Abstract
Description
不充分なままである。ほとんどの精神障害では、薬物治療によって全ての患者を
完治させることはできない。このことは、精神分裂病(schizophrenia)、分裂情
動性(schizoaffective)および分裂病型(schzotypal)障害、双極性障害(躁鬱病
)、一極性鬱病、痴呆症、パニック発作、不安、睡眠障害、注意、機能亢進およ
び行為(conduct)障害、自閉症、人格障害、および他のすべての精神医学の対象
となる状態について言える。例えば、鬱病では、患者の約3分の2は標準薬によ
って鬱病標準得点が50%低下する:その他の患者は反応しない。精神分裂病で
は、平均改善率は20−30%程度である(S Leucht et al, Schizophrenia Res
earch 1999;35:51-68)。ただし、個々の患者はこれよりもかなり改善する。 アルツハイマー病および他の痴呆症、パーキンソン病、多発性硬化症、脳卒中
などの発作、癲癇およびハンチントン病などの他の神経疾患についても同じこと
が言える。しかも、多くの患者は現在行われている治療に反応しないか、または
限られた程度でしか反応しない。これらのどの状態においても、現在の薬は症状
を確実に完治させることはない。そのため、新しい治療、特に新しい作用メカニ
ズムを持つ治療が強く求められている。 WO98/16216は、精神分裂病、鬱病、痴呆症の治療において、特定の
脂肪酸、すなわちエイコサペンタエン酸(EPA)とその誘導体の有用性に着目
している。EPAは、必須脂肪酸であるα−リノレン酸から一連の3つの反応(
図1)により誘導できる高度不飽和脂肪酸である。EPAは、20個の炭素原子
と5つの二重結合(すべてシス形)を含む脂肪酸である。二重結合は5、8、1
1、14及び17位にある。したがって正式な化学名はオールシス(またはオー
ルz)5、8、11、14、17−エイコサペンタエン酸(アイコサペンタエン
酸とも言う)である。常に使用される略語はEPAである。EPAは高度不飽和
脂肪酸の1つで、そのうちの主なものを図2に示す。α−リノレン酸をEPAに
変換する反応は人の体内では遅く、摂取されたα−リノレン酸の非常に僅かな部
分しかEPAに変換されない。また、EPAは海洋微生物中に見つかっており、
食物連鎖により油の多い魚や海洋哺乳類から取られた天然海産油の3〜30%を
構成する。EPAは多くの異なる化学構造に結合する。リン脂質、トリ−、ジ−
およびモノグリセリド、アミド、多数の異なるタイプのエステル、塩および他の
化合物の形で見出すことができる。いずれの場合においても、通常EPAの分子
部分が複雑な分子から分かれ、再び他の複雑な分子に結合することができる遊離
酸の形になる。 WO98/16216に記載されているように、EPAを多く含む油が精神分
裂病の治療に効果があり、近い関係にある脂肪酸であるドコサヘキサエン酸(D
HA)を多く含む油は効果がないことが偶然に発見された。DHAは人の脳内に
多量に見つかるが、EPAはごく微量しか見つからない。したがって、DHAは
効果があり、EPAは効果がないと予測されていた。ところが、実際にはその逆
であったのである。WO98/16216は、精神障害の治療のためのEPAお
よびその誘導体の使用を開示している。

Aの形態であり、5%未満、好ましくは3%未満がドコサヘキサエン酸の形態で
あることを特徴とする薬剤を提供する。このような薬剤は、末梢血管の疾患およ
び高トリグリセリド血症を除く、あらゆる疾患の治療に用いることができる。

体の10%未満、好ましくは5%未満である。 EPAは、エチル−EPA、リチウム−EPA、モノ−、ジ−またはトリグリ
セリドEPAまたはEPAの他のエステルまたは塩、またはEPAの遊離酸の形
であってもよい。また、EPAは2-置換された誘導体、または、EPAの酸化
速度を低下させるが、それ以外は精神・脳疾患に対する生物学的作用を本質的に
変えない他の誘導体の形でもよい(N. Willumsen et al, Biophysica Acta, 1998
, 1369:193-203)。 このような薬剤は、精神分裂病、分裂情動性障害または分裂病型障害;鬱病また
は躁鬱病(双極性障害);不安またはパニック障害または対人恐怖症(social ph
obia)、睡眠障害または注意力欠如、行為障害、機能亢進または人格障害;アル
ツハイマー病、血管性痴呆または他の痴呆−多発脳塞性痴呆を含む、レーヴィ体
病およびプリオン疾患に原因があるとされる疾患;パーキンソン病、または他の
運動系障害;多発性硬化症;脳卒中(stroke);癲癇;ハンチントン病またはすべ
ての他の神経変性障害を含む、精神または中枢神経系障害の治療に使用できる。
薬が、本発明の第1の視点に係る薬剤と同時に投与するために調製された、精神
・神経疾患に使用される調合物、および主として神経伝達物質の代謝またはレセ
プターに作用する薬とともに本発明の第1の視点に係る製剤を含む医薬調合物を
提供する。従来の薬は従来の投薬量で投与され、本発明の第1の視点に係るEP
Aの調合物は別に投与される。従来の薬は組み合わせ調合物中に本発明の第1の
視点のEPA製剤と組み合わせてもよいし、この2つを別々の調合物として1つ
の包みで与えてもよい。 本発明のEPAを含む製剤は、精神または中枢神経系障害の治療に効果のある
ことが知られている薬とともに、その薬の有効性を高めるため、または副作用を
軽減するために投与することができる。 本発明の第1の視点のEPA製剤と同時に投与するのに適した薬は、クロザピ
ン;クロルプロマジン、ハロペリドール、リスペリドン、オランザピン、セルチ
ンドール、ジプラジドン、ゾテピンまたはアミスルピリドを含む、定型または非
定型抗精神病薬のいずれか1つである。他は後述する。
の薬と本発明の第1の視点に係る製剤の投与により、治療または防止する方法を
提供する。 精神障害の治療には、純粋な、またはほぼ純粋なEPAおよびEPAの誘導体を
使用することが重要である。DHAや関連する脂肪酸が有効ではないだけでなく
、実際にはEPAおよびその誘導体の有効性を減じる可能性があるということは
驚くべきことである。
知られるリン脂質の中央の炭素原子は、通常はDHA、アラキドン酸(AA)、
時にはEPAなどの高度不飽和脂肪酸(HUFA)に結合している。HUFA類
は、18−26個の炭素原子と、3つまたはそれ以上の二重結合を含む脂肪酸で
ある。神経細胞が例えばドーパミンあるいはセロトニンにより活性化されたとき
は、ホスホリパーゼA2(PLA2)として知られる酵素群の活性が頻繁に高まる
。PLA2はSn2の位置からHUFAを切り離し、HUFAの遊離分子とリゾ
リン脂質(LyPL)(Sn2の位置に結合する脂肪酸がないリン脂質)(図3
)として知られる分子を与える。これらの分子はともに高活性の細胞シグナリン
グ因子であり、多数の異なるやり方で細胞の機能を変化させることができる。さ
らに、HUFAは、プロスタグランジン、ロイコトリエン、ヒドロキシ酸および
全ての神経の機能を制御する短命の分子に変えられ得る。例えば、アラキドン酸
から誘導されるこれらの分子の1つであるロイコトリエンC4は、正常な神経細
胞の成長と発生のために無条件に必要であるとみられている(E Mayatepekおよび
