本明細書で用いられる用語「対象」または「患者」は、ヒトがん患者(例えば、進行難治性固形腫瘍などの進行固形腫瘍を有する患者)である。
本発明における使用にとって好適な抗ヒトLAG−3抗体(またはそれから誘導されるVH/VLドメイン)を、当技術分野で周知の方法を用いて産生することができる。あるいは、当技術分野で認識される抗LAG−3抗体を使用することができる。例えば、モノクローナル抗体25F7と呼ばれる(「25F7」および「LAG3.1」としても知られる)、US2011/0150892A1(その教示は参照により本明細書に組み込まれる)に記載の抗ヒトLAG−3抗体を使用することができる。使用することができる他の当技術分野で認識される抗LAG−3抗体としては、US2011/007023(その教示も参照により本明細書に組み込まれる)に記載のIMP731が挙げられる。
本発明における使用にとって好適な抗ヒトPD−1抗体(またはそれから誘導されるVHおよび/もしくはVLドメイン)を、当技術分野で周知の方法を用いて産生することができる。あるいは、当技術分野で認識される抗PD−1抗体を使用することができる。例えば、WO2006/121168(その教示は参照により本明細書に組み込まれる)に記載された、モノクローナル抗体5C4(本明細書ではニボルマブまたはBMS−936558と呼ばれる)、17D8、2D3、4H1、4A11、7D3、および5F4を用いることができる。他の公知のPD−1抗体としては、WO2008/156712に記載のランブロリズマブ(MK−3475)およびWO2012/145493に記載のAMP−514(これらの教示は参照により本明細書に組み込まれる)が挙げられる。さらに公知のPD−1抗体および他のPD−1阻害剤としては、WO2009/014708、WO03/099196、WO2009/114335およびWO2011/161699(これらの教示は参照により本明細書に組み込まれる)に記載のものが挙げられる。PD−1への結合についてこれらの当技術分野で認識される抗体または阻害剤のいずれかと競合する抗体を使用することもできる。
本発明における使用にとって好適な抗ヒトPD−L1抗体(またはそれから誘導されるVHおよび/もしくはVLドメイン)を、当技術分野で周知の方法を用いて産生することができる。あるいは、当技術分野で認識される抗PD−L1抗体を使用することができる。例えば、米国特許第7,943,743号(その内容は参照により本明細書に組み込まれる)に開示されたヒト抗PD−L1抗体を使用することができる。そのような抗PD−L1抗体としては、3G10、12A4(BMS−936559とも呼ばれる)、10A5、5F8、10H10、1B12、7H1、11E6、12B7、および13G4が挙げられる。用いることができる他の当技術分野で認識される抗PD−L1抗体としては、例えば、米国特許第7,635,757号および第8,217,149号、米国特許出願公開第2009/0317368号、ならびにPCT公開第WO2011/066389号および第WO2012/145493号(これらの教示も参照により本明細書に組み込まれる)に記載のものが挙げられる。PD−L1への結合についてこれらの当技術分野で認識される抗体または阻害剤のいずれかと競合する抗体を使用することもできる。
ヒト患者への投与にとって好適な医薬組成物は、典型的には、非経口投与のために、例えば、液体担体中で製剤化されるか、または静脈内投与のための液体溶液もしくは懸濁液中での復元にとって好適である。
本明細書で提供されるのは、抗LAG−3抗体と抗PD−1抗体との組合せを用いてヒト患者における固形腫瘍がん(例えば、進行難治性固形腫瘍)を処置するための臨床方法である。
本明細書で提供される併用療法は、固形腫瘍(例えば、進行難治性固形腫瘍)を有する対象を処置するための、抗LAG−3抗体と、阻害免疫受容体(例えば、その天然のリガンドへの結合時に、細胞傷害活性などの活性を阻害/中和する受容体)を遮断する別の抗体、特に、抗PD−1抗体との投与を含む。
ヒト患者における固形腫瘍を処置するための好適な処置プロトコールは、例えば、

BMS−936558は、Biacore(登録商標)バイオセンサーシステムを用いる表面プラズモン共鳴により測定された場合にナノモル濃度の親和性および高い特異度でPD−1に結合し、かくして、そのリガンドPD−L1およびPD−L2への結合を不可能にする完全ヒトIgG4(カッパ)アイソタイプのモノクローナル抗体である。BMS−936558は、BTLA、CTLA−4、ICOSまたはCD28などの、他の関連するファミリーメンバーに結合しない。BMS−936558の前臨床試験により、PD−1への結合がインビトロ(in vitro)でT細胞増殖の増強およびインターフェロン−ガンマ(IFN−ガンマ)の放出をもたらすことが示された。BMS−936558の重鎖および軽鎖アミノ酸配列を、それぞれ配列番号1および配列番号2に示す。
カニクイザルにおける毒性試験により、BMS−936558は、週に2回投与される50mg/kgまでの用量で、27用量が良好に許容されることが確認された。薬物関連知見は、トリヨードサイロニン(T3)の28%の可逆的減少に限られ、同時に甲状腺機能の他のマーカーにおける異常性はなかった(データは示さない)。
単剤療法としての、または他の治療剤と組み合わせたBMS−936558に関する全体的な安全性の経験は、現在まで処置された約1500人の対象における経験に基づくものである。一般に、単剤療法については、安全性プロファイルは腫瘍型間で類似する。1つの例外は肺浸潤有害事象(AE)であり、これは、いくつかの場合、肺症状およびX線像的変化の、BMS−936558と関連する原因と関連しない原因とを識別することが困難であることがあるため、NSCLCを有する対象において数字が大きくなり得る。安全性プロファイルは一般に、完了した、および進行中の臨床試験にわたって一致し、試験した10mg/kgまでの用量のいずれでも最大耐量に達しなかった。BMS−936558用量レベルに対する有害事象の発生率、重篤度、または原因にパターンは存在しなかった。
BMS−936558の単回用量の薬物動態(PK)を、0.3〜10mg/kgの用量範囲でCA209001における複数の腫瘍型を有する39人の対象において評価した。用量レベルにわたる中央値Tmaxは、1.6〜3.1時間の範囲であり、個々の値は0.9〜7時間の範囲であった。BMS−936558のPKは、0.3〜10mg/kgの範囲で線形であり、それぞれの用量レベルでCmaxおよびAUC(INF)の用量比例的増加ならびに低〜中程度の対象間の変動が観察された(すなわち、変動係数[CV]は16〜45%の範囲であった)。単回IV用量後の幾何平均クリアランス(CLT)は0.13〜0.19mL/h/kgの範囲であったが、平均分布容量(Vz)は用量間で83〜113mL/kgで変化した。BMS−936558の平均終末相T−HALFは17〜25日であり、内因性IgG4の半減期と一致していたが、これは、BMS−936558の排出機構がIgG4のものと類似し得ることを示している。BMS−936558の排出と分布は両方とも、試験した用量範囲内では用量とは無関係であると考えられた。複数の腫瘍型の複数用量試験(CA209003)の128人の対象からの入手可能なデータにおいて、平均T−HALFは21〜24時間であり、中央値T−maxは用量レベルにわたって0.6〜3.3であり、単回用量データと合致していた。
