背景技术
胃肠道运动是消化系统生理功能的重要组成部分,胃肠功能紊乱是引起消化系统症状的因素之一,研究表明胃肠动力障碍是引起功能性消化不良的常见原因。胃肠道生理病理状态还与机体免疫密切相关。刘崇海等研究发现肠道菌群失调后,迟发型变态反应、脾指数、诱导的淋巴转化率、抗体生成细胞数和脾脏的和基因表达水平均低于正常组。梁庆红等通过灌胃抗生素造成肠道菌群失调模型,结果表明小鼠脾脏的重量减轻,淋巴细胞数量及血清中IL2的含量减低、脾脏溶血空斑数减少、巨噬细胞吞噬率降。说明肠道菌群失调导致机体特异性和非特异性免疫功能均降低。免疫功能下降又会导致机体的二重感染、或新的感染,这些不良后果又可加重菌群紊乱,形成恶性循环。因此,肠道健康与否直接关乎到生命的安危,并且是保证其他器官发挥正常功能的基础。基于此,本发明提供一种适用肠胃患者多维营养素配方。

(1)南瓜

(4)胡萝卜和白萝卜
胡萝卜又称红萝卜或甘荀,为伞形花科,一、二年生草本的根茎类蔬菜。萝卜汁和胡萝卜浆是常见的胡萝卜深加工产品,其加工附产物胡萝卜残渣富含膳食纤维,具有润肠通便功能的影响,可以通过增加粪便量和排便次数、以及通过促进肠道蠕动、缩短粪便通过肠道时间而改善肠道功能。
白萝卜又称莱菔、太根、菜头,为十字花科萝卜属一或二年生草本植物,白萝卜作为食物及药用植物应用久远,传统中医认为其味辛、甘、凉,具有消积滞、化痰热、下气、宽中、解毒的作用,可用于食积胀满、呃噎反胃等症。在胃肠动力方面,国外已有学者对白萝卜提取物刺激大鼠肠动力的生理机制及白萝卜叶提取物对豚鼠回、结肠刺激活动的药理学机制进行了研究,初步明确了其对胃肠动力的促进作用[9]。
白萝卜还含有一般蔬菜及水果不具备的莱菔甙,其与多种酶作用会产生异硫氰酸酯,可刺激肠胃蠕动。白萝卜中多样成分的组合,决定了其药理作用的复杂性。也有报道指出白萝卜中含有辛辣成分的芥子油,可促进胃肠蠕动,增食欲,助消化;芥子油还具有促进脂肪新陈代谢,可避免脂肪在皮下堆积的作用。

3.补充谷物膳食纤维可促进肠道蠕动,改善胃肠道功能
根据GB/Z 21922—2008《食品营养成分基本术语》的定义,膳食纤维(dietaryfiber)是指植物中天然存在的、提取或合成的碳水化合物的聚合物,其聚合度(degree ofpolymerization,DP)≥3,不能被人体小肠消化吸收、对人体有健康意义。除了蔬果,全谷物,包括藜麦、苦荞、薏米、青稞、玉米、小米、燕麦、糙米、黑米等也是膳食纤维的重要来源。建议在每天的主食中全谷物含量占三分之一左右。《中国居民膳食指南》(2016)对一般人群的膳食推荐一中提出,“食物多样,谷类为主”,每天建议摄入全谷物和杂豆50~150克。研究结果表明,谷物纤维能够增加排便次数、缩短肠道通过时间,对肠道运动具有促进作用,同时不增加胃肠道副反应[11]。

燕麦又称之为雀麦,是一种低糖、高营养、高能食品。燕麦中蛋白质含量十分丰富(15.6%),是大米、小麦粉的1.6-2.3倍,在禾谷类粮食中居首位。燕麦蛋白营养价值很高,含有18种氨基酸,其中8种是人体必需氨基酸。8种必需氨基酸不仅含量丰富且配比合理,接近FAO/WHO推荐的营养模式,人体利用率高。燕麦兼具可溶性和不溶性两种膳食纤维,因而又被誉为天然膳食纤维家族中的“贵族”。燕麦总纤维素含量为17-21%,其中可溶性膳食纤维(主要成分是β-葡聚糖)约占总膳食纤维的1/3,明显高于其他谷物,可降低甘油三脂的低密度脂肪蛋白,促使胆固醇排泄,同时功能通便导泄,对于习惯性便秘患者有很好的帮助。
