Description
- Diese Erfindung betrifft ein chirurgisches Textilimplantat.
- Die Herstellung von chirurgischen Textilimplantaten macht es oft erforderlich, daß geringe Stückzahlen hergestellt werden. Moderne Textilherstellungsverfahren jedoch sind im allgemeinen nur dann kosteneffektiv, wenn große Stückzahlen produziert werden. Ein Vorteil der automatischen Herstellungsverfahren mit großen Stückzahlen ist, daß die hergestellten Artikel fast genau identisch sind. Diese Gleichartigkeit ist ein Erfordernis der modernen wissenschaftlichen Chirurgie, damit alle Patienten gleich behandelt werden und von Implantat zu Implantat keine Abweichung auftritt, die von Nachteil ist.
- Es ist daher wünschenswert, daß die Möglichkeit besteht, Textilimplantate kosteneffektiv in kleinen Stückzahlen auf Maschinen herzustellen, die mechanisch oder elektronisch derart gesteuert sind, daß jeder in derselben Ausführung hergestellte Artikel praktisch identisch ist.
- Es ist auch wünschenswert, daß das angewandte Konstruktionsverfahren einfach und schnell durchzuführen ist, um die Konstruktionskosten zu minimieren. Niedrige Konstruktionskosten erleichtern auch die kosteneffektive Produktion von nach Maß angefertigten Implantaten, die für ungewöhnliche medizinische Anwendungen oder zur Verwendung bei Patienten wünschenswert sein können, bei denen es erforderlich ist, ein Implantat einer genauen Größe herzustellen, das für diese Person paßt.
- Vorzugsweise sind bei einem Textilimplantat die textilen Fasern in einer Position und Richtung angeordnet, die mit den Konstruktionserfordernissen übereinstimmen, so daß sie ihre Funktion fehlerfrei erfüllen können, ob diese nun ein tragende oder sonstwie gestaltet ist.
- Mit der vorliegenden Erfindung sollen die obengenannten Probleme behandelt werden.
- Die Druckschrift WO 9317635 A offenbart eine zusammengesetzte Prothese, die ein Textilimplantat mit einer Anordnung von Fasern und eine Adhäsionsbarriere enthält. Das Implantat und die Barriere werden durch Kleben, Nähen oder Formgießen verbunden.
- Gemäß der vorliegenden Erfindung ist ein chirurgisches Textilimplantat in Kombination mit einem Grundgewebe geschaffen, wobei das Implantat eine Anordnung von Fasern umfaßt, dadurch gekennzeichnet, daß das Implantat dadurch hergestellt wird, daß die das Implantat bildenden Fasern durch Sticken auf das Grundgewebe aufgebracht werden.
- Der hier verwendete Ausdruck Implantat ist nicht nur auf Implantate beschränkt, die chirurgisch oder in sonstiger Weise in den Körper eingesetzt werden sollen, sondern umfaßt auch Stents und dergleichen, die in die Speiseröhre eingesetzt werden.
- Sticken wird normalerweise als Arbeit mit Nadel und Faden auf Stoff definiert. Bei der herkömmlichen Stickerei werden die Fäden mit einer Nadel nach den Vorgaben des gewählten ästhetischen Designs plaziert. Bei der Herstellung eines chirurgischen Textilimplantats können die Fäden gemäß den funktionellen Erfordernissen der Konstruktion plaziert werden, beispielsweise so, daß ein oder mehrere Fäden in der Struktur in wirksamer Weise eine Last in einem künstlichen Band tragen können.
- Viele moderne Stickmaschinen umfassen einen Nähkopf über einem XY-Plotter. Der Plotter kann mit großer Genauigkeit unter dem Nadelkopf bewegt werden (oder umgekehrt), so daß sich der Nähkopf in der erforderlichen Weise relativ zu dem Grundgewebe bewegt. Die Bewegungen werden von einer Lochkarte oder vorzugsweise von einer elektronischen Computersteuerung gesteuert. Bei einer alternativen Anordnung bewegt sich der Nähkopf, während das Grundgewebe stationär bleibt.
- Vorzugsweise ist das Implantat im wesentlichen von dreieckiger Form.