BMS−936558は、NSCLC、メラノーマ、RCC、および他の悪性腫瘍を有する対象における完了した1つの第1相単回用量試験および進行中の2つの複数用量漸増試験(第1相単剤療法:CA209003およびイピリムマブとの第1b相併用療法)において臨床活性を示した。腫瘍応答を、NCIにより確立された改変型の固形腫瘍における応答評価基準(RECIST)により決定した。評価可能な集団は、ニボルマブで現在処置されている様々な固形悪性腫瘍(メラノーマ、n=138;NSCLC、n=122;RCC、n=34)を有する294人の対象からなる。
BMS−986016は、ヒト免疫グロブリン遺伝子を発現する免疫化トランスジェニックマウスから単離されたヒトLAG−3に特異的な完全ヒト抗体である。BMS−986016はIgG4アイソタイプ抗体として発現し、このアイソタイプ抗体は、抗体または補体の媒介により標的細胞を死滅させてしまう可能性を低減または排除するために、Fc受容体結合を減弱させる安定化ヒンジ突然変異(S228P)を含む。BMS−986016の重鎖および軽鎖アミノ酸配列は、それぞれ配列番号17および配列番号18に示される。
以下の前臨床毒性試験を実施した:
A.抗LAG3.1抗体(BMS−986016の前駆体)およびBMS−936558を用いるカニクイザルにおける4週の間欠的(QW)静脈内投与による探索的組合せ薬力学および毒性試験
重要な結果は以下の通りであった。単独で、または50mg/kg/週のBMS−936558と組み合わせて50mg/kg/週で投与された抗LAG3.1は、いかなる有害な変化ももたらさなかった。単剤の抗LAG3.1に関する無有害作用量[No−observed−adverse−effect level(NOAEL)]は、50mg/kg/週(AUC[0−168h]=231,000μg・h/mL)であると考えられ、50mg/kg/週のBMS−936558と組み合わせた抗LAG3.1に関するNOAELは50mg/kg/週(平均抗LAG3.1 AUC[0−168h]=210,000μg・h/mL;平均BMS−936558AUC[0−168h]=159,500μg・h/mL)であると考えられた。
重要な結果は以下の通りであった。最大で100mg/kg/週で投与された単剤BMS−986016は、有害な変化をもたらさなかった。50mg/kg/週で投与された単剤BMS−936558は、脳の脈絡叢のわずかなものから最小限の不可逆的リンパ形質細胞炎症をもたらし、これは、BMS−986016およびBMS−936558を用いる組合せ処置と比較してリンパ形質細胞炎症の重症度および発生率がより低いこと、血管炎または組織破壊を欠くこと、ならびに処置経過中の臨床徴候が存在しないことを考慮すれば非有害であると考えられた。BMS−986016とBMS−936558と(それぞれ、100および50mg/kg/週)の組合せ投与は、試験の29日目に9匹のサルのうちの1匹のオスの瀕死をもたらした。26日目から29日目まで、このサルは、体温上昇、震え、赤いまたは透明な鼻汁、糞便の変化(不定形の糞便、不十分な糞便、または糞便の不在)、給餌行動の減少、軽度の脱水、くしゃみ、活動低下、および猫背の姿勢を呈した。獣医医療および抗生物質処置の2日後、この動物は改善を示さず、臨床状態不良のため29日目に安楽死させた。
BMS−986016−FITCによる陽性染色が、以下のヒト組織において観察された:膀胱、血液細胞、結腸−大腸、眼、食道、小腸、胃、腎臓、肺、リンパ節、胎盤、唾液腺、皮膚、脾臓、胸腺、扁桃腺、子宮頸部、および子宮内膜の単核白血球;ならびに骨髄の造血細胞の細胞膜または細胞膜顆粒。さらに、BMS−986016−FITCによる染色が、ヒト下垂体内分泌細胞上皮の細胞質において観察された。評価したカニクイザル組織の限られたパネル内で、BMS−986016−FITCによる染色が、脾臓の単核白血球の原形質膜または原形質膜顆粒において観察された。正常なヒトリンパ系組織(リンパ節、扁桃腺、脾臓、胸腺、骨髄および粘膜関連リンパ系組織)の胚中心および毛包間T細胞領域中の、リンパ球の形態および分布を有するLAG−3発現細胞の科学的報告に関して、この試験(ヒトおよびカニクイザル組織における)におけるBMS−986016−FITCによる単核白血球および造血細胞の染色が予測された。LAG−3 mRNAがヒト下垂体中で発現し、LAG3.1−G4P−FITC染色がパイロット組織交差反応性試験におけるヒト下垂体の下垂体前葉において観察されたことを考慮すれば、ヒト下垂体内分泌細胞上皮細胞質および細胞質顆粒のBMS−986016−FITC染色も予測された。BMS−986016はインビボで細胞質コンパートメントへのアクセスを有するとは予想されず、サルにおける反復用量毒性試験は下垂体に対する効果を示さなかったが、これらの所見は臨床的に意義があるものであり得る。
BMS−986016は、濃度、ドナー、またはインキュベーション時間とは無関係にヒトPBMCに提示された場合、サイトカイン放出を誘導しなかった。観察されたサイトカインのレベルは、定量化のアッセイ下限にあったか、またはその近辺にあり、ドナー間で用量依存性またはパターンの証拠はなく(IL−1β、IL−2、IL−5、IL−10、IL−12p70、およびIFN−γ)、または一般には、陰性対照と共にインキュベートされたPBMCからのサイトカインレベルと重複していた(IL−6、IL−8、TNF−α)。
バイオテクノロジー由来医薬品に関する規制ガイドライン(ICH Harmonised Tripartite Guideline、S6(R1)Preclinical Safety Evaluation of Biotechnology−Derived Pharmaceuticals.International Conference on Harmonisation、2011)に従って、BMS−986016に関する代謝試験は動物において行わなかった。モノクローナル抗体(mAb)の予想されるインビボでの分解は、シトクロムP450酵素とは無関係である生化学経路による小ペプチドおよびアミノ酸までである。