B Flock, Leukotriene C4-synthesis deficiency: a new inborn error of meta
bolism linked to a fatal developmental syndrome Lancet 1998;352:1514-7)
。 細胞の機能が正常であるならば、この活性化は一時的であって、遊離HUFA
およびLyPLを取り除くことによって終了されなければならない。そうでなけ
れば、LyPLは破壊作用を持ち得るため、膜の損傷が生じる可能性がある。さ
らに、HUFAは容易に酸化されて、重大な損傷を与え得る高活性の遊離基にな
る。このような膜の損傷が、アルツハイマー病および他の痴呆、パーキンソン病
、脳卒中、ハンチントン病、あらゆるタイプの虚血損傷、多発性硬化症を含む、
多くの神経変性障害に対する基礎病理学の出発点であるというコンセンサスが得
られつつある。さまざまな開始原因因子は共通の経路により損傷を引き起こす可
能性がある。リン脂質のLyPLへの分解と遊離HUFAは癲癇においても重要
であろう。 HUFAやLyPLを含む信号伝達過程は、多くの場合、2段階の反応により終
了する。まず、HUFAは脂肪酸補酵素Aリガーゼ(FACL)として知られて
いる1群の酵素により補酵素Aに結合される。これらの酵素はアシルCoA合成
酵素としても知られている。このHUFA−補酵素A誘導体は、次に、アシルC
oA:リゾリン脂質アシルトランスフェラーゼ(ACLAT)として知られる1
群の酵素によりLyPLに結合される。この過程で補酵素Aが遊離される (A Ya
mashita et al, Acyltransferases and transacylases involved in fatty acid
remodeling of phospholipids and metabolism of bioactive lipids in mamma
lian cells. J Biochem 122: 1-16, 1997)。この一連の過程は、このように神経
細胞からHUFAおよびLyPLを取り除き、信号伝達に関係するイベントを終
了させ、そのニューロンを次の刺激に対して準備させる(図3)。 現在では、3つの主要な精神病において、ホスホリパーゼ酵素群のうちの1つ
または複数、特にPLA2群の1つまたは複数の活性が高まっていることを示す
多くの証拠がある。これらの3つの疾患とは、精神分裂病、双極性障害、大鬱病
である。これらの疾患はしばしば同一の家系に見つかり、症状に重なり合う部分
がある。少なくともそれらの生化学的な根元の1部は3つの疾患全てに共通して
おり、PLA2の異常がその共通の要因をなしている可能性があると考えられて
きた。精神分裂病では、血液中のPLA2の循環レベルの増大がある。双極性障
害では、確立された主要な治療法であるリチウムがPLA2の活性を抑制するこ
とが示されている。大鬱病では、PLA2の高められた活性とともに起こる炎症
反応の活性化と結びついて、膜からn−3系列のHUFAの減少がある。しかし
、それらの疾患のそれぞれは、1つ以上の生化学的な異常を含んでいる。PLA 2 または関係するホスホリパーゼの異常は3つの疾患全てに共通するが、その他
の異常はおそらく各疾患に特有であろう。 精神分裂病では、脂肪酸サイクルに第2の異常があることが最近発見された。
これは、人の脳中の補酵素AにHUFAを結合する酵素であるFACL−4の機
能の不足である(Y Cao et al, Clonig, expression and chromomal localizati
on of human long-chain fatty acids-Coa ligase 4(FACL4) Genomics 1998;49:
327-330)。精神分裂病では、HUFAのリン脂質への取り込みの不足があるこ
とが知られているが、正確な酵素は知られていない。しかし、FACL−4は脳
内で見つかり、HUFAに対して特異性があり、無いときは脳の異常と精神分裂
病の患者によく見られる高いアーチ形をした口蓋のような軽い身体的異常の両方
を引き起こす(M Piccini et al, FACL-4, a new gene encoding long-chain ac
yl-Coa synthetase 4, is deleted in a family with Alport Syndrome, ellipt
ocytosis and mental retardationd.Genomics 1998;47:350-358)。この疾患を
引き起こすのは、両方の酵素の異常が併存することにある。
Sn3の位置に作用してイノシトールリン酸およびジアシルグリセロールを遊離
させるもう1つのホスホリパーゼPLCは、双極性障害に関係している可能性が
ある。LyPLとHUFAのように、これらの分子はともに細胞の信号伝達に関
係している:PLCとPLA2の過剰活性化が、双極性障害に関係しているよう
である。 神経変性状態では、ホスホリパーゼのような膜分解酵素の制御されていない活
性化に、HUFAの酸化に関係して増加する遊離基の生成とLyPLにより引き
起こされる膜の損傷が加わっているようである。このような現象は、過度なホス
ホリパーゼの活性に関連する膜の損傷とともに、アルツハイマー病および他の痴
呆、多発性硬化症、脳卒中および他の虚血または傷害により引き起こされる脳障
害、パーキンソン病、癲癇、およびハンチントン病について多くの研究者により
詳しく述べられている。機能亢進としても知られている注意力欠如障害では、ホ
スホリパーゼにより作用されることができる高度不飽和脂肪酸が血液中に不足し
ている。 これらの状態の全てにおいて、増大したホスホリパーゼの活性と、正常なやり
方で終了されていないかもしれない信号伝達についてのいくつかの証拠がある。
したがって、ホスホリパーゼ、FACLおよびアシルトランスフェラーゼは、薬
が作用するための新しい標的を提供する。したがって、EPAに富む物質が精神
障害に有益であるという我々の所見は、数通りの説明ができるだろう。
CR Lovell, Biochem Soc Transactions, 1991; 19: 915)、最初の活性化過程を
抑えるのを助けるであろう。興味深いことに、この分析系では、関係する脂肪酸
であるドコサヘキサエン酸(DHA)は効果がなかった。 EPAは、人の脳の酵素FACL−4に対して並はずれて高い親和性を持つ(
Y Cao他、1998、上記参照)。通常、HUFAに作用する酵素に関して
は、EPA、DHAおよびAAのようなHUFAに対する活性はほぼ同じである
か、またはDHAおよびAAがEPAよりも活性が大きいことがよくある。しか
し、FACL−4に関して言えば、AAに対する活性はDHAに対する活性の2
倍以上であり、またEPAに対する活性はAAに対する活性より50%高い。こ
れは、EPAが他のHUFAよりもより容易にサイクルに入り、EPA−CoA
誘導体を作り、LyPLに結合し、それにより遊離LyPLの活性を終わらせる
であろうことを意味する。したがって、EPAは他のHUFAよりもより効果的
に、活性化が開始された後に活性化を停止させるであろう。 EPAは、リン脂質のSn2の位置への取り込みに対してAAと競合するため
、EPAはその位置に取り込まれるAAの量を低減させるであろう。このことは
、AAのレベルが相対的にまたは絶対的に異常に高い鬱病において特に重要であ
