Abstract
Implante intervertebral que comprende un separador (2) en el cual están formadas dos depresiones opuestas (6a, 6b) adecuadas para recibir los dos procesos espinales (26a, 26b) de dos vértebras, cada depresión define dos pestañas (8a, 10a, 8b, 10b) cada una teniendo una pared interior, el implante también tiene una ligadura (4a, 4b) para sostener a dicho separador en dichos procesos espinales, dicho implante comprende: - dicha ligadura está constituida con por lo menos una banda que tiene una porción que rodea una porción de la superficie del proceso espinal opuesto a la parte inferior de la depresión; dicho implante comprende también: - primeros medios de fijación (12a, 12b) formados por lo menos por una de las susodichas pestañas (8a, 8b) para fijar un primer extremo (18a, 18b) de dicha banda; y dicho implante comprende - segundos medios de fijación autoasegurables (14a, 16a, 14b, 16b) formados por lo menos por otra susodicha pestaña (10a, 10b), el segundo extremo (20a, 20b) de dicha bandase inserta a través de los medios de fijación autoasegurables y después se tira para sostener dicha banda (4a, 4b) en posición, asegurando así al separador (2) en los procesos espinales (26a, 26b).
Description
Implante intervertebral.
La presente invención se refiere a un implante
intervertebral que comprende un separador y una ligadura para
sostener el separador entre dos vértebras.
El separador compensa a un disco vertebral cuando
este está deficiente, y en particular el separador limita la
extensión a la cual las porciones posteriores de dos vértebras se
pueden mover una hacia la otra cuando se extiende la espina. Las
porciones posteriores de dos vértebras que se juntan demasiado dan
lugar a la incomodidad, y en particular al dolor.
Cuando un disco vertebral se ha degenerado, los
movimientos de la espina pueden hacer que se dé un contacto entre
vértebras, lo cual comprime las raíces de los nervios que sobresalen
lateralmente entre las vértebras.
Para remediar este inconveniente, se fija por lo
menos un separador entre dos procesos espinales de dos vértebras
consecutivas. Los procesos espinales se proyectan desde las
vértebras en forma de salientes en la porción posterior de la
columna vertebral.
De este modo, poniendo un separador entre dos
procesos espinales sucesivos, se evita que las vértebras entren en
contacto cuando el disco vertebral es deficiente, como muestran las
características del preámbulo de la Reivindicación 1 de la Patente
FR 2,717,675.
Sin embargo, el separador se debe fijar a las
vértebras de una manera que sea lo suficientemente rígida para
mantenerlo en posición a pesar de los movimientos de la espina, y
debe estar lo suficientemente libre con relación a las mismas
vértebras para evitar la rigidez excesiva de la columna vertebral.
Se conoce la fijación de dicho separador, el cual tiene uno o más
agujeros formados en el mismo, por medio de ligamentos insertados
en los agujeros y que forman asas en las cuales se acoplan los
procesos espinales. Este sistema de fijación no es muy rígido y
requiere de un gran número de manipulaciones, aumentando por tanto
el tiempo de la intervención quirúrgica.
Un objetivo de la presente invención es proveer
un implante intervertebral cuyos medios de fijación a los procesos
espinales puedan ser puestos en su lugar en una cantidad de tiempo
que sea relativamente corta, en comparación con la técnica anterior
y que provea una fijación que sea más rígida.
Para lograr este objetivo, la invención provee un
implante intervertebral según la Reivindicación 1 que comprende un
separador en el cual están formadas dos depresiones opuestas
adecuadas para recibir los dos procesos espinales de dos vértebras,
cada depresión define dos pestañas que tiene cada una, una pared
interior, el implante también tiene una ligadura para sostener dicho
separador en los procesos espinales, dicho implante comprende:
- -
- dicha ligadura esta constituida con por lo menos una banda que tiene una porción que rodea a una porción de la superficie del proceso espinal opuesto a la parte inferior de la depresión;
comprendiendo dicho
implante:
- -
- primeros medios de fijación formados en por lo menos una pestaña para fijar un primer extremo de dicha banda; y comprendiendo dicho implante:
- -
- segundos medios de fijación autoasegurables formados en por lo menos otra pestaña, el segundo extremo de dicha banda se inserta a través de dichos medios de fijación autoasegurables y después se tira para sostener a dicha banda en posición, asegurando así el separador a dichos procesos espinales.
