虽然大部分癌症患者仍用一个或多个疗程的化疗治疗,众所周知的事实是癌症的细胞毒性治疗也损害了免疫系统。免疫细胞属于人体内快速分裂的细胞,任何杀死分裂细胞的治疗也将杀死免疫细胞。因此,包括放射疗法、损伤DAN的化学品、DNA合成抑制剂和微管功能抑制剂在内的治疗都将造成免疫系统的损伤。癌症治疗之所以需要骨髓移植作为辅助疗法,恰恰是因为免疫系统受到损伤并需要得到补充。氨甲蝶呤以及其它抗癌药物也常用做免疫抑制剂。有证据表明抗癌治疗可特异性地抑制T细胞功能。例如,采用全身放疗治疗何杰金病的患者体内天然T细胞显然永久丧失(Watanabe等,1997,血液90:3662)。
基于当前的知识,标准治疗(化学疗法和放射疗法)和局部免疫刺激不可能成为用于有效治疗癌症的有用联合治疗方法。因此,本领域当前需要增强抗体-细胞因子融合蛋白介导的抗预先选定的细胞类型(例如肿瘤细胞)的免疫反应的方法,以及需要在此类方法中使用的组合物。
发明概述
现已发现,当抗体-细胞因子融合蛋白(免疫细胞因子)施用于长有肿瘤或转移瘤的哺乳动物时,如果其在使用通过促进或增强肿瘤细胞对其摄入而提高或增强治疗效果的免疫细胞因子摄入促进剂处理哺物动物之前、同时或之后施用,可产生较强烈的抗肿瘤反应。已经发现的免疫细胞因子摄入促进剂包括烷化剂类化疗剂和紫杉烷类化疗剂如紫杉醇。特别是,已经发现此类联用在介导对预先选定的细胞类型如肿瘤细胞或病毒感染的细胞的免疫杀伤中是有用的。
一方面,本发明提供了用以在哺乳动物中诱导抗预先选定的细胞类型的杀细胞免疫反应的方法。该方法包括向哺乳动物施用(i)包含可结合预先选定的细胞类型的抗体结合位点和可诱导抗预先选定细胞类型的免疫反应的细胞因子的免疫细胞因子,和(ii)与免疫细胞因子单独激起的免疫反应相比,可增强免疫反应的足够量的免疫细胞因子摄入促进剂。
在一个优选的实施方案中,预先选定的细胞类型可为存在于如实体瘤中的癌细胞,更优选地为存在于较大的实体瘤(即大于约100mm3)中的癌细胞。另外,预先选定的细胞类型可为存在于小转移灶中的癌细胞。
在另一优选的实施方案中,免疫细胞因子摄入促进剂可与免疫细胞因子同时施用。或者,免疫细胞因子摄入促进剂可在免疫细胞因子施用之前施用。进而,认为可将免疫细胞因子与多种不同的免疫细胞因子摄入促进剂一起施用。另外认为一种免疫细胞因子摄入促进剂可与多种不同的免疫细胞因子一起施用。
另一方面,本发明提供了用以在哺乳动物中诱导抗预先选定的细胞类型的杀细胞免疫反应的组合物。组合物包括以下两者的组合(i)包含可结合预先选定的细胞类型的抗体结合位点和可在哺乳动物中诱导抗预先选定的细胞类型的免疫反应的细胞因子的免疫细胞因子,和(ii)与免疫细胞因子单独激起的杀细胞反应相比,可增强由本组合中使用的免疫细胞因子诱导的杀细胞反应的足够量的免疫细胞因子摄入促进剂。
在一个优选的实施方案中,免疫细胞因子的抗体结合位点优选地包含免疫球蛋白重链或其抗原结合片段。免疫球蛋白重链自氨基末端到羧基末端优选地包含可结合预先选定抗原的免疫球蛋白重链可变区结构域(VH),免疫球蛋白重链恒定区结构域1(CH1),免疫球蛋白重链恒定区结构域2(CH2),以及任选地还可包含免疫球蛋白重链恒定区结构域3(CH3)。在一个更优选的实施方案中,免疫细胞因子为包含免疫球蛋白重链或其抗原结合片段通过一段多肽链与细胞因子结合而形成的融合蛋白。因此,优选的抗体-细胞因子结合蛋白自氨基末端到羧基末端包含(i)含有可结合预先选定的细胞类型上的细胞表面抗原的免疫球蛋白可变区、免疫球蛋白CH1结构域、免疫球蛋白CH2结构域(任选地还含有免疫球蛋白CH3结构域)的抗体结合位点,和(ii)细胞因子。制备和使用此类融合蛋白的方法在Gillies等(1992)美国国家科学院院报(Proc.Natl.Acad.Sci.USA)89:1428-1432;Gillies等(1998)免疫学杂志160:6195-6203;和美国专利号5,650,150中有详细描述。
免疫球蛋白恒定区结构域(即CH1、CH2和/或CH3结构域)可为在天然存在的抗体中与可变区结构域正常相连的恒定区结构域。或者,免疫球蛋白恒定区结构域中的一个或多个恒定区结构域可来自于与用做可变区结构域来源的抗体所不同的抗体。换句话讲,免疫球蛋白可变区和恒定区结构域可能来自不同的抗体,例如来自不同物种的抗体。参见如美国专利号4,816,567。此外,免疫球蛋白可变区可包含来自一种物种(例如人)的构架区(FR)序列和插入在FRs间的来自第二种不同物种(例如小鼠)的互补决定区(CDR)序列。制备和使用此类嵌合免疫球蛋白可变区的方法如在美国专利No.5,225,539和5,585,089中公开。
优选地,基于抗体的免疫细胞因子还包含免疫球蛋白轻链,后者优选地通过如二硫键与免疫球蛋白重链共价结合。连结在一起的免疫球蛋白重链和轻链可变区共同决定了一个单一的和完整的用于结合预先选定抗原的结合位点。在其它实施方案中,免疫细胞因子包含两条嵌合链,每一嵌合链均包含与细胞因子融合的至少一部分免疫球蛋白重链。该两条嵌合链优选地通过如一个或多个链间二硫键共价结合在一起。
因而,本发明提供了将抗体的抗原结合特异性和活性与细胞因子的强生物活性相结合的融合蛋白。本发明的融合蛋白可用于在体内将细胞因子选择性地递送至靶细胞,从而细胞因子可在靶细胞附近发挥局部生物学效应。在一个优选的实施方案中,融合蛋白的抗体成分与癌细胞表面或内部的抗原特异性地结合,结果融合蛋白发挥了局部抗癌活性。在另一优选的实施方案中,融合蛋白的抗体成分与病毒感染的细胞如HIV感染的细胞特异性结合,结果融合蛋白发挥了局部抗病毒活性。
可掺入本发明的免疫细胞因子中的细胞因子包括如肿瘤坏死因子、白细胞介素、集落刺激因子、淋巴因子以及本领域公知的其它因子。优选的肿瘤坏死因子包括如肿瘤坏死因子α(TNFα)。优选的白细胞介素包括如白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、白细胞介素-5(IL-5)、白细胞介素-7(IL-7)、白细胞介素-12(IL-12)、白细胞介素-15(IL-15)和白细胞介素-18(IL-18)。优选的集落刺激因子包括如粒细胞-巨噬细胞集落刺激因子(GM-CSF)和巨噬细胞集落刺激因子(M-CSF)。优选的淋巴因子包括如淋巴毒素(LT)。其它有用的细胞因子包括干扰素,有IFN-α、IFN-β和IFN-γ三种形式,均具有独立于其抗病毒活性之外的免疫学效应和抗血管生成效应。
现已发现有几种类型的化疗剂是有效的免疫细胞因子摄入促进剂。特别是,有用的免疫细胞因子摄入促进剂包括紫杉烷类和烷化剂类化疗剂。目前已知有几种紫杉烷类化疗剂(参见Bissery和Lavelle,1997,癌症疗法:实验和临床药剂(Cancer Therapeutics:Experimental and ClinicalAgents.),第8章,B.Teicher编辑)。在一个优选的实施方案中,紫杉烷类为紫杉酚(Taxol),也称为紫杉醇(Paclitaxel)。其它的实施方案包括在某些肿瘤模型和临床适应症上比紫杉醇更有效的半合成紫杉烷-多烯紫杉醇(docetaxel)。进一步的实施方案包括另外的紫杉烷衍生物,如那些来自天然起始物质10-脱乙酰浆果赤霉素III的紫杉烷衍生物,其中所述10-脱乙酰浆果赤霉素III提取自欧洲紫杉树的针叶。此类衍生物的实例之一是可口服利用的化合物IDN5109,它是P-糖蛋白的不良底物,通常对多药抗性肿瘤更为有效。除口服可生物利用外,IDN5109还具有较高的耐受剂量,并显示出较小的神经毒性副作用(Polizzi等,1999,癌研究59:1036-1040)。
