Description
- Die vorliegende Erfindung betrifft neue Impfstoffformulierungen, die ein konjugiertes Polysaccharid-Antigen umfassen, das mit einem Trägerprotein verknüpft ist. Insbesondere betrifft die Erfindung eine Impfstoffformulierung zur Verhinderung von Haemophilus influenzae Typ B (Hib)-Infektionen, wobei das Antigen an Aluminiumphosphat adsorbiert ist. Die Erfindung betrifft auch einen Mehrfachimpfstoff, d. h. einen Impfstoff für die Linderung oder Behandlung von mehr als einem Krankheitszustand. Die vorliegende Erfindung betrifft auch die Herstellung und die Verwendung solcher Impfstoffe in der Medizin.
- Impfstoffe, die Polysaccharide verwenden, sind auf dem Fachgebiet bekannt. Zum Beispiel basiert ein Impfstoff zur Verhinderung von Haemophilus influenzae B (Hib)-Infektionen auf dem Kapselpolysaccharid (PRP), das mit einem Trägerprotein konjugiert ist. Das Polysaccharid ist ein Polymer von Ribose, Ribitol und Phosphat. Diese Impfstoffe werden typischerweise als reine (d. h. ohne Adjuvantien) Formulierungen angeboten. Obwohl in einem Fall (Pedvax Hib, Produkt von Merck) ein Verdünnungsmittel, das Aluminiumhydroxid enthält, zur Rekonstitution des lyophilisierten Konjugats verwendet wird. Typischerweise ist das Trägerprotein Diphtherie- oder Tetanus-Toxid oder ein Protein der äußeren Membran von N. meningitidis. Beispiele für solche Konjugatimpfstoff-Antigene werden in
US 4 365 170 ,US 4 673 574 ,EP 208 375 EP 477508 EP 161 188 - Es ist wünschenswert, solche Konjugatimpfstoffe gleichzeitig mit weiteren Antigenen oder Impfstoffen zu verabreichen, und es kann mehrfache Injektionen umfassen. Probleme, die mit mehrfachen Injektionen einhergehen, umfassen ein kompliziertes Verabreichungsverfahren und ein großes Gesamtinjektionsvolumen. Dies ist ein besonders akutes Problem, wenn der Impfstoff für Kinder gedacht ist.
- Es ist daher vorgeschlagen worden, Kombinationsimpfstoffe herzustellen.
EP 594950 - Es wäre wünschenswert, Hib-Polysaccharid-Konjugatimpfstoffe zu einer solchen Kombination hinzuzufügen. Wir haben jedoch festgestellt, dass das einfache Vermischen der Komponenten zu einer Reduzierung der Antikörpertiter gegen die Polysaccharidkomponente führt.
- Die Erfinder haben entdeckt, dass diese Reduzierung verhindert werden kann, wenn das Hib-Polysaccharid-Konjugatantigen an Aluminiumphosphat adsorbiert wird. Wenn im Gegensatz dazu das Antigen an Aluminiumhydroxid adsorbiert wird, tritt eine vollständige Reduzierung der Antikörpertiter gegen die Hib-Polysaccharidkomponente auf.
- Entsprechend stellt die vorliegende Erfindung eine Kombination nach Anspruch 1 bereit.
- Vorzugsweise ist das Trägerprotein entweder Diphtherie- oder Tetanustoxoid, Diphtherie Crm197-Protein oder ein Protein der äußeren Membran von einem Bakterium wie N. meningitidis.
- Das Polysaccharidkonjugat kann mittels jedes bekannten Kopplungsverfahrens hergestellt werden. Zum Beispiel kann das Polysaccharid über eine Thioetherbindung gekoppelt werden. Dieses Konjugationsverfahren beruht auf der Aktivierung des Polysaccharids mit 1-Cyano-4-dimethylamino-pyridinium-tetrafluorborat (CDAP), um einen Cyanatester zu bilden. Das aktivierte Polysaccharid kann so direkt oder über eine Abstandsgruppe an eine Aminogruppe auf dem Trägerprotein gekoppelt werden. Vorzugsweise wird der Cyanatester mit Hexandiamin gekoppelt und das Amino-derivatisierte Polysaccharid wird unter Verwendung der Heteroligierungs-Chemie an das Trägerprotein konjugiert, was die Bildung einer Thioetherbindung beinhaltet. Solche Konjugate sind in der offengelegten PCT-Anmeldung
WO93/15760 - Die Konjugate können auch durch direkte reduktive Aminierungsverfahren hergestellt werden, wie beschrieben in
US 4365170 (Jennings) undUS 4673574 (Anderson). Andere Verfahren sind inEP-0-161-188 ,EP-208375 EP-0-477508 beschrieben. - Ein weiteres Verfahren beinhaltet die Kopplung eines mit Bromcyan aktivierten Polysaccharids, das mit Adipinsäurehydrazid (ADH) derivatisiert ist, an das Trägerprotein mittels einer Carbodiimid-Kondensation. Eine solche Konjugation ist in Chu C. et al. Infec. Immunity, 1983, 245–256 beschrieben.
