- Impfstoffe zur Prophylaxe von Hepatitis A- und Hepatitis B-Infektionen sind allgemein bekannt. Zum Beispiel wird der Impfstoff Engerix-B (Marke) von SmithKline Beecham Biologicals zur Prävention von Hepatitis B verwendet. Dieser Impfstoff umfaßt Hepatitis B-Oberflächenantigen (spezifisch das S-Antigen mit 226 Aminosäuren, beschrieben in Harford et al., Postgraduate Medical Journal, 1987, 63 (Suppl 2) 65–70) und wird unter Verwendung von Aluminiumhydroxid als Hilfsstoff formuliert. Der Impfstoff Havrix (Marke), auch von SmithKline Beecham Biologicals, kann zur Prävention von Hepatitis A-Infektionen verwendet werden und wird auch mit Aluminiumhydroxid als Hilfsstoffs formuliert. Dieser Impfstoff umfaßt einen abgeschwächten Stamm des HM-175 Hepatitis A-Virus, inaktiviert mit Formol (Formaldehyd); siehe Andre et al. [Prog. Med. Virol. 1990, Bd. 37; S. 72–95]. Der Impfstoff Twinrix (Marke), der eine Kombination aus den obigen Hepatitis A- und Hepatitis B-Antigenen ist, kann zum gleichzeitigen Schutz gegen Hepatitis A und Hepatitis B verwendet werden.
- WO 96/26741 (SmithKline Beecham) beschreibt eine Impfstofformulierung, die eine Hepatitis B-Komponente, insbesondere Hepatitis B-Oberflächenantigen, in Kombination mit Aluminiumphosphat und 3-des-O-acyliertem Monophosphoryllipid A umfaßt.

EP 0 339 667 (Chemo Sero) beschreibt das allgemeine Konzept der Kombination eines Hepatitis A-Antigens und eines Hepatitis B-Antigens zur Herstellung eines Kombinationsimpfstoffs. In der Beschreibung wird angegeben, daß der Hilfsstoff; der verwendet wird, nicht kritisch ist: er muß nur die Immunaktivität in einem gewünschten Ausmaß steigern können und darf keine Nebenwirkungen verursachen. Es wird angegeben, daß Aluminiumgel verwendet werden kann, insbesondere Aluminiumhydroxidgel und Aluminiumphosphatgel. - WO 93/24148 (SmithKline Beecham) beschreibt die Herstellung von Impfstoffen, die Hepatitis B-Oberflächenantigen umfassen, worin Aluminiumphosphat als Hilfsstoff verwendet wird. Multivalente Kombinationsimpf stoffe, die gegebenenfalls ein Hepatitis A-Antigen enthalten können, werden beschrieben. Die Verwendung von Formol wird nicht offenbart.

EP 0 633 784 (SmithKline Beecham) beschreibt neue Impfstofformulierungen, die eine Hepatitis B-Komponente, insbesondere Hepatitis B-Oberflächenantigen, in Kombination mit Aluminiumphosphat und 3-des-O-acyliertem Monophosphoryllipid A umfassen. - F. Ambrosh et al. (Journal of Medical Virology (1994) 44:452–456) beschreiben monovalente und kombinierte Impfstofformulierungen gegen Hepatitis A und/oder B, die Aluminiumhydroxid umfassen, zur Immunisierung von jungen Erwachsenen in einem 3-Dosis-Schema.
- Es wurde jetzt überraschend festgestellt, daß Impfstoffe, die Hepatitis B- und/oder Hepatitis A-Antigene umfassen, außergewöhnlich gute Ergebnisse ergeben, falls der Impfstoff in einer spezifischen Weise formuliert wird.
- Unter Verwendung der erfindungsgemäßen Impfstofforumulierungen ist es möglich, die Impfstoffe in einem 2-Dosis- anstelle eines 3-Dosis-Schemas zu verabreichen.
- In einem ersten Aspekt der Erfindung wird die Verwendung einer wäßrigen Impfstoffzusammensetzung, die:
Hepatitis B-Oberflächenantigen;
ein inaktiviertes Hepatitis-A-Virus (HAV);
und Formol umfaßt,
worin 0,015–0,1 mg Aluminiumphosphat pro μg Hepatitis B-Oberflächenantigen und 0,05–0,10 mg Aluminiumhydroxid pro ml vorhanden sind,
in der Herstellung einer immunprotektiven Hepatitis A- plus B-Zusammensetzung zur Verabreichung an Jugendliche oder Kinder in einem 2-Dosis-Schema bereitgestellt. - Bevorzugt ist das Aluminiumphosphat im Bereich von 0,02 bis 0,08 mg pro μg HBsAg.
