Abstract
通过给宿主施用免疫学上有效量的螺杆菌脲酶或脲酶亚单位作为抗原,以使在哺乳动物宿主体内产生对螺杆菌感染的保护性免疫反应的方法。也提供疫苗组合物。
Description
本发明涉及预防和治疗哺乳动物(包括人)的胃感染,更具体说来,本发明涉及适用于预防和治疗哺乳动物(包括人)的螺杆菌(Helicobacter)感染的疫苗、以及涉及罹患胃感染(例如慢性胃炎或消化道溃疡)的人的治疗方法和预防胃癌。
人胃上皮的螺杆菌(Helicobacter)感染引起胃炎是发展成消化道溃疡和胃淋巴痛的主要因素,并可能是发展成胃癌的危险因素[1-3]。最常见的感染因子是幽门螺杆菌(Helicobacter Pylori).而由Helicobacter heilmanii引起的机率要小得多。幽门螺杆菌(H.Pylori)是一种细长S形革兰氏阴性微生物,它一般可从组织学上显示胃炎或消化道溃疡的成人和儿童的胃活组织检查中收集到。从对人类自愿受试者,患溃疡和胃癌的病人,限菌猪,以及无病菌啮齿动物的研究,可找到幽门螺杆菌(H.Pylori)和胃十二指肠病之间相关的证据。关于病原学,通过先前未感染的个体,随后消耗存活的微生物来从组织学确信罹患胃炎[4-11],并通过治疗根除幽门螺杆菌(H.Pylori)随之胃炎消除,并且患消化道溃疡的病人复发率降低[12]这些事实,证明Koch的假设是令人满意的。
尽管对许多抗生素在体外是敏感的,而以抗生素于体内根除出现的幽门螺杆菌(H.Pylori)感染却是通常难以成功的[13]。该微生物发现在覆盖于胃表皮的粘膜层中及胃窝中。即使以高剂量口服抗生素,这些位置也没有显示出达到所估计的足够的抗生素水平。最近,很多权威推荐所谓“三倍治疗法”,即将铋盐与诸如四环素和甲硝达唑等药物结合使用2-4星期。然而该方法或其它化疗制均不容乐观,而且该治疗法可能产生严重不良药物反应。
目前为止很少了解有关胃中粘膜免疫体系的作用。在正常胃窦中免疫球蛋白(Ig)产生细胞的分布表明,IgA浆细胞占总浆细胞群体的80%。此外,存在于胃窦中的浆IgA细胞数与其它粘膜差不多[14、15]。许多以人[16]和动物模式[8.10]进行的研究证明了,在血清和在对螺杆菌(Helicobacter)感染应答胃分泌物中,特异性IgG和IgA应答。然而所观察结果是幽门螺杆菌(H.Pylori)尽管诱导出局部和全身免疫反应,但由于它是多年慢性感染,因此并不能促进免疫策略的进展。
Lee等人曾报导过,能以Helicobacter felis(一种与幽门螺杆菌密切相关的细胞)感染无菌啮齿动物,并反复地记载患组织学胃炎[9.10]。自此,该细菌-宿主配对,对于研究螺杆菌介导的胃炎及其引发因子[17],作为一种良好的模式而被采用。Czinn等人证明,用幽门螺杆菌的粗溶菌液加上霍乱毒素佐剂反复口服接种,可在鼠和白鼬体内诱导出强烈的胃肠道IgA抗H.Pylori反应[13]。此外,Chen等人及Czinn等人最近报道,用H.felis粗溶菌液口服接种,在鼠体内诱导出抗H.felis感染的保护作用[21.22]。然而能诱导该保护作用的抗原(一种或多种)的确切性质尚未被测定出,也没有信息表明,诱导抗该病原体保护作用的H.felis保护抗原,将诱导出扩大到其它螺杆菌种的交叉反应保护作用。
我们首先证明了,用幽门螺杆菌或H.felis声处理物给鼠口服接种后,通过获得单克隆抗体幽门螺杆菌和H.felis共享抗原决定基,并且表明,抗幽门螺杆菌的这些抗体的某些,能与H.felis交叉反应,反之亦然[24;25]。这些交叉反应的基础是未知的。
根据不同已知脲酶氨基酸序列之间存在的同源性,有人已建议可用脲酶作为抗幽门螺杆菌的疫苗[26]。然而尽管不同脲酶序列中有同源性,但交叉反应却并不是普遍规律。Guo和Liu几年前证明Proteus mirabilis.Proteus vulgaris及Providencia rettgeri脲酶表现出相互有交叉反应性,而刀豆(jack bean)和Morganellamorganii脲酶则在免疫学上有别于前三种脲酶[23]。即使幽门螺杆菌脲酶与其它螺杆菌脲酶的抗原交叉反应是一种合理的假设,但是,直到我们证明某些H.felis单克隆抗体与幽门螺杆菌脲酶交叉反应[25]为止,并没有资料证明这确实是事实。J.Pappo进一步证明,受H.felis感染的鼠产生与幽门螺杆菌脲酶交叉反应,但不与刀豆脲酶交叉反应的抗体。(J.Pappo,未发表的资料,1993)。
