本発明は一般的には医療機器及び方法に関する。より具体的には、本発明は血管及び肺領域又は他の体の内腔内に治療剤又はコントラスト剤を超音波を利用して移送する装置及び方法に関する。
【背景技術】

医療技術が大幅に進歩しているにもかかわらず、血管や肝の病気は、動脈の血栓症(動脈中の血塊)と同様、保健上コスト的にも治療上も種々の問題を残している。薬剤や中間機器やバイパス手術等の最近の治療方法は通常高価で常に有効というわけではなく、場合によっては更なる問題を生ずることさえある。たとえば、薬剤は有為なクロットをも溶かしうるし、中間機器は健康な組織に害を与えて致命的な出血合併症を起こしたり瘢痕を形成したり血管等の中に(再発狭窄症として知られるプロセス)重大な閉塞となりうる細胞の生長をもたらしたりする。

本発明の目的はカテーテル薬剤移送用の改良された方法及び装置を提供することにある。
本発明のさらなる目的は、超音波エネルギーを用いるカテーテル薬剤移送用の改良された方法及び装置を提供することにある。
本発明のさらなる目的は、異なる薬剤を超音波で混合して所望の人の内腔に超音波で移送する方法及び装置を提供することにある。
本発明のさらなる目的は薬剤−液状溶液とガス(たとえば生理食塩水と酸素)を超音波で混合し、混合物を所望の人の内腔に超音波で移送する方法及び装置を提供することにある。
本発明のさらなる目的は薬剤は血管内領域及び/又は内腔に移送して凝血塊を溶解する方法及び装置を提供することにある。
本発明のさらなる目的はブロックされ狭くなった血管を超音波で処置する方法及び装置を提供することにある。
【課題を解決するための手段】

本発明は治療剤又はコントラスト剤を血管及び肺その他の所望の人の内腔内に超音波で移送する装置及び方法に関する。超音波変換器チップと患者の体とを直接接触させる必要なしに、特に凝血塊を溶かすために、超音波を血管内領域、肺又は適宜の人の内腔内に適用する。

本発明のカテーテル系は、放射状表面と遠位端表面をもつ遠位チップをもつ超音波変換器をもつ。超音波変換器はカテーテル内の近位端に配され、変換器照射表面又はチップが超音波又はエネルギーを共軸的に液体を介してカテーテル内方向に向ける。長さ方向の超音波が変換器遠位端に隣接する液体内に波動を誘発する。本発明のカテーテル系は、人の内腔に移送した薬剤類の吸収を高めることを主目的としているが、超音波エネルギーを所望の部位に移送するのにも有効である。変換器照射表面又は変換器チップは超音波エネルギーをカテーテル内に照射するために、円筒状、平坦状、凹状、凸状、不規則状その他適宜の異なる形状をもちうる。

本発明のカテーテルは受器からポンプ又はシリンジで薬剤を運ぶため近位チューブをもちうる。このチューブは、照射表面の前又はうしろに位置しうる。
本発明の第1の態様では、超音波変換器又はチップがカテーテル本体の近位部分に配され、患者の体の外側に位置する。上記の近位部分から離れているカテーテルの部分は血管に挿入されるか又は内腔に取りつけられて薬剤を超音波で駆動するか超音波エネルギーを移送する。

本発明の第2の態様では、変換器がオリフィスをもたない。これは超音波エネルギーを血管又は内腔に十分に移送するのに極めて重要である。
本発明の第3の態様のカテーテル系は、カテーテルの外側にある超音波変換器とハウジング又はチップ管(node)を介して機械的に結合しているカテーテル本体をもつ。このカテーテル本体は混合され超音波で移送される異なる薬剤(1以上の酸素等の異なるガスも含む)用の2以上のチューブ入口(スリーブ)をもちうる。

本発明のカテーテル系は薬剤を局所付与することが望まれる組織で接触を避けるべき組織に対し特に有効である。また本発明方法に用いる超音波は薬剤にエネルギーを付与し、凝血塊を溶かし、狭くブロックされた血管内に薬剤を浸透させる。
【発明を実施するための最良の形態】

