Die erfindungsgemäß verwendeten Verbindungen haben den Vorteil, daß sie wirksam den intraokularen
Druck verringern, nicht toxisch sind und keine Nebeneffekte aufweisen.
Die C ι -C4-alkylgnippen in den Verbindungen de allgemeinen Formel sind geradkettige oder verzweigtkettige
Alkylgruppen, insbesondere Methyl-, Ethyl-, Propyl-, Isopropyl-, Butyl- oder tert-Butyl-Gruppen.
Die C,-C4-alkenylgruppen sind geradkettige oder verzweigtkettige Alkenylgruppen, insbesondere Vinyl-,
Allyl-, Crotyl- oder 1-Methylallyl-Gruppen.
Die Cj-Cg-Cycloalkylgruppen sind insbesondere Cyclopropyl-, Cyclopentyl-, Cyclohexyl- oder Cyclooctyl-Gruppen.
Die C ι -Q-alkylgruppen, substituiert mit einem Phenylrest mit oder ohne einem C1 -Q-alkoxysubstituenten
am Phenylring sind solche, die eine geradkettige oder verzweigtkettige Alkylengruppe mit 1 bis 4 C-Atomen und
einem Phenylrest verbunden mit der Alkylengruppe enthalten und wobei der Phenylring ggf. mit 1 bis 3 geradkettigen
oder verzweigtkettigen C,-C4-alkoxygruppen substituiert ist, insbesondere Benzyl, e-Phenethyl, ß-Phenethyl,
4-Phenylbutyl, l,l-Dimethyl-2-phenylethyl,ß-3,4-Dimethoxyphenethyl, 4-Methoxybenzyl undjS-3,4,5-Trimethoxyphenethyl.
Besonders geeignete Niedrigalkinyloxygruppen sind Ethinyloxy, Propinyloxy, Butinyloxy und 2-Methylpropinyloxy.
Geeignete C|-C4-alkoxygruppen mit einem C2-C2-alkanoylsubstitutnten sind z. B. Methylcarbonylmethoxy,
2-Methylcarbonylethoxy, 2-Ethylcarbonylethoxy, 3-Methylcarbonylpropoxy, 4-Methylcarbonylbutoxy,
F.thylcarbonylmethoxy, Butylcarbonylmethoxy und 4-Butylcarbonylbutoxy.
Die Ci-C^alkanoylaminogruppen sind geradkettige oder verzweigtkettige Alakanoylaminogruppen, insbesondere
Formylamino, Acetylamino, Propionylamino und Butyrylamino.
Geeignete C2-C4-alkenyloxygruppen sind z. B. Vinyloxy, Allyloxy, Crotyloxy und 1-Methylallyloxy.
Die Säureadditionssalze der Carbostyrilderivate der allgemeinen Formel sind die, die pharmakologisch verträglich
sind, z. B. die Salze der anorganischen und organischen Säuren, wie Salzsäure, Schwefelsäure, Salpetersäure,
liromwasserstoffsäure, Oxalsäure, Maleinsäure, Fumarsäure, Zitronensäure und Weinsäure.
Von den Verbindungen der allgemeinen Formel und den pharmakologisch verträglichen Säureadditionssalzen
davon sind als Wirkstoff für Behandlung von Glaucoma insbesondere die geeignet, in denen R1 und R3 für Wasserstoff,
R2 für eine

der 4-Stcllung des Carbostyrilrestes eine Einfachbindung ist. Diese bevorzugt eingesetzten Verbindungen sind

Weitere bevorzugte erfindungsgemäße Verbindungen der obigen allgemeinen Formel sind die, in denen die
Kohlenstoff-Kohlenstoff-Bindung zwischen der 3-Stellung und der 4-Stellung des Carbostyrilrestes eine Doppel^indung
ist.
Besonders bevorzugte Verbindungen gemäß der allgemeinen Formel, die als besonders wirksame Verbindungen
für die Behandlung von Glaucoma geeignet sind, sind nachfolgend zusammengefaßt.