ろう。 EPAは、プロスタグランジンI3(PGI3)やプロスタグランジンE3(P
GE3)のような望ましい化合物に変えられ得るHUFAの1つである。プロス
タグランジンI3やE3は、特に神経変性障害および欝病に有効であると思われる
範囲の抗炎症作用および抗血栓作用を有する。EPAから誘導された化合物は、
AAから誘導された同価の化合物よりも有害である可能性が低いと思われる。し
たがって、AAをEPAで置換することは、損傷の少なくとも1部が、後で前炎
症性化合物に変えられ得るAAを解離するホスホリパーゼの過剰活性に起因する
、上述した神経変性障害のすべてにおいて特に有効である。 EPAの精製は困難で複雑である。なぜなら、その5つの二重結合がすべて炭
素鎖中の正しい位置になければならず、さらにすべてがシス形でなければならな
いからである。EPAの合成は難しい。自然界では、EPAはほとんどがトリグ
リセリドやリン脂質の形で他の脂肪酸と混じった状態で見出される。EPAの精
製の原理は当業者には周知であり、低温結晶化、尿素分画、リチウム結晶化、分
留、高速液体クロマトグラフィー、超臨界二酸化炭素クロマトグラフィー、シリ
カゲルおよび他のカラム充填剤を使ういろいろな他の方式のクロマトグラフィー
を含む。これらの公知の技術は実際に大規模に適用することは困難であった。最
近になって、ようやく純粋なEPA(90%より高い純度、好ましくは95%よ
り高い純度)が精神およびCNS障害においてテストするために利用できるよう
になった。ある精製工程においては、EPAを多く含む天然の魚油のトリグリセ
リドが鹸化され、その脂肪酸はエチルエステルの形に変えられる。次に、適当な
留分を集める分子分留によりエチルEPAを多く含む製剤が得られる。次に、こ
の留分は尿素沈殿により80%を越えるエチルEPAを含む製剤に変換される。
さらに、シリカゲルクロマトグラフィーまたは高速液体クロマトグラフィーのい
ずれかにより96%以上の純度の最終的な製剤に調製される。 従来、EPAおよび関係する脂肪酸の使用に関する研究のほとんどは、EPA
がある程度濃縮されているが、かなりの量の他の脂肪酸、特にたいていの天然の
油にEPAと一緒に見出されるドコサヘキサエン酸(DHA)も含む物質を使用
していた。それらの脂肪酸は、通常はトリグリセリドまたはエチルエステルの形
であったが、遊離酸およびリン脂質の形であることもあった。ドコサペンタエン
酸(DPA n−3)もこのような物質のよくある構成要素である。しかし、我
々が明らかにした考え得るEPAの作用メカニズムの新しい理解によって、EP
Aの純度が高ければ高いほど活性が良いのではないかという認識に至った。1回
分の投与量は確かに純粋なEPAの適用の重要な1面であるが、これは単に1回
分の投与量の問題ではない。患者、特に精神障害の患者の立場からは、例えば1
gのEPAを純度95%の製剤として与えるほうが、EPAの総量が同じになる
、例えば5gの純度19%の製剤を与えるよりも良い。患者は、高度に精製され
た化合物で必要とされるより少ない量にずっとよく応じるであろう。 より重要なことは、EPAに構造が比較的よく似ているが同じ範囲の生物活性
を共有しないAA、DPAn−3、DHAのような他の脂肪酸は、すべての関係
する酵素の活性部位に結合するためにEPAと競合する。したがって、これらの
他の脂肪酸は、これらの活性部位を占めるためにEPAと競合し、その活性を弱
める。EPAの製剤は、純度が高ければ高いほど関係する活性結合部位を占める
可能性が高まり、望ましい生物学的効果を与える可能性が増大する。 我々は、35歳の鬱病の女性患者についての我々の経験によって予期せずに高
純度のEPAの重要性に注意を引きつけられた。この患者は、ハミルトン鬱病評
価基準の点数が15−25の範囲の長い鬱病歴を持っていた。伝統的な3環式化
合物やより新しい選択的セロトニン再利用阻害剤を含むいくつかの異なる抗欝剤
を試したが、目覚ましい効果はなかった。鬱病におけるEPAの重要性を証拠づ
けるため、8%のDHAおよび10%の他の高度不飽和脂肪酸とともに25%の
EPAを含むEPAに富む魚油を、4g/日で3ヶ月間この患者に与えた。この
試みの開始時には、この患者は20−22の範囲内のハミルトン点数で、終了時
には16−18の範囲内の点数であり、小さく、それほど重要でない改善であっ
た。患者はこのEPAに富む油をさらに2ヶ月間与え続けたが、さらなる変化は
みられなかった。その後、DHAが3%未満の、純度96%のエチル−EPAが
使用できるようになり、4g/日の25%のEPA製剤のかわりに、1g/日の
96%の製剤に切り替えるべきであるとの提案があった。我々は、こちらの方が
より便利であろうと考えたが、EPAの1日の投与量は変わらなかったので臨床
的な改善は何ら期待していなかった。しかし、驚くべきことに、約3−4週間後
、患者は著しい改善を見せ始め、数年間で初めてハミルトン点数が6より下がっ
た。この結果として、またFACL−4およびおそらく他のタンパク質に対する
EPAの結合に関する証拠について考えた結果として、我々は、最大の臨床効果
を得るにはEPAを高度に精製された形で投与することが、臨床効果のためには
重要であるという結論に達した。驚くべきことに、精製されたEPAの同じ1日
の投与量は、作用の関係する部位への結合についてEPAと競合する可能性のあ
る他の脂肪酸と混合されたときの同じ投与量に比べて、かなり効果が高いように
見える。
、現在では確認されている。行った観察のうちのいくつかの例は下記のものを含
む。 精神分裂病:10年の病歴を持つ患者が、24%のEPAと8%のDHAを含
む魚油16×0.5gカプセルを毎日服用することにより、その症状が抑えられ
た。これは、約1.9g/日のEPAと0.6g/日のDHAに相当する。患者
は他の高精神薬は服用していなかた。しかし、患者は毎回たくさんの魚油を服用
することを好まず、服用量を減らそうとした。患者が服用量を減らすと症状がぶ
り返し、治った状態を維持するためには、患者は約2g/日のEPAを服用しな
ければならないと結論付けた。精製されたエチル−EPAが入手できるようにな
ると、患者には4×0.5gカプセルをやはり約2g/日与えた。これによって
患者の症状は非常に良く抑えられた。そのため、患者は服用量を1g/日に落と
した。1.9g/日のEPAと0.6g/日のDHAを与える混合魚油を使って
いた前回の場合とは異なり、エチル−EPAの投与量を1g/日に減らしても、
症状の再発にはつながらなかった。したがって、純粋な形での1g/日は効きめ
があり、おそらくDHAおよび他の脂肪酸と混ざっているときの1.9g/日の
EPAよりもより効果があるように思われた。 双極性障害(躁鬱病):双極性障害は、欝または躁の状態のどちらかが気まぐ
れに起きるか、または欝または躁の状態が交互に起こる状態である。標準治療は
リチウムまたはバルプロエート誘導体、または特に躁状態を抑えるために使用さ
れる抗精神病神経弛緩薬である。標準の抗欝剤はときどき使用されるが、欝状態
を引き起こすかもしれないリスクがあるので、注意して与えられなければならな
い。
害患者を管理し、欝を改善し、欝または躁状態のどちらかにぶり返す危険を減ら
すうえで役立つ(A Stoll et al, Omega-3 fatty acids in bipolar disorder.
