一態様では、本明細書に記載の増殖された操作されたT細胞は、異常なアポトーシスまたは分化プロセス(例えば、がんなどの細胞の増殖障害または細胞の分化障害)に関連した障害を治療するのに有用である。本発明の方法を用いた治療に適していてもよいがんの非限定的例が、以下に記載される。
T細胞の増殖および遺伝子修飾前に、T細胞源が対象から得られてもよい。T細胞は、末梢血単核細胞、骨髄、リンパ節組織、臍帯血、胸腺組織、感染部位からの組織、腹水、胸水、脾臓組織、および腫瘍をはじめとするいくつかの起源から得られ得る。特定の実施形態では、当該技術分野で利用できる任意の数のT細胞株が使用されてもよい。特定の実施形態では、T細胞は、Ficoll(商標)分離などの当業者に知られている任意の数の技術を用いて、対象から採取された血液の単位から得られ得る。好ましい一実施形態では、個人の循環血液からの細胞は、血液成分分離によって得られてもよい。血液成分分離製品は、典型的には、T細胞、単球、顆粒球、B細胞をはじめとするリンパ球、その他の有核白色血液細胞、赤血球、および血小板を含有する。血液成分分離によって採取された細胞は、洗浄されて血漿画分が除去され、後続の処理ステップのために適切な緩衝液または培地に入れられてもよい。細胞は、リン酸緩衝生理食塩水(PBS)で洗浄されてもよく、カルシウムを欠き、マグネシウムを欠いてもよく、または全部でないとしても多くの二価の陽イオンを欠いてもよい、洗浄液で洗浄されてもよい。カルシウムの非存在下での初期活性化ステップは、拡大された活性化をもたらし得る。当業者は容易に理解するであるように、洗浄ステップは、製造業者の使用説明に従って、半自動化「通過型」遠心分離機(例えば、Cobe 2991 ceil processor、Baxter CytoMate、またはHaemonetics Cell Saver 5)などを使用することによって、当業者に知られている方法によって達成されてもよい。洗浄後、細胞は、例えば、Ca3+非含有、Mg2+非含有PBS、PlasmaLyte Aなどの様々な生体適合性緩衝液に、または緩衝液ありまたはなしのその他の生理食塩水に再懸濁されてもよい。代案としては、血液成分分離サンプルの望ましくない成分が除去され、細胞が培養液に直接再懸濁されてもよい。
自己由来細胞は、当技術分野で公知の任意の適切な経路によって投与され得る。好ましくは、細胞は、約30〜約60分間持続する、動脈内または静脈内注入として投与されてもよい。その他の例示的な投与経路としては、腹腔内、クモ膜下腔内、およびリンパ管内が挙げられてもよい.
1つのACTストラテジーは、腫瘍転移の腫瘍断片または単一細胞酵素消化物から生体外で増殖された、自己由来TILの移植を伴う。腫瘍のT細胞浸潤物は本質的にポリクローナルであり、複数の腫瘍抗原を集合的に認識する。例えば、その全体が参照により本明細書に援用される、Rosenberg et al.,N.Engl.J.Med.(1988)319:1676−1680を参照されたい。
自己由来末梢血単核細胞(PBMC)は、抗原によって生体外で刺激され、ACTに使用され得る腫瘍抗原特異的またはポリクローナルCD8+および/またはCD4+T細胞クローンを産生し得る。例えば、それぞれの内容が参照により本明細書に援用される、Mackensen et al.,J.Clin.Oncol.(2006)24(31):5060−5069;Mitchell et al.,J.Clin.Oncol.(2002)20(4):1075−1086;Yee et al.,Proc.Natl.Aad.Sci.USA(2002)99(25):16168−16173;Hunder et al.,N.Engl.J.Med.(2008)358(25):2698−2703;Verdegaal et al.,Cancer Immunol.Immunother.(2001)60(7):953−963を参照されたい。ナイーブPBMC集団から腫瘍特異的T細胞を増殖させるという、時間がかかる労力集約的な工程を回避するために、ACTの抗原特異的T細胞は、HLA−A0201を発現する人工抗原提示細胞(aAPC)、共刺激分子、および膜結合サイトカインを使用する、自己由来PBMCの複数の刺激を使用して生成されてもよい。例えば、その全体が参照により本明細書に援用される、Suhoski et al.,Mol.Ther.(2007)15(5):981−988;Butler et al.,Sci.Transl.Med.(2011)3(80):80ra34を参照されたい。