- Das Hepatitis A-Antigen ist bevorzugt der im kommerziellen Produkt Havrix (SmithKline Beecham Biologicals) verwendete HM-175-Stamm.
- Die Konzentration von Hepatitis A-Antigen in der Impfstofformulierung der Erfindung ist bevorzugt ca. 720–2880 EU-Einheiten pro ml. Zur Definition von EU-Einheiten siehe Andre et al. (1990), loc. cit.
- In der Impfstoffzusammensetzung der Erfindung ist es vorteilhaft, Formol (Formaldehyd) hinzuzugeben, so daß die Formolkonzentration 10–200 μg/ml ist.
- Bevorzugt beträgt die Formolkonzentration ca. 20–160 μg/ml.
- Normalerweise wird das Hepatitis B-Antigen Hepatitis B-Oberflächenantigen (HBsAg) sein. Die Herstellung von Hepatitis B-Oberflächenantigen (HBsAg) ist gut dokumentiert. Siehe z.B. Harford et al., Develop. Biol. Standard 54, S. 125 (1983), Gregg et al., Biotechnology 5, S. 479 (1987), EP-A-0 226 846, EP-A-0 299 108 und Verweise darin.
- Wie hier verwendet schließt der Ausdruck "Hepatitis B-Oberflächenantigen" oder "HBsAg" jedes HBsAg-Antigen oder Fragment davon ein, das die Antigenität von HBV-Oberflächenantigen zeigt. Es versteht sich, daß zusätzlich zur Sequenz mit 226 Aminosäuren des HBsAg-S-Antigens (siehe Tiollais et al., Nature 317, 489 (1985) und Verweise darin) HBsAg wie hier beschrieben nach Wunsch die gesamte oder einen Teil einer Prä-S-Sequenz wie in den obigen Verweisen und in EP-A-0 278 940 beschrieben enthalten kann. HBsAg wie hier beschrieben kann auch Varianten bezeichnen, zum Beispiel die in WO 91/14703 beschriebene "Escape"-Mutante. In einem weiteren Aspekt kann das HBsAg ein als SL* in
EP 0 414 374 beschriebenes Protein umfassen, das heißt ein Protein, dessen Aminosäuresequenz aus Teilen der Aminosäuresequenz des großen (L) Proteins des Hepatitis B-Virus (ad- oder ay-Subtyp) besteht, dadurch gekennzeichnet, daß die Aminosäuresequenz des Proteins aus entweder: - (a) Resten 12–52, gefolgt von Resten 133–145, gefolgt von Resten 175–400 des L-Proteins; oder
- (b) Rest 12, gefolgt von Resten 14–52, gefolgt von Resten 133–145, gefolgt von Resten 175–400 des L-Proteins besteht.
- HBsAg kann auch in
EP 0 198 474 oderEP 0 304 578 beschriebene Polypeptide bezeichnen. - Normalerweise wird das HBsAg in Partikelform sein. Es kann S-Protein allein umfassen oder kann als Verbundpartikel vorliegen, zum Beispiel (L*, S), worin L* wie oben definiert ist und S das S-Protein von Hepatitis B-Oberflächenantigen bezeichnet.
- Bevorzugt wird das HBsAg an Aluminiumphosphat adsorbiert, wie in WO 93/24148 beschrieben.
- Bevorzugt ist das Hepatitis B-Antigen das HBsAg-S-Antigen wie im kommerziellen Produkt Engerix-B (SmithKline Beecham Biologicals) verwendet.
- Die erfindungsgemäßen Impfstofformulierungen werden eine immunprotektive Menge der Antigene enthalten und können durch herkömmliche Techniken hergestellt werden. Die Impfstoffherstellung wird allgemein beschrieben in New Trends and Developments in Vaccines, herausgegeben von Voller et al., University Park Press, Baltimore, Maryland, USA 1978. Die Verkapselung in Liposomen wird zum Beispiel beschrieben von Fullerton, US-PS 4,235,877. Die Konjugation von Proteinen mit Makromolekülen wird zum Beispiel offenbart von Likhite, US-PS 4,372,945, und von Armor et al., US-PS 4,474,757.