Archives of General Psychiatry, 1999; 56:407-412)。純粋なエチル−EPA
が入手可能になる前に、躁状態を抑えるための間欠的な神経遮断薬とリチウムに
よって状態が部分的にしか抑えられていなかった双極性障害の5人の患者に、1
8%のEPAと12%のDHAを含む魚油10−20g/日を与えた。これは、
一日当たり1.8−3.6gのEPAと1.2−2.4gのDHAに相当する。
1年間以上の臨床コースを辿ることにより示されているように、この治療を受け
ているときには患者は欝が軽減され、気分が変わりにくかった。しかし、患者は
完全に正常であることからは程遠かった。純粋なエチル−EPAが入手できるよ
うになったとき、魚油から4個の0.5gのソフトゼラチンカブセルの形で投与
量が2g/日の純粋なEPAに切り替えられた。各ケースにおいて、患者の臨床
状態はかなり改善された。患者の鬱病はかなり良くなり、また気分が変わりやす
い傾向が弱まっていた。特に、患者はみな従来の精神分裂病の評価基準では測る
ことが難しかったことが改善されたことに気が付いた。これは内部緊張の感覚す
なわち身体違和感と呼ばれ、鬱病に関係した1つの状態であるが、ときどき脳が
真綿で置き換えられた感覚、理路整然と明瞭に考えることが困難な感覚として表
現される混乱した緊張の状態にまで、抗精神病薬によって悪化させられたり、ま
たは変えられたりすることがある。珍しいことに、患者達はこの身体違和感、緊
張と真綿で考える感覚が大いに軽減されたり、完全に消えたと報告した。患者は
、魚油中のEPA/DHAの混合物の形でEPAの大量服用で治療されていたと
きには、この効果に気が付かなかった。このように、精製されたEPAは、混合
されたEPAとDHAに比べて、大きく質的に異なる効果を示した。そして、D
HAは実際にはEPAの治療効果を減じる、高純度のEPAは治療により大きな
効果があるという考えを支持している。 注意力欠如機能亢進障害(ADHD):ADHDおよび密接に関係した注意力
欠如障害や行為障害のような状態は、子供達の中によく見つかる。彼らは、しば
しば行動修正や他の心理学の技術、または食べ物に対する注意深い気配りにより
治療されて治るかも知れない。しかし、たいていの場合には、それらの作用にお
いてアンフェタミンに関係ある興奮剤を使用する逆説的な治療が必要とされる。
メチルフェニダートやデキストロアンフェタミンが最もよく使用される。幼児期
から重いADHDに罹っている7歳の少年がおり、両親は苦悩していた。心理学
的な手段や食餌療法は完全に失敗した。少年の症状はメチルフェニダートにより
部分的にしか抑えられていなかった。両親は、興奮剤の長期間の結果について心
配していた。そのため、少年には一日あたり純粋なエチル−EPAのカプセル2
個が与えられた。最初の2週間はほとんど変化はなかったが、それから行動にか
なりの改善が現れた。少年は、緊張と苛立ちやすさが低下し、機能亢進が軽減さ
れ、よく考えながらの議論により受容的になり、自宅および学校の両方で目覚ま
しく破壊的ではなくなった。メチルフェニダートを止めたときも、少年の行動は
改善されたままだった。 痴呆:75歳の女性が痴呆になった。おそらくアルツハイマー型であろうが、
関係する心臓血管病のために多発梗塞症が一因になっているかもしれない。しば
しば見られるように、彼女も引きこもりそして鬱病になった。不幸なことに、こ
の状態は理性を失った暴力的な激発によって中断され、それは通常は彼女の家族
に向けられるが、ときには全く知らない人に向けられる。彼女は2つのコリン作
用剤で試された。これらは彼女の認識状態に小さな改善をもたらしたが、鬱状態
、苛立ちやすさおよび怒りに対しては効果がなかった。彼女には、純粋なエチル
−EPAの0.5gのカプセルを朝と夕方にそれぞれ2つずつ与えた。4週間の
内に彼女は著しい改善を見せ、より快活にかつ友好的になり、苛立ちやすさも大
幅に低減した。彼女の家族もまた彼女の認識が改善したと考えていたが、彼女の
改善した気分のためにこれは確信することが困難である。
起こす要因がまったくなしに突然に起こる可能性があるパニック発作が伴うか、
または普通の人々にはそのような反応を引き起こさない温和にストレスがかかっ
た状態が伴うかもしれない。パニック発作の特徴は、重度の不安感、呼吸困難、
発汗、「胃の中に蝶がいる」として知られている腹部の不快感、および脚の虚脱
感である。これらの心理症状の全てが非常に高頻度で伴なうものに、心悸亢進も
あり、これには心臓の動悸の自覚感覚が伴う。不眠症もよくあり、患者はしばし
ば心悸亢進やパニックで夜中に衝動的に目を覚ます。また、患者はしばしば鬱状
態になる。広く行われている治療は、ベンゾジアゼピン、心悸亢進を抑えるため
のベータブロッカー(β遮断剤)および抗欝剤のような抗不安薬を含む。 我々はパニックおよび不安障害を持つ数人の患者を、精製されたエチル−EPA
で治療してすばらしい効果を得た。3つの病歴を以下に示す。29歳の女性が、
頻発しかつ抑えることができないパニック発作と心悸亢進の結果、身体障害者に
なった。ベンゾジアゼピン、ベータブロッカーおよび抗欝剤による治療は、彼女
の症状を抑えるのに部分的にしか効かないことがわかった。彼女は次に投薬量の
1日2gのエチル−EPAを4つの500mgのカプセルとして与えられた。彼
女の症状は劇的に改善して、3ヶ月後には彼女のパニック発作と心悸亢進はほと
んどなくなり、たった2回の比較的軽い発作があっただけだった。この治療は、
彼女が受けた前のどの治療よりもずっと効果があった。56歳の女性は人生の大
部分の間、軽い不安およびパニック発作を患っていた。しかし、これらは彼女の
子供達のいろいろな問題と重なって数年間の間にずっと悪化した。抗不安薬と抗
欝剤はあまり効果がなく、彼女の心悸亢進はベータブロッカーの比較的大量投与
によっても抑えられなかった。彼女も4ヶ月間エチル−EPAを使って治療した
。彼女の不安およびパニック発作はずっと軽度になり、たとえ不安およびパニッ
ク発作が起きたときも、以前には彼女をものすごく不安にした心悸亢進がなくな
った。第3の患者は、2回の離婚を経て現在は長期の関係にある55歳の主婦で
あった。彼女は毎週数回、ときどきは夜中に、パニック発作におそわれた。そし
ていつでも激しい心悸亢進が伴った。この激しい心悸亢進が、彼女には非常に心

き、彼女は心悸亢進の発作がやってくるかもしれないと感じたが、めったにこな
かった。その結果、彼女は強まる自信を感じ、ずっと正常な生活を取り戻すこと
ができた。これらの報告は、精製されたエチル−EPAがパニック障害の管理に
おいても効果があることを示している。 エチル−EPAの他の使い方は、電気ショック療法(ECT)を使用するとき
に、重い鬱病の管理に対する補助薬としてである。ECTは、ほとんどの他の治
療が効果がなかったときに使われる。ECTは麻酔下でショックを加えることを
含み、効果があっても、脳に損傷を与えたり記憶の喪失を引き起こしたりするか
もしれないという心配のために、患者と医師の両方によって、ためらいながら使
用される。5〜20回の任意の回数またはさらに多数回のショックが、治療のコ
ース中に加えられるかもしれない。有益な効果を生じさせるために必要なショッ
クの回数を制限することができることは、大変望ましいであろう。また、我々は
、少数の患者についてこれが事実であるケースを見出した。

い精神分裂病の病歴を持つ一人の中年の男性患者が定型神経弛緩薬に反応しなか
った。定型神経弛緩薬は、ドーパミンD2レセプターに対して強い遮断作用を持
つ薬である(B Leonard, Fundamentals of Psychopharmacology, 2nd edition,
Wiley, New York,1997)。精神分裂病のいわゆる「ポジティブ」症状(幻覚、妄
想および思考障害など)に対するそれらの薬の速い効きは、このメカニズムによ
るものであると考えられている。しかし、これらの薬は無快感症、情動欠如、感
情的および社会的引きこもりのような精神分裂病のいわゆる「ネガティブ」症状
に対しては、ほとんど効果がない。また、D2遮断作用は、これらの薬の多くの
逆効果の原因となる。この逆効果には、錐体外路運動障害、過剰なプロラクチン
の分泌、晩発性運動障害が含まれる。このクラスにおいて広く使用される薬の例
には、クロルプロマジン、チオリダジン、ハロペリドール、ピモザイド、フルペ