上述のような所望の特異性を有するTCRを発現させるためのT細胞の遺伝子操作は、ACTのための非常に有望なアプローチであってもよい。それにもかかわらず、操作されたTCRα鎖およびβ鎖と、内因的TCR鎖とのミスペアリングの可能性がある。さらに、操作されたTCRを発現する細胞を使用したACTの成功は、標的とされるがん細胞における、TCRによって認識される特定のMHC分子の発現に依存する。これらの潜在的な複雑さを回避するために、T細胞は、代案としてキメラ抗原受容体(CAR)を発現するように操作されてもよい。
サイトカイン感受性アッセイ(CSA)
T細胞の適応度に対する生体外T細胞増殖の長さの役割を調査するために、T細胞を4、7、または10日間かけて製造した。この製造後、CSAを介してT細胞を解析し、以下のメトリックスを解析した:(1)T細胞の増殖倍数によって測定される細胞生存、(2)ヨウ化プロピジウムおよびアネキシンV染色を介して測定されるアポトーシス、(3)増殖染料PkH67の希釈率によって測定される分裂、(4)フローサイトメトリーによって測定されるサイトカイン受容体発現、および(5)フローサイトメトリーによって測定されるT細胞記憶表現型。
T細胞の生体外増殖の短縮は、21日間のアッセイにわたり、持続的なTscm様の表現型を示す(生体内有効性のために望まれる)
図5A〜5Dは、健常ドナーから得られ、(A)0日、(B)4日、(C)7日、および(D)10日間培養された、TCR導入T細胞の表現型を示す。増殖されたT細胞をCD45RO染色によってリンパ球から分離し、引き続いてCCR7染色によってTnaive/Tscm(CD45RO−CCR7+)を識別したところ、例えば、23.2%(4日目の増殖されたT細胞)、16.4%(7日目の増殖されたT細胞)、22.9%(10日目日目の増殖されたT細胞)であった。0日め(49.4%、増殖なし)と比較して、4日、7日、および10日目の増殖されたT細胞は、Tscm様の表現型を有する細胞数の減少を示す。
T細胞の生体外増殖の短縮は生存率の向上と相関する
解凍された増殖されたT細胞を追加的な抗原またはCD3刺激の非存在下、IL−7、IL−15、またはIL−2の存在下で生存する能力について評価した。4日目の増殖されたT細胞は、IL−7、IL−15、およびIL−2中で、約10倍、30倍、および15倍のピーク増殖倍数を示し、3つ全てのサイトカイン条件において、より後期の増殖されたT細胞よりも実質的により成長できた。逆に、7日目および10日目の増殖されたT細胞は、いずれのサイトカイン条件でも実質的な成長を維持できなかった。さらに、全てのサイトカインの非存在下では、各T細胞集団は、増殖プロトコルの長さに関係なく、同程度の速度で死亡した。
T細胞の生体外増殖の短縮はアポトーシスの減少と相関する
より初期の増殖された細胞の増殖倍数の増加、および分裂の増加があったことから、ヨウ化プロピジウム(PI)およびアネキシンVによる染色を介して評価されるように、アポトーシスに対応する減少があり得た。

増殖されたT細胞の細胞分裂に対するサイトカインの影響を判定するために、IL−2、IL−7、またはIL−15の存在下での増殖されたT細胞の細胞分裂を測定した。図13A〜13Cは、(A)IL−7(10ng/ml)、(B)IL−15(10ng/ml)、および(C)IL−2(300IU/ml)の存在下で、例えば7日目および10日目などのより長期間増殖されたものと比較して、例えば、4日目などのより初期の増殖されたTCR導入T細胞が、より多くの分裂細胞を含むことを示す。アッセイの10日後の10日目の細胞が欠如しているため、10日目までのデータを示す。アッセイの10日目に、例えば、IL−2(300U/ml)(図14A)、IL−7(10.0ng/ml)(図14B)、またはIL−15(10.0ng/ml)(図14C)などのより高濃度のサイトカインの存在下で、例えば、7日目および10日日目などのより長期間増殖されたものの増殖よりも、例えば、4日目などのより初期の増殖されたTCR形質導入T細胞のより多くの分裂細胞が増殖した。