本发明也提供了供免疫细胞因子和免疫细胞因子摄入促进剂联用的优选的剂量和施用方案。

发明详述
研究显示,大的实体瘤通常比已播散的转移灶对抗体介导的治疗性干预措施和免疫治疗更具抗性(Sulitzeanu等(1993)癌研究进展(Adv.CancerRes.)60:247-267)。认为对基于抗体的治疗的低反应性部分是由于肿瘤产生的免疫抑制性因子。
尽管对肿瘤根除的机制还没有完全了解,现在认为细胞毒性T淋巴细胞(CTL)反应可破坏癌细胞并提供免疫记忆。此外,认为在某些情况下当缺乏CTLs时自然杀伤(NK)细胞负责根除肿瘤。某些肿瘤所产生的不同类型或数量的下调T细胞的物质可导致不同的免疫反应。对于已经达到危象体积的、可以产生和分泌足够量的免疫抑制因子来调节抗肿瘤免疫反应的实体瘤而不是对微小的转移灶而言尤其如此。
现已发现,由免疫细胞因子引发的抗预先选定细胞类型的杀细胞免疫反应可通过免疫细胞因子和免疫细胞因子摄入促进剂同时施用得到显著增强。联合治疗在介导疾病组织(如已建立的肿瘤)的免疫破坏方面尤其有效。这里不受理论所束缚,认为免疫细胞因子摄入促进剂增强了免疫细胞因子向肿瘤微环境内的渗透,使其可以克服免疫抑制效应和更为有效地活化抗肿瘤细胞免疫反应。与此相似,认为此方法在治疗相似的免疫抑制机理阻碍了有效的细胞免疫产生的某些病毒性疾病时也同样有用,例如在治疗HIV感染时。认为免疫细胞因子摄入促进剂与免疫细胞因子协同作用介导了对疾病组织如已建立的肿瘤或病毒感染细胞的免疫破坏。本发明也描述了制备和使用有用的免疫细胞因子的方法,并描述了当与合适的免疫细胞因子摄入促进剂联用时,用于临床前体内动物模型中检测其药物动力学活性的测定方法。
如本文中所用的,术语“免疫细胞因子摄入促进剂”是指任何可增强免疫细胞因子所诱导的抗预先选定的细胞类型的杀细胞免疫反应的药剂。更明确地,优选的免疫细胞因子摄入促进剂是增强免疫细胞因子向肿瘤内渗透的肿瘤摄入促进剂。免疫细胞因子摄入促进剂的实例包括但不限于化疗剂如紫杉烷类、包括烷化剂类化疗剂在内的DNA损伤药剂、放射治疗药剂和调节血压的药剂。优选的紫杉烷类包括紫杉醇、多烯紫杉醇、10-脱乙酰浆果赤霉素III以及它们的衍生物。优选的烷化剂为环磷酰胺、卡铂、顺铂以及它们的衍生物。优选的放射治疗形式为伽玛射线照射。优选的血压调节剂为血管紧张素II激动剂,例如血管紧张素II本身,其优选地根据Netti等(癌研究[1995]55:5451-8)和Netti等(美国国家科学院院报[1999]96:3137-3142)描述的总体方案周期性施用。免疫反应通过为本领域内任何一个普通技术人员所知的方法和/或本文中所描述的方法测定。
如本文中所用的,术语“杀细胞免疫反应”是指被免疫细胞因子激起的杀死或降低该哺乳动物体内预先选定的细胞类型的成活力的哺乳动物体内的任何免疫反应,其实质上为体液免疫反应或细胞免疫反应。免疫反应包括T细胞、NK细胞和巨噬细胞三者中的一种或多种细胞类型。
如本文中所用的,术语“免疫细胞因子”是指这样的融合蛋白,其(i)具有结合特异性并可结合预先选定的抗原如细胞型特异性抗原的抗体结合位点和(ii)能够特异诱导或激起抗癌细胞或病毒感染细胞的杀细胞免疫反应的细胞因子。预先选定的抗原的实例包括如癌细胞或病毒感染细胞上的细胞表面抗原和如仍可附着在细胞膜上的坏死细胞的不可溶胞内抗原。优选的抗原是表明肿瘤细胞特征的靶抗原,如肿瘤特异抗原。因此,免疫细胞因子能够在体内选择性地将细胞因子递送到靶位点(一般是细胞)从而使细胞因子可以介导抗靶细胞的局部免疫反应。例如,如果免疫细胞因子的抗体成分选择性地结合癌细胞(例如实体瘤内,特别是体积比约100mm3大的实体瘤内的癌细胞)上的抗原,免疫细胞因子即发挥局部抗癌活性。另外,如果免疫细胞因子的抗体成分选择性地结合病毒感染的细胞(例如HIV感染的细胞)上的抗原,免疫细胞因子即发挥局部抗病毒活性。
如本文中所用的,术语“抗体结合位点”是指免疫球蛋白重链的至少一部分,例如可结合预先选定的抗原(如某种细胞类型)的免疫球蛋白可变区。抗体结合位点也优选地包含免疫球蛋白恒定区(包括如CH1结构域、CH2结构域和任选地CH3结构域)的至少一部分,或者至少一个CH2结构域,或者其一个或多个部分。此外,免疫球蛋白重链可共价或非共价地与包含如免疫球蛋白轻链可变区和任选地轻链恒定区的轻链结合。因此,认为抗体结合位点可包括能结合预先选定抗原的完整抗体或其片段、或单链抗体。
关于免疫细胞因子,认为抗体片段可通过为本领域普通技术人员所周知的多种方法与细胞因子连接。例如,抗体结合位点优选地经多肽键或接头与融合蛋白构建体中的细胞因子相连接。或者,抗体结合位点可经抗体结合位点和细胞因子内氨基酸侧链中存在的反应基如巯基与细胞因子化学偶联。
如本文中所用的,术语“细胞因子”是指在哺乳动物中可刺激或诱导抗预先选定的细胞类型,如癌细胞和病毒感染细胞的杀细胞免疫反应的任何蛋白质或多肽、其类似物或功能片段。因此,认为多种细胞因子均可连接入本发明的免疫细胞因子中。有用的细胞因子包括如肿瘤坏死因子(TNFs)、白细胞介素(ILs)、淋巴因子(Ls)、集落刺激因子(CSFs)、干扰素(IFNs),也包括其可刺激或诱导此类杀细胞免疫反应的种内变体、截短的类似物。有用的肿瘤坏死因子包括如TNFα。有用的淋巴因子包括如LT。有用的集落刺激因子包括如GM-CSF和M-CSF。有用的白细胞介素包括如IL-2、IL-4、IL-5、IL-7、IL-12、IL-15和IL-18。有用的干扰素包括如IFN-α、IFN-β和IFN-γ。
编码特定的目标细胞因子的基因可从头克隆、自可获得源获取或通过标准DNA合成方法按已知的核苷酸序列合成。例如,LT的DNA序列是已知的(参见如Nedwin等(1985)核酸研究(NUCLEIC ACIDS RES.)13:6361),同样已知IL-2的序列(参见如Taniguchi等(1983)自然(NATURE)302:305-318)、GM-CSF的序列(参见如Gasson等(1984)科学(SCIENCE)266:1339-1342)和TNFα的序列(参见Nedwin等(1985)核酸研究13:6361)。
在一个优选的实施方案中,免疫细胞因子为使用常规重组DNA技术,即通过形成编码该嵌合免疫细胞因子的核酸构建体而产生的重组融合蛋白。在现有技术中已描述了重组抗体-细胞因子融合蛋白的构建。参见如Gillies等(1992)美国国家科学院院报89:1428-1432;Gillies等(1998)免疫学杂志160:6195-1203和美国专利号5,650,150。优选地,编码本发明的免疫细胞因子的基因构建体从5`至3`方向上包括编码免疫球蛋白重链可变区结构域的DNA片段、编码免疫球蛋白重链恒定区结构域的DNA片段和编码细胞因子的DNA。将融合基因组装进或插入表达载体以转染融合基因在其中表达的适当受体细胞。优选地,将杂合多肽链与免疫球蛋白轻链结合以使免疫球蛋白重链可变区(VH)和免疫球蛋白轻链可变区(VL)结合产生单一且完整的结合预先选定的抗原的位点。在一个优选的实施方案中,免疫球蛋白的重链和轻链如通过链间二硫键共价偶联。此外,其中之一或两者都与细胞因子融合的两条免疫球蛋白重链可如通过一个或多个链间二硫键共价偶联。
因此,本发明的方法用于增强在治疗肿瘤的治疗方法(包括W099/29732、WO99/43713、WO99/52562、WO99/53958和WO01/10912中公开的免疫细胞因子组合物和方法)中使用的免疫细胞因子和连接区氨基酸序列发生改变的基于抗体的融合蛋白的抗肿瘤活性。在一个实施方案中,本发明的方法用于与Fc融合蛋白如Fc-干扰素α融合蛋白联合应用。