ンチクソールおよびスルピリドが含まれる。

ン、オランザピン、クエチアピン、リスペリドン、ジプラジドン、セルチノドー
ル、そしておそらくゾテピンとアミスルピライドが含まれる。初期開発段階のこ
のタイプの多数の薬があり、すべての薬が定型神経弛緩薬よりもD2レセプター
に対する親和性が低いことにより特徴づけられる。 患者はクロザピンで治療され、やがて900mg/日の最大投薬量に達した。
この治療はいくらかの改善をもたらしたが、患者は重い疾患のままである。彼は
緊張していて、引きこもっており、疑い深く、奇異な行動を示した。例えば、彼
はテレビのスイッチを入れたとき、どんな音が出るのも拒否した。これは、彼に
話しかける人々についてのパラノイアのためである。彼と一緒にテレビを見よう
としている誰にとっても、これは当然いらいらすることである。彼はオランザピ
ン、クエチアピン、リスペリドンの試みを与えられたが、十分な効果はなかった
。臨床的に彼はクロザピンでもっとも成績が良かったように見えた。それで彼は
何ヶ月間も900mg/日で治療された。彼が最も苦しんだクロザピンの主な副
作用は唾液分泌過多だった。 その後、E−EPAが2g/日の投薬量でクロザピンに加えて与えられた。E
PAは精神分裂病に対して効果のある治療であるが、その作用の開始は遅く、か
なりの効果が見られるのは通常治療開始後4−8週間経過してからである(B Pu
ri ea al., Sustained remission of positive and negative symptoms of schi
zophrenia following treatment with eicosapentaenoic acid, archives of Ge
neral Psychiatry 55:188-189, 1998)。しかし、長年の間大変重く病んできた
このクロザピンで治療されていた患者において、数日内にその効果が目に見え始
め、2週間以内に劇的なものとなった。それらの効果には、患者自身と家族や友
人達の両方が気が付いた。特に、欝になっている代わりに彼は陽気になり、緊張
する代わりにくつろぐようになり、引きこもって話すことを拒絶する代わりに社
交的で話好きになった。彼の家族と友人達にとって、最初のそして最も印象的な
効果の1つは、彼が普通の音の大きさのままテレビをつけさせたことだった。こ
の患者を担当していた精神科医は「私は2週間未満で精神分裂病においてこのよ
うな劇的な改善をこれまで見たことを思い出せない」と書いている。このケース
の1つの特に重要な面は、クロザピンで引き起こされる唾液分泌過多がかなり軽
減されたことである。 この患者において、クロザピンおよび他の神経弛緩薬、定型および非定型のど
ちらも明らかに非常に限られた効果しかなかった。他方、E−EPAは、もしE
−EPAが単独で投与されていたなら通常は見られない驚くくらい大きく、予想
外に速い効果を示した。これはクロザピンとE−EPAの間の強い有益な相互作
用があること、そして少なくとも患者によってはクロザピンとE−EPAを同時
に投与すべきであるということを示唆している。 なぜE−EPAが、精神分裂病に対してだけでなく他の精神および神経障害に
対しても、従来の向精神薬と連携して加算さらには相乗効果さえ生じさせるのか
については納得できる理由がある。これは精神および神経障害のために使用され
るほとんどの薬が神経伝達物質代謝または神経伝達物質のレセプターに作用して
、ノルアドレナリン、セロトニン、ドーパミン、グルタメート、アセチルコリン
、ガンマ−アミノ酪酸(GABA)のような神経伝達物質に対して、合成、不活
性化、再利用または反応を調節する。しかし、神経細胞内の多くの現象は、1つ
のレセプターが1つの神経伝達物質または遮断剤により占拠された後に起こる。
レセプターの占拠は、「信号伝達」または「細胞シグナリング」という概括的な
名前の下で進行する細胞機能の多数の変化を引き起こす。この細胞機能の変化に
は、G蛋白、カルシウムの移動、ナトリウムおよびカリウムの移動、リン脂質お
よびエイコサノイドの代謝、環状ヌクレオチドの変化、遺伝子調節の変化が含ま
れる。これらは信号伝達という概括見出しの下で起こる変化過程のいくつかの例
にすぎない。ドーパミン、セロトニン、アセチルコリンまたはグルタメートレセ
プターの活性化に続くであろうPLA2の活性化の具体的な例が図3に示されて
いる。 精神および神経障害を引き起こす異常のなかには、神経伝達物質またはレセプ
ターのレベルではなく、レセプターより後の信号伝達レベルのものがあることを
示す証拠が増えつつある。精神障害に関係するこの証拠の多くは、最近出版され
た本に要約されている(Phospholipid Spectrum Disorder in Psychiatry, ed.
M Peet, I Gren & DF Horrobin, Marius Press, Carnforth, UK,1999)。もしこ
れが本当なら、神経伝達物質またはレセプターのレベルで作用する、現在使用さ
れているものの大部分のような薬は、限られた効果しか持たないであろう。この
ことは、多数のタイプの精神および神経障害のための薬についてもあてはまる。
それらの大部分の主な作用は神経伝達物質またはレセプターレベルである。神経
伝達物質の代謝または神経伝達物質のレセプターに影響を与える薬により現在治
療される疾患は、精神分裂病、鬱病、双極性障害、注意力欠如、行為障害および
関連する障害、パーキンソン病、痴呆、不安障害、パニック障害および対人恐怖
障害、および月経前症候群などである。現在の薬は、レセプターより後の信号伝
達レベルで作用する薬とともに、加算または相乗の効果を生じることが期待され
る。なぜなら、その時その薬の組合せはニューロン内の2つの異なるレベルで作
用しているであろうからである。 E−EPAの主な作用は、レセプターより後のリン脂質に関係した信号伝達を
調節することである。次に、これらの作用は間接的にプロテインキナーゼ、カル
シウムの移動、環状ヌクレオチドの代謝または遺伝子の発現などの他の過程また
は酵素を調節する。したがって、EPAとこれらの他のクラスの薬との間には、
強く、潜在的に価値がある相互作用がある。 一人の患者のこの病歴が、思いがけず我々の注意を1つの原則、すなわち、E
−EPAまたは他の形の生物的に活性なEPAは、主として神経伝達物質の代謝
または神経伝達物質のレセプターに作用する向精神薬または神経薬と共同投与し
たら効果があるかもしれないという着想に引きつけた。
病薬、すべてのクラスの抗欝剤、不安またはパニックを抑えるのに使用される薬
、眠りを調節するために使用される薬、およびパーキンソン病または痴呆の処置
に使用される薬などである。これらの薬はすべて神経伝達物質またはそれらのレ
セプターのレベルで作用する。 E−EPAが標準薬による治療の効果を高めることができるという原則は、さ
らなる症例により例証され得る。重い鬱病にかかっていた50歳の男性がいくつ
かの抗鬱剤で治療されたが効果はなかった。やがて、彼はシタロプラムと呼ばれ
る選択的セロトニン再利用阻害剤(SSRI)で治療を受けた。他のSSRIは
効かなかったが、驚くべきことに、この薬によって欝状態の1部が取り除かれた
。これは数ヶ月の間持続した改善をもたらした。この期間、彼は軽い欝状態が続
いた。シタロプラムに加えて1g/日のエチル−EPAを与えられた。4週間後
、彼は数年間のうちで初めて気分が晴れやかになるのを経験し、永久に失ったと
思っていた人生に対する興味の多くを取り戻した。もう1つの症例は、精神分裂
による衰弱のため大学を辞めた21歳の若い女性である。彼女は、彼女の教授の
一人が彼女の大学の学歴を破壊するためにやってくると彼女に言い続ける幻聴を
経験し、彼女の親しい学生の友人たちについても全く信用しなくなり、重い思考
障害であることを示す奇妙な一連の宿題の小論文を書いた。彼女は10mg/日
のオランザピンで治療された。この薬は、幻覚を止め、パラノイアと思考障害を
軽減したが、彼女が主張するには、不快になり、ぼうっとして明瞭に考えること
ができなくなった。また、この薬によって彼女は非常に眠くなり、食欲が増進し
て体重が増加した。彼女は2g/日のエチル−EPAを与えられ、約4週間後に
、より明瞭に考えていること、食欲が低下し体重を減らすことができたことを話