例えば、4日目などのより初期の増殖された細胞は、CSA中の10日間にわたる各時点で、増殖色素を希釈した細胞の百分率によって計算されるように分裂を起こした。より後期の増殖された細胞は、10日を過ぎた後の正確な分析にとって十分な細胞がなかったため、分析は10日目まで行った。IL−2では、4日目と7日目の増殖された細胞拡大細の間に約30%の統合分裂の低下があり、4日目と10日目の増殖された細胞の間に約50%の低下があり、p=0.0307であった。IL−7でも同様の傾向が見られ、4日目と7日目の増殖された細胞の間に約40%、4日目と10日目の増殖された細胞の間に約80%の低下があり、p=0.0006であった。IL−15でも同様の傾向が観察され、4日目と7日目の増殖された細胞の間に約20%、4日目と10日目の増殖された細胞の間に約40%の低下があり、p=0.0025であった。
生体外増殖の短縮はサイトカイン感受性の増加と相関する
CSAにおいて、リンパ球の百分率に基づくCD25発現(R2=0.82)またはCD25発現の平均蛍光強度(MFI)(R2=0.89)と、IL2誘導生存率との間には強い相関関係があった。CSAにおいて、リンパ球の百分率に基づくCD127の発現(R2=0.04)と、IL7誘導生存率との間には相関関係はなかった。注目すべきことに、CD127発現のMFI(R2=0.76)とIL7誘導生存率との間には、中等度の相関があった。CSAにおいて、リンパ球の百分率に基づくCD122発現(R2=0.42)とIL15誘導生存率に対する応答との間には、弱い相関関係があり、またはCD122発現のMFI(R2=0.67)とIL15誘導生存率に対する応答との間には、中等度の相関関係があった。
CD3/CD28製造中の細胞の作用機序(MOA)表現型決定
CSAの結果から、T細胞は増殖性サイトカインに応答する機能が低下しているようであったが、これはサイトカイン受容体発現が喪失していることが一因であってもよい。これらのデータは、10日目の増殖されたT細胞のわずかな一部が、サイトカインに応答する能力を維持してもよいことを示唆する。この観察は、T細胞集団の不均一性が、観察された挙動に関与していてもよいことを示唆する。この多様性と可能性の喪失を調べるために、(1)最終的相対テロメア長、(2)テロメラーゼ活性、(3)共刺激分子発現、(4)全RNA配列解析に対する、T細胞増殖の影響を分析した。
細胞は高度に分化して最終的には老化するので、テロメア長の損失は、機能不全細胞の証明である。この効果が、本発明者らの差次的に増殖されたT細胞で起こっているかどうかを調べるために、蛍光原位置ハイブリダイゼーションアッセイを用いて、内部細胞株対照と対照してT細胞の相対的テロメア長(RTL)を評価した。
CD3+CD28刺激に続いて、テロメア長の減少およびテロメラーゼ誘導の不均一性の減少に基づいて、活性テロメラーゼのレベルを酵素結合免疫吸着測定法(ELISA)を介して判定した。
CSAの結果は、差次的に増殖された細胞間で、開始記憶コンパートメントに明白な差があってもよいことを示す。開始記憶コンパートメントに対してより高解像度の分析を実施して、差次的に増殖されたサンプル間の差を検出した。
従来の記憶コンパートメントに加えて、どちらも生体内でのT細胞の持続性の増加に伴うことが知られている、共刺激マーカーCD28およびCD27の発現について、細胞を表現型同定した。
方法
T細胞製造
健常ドナー全血をHemacareから購入し、Ficoll勾配によってPBMCを単離した。4℃のPBS(Lonza17−516F)中の1μg/mlの抗CD3(eBioscience 16−0037−85)および1μg/mlの抗CD28(eBioscience 16−0289−85)抗体で一晩被覆された組織培養フラスコ上に、1×106個の生PBMC/mlを蒔種することによって、5%ヒトAB血清(Gemini100−318)を補充したTexMACS(Miltenyi130−097−196)培地中で、PBMCを16〜24時間活性化した。