图1显示了代表性的免疫细胞因子1的结构示意图。在此实施方案中,细胞因子分子2和4为结合于抗体重链14和16的CH3区10和12的羧基末端6和8的肽。在典型的IgG构型中VL区26和28与VH区18和20配对,从而在免疫细胞因子1的氨基末端提供了2个抗原结合位点30和32,并在免疫细胞因子1的羧基末端提供了2个细胞因子受体结合位点40和42。当然,在其更广的方面,免疫细胞因子并不需要如图所示配对,或仅两条免疫球蛋白重链之一需要与细胞因子分子融合。
本发明的免疫细胞因子由于它们结构的两个方面可被认为是嵌合的。首先,免疫细胞因子是嵌合的,这是因为它包含有与给定细胞因子相连接的具有抗原结合特异性的免疫球蛋白重链。其次,本发明的免疫细胞因子从它包含来自不同抗体的免疫球蛋白可变区(V)和免疫球蛋白恒定区(C),以至产生V/C嵌合体蛋白这个意义上讲是嵌合的。例如,恒定区和可变区可来源于从不同物种分离的天然存在的抗体分子。参见如美国专利4,816,567。也包括免疫球蛋白可变区之一或两者均包含来源于不同物种的构架区(FR)序列和互补决定区(CDR)序列的构建体。此类构建体如在Jones等(1986)自然321:522-525,Verhoyen等(1988)科学239:1534-1535和美国专利号5,225,539和美国专利号5,585,089中公开。此外,认为可变区序列还可通过筛选文库例如噬菌体展示文库而获得以预期的亲和力结合预先选定的抗原可变区序列。制备和筛选噬菌体展示文库的方法如在Huse等(1989)科学246:1275-1281和Kang等(1991)美国国家科学院院报88:11120-11123中公开。
免疫细胞因子的免疫球蛋白重链恒定区结构域可选自任一称为IgA(Igα)、IgD(Igδ)、IgE(Igε)、IgG(Igγ)和IgM(Igμ)的5种免疫球蛋白类。但是,优选的是来自免疫球蛋白IgG类的重链恒定区。此外,认为免疫球蛋白重链恒定区可来自任一称为IgG1、IgG2、IgG3和IgG4的IgG抗体亚类。已知每一免疫球蛋白重链恒定区包含4或5个结构域。结构域依次命名如下:CH1-铰链区-CH2-CH3-(-CH4)。CH4存在于没有铰链区的IgM。在免疫球蛋白的不同类中,重链结构域的DNA序列具有交叉同源性,例如IgG的CH2结构域与IgA和IgD的CH2结构域同源,与IgM和IgE的CH3结构域同源。免疫球蛋白轻链具有卡帕(κ)或拉姆达(λ)恒定链两者之一。这些免疫球蛋白区域的序列和序列比较在现有技术中已经众所周知(参见如Kabat等“免疫学重要性蛋白的序列”(“Sequences of Proteins of Immunological Interest”)美国健康和福利部(U.S.Department of Health and Human Services),第三版1983,第四版1987,和Huck等(1986)核酸研究14:1779-1789)。
在优选的实施方案中,可变区来源于特异性识别预先选定的细胞表面抗原(与疾病细胞例如癌细胞或病毒感染细胞相关的抗原)的抗体,且恒定区包含来自与作为可变区来源的抗体相同或不同的抗体的CH1和CH2(任选地CH3)结构域。在本发明的实施中,免疫细胞因子的抗体部分优选地在预期的受体体内没有免疫原性或有弱免疫原性。因此,优选地,抗体部分尽可能地来源于与预期受体相同的物种。例如,如果免疫细胞因子将施用于人,恒定区结构域优选地为人源的。参见如美国专利号4,816,567。此外,当免疫球蛋白可变区来源于与预期受体不同的物种时,例如当可变区序列为鼠源性的而预期的受体是人时,则可变区优选地包含将鼠CDR序列插入人FR序列之间的人FR序列,以产生具有对预先选定抗原的结合特异性但同时在预期宿主体内又具有最小的免疫反应性的嵌合可变区。此类嵌合可变区的设计和合成在Jones等(1986)自然321:522-525,Verhoyen等(1988)科学239:1534-1535和美国专利号5,225,539和美国专利号5,585,089中公开。人源化的抗体-细胞因子融合蛋白:KS-1/4抗Ep-CAM抗体-IL-12融合蛋白的克隆和表达以及其清除已建立的结肠癌转移的能力在Gillies等(1998)免疫学杂志160:6195-6203中已经做了描述。
将编码细胞因子的基因直接地或者通过接头例如通过编码(Gly4-Ser)3接头的DNA与编码免疫球蛋白恒定区(如CH2或CH3外显子)基因的3’端按正确的编码框相连接。在某些实施方案中,接头可包含编码蛋白水解裂解位点的核苷酸序列。当此位点插入免疫球蛋白恒定区和细胞因子之间时,设计此位点可在靶位点通过蛋白水解释放该细胞因子。例如,众所周知纤溶酶和胰蛋白酶在赖氨酸和精氨酸之后蛋白酶可到达的位点处切割。其它位点特异性蛋白内切酶和它们切割的氨基酸序列目前已经广为人知。优选的蛋白水解裂解位点和识别此类裂解位点的蛋白水解酶在美国专利号5,541,087和美国专利号5,726,044中已经公开。
任选地,核酸构建体可包括用于可变区编码基因的内源性启动子和增强子以调节嵌合免疫球蛋白链的表达。例如,包含前导肽、轻链的VJ基因(功能性重排的具有连接片段(J)的可变区(V))或重链的VDJ基因以及这些基因的内源性启动子和增强子的DNA片段可作为编码可变区的基因。另外,也可获得不含内源性调控元件的可变区编码基因并在提供了这些元件的表达载体中使用。
可变区基因可通过标准的DNA克隆步骤自产生目标抗体的细胞中获得。使用适当的DNA探针如用包含J区DNA序列及其下游序列的DNA片段筛选基因组文库以获得特异的功能性重排的可变区。正确克隆的鉴定和确认可通过对已克隆的基因进行序列测定,然后将测定的序列与全长的、正确剪接的mRNA的对应序列进行比较来进行。
靶抗原可为肿瘤细胞或癌细胞、病毒感染的细胞或其它疾病细胞的细胞表面抗原。靶抗原也可为坏死细胞的不可溶胞内抗原(参见如美国专利号5,019,368)。编码合适可变区的基因通常可自产生免疫球蛋白的淋巴细胞系获得。例如,通过本领域广为人知的标准体细胞杂交技术可获得产生针对特异性抗肿瘤相关抗原或病毒抗原的免疫球蛋白的杂交瘤细胞系(参见美国专利号4,196,265)。这些产生免疫球蛋白的细胞系提供了功能性重排形式的可变区基因的来源。因为该鼠杂交瘤体系可产生具有预期特异性的各种各样的免疫球蛋白,所以可变区基因一般为鼠源性的。此外,可变区序列也可来源于筛选文库,如筛选噬菌体展示文库用以获得以预期的亲和力结合预先选定抗原的可变区序列。制备和筛选噬菌体展示文库的方法如在Huse等(1989)科学246:1275-1281和Kang等(1991)美国国家科学院院报88:11120-11123中已经公开。
编码功能上具有活性的可变区基因的DNA片段与包含编码预期的恒定区(或其一部分)基因的DNA片段相连接。免疫球蛋白恒定区(重链和轻链)可通过标准的基因克隆技术自抗体产生细胞中获得。两类人抗体轻链(κ和λ)和五类人抗体重链(α、δ、ε、γ和μ)的基因已经得到克隆,因此人源性的恒定区易于自这些克隆中获得。
将编码杂合免疫球蛋白重链的融合基因组装或插入用以掺入受体细胞的表达载体中。通过标准的基因剪接步骤可实现将基因构建体导入质粒载体。嵌合免疫球蛋白重链可与相应的免疫球蛋白轻链在相同的细胞中共表达,这样可同时表达并组装完整的免疫球蛋白。为此,重链和轻链构建体可位于同一载体中或可位于分别的载体中。