した。彼女は1年後に大学に戻ることができ、AからB評価の成績をとっている
。 ある40歳の男性は25歳のときに精神分裂病にかかった。彼は盛んな幻覚と
パラノイアに悩まされ、ほとんどの交友関係を絶って引きこもり一人で暮らした
。彼はいろいろな薬を試され、それらのいくつかは好ましからざる副作用を生じ
た。やがて、定型神経弛緩薬であるハロペリドールの投与が見いだされた。この
薬によってほとんどの幻覚がなくなり、パラノイアは適度の範囲に閉じこめられ
たが、彼をさらにいっそう引きこもらせたようにも見えた。彼は一人で暮らし、
友人のほとんどと連絡を断ち、一人で街に出てひとり言を言った。彼の精神科医
はエチル−EPAについて聞き、標準の薬に加えてそれを試すことを決めた。4
g/日による治療で8週間後に彼の外観は一変した。残っていた幻覚は消え、彼
は感情面でずっと温かくなり、より親切になった。さらに、彼は説得されて休暇
を取って家族と旅行に出かけ、そこで楽しみ新しい友人も作った。したがって、
EPAはハロペリドールによって悪化させられていたように見えた極度の引きこ
もりを逆転させた。 定型神経弛緩薬は、患者によっては晩発性運動障害と呼ばれる悲惨な状態を発
生させる。これは部分的に脳内の不可逆の変化に起因している。なぜなら、薬に
よる治療を止めても、この状態はしばしば軽減しないからである。患者は、抑制
できない痙攣や他の運動を経験する。これらは身体のどの部分にでもあらわれる
が、通常は彼らが特に苦痛を受ける顔面にあらわれる。舌の突き出しや抑制でき
ない運動−を含む顔面の痙攣と動きが起こる。我々がEPAで治療した何人かの
患者は、晩発性運動障害から実質的に解放された。運動は顕著に少なくなり、患
者によっては完全に消えてしまった。
さまざまなひどい副作用がある。患者の立場からは、最も煩わしいものの1つが
唾液分泌過多である。これは非常に深刻なものとは思われないかも知れないが、
大量の唾液の絶え間ない流出は、実際には、患者にとってもまた患者に付き添っ
ている人々にとっても非常に大きな悩みである。唾液分泌過多のメカニズムは知
られていない。完全に予想外であったが、唾液分泌過多が1つの大きな問題であ
ったクロザピンで治療された3人の患者に対するエチル−EPAによる治療は、
精神病の状態を大きく改善しただけでなく、唾液の分泌を正常なレベルにまで劇
的に低下させ、患者と患者の世話をする人々に大きな安心を与えた。 肥満は抗精神病薬のもう1つの共通の副作用である。この観点からは、オラン
ザピンのような新しい薬のなかには、しばしば定型神経弛緩薬よりも副作用がず
っとひどいと思われるものがある。20代のはじめに精神分裂病により衰弱した
若い女性患者が、定型神経弛緩薬による治療を約10年間受け、その後の2年間
はオランザピンによる治療を受けた。患者はオランザピンの方を好んだが、オラ
ンザピンによっては患者を引きこもって友人や家族と正常にコミュニケートする
ことができない状態から救えなかった。さらに患者を悲しませたことには、オラ
ンザピンによって患者の体重は10kg以上増加した。医師はオランザピンに加
えてエチル−EPAを試してみることを決めた。患者の引きこもり状態は次の6
ヶ月間で次第に改善し、患者は数年間行っていなかった音楽家としての活動を再
開した。患者の自尊心にとって同様に重要だったのは、最初にオランザピンによ
る治療を始めたときに増加した体重のほとんどが失われていたことだった。 これらの症例の履歴は明瞭に下記のことを証明している。 1.純粋なエチル−EPAは、精神分裂病、双極性障害、鬱病、注意力欠如、痴
呆に対して、それ自体で強い治療効果を持つ。抗精神病作用のある他の薬と同じ
ように、純粋なエチル−EPAはほとんどの精神障害および膜の損傷に関係する
神経障害にも有益な効果があるかもしれない。これらは、いろいろなタイプの痴
呆(アルツハイマー病、多発梗塞血管性痴呆、レーヴィ体病を含む)、多発性硬
化症、パーキンソン病、ハンチントン舞踏病を含む。 2.純粋なエチル−EPAは、広範囲の精神病および神経病の薬の有益な効果を
高める。精神および神経の異常の多くは、ニューロンおよび他の細胞の信号伝達
系の神経伝達物質のレセプターを越えたところにあることが明らかになりつつあ
り、このことは大変よく理解できる。EPAはこれらの信号伝達系に作用し、E
PAと他の薬との間の明白な相互作用に対する合理的な説明を与える。 3.全く予想されていなかったのは、精神病および神経病の薬の有害な作用を減
らすEPAの能力である。このメカニズムは解明されていないが、薬によって引
き起こされる肥満、運動障害、鎮静状態、神経不安、唾液分泌過多に関して注目
されているように、比較的一般的な現象であるように思われる。 エチル−EPAの効果の症例報告は説得力があるが、その有効性の明白な証拠
は、無作為化された偽薬対照試験から得られたものでなければならない。したが
って、我々は精神分裂病にかかっている34人の患者に、米国精神医学協会の精
神障害の診断・統計マニュアル(第4判、DSM−IV)に記載された基準に従
って、無作為化された偽薬対照試験を綿密に計画した。これらの患者はみなクロ
ザピン薬で治療されたが、全員が重い精神分裂病の症状のままだった。多くの精
神分裂病の患者と同様に、患者のうちの何人かは欝状態だった。12週間後、 P
ositive and Negative Symptom Scale for Schizophrenia(PANSS)およびMontgom
ery-Asberg Depression Rating Scale (MADRS)を使い、基線上にあると評価され
た。
作為に3つのグループ、すなわち、0.5gのカプセルで偽薬または一日あたり
1gまたは2gのエチル−EPAを与えるグループの1つに入れられた。偽薬と
エチル−EPAのカプセルは、外観からは互いに区別できなかった。

ループよりもかなり良い効果があった。E−EPA(エチル−EPA)は、精神
分裂病の症状、欝病の症状、現用の薬の副作用を軽減する効果があった。MAD
RS基準により示される鬱病とPANSS基準により示される精神分裂病に対す
る同時効果は特に重要である。したがって、これらの結果は個々の患者について
の報告を確認し、E−EPAが強力な治療効果を有することを明確に示している
。 特に注目すべきは、E−EPAにより引き起こされた可能性のある副作用の報
告がないこと、そして報告された副作用について偽薬と進行中の治療との間に全
く差がないことである。これは事実上、精神病または神経病において使用されて
いるほとんど全ての薬と劇的な対照をなし、EPAが重大な副作用を全く持たな
いことを意味する。 ハンチントン病は、多くの点で、脳に影響を与える全ての疾患のなかでは最も
破壊的な疾患である。ハンチントン病は優性に受け継がれる遺伝病であり、通常
30歳から50歳の間に発現し、そして通常約15−25年間続き、死に至る。
患者の子供たちの半数はこの疾患を発症する(WJ Koroshetz、pp.654-661 in "
Office Practice of Neurology、ed MA Samuels & S Feske、Churchill Livings
tone、New York、1996)。 影響を受けている遺伝子は、染色体4p16.3上にある huntingtinと呼ば
れるタンパク質のためのものである。この遺伝子は多形トリヌクレオチドCAG
が反復する領域を含む。正常な個人では、CAG反復の数は通常35未満である
が、ハンチントン病ではCAG反復の数は35以上である。反復の数が大きいほ
ど、ハンチントン病がより早く発症する可能性が高い。この反復のために、影響
を受けたニューロン内に凝集体を形成するhuntingtinの過剰発現が起こる。この
凝集体は、膜および細胞骨格と結合し、関係するニューロンの機能不全と結果と
して生じる死を引き起こす。脳のほとんどの部分にも損傷が起こり得るが、基底
ガングリア構造、尾状部および被殻に特にはっきりと見られる。 現在では、いくつかの他の神経病が、他の遺伝子内のトリヌクレオチド反復の
過大な数と関係づけられることが知られている。タンパク質の蓄積とニューロン
の損傷の類似のメカニズムが関与している可能性があるとみられている(ST War
ren, The expanding world of trinucleotide repeats, Science 1996; 271:13
