Definitions

  • the invention relates to a device for supplying an artificial urinary output, which can both close the exit and collect the body excretion.
  • Carcinomas, malformations or nerve-related dysfunctions can make it necessary, for example, to have an artificial urine outlet.
  • the drainage of the urine via the ureter that leads directly to the outside has proven to be unfavorable, since this can easily lead to stenosis or infection.
  • This reservoir is usually emptied into a bag glued to the outside of the stoma, for example according to DE-BZ-1 111 776, which has to be replaced or emptied several times a day.
  • the aim is therefore to use the colon section, which has a capacity of approximately 100 to 300 ml, to create a largely genuine bladder replacement by means of a novel supply or closure device, which makes it unnecessary to carry a conspicuous bag.
  • a device made of at least one liquid-tight plastic film which is coated on one side with a skin-compatible self-adhesive, and in its simplest embodiment is characterized in that it is surface-shaped and consists of only one film layer made of an air and water vapor permeable material , highly stretchable film made of a thermoplastic polymer, which, glued over the stoma, forms a largely genuine bladder replacement due to its elasticity.
  • it is usually covered on the non-adhesive side with a weakly coated, supportive auxiliary carrier.
  • the most important feature of the film in addition to the necessary impermeability to moisture, is high elasticity (elongation at break), which must be in the range of at least about 300 to 700, preferably 500 to 700%. If such a film, which is attached to a reinforcing subcarrier to make it easier to handle and should be approximately 10 x 10 to 15 x 15 cm in size, is stuck over the stoma hole (after which the subcarrier is removed), it hermetically seals the exit.
  • the foil gives way due to its high elasticity, inflates like a small balloon in front of the stoma hole and creates an additional volume of about 25 ml to absorb the liquid without loosening its adhesive on the skin. The patient then feels that emptying is necessary.
  • polyurethane films are particularly suitable for this purpose, as described, for example, in US Pat. No. 2,871,218, made of a thermoplastic, largely linear, ie uncrosslinked, polyurethane with a thickness of approximately 40-150 ⁇ m, preferably approximately 70-80 ⁇ m.
  • Such films are extremely pliable and can adapt to any unevenness and movement of the skin around the stoma, which is necessary for a good seal, and give in slightly to the pressure of the liquid from the inside without tearing (elongation at break approx. 500-700% ).
  • they have a comparatively high water vapor permeability (e.g.
  • the films can be produced by extrusion, by blowing or preferably by solvent casting.
  • foils made of polyurethane In addition to these particularly advantageous foils made of polyurethane, corresponding foils made of other materials, such as B. butadiene-styrene block copolymers or natural rubber, provided that they only meet the properties shown.
  • the device according to the invention is designed in such a way that the film to be glued has an opening in the middle corresponding to the size of the stoma - the stoma diameter is usually approximately 22-25 mm - and a second film made of a highly stretchable thermoplastic polymer , which is preferably somewhat smaller than the base film, is attached concentrically on the outside of this film by means of a weld seam.
  • Such a device has, compared to a simple embodiment, an increased urinary absorption capacity corresponding to the size of the second film, which is now inflated within the confining welding ring.
  • the same film can be used as the base film as already described in the simple form or else, for example, a thin polyurethane film of only about 28 fJ.m thick and one higher water vapor permeability of about 600-650 g / m2. 24 hours. It is essential that the film is skin-friendly, conformable and weldable to the second, lumen-forming film.
  • the second film is the same material as described in the simple embodiment.
  • a third film acting as a non-return valve, is provided between the base film to be adhered to the body and the inflatable second film welded thereon, which film is also arranged concentrically and is somewhat larger than the opening for the stoma and is connected to the base film by means of several individual, preferably welded, fastening points.
  • valve film can in principle consist of a wide variety of materials provided that they can only be welded to the base film, but preferably it is also a thin thermoplastic polyurethane film of about 40 to 150 fJ.m thick like the other two films.
  • the devices according to the invention are very thin and not very stiff due to the film used, they each require on their outer side a supporting support made of a firmer film or paper, which also serves as an application aid.
  • This auxiliary carrier is coated with weak adhesive and can be easily removed again after the device has been attached. It is advantageously provided here with a small tab, for example in the form of a folded edge.
  • the side of the base film of the devices facing the skin is coated with one of the customary, highly adhesive self-adhesive compositions based on rubber / resin or synthetic polymers, preferably acrylic ester polymers or copolymers, which may additionally contain hydrophilic groups and / or hydrophilic substances such as hydrocolloids.
  • the adhesive layer should be covered with an easily removable protective sheet made of, for example, siliconized paper.
  • the devices are generally glued directly onto the skin, but they can also be glued onto an interposed so-called skin protection film which remains glued around the stoma for a long time.
  • Figure 1 shows approximately true to scale the device according to the invention in its simple embodiment from above, 3 mean the submount and 10 the handle tab.
  • FIG. 2 shows a cross section through the device according to FIG. 1, where 1 denotes the highly stretchable carrier film, 2 the self-adhesive layer applied thereon, 3 the auxiliary carrier and 9 the protective sheet.
  • FIG. 3 shows, approximately to scale, the device according to the invention in its second embodiment from above, the auxiliary carrier having already been removed.
  • 1 again means the carrier film, 4 the opening for the stoma, 5 the highly stretchable second film and 6 the weld seam.
  • FIG. 4 shows a cross section through the device according to FIG. 3, 1 meaning the backing film, 2 the self-adhesive layer, 3 the auxiliary backing, 4 the opening, 5 the highly stretchable second film, 6 the weld seam and 9 the protective sheet for the adhesive layer.
  • FIG. 5 shows, approximately to scale, the device according to the invention in its third embodiment from above, the subcarrier again being removed.
  • 1 again means the carrier film, 4 the opening for the stoma, 5 the highly stretchable second film, 6 the weld seam, 7 the third film (valve film) located between the base film 1 and lumen-forming film 5, and 8 the fastening points (welding points) , by means of which the film 7 is attached to the carrier film 1.
  • FIG. 6 shows a cross section through the device according to FIG. 5, with 1 the carrier film, 2 the self-adhesive layer, 3 the auxiliary carrier, 4 the opening, 5 the highly stretchable second film, 6 the weld seam, 7 the valve film, 8 the weld points and 9 the Protective sheet for the adhesive layer mean.
  • the devices according to the invention allow the patient to achieve practically perfect continence again. As long as the amount of urine excreted does not exceed the volume of the intestinal volume, they close the stoma opening smoothly and tightly. However, if it is not possible for the person concerned to ensure that they are emptied in good time, they offer a sufficiently large collection volume without the unnecessary empty space and the unpleasant noise generated as with conventional ureterostomy bags.