- Die Definition von Stickerei beinhaltet, daß die Stickereistiche auf ein Grundgewebe aufgebracht werden müssen. Gemäß einer Ausführungsform der vorliegenden Erfindung ist jedoch ein Grundgewebe vorgesehen, das löslich ist, so daß nach Ausbilden des Implantats auf dem Grundgewebe das Gewebe aufgelöst werden kann und nur die gestickte Struktur zurückbleibt. Bei Anwendung von Konstruktionsregeln, die dem Fachmann geläufig sind, können gestickte Strukturen so hergestellt werden, daß plazierte Fäden der Stickerei ihre struktu relle Integrität beibehalten und zusammenhalten, nachdem das Grundgewebe aufgelöst worden ist. Die Auflösung kann durch ein wässriges Medium erfolgen, wenn ein wasserlösliches Grundgewebe verwendet wird, wie z. B. ein Grundgewebe aus Polyvinylacetat oder Alginat. Alternativ kann ein Lösungsmittel wie z. B. Aceton benutzt werden, wenn ein Acetat-Grundgewebe verwendet wird. Es können andere Grundgewebe verwendet werden, die z. B. mit einem sauren, alkalischen oder organischen Lösungsmittel oder mit Wasser oder durch Wärme oder ein sonstiges Verfahren entfernt werden können.
- Bei einer Ausführungsform der Erfindung wird ein Flachmaterial, beispielsweise aus Polyglykolsäure, verwendet, das sich nach der Implantation abbaut oder absorbiert wird, wobei es beständigere Teile der gestickten Struktur im Körper zurückläßt. Das Implantat oder Grundgewebe kann mit einem oder mehreren Wachstumsfaktoren oder gefäßbildenden oder neurogenen Stoffen imprägniert sein, die die Bildung von Blutgefäßen, Nerven oder anderen Gewebearten um und/oder in dem Implantat anregen können.
- Im folgenden sind spezielle Ausführungsformen der Erfindung unter Bezugnahme auf die beigefügten Zeichnungen beschrieben. Es zeigen:
-
1 eine Draufsicht auf eine Netzstruktur; -
2 eine Draufsicht auf eine weitere Netzstruktur, die zur Wiederherstellung einer Sehne oder eine Bandes geeignet ist; -
3 einen schlauchförmigen Stent zur Einführung in hohle innere Organe; und -
4 ein Material, das unter anderem für den in3 gezeigten Stent verwendet werden kann. - In
1 umfaßt eine Netzstruktur sich kreuzende Fäden, die im wesentlichen rechtwinklig zueinander angeordnet sind. Die Verkreuzung kann durch eine Anzahl von Verkreuzungsverfahren erzielt werden, die dem Fachmann bekannt sind. Im allgemeinen wird die Verkreuzung durchgeführt, wenn die erste gelegte Schicht von dem zweiten gelegten Teil gekreuzt wird, wenn Fäden vom Unterfaden und vom Oberfaden unter und über dem ersten gelegten Faden hindurchgeführt und durch Doppelsteppstich festgelegt werden, wie in der Stickerei üblich. Unterschiedliche Arten der Verkreuzung verleihen dem Implantat unterschiedliche Lastausdehnungscharakteristika für unterschiedliche Anwendungen oder unterschiedliche Patienten. - Ein solches Netz kann als Verstärkungsflicken innerhalb des Körpers verwendet werden. Beispielsweise kann ein Patient an einem Narbenbruch leiden, bei dem einige der inneren Gewebe des Darms durch die Wände des Bauches austreten und die inneren Gewebe nur durch eine dünne Schicht der Bauchwand im Körper gehalten werden. Die Ränder des Bruchs können infolge des Bruches oder aus anderen Gründen nicht mehr stabil sein, was eine Wiederherstellung mit herkömmlichen Nähten schwierig macht. Die Zuhilfenahme eines Flickens hilft dem Chirurgen, eine gute Wiederherstellung zu erzielen, weil es ihm ermöglicht, das Gewebe an einer Stelle fern von den schwachen Rändern zu nähen. Der Flicken kann auch ein Gerüst bilden, auf dem neues Gewebe wachsen kann. Zu den Erfordernissen eines jeden Flickens gehört es, daß er eine hohe Berstfestigkeit hat, daß er leicht in Form zu schneiden ist entsprechend den Erfordernissen des Patienten, ohne daß die Schnittkanten ausfransen oder ausfasern. Viele gewebte Strukturen haben gute Eigenschaften hinsichtlich der Berstfestigkeit, fransen aber an einer Schnittkante leicht aus. Die Verwendung eines heißen Messers zum Schneiden von thermoplastischen Fäden, um ihr Ausfransen zu verhindern, ist unzweckmäßig, weil es unpraktisch ist, dies unter Operationssaal-Bedingungen durchzuführen, und aufgrund der scharfen Kanten (und möglicherweise giftiger Abbauprodukte), die thermisches Schneiden oft zurückläßt.
