Claims (16)
- ヒトα４β７インテグリンに対して結合特異性を有するヒト化抗体を含み、下記投与レジメン：
ａ．静脈内注入による前記ヒト化抗体３００ｍｇの初期用量；
ｂ．前記初期用量の約２週間後の静脈内注入による前記ヒト化抗体３００ｍｇの第２後続用量；
ｃ．前記初期用量の約６週間後の静脈内注入による前記ヒト化抗体３００ｍｇの第３後続用量；
ｄ．前記初期用量の約１４週間後の静脈内注入による前記ヒト化抗体３００ｍｇの第４後続用量；
ｅ．必要に応じて前記ヒト化抗体の前記第４後続用量の後、８週間毎の静脈内注入による前記ヒト化抗体３００ｍｇの第５及びそれに続く後続用量
に従って前記ヒト化抗体を投与するためのものであり、
治療対象のヒト患者は、前記抗体の投与前に、排液線が外科的に設置されており、
前記投与レジメンにより、１つまたは複数の瘻孔の治癒が誘導され；
前記ヒト化抗体は、非ヒト起源の抗原結合領域と、ヒト起源の抗体の少なくとも一部とを含み、前記ヒト化抗体は、α４β７複合体に対して結合特異性を有し、前記抗原結合領域は、下記ＣＤＲ：
軽鎖：ＣＤＲ１ 配列番号７
ＣＤＲ２ 配列番号８及び
ＣＤＲ３ 配列番号９；ならびに
重鎖：ＣＤＲ１ 配列番号４
ＣＤＲ２ 配列番号５及び
ＣＤＲ３ 配列番号６
を含む、瘻孔を伴うクローン病の治療剤。 - 前記投与レジメンが、前記初期用量の１０週間後、静脈内注入による前記ヒト化抗体３００ｍｇのさらなる後続用量のステップをさらに含む、請求項１に記載の治療剤。
- 前記１つまたは複数の瘻孔が、１つ以上の排液性瘻孔および／または１つ以上の肛門周囲瘻孔である、請求項１または２に記載の治療剤。
- 前記１つまたは複数の瘻孔の治癒が、３０週目に、１つまたは複数の瘻孔の治癒を磁気共鳴画像法（ＭＲＩ）により評価することによって測定される、請求項１または２に記載の治療剤。
- 少なくとも５０％の排液性瘻孔を閉じさせる、請求項３に記載の治療剤。
- 前記患者が、免疫調節薬、腫瘍壊死因子α拮抗薬またはその組み合わせの少なくとも１つに対して十分に反応しなかったか、またはそれに対して反応しなかったか、またはそれによる治療に耐性でなかった、請求項１〜５のいずれか１項に記載の治療剤。
- 前記クローン病が、中等症から重症の活動期のクローン病である、請求項１〜５のいずれか１項に記載の治療剤。
- 前記ヒト化抗体が、約３０分かけて患者に投与するためのものである、請求項１〜５のいずれか１項に記載の治療剤。
- 前記ヒト化抗体が、凍結乾燥製剤から再構成されたものである、請求項１〜５のいずれか１項に記載の治療剤。
- 前記患者が、コルチコステロイドに対して十分に反応しなかったか、またはコルチコステロイドに対して反応しなかった、請求項１〜５のいずれか１項に記載の治療剤。
- 前記免疫調節薬が、アザチオプリン、６−メルカプトプリン、またはメトトレキサートである、請求項６に記載の治療剤。
- 前記治療が、前記患者のＰＤＡＩスコアまたはＣＤＡＩスコアを用いて評価される、請求項１〜５のいずれか１項に記載の治療剤。
- 前記患者のＰＤＡＩスコアが、ベースラインから少なくとも３点減少する、請求項１２に記載の治療剤。
- 前記ヒト化抗体が、配列番号１のアミノ酸２０〜４７０を含む重鎖及び配列番号２のアミノ酸２０〜２３８を含む軽鎖を有する、請求項１〜１３のいずれか１項に記載の治療剤。
- 前記ヒト化抗体がベドリズマブである、請求項１〜１４のいずれか１項に記載の治療剤。
- 瘻孔を伴うクローン病にかかっているヒト患者の治療のための医薬の製造におけるヒトα４β７インテグリンに対して結合特異性を有するヒト化抗体の使用であって、前記治療は、瘻孔を伴うクローン病にかかっている患者に、前記医薬を投与する工程を含み、前記医薬は、下記投与レジメン：
ａ．静脈内注入による前記ヒト化抗体３００ｍｇの初期用量；
ｂ．前記初期用量の約２週間後の静脈内注入による前記ヒト化抗体３００ｍｇの第２後続用量；
ｃ．前記初期用量の約６週間後の静脈内注入による前記ヒト化抗体３００ｍｇの第３後続用量；
ｄ．前記初期用量の約１４週間後の静脈内注入による前記ヒト化抗体３００ｍｇの第４後続用量；
ｅ．必要に応じて前記ヒト化抗体の第４後続用量の後、８週間毎の静脈内注入による前記ヒト化抗体３００ｍｇの第５及びそれに続く後続用量
に従って投与するためのものであり、
