で3週間保管されたP05製剤について試験した。さらに、同一製剤は、10%(v/v)の0.02%過酸化水素をサンプルに加えた場合に、強制酸化を使用して試験した。サンプルは、酸化レベルを評価するためにRP−HPLCによって試験した。表22は、データの要約を提供する。
EBI−005製剤及びビヒクル製剤の治療作用
中等度から重度のドライアイ疾患を有する患者におけるEBI−005の水性製剤の安全性及び生物学的活性を評価するために、多施設共同二重遮蔽無作為化プラセボ対照臨床試験が完了された。本試験で使用した製剤は20mg/mL若しくは5mg/mL(下記参照)のいずれかの濃度のEBI−005(本明細書ではP05とも呼ぶ)、0.25%(w/v)の濃度のカルボキシメチルセルロースナトリウム、0.1%(w/v)の濃度のポロキサマー188、5%(w/v)の濃度のソルビトール及び10mMの濃度のリン酸ナトリウムを含有していた。本臨床試験は、米合衆国内の8施設で患者74例を対象に実施された。本試験は自然環境(管理有害環境チャンバーは使用されなかった)で実施された。
中等度から重度のドライアイ疾患(DED)を有する被験者におけるEBI−005(5mg/mL)局所点眼液及びビヒクルの二重遮蔽無作為化対照臨床試験
各眼に12週間にわたり1日3回局所点眼液として与えられるビヒクル製剤(10mMのクエン酸ナトリウム、pH6.0、5%(w/v)のソルビトール及び0.1%(w/v)のポロキサマー188)並びに治療用製剤(5mg/mLのP05を含有する10mMのクエン酸ナトリウム、pH6.0、5%(w/v)のソルビトール及び0.1%(w/v)のポロキサマー188)の有効性を決定する試験を実施する。

に安定であり、前記タンパク質の凝集形に比較したモノマー形のパーセンテージがSEC−HPLCを使用して評価される、請求項6〜14のいずれか一項に記載の水性製剤。 - 2〜8℃及び60%の相対湿度で少なくとも1カ月間にわたり保管された後に安定である、請求項6〜15のいずれか一項に記載の水性製剤。
- 周囲条件下、例えば室温、例えば25℃で少なくとも1カ月間保管された後に安定である、請求項6〜15のいずれか一項に記載の水性製剤。
- 例えば顕微鏡粒子計数試験を使用して評価して、≧10μmの粒子に対して50個/mL以下の粒子、≧25μmの粒子に対して5個/mL以下の粒子及び≧50μmの粒子に
対して2個/mL以下の粒子の存在によって特定されるように安定である、請求項6〜17のいずれか一項に記載の水性製剤。 - SEC−HPLCを使用して評価した凝集形に比較して>90%のモノマー形の前記タンパク質の存在によって特定されるように安定である、請求項6〜18のいずれか一項に
記載の水性製剤。 - 還元SDS−PAGE又は非還元SDS−PAGEにおける参照標準物質への主バンドの適合度によって特定されるように安定である、請求項6〜19のいずれか一項に記載の水性製剤。
- 前記水性製剤を弱カチオン交換HPLC(WCEX−HPLC)を使用して評価した場合に、85%以上の主ピークによって特定されるように安定である、請求項6〜20のいずれか一項に記載の水性製剤。
- ブローフィルシール容器内に包装される、請求項6〜21のいずれか一項に記載の水性製剤。
- IL−1関連障害の治療に使用するための、請求項1〜22のいずれか一項に記載の水性製剤。
- 前記IL−1関連障害は癌である、請求項23に記載の水性製剤。
- 8mM〜12mMの濃度のクエン酸ナトリウム若しくはリン酸ナトリウムの一方または両方;
4%〜6%(w/v)のソルビトール;
0.08%〜0.12%(w/v)の濃度のポロキサマー188;及び
0.1〜1%の濃度のカルボキシメチルセルロースナトリウムを含む水性製剤であって、
5.5〜7.5のpHを有し、
治療用タンパク質を実質的に含まない水性製剤。 - 1〜25mg/mLのP05;
8mM〜12mMの濃度のクエン酸ナトリウム若しくはリン酸ナトリウム;
4%〜6%(w/v)のソルビトール;
0.08%〜0.12%(w/v)の濃度のポロキサマー188;及び
1〜20mMの濃度のメチオニンを含む水性製剤であって、
カルボキシメチルセルロースナトリウムを含んでいてもよく、
5.5〜7.5のpHを有する水性製剤。 - 1mg/mL〜25mg/mLのP05;
9mM〜11mMの濃度のクエン酸ナトリウム;
4.5%〜5.5%(w/v)のソルビトール;
0.09%〜0.11%(w/v)の濃度のポロキサマー188;及び
1〜20mMの濃度のメチオニンからなる水性製剤であって、
5.7〜6.3のpHを有する、請求項26に記載の水性製剤であって、IL−1関連障害の治療に使用するための前記製剤。 - 水性製剤が25℃で少なくとも4週間にわたり保管された場合に、メチオニンを含まない対応する水性製剤に比較して酸化が減少する、請求項27に記載の使用のための水性製剤。
- 皮下、静脈内または筋肉内投与するためのものである、請求項23、24、27および28のいずれか一項に記載の使用のための水性製剤。
- 注入するためのものである、請求項29に記載の使用のための水性製剤。
- 請求項1〜22のいずれか一項に記載の水性製剤を含む容器又は薬物送達デバイス。
- ブローフィルシール容器である、請求項31に記載の容器又は薬物送達デバイス。
- 多用量容器である、請求項31に記載の容器又は薬物送達デバイス。
- 不活性ガスを含有するパウチ内に密封される、請求項31〜33のいずれか一項に記載の容器又は薬物送達デバイス。
- 被験者におけるIL−1関連障害を治療するための医薬品の製造における、請求項1〜22のいずれか一項に記載の水性製剤の使用。
- 請求項31〜34のいずれか一項に記載の容器又は薬物送達デバイスを含むキットであって、及び取扱説明書も含んでいてもよいキット。
- 前記IL−1関連障害は癌である、請求項35に記載の使用。