受体细胞系通常为淋巴样细胞。优选的受体细胞是骨髓瘤(或杂交瘤)。骨髓瘤可合成、组装和分泌转染基因所编码的免疫球蛋白,并且它们也可糖基化蛋白。特别优选的受体或宿主细胞包括正常情况下不产生内源性免疫球蛋白的Sp2/0骨髓瘤和小鼠骨髓瘤NS/0细胞。在转染后,细胞只产生由转染基因构建体所编码的免疫球蛋白。转染的骨髓瘤可在培养物中生长或在小鼠腹腔中生长,在小鼠腹腔中生长时分泌的免疫球蛋白可自腹水中收集。其它的淋巴样细胞如B淋巴细胞可用作受体细胞。
有几种方法用于将包含编码嵌合免疫球蛋白链的核酸构建体的载体转染淋巴样细胞。例如,载体可通过原生质体融合(参见如Gillies等1989)生物技术(BIOTECHNOL.)7:798-804)导入淋巴细胞。其它有用方法包括电穿孔或磷酸钙沉淀(参见如Sambrook等编著(1989)“分子克隆:实验室手册”(“Molecular Cloning:A Laboratory Manual”)冷泉港出版社)。
其它产生免疫细胞因子的有用方法包括制备编码该构建体的RNA序列,并将该RNA序列在适当的体内或体外表达系统中翻译。可以认为:用以合成编码抗体-细胞因子融合蛋白基因、用以将基因导入宿主细胞、用以在宿主中表达基因以及用以收获所得融合蛋白的重组DNA方法学在本领域是公知的并且已被彻底证明了。具体的方法如在Sambrook等编著(1989)“分子克隆:实验室手册”冷泉港出版社出版的一书中已经描述。
已经了解化学偶联的免疫细胞因子可使用目前广为本领域技术人员所知的多种方法制备。如可利用抗体或抗体片段和细胞因子中化学上具反应性的氨基酸侧链将抗体或抗体片段与细胞因子偶联。氨基酸侧链可通过如二硫键共价连接,或通过包括如N-琥珀酰亚胺基3-(2-吡啶基二硫代)丙酸酯、间马来酰亚胺苯甲酰基-N-羟基琥珀酸酯、间马来酰亚胺苯甲酰基-N-羟基磺基琥珀酰亚胺酯和1,4-二-[3’(2`-吡啶基硫代)丙酰胺基]丁烷的同型或异型双功能交联剂共价连接,所有这些试剂均可从Pierce,Rockford,IL公司购得。
根据本发明的方法,免疫细胞因子与免疫细胞因子摄入促进剂的联合用于已增强的免疫系统的刺激,从而在靶向的细胞类型如肿瘤细胞或其它疾病细胞的位点处产生细胞毒反应。由于免疫细胞因子单独没有细胞毒性,可以预期免疫细胞因子与免疫细胞因子摄入促进剂的联合在体外将没有组合的或协同的抗肿瘤效应。
这里不受理论所束缚,认为体内组合治疗的效应可包括通过某一作用剂的作用增强了另一作用剂的摄入,从而导致下列之一或两者:(1)增加了化学治疗的细胞毒性(如果免疫细胞因子增加了肿瘤细胞对化学治疗性免疫细胞因子摄入促进剂的摄入);和/或(2)增加了免疫刺激(如果免疫细胞因子摄入促进剂以某种方式增加了肿瘤对免疫细胞因子的摄入)。关于机理(1),早期研究已显示通过预先使用诱导局部血管渗漏的高剂量抗体-IL2免疫结合物处理可增加肿瘤对放射性标记抗体(并估计可增加对小分子药物)的摄入(参见如Hornick等,1999,临床癌研究(CLINCANCER RES)5:51-60)。如果这一特定机理在免疫细胞因子和免疫细胞因子摄入促进剂联合治疗中是起作用的,那么首先以免疫细胞因子处理荷瘤动物将是必须的。然而,如果在免疫细胞因子处理前首先施用一次免疫细胞因子摄入促进剂导致抗肿瘤活性上的协同效应,则机理(1)就不起作用。相反地,更可能的解释是用免疫细胞因子摄入促进剂的处理通过机理(2)增加了免疫细胞因子的摄入。通过证明将放射性标记免疫细胞因子与免疫细胞因子摄入促进剂共同施用增加了实体瘤对放射性标记免疫细胞因子的摄入可进一步支持这一假设。
根据本发明的方法,联合治疗的优势在于:免疫细胞因子的施用增强了起免疫细胞因子摄入促进剂作用的化学治疗剂的细胞毒效应。因此,可对患者施用较低剂量的化学治疗剂。所以,经常与化学治疗药剂相关的对患者免疫系统某些方面的抑制就被降低了。在本发明的一个实施方案中,在免疫细胞因子施用前施用了一次化学治疗性免疫细胞因子摄入促进剂。化学治疗性免疫细胞因子摄入促进剂优选地在免疫细胞因子施用前约4天和4小时之间施用,且最优选地是在约24-48小时间施用。在本发明的另一实施方案中,在免疫细胞因子施用前对患者施用了多次化学治疗性免疫细胞因子摄入促进剂。在本发明的其它实施方案中,化学治疗性免疫细胞因子摄入促进剂可在免疫细胞因子施用前施用、同时施用和/或施用后施用。
紫杉醇是可抑制或危害患者免疫系统的化学治疗性免疫细胞因子摄入促进剂的一个例子。虽然紫杉醇的大部分免疫加强效应是由巨噬/单核细胞介导的,一些关于淋巴细胞功能的研究显示紫杉醇对这一亚群的细胞具有危害。例如,发现紫杉醇处理可严重降低正常和荷瘤小鼠淋巴细胞的增殖能力(Mullins等,1998,免疫药理学和免疫毒理学(IMMUNOPHARMACOL IMMUNOTOXICOL)20:473-492),并且损害NK细胞的细胞毒活性和在含IL-2的细胞培养物中损害淋巴因子活化的细胞毒活性的产生(Chuang等,1993,妇科学和癌症学(GYNECOLONCOL)49:291-298)。事实上,现有的证据证明淋巴细胞的细胞亚群是免疫细胞因子抗肿瘤活性的主要效应细胞群(Lode等,1998,药理学与治疗(PHARMACOL THER)80:277-292)。本发明举出的实验证据已经揭示出多个现有技术不能预见的新发现,尤其是关于药物施用顺序的新发现。
紫杉烷类可与免疫细胞因子同时一起施用,或者同时通过不同的施用途径分别而施用。本发明的组合物可通过适用于特定分子的任何途径施用。因此,根据需要可口服或肠胃外施用(包括静脉内施用和腹腔施用)。
本发明的组合物可通过任一合适的手段直接(例如局部地,通过注射、植入或局部施用至组织位点)或全身性地(例如肠胃外地或口服地)提供给动物。组合物肠胃外,例如通过静脉、皮下、眼、腹腔、肌内、口腔、直肠、阴道、眼窝内、脑内、头盖内、脊髓内、心室内、膜鞘内、池内、囊内、鼻内施用或通过气溶胶施用时,组合物优选地包含含水的或生理相容的流体悬液或溶液。因此,载体是生理可接受的物质,所以载体除将所需的组合物递送给患者外,它不能反过来影响患者的电解质和/或容积平衡。因此药剂的流体介质可包含生理盐水(例如9.85%NaCl水溶液,0.15M,pH7-7.4)。对于许多紫杉烷类来说,由于它们通常溶解性不好,配方一般是比较复杂的。例如,紫杉醇的标准配方是10%Cremophor、10%乙醇和80%盐水(0.9%NaCl),而多烯紫杉醇的配方则为在施用前以5%葡萄糖溶液按1∶10进行稀释的1∶1的乙醇∶多乙氧基醚溶液(Bissery和Lavelle,1999)。然而,其它的包含紫杉烷类和新合成的紫杉烷类似物的配方将被本领域的技术人员认可和/或常规性地开发出来。
每次施用免疫细胞因子的优选剂量在0.1mg/m2-100mg/m2的范围之间,更优选地在1mg/m2-20mg/m2,且最优选地在2mg/m2-6mg/m2的范围之间。免疫细胞因子摄入促进剂的优选剂量通常取决于使用的免疫细胞因子摄入促进剂的类型,但最适剂量可通过常规实验来确定。可通过定期大丸剂注射,或通过置于外部(如静脉袋)或内部(如生物降解性植入物)的储存容器连续静脉内或腹腔施用来进行免疫细胞因子和/或免疫细胞因子摄入促进剂的施用。此外,认为本发明的免疫细胞因子也可与多种不同的免疫细胞因子摄入促进剂一起施用于预期的受体。但是,认为免疫细胞因子和免疫细胞因子摄入促进剂的最佳组合、施用模式和剂量可通过本领域技术水平之内的常规实验确定。
