Abstract
Description
コラーゲン軟骨修復パッチ
コラーゲンシート22(Xenoderm-ブタタイプ1およびタイプ2コラーゲン)を、下側の層22に使用した。下側の層は、剪断応力および引張応力に耐える機械的特性を有し、約6週間で吸収可能であった。コラーゲンシート22を、型(form)に入れ、次いで、凍結乾燥および殺菌後にパッチ内で乾燥重量5〜50マイクロモルの濃度を得るようにジアセレインの溶液またはジアセレイン粉末を加えた、コラーゲン-HA懸濁物を負荷した。その結果、下側の層が骨表面に隣接して配置されることになる、二重層コラーゲンパッドが得られる。製造後、殺菌前に、パッドを、0.5〜2mmの厚さを得るように機械プレスに入れる。乾燥凍結された最終生成物中のHA濃度は、約0.1%〜2%の範囲であった。HAは、天然HA、すなわち、発酵由来の非化学修飾HAである。

Claims (12)
- 関節内病変におけるin situでの軟骨修復のための、生体適合性があり、生理的に吸収可能である、移植可能な積層状軟骨修復パッチであって、
第1の外側の細胞密閉層と、
軟骨下骨創傷部位に近接して配置されるように適合された第2の外側の細胞透過層と、
前記第1の外側の層と前記第2の外側の層との間に配置された軟骨形成マトリックスとを含んでおり、前記軟骨形成マトリックスが、自己幹細胞の拡散のためのシンクであり、かつ、前記自己幹細胞の存在下でヒアリン様軟骨の生成を促進する化学成分を含んでおり、
組み合わせると、前記修復パッチが、生理的に吸収可能な生体適合性材料で作製された可撓性のものになる、移植可能な積層状軟骨修復パッチ。 - 前記軟骨形成マトリックスが、天然ヒアルロン酸とコラーゲンとを含む、生物学的に適合性があり生理的に吸収可能な複合体である、請求項1に記載の移植可能な積層状軟骨修復パッチ。
- 前記軟骨形成マトリックスの前記コラーゲンが、非ヒト源由来のコラーゲンから本質的になる、請求項2に記載の移植可能な積層状軟骨修復パッチ。
- 前記軟骨形成マトリックスの前記天然ヒアルロン酸が、天然の非ヒトヒアルロン酸から本質的になる、請求項2に記載の移植可能な積層状軟骨修復パッチ。
- 前記軟骨形成マトリックスの前記天然ヒアルロン酸が、細菌発酵源由来の天然の非ヒトヒアルロン酸から本質的になる、請求項4に記載の移植可能な積層状軟骨修復パッチ。
- 前記軟骨形成マトリックスに軟骨再生を促進する化学組成物が埋め込まれている、請求項1に記載の移植可能な積層状軟骨修復パッチ。
- 前記軟骨形成マトリックスに、成長因子組成物、ジアセレイン組成物、レイン組成物、ポリ乳酸組成物、および天然の非哺乳類ヒアルロン酸組成物からなる、軟骨再生を促進する化学組成物群から選択される、少なくとも1つのそのような組成物が埋め込まれている、請求項1に記載の移植可能な積層状軟骨修復パッチ。
- 患者の関節内創傷部位におけるin situでの関節内軟骨修復方法であって、
請求項1に記載の移植可能な積層状軟骨修復パッチを提供するステップと、
前記創傷部位を準備するステップと、
フィブリン糊および血餅の層を前記創傷部位に配置させるステップと、
前記創傷部位のところで前記フィブリン糊および血餅の層を覆う適正な向きに前記軟骨修復パッチを適用し、前記創傷部位を閉じ、前記軟骨修復パッチに前記創傷部位でヒアリン様軟骨の成長を開始させ、前記軟骨修復パッチを時間とともに吸収させるステップとを含む方法。 - 前記創傷を準備する前記ステップが、
前記患者から血液サンプルを採取し、間葉系幹細胞の分化を引き起こす成分を含む内因性創傷治癒成分を有する自己血清分画を得るステップと、
前記創傷部位で軟骨下骨表面を微小骨折させて、局所出血により、前記微小骨折に応じて前記軟骨下骨より放出される自己間葉系幹細胞を含む新鮮な血液で前記創傷部位を潅流させるステップとを含む、請求項8に記載の関節内損傷におけるin situでの軟骨修復方法。 - 前記微小骨折ステップが、さらに、
前記軟骨下骨によって放出される前記自己間葉系幹細胞が、前記軟骨修復パッチの存在下で軟骨細胞へと分化し、前記創傷部位で細胞外ヒアリン様軟骨を生成することになる、多能性間葉系幹細胞であることを含む、請求項9に記載の関節内損傷におけるin situでの軟骨修復方法。 - 前記フィブリン糊および血餅の層を前記創傷部位に配置させる前記ステップが、さらに、前記軟骨修復パッチを適所に縫い付ける工程を含む、請求項8に記載の関節内損傷におけるin situでの軟骨修復方法。
- 前記配置させるステップが、さらに、トロンビン分画が遠心分離された自己血清により50/50の比率で置き換えられて前記創傷部位の微小骨折/剥離部位から得られる血液と混合された、フィブリン糊を含んでおり、このフィブリン糊/血餅が、次いで、前記軟骨修復パッチの下の創傷床内に置かれて、封止し、機械的に安定な構造を得る、請求項8に記載の関節内損傷におけるin situでの軟骨修復方法。