(2)青稞
青稞营养价值高,富含蛋白质、维生素、膳食纤维,并且含有较少的脂肪和糖类,是藏民补充营养的主要食物,也是维持藏民身体健康的重要元素来源之一。青稞含有β-葡聚糖、多酚类物质、黄酮类物质和麦绿素等对人体有益的功效成分,青稞提取物对于胃肠道健康具有一定的调节作用。Chen等从青稞中分离出β-葡聚糖,应用在大鼠胃溃疡模型中,发现β-葡聚糖治疗可减轻乙醇对胃的损伤和胃黏膜的损害,可能与β-葡聚糖抗氧化、抗炎机制有关;应用在小鼠肠道健康促进模型中,β-葡聚糖的处理增加了小鼠结肠及盲肠的结肠长度、含水量和总短链脂肪酸浓度,从而证明β-葡聚糖对实验室啮齿动物的胃肠道有保护作用。Chen等从青稞酒中分离出一株罗伊氏乳杆菌WHH1689,采用右旋糖酐硫酸钠诱导的小鼠模型评价其在体内的影响,结果表明:WHH1689菌株增加了小鼠粪便中双歧杆菌的存活数,对模拟胃肠道条件具有抵抗能力,并且可以在试验性结肠炎小鼠模型中有益地修饰肠道菌群;
四、药食同源中药用于调节胃肠道的现代医学研究
胃肠道失调多伴随反酸、嗳气、腹痛、腹泻、便秘等症状,反复发作。中医认为其根本内因均为脾胃受损,运化失司,水谷不化,清浊不分。茯苓、山药、芡实、莲子均为药食同用之物,药性平和,既能调和脾胃,又能止泻安神。其中,茯苓性平和,味甘淡,统归心肺脾肾经,益气健脾除湿,同时有利尿通淋之效,一方面实脾,一方面利小便实大便。方中的茯苓,山药,莲子,芡实为中医健脾名方,有四臣汤的传说,因为乾隆服用药方之后,效果立显,故就变成‘四神汤’了。方中山药的性味平甘,为《神农本草经》中的上品,取其补缓不骤,香微不燥,其归脾、肺、肾经,具有益肾气,健脾气,止泄痢,化痰润毛等功效,山药还可以与茯苓起健脾相协之功。芡实,归于脾肾经,性味平甘涩,首载于《神农本草经》,具有益肾固精、补脾止泻、除湿止带的疗效,临床广泛用于遗精滑精,遗尿尿频,脾虚久泻,白浊带下等疾病的治疗。莲子是莲的干燥成熟种子,营养价值高,具有“莲参”之称,性平,味甘涩,归于脾肾心经,具有健脾厚肠、益肾涩精、止带调经等功效。全方配伍,可以调理脾胃,补而不滞,温而不燥,对于胃肠道疾病具有很好的调养作用。
1.茯苓对胃肠道的改善作用
茯苓是多孔菌科真菌茯苓的干燥菌核;性味甘、淡,平,归心、肺、脾、肾经;利水渗湿、健脾,宁心,无毒,功效广泛,与适宜的药物配伍,不论寒、温、风、湿诸疾,均能发挥其独特功效。
脾胃湿热证是中医常见的证型,而胃黏膜AQP3、AQP4的异常表达可能是其发病机制之一。倪文娟等筛选了茯苓三萜类利水成分,发现去氢松香酸甲酯与AQP1、AQP4、AQP5结合的活性较强,其或许是茯苓治疗脾胃湿热症的关键活性成分。张兵影等探讨茯苓水煎液、茯苓总三萜及茯苓多糖对饮食失节、过食肥甘所致的脾虚小鼠的作用,发现茯苓总三萜对脾虚小鼠具有明显的恢复作用。经研究表明,茯苓水煎液对脾气虚腹泻大鼠的小肠推进具有抑制作用,茯苓粗提物能抑制家兔离体空肠和盲肠平滑肌收缩运动,使收缩张力和舒张张力减弱,收缩振幅减小,其剂量越大作用越强,并呈量效关系。Xu等研究证明16α-羟基松苓酸能够改善肿瘤坏死因子-α(TNF-α)诱导的结肠腺癌细胞Caco-2单层肠上皮屏障损伤,并抑制磷脂酰肌醇-3激酶(PI3K)和蛋白激酶B(Akt)的激活,从而通过激活糖皮质激素受体(GR)以减弱下游核转录因子-κB抑制蛋白(IκB)和核因子-κB(NF-κB)磷酸化,改善肠屏障功能。