- In einer bevorzugten Ausführungsform der Erfindung ist das Verhältnis von PRP-Polysaccharid zu Trägerprotein reduziert von typischerweise 1:3 auf 1:0,3 bis 1:2. Solche Konjugate mit niedrigem Verhältnis sind von Vorteil, denn selbst in einem Zustand ohne Adjuvans treten keine Interferenzprobleme auf.
- Vorzugsweise umfassen die weiteren Antigene in dem Kombinationsimpfstoff zumindest ein Antigen, das Schutz gegen eines oder mehrere der Folgenden verleiht: Hepatitis A-Virus (HAV), Diphtherie, Tetanus, Pertussis, Hepatitis B und Polio.
- Spezielle Kombinationsimpfstoffe im Umfang der Erfindung umfassen eine DTPa(Diphtherie-Tetanus-azelluläre Pertussis)-Hib-Kombinationsimpfstoffformulierung, eine DTPa-Hib-Hepatitis B-Impfstoffformulierung und eine IPV(inaktivierter Polioimpfstoff)-DTPa-Hib-Hepatitis B-Impfstoffformulierung.
- Die vorstehenden Kombinationen können gegebenenfalls eine Komponente enthalten, die gegen Hepatitis A schützt.
- Geeignete Komponenten zur Verwendung in solchen Impfstoffen sind bereits kommerziell erhältlich und Einzelheiten können von der Weltgesundheitsorganisation erhalten werden. Zum Beispiel kann die IPV-Komponente der inaktivierte Polioimpfstoff von Salk sein. Der Diphtherie-, Tetanus- und Pertussis-Impfstoff umfasst ein azelluläres Produkt wie Infanrix DTPa (SmithKline Beecham Biologicals). Die Komponente, die Schutz gegen Hepatitis A verleiht, ist vorzugsweise das Produkt, das als ”Havrix” (SmithKline Beecham Biologicals) bekannt ist, welches ein abgetöter, attenutierter Impfstoff ist, der von dem Stamm HM-175 von HAV abgeleitet ist (vgl. ”Inactivated Candidate Vaccines for Hepatitis A” von F. E. Andre, A Hepbum und E. D'Hondt, Prog. Med. Virol., Band 37, Seiten 72–95 (1990) und die Produktmonografie ”Havris”, veröffentlicht von SmithKline Beecham Biologicals (1991)). Die Hepatitis-B-Komponente kann das ”S”-Antigen, wie in ”Engerix-B”, enthalten.
- Vorteilhafterweise ist der Kombinationsimpfstoff gemäß der Erfindung ein pädiatrischer Impfstoff.
- Die Impfstoffherstellung wird im Allgemeinen beschrieben in Vaccine Design – The Subunit and adjuvant approach Ed Powell and Newman; Pellum Press. Die Verkapselung in Liposomen wird z. B. von Fullerton,
US-Patent 4,235,877 , beschrieben. Die Konjugation von Proteinen an Makromoleküle wird z. B. von Likhite,US-Patent 4,372,945 und von Amor et al.,US-Patent 4,474,757 beschrieben. - Die Menge an Konjugatantigen in jeder Impfstoffdosis wird als eine Menge ausgewählt, die bei einem typischen Impfling eine immunprotektive Antwort ohne signifikante, schädliche Nebenwirkungen induziert. Eine solche Menge wird variieren in Abhängigkeit der spezifisch verwendeten Immunogene. Im Allgemeinen ist zu erwarten, dass jede Dosis 1 bis 1000 μg an Immunogen insgesamt enthalten wird, vorzugsweise 2 bis 100 μg und am meisten bevorzugt 4 bis 40 μg. Eine optimale Menge für einen spezifischen Impfstoff kann durch Standarduntersuchung sichergestellt werden, einschließlich der Beobachtung der Antikörpertiter und anderer Antwortender Individuen. Nach einer Anfangsimpfung können die Individuen eine oder zwei Verstärkungsinjektionen in etwa 4-wöchigen Abständen erhalten.