抗LAG−3抗体と抗PD−1抗体との組合せが抗腫瘍効能を強化するという仮説を検証するために、マウス腫瘍モデルにおいて実験を行った。これらの試験は、同系腫瘍モデル(Sa1N線維肉腫およびMC38結腸腺癌)における腫瘍増殖阻害を評価し、非肥満糖尿病(NOD)モデルにおける自己免疫の促進をモニタリングした。抗LAG−3抗体の投与により、図1に示されるように、これらの処置群において全腫瘍増殖阻害と無腫瘍(TF)マウスの数の増加との両方が得られた。抗PD−1抗体と組み合わせて投与された抗LAG−3抗体は、いずれかの薬剤単独での活性を超える、増強された抗腫瘍活性をもたらした。例えば、複数のSa1N腫瘍モデルにおいて、抗LAG−3抗体により、対照および抗PD−1抗体で処置されたマウス(0%〜10%のTFマウス)と比較して20%〜30%のTFマウスが得られたが、抗LAG−3抗体と抗PD−1抗体との組合せにより60%〜90%のTFマウスが得られた。MC38モデルにおいては、抗LAG−3抗体は適度の腫瘍増殖阻害のみを示したが、抗PD−1抗体と組み合わせて投与した場合、抗PD−1抗体のみについて観察されるものを超える増強された抗腫瘍活性が得られた(それぞれ、80%対40%のTFマウス)。
抗LAG−3抗体(BMS−986016)および抗PD−1抗体(BMS−936558)の第1相試験は、組合せ処置としてBMS−986016およびBMS−936558を投与する効能を証明するために進行固形腫瘍を有する患者において行われる。
この試験の主な目的は、進行固形腫瘍を有する対象における、BMS−936558と組み合わせて投与されたBMS−986016の安全性および忍容性を評価すること、ならびに用量制限毒性(DLT)および最大耐量(MTD)を同定することである。
これは、進行固形腫瘍を有する対象に、単剤として、およびBMS−936558(ニボルマブ)と組み合わせて投与されるBMS−986016の第1相、オープンラベル試験である。試験は3つのパートで行われる。パートAおよびパートBは、進行固形腫瘍を有する対象において単剤(パートA)またはBMS−936558と組み合わせて(パートB)投与されるBMS−986016を用いる3+3+3の用量漸増設計からなる。パートBにおける処置は、パートAにおける第3の用量コホートに漸増させる決定の際に開始される(用量漸増規則に従う)。続いて、2つのパートにおける漸増は同時に進行する。BMS−936558(パートB)と組み合わせたBMS−986016の用量が、単剤療法用量漸増群(パートA)上で安全であると以前に示されたBMS−986016の用量を超える点はない。パートCは、BMS−936558と組み合わせて投与されたBMS−986016を用いる、それぞれ約16人の対象の6つの疾患に限定された集団におけるコホート拡大からなる。パートCにおける処置は、パートBに関する最大耐量(MTD)(またはMTDが決定されない場合、最大投与量(MAD))が決定された場合に開始される。パートCのために選択される用量は、パートBのMTDまたはMADを超えないが、用量決定はパートAおよびBからの毒性およびPKおよび薬力学データを含む他のデータの評価を含んでもよい。試験の略図を図2に示す。
パートA
パートAにおいては、3+3+3の設計を用いて、単剤として投与されるBMS−986016の安全性を評価する。対象が同じ用量漸増コホート中の第3の対象の約1週間以内に投薬の1日目を開始することができる場合、第4の対象を用量漸増コホートの開始時に登録することができる。用量漸増中の投与量を、図2Aおよび図2Bおよび表1(以下に記載される)に示す。3人の対象(または、妥当な場合、4人)を、各用量コホートにおいて最初に処置する。用量コホート1において、最初の3人の対象(または妥当な場合、4人)のそれぞれを、監視対象と指定し、少なくとも5日間隔で処置を開始する。その後のコホートにおける対象は、処置開始日間で5日間隔で観察する必要はない。パートAにおける用量漸増は以下のように進行する:
・最初の3人の対象(または妥当な場合、4人)のうちの0人がDLT評価間隔内で用量制限毒性(DLT)を経験する場合、3人の対象(または妥当な場合、4人)の新しいコホートを、次により高い用量レベルで処置する。
・3人の対象(または妥当な場合、4人)のうちの1人がDLT評価間隔内でDLTを経験する場合、そのコホートを6人の対象に拡大する。
・6人の対象のうちの2人がDLT評価間隔内でDLTを経験する場合、そのコホートを9人の対象に拡大する。
・3人(または妥当な場合、4人)の対象のうちの2人以上、6人の対象のうちの3人以上、または9人の対象にうちの3人以上がDLT評価間隔中に用量コホート内でDLTを経験する場合、その用量レベルはMTDを超えたと決定される。
パートBにおける処置は、パートAにおいて、第3の用量コホートへと漸増する決定がなされた後に開始される(用量漸増規則に従う)。続いて、2つのパートにおける漸増は同時に進行する。どの時点においても、BMS−936558と組み合わせて投与されたBMS−986016の用量(パートB)が、単剤療法用量漸増群において安全であると以前に示されたBMS−986016の用量(パートA)を超えることはない。パートAとパートBとの両方について適格である対象にする処置の割り当ては、2つのパート間で交互であり、引き続き処置される対象は可能な場合はいつでも音声自動応答システム(IVRS)を介して異なるパートに割り当てられる。対象がこのアルゴリズムにより割り当てられるパートにおいて利用可能なオープニングが存在しない場合、対象は次のオープンなコホートまたはパートに割り当てられる。パートAと同様、3+3+3の設計をパートBにおいても用いて、ニボルマブと組み合わせて投与されたBMS−986016の安全性を評価する。同じ用量漸増コホート中の第3の対象の約1週間以内に投薬の1日目を開始することができる場合、第4の対象を用量漸増コホートの開始時に登録することができる。用量漸増中に評価された投与量を図2Aおよび図2Bおよび表Bに示す(以下に記載される)。パートAと同様、パートBにおける第1の用量コホート中の最初の3人の対象(または妥当な場合、4人)のそれぞれを、監視対象と指定し、少なくとも5日間隔で処置を開始する。
コホート拡大の目的は、BMS−986016とBMS−936558との組合せに関するさらなる安全性、忍容性、予備効能、薬物動態、および薬力学情報を集積することである。パートCのために選択される用量はパートBにおけるMTD(またはMTDが決定されない場合、MAD)を超えないが、パートAおよびBからの毒性およびPKおよび薬力学データを含む他のデータの評価を含んでもよい。用量は、パートBにおいて評価された用量レベルにおける中間の用量を含む。MTD未満の用量が選択される場合、パートCにおいて用いる組合せ用量レベルの選択について情報を与えるための補助として、モデリングを用いる。6つの拡大コホートは以下の表3Aおよび表3Bに列挙される腫瘍型に限定される。コホート拡大における毒性事象の連続評価を、拡大コホートにおける登録を通して実施する。任意の時点で、DLT基準を満たす処置関連毒性の統合率がパートCのコホート拡大において処置される全対象にわたって33%を超える場合、さらなる登録は中断される。毒性の性質および等級に応じて、ならびにリスク:利益比を評価した後、全コホートに関する新しい用量を、以前に試験したより低い用量レベルで、または以前に試験したより低い用量レベルにおける中間の用量レベルで開始する。
