Abstract
Description
そうであるため、容器構成は、一般に所定の臨床医のサイトについて典型的なスケジュール変動のみが考慮される。極端なスケジュール変更によって、注文済みの多用量容器が予定患者スケジュールに送達するには不適切なものとなることがでてくる。 臨床医は、実際に不足することになるときには、より多くの用量を注文するなどの修正措置をとる必要がある。用量を注文する回転時間は長いため、臨床医が、できるだけ早い時期に多用量容器に不足の恐れがあることに気づくことが不可避である。したがって、多用量容器使用を監視し、可能性のある不足をできる限り早い時点で予測するためのシステムおよび方法のさらなる必要性が存在する。
こうした任意の周辺出力装置が情報およびデータをオペレータに供給するために使用されることが想定される。
このために、Tコネクター205、逆止め弁214、および逆止め弁215(これ以降、弁V1)をポート「c」および「b」を接続する状態にし、一方で、Tコネクター222、ピンチ弁170、およびピンチ弁172(これ以降、弁V2)は、「d」および「e」の位置に配置される。ポンプ22は、生理食塩水を点Bまでフラッシュする(図5を参照)。次に、チュービング部分210を、生理食塩水を含む容器に挿入する。弁V1は、ポート「a」および「b」が接続する状態にされ、その一方で、弁V2は、なおも「d」および「e」を接続させている。ここで、ポンプ180は、チュービングが完全に生理食塩水で充填されるまで生理食塩水を点A(図5を参照)から弁V2を越える目標地点までフラッシュすると、空気はシステムから完全にパージされる。次に、チュービング部分210が、放射性医薬品を含む容器902に挿入される。弁V1が、ポート「a」および「b」を接続する状態にし、その一方で、弁V2が、ポート「d」および「f」を接続する。
ポンプ180が作動し、容器902内の放射性医薬品を入口点Aから、弁V1の点Bを通過し、第三のチュービング部分216内のどこかの点Cまで送り出す。第三のチュービング部分216にある点Bと点Cの間の放射性医薬品の容積は、正確にわかっている必要はなく、AからBまでのチュービングの部分を、放射性医薬品で完全に充填させることと、BおよびCの間の容積内の放射線量が、希望する最終放射線量Arを上回らないことが確保されれば十分である。手順911の終了時の状態は、図5に図示したとおりで、ここで、点Bと点Cの間の放射性医薬品の容積は、参照番号800で示されている。
コイル部分444内での連続した放射線量測定が分散していることは、安全性を向上させる別の特徴である。手順932では、コイル部分444の放射線量は、事実上予め分かっており、測定M3は、単に正しい量の放射線量がコイル部分444内に存在することを確認する役目をする。期待される結果と実際の測定との間に有意な不一致が検出された場合、動作は直ちに停止され、およびアラームが鳴る。
検定の時間は一般に、通常の送達スケジュールをもとに処方され、臨床医により定義できる。決定される必要がある不明の要因は、検定の容積および検定の放射線量である。システムにより、これらの要因が決定されると、使用可能な全ての解が、図13に示すとおり、グラフ1314上にブロック1312としてプロットされる。
こうしてオペレータは、流体送達システム10の操作のための様々なタスクを直感的に理解し実行することができる。
いったんオペレータがスケジュールに満足すると、処理ボタン1123を押せば、メインの操作スクリーン1101に戻り、図22に示すとおり、スケジュールペイン1136にインポートしたスケジュールが自動的に表示される。動作中に何らかの理由で、システムが現在の多用量容器構成が予定された患者に予定された放射線量を予定された時間に供給できないと判断した場合、システムは、オペレータに対して情報ペイン1142に警告を提示し、図23に示すとおりスケジュールウィンドウ1134の予定された注入処置1144を強調表示する。システムコントローラ5は、スケジュールが容認できるかたちで患者に注入が可能となるように解決策を提示するが、これには、オペレータに患者が利用できる放射線量レベル1146の範囲を提示するなどにより、注入処置の時間の調節か、または予定された用量の放射線量の調節かをすることが含まれうる。たとえば、オペレータが、放射線量レベル925MBqを持つ放射性医薬品を必要とする16:00の注入処置のスケジュールを試みたとする。現在の多用量容器構成が、こうした注入処置に対応できない場合、予定された注入処置1144が強調表示され、放射線量レベルの範囲1146(18〜430MBqなど)がオペレータに提示され、これにより、オペレータは、16:00に患者をスケジュールできるようになる。この範囲は、その患者について達成可能な最小用量放射線量および最大用量放射線量を表す。この放射線量レベルがオペレータにとって容認できる場合、オペレータは、16:00の注入処置を、放射線量レベルが18〜430MBqの間の値になるよう調節できる。このレベルが容認できない場合、オペレータは、処置を取消すか、または注入時間を調節する必要がある。

付記項1の方法であって、さらに
複数の放射性医薬品注入処置中に前記多用量容器構成を監視する手順と
複数の放射性医薬品注入処置のうち少なくとも一つが適切に完了されない恐れがあるというリスクがあるかどうかを決定する手順と
判断されたリスクに対応してアラートを出す手順とを含む付記項1の方法。

前記多用量容器構成の監視が、
残存している放射線量の推定をするための前記多用量容器構成の残存している放射線量および容積を決定する手順と、
放射性同位元素の崩壊に基づき残存している放射線量の推定を調節する手順とを含む付記項5の方法。

多用量容器較正の残存放射線量の決定が、電離箱、CZT結晶検波器、Geiger−Mullerカウンタ、またはシンチレーションカウンタのうちどれか一つにより実施される付記項6の方法。

前記ポンピング機構に動作できるよう結合され、複数の放射性医薬品注入処置のスケジュールを受信し、前記ポンピング機構を制御して前記スケジュールに従い前記複数の放射性医薬品注入処置を実施するように構成された制御ユニットと、
スケジュールを表示するために前記制御ユニットに動作可能なように結合されたディスプレーユニットとを備えた装置。

付記項21の放射性医薬品流体送達システムであって、前記制御ユニットがさらに、
複数の放射性医薬品注入処置中に前記多用量容器構成を監視するよう手順と、

- 放射性医薬品使用の計画および監視をする方法であって、
複数の放射性医薬品注入処置のための患者スケジュールを受信する手順と
患者スケジュールに基づき、複数の放射性医薬品注入処置中に使用するための多用量容器構成を決定する手順と
患者スケジュールを放射性医薬品流体送達システムに転送する手順と
多用量容器構成を放射性医薬品流体送達システムに供給する手順と
患者スケジュールに基づき、前記複数の放射性医薬品の注入処置を実施する手順と
を含む方法。
