Abstract
Description
- Die Erfindung betrifft ein Verdünnungssystem zur Verdünnung hochviskoser Lösungen aus Blutbestandteilen, bestehend aus einem Behälter zur Aufnahme der Verdünnungsflüssigkeit und einem Verbindungselement zum Transport der Verdünnungsflüssigkeit zu einem als Blutkonserve bezeichneten Behälter zur Aufnahme der hochviskosen Lösungen aus Blutbestandteilen.
- In der klinischen Praxis ist die Blutkomponententherapie als gezielte Substitution von zellulären Blutelementen und Plasmabestandteilen ein bekanntes Verfahren. Jedoch ist die Vollbluttransfusion bis heute - obwohl von verschiedenen Wissenschaftlern als obsolet bezeichnet - in nur geringem Umfang durch die Transfusion von Erythrozytenkonzentrat ersetzt worden. Die Befürworter der Vollbluttransfusion führen gegen die Transfusion von Erythrozytenkonzentrat stets als einen wesentlichen Nachteil an, daß die hohe Viskosität von Erythrozytenkonzentrat - und die damit notwendige Verdünnung desselben - technische Probleme mit sich bringe. Die in den Kliniken üblichen Verfahren zur Verdünnung von Erythrozytenkonzentrat sind in der Tat auch heute noch zeitaufwendig, umständlich und hygienisch nicht immer optimal, so daß die Notwendigkeit der Verdünnung des Erythrozytenkonzentrats - obwohl auf dem ersten Blick nebensächlich - als ein entscheidener Nachteil anzusehen ist. Als eine in der klinischen Praxis nicht befriedigende Verfahrensanordnung zur Verdünnung des Erythrozytenkonzentrats sei hier beispielhaft die Verbindung des Behälters zur Aufnahme der Verdünnungsflüssigkeit mit der Blutkonserve mittels eines auf beiden Seiten mit Einstichdornen versehenen Schlauches erwähnt.
- "Der Erfindung liegt daher die Aufgabe zugrunde, ein Verdünnungssystem zur Verdünnung von hochviskosen Lösungen aus Blutbestandteilen zu schaffen, das in der klinischen Praxis die oben erwähnten Nachteile beseitigt und insbesondere folgenden Anforderungen gerecht wird:
- 1. Die hygienische Sicherheit muß gewährleistet sein
- 2. Das Verdünnungssystem muß einfach im Aufbau und praktikabel in der Anwendbarkeit sein.
- Diese Aufgabe wird erfindungsgemäß dadurch gelöst, daß der Behälter zur Aufnahme der Verdünnungsflüssigkeit und das Verbindungselement eine Einheit bilden, wobei das Verbindungselement mit einem Einstichdorn und einem gegenüber der Umgebung steril gehaltenen von außen zu öffnenden Verschluß versehen ist. Weitere Merkmale des erfindungsgemäßen Verdünnungssystem sind Gegenstand der Unteransprüche.
- Ein Ausführungsbeispiel der Erfindung ist in der Zeichnung dargestellt und wird im folgenden näher beschrieben:
- Es zeigen
- Figur 1 eine Ansicht des Verdünnungssystems, bestehend aus einem Behälter und einem Verbindungselement, eingebettet in einen steril gehaltenen, das Verdünnungssystem gegen die Umgebung abschließenden Umbeutel.
- Figur 2 eine Seitenansicht des Verbindungselements.