BMS−986016は、BMS−936558と関連する以前に記載された有害事象の頻度および重篤度を増大させるか、または新しい毒性をもたらす可能性を有する。パートAおよびパートBにおける用量漸増に関する決定を指導するために、試験薬物と関連し、試験薬物の開始の56日(8週)(すなわち、サイクル1の完了までのDLT評価間隔)以内に生じる有害事象の発生率、強度、および持続期間に基づいて用量制限毒性(DLT)を決定する。有害事象の重篤度は、National Cancer Institute(NCI)Common Terminology Criteria for Adverse Events(CTCAE)v4.0に従って等級付けられる。対象の管理のために、用量漸増(パートAおよびパートB)の間であろうと、またはコホート拡大(パートC)の間であろうと、いかなる時点であってもDLTが生じたなら、全ての試験薬物が停止され、その事象の試験薬物との関連性の評価は保留される。試験処置の再開始にはまず、対象が再処置のための基準を満たさなくてはならない。
以下の薬物関連事象はいずれも肝臓DLTと考えられる:
・持続期間にかかわらず、ALTもしくはAST>8xULNである、または
・医学的介入にもかかわらず2週間以内に等級1以下に戻ることができない、ALTもしくはAST>5xおよび≦8xULNである、または
・総ビリルビン>5xULNである、または
・ALTもしくはAST>3xULNおよび同時的総ビリルビン>2xULNである。
以下の薬物関連事象はいずれも非血液学的DLTと考えられる:
・全身的処置を要する等級2の免疫関連眼疼痛または視力低下、または
・局所療法に応答せず、局所療法の開始の2週間以内に等級1まで改善しない、等級2の眼疼痛もしくは視力低下、または
・以下に記載のものを除く等級3以上の非肝臓もしくは非血液学的毒性。
・72時間未満続く、臨床的に悪化していない、および自発的に解消する、または従来の医学的介入に応答する等級3の電解質異常
・3週間未満持続する、および膵炎の臨床的もしくはX線像の証拠と関連しないアミラーゼまたはリパーゼの等級3の増加
・48時間未満続く、および自発的に、または従来の医学的介入により等級1以下に解消する、等級3の悪心または嘔吐
・72時間未満続く、および血行動態の悪化(低血圧、または末端臓器かん流障害の臨床的もしくは実験室的証拠を含む)と関連しない、等級3の発熱
・ホルモン補充により良好に制御される等級3の内分泌障害
・等級3の腫瘍フレア(既知の腫瘍または疑われる腫瘍の部位に局在する疼痛、刺激、または発疹と定義される)
・7日未満の等級3の疲労。
以下の薬物関連事象はいずれも血液学的DLTと考えられる:
・任意の持続期間の等級4の発熱性好中球減少症
・5日より長く続く等級4の好中球減少症
・等級4の血小板減少症
・等級4の貧血
・臨床的に意義のある出血と関連する等級3の血小板減少症
・48時間より長く続く等級3の発熱性好中球減少症
・等級3の溶血。
署名されたインフォームドコンセント
対象は、標準治療の一部とは考えられないいずれの試験関連手順の実施の前にも、IRB/IECに認可されたインフォームドコンセント文書に署名し、日付を記入しなければならない。
a)対象は、進行した(転移性および/または切除不能である)治癒不可能な固形悪性腫瘍の組織学的または細胞学的確認を行わなければならない:
i)パートA。用量漸増:BMS−986016単剤療法
(1)原発性CNS腫瘍を有する対象を除く全ての組織学的固形腫瘍症例が容認される;
(2)限定されるものではないが、CTLA−4、イピリムマブ、トレメリムマブ、抗PD−1抗体、抗PD−L1抗体、抗PD−L2抗体、抗KIR抗体、抗CD137抗体、または抗OX40抗体などの免疫細胞調節性抗体レジメン(ICMAR)に以前に曝露されていない対象のみが許容される;
(3)対象は、そのような療法が存在する場合、進行または転移の状況における少なくとも1つの標準処置レジメンを受けた後、進行したか、またはその標準処置レジメンに認容性がなかったのでなければならない。
(1)原発性CNS腫瘍を有する対象を除く全ての組織学的固形腫瘍症例が容認される。以前の抗PD−1抗体療法または抗PD−L1抗体療法を行った、または行っていない対象が適格である。あるいは、原発性CNS腫瘍を有する対象を除いて、限定されるものではないが、抗CTLA−4抗体、抗PD−1抗体、抗PD−L1抗体、抗PD−L2抗体、抗KIR抗体、抗CD137抗体、または抗OX40抗体などのICMARにナイーブな全ての組織学的固形腫瘍症例が容認される。
(2)以前の抗PD−1抗体療法または抗PD−L1抗体療法を行っていない対象は、限定されるものではないが、イピリムマブ、トレメリムマブ、抗PD−L2抗体、抗KIR抗体、抗CD137抗体、または抗OX40抗体などの他のいかなるICMARに対しても、以前に曝露されていてはならない。あるいは、以前の抗PD−1抗体療法または抗PD−L1抗体療法を行っていない対象は、限定されるものではないが、イピリムマブ、トレメリムマブ、抗PD−L2抗体、抗KIR抗体、抗CD137抗体、または抗OX40抗体などの他のいかなるICMARに対しても、以前に曝露されていてはならない。
(3)直近の療法としての抗PD−1抗体または抗PD−L1抗体を用いる療法の間またはその後に疾患が進行するNSCLC対象。あるいは、直近の療法が、以前の抗PD−1抗体または抗PD−L1抗体である対象:
(a)疾患は抗PD−1抗体療法または抗PD−L1抗体療法の間に非応答性であり、療法開始から16週以内にPD(RECISTによる)を呈する。
(b)重篤な、および/または生命を脅かす抗PD−1抗体または抗PD−L1抗体と関連する毒性(例えば、以前の試験における用量制限毒性)のために療法が中止されていたのであってはならない。
(c)抗PD−1抗体療法または抗PD−L1抗体療法の最終投与後60日以内にインフォームドコンセントを提供しなければならない。
(d)限定されるものではないが、抗CTLA−4抗体療法、イピリムマブ、トレメリムマブ、抗PD−L2抗体、抗KIR抗体、抗CD137抗体、または抗OX40抗体などのICMARに対して以前に曝露されていてはならない。
(4)抗CTLA−4抗体療法および抗PD−1抗体療法または抗PD−L1抗体療法を受けている間またはその後に疾患が進行しているメラノーマ対象。(a)抗CTLA−4抗体療法および抗PD−1抗体療法または抗PD−L1抗体療法を連続的または組合せレジメンで受けていてもよい。(b)抗CTLA−4抗体療法の最終投与を、試験薬の初回投与の100日以上に受けていなければならない。(c)重篤な、および/または生命を脅かす抗体と関連する毒性(例えば、以前の試験における用量制限毒性)のために療法が中止されていたのであってはならない。(d)抗CTLA−4抗体療法および抗PD−1抗体療法または抗PD−L1抗体療法以外のいかなるICMARに対しても、以前に曝露されていてはならない。
(5)対象は、少なくとも1つの標準処置レジメンを受けた後、進行したか、またはその標準処置レジメンに認容性がなかったのでなければならない。