Deberá entenderse que el separador se inserta
entre dos vértebras, y que cada proceso espinal que se extiende
desde las mismas se apoya contra una depresión opuesta respectiva
en dicho separador. Las numerosas flexiones de la espina dan lugar
a que cada proceso espinal tenga cierta cantidad de movilidad con
respecto una a la otra, y el separador se mantiene en posición por
medio de una banda que provee un área de contacto con el proceso
espinal, que es más grande que la que proporciona una ligadura del
tipo ligamento.
En consecuencia, la ligadura es sostenida de
manera más segura al proceso espinal. Además, un extremo de la
banda se fija en una de las dos pestañas que constituyen la
depresión y, ventajosamente, los primeros medios de fijación
comprenden una ranura formada a través de dicha pestaña, dicho
primer extremo de la banda es adecuado para acoplarse en dicha
ranura de tal manera que forme un asa, por medio de la cual se
asegura dicho primer extremo de la banda a la pestaña. Este método
de fijación es relativamente fácil de instalar. Así, el extremo de
la banda se fija a dicho separador ligando la punta de la banda a
una porción de su extremo después de que ésta ha pasado a través de
la ranura y que se ha formado un asa. Esta operación se lleva a
cabo antes de instalar el separador entre dos vértebras.
El segundo extremo de la banda se fija a dicha
otra pestaña con medios de fijación autoasegurables que comprenden
de preferencia una primera ranura y una segunda ranura que son
paralelas una a la otra, dichas ranuras están formadas a través de
la otra pestaña, dicha segunda ranura se sitúa entre la primera
ranura y el fondo de la depresión, de tal manera que la porción de
la banda que pasa a través de la primera ranura y que se proyecta
desde dicha pared interior de la otra pestaña, es presionada contra
la porción de la banda que penetra dentro de la segunda ranura,
haciendo posible de esta manera que el extremo de dicha banda sea
asegurado por fricción.
Ventajosamente, dicha ligadura tiene dos bandas y
de esta manera cada depresión tiene primeros medios de fijación en
una de sus pestañas y medios de fijación autoasegurables formados
en su otra pestaña, de tal manera que se puedan fijar las dos
bandas, cada banda siendo adecuada para pasar alrededor de un
proceso respectivo de los procesos espinales, con lo que dichos
procesos espinales son sujetados independientemente uno del
otro.
Deberá entenderse que las bandas son adecuadas
para ser pre-montadas, con el fin de formar un asa
en cada depresión. La punta del segundo extremo de cada banda se
acopla entonces en cada segunda ranura por la pared interior de
cada una de dichas pestañas, y se vuelve a insertar en la depresión
por dicha primera ranura con el fin de que se proyecte desde la
pared interior de la pestaña.
Cuando el separador es insertado entre las
vértebras, los procesos espinales pasan a través de las asas y se
termina la fijación tirando las puntas de las bandas para sostener
firmemente los procesos espinales en las depresiones.
Deberá entenderse también que los medios
autoasegurables se usan durante la operación de apriete. Mientras
más se apriete la banda en el proceso espinal, más grande es la
porción de la banda que se proyecta desde la pared interior de la
pestaña, la cual es presionada contra la porción de la banda que
penetra adentro de la segunda ranura. Así, las dos porciones de la
banda son atrapadas entre la pared interior de la pestaña y el
cuerpo del proceso espinal, evitando así que dicho extremo de la
banda se mueva con relación a la pestaña.
En una primera modalidad, el separador se fija en
paralelo a los procesos espinales por medio de dos bandas
separadas.
En una segunda modalidad, el separador se fija a
los procesos espinales por medio de una sola banda.
Aparecerán otras características y ventajas de la
invención al leer la siguiente descripción que se da por medio de
una indicación no limitativa, y que se hace con referencia a los
dibujos que la acompañan, en donde:
La figura 1 es una vista en perspectiva de una
primera modalidad del implante intervertebral que muestra un
separador provisto con dos bandas para fijar el separador en dos
procesos espinales consecutivos;
La figura 2 es una vista del implante
intervertebral en sección en el plano A-A de la
figura 1, que muestra la posición de los procesos espinales con
respecto al separador;
La figura 3 es una vista en perspectiva de una
segunda modalidad del implante intervertebral ajustado con una sola
banda; y
La figura 4 es una vista en perspectiva de una
tercera modalidad del implante intervertebral fijado con una sola
banda.