- Gemäß Figur 1 ist in einen zylinderförmigen Behälter 1 aus deformierbarem Kunststoffmaterial ein durchsichtiger
- Schlauch 2, vorzugsweise aus Kunststoff gefertigt, eingesteckt, der mit dem Behälter 1 verschweißt ist. In den Schlauch 2 ist das nachstehend detailliert beschriebene Verbindungselement - siehe Figur 2 - eingepreßt. Das Verbindungselement besteht aus zwei Teilen, vorzugsweise aus Kunststoff gefestigt, dem Einstichdorn 5 und dem Verschlußteil 6 sowie einer innen im Verschlußteil 6 liegenden Kugel 7., vorzugsweise aus Stahl gefertigt. Der Einstichdorn 5 besteht aus einem außen leicht konischen Teil 8 auf der einen Seite und einem zylindrischen Teil 9 mit einer kurzen Verdickung 10 auf der anderen Seite. Beide Teile sind durch einen Bund 11, der auf der Seite des zylindrischen Teils 9 leicht hinterdreht ist, unterteilt. Der konische Teil 8 hat an dem anschließenden Bund 11 seinen größten Durchmesser. Die Verdickung 10 schließt ebenfalls an den Bund 11 an. Der Einstichdorn 5 ist auf seiner gesamten Länge hohl gebohrt. Der zylindrische Teil 9 ist vorne von zwei Seiten aus keilförmig angeschrägt, während sich am Ende des konischen Teils 8 gleichmäßig am Umfang verteilt vier gleichgeformte Höcker 12 befinden. Der Verschlußteil 6 besteht aus drei ineinander übergehende jeweils im Durchmesser größer werdende Zylinder 13, 14, 15 mit einem anschließenden Bund 16. Die beiden größeren Zylinder 14, 15 sind bis zum Übergang des mittleren Zylinders 14 zum kleinsten Zylinder 13 hohlgebohrt, und zwar der Zylinder 14 mit demselben Innendurchmesser wie der Einstichdorn 5 und der Zylinder 15 mit einer konischen Bohrung, deren Steigung exakt der Steigung des konischen Außendurchmessers des Einstichkorns 5 entspricht. Beim Übergang der konischen Bohrung in die zylindrische Bohrung ist die zylindrische Bohrung mit einer kugelförmigen Ansenkung versehen.
- Verschlußteil 6 und Einstichdorn 5 sind über die konische Innenbohrung des Verschlußteils 6 und die konische Außenform des Einstichdorns 5 zu dem Verbindungselement 3 zusammengepreßt. In den konisch gebohrten Teil des Verschlußteils 6 wird vor dem Zusammenpassen der 2 Einzelteile die Kugel 7 eingelegt, deren Durchmesser einerseits größer als die zylindrische Bohrung und andererseits kleiner als der kleine Durchmesser der konischen Bohrung ist und deren Durchmesser der oben erwähnten Ansenkung entspricht. Durch diese Art der Konstruktion ist die Kugel 7 im konisch gebohrten Teil des Verschlußteils 6 beweglich gelagert. Durch Einstecken des zusammengefügten Verbindungselements 3 in den Schlauch 2 des Behälters 1 - der Zylinder 15 des Verschlußteils 6 bildet mit dem Schlauch 2 eine luftdicht abschließende Verbindung - sind Behälter 1 und Verbindungselement 3 zu einer Einheit, dem Verdünnungssystem, zusammengefügt. Der Schlauch 2 ist gerade so lang, daß im zusammengefügten Zustand der Zylinder 13, der nicht durchbohrt und leicht abbrechbar ist, im Schlauch 2 und nicht im Behälter 1 liegt.
- Die Art der Anwendung des erfindungsgemäßen Verdünnungssystems wird nachstehend kurz beschrieben. Der mit Verdünnungsflüssigkeit, beispielsweise 0,9iger Kochsalzlösung, Ringerlösung oder Ringerlactat gefüllte Behälter 1 wird mittels des Einstichdorns 5 mit der Blutkonserve verbunden. Andererseits wird der Empfängerorganismus mittels eines Transfusionsgerätes ebenfalls mit der Blutkonserve verbunden. Nach Herstellung dieser Verbindung Behälter 1/Empfängerorganismus wird der unter dem Schlauch 2 liegende Zylinder 13 des Verbindungselements 3 abgebrochen und die in Behälter 1 liegende Verdünnungsflüssigkeit durch Zusammendrücken des Behälters 1 in die Blutkonserve übergeleitet; dadurch wird das Erythrozytenkonzentrat verdünnt und das verdünnte Erythrozytenkonzentrat wird über das Transfusionsgerät dem Empfängerorganismus zugeführt. Die im Verbindungselement 3 lagernde KUfel 7 wirkt dabei als Rückschlagventil. Beim Leerdrilcken des Fehälters 1 wird die Kugel 7 gegen die Höcker 12 gedrückt und die Verdünnungsflüssigkeit wird zwischen den Höckern 7 hindurchgedrückt; ein Rückfluß in den Behälter 1 wird dadurch verhindert, daß sich bei einem eventuellen Rückfluß die Kugel 7 sofort in die oben erwähnte kugelförmige Ansenkung hineinlegt.
- Die mit der Erfindung erzielten Vorteile bestehen insbesondere darin, ein Verdünnungssystem zur Verdünnung hochviskoser Lösungen aus Blutbestandteilen zu schaffen, das speziell in Notfällen einfach und schnell anwendbar ist.
- Durch Sterilisation des Verdünnungssystems im Umbeutel ist die hygienische Sicherheit gewährleistet.
