Claims (17)
- 人工心臓弁(30)の経血管的植え込みのための自己拡張可能な医療器具(100)であって、前記医療器具(100)が、患者の身体内に、カテーテルシステム(40)を用いて低侵襲性処置にて導入されることができ、且つ、細いワイヤ又はフィラメント(2’)から成る柔軟なメッシュ(2)からつくられたステント(1)を含み、前記柔軟なメッシュ(2)から構成された前記ステント(1)は、前記患者の身体内への前記医療器具(100)の挿入中に、第1の予め画定可能な形状を呈し、前記医療器具(100)の植え込み状態においては、予め画定可能な第2の形状を呈し、前記医療器具(100)が、前記柔軟なメッシュ(2)から構成された前記ステント(1)の前記第1の形状において圧潰された状態にあり、また、前記柔軟なメッシュ(2)から構成された前記ステント(1)の前記第2の形状においては拡張状態にあり、前記柔軟なメッシュ(2)から構成された前記ステント(1)が、予め画定可能なその第2の形状を呈し且つ前記医療器具(100)が拡張状態にあるときに、前記医療器具(100)は、
−前記医療器具(100)の植え込み状態にて前記患者の罹患した心臓弁(50)の少なくとも1つのポケット(51)に係合可能である横方向に裏返されたビーズ状部(12)を有する遠位側保持領域(10)と、
−近位側保持領域(20)と、
−前記遠位側保持領域(10)と前記近位側保持領域(20)との間に配置された中央領域(15)とを含み、
拡張された前記医療器具(100)の前記ステント(1)の前記中央領域(15)が、前記近位側保持領域及び/又は前記遠位側保持領域(10,20)の直径よりも小さい直径を有し、且つ、前記中央領域(15)が、前記医療器具(100)の植え込み状態において、前記罹患した心臓弁(50)の高さにおける血管壁(52)と確実な連結を形成するように構成され、
拡張された前記医療器具(100)における前記ステント(1)の前記遠位側保持領域(10)における前記ビーズ状部(12)が、前記メッシュ(2)の前記遠位端をマッシュルーム形状に外側に裏返すことにより形成され、
植え込まれた状態における拡張された前記医療器具(100)の前記ステント(1)の前記遠位側保持領域(10)における前記ビーズ状部(12)が、裏返されて、前記患者の前記罹患した心臓弁(50)の前記少なくとも1つのポケット(51)内に入ることができ、これにより、前記医療器具(100)を前記患者の前記罹患した心臓弁(50)の高さに位置決めするための手段として機能する、自己拡張可能な医療器具(100)。 - 前記ステント(1)が、さらに、前記中央領域(15)に配置された自己拡張可能な人工心臓弁(30)を有し、前記人工心臓弁(30)は、前記医療器具(100)が前記カテーテルシステム(40)から解放されているときに自己拡張する、請求項1に記載の自己拡張可能な医療器具(100)。
- 前記メッシュ(2)はメッシュチューブであり、前記医療器具(100)は、その拡張状態において、近位端及び遠位端に対して開放されている形態を呈する、請求項1又は2に記載の医療器具(100)。
- 拡張された前記医療器具(100)における前記ステント(1)の前記近位側保持領域(20)が、前記メッシュ(2)の自己拡張特性に基づいて、前記医療器具(100)の植え込み状態において、前記罹患した心臓弁(50)のすぐ近位における前記血管壁(52)と圧力嵌め連結を形成するように構成される、請求項1〜3のいずれか一項に記載の医療器具(100)。
- 拡張された前記医療器具(100)における前記ステント(1)の前記中央領域(15)が、前記メッシュ(2)の自己拡張特性に基づいて、前記医療器具(100)の植え込み状態において、前記患者の前記罹患した心臓弁(50)を前記罹患した心臓弁(50)より遠位の前記血管壁(52)に押し付けるように構成されている、請求項1〜4のいずれか一項に記載の医療器具(100)。
- 前記医療器具(100)の拡張状態において、前記ステント(1)は、バーベルの形状を呈する、請求項1〜5のいずれか一項に記載の医療器具(100)。
- 拡張された前記医療器具(100)において、前記ステント(1)の前記中央領域(15)は、前記近位側保持領域及び前記遠位側保持領域(10,20)の直径よりも小さい直径を有し、且つ、前記中央領域(15)は、前記罹患した心臓弁(50)の長さにほぼ対応する長さを有する、請求項1〜6のいずれか一項に記載の医療器具(100)。
- 自己拡張可能な前記人工心臓弁(30)がその前記中央領域(15)に配置された、前記メッシュ(2)からつくられた前記ステント(1)が、経血管的外科手術にて用いられる前記カテーテルシステム(40)の直径に向って先細り状になる、請求項1〜7のいずれか一項に記載の医療器具(100)。
- 前記ステント(1)は、その前記近位端及び/又は前記遠位端にて取り出し前記カテーテルシステム(40)と係合可能なマウンティング(4)を有し、前記医療器具(100)が、外部操作により前記ステント(1)が拡張状態から圧潰状態に切り替えられるように構成されている、請求項1〜8のいずれか一項に記載の医療器具(100)。
- 拡張された前記医療器具(100)の植え込み状態において、前記ステント(1)の前記柔軟なメッシュ(2)が、前記罹患した心臓弁(50)の周囲に多層構造で配置される、請求項1〜9のいずれか一項に記載の医療器具(100)。
- 自己拡張可能な前記人工心臓弁(30)がその前記中央領域(15)に配置された、前記柔軟なメッシュ(2)からつくられた前記ステント(1)が、前記ステント(1)の前記第2の形状が解剖学的状況に適合可能であるように構成され、この適合により、前記ステント(1)が、拡張された前記医療器具(100)の植え込み状態において前記人工心臓弁(30)の最大拡張を達成し、尚且つ、前記血管壁(52)の最適な横方向封止を達成する、請求項1〜10のいずれか一項に記載の医療器具(100)。
- 自己拡張可能な前記人工心臓弁(30)がその前記中央領域(15)に配置された、前記柔軟なメッシュ(2)からつくられた前記ステント(1)が、前記人工心臓弁(30)を有する前記ステント(1)がカテーテルシステム(40)内に引き戻されることができ、且つ、前記医療器具(100)の植え込み中の何時でも患者の身体から除去され得るように構成されている、請求項1〜11のいずれか一項に記載の医療器具(100)。
- 自己拡張可能な前記人工心臓弁(30)がその前記中央領域(15)に配置された、前記柔軟なメッシュ(2)からつくられた前記ステント(1)が、前記人工心臓弁(30)を有する前記ステント(1)が成功裡に解放された後に前記カテーテルシステム(40)及びガイドワイヤ(41)を用いて再び引き込まれることができ且つ取り出され得るように構成されている、請求項1〜12のいずれか一項に記載の医療器具(100)。
- 自己拡張可能な前記人工心臓弁(30)がその前記中央領域(15)に配置された、前記柔軟なメッシュ(2)からつくられた前記ステント(1)が、患者の大動脈弁、僧帽弁、肺動脈弁又は三突弁を交換するように構成されている、請求項1〜13のいずれか一項に記載の医療器具(100)。
- 前記柔軟なメッシュ(2)は、ワイヤメッシュである、請求項1〜14のいずれか一項に記載の医療器具(100)。
- 前記柔軟なメッシュ(2)は、記憶作用を有する材料からつくられる請求項1〜15のいずれか一項に記載の医療器具(100)。
- 前記記憶作用を有する材料は、特定のニチノール又は形状記憶プラスチックである、請求項16に記載の医療器具(100)。