Besonders bevorzugt ist die Verwendung von 5"(2-Hydroxy-3-tert-butylamino)propoxy-S,4-dihydrocarbostyril und 5-(2-Hydroxy-3-tert-butylamino)propoxy-carbostyril.
Die erfindungsgemäß eingesetzten Verbindungen können formuliert werden als Tabletten, Granulate, Injektionslösungen, vorzugsweise als Salbe oder Lösung für die örtliche Anwendung.
Obgleich die Dosis für die Verabreichung nicht begrenzt ist, sollte sie jedoch im allgemeinen bei einer täglichen Dosis von etwa 0,01 bis 5 mg, vorzugsweise etwa 0,01 bis 1,0 mg vorzugsweise etwa 0,01 bis 0,5 mg bei
Erwachsenen liegen, berechnet für den Wirkstoff der Mittel. Die tagliche Dosis sollte in zwei oder drei Teilen
verabreicht werden. Die Menge des Wirkstoffs liegt, in Abhängigkeit von der Form der Verabreichung, vorzugsweise bei etwa 0,1 bis 70 Gew.-% und bei der ophtalmischen Lösung vorzugsweise bei 0,1 bis 2 Gcw.-%.

lösliche Zusatzstoffe und suspendierbare Zusatzstoffe, wie Vaseline, gereinigtes wasserhaltiges Lanolin und
flüssiges Paraffin. Ein geeignetes Verdünnungsmittel für die Herstellung von ophtalmischen Lösungen ist steriles
destilliertes Wasser.
Die Zubereitungen können auch übliche löslichkeitssteigernde Mittel, Stabilisatoren, Puffer, Antioxidantien
und Konservierungsmittel enthalten. 5
Die ophtalmischen Lösungen werden vorzugsweise auf einen pH-Wert von etwa 5,5 bis 8,5, vorzugsweise etwa
6,5 bis 7,5 eingestellt.
Die Erfindung wird durch die folgenden Beispiele und medizinischen Wirkungstests näher erläutert.

Die Bestandteile werden in destilliertem Wasser gelöst, die Lösung wird durch ein geeignetes Papierfilter filtriert
und dann sterilisiert, um eine Zubereitung für die Behandlung von Glaucoma in Form einer ophthalmischen
Lösung herzustellen.
Gemäß dem Verfahren von Herstellungsbeispiel 1 wurde eine ophthalmische Lösung für die Behandlung von
Glaucoma unter Verwendung der obigen Bestandteile hergestellt.

Wirkungstest 1
Zwei Tropfen der ophthalischen Lösung gemäß Herstellungsbeispiel 1 wurden in die an Glaucoma erkrankten
Augen von drei verschiedenen Patienten getropft. Der intraokulare Druck wurde in der Zeit von 10 Uhr bis 50
!! Uhr vormittags gemessen., und zwar vor der Verabreichung der Tropfen und einen Tag und zwei Tage nach der
Verabreichung mit einem Goldmann Applanations-Tonometer. Die Ergebnisse sind in der Tabelle 1 zusammengefaßt.

Die in der Tabelle 1 zusammengefaßten Ergebnisse zeigen, daß die Verwendung der Verbindung der allgemeinen Formel zu einer wirksamen Verringerung des intraokularen Druckes fuhrt. Die Beobachtung des behandelten Auges nach der Durchführung des Tests zeigte keine abnormale Entwicklung.
Zwei Tropfen der ophthalischen Lösung gemäß Herstellungsbeispiels 2 wurden in die an Glaucoma erkrankten Augen von drei Patienten getropft. Wie im Wirkungstest 1 beschrieben, wurde der intraokularc Druck
gemessen, und zwar vor dem Eintropfen der Augentropfen und 1,4 und 24 Stunden nach der Behandlung. Die
Ergebnisse sind in Tabelle 2 zusammengefaßt.