可应用多种方法评价使用抗体-细胞因子融合蛋白和免疫细胞因子摄入促进剂联合治疗对免疫反应的功效。例如,下面实施例中描述的动物模型或其它合适的动物模型,可被熟练的技术人员用来检测何种免疫细胞因子摄入促进剂或何种免疫细胞因子摄入促进剂的组合在与免疫细胞因子(如抗体-IL2融合蛋白)协同增强对已建立的肿瘤的免疫破坏时最为有效。免疫细胞因子摄入促进剂或免疫细胞因子摄入促进剂的组合可在免疫细胞因子治疗之前施用,或在免疫细胞因子治疗的同时施用,且对肿瘤的效应可通过对肿瘤体积的测量方便地监控。此外,当鉴定出新的免疫细胞因子摄入促进剂时,熟练的技术人员能够使用此处描述的方法评价这些新化合物增强或改善抗体-细胞因子融合蛋白抗癌活性的潜力。
另外,在治疗之后,为了评价联合治疗对免疫反应的效应,肿瘤可被切除、切片和经标准的组织学方法或经特异性的免疫组织学试剂染色。例如,简单的苏木素和伊红染色可揭示实体瘤内指示细胞免疫反应的淋巴细胞侵润的差异。此外,使用抗特定群的免疫细胞的抗体进行切片的免疫染色可揭示所诱导反应的性质。例如,结合CD45(通用淋巴细胞标志)、CD4和CD8(用于T细胞亚群鉴定)和NK1.1(NK细胞上的标志)的抗体可用来评价本发明的免疫细胞因子介导的免疫反应的类型。
另外,免疫细胞因子介导的免疫反应的类型可通过常规的细胞亚群耗竭研究,例如Lode等,(1998)血液91:1706-1715描述的研究方法来评价。耗竭性抗体的实例包括那些与T细胞标志CD4和CD8,以及那些与NK细胞标志NK1.1和脱唾液酸GM结合的抗体。简而言之,这些抗体在开始抗体-细胞因子处理前以相当高的剂量(如剂量为约0.5mg/小鼠)注射入哺乳动物体内,然后以周为间隔施用直到实验结束。此技术可鉴定在哺乳动物体内为引起观察到的免疫反应所必需的细胞类型。
在另一方法中,分离自经联合治疗处理的动物的脾细胞的细胞毒活性可与那些分离自其它治疗组的动物的脾细胞相比较。通过在大部分免疫学实验室手册中都有的标准技术将采集的无菌脾脏机械切碎后制备脾细胞培养物。参见Coligan等(编著)(1988)“最新免疫学方法”(“CurrentProtocols in Immunology”),John Wiley & Sons,Inc。然后将收获的细胞培养在合适的含有血清、抗生素和低浓度IL-2(~10U/mL)的细胞培养基中(如GIBCO的DMEM培养基)。例如,为比较NK细胞活性,一般培养3天是最适的,而为比较T细胞细胞毒活性,一般培养5天是最适的。细胞毒活性可通过51Cr放射性标记肿瘤靶细胞(如LLC细胞)30分钟而测定。然后除去多余的放射性标记物,将标记细胞与不同浓度的培养脾细胞混合孵育4小时。孵育结束后,使用伽玛计数仪进行细胞51Cr释放的测定,其接下来用来定量免疫细胞诱导的细胞裂解程度。传统的细胞毒性T淋巴细胞(或CTL)的活性就用这种方法测定。
通过下述非限制性实施例对本发明进行进一步的说明。
实施例1.动物模型
已经发展了小鼠癌症模型用于研究免疫细胞因子和紫杉烷类联合在介导有效的抗肿瘤细胞毒性反应中的效应。下述实施例中使用的免疫细胞因子结合EpCAM,EpCAM为在大部分上皮源性肿瘤中发现的人肿瘤抗原(参见Perez和Walker(1989)免疫学杂志142:3662-3667)。为了在免疫健全的小鼠模型中测定功效,必须在与小鼠宿主同源的小鼠肿瘤细胞表面表达人抗原。Lewis肺癌(LLC)细胞为一种广为人知的小鼠肺癌细胞系,它是为此目的所选择的第一个细胞系。已知LLC细胞系产生高水平的免疫系统抑制因子,并且在肿瘤微环境中诱导免疫细胞产生IL-10从而导致局部免疫抑制(Sharma等,1999,免疫学杂志163:5020-5028)。人肿瘤抗原EpCAM(也称为KSA)表达于LLC细胞表面,从而其可用衍生自小鼠抗-EpCAM抗体KS-1/4的免疫细胞因子体内靶向。通过利用所述的重组逆转录病毒载体(Gillies,美国专利申请09/293,042)转导EpCAMcDNA序列实现了EpCAM表达于LLC细胞表面,产生的细胞系命名为LLC/KSA。这些细胞在补充有10%热灭活的胎牛血清、L-谷氨酰胺、青霉素/链霉素和Geneticin(GIBCO)的DMEM培养基中于37℃和7%CO2的条件下培养。
代表不同组织来源癌症的其它细胞系也利用类似的方式人工设计。无免疫原性的鼠乳腺癌细胞系4T1由Paul Sondel博士(威斯康星大学)提供。4T1细胞系在皮下植入后缓慢但进行性地生长而且在手术切除原发肿瘤之前自然转移至许多器官。4T1细胞通过静脉注射也可能诱导实验性肺转移。鼠结肠癌细胞系CT26通过向BALB/c小鼠直肠内注射N-亚硝基-N-甲基氨基甲酸乙酯获得,由I.J.Fidler博士(MD Anderson cancer center,休斯顿,TX)提供。4T1和CT26细胞利用如(Gillies等,1998,免疫学杂志160:6195-6203)描述的方法转染Ep-CAM。4T1/KSA细胞以补充有10%热灭活的胎牛血清、L-谷氨酰胺、青霉素/链霉素和Geneticin(GIBCO)的RPMI培养基中于37℃和7%CO2的条件下培养。CT26/KSA细胞以补充有10%热灭活的胎牛血清、L-谷氨酰胺、维生素、丙酮酸钠、非必需氨基酸、青霉素/链霉素和Geneticin(GIBCO)的DMEM培养基中于37℃和7%CO2的条件下培养。转染细胞的培养基中添加Geneticin是为了给持KSA的表达。尽管所有转染细胞系都在其细胞表面表达人Ep-CAM分子(潜在的外来抗原),但它们都以皮肤肿瘤(皮下注射后)或转移癌(静脉注射后)的形式进行性地生长并杀死小鼠。
为进行肿瘤生长研究,LLC/KSA或CT26/KSA肿瘤被植入小鼠背部皮下。对于LLC/KSA研究,肿瘤取自多个通过注射悬于100μL PBS中的1×106个细胞的细胞悬液而制备的储备肿瘤。在约2周后,无菌收集肿瘤,通过150μm直径的筛子。细胞再通过注射器和23号针头2-3次,洗涤2次,然后重悬于PBS中。悬于100μL PBS中的1×106个LLC/KSA细胞的单细胞悬液通过301/2号针头注射入小鼠背部皮下。对于CT26/KSA的研究,在培养基中处于指数生长期的细胞以悬于100μL PBS中的1×106个细胞的单细胞悬液注射。在肿瘤已经建立后,约在植入2周后开始自第0天施用药剂。使用卡尺每周2次3维测量肿瘤。使用等式计算肿瘤体积:
肿瘤体积=1/2×3/4π(L/2×W/2×H)
其中L=肿瘤的长度,W=肿瘤的宽度,H=肿瘤的高度。研究过程中称量动物体重并监测动物总体健康状况。当肿瘤开始坏死或动物垂死时,使用CO2窒息法对动物施行安乐死。

对于实验性肝转移研究,小鼠以80mg/kg盐酸氯胺酮(Ford DodgeAnimal Health,Fort Dodge,IA)和5mg/kg甲苯噻嗪(Bayer,ShawneeMission,KS)麻醉。在第0天,悬于100μL含有25mM HEPES(GIBCO)的DMEM中的1×105个CT26/KSA细胞的单细胞悬液使用2711/2号针头脾囊下于60秒的期间注射。在另一个2分钟后脾血管以烧灼仪(Roboz,Rockville,MD)烧灼并切除脾脏。使用自动针缝合动物。接种3周后处死动物,切除动物肝脏并称重。然后固定肝脏并在Bouin’s溶液(Sigma,St.Louis,MO)中染色。

实施例2.抗体-细胞因子融合蛋白(免疫细胞因子)的制备
下述实施例中讨论了几种抗体-细胞因子融合蛋白。
huKS-huγ1-huIL2(缩写为KS-IL2)
基本如Gillies等(1998)免疫学杂志160:6195-6203和美国专利No.5,650,150中所述制备和表达编码huKS-huγ1-huIL2融合蛋白的基因。简而言之,使用Jones等(1986)自然321:522-525中公开的方法,其涉及将每一KS1/4可变区的CDRs插入人可变区具有最高度同源性的共有构架序列中,从而模建人源化的小鼠KS1/4抗体可变区(Varki等,(1984)癌症研究44:681-687)。通过使用Silicon Graphics Indigo工作站执行BioSym软件进行分子模建确认保持了CDRs的形状。然后,对蛋白质序列进行反向翻译,通过连接重叠寡核苷酸构建该基因。
