Abstract
Description
(a)発明の分野
本発明は、プロテーゼ水晶体嚢、およびその挿入方法に関する。
白内障手術は、米国において、最も成功裏に、かつ最も頻繁に行われる外科処置の一つである。毎年、何百万人もの人々が、この処置のおかげで、自分の視覚機能の劇的な改善を遂げている。退職する年齢に近づく米国人口の割合の増加に伴って、今後20年間の白内障手術に対する需要は、年間で330万人から600万人超へと、2倍になることが予測されている。この需要の増加により、より多くの眼科医が、白内障手術を実行するために養成されること、ならびに、養成された各眼科医が毎年より多くの白内障手術を行うことが必要となるであろう。
本発明は、プロテーゼ水晶体嚢、およびその挿入方法に関する。プロテーゼ水晶体嚢は、天然の水晶体嚢の体積を維持するのを助け、これにより、IOLの有効レンズ位置を安定させ、かつ白内障手術で屈折力成果を改善することによって、前述した問題に取り組むものである。プロテーゼ水晶体嚢は、統合屈折面をさらに提供し、IOLの挿入前に有効レンズ位置を実験的に判定する手段をもたらす。本明細書中、本発明のプロテーゼ水晶体嚢は、完璧なプロテーゼ水晶体嚢(Perfect Prosthetic Lenticular Capsule)すなわち「PPL−C」と呼ぶ。
図1〜図3を参照すると、本発明のPPL−C10は、人間の天然の水晶体のサイズ、形状、および体積に近似して、図示されている。その寸法は、可変性であり、医師は、手術を受けている目12の水晶体に最も厳密に適合するインプラントを注文することができる。人間の水晶体は、厚さが、約3.5mmから5.5mmまでさまざまである。天然の水晶体は、より遠視の目では厚くなり、かつ、より近視の目では薄くなる傾向がある。また、水晶体は、時間と共に厚くなり、加齢が、概して、より厚い水晶体に関連している。人間の水晶体の直径は、約9mmである。したがって、一実施形態では、PPL−C10は、厚さが約4.5mmで、直径が9mmの、実質的に円盤状の形状である。明瞭化のために、PPL−C10の厚さは、視軸15に沿った、PPL−C10の前面14と後面16との間の距離である。前面14は、直径が約6mmの円形開口部18を含み、例えば1mmのフランジ20などの外側輪郭が、開口部18を囲み、かつ開口部18から半径方向に延びている。フランジ20は、毛様溝22内に延び、かつ毛様溝22内部に嵌まることにより、PPL−C10の安定化およびセンタリングをさらに補助する。PPL−C10のこのサイズは、フェムト秒レーザーにより作られる嚢切開部(capsulorhexis)内部に正確に嵌まるように機能する。

PreRx=術前屈折力(PPL−Cが設置された後の術中屈折力も表わし得る)
DPostRx=所望の術後屈折力。
有効レンズ位置(ELPo)は、角膜の二次主平面から、薄いIOL等価物の主平面までの距離である。角膜の角膜曲率測定度数(Kk)は、以下のとおり、角膜の正味の屈折力(Ko)に変換される:Ko=Kk×0.98765431。例えばKkが44.50Dである場合、Ko=44.50D×0.98765431=43.95Dとなる。よって、角膜の正味の屈折力(Ko)は、43.95Dである。