(1)以下の群が登録される:
(a)メラノーマ − ICMARにナイーブな対象
(b)メラノーマ − 直近の療法としての抗PD−1抗体療法または抗PD−L1抗体療法の間に疾患が非応答性であり、療法開始から16週以内にPD(RECISTによる)を呈する対象。対象は抗PD−1抗体療法または抗PD−L1抗体療法の最終投与後60日以内にインフォームドコンセントを提供しなければないので、重篤な、および/または生命を脅かす毒性(例えば、以前の試験における用量制限毒性)のために抗体療法を中止していないはずである。これらの対象は、限定されるものではないが、イピリムマブ、トレメリムマブ、抗PD−L2抗体、抗KIR抗体、抗CD137抗体、または抗OX40抗体などの他のいかなるICMARに対しても、以前に曝露されていてはならない。あるいは、抗CTLA−4抗体療法および抗PD−1抗体療法または抗PD−L1抗体療法を受けている(連続的または組合せレジメンで)間またはその後に疾患が進行している対象は適格である。抗CTLA−4抗体療法の最終投与を、試験薬の初回投与の100日以上に受けていなければならない。対象は、重篤な、および/または生命を脅かす毒性(例えば、以前の試験における用量制限毒性)のために抗体療法を中止していないはずである。これらの対象は、抗CTLA−4抗体療法および抗PD−1抗体療法または抗PD−L1抗体療法以外のいかなるICMARに対しても、以前に曝露されていてはならない。
(c)非小細胞肺がん(NSCLC) − ICMARにナイーブな対象。
(d)NSCLC − 直近の療法としての抗PD−1抗体療法または抗PD−L1抗体療法の間に疾患が非応答性であり、療法開始から16週以内にPD(RECISTによる)を呈する対象。対象は抗PD−1抗体療法または抗PD−L1抗体療法の最終投与後60日以内にインフォームドコンセントを提供しなければならいので、重篤な、および/または生命を脅かす毒性(例えば、以前の試験における用量制限毒性)のために抗体療法を中止していないはずである。これらの対象は、限定されるものではないが、イピリムマブ、トレメリムマブ、抗PD−L2抗体、抗KIR抗体、抗CD137抗体、または抗OX40抗体などの他のいかなるICMARに対しても、以前に曝露されていてはならない。あるいは、直近の療法としての抗PD−1抗体または抗PD−L1抗体を使用する療法の間またはその後に疾患が進行するNSCLC対象。対象は、重篤な、および/または生命を脅かす毒性(例えば、以前の試験における用量制限毒性)のために抗体療法を中止していないはずである。これらの対象は、抗CTLA−4抗体、抗PD−L2抗体、抗KIR抗体、抗CD137抗体、または抗OX40抗体などの他のいかなるICMARに対しても、以前に曝露されていてはならない。
(e)HPV関連頭頸部腫瘍 − ICMARにナイーブであり、p16免疫組織化学(IHC)陽性および/またはHPV−16 インサイチュ(in situ)ハイブリダイゼーション(ISH)陽性により定義される場合にHPV陽性である対象。(i)組織学的症例は扁平上皮癌に限定される。あるいは、進行/転移頭頸部腫瘍を有する対象−ICMARにナイーブである。(i)組織学的症例は扁平上皮癌に限定される。
(f)ICMARにナイーブな胃腺癌対象。
(i)HER2(+)およびHER2(−)の対象が許容される。
c)任意の以前の処置レジメンを有する対象は適格である。以下は、別系統の処置と考えない:進行中のレジメンへの化合物の追加、休薬期間後の同じレジメンの再開始、またはIVから経口療法への切替え。
d)応答評価のためのRECISTv1.1基準により定義された測定可能な疾患を有する少なくとも1つの病変の存在。測定可能な疾患の唯一の部位として以前に照射された領域中に病変を有する対象は、その病変がインフォームドコンセントの時点より前に明確な進行を示し、正確に測定できることを条件として登録を容認される。
e)0または1のECOG状態。
f)インフォームドコンセントの時点で12週以上の平均余命。
g)以下により定義される十分な臓器機能:
i)2000/μL以上の白血球(WBC)(最初の試験薬物投与の4週以内であれば、増殖因子なしでも安定)
ii)1500/μL以上の好中球(最初の試験薬物投与の4週以内であれば、増殖因子なしでも安定)
iii)100x103/μL以上の血小板(このレベルを達成するための輸注は最初の試験薬物投与の2週間以内は容認されない)
iv)8.5g/dL以上のヘモグロビン(このレベルを達成するための輸注は最初の試験薬物投与の2週間以内は容認されない)
v)1.5xULN未満のクレアチニンまたは40mL/min以上のクレアチニンクリアランス(Cockcroft−Gault式)
vi)3xULN以下のALTおよびAST
vii)1.5xULN未満のリパーゼおよびアミラーゼ
viii)1.5xULN以下の総ビリルビン(正常な直接ビリルビンを有さなければならないギルバート症候群を有する対象を除く)
h)正常な甲状腺機能、またはホルモン補充に対して安定である。
i)処置、PK、および薬力学試料採取ならびに必要とされる試験追跡に従う能力。
j)対象の再登録:この試験は、処置前の失敗として試験を中止した対象(すなわち、無作為化されていない/処置されていない対象)の再登録を容認する。再登録される場合、対象は再度同意しなければならない。
a)インフォームドコンセントの時点で18歳以上の年齢の男性および女性。
b)出産能力のある女性(WOCBP)は避妊方法を使用しなければならない。催奇形性試験薬物について、および/または催奇形性を評価するのに十分な情報がない場合(前臨床試験が行われていない)、2つの形態の避妊が必要である。1つの方法は高度に有効でなければならず(一貫して、また正確に用いられた場合に1%未満の失敗率)、第2の方法も高度に有効であってよい。個々の避妊方法は治験責任医師との相談で決定するべきである。WOCBPは被験薬の最終投与の後、合計24週間(30日間+被験薬を5半減期受けるのに必要とされる時間)にわたって避妊に関する指示に従わなければならない。出産能力のある女性(WOCBP)は、初潮を経験し、避妊手術(子宮摘出または両側卵巣摘出)を受けていない、閉経後ではない任意の女性と定義される。閉経は、他の生物学的または生理学的原因の非存在下での45歳を超える女性における12カ月の無月経と定義される。さらに、55歳未満の女性は、閉経を確認するために40mIU/mLを超える文書化された血清卵胞刺激ホルモン(FSH)レベルを有さなければならない。ホルモン補充療法(HRT)で処置された女性は、人工的に抑制されたFSHレベルを有する可能性があり、生理的FSHレベルを得るために排出期間を必要としてもよい。排出期間の持続時間は、用いられるHRTの型の関数である。以下の排出期間の持続時間は提言された指針であり、治験責任医師は血清FSHレベルをチェックする際にその判断を使用するべきである。排出期間中の任意の時点で血清FSHレベルが40mIU/mlを超える場合、その女性は閉経後であると考えることができる。