本発明の方法及び装置は、凝血塊、悪疫その他の狭窄性障害の溶解又は破壊及び狭くなった血管の拡張を主目的とするような内腔状態の処置、特に冠状及び血栓症の処置のための方法及び装置である。本発明の方法及び装置はまた凝血塊その他の狭窄性物質の破壊に主に応答しうる薬剤の効果を高めるのに有効である。超音波エネルギーは狭窄性物質への薬剤の浸透を刺激し、促進する。
薬剤の移送と薬剤を通しての超音波エネルギーの移送により、本発明の方法及び装置は尿道、尿管、ファロピオ管等の他の人の内腔の処置又は前立腺に関連する泌尿器系疾患(BHP)の処置、さらには性的器管、尿道等を超音波で刺激する不能症の処置にも有効である。

本発明はまた肺、脈管系、血管神経症及び腫瘍の処置用の標的又は局所薬剤移送用にも有効である。また本発明はバクテリア細胞を破壊して健康な組織細胞を刺激することによって、フィスチル、管その他の閉じた創傷の処置にも極めて有効である。

図1は超音波カテーテル薬剤移送システム2の説明図であり、これは超音波発生器4、超音波発生器4と変換器8を操作的に連結するコネクタ6、変換器8を囲むハウジング10、及び薬剤18を含有するチェンバ16をもつ近位部分14をもつカテーテル12をもつ。変換器8は放射状表面22と遠位照射表面24をもつ。チェンバ16はチューブ26を介して流体源28と流体連通しており、カテーテル遠位端34に至るカテーテル12の遠位部分32の少なくとも1の内腔30と直接流体連通している。流体源28はたとえば圧力ポンプ又はシリンジをもつ受器でありうる。

図1に示す本発明の態様では、予め選択した周波数の超音波エネルギーが移送部材として薬剤等の流体を伴ってカテーテル10を通って送られる。超音波エネルギーは薬剤18を通過してカテーテル遠位端34に至る。カテーテル12は適用部位等に応じ一般的に知られた剛性材料や可塑性材料からつくられうる。カテーテルを蛇行性の血管分布に挿入したりカテーテル遠位端34を血管やフィステル等に取りつける場合には可撓性材料でカテーテル12をつくることが望ましい。

図2に示す本発明の第2の態様では、変換器50がカテーテル56の近位部分54の近位セクション52に固定して、操作的に取りはずし可能に取りつけられている。変換器50は放射状表面60と遠位照射表面62をもつチップ58をもつ。カテーテル近位部分54は2つの流体源68、70の各々と内腔72、74を通してそれぞれ流体連通している薬剤66をもつ。流体源68、70は超音波で混合され内腔76を通ってカテーテル遠位端78に移送される2以上の流体、たとえば生理食塩水又は酸素等の液体又はガス、を提供しうる。

変換器遠位チップ154は中央オリフィス170をもつ。チェンバ164は中央オリフィス170を介して少なくとも1の流体源172と流体連結しており、オリフィス170は平坦、波状、リング状、溝状、ネジ状等の形状でありうると共に、チューブ176内に注入用内腔174をもつ。チェンバ164はまた遠位端184に至るカテーテル160の遠位部分182内の内腔と流体連通している。リング状やネジ状等の滑らかでないオリフィス表面がチェンバ164内の液体の圧力を増加する。

これらのカテーテル系は周知の材料でつくりうる。変換器とカテーテルチェンバは適宜の金属又はポリマー材料から好ましくつくられる。最も好ましくは変換器遠位チップはチタンやニチノール等の金属からつくられる。

上記したように本発明の装置は1以上の液体又はガス状物質をカテーテル遠位端に移送しうる。これらの物質の非限定的な例としては抗菌剤又は防腐剤、生理食塩水、油、水、酸素、抗凝血剤、たとえばへパリン又はクマジン、さらには液体ポリマー状薬剤や2以上のこれらの混合物がある。
遠位照射表面は一定の変調又はパルス化周波数で駆動されうる。また遠位照射表面は正弦波、方形、てい形又は三角形の波形で駆動されうる。また変換器は10kHzから10,000MHzの周波数で操作しうる。