74-1375: P Djion et al, Coden repeats in genes associated with human dis
eases, Proceedings of the National Academy of Science of the USA 1966; 9
3:417-421)。 ハンチントン病に加えて、このようにして引き起こされること
が知られている疾患には、脆弱X症候群、フリードライヒ運動失調、脊髄性およ
び延髄性筋萎縮、脊髄小脳性運動失調タイプI、歯状核−赤核−淡蒼球萎縮、ホ
ーリバー症候群、マシャド−ヨセフ病、および筋強直性ジストロフィーが含まれ
る。 ハンチントン病は、異常に広範囲の症状と徴候を示すことがあり(WJ Koroshe
tz,1996)、精神分裂病、鬱病、不安、過敏性、感情制御の喪失、癲癇、パーキ
ンソン病、痴呆の特徴を示すこともある。ハンチントン病、筋萎縮性側索硬化症
、パーキンソン病、アルツハイマー病および他の痴呆、小脳変性を含む広範な神
経病における基本的なendメカニズムは、膜およびタンパク質に対する酸化に
よる損傷であるということが提案されている(MF Beal, Aging, energy and oxi
dative stress in neurodegenerative diseases, Annals of Neurology 1995; 3
8:357-366)。これらの疾患においては、正常または異常なタンパク質が凝集し
て、内部および外部の膜に損傷を与えるということが主張されている。疾患間の
差異はタンパク質の性質と、最も影響を受けるニューロンの部位に関係するが、
全体のプロセスは類似している。 典型的なハンチントン病患者においては、疾患は軽い協同運動失調、健忘、動
作緩慢、苛立ちを伴う情動障害から始まる。協同運動失調は次に突然の痙攣運動
を伴う明白な神経損傷と、反復される制御できない身悶えと絶え間ない運動、す
なわち舞踏病という一般的な名前の下に進むパターンへと進行する。しばしば顕
著な体重の減少があり、これは制御できない運きにより引き起こされる運動に関
係があるかもしれない。患者はやがて歩くことができなくなり、重い記憶喪失を
伴う痴呆になり、24時間の介護を必要とするようになる。死は、正常に飲み込
むことができないことの結果として食物または液体を肺に吸い込むことに起因す
る肺炎により引き起こされることが多い。 既知の治療法は、実質的な価値はない。異常な運動と精神障害は標準の抗精神
分裂病薬に部分的に反応するかもしれない。それに対して、抗鬱剤、トランキラ
イザー、抗癲癇薬はいくつかの症状を一時的に軽減するのに役立つかもしれない
。どれも容赦のない悪化する方向を止めるものはない。 初期のハンチントン病は精神分裂病のような特徴を示すかもしれないので、ハ
ンチントン病の最終段階にある7人の重度に無能力になった患者に、純度96%
のエチル−EPAによる任意抽出の試みを計画した。全員24時間の介護を必要
とし、重い運動障害があり、苛立ちやすく、部分的に痴呆になっていた。彼らは
二重盲検法に基づいて、6ヶ月間2g/日のエチル−EPAまたは2g/日の偽
薬を受けるように無作為に分けられた。6ヶ月間で、4人の患者は進行性の悪化
を示したが、3人の患者は疾患のコースを逆に辿り、異常な動きの減少、情動障
害および苛立ちやすさの軽減、記憶および認識機能の増進による改善を示した。
コードが破られたとき、悪化した4人の患者は偽薬で治療され、改善した3人の
患者はエチル−EPAを服用していたことが判明した。患者のうち、エチル−E
PAおよび偽薬で治療されたそれぞれ2人ずつについて、研究の開始時と終了時
に磁気共鳴撮影(MRI)を使用して脳の劣化を評価した。MRIによって、側
脳室、すなわち大脳半球内の流体で満たされた空間のサイズの正確な評価が可能
となる。ハンチントン病が進行するにつれて、側脳室が大きくなり、脳の組織が
失われたことを示している。6ヶ月以上偽薬を投与された2人の患者では、予想
どおり側脳室が拡大していた。エチル−EPAを投与された2人の患者では、側
脳室のサイズが縮小しており、実際に脳組織の消失の逆転があったことをMRI
は示している。 タンパク質の異常な蓄積により引き起こされ、以前は治療できなかったハンチ
ントン病の最終段階の患者におけるこれらの劇的な結果によって、神経変性障害
におけるエチル−EPAの価値が実証された。このような障害は、すべて膜の損
傷、酸化ストレス、ホスホリパーゼの活性化に関係している。EPAは膜を修復
し、ホスホリパーゼを阻害し、酸化に対する防御を高める。したがって、ハンチ
ントン病の結果は、トリヌクレオチドの反復に関係づけられる神経障害、および
異常なタンパク質の蓄積に起因する可能性がある酸化による膜の損傷に関係づけ
られる神経障害への、純粋なエチル−EPAの使用を支持している。それらの神
経障害には、アルツハイマー病および他の痴呆、レーヴィ体病痴呆、パーキンソ
ン病、癲癇、多発性硬化症、脳卒中、頭部損傷および脳卒中後および術後症候群
、クル病、狂牛病、クロイツフェルト・ヤコブ病、スクレイピーのようなプリオ
ンの蓄積に起因する動物または人の疾患が含まれる。 ごく最近、ハンチントン病がカスパーゼIと呼ばれる酵素、すなわちインター
ロイキン変換酵素(ICE)の活性化に関係づけられることが示された。ICE
は広範囲の神経が変性する脳の疾患に関与している。ICEは、インターロイキ
ン−1−ベータを生成し、インターロイキン−1−ベータは広範囲の前炎症性の
細胞損傷作用を活性化する(VO Ona et al, Inhibition of caspase-I slows di
sease progression in a mouse model of Huntington's disease. Nature 1999
; 399:263-267)。したがって、可能性のあるエチル−EPAの作用の追加のメ
カニズムはカスパーゼIの阻害である。 本発明は、エチル−EPAを非常に効果的であると確認した。しかし、血液中
のEPAのレベルを上げることができる高純度EPAは、どのような形であれ精
神障害に効果があるということは確実らしい。これらのEPA化合物はすべて、
純粋な形で調製されれば、精神・神経障害の治療に効果があるであろう。EPA
に干渉するかも知れない関係する脂肪酸の存在を絶対最小に保つことが常に重要
であろう。公知ではあるが、以前は精神とCNS障害に使用するためにはこのよ
うな純粋な形で調製されたり提案されたりしたことがなかったこれら化合物は:
1.エチル−EPA:心臓血管障害の治療に使用するために日本で広く使われて
いる。 2.リチウム−EPA:米国特許第5、252、333号に開示されているが、
当時は精神病で特に価値があるとして他のリチウムHUFA誘導体から区別され
ていなかった。この誘導体の価値は、リチウム自体が双極性障害の選択治療であ
り、精神分裂病、分裂情動性障害、鬱病に治療効果があることも知られていると
いう事実にある。最近、PLA2の阻害およびPLCイノシトールホスフェート
サイクルの阻害がリチウムの重要な作用であることが証明された。EPAは関係
する作用があるので、EPAのリチウム誘導体は特に重要であろう。 3.トリグリセリド、モノグリセリド、ジグリセリド:分子中に存在する脂肪酸
の90%より多く、好ましくは95%より多くがEPAからなるもの。 4.体内に脂肪酸を放出できる他のエステルまたは化合物:存在する脂肪酸の9
0%より多く、好ましくは95%より多くがEPAの形であるもの。 上記の各々において、DHA、DPA、AAまたは他の競合する脂肪酸は、そ
れぞれ5%未満、好ましくは3%未満でなければならない。これらの競合する脂
肪酸は、合計で10%未満、好ましくは5%未満でなければならない。

は本発明の範囲に含まれる。 EPA化合物は、患者によっては精神病学または神経学において使用される他
の薬と有利に並行投与することもできる。このような薬には、代表的なものとし
てクロルプロマジン、ハロペリドール、チオキサンチン、スルピリド、ドロペリ
ドール、ペルフェナジン、ロキサピン、チオリダジン、フルフェナジン、ペリシ
アジン、メトトリメプラジンまたはピモザイドのような定型神経弛緩薬;クロザ
ピン;オランザピン、リスペリドン、セルチンドール、ジプラジドン、クエチア