膣内ホルモン製品(リング、クリーム、ゲル)について最小で1週間
経皮製品について最小で4週間
経口背品について最小で8週間
他の非経口製品は6カ月程度の排出期間を必要とし得る。
c)女性は、治験薬の開始前24時間以内で血清または尿妊娠検査で陰性でなければならない(最小感度25IU/Lの総ヒト絨毛ゴナドトロピン(hCG)またはそのベータ画分の尿妊娠検査)。
d)女性は授乳中であってはならない。
e)WOCBPと性的に活発である男性は避妊方法を使用しなければならない。催奇形性試験薬物について、および/または催奇形性を評価するのに十分な情報がない場合(前臨床試験が行われていない)、2つの形態の避妊が必要である。1つの方法は高度に有効でなければならず(一貫して、また正確に用いられた場合に1%未満の失敗率)、第2の方法も高度に有効であってよい。WOCBPと性的に活発である男性は被験薬の最終投与の後、合計33週間(90日間+被験薬を5半減期受けるのに必要とされる時間)にわたって避妊に関する指示に従わなければならない。
標的疾患の例外
既知の、もしくは疑われるCNS転移または活動性疾患の唯一の部位としてのCNSを有する対象は、以下の例外を除いて除外される:
i)制御された脳転移を有する対象は登録を許可される。制御された脳転移は、同意の時点での照射および/または外科的処置の後、少なくとも4週間にわたってX線像による進行が見られないものと定義される。対象は、インフォームドコンセントの前の少なくとも2週間にわたってステロイドを除去されていなければならず、新しいか、または進行性の神経兆候および症状を有してはならない。
ii)脳転移の兆候または症状を有する対象は、脳転移がコンピュータ断層撮影(CT)または磁気共鳴イメージング(MRI)によって除外されない限り、適格ではない。
全生存が主要評価項目または副主要評価項目として列挙され、主要評価項目に基づく分析を完了しなかった、比較群における対象などの、BMS−936558を用いる以前の臨床試験への参加。
適切に処置された基底細胞もしくは扁平上皮皮膚がん、局所的前立腺がん、子宮頸部上皮内癌もしくは膀胱上皮内癌、または非浸潤性乳管がんもしくは上皮内小葉がんを除く、以前の悪性腫瘍を有する対象は除外される。2年前より前に(インフォームドコンセントの時点で)診断された悪性腫瘍を有する対象であって、治療目的で療法を受けており、間隔中に疾患の証拠がなく、再発の危険性が低いと考えられる対象は適格である。
単離された白斑、解消された小児喘息/アトピー、制御された副腎機能低下症または下垂体機能低下症を有する対象、およびグレーブス病の病歴を有する甲状腺機能正常患者(制御された甲状腺機能亢進症を有する対象は試験薬物投与の前にサイログロブリンおよび甲状腺ペルオキシダーゼ抗体および甲状腺刺激免疫グロブリンについて陰性でなければならない)を除く、いずれかの活動性自己免疫疾患または既知もしくは疑われる自己免疫疾患の病歴を有する対象。
対象にとって危険な試験薬物の投与を行うか、または試験に従うか、もしくは試験を許容する対象の能力に有害な影響を与え得る既知の、または基礎となる医学的状態。
毎日の酸素補給のための要件。
限定されるものではないが、以下のいずれかを含む制御されていないか、または有意な心血管疾患:同意前の6カ月以内の心筋梗塞または脳卒中/一過性脳虚血発作(TIA)、同意前の3カ月以内の制御されていない狭心症、臨床的に意義のある不整脈(心室性頻脈、心室細動、または心室性不整脈)の病歴、480msecを超えるQTc延長、他の臨床的に意義のある心疾患(すなわち、心筋症、New York Heart Association[NYHA]機能分類III〜IVを有する鬱血性心疾患、心膜炎、有意な心外膜液)の病歴。
毎日の酸素補給のための心血管疾患に関連する要件。
インフォームドコンセントの前年における脳炎、髄膜炎、または制御されていない発作の確認された病歴。
ヒト免疫不全ウイルス(HIV)または既知の後天性免疫不全症候群(AIDS)に関する陽性の血液スクリーニング。
A型肝炎抗体(HepA IgM)(注記:解消されたA型肝炎ウイルス感染の病歴は除外基準ではない)について陽性の試験、B型肝炎表面抗原(HBsAg)および/またはB型肝炎コア抗原について陽性の試験、定性的C型肝炎ウイルス量(PCRによる)について陽性の試験により証明される任意の慢性肝炎の病歴。
試験薬物療法の開始前の7日以内に全身性抗細菌、抗ウイルス、または抗真菌療法を必要とする活動性感染の証拠。
他のいずれかの有意な急性または慢性の医学的疾病。
静脈穿刺を受けることができない、および/または静脈アクセスを許容できない対象。
他のいずれかの正当な医学的、精神医学的、および/または社会的理由。
以下の手順または薬のいずれか:
インフォームドコンセントの時点より前の2週間以内:免疫抑制用量(7.5mg/日以上のプレドニゾンもしくは等価物)の全身もしくは局所コルチコステロイド、CNSにおける緩和的放射線療法およびガンマナイフ放射線手術、または医学的薬草調製物。
試験薬物投与前の4週間以内:任意の被験薬またはプラセボ、任意の抗がん療法(化学療法、生物製剤、治療用ワクチン、放射線療法、もしくはホルモン処置)、生ウイルスを含有する非腫瘍ワクチン、アレルゲン減感作療法、増殖因子、例えば、顆粒球−コロニー刺激因子(G−CSF)、顆粒球マクロファージ−コロニー刺激因子(GM−CSF)、エリスロポエチン、大手術、またはビスホスホネート(biphosphonate)。
試験薬物投与前の10週間以内:核因子カッパ−Bリガンドの受容体活性化因子(RANK−L)阻害剤。
抗PD−1抗体療法もしくは抗PD−L1抗体療法、または他のモノクローナル抗体もしくは関連化合物またはそれらの成分のいずれかに対するアレルギーの病歴(例えば、ポリソルベート80と共に製剤化された薬物に対する重篤な過敏反応の病歴)。
囚人または不本意に投獄される対象。
精神医学的または身体的(例えば、感染症)疾病の処置のために強制的に拘留される対象。
制限ならびに禁止された活動および処置を順守できないこと。
BMS−986016またはBMS−936558の対象内用量漸増はこの試験では容認されない。MTDを超えるとその後見なされる用量レベルで処置される対象を可能な限り除いて、BMS−986016またはBMS−936558の対象内用量減少は容認されない。

− 等級1以上の肺臓炎

− 等級2以上の下痢または大腸炎
− 等級2以上の神経有害事象
・治験責任医師の判断において、試験薬物の投与を遅延させることを正当化する有害事象、検査所見異常、または合併症。
試験中および最後の処置後135日にわたって連続的に有害事象を評価する。有害事象を、NCI CTCAEバージョン4.0に従って評価する。有害事象を、Medical Dictionary for Regulatory Activities(MedDRA)の最新バージョンを用いて暗号化し、潜在的な有意性および重要性について再調査する。