基本如Gillies等(1992)美国国家科学院院报89:1428-1432中所述将获得的可变区插入至携带人κ轻链和人Cγ1重链恒定区的表达载体中,所不同的是为了表达两条链,将金属硫蛋白启动子和免疫球蛋白重链增强子替换为CMV启动子/增强子。除IL-2基因的3`非翻译区来源于SV40多聚(A)区外,通过如Gillies等(1992)美国国家科学院院报89:1428-1432中所描述的方法制备将IL-2成熟序列与人重链羧基末端融合的融合子。
通过将获得的质粒转染NS/0骨髓瘤细胞系并使用含有0.1μM氨甲蝶呤(MTX)的选择培养基使IL-2融合蛋白获得表达。简而言之,为了获得稳定转染的克隆,质粒DNA通过电穿孔方法导入小鼠骨髓瘤NS/0细胞中。NS/0细胞在含有10%胎牛血清的DMEM培养基中生长。收集约5×106个细胞,以PBS洗1次然后重悬于0.5mL的PBS中。10μg线性化的质粒DNA与细胞一起在Gene Pulser Cuvette(0.4cm的电极间隙,BioRad)中冰浴10分钟。使用设定为0.25V和500μF的Gene Pulser(BioRad,Hercules,CA)进行电穿孔。细胞在冰浴上恢复10分钟,然后以生长培养基重悬细胞并接种于2个96孔培养板中。转染后两天加入氨甲蝶呤,通过在100nM氨甲蝶呤存在下培养筛选稳定转染的克隆。细胞每3天更换培养基共更换3次以上,直至2-3周后出现MTX-抗性克隆。
通过使用合适的抗体进行Fc或细胞因子ELISA鉴定表达克隆(参见如Gillies等(1989)生物技术(Biotechnol.)7:798-804)。按照生产厂商的用法说明书,所得的融合蛋白通过与蛋白A琼脂糖凝胶(Pharmacia)的结合和洗脱获得纯化。
huKS-huγ4-huIL2
基本上如1999年2月24日申请的,要求1998年2月25日申请的U.S.S.N 60/075,887的专利优先权的U.S.S.N 09/256,156中所描述的方法构建和表达编码huKS-huγ4-huIL2融合蛋白的基因。
简而言之,通过自huKS-huγ1-huIL2表达载体中去除免疫球蛋白恒定区Cγ1基因片段并代之以来自人Cγ4基因的相应序列制备了huKS-huγ1-huIL2融合蛋白的Igγ4变体。人重链恒定区Cγ1、Cγ2、Cγ3和Cγ4的序列和序列比较在Huck等(1986)核酸研究14:1776-1789中已经公开。
通过以Hind III和Xho I消化原始携带Cγ1基因的质粒DNA并通过琼脂糖凝胶电泳纯化7.8kb的大片段实现了Cγ1和Cγ4片段的交换。携带Cγ4基因的第二种质粒以Hind III和Nsi I消化并纯化1.75kb的片段。第三种携带融合于人Cγ1羧基末端的人IL-2cDNA和SV40polyA位点的质粒以Xho I和Nsi I消化并纯化470bp的小片段。所有的3个片段以大致上相等的摩尔量连接在一起。连接产物用来转化大肠杆菌(E coli.)感受态细菌并通过在含有氨苄青霉素的平板上生长筛选菌落。通过对制备自单独的转化子的质粒DNA进行限制性酶切分析鉴定正确组装的重组质粒,进而通过Fsp I消化区分Cγ1(无Fsp I位点)和Cγ4(1个Fsp I位点)基因的插入。
最终获得的携带Cγ4-IL2重链替代的载体通过电穿孔(0.25V和500μF)方法导入小鼠骨髓瘤NS/0细胞中,通过在含有氨甲蝶呤(0.1μM)的培养基中生长筛选转染子。鉴定、扩增表达高水平huKS-huγ4-huIL2融合蛋白的细胞克隆,使用蛋白A琼脂糖凝胶色谱层析法自培养上清中纯化融合蛋白。SDS-PAGE凝胶电泳鉴定Cγ4融合蛋白的纯度和完整性。应用T细胞增殖试验(Gillis等(1978)免疫学杂志120:2027-2032)测定IL-2的活性,发现其IL-2活性与γ1-构建体的IL-2活性相同。
huKS-muγ2a-muIL2
如上所述,通过将huKS-huγ1-huIL2融合蛋白的人抗体恒定区和人IL-2以相应的鼠源性序列替代构建了编码huKS-muγ2a-muIL2融合蛋白的基因。具体地说,人Cγ1-IL2DNA被融合至编码鼠IL-2的DNA的鼠Cγ2a cDNA片段所替代。简而言之,使用下列重叠寡核苷酸引物进行重叠PCR将huKS的VH区按正确的读码框架与鼠γ2a cDNA连接:

huKS-muγ2a-muIL12
通过如1997年12月8日申请的U.S.S.N.08/986,997和Gillies等(1998)免疫学杂志160:6195-6203中描述的方法构建和表达编码huKS-muγ2a-muIL12融合蛋白的基因。简而言之,通过将鼠p35IL-12亚单位cDNA与以前制备的huKS-muγ2a重链编码区相融合而实现。获得的载体转染已经使用小鼠p40IL-12亚单位转染并能够表达p40IL-12亚单位的NS/0骨髓瘤细胞系。换句话说,仅使用小鼠p40转染细胞系,筛选获得稳定地高表达细胞用做转染包含p35的融合蛋白的受体细胞(即连续转染)。
小鼠p35和p40IL-12亚单位通过PCR自以伴刀豆凝集素A活化的脾细胞(培养基中含5μg/mL刺激3天)中制备的mRNA中分离获得。用来分离p35编码核酸序列的PCR引物也使p35 cDNA成为具有XmaI-XhoI限制性位点的片段,引物包括:

如(Gillies等,免疫学方法杂志(J.Immunol.Methods)125:191)中所描述构建了携带有dhfr选择性标志基因、编码人源化KS抗体轻链的转录单位和编码与鼠IL-12的p35亚单位融合的鼠重链的转录单位的质粒载体(pdHL7-huKS-muγ2a-p35)。通过将改变的p35亚单位cDNA的XmaI至XhoI片段与以前制备的小鼠γ2a基因CH3外显子末端唯一的XmaI位点相连接完成了融合。H和L链转录单位都包括5`末端的巨细胞病毒(CMV)启动子(取代原始质粒的金属硫蛋白启动子)和3`末端的多聚腺苷酸位点。
为表达游离的p40亚单位,构建了包含选择性标志基因(新霉素抗性基因)但仍用CMV启动子进行转录的相似载体(pNC-p40)。此时该载体的编码区包含了p40亚单位的天然前导序列,用于将p40亚单位正确地输送到内质网并与融合蛋白组装。质粒pNC-p40通过电穿孔法导入细胞,然后接种入培养板并以含有G418的培养基进行筛选。如此,通过ELISA检测抗药性克隆培养上清中p40亚单位的产生。
如Gillies等(1998)免疫学杂志160:6195-6203中所描述的,通过电穿孔法将pdHL7-huKS-muγ2a-p35表达载体电穿孔入已经表达鼠p40的NS/0细胞中。通过标准ELISA法检测氨甲蝶呤抗性转染克隆抗体决定簇和小鼠IL-12的分泌。根据生产厂商的使用说明,所得蛋白质通过与蛋白A琼脂糖凝胶柱的结合和自蛋白A琼脂糖凝胶柱上洗脱得到纯化。
实施例3.联合治疗的体外细胞毒活性