翌日、全細胞を単離して、1×106個の生細胞/mlに再懸濁し、5mlをGrex24ウェルプレート(Wilson Wolf 80192M)のウェル内に播種した。細胞は、10ng/mlのIL−7(peprotech 200−07)、100ng/mlのIL−15(peprotech 200−15)、および10μg/mlの硫酸プロタミンの存在下で、模擬形質転換するか、またはTCRレンティウイルスコンストラクト(Lentigenによって製造される)で形質転換した。翌日、細胞に、上記濃度でIL−7およびIL−15を補充した35mlの完全TexMACSを供給した。細胞は、所望の製造時間(合計4、7、または10日間)に応じて、2、5、または8日間さらに培養した。製造後、細胞をカウントし、Cyrostore10内で5×106/mlで凍結し、−80℃に16〜24時間置き、必要になるまでLN2気相で長期保存した。
細胞分裂は、増殖色素PkH67を希釈することによって測定してもよい。PkH67(Sigma PKH67GL)染色は、7日目または10日目の製造された細胞に比べてより大きな細胞サイズを考慮して、4日目の製造された細胞を2倍の濃度で染色したことを除いて、製造業者のプロトコルに従って実施した。PkH染色は、フローサイトメトリー生存色素染色前に実施した。
T細胞生成物は、5%ヒトAB血清および100U/mLベンゾナーゼ(Sigma E10114)を補充したTexMACS中で、1〜2x106/mlでおよそ4時間解凍し、休止させた。休止期間に続いて細胞をPkHで標識し、IL−7、IL−15、またはIL−2(R&D Systems 202−IL)を滴定したGrex24ウェルフラスコ内で、2×105個のリンパ球を合計21日間培養した。この間、3〜4日毎に、容積測定フローサイトメトリーによって細胞をカウントし、7日毎に記憶T細胞パネルで表現型同定した。サイトカインは7日毎に、出発濃度まで補充した。
生細胞を定量化し、PBS中で1〜2×106個の生細胞/mlに再懸濁し、次に製造業者のプロトコルに従って、生死染色で染色した。次に、細胞を流動緩衝液で洗浄し、下の表に示されるように所望の抗体濃度で再懸濁して4℃の暗所で15〜30分間染色したが、例外としてCCR7染色は、37℃の血清非含有RPMI(Gibco11835−030)中で行った。次に、細胞を流動緩衝液で洗浄し、固定緩衝液中に再懸濁して、BD FortessaまたはMiltenyi MACSQuant分析装置上で取得するまで4℃で保存した。以下の表は、全てのフローサイトメトリー染色で使用される試薬を含む。
相対テロメア長は、製造業者の指示(Dako/Agilent K5327)に従って判定した。簡潔に述べると、T細胞と対照腫瘍細胞1301(4Nゲノム)とを1:1の比率で混合した。次に、細胞を透過処理し、テロメアPNA FITCプローブを一晩ハイブリダイズした。翌日、対比ヨウ化プロピジウム染色を実施して無傷の細胞を識別し、フローサイトメトリーによって細胞を取得した。試験細胞のテロメア長は、対照腫瘍細胞株1301のテロメア長との比として計算した。
ヒトTCRβ鎖のCDR3領域の免疫配列決定は、immunoSEQ(登録商標)アッセイ(ワシントン州シアトルのAdaptive Biotechnologies)を用いて実施した。抽出されたゲノムDNAは、バイアス制御されたマルチプレックスPCR中で増幅させ、高スループット配列決定がそれに続いた。さらなる解析のために、各固有のTCRβ CDR3領域の絶対量を同定して定量化するために、配列を畳み込んでフィルタリングした。
クローン性は1−Peilouの均一性と定義され、
下流解析は、STAR、HTseq、Cufflink、および本発明者らによるラップスクリプトを含む、プログラムの組み合わせを用いて実施した。アライメントはTophatプログラムを用いて解析し、DESeq2/edgeRを通じて差次的発現差動発現を判定した。GOおよびKEGG濃縮は、ClusterProfilerによって実行した。Star−fusionおよびrMATSソフトウェアによって、遺伝子の融合、そして代替スプライシング事象の差を検出した。