- Die Verwendung eines Flickens aus thermoplastischem Monofilament, wo sich kreuzende Fäden in einem Netz thermoverfestigt sind, ist bekannt, doch können etwaige Schnittkanten des den Flicken bildenden Monofilaments unerwünscht scharf sein, und das Heißverkleben begrenzt die Auswahl an Fäden auf diejenigen, die thermoplastisch sind. Die Verwendung von Klebemitteln zum Verkleben der Schnittstellen des Gitters ist unpraktisch und kann eine biologisch unverträgliche Komponente in das Implantat einbringen. Gemäß der vorliegenden Erfindung kann jeder implantierbare Faden verwendet werden, da die Schnittstellen des Netzes genäht und unempfindlich gegen Ausfransen sind, wenn das Netz geschnitten wird.
- Die Erfindung schafft einen Flicken zur Wiederherstellung einer Sehne oder eines Bandes. So ist es z. B. schwierig, die Rotatorenmanschette der Schulter ohne Verwendung einer textilen Verstärkung wiederherzustellen. Häufig wird eine Vielzahl von Nähfäden verwendet, aber manchmal ist dieses Verfahren der Verstärkung unzureichend, und es ist ein textiles Verstärkungsgewebe erforderlich. Die Verwendung einer Haube, wie in der internationalen PCT-Patentanmeldung Nr. WO 91/03993 beschrieben, ist ein Beispiel. Dieses sieht ein gewebtes oder gestricktes flexibles Gewebe vor. Die Offenheit des Netzes ist so gewählt, daß Gewebe hineinwachsen kann, ohne so offen zu sein, daß die strukturelle Integrität verlorengeht. Die vorliegende Erfindung schafft ein gesticktes Netz, das Bereiche von besonderer Festigkeit aufweist, um örtlich begrenzte, schwerere Belastungen zu tragen. Eine Ausführungsform dieses Gedankens der Erfindung ist in
2 gezeigt. Das Netz ist vorzugsweise so ausgebildet, daß die Löcher einen Abstand von etwa 2 mm haben und etwa 1 mm2 groß sind. Die Festigkeit des Netzes ist so gewählt, daß es als minimale Belastung etwa ein Körpergewicht tragen kann. Die Verwendung von Stickerei zur Bildung des Netzes1 hat den Vorteil, daß das Netz örtlich begrenzt verstärkt werden kann, indem eine andere Art von Faden oder eine höhere Konzentration oder Dichte von Fäden oder eine Vielzahl desselben Fadens verwendet wird, wie durch die mit2 bezeichneten, fetten Linien dargestellt. Bei einer bevorzugten Ausführungsform wird das Hauptnetz1 unter Verwendung eines geflochtenen Polyesterfadens von 0,35 mm Durchmesser hergestellt, und die Verstärkung2 besteht aus einem Polyestergeflecht von 1,5 mm Durchmesser. - Die Erfindung schafft auch ein Implantat zur Bildung eines Flachmaterials, dessen Steifigkeit in der einen Richtung sich von der Steifigkeit in der anderen Rich tung unterscheidet. Ein Implantat dieser Art, das als Stent bekannt ist, und von dem ein Beispiel in der britischen Patentanmeldung Nr. GB 2270264 offenbart ist, hat die Form eines Schlauches, der zum Einführen in den menschlichen oder tierischen Körper zusammengedrückt werden kann, um einen Verschluß der Speiseröhre oder anderer hohler innerer Organe durch Tumoren oder sonstige Störungen zu beseitigen. Wenn der gewendelte Stent aus einem steifen anisotropen Flachmaterial hergestellt und zur Verringerung seines Durchmessers fest aufgewickelt wird, um ihn in die Speiseröhre einzuführen, wird der Schlauch so starr, daß der Stent bei manchen Patienten schwer einzuführen ist, beispielsweise bei älteren Patienten mit steifen Hälsen oder vorstehenden Zähnen.
