Description
Claims (41)
- 腹壁を横断して移動し、患者の腹部領域を通気するのに適した通気用の外科手術器具であって、該器具は、
近位端と遠位端との間の軸線に沿って延びる第1管腔を形成する第1の壁を備えたカニューレであって、前記近位端で圧力下にある流体源に接続されるようになっている前記カニューレと、
このカニューレの前記第1管腔に挿入可能であり且つ前記第1管腔から取り外し可能である套管針と、を有し、
前記套管針は、近位端と遠位端との間を延びる第2管腔を形成する第2の壁を備えたシャフトを有し、このシャフトは、腹腔鏡が挿入される近位端と、前記腹腔鏡が挿入され且つ前記カニューレから前記套管針への流体移動を可能にする通気路と、を備え、腹腔鏡を受け入れるようになっており、前記シャフトは、このシャフトの遠位端に設けられ、外科手術器具が前記シャフトの遠位端を貫かないように前記シャフトの遠位端を封入する先端部と、この先端部に形成され、前記通気路と連絡状態にあるとともに、加圧流体を噴射して腹部領域を通気するようになっている少なくとも1個の換気穴と、を有し、前記先端部は、腹壁と腹部領域の視認を容易にする透明素材から形成されていることを特徴とする通気用の外科手術器具。 - 前記シャフトは、透明素材から形成されて、腹壁と腹部領域の視認を容易にしたことを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記第2管腔及び前記通気路は、分離されていることを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記第2管腔及び前記通気路は、1本のチャネルとして兼用されることを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記通気路と連絡状態にあって、前記シャフトに沿って形成された第2換気穴を更に備えている、請求項1に記載の通気用の外科手術器具。
- 前記先端部は鈍らな鈍先端部であることを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記シャフトおよび前記先端部は一体成形されていることを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記換気穴の幾何学的形状は、円形、楕円形、正方形、矩形のうちのいずれであってもよいことを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記鈍先端部は、前記換気穴が通気のために位置決めできたことを示すマーカーを有していることを特徴とする、請求項6に記載の通気用の外科手術器具。
- 前記マーカーは、前記換気穴が腹壁を刺し通した点を示すことを特徴とする、請求項9に記載の通気用の外科手術器具。
- 前記シャフトにはスコープ止め具が設けられて、腹腔鏡がシャフトの中の深すぎる位置まで挿入されないようにするとともに、前記通気路と前記換気穴のうちの少なくとも一方を遮断するようにしたことを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記カニューレの近位端に配置されている封鎖ハウジングを更に備えている、請求項1に記載の通気用の外科手術器具。
- 前記封鎖ハウジングは、隔壁シールと、腹腔鏡が存在している場合には器具封鎖を形成し、器具が存在していない場合にはゼロ封鎖を設ける複数の葉状片とを有していることを特徴とする、請求項12に記載の通気用の外科手術器具。
- 前記葉状片はその厚さが、患者の体内の腹部圧が突然の急騰を受けた場合に、反転して圧力を解放する圧力解放機構を設けるのに望ましい寸法になるように形成されていることを特徴とする、請求項13に記載の通気用の外科手術器具。
- 前記鈍先端部は中空であることを特徴とする、請求項6に記載の通気用の外科手術器具。
- 前記先端部および前記シャフトのうち少なくとも一方は透光性素材から形成されていることを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記透光性素材または透明素材はポリカーボネートであることを特徴とする、請求項2または請求項16に記載の通気用の外科手術器具。
- 前記隔壁シールは、クラトン及びシリコーンを含む弾性材料から形成されていることを特徴とする、請求項13に記載の通気用の外科手術器具。
- 前記シャフトはその直径の範囲が約2mmから約5mmであることを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記先端部は鋭利であり、先が尖っており、または、刃が装備されており、体組織の刺し通しを容易にしていることを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記先端部は非対称的であることを特徴とする、請求項20に記載の通気用の外科手術器具。
- 前記先端部はその形状が略ドーム状か、または、略円錐状であることを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記シャフトまたは前記先端部に形成された少なくとも1枚の刃を更に備えている、請求項22に記載の通気用の外科手術器具。
- 前記シャフトの内径部に設置されて、腹腔鏡をロックするようにしたエラストマーロックを更に備えている、請求項1に記載の通気用の外科手術器具。
- 前記腹腔鏡のロックはシリコーンのO字型リングであり、その寸法と形状は、自らが自由に回転した結果、腹腔鏡を前記シャフトに対して自由に回転させることができるように設定されることを特徴とする、請求項24に記載の通気用の外科手術器具。
- 前記腹腔鏡のロックは回転式に固定されて、腹腔鏡が前記シャフトに対して回転することができないようにしたことを特徴とする、請求項24に記載の通気用の外科手術器具。
- 前記シャフトは可撓性のスリーブから形成されていることを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記先端部は、腹壁を刺し通している最中に閉鎖された第1位置と、腹壁を刺し通した後で腹部領域の通気を行うように加圧流体を噴出するために開放された第2位置とを有していることを特徴とする、請求項1に記載の通気用の外科手術器具。
- 前記先端部は引上げ蓋になっており、腹壁を縦断する際に自動的に開くようになっていることを特徴とする、請求項28に記載の通気用の外科手術器具。
- 前記シャフトおよび前記引上げ蓋を接続する維持部材を更に備えている、請求項29に記載の通気用の外科手術器具。
- 前記先端部はフラッパ弁になっており、ガスまたは流体が流体源に導入されて通気路に入ると、第2位置まで開くようにしたことを特徴とする、請求項28に記載の通気用の外科手術器具。
- 前記フラッパ弁は逆フラッパ弁であることを特徴とする、請求項31に記載の通気用の外科手術器具。
- 前記フラッパ弁はバネ偏倚フラッパ弁であることを特徴とする、請求項31に記載の通気用の外科手術器具。
- 前記維持部材はバネ、バネワイヤ、オフセット蝶番、一体蝶番のいずれか1つであることを特徴とする、請求項30に記載の通気用の外科手術器具。
- 前記引上げ蓋は2部材片の引上げ蓋であることを特徴とする、請求項29に記載の通気用の外科手術器具。
- 前記先端部は少なくとも2枚の花弁状片から構成されており、前記シャフトの側面に向けて前記開放された第2位置へ復位することを特徴とする、請求項29に記載の通気用の外科手術器具。
- 前記シャフトは、細長い管であり、前記通気用の外科手術器具は、更に、細長い管の遠位端に配置されて患者の腹壁と腹部領域の視認を容易にする光学素子を有することを特徴とする、請求項1に記載の通気用外科手術器具。
- 前記通気路は前記細長い管の周囲でコイル状に形成されていることを特徴とする、請求項37に記載の腹腔鏡通気用外科手術器具。
- 前記細長い管の前記遠位端は先細りになり、略円錐形先端を設けた端部を形成していることを特徴とする、請求項37に記載の腹腔鏡通気用外科手術器具。
- 前記カニューレは、その近位端と遠位端との間の軸線に沿って延びる前記第1の壁内に形成された通気路を備え、前記シャフトの通気路は、前記シャフトの遠位端に形成され、前記カニューレの通気路と整列状態になって加圧流体を噴出して少なくとも1個の換気穴によって腹部領域を通気するようになっている、請求項1に記載の通気用の外科手術器具。
- 前記套管針の近位端で挿入するようになっているとともに、套管針が腹壁を貫いて設置されると、遠位端まで前進させられる腹腔鏡を更に備えている、請求項40に記載の通気用の外科手術器具。