ピン、ゾテピンおよびアミスルピリドを含む非定型神経弛緩薬;3環式および関
係する抗欝剤、ノルアドレナリン再利用阻害剤、セロトニン再利用阻害剤、モノ
アミン酸化酵素阻害剤、および非定型抗欝作用を持つ薬を含む抗欝作用を持つ薬
;睡眠障害、不安障害、パニック障害、対人恐怖症、行為障害、人格障害、およ
び注意力欠如、機能亢進障害のための薬;アルツハイマー病、血管性および多発
梗塞性痴呆、レーヴィ体病、および他の痴呆を含むあらゆる形の痴呆のための薬
;パーキンソン病、多発性硬化症、ハンチントン病、および他の神経変性障害を
含むあらゆる形の神経病のための薬が含まれる。 上記のケースの各々において、EPA化合物と他の薬は、それぞれの処方で別
々に投与してもよい。それらは別々に包装されていてもよいし、同じパッケージ
に入れられていてもよい。また、1日当たり0.1gから10g、好ましくは1
日当たり0.5gから5gのEPAの1日の投与量が他の薬の標準の1日の投与
量とともに提供されるように、当業者によく知られている技術を使用してEPA
と他の薬を一緒に調合してもよい。 単独で投与されるときは、EPAの有効な1日の投与量は0.05gから50
g/日、好ましくは1日当たり0.1gから10g/日、さらに好ましくは1日
当たり0.5gから5g/日の範囲である。
生殖系および他のすべての臓器・器官の炎症性の疾患の治療に有益であるかもし
れないことは以前から提案されていた。しかし、EPAの純度の重要性は以前に
は認識されていなかった。したがって、本発明はこれらの疾患に対しても有益で
あろう。また、本発明はこのような炎症性疾患の治療のための医薬化合物を提供
する。
ひRK Bergeによって説明されているEPAの2-置換誘導体である(Biochim Bio
phys Acta 1396; 193-203, 1998)。これらは、酸化されにくいのでより長い作
用時間を持つという利点がある。 実施例 調合物 下記の例の各々において、製品の純度は少なくとも90%、好ましくは95%
以上である。他の脂肪酸は結合部位への結合においてEPAと競合し、製品の有
効性を低下させるので、これは非常に重要である。特に、DHA、AA、DPA
−n−3のような脂肪酸は、それぞれが5%未満、好ましくは3%未満の濃度で
存在する。このような競合する化合物は合計で10%未満、好ましくは5%未満
でなければならない。また、この純度は毎日消費すべき物質の体積を最小限にし
、指示に従わないことが重大な問題である精神病患者が指示に従うのを容易にす
るための主要因であるうえで有益である。 1.250mg、500mgまたは1000mgのエチル−EPA、トリグリセ
リドEPA、または他の適当な形のEPAを含むハードまたはソフトゼラチンか
らなるカプセル。 2.250mg、500mgまたは1000mgのリチウム−EPAを含む錠剤
、または同量を含むハードゼラチンカプセル。 3.リチウム−EPA、エチル−EPA、トリグリセリドEPA、または他の適
当な形のEPAが、経口投与のために飲みやすい液体に調製された乳濁液、溶液
または懸濁液。 4.100mgから5gのEPA化合物の1つが調合されている座薬または膣座
薬。 5.10mgから500gのEPA化合物の1つを含む静脈注射用の溶液または
乳濁液。 5−10 実施例1―5と同じであるが、EPAの2-置換誘導体を使用したも
の。 11−20 1−10と同じであるが、EPA化合物が精神または神経障害の治
療のために使用される薬の通常の投与量とともに調合されているもの。 21−30 1−10と同じであるが、EPA化合物がクロザピンとともに調合
されているもの。

Claims (27)
- 【請求項1】 適当に吸収可能な形のEPAを含む薬剤であって、製剤中に
存在するすべての脂肪酸のうちの少なくとも90%、好ましくは少なくとも95
%がEPAの形であり、5%未満、好ましくは3%未満がドコサヘキサエン酸(
DHA)の形であることを特徴とする薬剤。 - 【請求項2】 存在する他の脂肪酸のうち、AAまたはDPA−n−3が、
それぞれ5%未満、好ましくは3%未満である、請求項1に記載の薬剤。 - 【請求項3】 DHA、AAおよび/またはDPA−n−3の総含有量が、
存在する全脂肪酸の10%未満、好ましくは5%未満である、請求項1または2
に記載の薬剤。 - 【請求項4】 EPAが、エチル−EPA、リチウム−EPA、モノ−、ジ
−またはトリグリセリドEPAの形またはEPAの他のエステルまたは塩、EP
Aの遊離酸、または生物により利用でき、体内のEPAレベルを上げる他の適当
な誘導体の形である、前記請求項のいずれかに記載の薬剤。 - 【請求項5】 EPAが、2-置換誘導体または他の生物活性を低下させる
ことなく酸化速度を低下させる誘導体の形である、前記請求項のいずれかに記載
の薬剤。 - 【請求項6】 精神または中枢神経系の疾患の治療または予防のための前記
請求項のいずれかに記載の薬剤。 - 【請求項7】 疾患が精神分裂病、分裂情動性障害または分裂病型障害であ
る、請求項6に記載の薬剤。 - 【請求項8】 疾患が鬱病または躁鬱病(双極性障害)である、請求項6に
記載の薬剤。 - 【請求項9】 疾患が不安、パニック障害、または対人恐怖症、睡眠障害、
または注意力欠如、行為、機能亢進または人格障害である、請求項6に記載の薬
剤。 - 【請求項10】 疾患がアルツハイマー病または多発梗塞性痴呆、レーヴィ
体病、およびプリオン疾患に起因する疾患を含む他の痴呆である、請求項6に記
載の薬剤。 - 【請求項11】 疾患がパーキンソン病または他の運動系の障害である、請
求項6に記載の薬剤。 - 【請求項12】 疾患が多発性硬化症である、請求項6に記載の薬剤。
- 【請求項13】 疾患が脳卒中、脳卒中後症候群、または頭部損傷または頭
部損傷後症候群である、請求項6に記載の薬剤。 - 【請求項14】 疾患が癲癇である、請求項6に記載の薬剤。
- 【請求項15】 疾患がハンチントン病または他の神経変性障害、特にフリ
ードライヒ運動失調、筋強直性ジストロフィーおよび脆弱X症候群を含むトリヌ
クレオチド反復に起因する障害である、請求項6に記載の薬剤。 - 【請求項16】 主として神経伝達物質の代謝またはレセプターに作用する
薬が、請求項1ないし5のいずれかに記載の薬剤との並行投与のために調製され
る、精神または神経障害に使用するための調合物。 - 【請求項17】 主として神経伝達物質の代謝またはレセプターに作用する
薬とともに、請求項1ないし5のいずれかに記載の薬剤を含むことを特徴とする
医薬調合物。 - 【請求項18】 薬がクロザピンである、請求項16または17に記載の調
合物。 - 【請求項19】 薬が、クロルプロマジン、ハロペリドール、リスペリドン
、オランザピン、セルチンドール、ジプラジドン、ゾテピンまたはアミスルピリ
ドを含む、定型または非定型神経弛緩薬のいずれかである、請求項16または1
7に記載の調合物。 - 【請求項20】 関節、呼吸系、胃腸系、腎臓、皮膚、生殖系および他のす
べての臓器・器官の炎症性障害の治療のためのものである、請求項1ないし5の
いずれかに記載の薬剤。 - 【請求項21】 主として神経伝達物質の代謝またはレセプターに作用する
薬を、請求項1ないし5のいずれかに記載の薬剤とともに使用することを特徴と
する、精神または神経障害の治療法。 - 【請求項22】治療される疾患が、精神分裂病、分裂情動性障害または分裂
病型障害である、請求項21に記載の治療法。 - 【請求項23】 治療される疾患が、鬱病または双極性障害である、請求項
21に記載の治療法。 - 【請求項24】 治療される疾患が、睡眠障害、不安障害、パニック障害、
対人恐怖症、行為障害、人格障害、および注意および機能亢進障害を含む他の精
神障害である、請求項21に記載の治療法。 - 【請求項25】 治療される疾患が、アルツハイマー病、レーヴィ体病、血
管性痴呆および他の痴呆を含む、いずれかの痴呆である、請求項21に記載の治
療法。 - 【請求項26】 治療される疾患が、パーキンソン病、ハンチントン病、多
発性硬化症、脳卒中、癲癇および他の疾患を含む、いずれかの神経病である、請
求項21に記載の治療法。 - 【請求項27】 精神または神経障害の治療に使用される薬と、請求項1な
いし5のいずれかに記載の薬剤の投与により、精神または神経障害の治療に使用
される薬の副作用を治療または予防するための方法。