3 zeigt einen selbstexpandierenden festwandigen Schlauch ähnlich einem spiralfederartigen Schlauch. Der Schlauch kann fest aufgewickelt werden, so daß ein Schlauch geringen Durchmessers oder ein Stent gebildet ist, der in die hohlen inneren Organe des Körpers eingeführt werden und sich dann ausdehnen kann, wobei, er einen etwaigen Verschluß der hohlen inneren Organe aufhebt, der durch einen Tumor oder eine sonstige Störung verursacht worden sein kann. Ein Beispiel für eine Anwendung sind Patienten, bei denen die Speiseröhre durch einen malignen Tumor blockiert sein kann, was das Schlucken schwierig macht. - Bei der in
3 gezeigten Ausführungsform wird das den Stent bildende Material dadurch hergestellt, daß steife Fäden in der einen Richtung des Stents auf ein gesticktes Grundmaterial und in der anderen Richtung weniger steife oder gar keine Fäden gelegt werden. Dies ergibt ein anisotropes Material, das, wenn es fest um eine Achse parallel zu der anderen Richtung zu einem Schlauch geringen Durchmessers zum Einführen in die hohlen inneren Organe gewickelt wird, in Richtung der Längsachse sehr viel weniger steif ist und in radialer Richtung dennoch sein Federungsvermögen behält. Bei der in3 gezeigten Ausführungsform wird der Stent dadurch gebildet, daß ein Gewebestreifen in der Weise um eine Achse gewickelt wird, daß jede Windung mit Ausnahme der ersten die vorhergehende Windung leicht überlappt, so daß ein Schlauch mit einer im wesentlichen durchgehenden Wandfläche gebildet ist. Im Gebrauch wird der Stentdurchmesser durch Festziehen der Windungen verringert, und der Stent wird dann an der gewünschten Stelle in Position gebracht, beispielsweise in einem hohlen inneren Organ, wobei es die geringere Steifigkeit des Stents in axialer Richtung leicht macht, ihn in Position zu bringen. Sobald er in Position gebracht ist, kann der Stent in dem hohlen inneren Organ expandieren, wobei er einen Tumor oder ein sonstiges Hindernis zur Seite drückt und dennoch seine Flexibilität in Längsrichtung behält. - Bei einer besonders bevorzugten Ausführungsform sind die Fäden in der Weise angeordnet, wie es in
4 gezeigt ist, wobei die Längsfäden10 Polyester-Monofilamente von hoher Steifigkeit und die seitlichen Fäden12 Polyester-Monofilamente von geringerer Steifigkeit sind. Wenn dieses Material zu einem Stent der in3 gezeigten Art aufgewickelt und in Form wärmegehärtet, wird, bleibt der Stent, wenn er zur Einführung in die Speiseröhre des Patienten fest zusammengerollt wird, in der Längsrichtung flexibel und kann leichter eingeführt werden. - Aus der obigen Beschreibung und den Beispielen wird deutlich, daß die Erfindung eine Vielzahl von Anwendungsmöglichkeiten hat und nicht auf die hier beschriebenen beschränkt ist.
Claims (26)
- Chirurgisches Textilimplantat in Kombination mit einem Grundgewebe, wobei das Implantat eine Anordnung von Fasern umfaßt, dadurch gekennzeichnet, daß das Implantat dadurch hergestellt wurde, daß die das Implantat bildenden Fasern durch Sticken auf das Grundgewebe aufgebracht worden sind.
- Kombination nach Anspruch 1, bei der das Grundgewebe vor oder nach dem Einsetzen des Implantats in einen Patienten von den Fasern entfernt werden kann.