Haciendo referencia inicialmente a la figura 1,
se describe una primera modalidad del implante intervertebral, en
donde la ligadura está constituida por dos bandas.
El implante comprende un separador 2 y dos bandas
4a y 4b. El separador 2 generalmente tiene la forma de un
paralelepípedo rectangular que tiene dos extremos, cada uno
teniendo una depresión respectiva 6a y 6b. Estas depresiones son
opuestas una a la otra; son simétricas en un plano de simetría B
del separador, que es perpendicular a los lados grandes del
paralelepípedo, y sus planos de simetría C coinciden. Las
depresiones definen pestañas 8a, 10a, 8b, 10b en las cuales se
proveen medios de fijación 12a, 12b, 14a, 14b, 16a, 16b para fijar
los extremos 18a, 18b, 20a, 20b de las bandas.
Los medios de fijación 12a y 12b son idénticos y
se proveen respectivamente en las pestañas 8a y 8b. En
consecuencia, solamente se describe la fijación del primer extremo
18a de la banda 4a en la pestaña 6a.
El medio de fijación 12a está constituido por un
agujero que atraviesa la pestaña 8a, dicho agujero tiene la forma
de una ranura de dimensiones que son por lo menos iguales a la
sección transversal derecha de la banda 4a, para que sea posible
que ésta se inserte a través del mismo. Esta ranura es
sustancialmente paralela al plano de simetría C de las depresiones
y al plano de simetría B del separador.
Una porción 22a del extremo 18a de la banda 4a se
inserta a través de la pestaña 8a por la ranura 12a y después se
dobla hacia abajo sobre otra porción del extremo de la banda para
formar un asa. La punta del extremo de la banda 4a se liga a dicha
otra porción, de tal manera que se asegure el extremo 18a de la
banda 4a a la pestaña 8a.
El segundo extremo libre 20a de la banda 4a es
adecuado para moverse en el plano A del separador, y en particular
para conectarlo a la pestaña 10a que esta dispuesta de manera
simétrica a la pestaña 8a alrededor del plano de simetría C de las
depresiones.
Los medios de fijación autoasegurables 14a y 16a
para asegurar la banda 4a en la pestaña 10a que tendrá asegurado el
segundo extremo 20a a la misma, se describen a continuación. Por
analogía, esta descripción también se aplica a los medios
autoasegurables que se proveen en la pestaña 10b que es simétrica a
la misma alrededor del plano de simetría B del separador.
Estos medios de fijación del separador están
constituidos por dos agujeros 14a y 16a en forma de ranuras que
pasan a través de la pestaña10a. Estas ranuras son paralelas una a
la otra y de igual manera paralelas a la ranura 12a que está
formada en la primera pestaña 8a. Además, éstas tienen las mismas
dimensiones que dicha ranura 12a.
El segundo extremo 20a de la banda se fija
insertándolo a través de la ranura 16a desde la pared interior 24a
de la depresión 6a. El extremo de la banda 20a se inserta entonces
a través de la ranura 14a desde la cara opuesta de la pared de la
depresión, con el fin de proyectarla dentro de la depresión, y
después se tira para presionarlo contra la porción de la banda que
penetra dentro de la ranura 16a. La banda 4a, que está en
cooperación con la depresión 6a, constituye así una primera asa en
la cual se pueden acoplar los procesos espinales 26a.
En simetría, los medios de fijación antes
descritos son idénticos para la depresión opuesta 6b. De igual
manera, la segunda asa se hace de la misma manera que se describió
anteriormente.