- Die Impfstoffzusammensetzungen der Erfindung werden bevorzugt in einem 0/6-Monatsschema, das heißt eine erste Dosis zum Monat 0 und eine zweite Dosis zum Monat 6.
- Die folgenden Beispiele dienen zum Zweck der weiteren Veranschaulichung.
- Beispiele
- Beispiel 1. Spezifische Formulierungen
- Spezifische Formulierungen im Umfang der Erfindung schließen die folgenden ein:
- a) Eine Impfstoffzusammensetzung zur Verabreichung an Jugendliche und/oder Kinder, die folgendes umfaßt: 20 μg HBsAg 720 EU HAV 0,45 mg Aluminiumsalz (0,4 mg Aluminiumphosphat plus 0,05 mg Aluminiumhydroxid) 80 μg Formol in einer 1 ml-Dosis.
- b) Eine Impfstoffzusammensetzung zur Verabreichung an Jugendliche und/oder Kinder, die folgendes umfaßt: 20 μg HBsAg 720 EU HAV 0,45 mg Aluminiumsalz (0,4 mg Aluminiumphosphat plus 0,05 mg Aluminiumhydroxid) 80 μg Formol in einer 0,5 ml-Dosis.

- HAB 2-Dosis-Programm mit Kindern im Alter von 1–11 Jahren
- Impfstoffzusammensetzung umfaßt:
- 720 HAV EU/20 μg HBsAg
- 0,45 mg Al-Salz
- 80 μg Formol
- in einer 1 ml-Dosis mit einem Schema von 0/1 Monaten.
- Ergebnisse sind in Tabelle 5 gezeigt.
- Zusammenfassung
- Die in den obigen Beispielen gezeigten klinischen Ergebnisse weisen deutlich darauf hin, daß eine zufriedenstellende Immunreaktion erhalten wird, sowohl für Hepatitis A als auch für Hepatitis B nach dem Füllschema, d.h. zum Monat 7, bei Kindern und Jugendlichen.
Claims (8)
- Verwendung einer wässrigen Impfstoffzusammensetzung, die Hepatitis B-Oberflächenantigen; ein inaktiviertes Hepatitis A-Virus (HAV); und Formol umfasst, worin 0,015–0,1 mg Aluminiumphosphat pro μg Hepatitis B-Oberflächenantigen und 0,05–0,10 mg Aluminiumhydroxid pro ml vorhanden sind, zur Herstellung einer immunprotektiven Heptatitis A- plus B-Zusammensetzung zur Verabreichung an Jugendliche oder Kinder in einem 2-Dosis-Schema.
- Verwendung gemäß Anspruch 1, worin die Formolkonzentration 10–200 μg pro ml beträgt.
- Verwendung gemäß Anspruch 2, worin die Konzentration von HAV 720–880 EU pro ml beträgt.
- Verwendung gemäß Anspruch 3 zur Verabreichung an Jugendliche und/oder Kinder, die Folgendes umfasst oder daraus besteht: 20 μg HBsAg, 720 EU HAV, 0,45 mg Aluminiumsalz, bestehend aus 0,4 mg Aluminiumphosphat plus 0,05 mg Aluminiumhydroxid, 80 μg Formol, in einer 1 ml-Dosis.
- Verwendung gemäß Anspruch 3 zur Verabreichung an Jugendliche und/oder Kinder, die Folgendes umfasst oder daraus besteht: 20 μg HBsAg, 720 EU HAV, 0,45 mg Aluminiumsalz, bestehend aus 0,4 mg Aluminiumphosphat plus 0,05 mg Aluminiumhydroxid, 80 μg Formol in einer 0,5 ml-Dosis.
- Verwendung gemäß jedem vorhergehenden Anspruch, worin das Hepatitis B-Oberflächenantigen das S-Antigen ist.
- Verwendung gemäß jedem vorhergehenden Anspruch, worin das Hepatitis A-Antigen aus dem HM-175-Stamm abgeleitet ist.
- Verwendung gemäß jedem vorhergehenden Anspruch, worin die 2 Dosen zum Monat 0 und 6 verabreicht werden.