参照ゲノムおよび遺伝子モデルのアノテーションファイルは、ゲノムウェブサイトブラウザ(NCBI/UCSC/Ensembl)から直接ダウンロードした。STARを用いて参照ゲノムのインデックスを構築し、STAR(v2.5)を用いて、対合末端クリーンリードを参照ゲノムに整列させた。STARは、Maximal Mappable Prefix(MMP)法を用いて、ジャンクションリードに対する正確なマッピング結果を生成し得る。
HTSeq v0.6.1を用いて、各遺伝子のマッピングされたリード数をカウントした。次に、遺伝子の長さと、この遺伝子にマッピングされたリードカウントとに基づいて、各遺伝子のFPKMを計算した。FPKM、マップされた百万リード当たりのエクソンモデルのキロベース当たりリードは、配列決定深度および遺伝子長の影響を同時に考慮し、遺伝子の発現レベルを推定するために一般に用いられる。
生物学的複製物を有するDESeq2では、DESeq2 Rパッケージ(2_1.6.3)を用いて、2つの条件/グループ間の差次的発現解析(条件当たり2つの生物学的複製物)を実施した。DESeq2は、負の二項分布に基づくモデルを用いて、デジタル遺伝子発現データにおける差次的発現を判定するための統計的ルーチンを提供する。結果として得られたp値は、偽検出率(FDR)を制御するためのBenjaminiおよびHochbergのアプローチを用いて補正した。DESeq2によって検出された補正p値<0.05を有する遺伝子は、差次的に発現されるものとして割り当てた。
対数調整を可能にするために、0FPKMを有する遺伝子に、0.001の値を割り当てる相関関係は、Rのcor.test関数を用いて、オプションセットalternative="greater"およびmethod="Spearman"で判定した。
差分間の相関を同定するために、異なるサンプルを表現レベルFPKMを用いてクラスター化し、ヒートマップ、SOM(Self−organizationmapping)、シルエット係数を用いたkmeansの関数を備えた階層的クラスタリング距離法を用いて相関を確認し、Rのデフォルトパラメータで最適な分類を適応させた。
遺伝子長の偏りが補正されたcluster Profiler Rパッケージによって、差次的発現遺伝子の遺伝子オントロジー(GO)濃縮分析を実施した。補正されたp値が0.05未満のGO項は、差次的発現遺伝子によって、有意に濃縮されていると見なされた。KEGGは、分子レベルの情報から、特にゲノム配列決定およびその他の高スループット実験技術によって作成された大規模分子データセットから、細胞、生物、および生態系などの生体系の高レベル機能および有用性を理解するためのデータベース資源である。クラスタープロファイラーRパッケージを用いて、KEGG経路における差次的発現遺伝子の統計的濃縮を試験した。
本開示の利点としては、高スループットの患者特異的様式で、移入された細胞の増殖および生存を増加させてアポトーシスを減少させることによって生体内で持続し、ひいては腫瘍の退縮を改善してACTの有効性を高めてもよい、生体外で製造されたT細胞のタイプを判定するために用いられてもよいサイトカイン感受性アッセイが挙げられてもよい。

- 約3〜約5日間増殖された前記T細胞が、活性化後に約7日間以上増殖された活性化T細胞と比較して、増殖および生存率の増加を提示する、請求項30〜35のいずれか一項に記載の方法。
- 前記増殖させるステップの前に、前記活性化T細胞をウイルスベクターで形質導入するステップをさらに含んでなり、前記ウイルスベクターがT細胞受容体(TCR)を発現するレトロウイルスベクターである、請求項1、22、27、および30のいずれか一項に記載の方法。
- 前記ウイルスベクターが、T細胞受容体(TCR)を発現するレンチウイルスベクターである、請求項37に記載の方法。
- 請求項1、22、27、および30のいずれか一項に記載の方法によって生産される、T細胞集団。
- 請求項1〜16のいずれか一項に記載の方法によって生産される、T細胞集団。
- 前記培養するステップが、抗原提示細胞の非存在下である、請求項1〜16のいずれか一項に記載の方法。