以下の表4に列挙される対象の腫瘍型に応じて、様々な血清学的腫瘍マーカー、遺伝子突然変異状態、およびさらなる分析が必要である。血清学的腫瘍マーカーを除いて、以前の試験からの検査値データが利用可能である場合、評価を実施しない。
効能を、パートAおよびB(用量漸増)、ならびにパートC(コホート拡大)において評価する。各評価の時点での腫瘍測定および腫瘍応答の変化を決定する。スクリーニング期間のベースライン評価には、個々の対象の腫瘍型および/または病歴により示されるように胸部、腹部、および骨盤、ならびに他の解剖学的領域のCTまたはMRIスキャンが必要である。その後の時点においては、個々の対象の腫瘍型および/または病歴に基づくベースラインでスキャンされた胸部、腹部、および骨盤、ならびに他の解剖学的領域のスキャンが必要である。それ以外の場合、脳のスキャンが、臨床的に適応となるならば必要である。
BMS−986016薬物動態および抗薬物抗体(ADA)評価のための血清試料を、全ての対象について採取する。BMS−936558薬物動態およびADA評価のための血清試料を、パートBおよびCに登録された全ての対象について再採取する。血清試料を、検証されたイムノアッセイによりBMS−986016およびBMS−936558について分析する。さらに、選択された血清試料を、総BMS−986016および/またはBMS−936558を測定する探索的直交法(例えば、液体クロマトグラフィー[LC]−質量分析[MS]/MS)により分析する。
単独で、またはBMS−936558と組み合わせて投与されるBMS−986016の薬力学を、用量漸増(パートAおよびB)中の各用量レベルに登録された最初の3人の対象ならびに試験のコホート拡大(パートC)期にメラノーマおよび頭頸部がんを有する対象における末梢血および腫瘍組織中のバイオマーカーを定量することにより評価する。薬理学的評価の詳細なスケジュールを、以下の表5〜6に示す。試験のパートA、BおよびCにおける対象のための腫瘍組織要件に関する詳細を以下の表7に示す。
ケモカイン、サイトカイン、および腫瘍関連可溶性タンパク質の処置前および処置中の血清レベルを、限定されるものではないが、ELISAまたは多重アッセイを含む技術によって評価する。分析物としては、炎症のマーカー、免疫活性化、宿主腫瘍増殖因子、および腫瘍由来タンパク質が挙げられる。
BMS−986016およびBMS−936558を用いる処置は、腫瘍関連抗原に対する新規抗体の産生、または存在する抗体の増加をもたらし得る。8000を超えるタンパク質のパネルに対する抗体の評価を、多重アッセイおよびELISAにおける処置前および処置中の血清を用いて実施する。これらのデータを用いて、抗腫瘍抗体が臨床応答および安全性パラメータと関連するかどうかを探索し、ならびに薬物投与の薬力学に関する情報を与える。
末梢血単核細胞(PBMC)を用いて、多色フローサイトメトリーにより骨髄細胞およびリンパ系細胞の活性化および調節状態を特性評価および定量化する。免疫表現型決定を特徴とする細胞のサブセットとしては、ナイーブな、活性化された、および消耗したエフェクターおよび記憶T細胞集団、調節性T細胞、ならびに骨髄由来抑制細胞が挙げられる。
BMS−986016およびBMS−936558がT細胞の活性化および機能を回復させるかどうかを評価するために、PBMCを単離し、凍結保存する。限定されるものではないが、IFN−γおよびグランザイムBなどのエフェクターT細胞の機能状態を、フローサイトメトリー染色により評価する。
BMS−986016±BMS−936558に対する応答と関連する遺伝子の発現レベルを、全血および腫瘍試料中でのマイクロアレイおよび/または定量的逆転写ポリメラーゼ連鎖反応(RT−PCR)分析により定量化する。分析としては、必ずしも限定されるものではないが、BMS−986016に刺激されるエフェクター機能(パーフォリン、グランザイムB、およびIFN−γ)をコードする遺伝子ならびにT細胞共刺激受容体(PD−1、PD−L1、およびCTLA−4)をコードする遺伝子が挙げられる。
循環血液中の無細胞DNAの存在は、十分に立証された現象である。DNAの断片は細胞増殖または細胞死の間に分裂中の細胞から血流中に流れる。がんを有する患者においては、このDNAの画分は腫瘍由来であり、循環腫瘍DNA(ctDNA)と呼ばれる。小さいにも拘らず、平均180〜200bpのDNAの断片および特定のゲノム領域を、PCRを用いて増幅することができる。さらに、いくつかの研究により、親腫瘍からの突然変異と正確に一致するctDNA中の突然変異が検出された。メラノーマまたは頭頸部がんにおける既知のドライバー突然変異(driver mutation)を有する患者からの組織および血漿を用いて、BEAMing技術を用いて循環中の突然変異の頻度を計数する。
BMS−986016の安全性および効能と関連する潜在的な多型性を同定するために、単一ヌクレオチド多型(SNP)について選択された遺伝子を評価する。対象の遺伝子としては、限定されるものではないが、PD−1、PD−L1、MHCクラスII、LAG−3、およびCTLA−4が挙げられる。
腫瘍組織を、プロトコールの用量漸増部分において全ての対象から採取する。免疫組織化学分析を用いて免疫浸潤の数および組成を評価して、BMS−986016およびBMS−936558への曝露の前および潜在的には後のFFPE腫瘍組織内に存在する免疫細胞サブセットを定義する。これらのIHC分析は、必ずしも限定されるものではないが、以下のマーカー:CD4、CD8、LAG−3、MHCII、PD−1、PD−L1、およびPD−L2を含む。遺伝子発現とIHC発現との相関を、有益であると見なされる場合に実施されるアッセイ間で作製する。
対になった処置前および処置中の腫瘍生検は、パートC(コホート拡大)に登録されるメラノーマまたは頭頸部がんを有する全対象にとって必須である。十分な対になった処置前および処置中の生検が採取されない対象を置き換えることができる。
・TILおよび腫瘍抗原の特性評価。免疫組織化学分析を用いて免疫浸潤の数および組成を評価して、BMS−986016およびニボルマブへの曝露の前および後のFFPE腫瘍組織内に存在する免疫細胞サブセットを定義する。これらのIHC分析は、必ずしも限定されるものではないが、以下のマーカー:CD4、CD8、LAG−3、MHCII、PD−1、PD−L1、およびPD−L2を含む。遺伝子発現とIHC発現との相関を、有益であると見なされた場合に実施されるアッセイ間で作製する。

・T細胞レパートリーの特性評価。DNA配列決定を処置前および処置後のFFPE腫瘍組織上で実施して、T細胞レパートリーの組成を評価する。低いT細胞受容体多様性は、転移性乳がん患者における全生存の予後不良因子であり得る。現在、BMS−936558およびBMS−986016の主な機構がT細胞抗腫瘍免疫の機能的回復であると仮定されることを考えれば、免疫療法に対する応答の予測因子としてのT細胞受容体多様性はよく分かっていない。したがって、ベースラインで、および処置中の末梢中、および腫瘍内のT細胞コンパートメントの多様性の特性評価を、T細胞受容体次世代DNAシーケンシングにより実施する。また、T細胞レパートリー分析を、末梢血から単離されたDNAから実施して、処置前および処置後の腫瘍および末梢T細胞レパートリーの状態を比較する。
