Description
本発明は、一般に、組織、組織塊、組織腫瘍および病巣を治療する方法に関する。さらに特定すると、本発明は、腫瘍および組織塊の観血を最小にして治療的処置をする装置および方法に関する。さらに特定すると、本発明は、腫瘍および組織塊へのエネルギー送達を高めて、臨床上の成果を向上させつつ、さらに大きく速い切除容量を生じるために、流体を利用する方法および装置に関する。
RFエネルギーを使用して腫瘍を治療する現在の方法は、いくつかの重要な欠点があり、これには、不完全な切除容量、小さい切除容量、組織の乾燥および焦げ、または長引く切除時間が挙げられる。本発明は、これらの問題および他の関連した問題を解決する方法および装置を提供する。
本発明は、1局面では、組織切除装置を含み、該装置は、細長送達装置および複数の電極から構成され、該細長送達装置は、遠位末端で終わる管腔を有し、そして該電極は、後退位置と展開位置との間で移動するように、該装置に備え付けられ、該後退位置では、該電極は、該装置の管腔内に配置され、そして該展開位置では、該電極は、複数のアーチ形で側方に伸長して角度を付け間隔を開けた位置で、該遠位末端から展開される。各展開した電極は、個別電極切除容量を規定し、該容量は、切除の初期段階では、電極にRF電流を加えたときに、それらを組織内に展開しつつ、その電極に近接しており、この場合、該電極にRF電流を引き続いて加えると、該個別電極切除容量が増え、互いに併合して、合体電極切除容量を形成する。
本発明の実施形態は、従来の手段よりも速く大きくかつ一貫した切除容量を生じるために、切除電磁エネルギーを送達するように導電性を高めた溶液を利用することにより、腫瘍および外傷(例えば、肝腫瘍)を治療する方法および装置の利点を引き出す。しかしながら、中空チューブまたは中空電極を通って流体を注入する際の潜在的な問題点の1つには、電極を組織に注入するにつれた電極流体管腔の閉塞、もしくはエネルギー送達中での電極の加熱により起こる組織凝固、または両方の組合せがある。本発明のさらなる実施形態は、この電極の組織への挿入中または切除エネルギーの送達中に起こる電極および注入管腔の組織閉塞という問題に対する多数の解決法を提供する。
Claims (26)
- 組織切除装置であって、該装置は、以下の(a)〜(d)を含む:
(a)細長送達装置であって、該細長送達装置は、遠位末端で終わる管腔を有する;
(b)複数の電極であって、該電極は、後退位置と展開位置との間で移動するように、該装置に備え付けられ、該後退位置では、該電極は、該装置の管腔内に配置され、そして該展開位置では、該電極は、組織容量を規定し、該電極は、電源から該電極に供給される出力に応じて該容量内で切除を引き起こすために、該電源との接続に適合される;
(c)複数の細長受動センサ素子であって、該素子は、後退位置と展開位置との間で移動するように、該装置に備え付けられ、該後退位置では、該センサ素子は、該装置の管腔内に配置され、そして該展開位置では、該センサ素子は、複数の角度を付け間隔を開けた位置で、該遠位末端から展開され、該位置では、該展開したセンサ素子は、該合体電極切除容量に対応する容量内で分散される;
(d)少なくとも1つのセンサであって、該センサは、該容量での組織切除をモニターするために、各々の該センサ素子上に配置される;
(e)制御装置であって、該制御装置は、該電極および該センサ素子に、作動可能に連結され、(i)該電極が組織内で展開された状態で、電極に出力を供給し、個別電極切除容量から進行する組織切除を引き起こして、該合体電極切除容量を満たし、そして(ii)該センサ素子の領域での切除範囲を決定する;
ここで、該電極への出力の供給は、該合体電極切除容量の全体にわたって、組織切除レベルおよび範囲を制御するように、調節され得る、
装置。 - 前記電極およびセンサ素子が、それらの後退位置からそれらの展開位置へと1ユニットとして移動するように、作動可能に連結される、請求項1に記載の装置。
- 前記電極が、前記センサ素子の移動とは無関係に、それらの後退位置とそれらの展開位置との間で移動可能である、請求項1または2に記載の装置。
- 各センサ素子が、その展開状態で、展開状態にある隣接した電極対のほぼ中間で、前記初期個別電極切除容量の外に配置される、請求項1〜3のいずれか1項に記載の装置。