存在或缺乏包含人源化KS-1/4抗体在H链羧基末端与人IL2相融合、基于IL-2的免疫细胞因子(huKS-huγ1-huIL2,此后缩写为KS-IL2)的情况下,通过细胞培养检测了动物模型(实施例1)中使用的基因工程细胞系对tanane诱导的细胞毒性的敏感性。细胞按1000个细胞/孔接种于96孔平底培养板,在37℃,7%CO2的条件下培养24小时。紫杉醇从200ng/mL开始2倍倍比稀释至3.125ng/mL,KS-IL2 200ng/mL和IL233.3ng/Ml(与KS-IL2中的IL2相当的量)复孔加样加到细胞培养板中,在37℃,7%CO2的条件下培养6天。直接在96孔板上进行基于四唑鎓盐的细胞内转化而测定细胞活力的MTS比色检测(Promega)。在96孔板读数和记录后,活的粘附细胞以结晶紫(Sigma,St.Louis,MO)染色。结晶紫染色的培养板用以校验MTS检测结果。结果以表格的形式表示。IC50是产生对照组50%水平的细胞毒性的药物浓度。
进行了紫杉醇(3至200ng/mL)单独或与KS-IL2(200ng/mL)或IL-2(33.3ng/mL)联合抗CT26/KSA、LLC/KSA和4T1/KSA细胞的细胞毒性检测。KS-IL2或IL-2单独对3种检测的细胞系的细胞毒性几乎没有(对照的81%到101%,表1)。KS-IL2或IL-2的添加并没有影响紫杉醇的细胞毒性。因此,由于KS-IL2和IL-2都不能影响紫杉醇的细胞毒性,联合治疗对小鼠抗肿瘤活性的任何增强一定是因为其它仅在荷瘤动物体内发生的机理。

实施例4.KS-IL2和紫杉烷对LLC皮肤肿瘤的联合治疗
使用攻击性生长的肿瘤LLC/KSA进行了肿瘤生长回归检测,其中在单次紫杉醇(80mg/kg)施用1周后,接着进行经尾静脉注射施用KS-IL2(20μg)5天(图2)。紫杉醇或KS-IL2单独施用(在第0-4天)没有观察到任何效应。但是,当紫杉醇施用1周后接着施用KS-IL2时,观察到了平均肿瘤体积大大减小(对照组的41%)和约8天的肿瘤生长延迟(TGD),与紫杉醇单独施用相比差异具有显著性(p=0.023)。在紫杉醇治疗组,除<5%的体重降低之外,没有观察到药物相关的显著毒性。
接下来,通常认为是更为有效的化学治疗方案的紫杉醇多次施用的联合KS-IL-2效应与紫杉醇单次施用的联合KS-IL2效应进行了比较以确定方案是如何影响增强效应的。KS-IL2(20μg,0-4天)单独对LLC/KSA肿瘤生长没有效应,但是当多次单独施用紫杉醇时(50mg/kg,每隔1天)降低平均肿瘤体积至对照组的63%并导致4天的肿瘤生长延迟(TGD)(图3)。当KS-IL2免疫细胞因子在紫杉醇治疗1周后接着施用,肿瘤体积降至对照组的27%并观察到10天的TGD,与紫杉醇单独治疗组相比有显著性差异(p=0.016)。在紫杉醇治疗组,除<5%的体重降低之外,没有观察到药物相关的显著毒性。联合治疗组有更少的体重减轻。考虑到在化学治疗(和潜在的免疫损害)处理和基于刺激淋巴细胞增殖和细胞毒性的处理开始之间相对短的间隔,这些积极的联合治疗结果是另人惊讶的。
对联合效应的一种解释是紫杉烷诱导的生长中肿瘤团块的部分凋亡降低了肿瘤间质的压力,其接下来增加了肿瘤对KS-IL2的有效摄入。近期研究(Griffon-Etienne等1999,癌研究59::3776-3782)显示单次施用紫杉醇的效应有效地降低了间质内液体压力,最大效应发生在24-48小时间(Griffon-Etienne等1999,癌研究59:3776-3782)。尽管这可能是肿瘤摄入免疫细胞因子的最佳时间,但其也是化学治疗后一段非常短的时间间隔。而且,我们在紫杉醇单次施用刚刚24小时后开始以KS-IL2连续5天治疗荷LLC/KSA瘤小鼠。结果显示,当免疫细胞因子处理比在单次紫杉醇施用后1周更早一些开始时,产生了对LLC/KSA癌细胞系和结肠癌CT26细胞系的更好的联合效应(见下文)。
实施例5.KS-IL2和紫杉烷联合治疗4T1转移癌
由于我们发现在施用紫杉烷和免疫细胞因子之间的处理间隔应比预期的更短,我们检验了紫杉烷和免疫细胞因子在同一天施用的联合方案,并对单次(75mg/kg)和多次(25mg/kg×3天)紫杉醇与KS-IL2(15μg/剂×3天,紫杉醇施用4小时后施用)一起施用做了比较。为了这一实验我们使用了4T1/KSA乳腺癌细胞诱导的实验性肺转移模型。选择比它们单独应用时的最适剂量稍低的药物剂量,以便可观察到任何潜在的相加或协同活性。
每一药剂在单独施用时,在相似的程度上均显著(p<0.02)降低了平均肺重量:紫杉醇单次施用降低43%,紫杉醇多次单独施用降低49%,KS-IL2单独施用降低39%(图4)。紫杉醇和KS-IL2联合进一步稍微但不是相加地降低了肺转移:单次紫杉醇施用联合KS-IL2降低58%,多次紫杉醇施用联合KS-IL2降低68%。尽管没有观察到协同作用,但单次紫杉醇施用联合KS-IL2与紫杉醇单独施用相比有显著的差异(p=0.047)。
所有的实验组都观察到了<10%的体重降低,但是当紫杉醇以25mg/kg每隔1天共3次施用时,体重降低最大。基于以上数据,考虑到最佳的联合治疗效应,在该4T1肺转移检测中最好的方案为单次紫杉醇施用后接着进行KS-IL2处理,对LLC/KSA肿瘤生长回归模型也是这样的情况。由于在此情况下施用间隔仅为4小时,对有效的肿瘤摄入来说结果可能还不是最好的。
实施例6.KS-IL2和紫杉烷联合治疗CT26皮肤肿瘤
实施例5中描述的实验结果提示在两次施用之间4小时的时间间隔可能太短了。或许在KS-IL2施用时动物体内仍保持的紫杉醇水平可直接干扰淋巴细胞活化,因此降低了联合处理的潜在抗肿瘤活性。而且,在4小时时间点可能还没有达到对肿瘤间质压力的最大效应。因此,我们设计了另一实验,这次使用CT26/KSA结肠癌已经建立的皮肤肿瘤,其中我们把单次紫杉醇施用(75mg/kg)与在该紫杉烷施用24小时后开始的5天的KS-IL2处理结合起来。紫杉醇单独对肿瘤生长没有效果(图5)。以亚最佳剂量的KS-IL2(10μg,从第1至第5天)处理导致肿瘤体积降低到对照的71%。紫杉醇和KS-IL2联合处理观察到肿瘤体积显著性地和协同性地降低到对照组的8%,与紫杉醇单独处理相比有显著性差异(p<0.001)。两个紫杉醇处理组均观察到~5%的动物体重降低。
使用CT26/KSA模型进行了第二个实验,这次检测联合治疗对已经建立的肝转移的效果,并且再次在紫杉醇施用和KS-IL2处理间使用了24小时延迟。我们也比较了联合治疗中紫杉醇的剂量反应。小鼠在癌转移诱导之后第5天分别单独注射25、50和75mg/kg紫杉醇,或在1天后接着以KS-IL2(7μg)处理5天。紫杉醇单独施用观察到了剂量反应效应,其中25、50和75mg/kg的剂量分别降低肿瘤负荷至对照组的49%、23%和10%(图6)。紫杉醇与KS-IL2联合进一步降低肺转移到相同的每个紫杉醇剂量对照组的12%、9%和6%。最低紫杉醇剂量(25mg/kg)联合KS-IL2与较高紫杉醇剂量联合KS-IL2相比前者导致了肿瘤负荷最大的和最显著(p<0.001)的降低。因此,KS-IL2联合紫杉醇比两者单独应用产生了更强的抗肿瘤效应。此外,最低剂量的紫杉醇与KS-IL2联合产生了与最高剂量的紫杉醇单独施用相似的抗肿瘤效应。因此,使用较低剂量的紫杉醇与KS-IL2联合将在降低毒性的同时维持良好的功效。
实施例7.测定肿瘤对KS-IL2的摄入
如果在免疫细胞因子治疗前单次细胞毒性药物处理的效应是降低肿瘤间质压力并增加对肿瘤细胞的穿透,那么这将可使用放射性标记的免疫细胞因子例如KS-IL2来测定。通过与厂商(New England Nuclear,Billerica,MA)签定合同,使用标准方法(参考文献)将纯化的KS-IL2标记125I。如实施例1中所述将CT26/KSA皮肤肿瘤皮下植入并使之生长至100-200mm3。使用溶于载体的紫杉醇(50mg/kg)或单独用载体注射两组小鼠,每组4只小鼠,接下来1小时(实验1)或24小时(实验2)后注射10μg125I-KS-IL2(95μCi)。在注射放射性标记的免疫细胞因子6小时后,处死小鼠并外科切除肿瘤。作为对照,收集动物的肝脏并且所有的组织都被称重并在伽玛计数仪上计数。通过将组织的总CPM值除以组织重量,结果以每克组织的每分钟计数(CPM)表示。