・遺伝子発現プロファイリング。RNAlaterまたは同様の試薬中で採取される腫瘍生検を、Affymetrix遺伝子アレイ技術および/または定量的リアルタイムポリメラーゼ連鎖反応(qPCR)によりmRNA遺伝子発現について検査して、選択された免疫関連遺伝子の発現を検出する。
・インサイチュサイトカインおよび負の調節因子の発現。腫瘍生検を、CD3、IFN−γ、LAG−3、およびPD−1を含むRNAについて定量的に評価する。
時点毎に採取された血清試料を、検証された免疫原性アッセイにより分析する。選択された血清試料を、抗BMS−986016または抗BMS−936558を測定する探索的直交法により分析する。任意の直交法から生成される潜在的な結果は技術探索目的にとって有益であると意図され、再報告しない。
有害事象(AE)は、治験(医)薬を投与され、この処置との因果関係を必ずしも有さない臨床被験者における任意の新しい有害な医学的出来事または元々存在する医学的状態の悪化と定義される。したがって、AEは、治験薬と関連すると考えられるかどうかに関係なく、治験薬の使用と時間的に関連する好ましくない、および意図されないあらゆる兆候(異常な検査所見など)、症状、または疾患である。
関連:試験薬物投与とAEとの合理的因果関係が存在する。

重篤な有害事象(SAE)は、いずれかの用量で、
・死亡をもたらす
・生命を脅かすものである(対象が事象の時点で死亡のリスクがあった事象と定義される;それは、それがより重篤であった場合に死亡を引き起こしたと仮定される事象を指すものではない)
・入院を必要とするか、または既に始まっていた入院の延長を引き起こす
・持続的または有意な身体障害/無能性をもたらす
・先天異常/出生異常である
・重要な医学的事象(すぐに生命を脅かすものではないか、または死亡もしくは入院をもたらすものではないが、適切な医学的および科学的判断に基づいて、対象を危険にさらすか、または上記の定義に列挙された他の重篤な転帰の1つを防止するために介入(例えば、医学的、外科的)を必要とし得る医学的事象と定義される)であるあらゆる有害な医学的出来事である。そのような事象の例としては、限定されるものではないが、アレルギー性気管支けいれんのための救急処置室または家庭での集中的処置;入院をもたらさない血液疾患またはけいれんが挙げられる。潜在的な薬物誘導性肝障害(DILI)も重要な医学的事象と考えられる。
− 入院には至らない(重要な医学的事象または生命を脅かす事象と考えられない限り)、24時間未満の救急処置室または他の医療部門への訪問。
− 同意に署名する前に計画された待機手術。
− 計画された内科的/外科的手順のためのプロトコールによる入院。
− 健康状態のベースライン/傾向のための入院を要する日常的な健康評価(例えば、日常的な結腸内視鏡検査)。
− 不健康を改善するため以外の、および試験への登録の前に計画された内科的/外科的手順。これらの事例では適切な文書化が必要である。
− 健康状態に影響せず、内科的/外科的介入を必要としない別の生活環境(例えば、住居がない、経済的不十分性、介護者の休止、家族環境、管理上の理由)のために生じた入院。
非重篤有害事象は、重篤と分類されないAEである。非重篤AE情報の収集は、試験薬物の開始時に始まり、投薬の中止後135日間続く。非重篤AEを、解消もしくは安定化まで追跡するか、またはそれらが重篤になる場合、SAEとして報告する。試験薬物の中断もしくは中止を引き起こす非重篤AEについて、および必要に応じて試験処置の終わりに存在するものについても追跡が必要である。全ての同定された非重篤AEを記録し、CRF(紙または電子)の非重篤AEの頁に記述する。
標本サイズ決定
用量漸増(パートAおよびB):各用量での標本サイズは観察される毒性に依存し、正確に決定することはできない。パートAおよびパートBは、各コホート中に3〜9人の対象を有する。

効能を、irRECISTおよびRECISTv1.1について以下に記載される評価項目を用いて副次的目的として評価する。患者管理のために、臨床決定作製はRECISTに基づく。統計分析および報告は両基準に基づく。
・最良総合効果(BOR)は、RECISTv1.1またはirRECIST基準に基づいて、確認のための要件を考慮に入れて試験処置の開始から最後のプロトコール特異的腫瘍評価(例えば、30日の追跡訪問)まで記録される最良応答を意味する。BOR評価に含まれるCRまたはPRの決定は、応答に関する基準を満たす連続2回の(確認的)評価により確認され、応答に関する基準が最初に満たされた後、少なくとも4週間実施される。
・客観的奏功率(ORR)は、BORが対象の集団中の対象の総数で割ったCRまたはPRのいずれかである対象の総数と定義される。
・CRまたはPRのBORを有する対象についてのみ計算される奏功期間(DOR)は、どちらが最初に生じても、最初の応答の日付と、基準(RECISTv1.1もしくはirRECIST)に基づく客観的に文書化された疾患進行または死亡のその後の日付との間の日数と定義される。依然として生きている、進行していない、またはその後の療法を受けていない対象については、奏功期間は最後のプロトコール特異的腫瘍評価の日付で打ち切られる。その後の療法を受ける対象はその後の療法の開始時で打ち切られる。
・無進行生存(PFS)は、依然として無進行であり、生存する対象の確率と定義される。この確率は、試験薬物の初回投与と、進行性疾患(RECISTもしくはirRECISTにより定義される)または死亡との間の日数に基づいて計算される。依然として生きている、進行していない対象については、PFSは最後のプロトコール特異的腫瘍評価の日付で打ち切られる。
試料レベルで、個々の試料はADA陽性またはADA陰性と特徴付けられる。対象は、処置の開始前の最後の試料がADA陽性である場合、ベースラインで陽性試料を有すると考えられる。例えば、ベースライン時にADA陰性である対象からのベースライン後の試料は、ADAが検出される場合、ADA陽性であると考えられる。ベースライン時にADA陽性である対象からのベースライン後の試料は、関連する力価の増加がある場合、ADA陽性であると考えられる(関連すると考えられる力価の増加の規模は薬物およびアッセイによって変化してもよく、統計分析計画において示される)。対象レベルで、関連するADA評価項目は、

パートAおよびパートBにおいて、QTcを、追跡訪問1の時ならびにサイクル1およびサイクル3の1日目(投与前および投与の4時間後の時点)に中央の読取り者によって評価される。これらの評価を用いて、QTcに対する、単独で、およびBMS−936558と組み合わせて投与されるBMS−986016の効果を評価する副次的目的に取り組む。治験責任医師によって地方において評価されるECGも、各サイクルの開始時に取る。