前記プロテーゼ水晶体嚢は、眼内レンズを収容するように構成される、プロテーゼ水晶体嚢。
(3) 実施態様1または2に記載のプロテーゼ水晶体嚢において、
前記プロテーゼ水晶体嚢は、生物学的に適合性がある変形可能なプロテーゼ水晶体嚢であり、好ましくは、実質的に円盤状の形状である、プロテーゼ水晶体嚢。
(4) 実施態様1〜3のいずれかに記載のプロテーゼ水晶体嚢において、
前記プロテーゼ水晶体嚢を前記目の内部の特定の位置に機械的に保持するように構成された、外側輪郭、好ましくは毛様溝内に延びるフランジ、をさらに含む、プロテーゼ水晶体嚢。
(5) 実施態様1〜4のいずれかに記載のプロテーゼ水晶体嚢において、
前記屈折面は、以下の光学的およびデザイン品質:凹状形態、凸状形態、球状形態、非球面形態、波面形態、多焦点回折形態、多焦点屈折形態、多焦点帯状形態、調節性形態、UVフィルタリング形態、回折性色収差軽減形態、および乱視矯正トーリック形態、のうち少なくとも1つを含む、プロテーゼ水晶体嚢。
前記屈折面は、約+30ジオプター〜−30ジオプターの屈折力を有し、好ましくは、負の屈折力を有する、プロテーゼ水晶体嚢。
(7) 実施態様5に記載のプロテーゼ水晶体嚢において、
前記屈折面は、約0ジオプターの屈折力を有する、プロテーゼ水晶体嚢。
(8) 実施態様5に記載のプロテーゼ水晶体嚢において、
前記屈折面は、約+1ジオプターの屈折力を有する、プロテーゼ水晶体嚢。
(9) 目に挿入されるプロテーゼ水晶体嚢において、
後面、開口部を含む前面、および前記開口部から半径方向に延びるフランジを含む、概ね円盤状の形状を有する、プロテーゼ水晶体嚢。
(10) 目の前眼部と後眼部とを分離するためのプロテーゼバリアにおいて、
後面、開口部を含む前面、および前記開口部から半径方向に延びるフランジを含む、概ね円盤状の形状を有する、プロテーゼバリア。
a)プロテーゼ水晶体嚢を、患者の目の中に、好ましくは前記目内部の天然の水晶体嚢内部に、挿入する工程であって、前記プロテーゼ水晶体嚢は、開口部を有する前面、および後面を含み、前記後面の少なくとも一部が、屈折面である、工程と、
b)前記目内部における前記屈折面の位置を判定する工程と、
c)前記屈折面の前記位置に基づいて眼内レンズを選択する工程と、
d)前記眼内レンズを前記プロテーゼ水晶体嚢に挿入する工程であって、前記プロテーゼ水晶体嚢は、オプションとして、薬剤を含む、工程と、
を含む、方法。
(12) 実施態様11に記載の方法において、
工程a)に先立って、前記天然の水晶体嚢内部の場所から天然の水晶体が除去され、
前記天然の水晶体は、向かい合う前面および後面を含み、
前記プロテーゼ水晶体嚢は、前記天然の水晶体嚢に挿入されると前記屈折面が前記天然の水晶体の前記後面によりこれまで保持されていた前記場所と実質的に同一の位置にくるように、サイズ決めされている、方法。
(13) 実施態様11または12に記載の方法において、
工程a)に先立って、前記目に嚢切開部が形成され、
前記プロテーゼ水晶体嚢は、前記嚢切開部を通して前記目に挿入される、方法。
(14) 実施態様11〜13のいずれかに記載の方法において、
前記プロテーゼ水晶体嚢は、前記プロテーゼ水晶体嚢を、前記目の内部の特定の位置に機械的に保持するように構成された、外側輪郭を含む、方法。

- 目に挿入されるプロテーゼ水晶体嚢において、
開口部を含む前面と、

前記後面の少なくとも一部が、屈折面である、プロテーゼ水晶体嚢。 - 請求項1に記載のプロテーゼ水晶体嚢において、
前記プロテーゼ水晶体嚢は、眼内レンズを収容するように構成される、プロテーゼ水晶体嚢。 - 請求項1または2に記載のプロテーゼ水晶体嚢において、
前記プロテーゼ水晶体嚢は、生物学的に適合性がある変形可能なプロテーゼ水晶体嚢であり、好ましくは、実質的に円盤状の形状である、プロテーゼ水晶体嚢。 - 請求項1〜3のいずれか1項に記載のプロテーゼ水晶体嚢において、
前記プロテーゼ水晶体嚢を前記目の内部の特定の位置に機械的に保持するように構成された、外側輪郭、好ましくは毛様溝内に延びるフランジ、をさらに含む、プロテーゼ水晶体嚢。 - 請求項1〜4のいずれか1項に記載のプロテーゼ水晶体嚢において、