- In einem weiteren Aspekt gemäß der Erfindung wird ein Verfahren zur Herstellung des Impfstoffs bereitgestellt, das die Adsorption des Hib-Polysaccharid-Konjugatantigens an Aluminiumphosphat umfasst. Die Adsorption wird vorzugsweise bei einem pH-Wert von zwischen 5 und 6 vorgenommen, vorzugsweise bei einem pH-Wert von etwa 5,4. In einer Ausführungsform wird der adsorbierte Hib-Impfstoff nach Stehen von mehr als 24 Stunden gefriergetrocknet. Der adsorbierte Hib-Impfstoff der Erfindung kann mit den weiteren Antigenen in flüssiger Form kombiniert werden.
- Die Erfindung stellt weiterhin eine erste medizinische Verwendung für solch einen Impfstoff bereit.
- In einer weiteren Ausführungsform stellt die Erfindung eine Verwendung einer nicht-toxischen und wirksamen Menge des Kombinationsimpfstoffs der Erfindung bei der Herstellung eines Medikaments zur Behandlung eines Patienten bereit, der an einer Haemophilus influenzae B-Infektion leidet oder für sie empfänglich ist.
- Die folgenden Beispiele erläutern die Erfindung.
- Beispiel 1
- Impfstoffformulierung, die Hib-Polysaccharid umfasst, konjugiert mit Tetanustoxoid und adsorbiert an Aluminiumphosphat.
- Synthese von Haemophilus influenzae Typ B-Kapselpolysaccharid (PRP)-Tetanustoxoid (TT)-Konjugat
- 1.a Bromcyan-Kopplung
- Die kovalente Bindung von PRP und TT wird mittels einer Kopplungschemie durchgeführt, die am NIH (Chu C. et al. (1993), further studies on the immunogenicity of Haemophilus influenzae type b and pneumococctal type 6A polysaccharide Protein conjugates. Infec. Immunity, 245–256) entwickelt wurde. PRP wird unter kontrollierten Bedingungen mit Bromcyan aktiviert und mit einem Adipinhydrazid-Abstandshalter derivatisiert.
- Nach der Derivatisierung wird das aktivierte Polysaccharid (PRP-AH) mittels Diafiltration gereinigt. Die Kopplung der zwei gereinigten Komponenten (PRP-AH und TT) wird durch Carbodiimid-Kondensation bewirkt. Das Konjugat wird dann mittels Ultrafiltration und Gelfiltration gereinigt, um das Reagenz und nicht-konjugiertes PRP und TT zu entfernen. Synthese von PRP-TT-Konjugaten
- 1.b CDAP-Kopplung
- 30 mg natives Hib-PRP wurde in 6 ml 2M NaCl gelöst. 225 μl (mcl) CDAP (1-Cyano-4-dimethylamino-pyridinum-tetrafluorborat) wurde zu der Polysaccharidlösung zugegeben (von einer Stammlösung von 100 mg/ml in Acetonitril). 90 Sekunden später wurden 450 μl (mcl) 0,2 M Triethylamin zugegeben. Die Aktivierung wurde bei einem pH-Wert von 10,0 für 1 Minute auf Eis und Minute bei Raumtemperatur durchgeführt.
- 90 mg Tetanustoxoid (anfängliches Verhältnis PS/Protein von 1/3) wurden zu dem aktivierten Polysaccharid zugegeben und die Kopplungsreaktion wurde bei Raumtemperatur für 1 Stunde durchgeführt. Dann wurde die Reaktion mit 3 ml 1M Glycinlösung, pH 5,0, für 30 Minuten bei Raumtemperatur und über Nacht bei 4°C gequencht.
- Das Konjugat wurde mittels Gelfiltration auf einer Sephacryl HR 500-Säule gereinigt, die mit 0,2M NaCl äquilibriert worden war. In jeder Fraktion wurde der Kohlenhydrat- und der Proteingehalt bestimmt. Das Konjugat wurde vereint und steril filtriert (Membran Minisart ⊕ 0,222 μm).