- 前記センサ素子が、導電性ワイヤであり、そして前記制御装置が、該ワイヤの領域にて、該センサ素子の領域での切除範囲の尺度として、組織インピーダンスを決定するように作動可能である、請求項1〜4のいずれか1項に記載の装置。
- 前記センサ素子が、温度センサであり、そして該センサが、該センサ素子の領域での切除範囲の尺度として、該温度センサの領域にある組織の温度を決定するように作動可能である、請求項1〜5のいずれか1項に記載の装置。
- 前記センサ素子が、光ファイバーであり、そして前記制御装置が、該センサ素子の領域での切除範囲の尺度として、該ファイバーの領域での光学的性質を決定するように作動可能である、請求項1〜6のいずれか1項に記載の装置。
- 前記電極が、中空針電極であり、該電極を組織内に展開しつつ、該電極を通って組織内に液体を注入できる、請求項1〜7のいずれか1項に記載の装置。
- 各電極を個々に通って制御した流体流れを可能にするように設計される、請求項8に記載の装置。
- 各電極が、その遠位末端領域に沿って、複数の注入ポートを有し、そして鞘により覆われ、該鞘が、展開位置と注入位置との間で軸方向に移動可能であり、該展開位置および該注入位置では、該注入ポートが、それぞれ、覆われ、そして露出される、請求項8または9に記載の装置。
- 各電極が、その遠位末端領域に沿って、複数の注入ポートを有し、そして鞘により覆われ、該鞘が、一定間隙を含む、請求項8〜10のいずれか1項に記載の装置。
- 前記制御装置が、表示機能および調節可能出力機能を含み、該表示機能が、前記組織容量での切除範囲をユーザーに表示し、そして該調節可能出力機能により、該ユーザーが、該電極に加えた出力を調節できる、請求項1〜11のいずれか1項に記載の装置。
- 前記制御装置が、前記センサの領域での切除範囲に関連して該センサ素子から受信した情報に応答して、該電極に加えられた出力レベルを調節するように作動可能である、請求項1〜12のいずれか1項に記載の装置。
- 前記電極が、中空針電極であり、該電極を組織内に展開しつつ、該電極を通って組織内に液体を注入でき、そして前記制御装置が、表示機能および調節可能流体制御機能を含み、該表示機能が、該センサの領域での切除範囲をユーザーに表示し、そして該調節可能流体制御機能により、該ユーザーが、個々の電極に供給された液体の速度を調節できる、請求項1〜13のいずれか1項に記載の装置。
- 前記電極が、中空針電極であり、該電極を組織内に展開しつつ、該電極を通って組織内に液体を注入でき、そして前記制御装置が、前記センサ容量での切除範囲に関連して該センサ素子から受信した情報に応答して、該電極を通る液体の速度を制御するように作動可能である、請求項1〜14のいずれか1項に記載の装置。
- 前記電極が、展開したとき、所望の合体電極切除容量を規定するプラトンの立体の面の中心近くに位置している、請求項1〜15のいずれか1項に記載の装置。
- 前記センサ素子が、展開したとき、前記プラトンの立体の頂点近くに位置している、請求項16に記載の装置。
- 錐体を囲む実質的に球形の容量を切除するために、4個の電極および4個のセンサ素子を有し、該電極が、展開したとき、該錐体の面の中心近くに位置しており、そして該センサ素子が、展開したとき、該錐体の頂点近くに配置される、請求項16に記載の装置。
- さらに、患者の表面に適用するように適合された体表電極を含み、前記制御装置が、前記複数の電極と該体表電極との間で、出力を加えるように作動可能である、請求項1〜18のいずれか1項に記載の装置。
- 前記装置が、双極モードで作動するように構成されている、請求項1〜19のいずれか1項に記載の装置。
- 前記細長送達装置遠位末端の全部または一部が、プラスチックである、請求項1〜20のいずれか1項に記載の装置。
- 前記細長送達装置遠位末端の全部または一部が、エラストマーである、請求項1〜21のいずれか1項に記載の装置。
- 前記細長送達装置が、丸みを付けたかまたは滑らかな内部前縁を有するように構成される、請求項1〜22のいずれか1項に記載の装置。
- 前記電極が、RFエネルギー源に作動可能に連結されるように適合されたRF電極である、請求項1〜23のいずれか1項に記載の装置。
- 前記電極が、マイクロ波エネルギー源に作動可能に連結されるように適合されたマイクロ波電源である、請求項1〜24のいずれか1項に記載の装置。
- 前記細長送達装置が、剛性、半剛性および可撓性のいずれかである、請求項1に記載の装置。