En general, las bandas 4a, 4b están premontadas
en el separador 2 de tal manera que el separador 2 puede ser
insertado directamente entres dos vértebras con las asas ya
formadas siendo colocadas directamente alrededor de los procesos
espinales. Después, la punta 28a del extremo 20a se tira para
apretar el proceso espinal 26a entre la depresión 6a y la banda 4a
de una manera que se asegure el separador 2 del proceso espinal
26a. Como regla general, la tensión en la banda 4a en donde éste
pasa a través de los medios de fijación autoasegurables, es
suficiente para asegurarlo. De la manera en que la banda 4a pasa
sucesivamente a través de las dos ranuras 16a y 14a da lugar a
grandes fuerzas de fricción que deslizan de manera opuesta la banda
4a, en particular debido a los bordes agudos de las ranuras.
Además, la porción de la banda que se proyecta
desde la ranura 14a para proyectarse desde la pared interior 24a, y
la porción de la banda que penetra dentro de la ranura 16a se
comprimen juntas entre la pared interior 24a de la depresión 6a y el
proceso espinal 26a, acentuando de esta manera el aseguramiento
del extremo 20a de la banda 4a de la pestaña 10a del separador
2.
Las fuerzas de fricción y de compresión ejercidas
sobre el extremo 20a de la banda aumentan con el incremento de la
tensión de dicha banda 4a. Sin embargo, si el aseguramiento es
insuficiente, se puede fijar la punta del extremo 20a de la banda a
la porción de la banda que pasa alrededor del proceso espinal 26a,
por medio de un pasador 30a.
El separador 2 se asegura de una manera idéntica
al proceso espinal 26b. El separador 2 se fija así entre dos
vértebras en el proceso espinal 26a y 26b de estas dos
vértebras.
Haciendo referencia a la figura 2, deberá
entenderse que cuando la espina se encuentra en extensión, la
porción inferior 32a del proceso espinal 26a y la porción superior
32b del proceso espinal 26b tienden a moverse una hacia la otra y
entonces se bloquean respectivamente por las partes inferiores de
las depresiones 6a y 6b. Así, en ausencia de un disco vertebral o
en la presencia de un disco que está defectuoso, el separador 2
limita el contacto entre dos vértebras consecutivas.
El separador 2 está hecho generalmente de una
materia que es transparente a los rayos X, con el fin de hacerlo
visible en imágenes de rayos X, para evitar el enmascaramiento de
los órganos a los cuales es preferible poder visualizar. Sin
embargo, para situar el separador con relación a la espina, se
inserta un elemento transversal que es opaco a los rayos X, dicho
elemento es lo suficientemente delgado para evitar impedir la
observación de las imágenes de rayos X, y es recibido en un
alojamiento central 34.
La figura 3 muestra una modalidad variante del
implante intervertebral en donde dicha ligadura comprende una sola
banda 4 cuyo primer extremo 18'a está fijo a dicho medio de fijación
12'a, formado en una primera pestaña 8'a de la primera depresión
6'a, y cuyo segundo extremo 20'b está fijo a dichos medios de
fijación autoasegurables 16'b, 14'b formados en la segunda pestaña
10'b de la segunda depresión 6'b, que es simétrica a la primera
pestaña 8'a de la primera depresión 6'a con relación a dicho
separador 2'.
El separador también tiene medios de guía
formados en dicho separador 2' para guiar a la banda 4' a lo largo
de la segunda pestaña de la primera depresión 6'a y la primera
pestaña de la segunda depresión 6'b, para que dichos procesos
espinales 26'a y 26'b sean apretados de manera simultánea.
En esta modalidad, los medios de guía comprenden
una perforación rectilínea 36 formada en la pared exterior de dicho
separador 2' en línea con la segunda pestaña de la primera
depresión 6'a y la primera pestaña de la segunda depresión 6'b, con
la banda 4' siendo adecuado para deslizarse en dicha perforación.
Para sostener la banda 4' en la perforación 36, se proveen puentes
40'a y 40'b sobre la perforación 36 que coinciden con las
pestañas.
De esta modalidad sirve de manera ventajosa para
que ambos procesos espinales se puedan apretar de manera simultánea
en las depresiones 6'a y 6'b por medio de una sola acción en el
segundo extremo de la banda 4'.
La figura 4 muestra una modalidad variante del
implante intervertebral que tiene una sola banda, en donde los
medios de guía comprenden una ranura 38''a formada a través de la
segunda pestaña de la primera depresión 6''a y una ranura 38''b a
través de la primera pestaña de la segunda depresión 6''b.