当标记的KS-IL2在紫杉醇处理1小时后注射时(图7A),在接受药物的动物上切下的肿瘤中仅观察到放射活性的小的增加。相反,标记的KS-IL2在紫杉醇处理24小时后注射时,摄入的放射性活性与载体对照相比有了显著性的增加(>200%)。在1小时和24小时时间点肿瘤摄入的巨大差异与关于紫杉烷诱导的间质压力变化的数据(Griffon-Etienne等1999,癌研究59:3776-3782)相一致,这也与我们的肿瘤模型显示的紫杉醇施用24小时后开始处理比更早的时间(4小时)开始处理更为有效的数据相一致。
我们也检测了是否其它种类的药物也能增加实体瘤对放射性标记的免疫细胞因子的摄入。在这种情况下,小鼠在实验前24小时或3天单次注射环磷酰胺(40mg/kg)。包括以PBS预处理的对照小鼠在内的所有小鼠均注射125I标记的KS-IL2,16小时后测量切下肿瘤中的放射性活性的数量。结果(图8)显示在24小时前环磷酰胺预处理的小鼠KS-IL2摄入增加了48%,而预处理3天的小鼠KS-IL2摄入增加了70%。
实施例8 huKS-huγ4-IL2和紫杉烷的联合治疗
近来已描述了由于对Fc受体具较低的亲和力而具有循环半寿期增加和功效改善的新免疫细胞因子(参见Gillies等,1999,癌研究59:2159-2166)。检测了这些改良的IL-2免疫细胞因子中的一个代表huKS-huγ4-IL2与单次紫杉醇的联合治疗。再次表明:当两种药物依次施与负荷CT26/KSA皮肤肿瘤的小鼠具有改善的功效。
实施例9.huKS-muγ2a-muIL12和紫杉烷的联合治疗
为了检测协同治疗效应是否仅为基于IL-2的免疫细胞因子特异性的,我们首先以紫杉醇(单次施用75mg/kg)然后是5天的huKS-muγ2a-muIL12(每天5μg)处理了已建立的CT26/KSA大体积肿瘤。此免疫细胞因子代表了鼠源化HuKS抗体(即恒定区变为鼠源性的Cκ和Cγ2a)和鼠IL-12的融合。因为不象IL-2,IL-12是高度种属特异性的,人的IL-12在小鼠体内活性不很好,所以必须使用鼠IL-12序列。结果显示紫杉醇单独处理对肿瘤生长有很少的影响。以亚最适剂量的huKS-muγ2a-muIL12处理具有抗肿瘤效应,并且抗肿瘤效应在首先以单次紫杉醇处理的小鼠体内增加了。
实施例10.huKS-IL2和烷化剂药剂联合治疗
i.证实了huKS-IL2与归于烷化剂药剂类的化学治疗药物环磷酰胺联合提高了治疗效果。治疗前3天将4T1乳腺癌细胞静脉注射入免疫健全的小鼠体内以建立肺转移。小鼠以单次环磷酰胺(15、40或80mg/kg)处理,3天后继续接受5天的huKS-IL2(每天15μg)处理。尽管两个较低剂量单独施用仅造成肺转移肿瘤负荷的适当的降低,环磷酰胺和huKS-IL2联合与环磷酰胺单独处理相比前者造成肿瘤负荷显著性地(p<0.05,图9)大大降低。但是在最高剂量(80mg/kg)没有发生协同作用。
ii.在荷有建立的乳腺癌皮下肿瘤的免疫健全小鼠体内,肿瘤生长测定也证实huKS-IL2与环磷酰胺联合提高了治疗性效果。小鼠以单次环磷酰胺80mg/kg单独处理或者在环磷酰胺处理3天后联合5天的huKS-IL2单独施用(30μg)处理。对于huKS-IL2和环磷酰胺80mg/kg单独施用,平均肿瘤体积分别降低了31%和69%(图9B)。联合治疗在第25天100%地降低了平均肿瘤体积,其与huKS-IL2单独或环磷酰胺单独处理相比具有显著性差异(p<0.05),并且在开始治疗后最多12周的时间内8只小鼠中有6只完全清除了肿瘤。动物较好地耐受了这些处理,在所有的实验组中体重降低小于10%。
iii.在荷有已建立的肺癌皮下肿瘤的免疫健全小鼠体内,肿瘤生长测定也证实huKS-IL2与环磷酰胺联合提高了治疗性效果。小鼠以单次环磷酰胺80mg/kg单独处理或者在环磷酰胺处理3天后联合5天的huKS-IL2(20μg)处理。对于huKS-IL2单独施用和环磷酰胺80mg/kg单独施用,平均肿瘤体积分别降低了2%和27%(图9C)。联合治疗在第20天使平均肿瘤体积降低了48%,其与huKS-IL2单独或环磷酰胺单独处理相比具有显著性差异(p<0.05)。动物较好地耐受了处理,在所有的实验组中体重降低小于10%。
实施例11.huKS-IL2和烷化剂药剂联合治疗
证实了huKS-IL2与归于烷化剂药剂类的另一化学治疗药物卡铂联合提高了治疗效果。在第0天以卡铂(75mg/kg)处理荷有已建立的非小细胞肺癌皮下肿瘤(LLC/KSA)的小鼠,3天后继续接受5天的KS-IL2(每天20μg)处理。卡铂和KS-IL2处理单独均造成肿瘤生长适当地降低,但是仅有联合治疗在第20天显著地降低了平均肿瘤体积(p<0.05,图10)。此外,接受联合治疗的小鼠体内肿瘤的生长与卡铂单独处理小鼠相比具有显著性差异(p<0.05)。
不偏离本发明的精神和本质特征,本发明也可以有其它特定形式的具体体现。因此,前述的实施方案应理解为是全方位地举例说明本发明,而不是限制此处描述的本发明。本发明的范围通过附加的权利要求书而不是通过前边的描述来体现,在与权利要求书等同的本发明的意义和范围之内产生的所有改变都将包括在本发明之内。

Claims (15)
1.适用于在哺乳动物体内介导肿瘤的免疫破坏的组合物,其包含用于联合治疗的以下单独部分:
(i)抗体-细胞因子融合蛋白;和
(ii)增强所述抗体-细胞因子融合蛋白进入肿瘤的药剂,其选自化疗剂和DNA损伤剂,在用所述抗体-细胞因子融合蛋白治疗前使用。
2.根据权利要求1的组合物,其中抗体-细胞因子融合蛋白从氨基末端到羧基末端的方向包含(a)抗体结合位点,其包含可结合细胞表面抗原的免疫球蛋白可变区、免疫球蛋白CH1结构域和免疫球蛋白CH2结构域,和(b)细胞因子。
3.权利要求2的组合物,其中抗体-细胞因子融合蛋白还包含插入CH2结构域和细胞因子之间的CH3结构域。
4.根据权利要求1-3中任意一项所述的组合物,其中在所述抗体-细胞因子融合蛋白中的细胞因子选自肿瘤坏死因子、白细胞介素、集落刺激因子和淋巴因子。
5.根据权利要求1的组合物,其中所述化疗剂为紫杉烷类。
6.组合物的用途,用于制备治疗肿瘤或肿瘤转移的药物,其中所述组合物包含(i)抗体-细胞因子融合蛋白和(ii)增强所述抗体-细胞因子融合蛋白进入肿瘤的药剂。
7.根据权利要求6的用途,其中所述药物中的所述肿瘤进入增强剂选自化疗剂和DNA损伤剂。
8.根据权利要求6的用途,其中在所述药物的所述抗体-细胞因子融合蛋白中的细胞因子选自肿瘤坏死因子、白细胞介素、集落刺激因子和淋巴因子。
9.根据权利要求6的用途,其中肿瘤为实体瘤。
10.根据权利要求6-9中任意一项所述的用途,其中所述药物中的肿瘤进入增强剂在用所述药物中的抗体-细胞因子融合蛋白治疗前施用。
11.根据权利要求10的用途,其中所述肿瘤进入增强剂在用抗体-细胞因子融合蛋白治疗前4小时至4天之间施用。
12.根据权利要求11的用途,其中所述肿瘤进入增强剂在用抗体-细胞因子融合蛋白治疗前24小时至48小时之间施用。
13.根据权利要求12的用途,其中所述肿瘤进入增强剂在用抗体-细胞因子融合蛋白治疗前24小时施用。
14.根据权利要求10的用途,其中所述肿瘤进入增强剂以单剂量施用。
15.化疗剂或DNA损伤剂的用途,用于制备增强抗体-细胞因子融合蛋白摄入肿瘤微环境的药物。