- Adsorption an Aluminiumphosphat
- 1.c
- Zu 0,15 mg Aluminiumphosphat wurden 12,5 μg (mcg) des Polysaccharidkonjugats von Beispiel 1(a) zugegeben. Es wurde zwei Stunden gerührt und der pH-Wert auf 5,1 eingestellt. Das Gemisch wurde einen Tag bei Raumtemperatur stehen gelassen und das adsorbierte Konjugat dann für weitere 9 Tage bei 2 bis 8°C gehalten. Um ein gefriergetrocknetes Produkt herzustellen, wird das adsorbierte Produkt in Lactose (15,75 mg) verdünnt, um eine endgültige Zusammensetzung von 25 μg (mcg) Polysaccharid/ml und 0,4 mg Al/ml zu ergeben, und die resultierende Zusammensetzung wurde in 0,5 ml Gläschen gefüllt und gefriergetrocknet.
- Um ein flüssiges Produkt herzustellen, wird das adsorbierte Konjugat in Wasser zur Injektion mit 150 mM NaCl und 5 mg/ml Phenoxyethanol verdünnt, um eine endgültige Zusammensetzung von 20 μg (mcg) Polysaccharid/ml und 0,32 mg Al/ml zu ergeben.
- 1.d
- Die Formulierung eines Diphtherie-, Tetanus- und Pertussis-(azellulär) Impfstoffs mit und ohne Hepatitis B wurde entsprechend den Verfahren von
WO 93/24148 - 1.e Herstellung eines prä-adsorbierten PRP-TT-Aluminiumphosphat-Konjugats mit niedrigem Verhältnis
- Das Konjugat wurde in analoger Weise wie in Beispiel 1a hergestellt, aber mit reduzierten Mengen an verwendetem Tetanus [30 μg (mcg), 60 μg (mcg)], um ein Produkt mit einem Verhältnis Polysaccharid:Protein von 1:1 oder 1:2 zu ergeben. Das Konjugat wird dann an Aluminiumphosphat adsorbiert gemäß dem Verfahren von Beispiel 1.c. Das endgültige gefriergetrocknete Präparat enthält 12,5 μg Konjugat, 0,15 mg AlPO4, 15,75 mg Lactose. Diese wird vor der Verwendung in 0,5 ml Wasser zur Injektion rekonstituiert bei einem pH-Wert von 0,1 ± 0,1.
- Beispiel 2: Immunogenität von an Aluminiumphosphat prä-adsorbiertem PRP-TT-Konjugat und kombiniert mit DTPa oder DTPa-HB
- Das Hib-Konjugat von Beispiel 1.a), entweder rein oder prä-adsorbiert an AlPO4 (beide Impfstoffe wurden lyophilisiert), wurde mit DTPa oder DTPa HB nicht mehr als 1 Stunde vor der Injektion gemischt und an Baby-Ratten (1 Woche alt) mittels des subkutanen Wegs mit einer Dosis, die 1/20stel einer menschlichen Dosis (0,5 μg PRP) entspricht, injiziert. Die Ratten wurden 2 Wochen und 4 Wochen später nochmals immunisiert, und nach jeder Immunisierung wurde Serum gesammelt, um Anti-PRP-Antikörper zu messen. Kontrollen umfassten den in Salzlösung rekonstituierten Hib-Impfstoff (an AlPO4 adsorbiert oder nicht).
- Gruppen von 10 zufällig ausgewählten Baby-Ratten (1 Woche alt – OFA-Stamm) wurden 3-mal am Tag 0-14-28 mit 1/20stel der menschlichen Dosis des Hib-Impfstoffs subkutan immunisiert, allein oder kombiniert mit DTPa oder DTPa HB (1/20stel der menschlichen Dosis). Die Rekonstitution des lyophilisierten Hib-Impfstoffs mit Salzlösung oder die Kombinationen (DTPa oder DTPa HB) erfolgte weniger als 1 Stunde vor der Injektion.
- Den Ratten wurde unter Anästhesie am Tag 14-28-42 und 56 Blut entnommen. Die Anti-PRP-Antikörper wurden mittels ELISA in den einzelnen Seren gemessen und die Titer wurden unter Verwendung eines kalibrierten Standards in μg/ml (γ/ml) ausgedrückt. Der GMT wurde für jede Gruppe und für jeden Zeitpunkt berechnet. Die 95% Vertrauensgrenzen wurden für die Titer berechnet, die nach der dritten Immunisierung erhalten wurden.