研究也发现,茯苓对健康小鼠腹膜孔的调控作用不明显。茯苓的水煎液能直接松弛家兔离体肠肌、减小肠肌收缩振幅;能防治大鼠实验性胃溃疡,抑制胃液分泌。研究表明,茯苓三萜及其衍生物可抑制无水硫酸铜引起的呕吐。茯苓醇提液及其17种三萜类成分的镇吐作用显示,侧链上的C-24位含有末端双键基团的三萜具有极好的止吐作用。茯苓多糖的代谢主要在肠道中进行。
2.山药对胃肠道的调节作用
山药为薯蓣科植物薯蓣的干燥根茎,具有补脾养胃、生津益肺、补肾涩精的功效。现代研究表明山药主要化学成分包括多糖、氨基酸、脂肪酸、山药素类化合物、尿囊素、微量元素、淀粉等。具有降血糖、降血脂、抗氧化、调节脾胃、抗肿瘤、免疫调节等药理作用。山药在胃肠道运化方面具有显著作用,有动物实验表明麸炒山药水提液中的二氯甲烷萃取物可以抑制小鼠的胃排空,其中二氯甲烷和正丁醇萃取物可以有效制止肠道的推进作用。傅紫琴等通过观察山药多糖对脾虚小鼠的影响,发现其能抑制胃肠排空,提高胸腺及脾脏指数,初步推断其提高免疫力是其补脾的关键之一。
山药具有抗炎、扶正固本、增强免疫的作用,可将其作为益生元制剂使用。从基因水平验证纳米山药多糖结肠靶向制剂具有扶植优势菌群如双歧杆菌的生长,从而抑制大肠杆菌等致病菌群生产的作用,增加肠道菌群丰富度,提高菌群总DNA数量,改善肠道菌群环境,有助于菌群失调的治疗。中药微生态调节剂具有调节微生态失调、保持微生态平衡的作用。山药多糖具有调节肠道微生态的作用。谭春爱等应用纳米山药多糖对SD大鼠的研究表明,功能性多糖具有促进肠道有益菌生长、抑制有害菌繁殖的作用。纳米山药多糖合生元结肠靶向微生态调节剂具有调节优势菌厚壁菌门的作用,使优势菌群数量增加。从纲的水平上看,纳米山药多糖合生元结肠靶向微生态调节剂使优势菌纲梭菌纲的数量增加,并且使芽孢杆菌纲和产芽孢菌纲的数量也增加,成为优势菌。
纳米山药多糖合生元结肠靶向胶囊通过调整肠道菌群失调,使肠道有益菌占优势,改善肠道屏障。肠道是人体内最大的细菌存储库,胃肠粘膜是最敏感的,当肠粘膜受损,肠道内细菌易位,甚至引起内毒血症,加速了危重病的发展。目前,已经证实内毒血症是肝衰竭继发多脏器衰竭的使动因素。因此,重视胃肠功能在肝衰竭的发展中的作用,早起防治肠道功能障碍对缓解肝衰竭病情,阻止其发展,降低死亡率具有十分重要的作用。有研究发现,灌胃较高剂量的山药多糖不仅能使小鼠的体重发生明显变化,而且对小鼠盲结肠内的微生物有显著影响,在抑制条件致病菌的同时,显著提高了益生菌的增殖。
与现有技术相比,本发明具有如下的有益效果:
本发明芡实,归于脾肾经,性味平甘涩,首载于《神农本草经》,具有益肾固精、补脾止泻、除湿止带的疗效,临床广泛用于遗精滑精,遗尿尿频,脾虚久泻,白浊带下等疾病的治疗。李湘利等的实验证实芡实多糖能够抑制细菌生长,而且对于金黄色葡萄球菌抑菌最强。於怀龙等的研究表明芡实原料的醇、水提取物合并后可以降低小鼠急性胃黏膜损伤和胃溃疡指数,为临床芡实在治疗急性黏膜损伤的应用提供新方向。