Claims (32)
- チェンバをもつ近位部分と遠位端をもち少なくとも1の内腔が該チャンバから該遠位端に至っているカテーテル、該チェンバと流体連通している少なくとも1の流体源、及び該カテーテルチェンバ内に共軸的に及んでいる遠位端をもつ超音波変換器からなり、且つ該変換器が該チェンバ中の流体に波動を誘発して液体及び/又は超音波エネルギーをカテーテル遠位端に移送するようにしてなることを特徴とする超音波薬剤移送用カテーテル系。
- 変換器遠位端がチェンバ内に共軸的に位置する請求項1のカテーテル系。
- 波動が変換器遠位端と隣接する流体に誘発される請求項1のカテーテル系。
- 液体及び超音波エネルギーが同時にカテーテル遠位端に移送される請求項1のカテーテル系。
- 2つの流体源をもつ請求項1のカテーテル系。
- 変換器遠位端が少なくとも1の流体源及び該チェンバと流体連通しているオリフィスをもつ請求項1のカテーテル系。
- オリフィスの表面が滑らかでない請求項6のカテーテル系。
- オリフィスの表面がリング状、スロット状、波状、溝状又はネジ状である請求項7のカテーテル系。
- 少なくとも2つの流体源をもち、これら流体源からの流体をオリフィスで混合する請求項6のカテーテル系。
- 変換器が約10kHzから103MHzの周波数で操作可能である請求項1のカテーテル系。
- 変換器遠位端が放射状表面と遠位照射表面をもつ請求項1のカテーテル系。
- 放射状表面が滑らかでない請求項11のカテーテル系。
- 放射状表面がリング状、スロット状、波状、溝状又はネジ状である請求項12のカテーテル系。
- 遠位照射表面が、平坦状、円錐状、卵状、円状、半球状、四角状又は方形状である請求項11のカテーテル系。
- 流体が治療用薬剤である請求項1のカテーテル系。
- 超音波変換器がカテーテルの近位部分に接続している請求項1のカテーテル系。
- 超音波変換器の遠位端がカテーテルの近位部分に接続している請求項1のカテーテル系。
- 超音波変換器又はその遠位チップがカテーテル本体の前又は後方向に可動的であり超音波圧力及び/又は移送超音波エネルギーレベルを変えうる請求項1のカテーテル系。
- 少なくとも1の内腔をもつカテーテルを用意し、カテーテルの近位端と結合可能でそこから超音波エネルギーを放出可能な遠位照射表面をもつ変換器を用意し、カテーテルの近位端内に少なくとも1の液体を導入し、そして放出された超音波エネルギーを少なくとも1の内腔及び液体を通して体の内部領域に移送して体の内部領域を超音波エネルギーで処置することを特徴とする体の内部領域を超音波で処置する方法。
- 変換量が10kHzから10,000MHzの周波数で操作される請求項19の方法。
- 液体が抗菌剤、防腐剤、生理食塩水、油、抗凝血剤及び水からなる群から選ばれる1以上の成分からなる請求項19の方法。
- 放出された超音波エネルギーを体の内部領域に移送する工程が体内に治療効果をもたらし、この治療効果が体の内腔内の狭窄の量の低下による血流の増加、細胞生長の刺激、泌尿器系疾患の緩和及び性不能症の治療からなる群から選ばれる請求項19の方法。
- 変換器が一定のパルス比又は変調周波数によって駆動され、変換器の駆動波形が正弦波、方形、てい形及び三角形のいずれかの波形である請求項19の方法。
- さらに放出された超音波エネルギーの少なくとも1の特性を調節するためにカテーテルの近位端内で遠位照射表面を移動する工程をもつ請求項19の方法。
- さらに液体と混合して体の内部領域に移送するためにカテーテルの近位端内にガスを導入する工程を含む請求項19の方法。
- 液体が対応する流体源からカテーテルの近位端に導入され、この対応する流体源が受器又はシリンジである請求項19の方法。
- さらに遠位照射表面をカテーテルの近位端内に密封係合する工程を含む請求項19の方法。
- 少なくとも1の内腔をもつカテーテル、該カテーテルの近位端と結合して超音波エネルギーを放出するための遠位照射表面をもつ変換器、及びカテーテルの近位端内に少なくとも1の液体を導入するための手段をもち、放出された超音波エネルギーが超音波エネルギーで体の内部領域を処置するために少なくとも1の内腔と液体を介して体の内部領域に移送されることを特徴とする体の内部領域を超音波で処置するためのカテーテル系。
- 変換器が10kHzから10,000MHzの周波数で操作される請求項28のカテーテル系。
- さらに放出された超音波エネルギーの少なくとも1の特性を調節するためにカテーテルの近位端内で遠位照射表面を移動する手段をもつ請求項28のカテーテル系。