- Wie in Tabelle 1 gezeigt, ändert die Adsorption des Hib-Konjugats an AlPO4 nicht seine Immunogenität: einiges anti-PS wurde nach der zweiten Dosis produziert und ein guter Verstärkungseffekt wurde nach der dritten Dosis gezeigt, wie bei menschlichen Babies gesehen. Das Mischen des Hib-Impfstoffs mit DTPa oder DTPa HB reduziert die anti-PRP-Antwort 3- bis 8-fach und im Fall von DTPa HB ist dieser Abfall signifikant. Im Gegensatz dazu stellt die Prä-Adsorption des Hib-Impfstoffs an AlPO4 die Anti-PRP-Antwort zumindest auf einem Niveau wieder her, das äquivalent ist zu dem des reinen Impfstoffs.
- Schlussfolgerung:
- Die Hib/Aluminiumphosphat-Formulierung hat daher das Potenzial, die Kompatibilitätsprobleme zu überwinden, die beim Mischen von Hib mit anderen pädiatrischen Kombinationen auftreten. Tabelle 1

Claims (12)
- Kombinationsimpfstoff, umfassend: (i) ein Kapselpolysaccharid von Haemophilus influenzae B, konjugiert an ein Trägerprotein, dadurch gekennzeichnet, dass das Konjugat an Aluminiumphosphat adsorbiert ist; und (ii) weitere Antigene, die Schutz gegen Diphtherie-, Tetanus- und Pertussis-Erkrankung gewähren.
- Kombinationsimpfstoff nach Anspruch 1, worin dem Konjugat ein oder mehrere weitere Antigene, die Schutz gegen eine Erkrankung, ausgewählt aus der Gruppe: Hepatitis A, Hepatitis B und Polio, gewähren, beigemischt sind.
- Kombinationsimpfstoff nach Anspruch 1 oder 2, worin das Trägerprotein ausgewählt ist aus der Gruppe, umfassend: Diphtherietoxoid, Diphtherie CRM197-Protein und äusseres Meningokokken-Membranprotein.
- Kombinationsimpfstoff nach Anspruch 1 oder 2, worin das Trägerprotein, das an das Kapselpolysaccharid von Haemophilus influenzae B konjugiert ist, Tetanustoxoid ist.
- Kombinationsimpfstoff nach einem der Ansprüche 1 bis 4, worin das Verhältnis von Haemophilus influenzae B-Polysaccharid zu Trägerprotein bei 1:0,3 bis 1:2 (G:G) liegt.
- Kombinationsimpfstoff nach einem der Ansprüche 1 bis 5, worin das adsorbierte Konjugat vor seiner Zumischung zu den weiteren Antigenen gefriergetrocknet worden ist.
- Kit zur Herstellung eines Kombinationsimpfstoffs, umfassend einen Behälter eines gefriergetrockneten Impfstoffs, umfassend ein Kapselpolysaccharid von Haemophilus influenzae B, das an ein Trägerprotein konjugiert und an Aluminiumphosphat adsorbiert ist, und einen zweiten Behälter mit einem Impfstoff, der Schutz gegen Diphtherie-, Tetanus- und Pertussis-Erkrankung gewährt.
- Verfahren zur Herstellung eines Impfstoffs nach den Ansprüchen 1 bis 6, umfassend das Konjugieren eines Haemophilus influenzae Typ B-Kapselpolysaccharid-Antigens an ein Trägerprotein, Adsorbieren des Konjugats an Aluminiumphosphat und Mischen mit den weiteren Antigenen.
- Verfahren nach Anspruch 8, wobei die weiteren Antigene Schutz gegen eine Erkrankung, ausgewählt aus der Gruppe bestehend aus: Hepatitis A, Diphtherie, Tetanus, Pertussis, Hepatitis B und Polio, gewähren.
- Impfstoffzusammensetzung nach einem der Ansprüche 1 bis 6 zur Verwendung in der Medizin.
- Verwendung einer sicheren und wirksamen Menge einer Impfstoffzusammensetzung nach einem der Ansprüche 1 bis 6 bei der Herstellung eines Medikaments zur Behandlung eines Patienten, der an einer Haemophilus influenzae B-Infektion leidet oder für sie empfänglich ist.
- Kombinationsimpfstoff nach Anspruch 2, worin das Kapselpolysaccharid von Haemophilus influenzae B an Tetanustoxoid konjugiert ist und worin der Impfstoff folgende Antigene umfasst: Diphtherietoxoid, Tetanustoxoid, azelluläre Pertussis-Antigene, Hepatitis B-Oberflächenantigen und inaktivierter Polioimpfstoff.