- Kombination nach Anspruch 1 oder Anspruch 2, bei der das Grundgewebe löslich ist.
- Kombination nach Anspruch 1 oder Anspruch 2, bei der das Grundgewebe abgebaut -und/oder absorbiert werden kann, nachdem das Implantat in einen Patienten implantiert worden ist.
- Kombination nach einem vorhergehenden Anspruch, bei der die Fasern als Netz angeordnet sind.
- Kombination nach Anspruch 5, bei der zumindest ein Teil des Netzes verstärkt ist.
- Kombination nach einem vorhergehenden Anspruch, bei der das Implantat im wesentlichen von dreieckiger Form ist.
- Kombination nach einem der Ansprüche 1 bis 6 mit Fasern, die in einer Richtung eine größere Steifigkeit als in einer anderen Richtung aufweisen.
- Kombination nach Anspruch 8, bei der die Richtungen im wesentlichen rechtwinklig zueinander verlaufen.
- Kombination nach Anspruch 8 oder Anspruch 9, bei der das Implantat die Form eines Schlauches hat, dessen radiale Steifigkeit größer ist als die Steifigkeit in axialer Richtung.
- Kombination nach Anspruch 10, bei der der Schlauch aus einem Streifen geformt ist, dessen Fasern in zwei Richtungen angeordnet sind, wobei der Streifen um eine Achse gewickelt ist.
- Kombination nach Anspruch 11, bei der der Streifen in einer Vielzahl von Windungen gewickelt ist und jede Windung des Streifens außer der ersten Windung die vorhergehende Windung überlappt.
- Kombination nach einem der Ansprüche 10 bis 12, bei der der Schlauch zur Beibehaltung seiner Form wärmegehärtet ist.
- Kombination nach einem vorhergehenden Anspruch, die einen oder mehrere Wachstumsfaktoren oder gefäßbildende oder neurogene Stoffe umfaßt.
- Verfahren zur Herstellung eines Implantats in Kombination mit einem Grundgewebe, wobei das Implantat eine auf das Grundgewebe aufgebrachte Anordnung von Fasern umfaßt, wobei das Verfahren das Anbringen der Fasern an dem Grundgewebe durch Sticken umfaßt.
- Verfahren nach Anspruch 15, bei dem das Grundgewebe entfernt wird, nachdem die Fasern angebracht worden sind und bevor das Implantat in einen Patienten implantiert wird.
- Verfahren nach Anspruch 16, bei dem das Grundgewebe löslich ist und durch Lösen in einem Lösungsmittel entfernt wird.
- Verfahren nach Anspruch 15, bei dem das Grundgewebe abgebaut und/oder absorbiert werden kann, nachdem das Implantat in einen Patienten implantiert worden ist.
- Verfahren nach einem der Ansprüche 15 bis 18, bei dem die Fasern als Netz (
1 ) angeordnet sind. - Verfahren nach Anspruch 19, das das Verstärken zumindest eines Teiles (
2 ) des Netzes umfaßt. - Verfahren nach Anspruch 19 oder Anspruch 20, bei dem die Fasern in einem Netz angeordnet sind, das in einer Richtung eine größere Steifigkeit als in einer anderen Richtung aufweist.
- Verfahren nach Anspruch 21, bei dem die Fasern von größerer Steifigkeit im wesentlichen rechtwinklig zu Fasern anderer Steifigkeit angeordnet sind.
- Verfahren nach Anspruch 21 oder 22, bei dem die Anordnung von Fasern in Form eines Schlauches ausgebildet ist, dessen radiale Steifigkeit größer ist als die Steifigkeit in axialer Richtung.
- Verfahren nach Anspruch 23, bei dem der Schlauch aus einem Streifen, dessen Fasern in zwei Richtungen angeordnet sind, dadurch geformt wird, daß der Streifen um eine Achse gewickelt wird.
- Verfahren nach Anspruch 24, bei dem der Streifen in einer Vielzahl von Windungen gewickelt ist, wobei jede Windung des Streifens außer der ersten Windung die vorhergehende Windung überlappt.
- Verfahren nach einem der Ansprüche 23 bis 25, bei dem der Schlauch zur Beibehaltung seiner Form wärmegehärtet ist.
