Abstract
Description

本開示は、脈管構造において使用する最小侵襲性で、かつカテーテル送達される血管再生システムに関し、特に、鎖骨下動脈および総頚動脈の連結部の上での使用に適したシステムに関する。特に、本開示は、改良型神経学的医療用デバイスを含む、虚血性脳卒中における使用のための血管再生デバイスに関し、この改良型神経学的医療用デバイスは、繋留されるか、または再拘束可能な自己拡張型神経学的医療用デバイスである。
本発明の実施形態に従って、急性脳卒中血管再生/再疎通システムが開示され、このシステムは、改良型神経学的医療用デバイスにアクセスし、改良型神経学的医療用デバイスを脳血管系の中に配置するガイドワイヤーを有するカテーテルシステムを組み合わせて備え、このカテーテルシステムは、遠位のニチノールデバイスを有する近位のステンレス鋼プッシャを含む。
本発明の発明者らは、従来の自己拡張型血管再生デバイス送達プラットフォームを活用することによって、ポリモディック(poly−modic)システムは、塞栓に衝突し、塞栓を処理し、そして/または塞栓を横切り、半径方向に塞栓にフィルターをかけ、問題となっている塞栓を除去するか、またはオプションで塞栓を処理するように配置されるかのいずれかを行うことを反復し得ることを実現した。そのような組み合わせの療法に対して有効な現存のシステムの不足が、当業者の間で注目されている。
Claims (14)
- 急性脳卒中再疎通システムであって、該急性脳卒中再疎通システムは、
改良型神経学的医療用デバイスの脳脈管構造へのアクセスおよび脳脈管構造の中への配置に有効なガイドワイヤーを有するカテーテルベースのシステムと、
少なくとも1つの改良型神経学的医療用デバイスであって、該神経学的医療用デバイスは、近位端と遠位先端とを有する近位のステンレス鋼プッシャと、遠位のニチノールデバイスと、1つの小片のニチノールデバイスからなる群から選択された、神経学的医療用デバイスと
を組み合わせて備え、該少なくとも1つの改良型神経学的医療用デバイスは、該ガイドワイヤーの追跡可能性を改善するために該プッシャの中に組み込まれたポリマーライナーをさらに備え、それによって、該ポリマーライナーは、該ガイドワイヤーを導き、該ニチノールデバイスの中でのもつれを防ぐために該プッシャの該遠位先端を越えて延びる、急性脳卒中再疎通システム。 - 前記少なくとも1つの改良型神経学的デバイスは、前記ガイドワイヤーの追跡可能性を改善するために前記プッシャの中に組み込まれたポリマーライナーをさらに備え、それによって、該ポリマーライナーは、該ガイドワイヤーを導き、前記ニチノールデバイスの中でのもつれを防ぐために該プッシャの前記遠位先端を越えて延びる、請求項1に記載の急性脳卒中再疎通システム。

- 前記少なくとも1つの改良型神経学的医療用デバイスは、前記近位のステンレス鋼プッシャであり、近位端および遠位先端と、遠位のニチノールデバイスとを有し、前記ポリマーライナーは、該遠位のニチノールデバイスの中でもつれを防ぐように前記ガイドワイヤーを方向づけるために、該プッシャの該遠位先端を越えて該遠位のニチノールデバイスの長さよりも大きい長さまで延びる、請求項3に記載の急性脳卒中再疎通システム。
- 再捕捉可能性、柔軟性および追跡は、前記少なくとも1つの改良型神経学的医療用デバイスの複数の支柱によって可能になり、該少なくとも1つの改良型神経学的医療用デバイスは、屈曲/拡張が治療された血管を通して誘導することを可能にする、請求項4に記載の急性脳卒中再疎通システム。
- 前記少なくとも1つの改良型神経学的医療用デバイスは、均一な複数のセルを備えている、請求項5に記載の急性脳卒中再疎通システム。
- 前記少なくとも1つの改良型神経学的医療用デバイスは、可変のセルの大きさを備えている、請求項5に記載の急性脳卒中再疎通システム。
- 前記可変のセルの大きさは、該セルの中にメッシュで捕らえられた血塊の露出された表面領域に影響を与える、請求項7に記載の急性脳卒中再疎通システム。
- 画像化のために放射線不透過性をさらに含む、請求項6に記載の急性脳卒中再疎通システム。
- 前記放射線不透過性は、本質的にプラチナおよび金からなるグループから選択された少なくとも1つのマーカー材料をさらに含む、請求項9に記載の急性脳卒中再疎通システム。
- 前記複数のマーカーは、嵌合し該複数のマーカーを取り巻くように設計された予めレーザー切断された複数の開口の中に押し込められる、請求項10に記載の急性脳卒中再疎通システム。
- 改良型神経学的医療用デバイスであって、該改良型神経学的医療用デバイスは、
半径方向フィルター/血管再生の組み合わせデバイスおよびバスケットのようなアセンブリーを脳脈管構造の中の所望の位置に送達するのに有効なカテーテルシステムと、
少なくとも3つのそれぞれのモードで機能する該カテーテルシステムに取り外し可能に繋留された自己拡張型半径方向フィルター/血管再生デバイスおよびバスケットアセンブリーと
を組み合わせて備え、該半径方向フィルター/血管再生デバイスおよびバスケットのようなアセンブリーは、該カテーテルに取り付けられ、半径方向フィルター/血管再生デバイスおよびバスケットアセンブリーは、1モードについて少なくとも2つの状態、すなわち引き込まれた状態および拡張された状態を含み、
該半径方向フィルター/血管再生デバイスおよびバスケットのようなアセンブリーは、各モードにおいて拡張された状態での展開の後に、引き込まれた状態に引き込まれる、改良型神経学的医療用デバイス。 - 前記血管再生デバイスは、ニチノールおよびスプリングテンパーステンレス鋼のうちの少なくとも1つから作られる、請求項12に記載の改良型神経学的医療用デバイス。
- 薬剤が塞栓の部位まで送達されるように流体連通している注入カテーテルおよび薬剤送達システムをさらに備えている、請求項13に記載の改良型神経学的医療用デバイス。