Claims (7)
1. Implante intervertebral que comprende un
separador (2) en el cual están formadas dos depresiones opuestas
(6a, 6b) adecuadas para recibir los dos procesos espinales (26a,
26b) de dos vértebras, cada depresión define dos pestañas (8a, 10a,
8b, 10b) cada una teniendo una pared interior, el implante también
tiene una ligadura (4a, 4b) para sostener a dicho separador en
dichos procesos espinales,
dicho implante comprende:
- -
- dicha ligadura está constituida con por lo menos una banda que tiene una porción que rodea una porción de la superficie del proceso espinal opuesto a la parte inferior de la depresión;
dicho implante comprende también:
- -
- primeros medios de fijación (12a, 12b) formados por lo menos por una de las susodichas pestañas (8a, 8b) para fijar un primer extremo (18a, 18b) de dicha banda; y
dicho implante comprende
- -
- segundos medios de fijación autoasegurables (14a, 16a, 14b, 16b) formados por lo menos por otra susodicha pestaña (10a, 10b), el segundo extremo (20a, 20b) de dicha banda se inserta a través de los medios de fijación autoasegurables y después se tira para sostener dicha banda (4a, 4b) en posición, asegurando así al separador (2) en los procesos espinales (26a, 26b).
2. Implante intervertebral de conformidad con la
reivindicación 1, caracterizado además porque los primeros
medios de fijación (12a, 12b) comprenden una ranura formada a
través de dicha pestaña (8a, 8b), dicho primer extremo (18a, 18b)
de dicha banda (4a, 4b) es adecuado para acoplarse en dicha ranura
de tal manera que se forma una asa por medio de la cual dicho
primer extremo (18a, 18b) de dicha banda (4a, 4b) se asegura a
dicha pestaña (8a, 8b).
3. Implante intervertebral de conformidad con la
reivindicación 1 ó 2, caracterizado además porque los medios
de fijación autoasegurables (14a, 16a, 14b, 16b) comprenden una
primera ranura (14a, 14b) y una segunda ranura (16a, 16b) que son
paralelas una a la otra, dichas ranuras están formadas a través de
la otra pestaña (10a, 10b), dicha segunda ranura (16a, 16b) se
sitúa entre dicha primera ranura (14a, 14b) y la parte inferior de
la depresión (6a, 6b) de manera que la porción de la banda que pasa
a través de la primera ranura y que se proyecta desde la pared
interior de dicha otra pestaña, es presionada contra la porción de
la banda que penetra en la segunda ranura, haciendo posible de esta
manera que el extremo de dicha banda sea asegurado por
fricción.
4. Implante intervertebral de conformidad con
cualquiera de las reivindicaciones 1 a 3, caracterizado
además porque cada depresión (6a, 6b) tiene primeros medios de
fijación (12a, 12b) en una de sus pestañas (8a, 8b) y segundos
medios de fijación autoasegurables (14a, 16a, 14b, 16b) formados en
su otra pestaña (10a, 10b) con el fin de hacer posible que las dos
bandas (4a, 4b) sean fijadas, cada banda es adecuada para pasar
alrededor de un proceso respectivo de los procesos espinales (26a,
26b), con lo que dichos procesos espinales (26a, 26b) son
sostenidos de manera independiente uno del otro.
5. Implante intervertebral con cualquiera de las
reivindicaciones 1 a 3, caracterizado además porque una
primera pestaña (18a) de la primera depresión (6'a) tiene primeros
medios de fijación (12'a, 12'b) para fijar el primer extremo de la

depresión (6'b), con lo que dichos procesos espinales (26a, 26b)
son apretados de manera simultánea.
6. Implante intervertebral de conformidad con la
reivindicación 5, caracterizado además porque los medios de
guía comprenden una ranura (38''a) formada a través de la segunda
pestaña de la primera depresión (6''a) y una ranura (38''b) formada
a través de la primera pestaña de la segunda depresión (6''b).
7. Implante intervertebral de conformidad con la
reivindicación 5, caracterizado además porque los medios de
guía comprenden una perforación rectilínea (36') formada en la
pared exterior del separador (2') en línea con la segunda pestaña
de la primera depresión (6'a) y c n la primera pestaña de la
segunda depresión (6'b), y en donde dicha banda (4') es adecuada
para deslizarse.