Die Ergebnisse der Tabelle 2 zeigen, daß bei Verwendung der Verbindungen der allgemeinen Formel eine
wirksame Verringerung des intraokularen Drucks erreicht wird.
Männliche Neuseeland-Albino-Kaninchen, mit einem Gewicht von 1,8 bis 2,8 kg wurden als Versuchstiere
verwendet. Der intraokulare Druck wurde mit einem Alcon Pneumatik-Tonometer ohne lokale Anästhesie
gemessen, während die Versuchstiere in einem zylindrischen Kaninchenhalter gehalten wurden.
Die Testverbindung wurde in einer physiologischen Natriumchloridlösung einer Konzentration von 1%
(Gewicht/Volumen) gemessen, und die erhaltene Lösung wurde auf einen pH-Wert von 6,5 bis 7,0 mit einer
0,01 η-wäßrigen Natriumhydroxidlösung eingestellt Die Lösung wurde in eines der Augen der Tiere in einer
genauen Menge von 100 μΐ mittels einer Mikrosyringe eingeträufelt, nachdem der intraokulare Druck des Auges

gemessen worden war. Nach dem Einträufeln der Augentropfen wurde das Augenlid fur I Minute geschlossen,
um das Entweichen der Lösung zu verhindern.
Die Wirksamkeit der Testverbindung wurde bestimmt durch das Messen des intraokularen Drucks, und zwar
30 Minuten und 1 Stunde nach der Verabreichung und verglichen mit den Meßergebnissen vor der Verabreichung.
Eine 100 μΙ-Menge einer physiologischen Salzlösung, die keinen Wirkstoff enthält, ist gleichzeitig als
Vcrgleichslösung getestet worden. Die Ergebnisse sind in der Tabelle 3 zusammengefaßt.

Die Ergebnisse der Tabelle 3 zeigen, daß die erfindungsgemäß verwendeten Wirkstoffe (Verbindungen A bis
N) bei normalen Kaninchen ungefähr die gleiche intraokulare Druckverringerung bewirken, d. h., daß die Verbindungen
sehr wirksam für die Heilung von Glaucoma einsetzbar sind.

Untersuchung der akuten Toxizität
D'.e erfindungsgemäß ve:- sendeten Wirkstoffe zur Behandlung von Glaucoma wurden an Mäusen intravenös
verabreicht verabreicht, um die akute Toxizität zu bestimmen. In der Tabelle 4 sind die LD50-Werte zusammengefaßt.

mit oder ohne einem (Ci-C4)-alkoxysubstituenten am Phenylring ist, und R3 fur eine

worin R4 die obige Bedeutung hat, wenn R2 Wasserstoff ist, steht oder R3 für Wasserstoff, Hydroxyl, Ureido,
(C2-C4)-alkinyloxy, (C,-C4)-alkoxy mit einem (C2-C5)-alkanoyl-Substituenten, (C, -C4)-a!kanoylamino
oder (C2-C4)-alkenyloxy steht, wenn R2 eine

ist und die Kohlenstoff-Kohlenstoff-Bindung zwischen der 3-Stellung und 4-Stellung des Carbostyrilrestes
eine Einfach- oder Doppelbindung ist,
oder eines pharmakologisch verträglichen Säureadditionssalzes davon zur Behandlung von Glaucoma.
2. Verwendung von 5-(2-Hydroxy-3-tert-butylamino)propoxy-3,4-dihydro-carbostyril und den pharmakologisch
verträglichen Säureadditionssalzen davon nach Anspruch 1.
Die Erfindung betrifft die Verwendung von Carbostyrilderivaten für die Behandlung von Glaucoma.
Aus den DE-OS 23 02 027, DE-OS 27 11 719, US-PS 39 53 456, US-PS 41 47 869 und US-PS 40 81 447 ist
bekannt, daß Carbostyrilderivate für die Behandlung von Herzstörungen einsetzbar sind.
Der Erfindung liegt die Aufgabe zugrunde, neue wirksame Mittel zur Behandlung von Glaucoma zur Verfugung
zu stellen, die für die Verwendung in einem ophthalmischen Präparat geeignet sind.
Die Erfindung betrifft die Verwendung einer Verbindung der allgemeinen Formel