- 前記培養するステップが、前記増殖されたT細胞を約1,000〜約1×106個の細胞/cm2、約1,000〜約500,000個の細胞/cm2、約1,000〜約250,000個の細胞/cm2、約1,000〜約200,000個の細胞/cm2、約1,000〜約150,000個の細胞/cm2、約1,000〜約100,000個の細胞/cm2、約1,000〜約50,000個の細胞/cm2、約1,000〜約10,000個の細胞/cm2、または約1,000〜約5,000個の細胞/cm2の密度で播種するステップを含んでなる、請求項41に記載の方法。
- 前記培養するステップが、前記増殖されたT細胞を約1,000〜約200,000個の細胞/cm2、約1,000〜約150,000個の細胞/cm2、または約1,000〜約100,000個の細胞/cm2の密度で播種するステップを含んでなる、請求項41に記載の方法。
- 複数の増殖期間で増殖された凍結保存T細胞を解凍するステップと、
前記解凍されたT細胞をサイトカインの非存在下で休止させるステップと、
前記休止したT細胞を播種するステップと、
前記播種されたT細胞を少なくとも1サイクル期間培養するステップと
を含んでなる、固形腫瘍におけるT細胞の生体内持続性を予測する方法であって、
前記少なくとも1サイクル期間の開始時に、1つまたは複数のサイトカインを培養に添加し、
前記少なくとも1サイクル期間の終了時に、前記添加された1つまたは複数のサイトカインが枯渇し、
前記少なくとも1サイクル期間中に、前記培養されたT細胞を複数の時点で試料採取し、
前記試料採取されたT細胞のサイトカイン応答を測定し、
複数の増殖期間から、前記試料採取されたT細胞が最大サイトカイン応答を提示する増殖期間を同定し、
同定された増殖期間にわたって増殖された前記T細胞を固形腫瘍を治療するための組成物に製剤化する、方法。 - 前記T細胞が、活性化後約1日間〜約15日間、約2日間〜約14日間、約3日間〜約13日間、約3日間〜約12日間、約3日間〜約11日間、約3日間〜約10日間、約3日間〜約9日間、約3日間〜約8日間、約3日間〜約7日間、約3日間〜約6日間、約3日間〜約5日間、約3日間〜約4日間、約4日間〜約6日間、または約4日間〜約5日間増殖される、請求項44に記載の方法。
- 前記休止させるステップが、約0.5時間〜約48時間、約0.5時間〜約36時間、約0.5時間〜約24時間、約0.5時間〜約18時間、約0.5時間〜約12時間、約0.5時間〜約6時間、約1時間〜約6時間、約2時間〜約5時間、約3時間〜約5時間、または約1時間〜約24時間、約2〜約24時間、約12〜約48時間、約0.5時間〜約120時間、約0.5時間〜約108時間、約0.5時間〜約96時間、約0.5時間〜約84時間、約0.5時間〜約72時間、または約0.5時間〜約60時間内に実行される、請求項44〜45のいずれか一項に記載の方法。
- 前記播種するステップが、約1,000〜約200,000個の細胞/cm2、約1,000〜約150,000個の細胞/cm2、または約1,000〜約100,000個の細胞/cm2の密度である、請求項44〜46のいずれか一項に記載の方法。
- 前記1つまたは複数のサイトカインが、インターロイキン2(IL−2)、インターロイキン7(IL−7)、インターロイキン15(IL−15)、およびそれらの組み合わせからなる群から選択される、請求項44〜47のいずれか一項に記載の方法。
- 前記1サイクル期間が、サイクル当たり1〜10日間、サイクル当たり2〜10日間、サイクル当たり3〜10日間、サイクル当たり4〜10日間、サイクル当たり5〜10日間、サイクル当たり6〜10日間、サイクル当たり7〜10日間、サイクル当たり8〜10日間、またはサイクル当たり9〜10日間である、請求項44〜48のいずれか一項に記載の方法。
- 前記少なくとも1サイクル期間が、1サイクル期間、2サイクル期間、3サイクル期間、4サイクル期間、5サイクル期間、6サイクル期間、7サイクル期間、8サイクル期間、9サイクル期間、または10サイクル期間である、請求項44〜49のいずれか一項に記載の方法。
- 前記サイトカイン応答が、ナイーブT細胞(TN)および/または幹記憶T細胞(Tscm)/T中央記憶(Tcm)の個体数の増加、増殖の増加、アポトーシスの減少、およびそれらの組み合わせの1つまたは複数から選択される、請求項44〜50のいずれか一項に記載の方法。