以前A.Labigne在EPO 367,644[28]中已建议过用幽门螺杆菌脲酶或相关脲酶作为抗幽门螺杆菌感染的疫苗。然而,该申请并不包括以脲酶接种任何哺乳动物抗任何螺杆菌感染的内容。
而且,虽然与其它细菌脲酶的序列同源性可以支持使用脲酶作为抗幽门螺杆菌感染的疫苗候选物,但人类对幽门螺杆菌感染的现代知识却肯定不支持这种观点。第一,尽管事实是受感染的个体通常保持对脲酶很强的抗体反应,但抗脲酶免疫反应并不能导致消除或控制感.染。第二、幽门螺杆菌能将脲酶转移出细胞并从其表面脱落[19、20],这样脲酶可能不再代表为促进保护粘膜免疫反应的适当的靶。事实上,应考虑粘膜免疫保护作用是主要由分泌的IgA介导的,当识别抗原可由靶病原体排泄出并由此作为保护抗体的诱导物时,其粘结活性将受到损害。第三,在培养物中,脲酶看来对表皮细胞是有毒的,并被怀疑对粘膜破坏及体内消化溃疡起作用。因此将它作为抗原使用是有毒的。
然而,我们推论,假如满足下列条件,那么该抗原可能是一种有潜力的有效疫苗:
第一,我们以口服足够高剂量给药,以造成比天然出现的免疫反应较强的免疫反应;
第二,产生的抗体量高到足以粘连所有脲酶、脱落或不脱落;
第三,我们可使用脲酶亚单元或无毒的分子品种。
总之,目前仍存在着需要有效治疗及预防在人类中幽门螺杆菌诱导胃感染的方法。现有的资料表明生产抗该感染的疫苗的可能性,但并没有提供可加入到安全和有效的疫苗中的限定的抗原(对所有幽门螺杆菌菌株共同的)的清楚鉴定结果。
本发明中,我们鉴定了作为候选疫苗的幽门螺杆菌脲酶抗原,并在动物模式中证明了它的效力。这些结果以过去记载的天然的螺杆菌感染的观点来看是出乎予料的。
我们发现,利用显示于螺杆菌组织表面上或周围的脲酶抗原决定簇,并以之作为疫苗靶子,可以在易受胃肠道螺杆菌感染的哺乳动物体内诱导免疫性。该免疫性可由天然脲酶免疫作用诱导,但也可由作为酶促失活(因此是无毒的)形式产生的重组脲酶亚单位诱导出。本发明通过对哺乳动物粘膜表面施用一种多氨基酸制剂来提供诱导对螺杆菌感染的免疫作用的方法,该制剂即肽和/或蛋白与适当佐剂的混合物。该多氨基酸制剂存在多种抗原决定簇,它们以正感染的螺杆菌机体的内源脲酶为特征及表现形式。该多氨基酸制剂施药方式可以口服途径进行。
该制剂的活性成份可包含天然或生物合成的抗原决定簇,可以制成各种形状。可能的制剂的非包罗完全的名单包括提纯的天然或重组产生的细菌或其它来源的脲酶制剂,脲酶消化物,含脲酶抗原决定簇的融合蛋白,脲酶的截短形式或与脲酶氨基酸序列同源的肽。因为免疫性的发展取决于与感染螺杆菌机体相结合的体液和/或细胞免疫反应的诱导作用,优选的制剂是那些十分准确复制感染机体内源的脲酶抗原决定簇的制剂。例如,显示幽门螺杆菌脲酶抗原决定簇的制剂,对于施用于易受幽门螺杆菌感染的人是优选的,而显示H.heilmanii脲酶抗原决定簇的制剂,施用于易受H.heilmanii感染的人是优选的。然而,根据本发明的一个重要方面,已公开来自其它品种的脲酶也可使用。例如,我们已证明,施用来自幽门螺杆菌的脲酶可以预防鼠的H.felis感染。
根据本发明的一个方面,提供一种在哺乳动物宿主体内建立对螺杆菌感染的保护性免疫反应的方法,其中将能建立这样一种保护性免疫反应的免疫学上有效量的脲酶(优选幽门螺杆菌脲酶或幽门螺杆菌脲酶B亚单位)施用于宿主的粘膜表面。
根据本发明的另一方面,提供一种适用于预防螺杆菌感染的疫苗组合物,包含能在宿主体内建立对螺杆菌感染的保护性免疫反应的有效量脲酶抗原,优选幽门螺杆菌脲酶或幽门螺杆菌脲酶B亚单位,与药物学上可接受的载体或稀释剂相结合。
根据本发明的另一个方面,提供给与哺乳动物宿主对螺杆菌(Helicobacter)感染被动保护作用的方法,包括于宿主粘膜表面,施用能产生对螺杆菌(Helicobacter)感染起保护性免疫反应的,以脲酶(优选幽门螺杆菌脲酶或幽门螺杆菌脲酶B亚单位)接种的宿主体内产生的,免疫学上有效量的脲酶特异性抗体。
现本发明参考附图作进一步描述,其中图1至图6是表1至表6所述结果的图形说明。
本发明人发现给鼠口服显示幽门螺杆菌脲酶抗原决定簇的多氨基酸制剂,能在鼠(一种为研究对螺杆菌感染的免疫反应的有普遍采纳价值的动物模式[9])体内产生对H.felis的保护性免疫学反应。该保护性免疫反应的效果是,经免疫接种的动物,当受病原体攻击时,其与未接种的动物相比大大减少了受感染的机率。而且,本发明人发现了,对鼠以幽门螺杆菌脲酶B亚单位(作为酶促失活重组蛋白产生的)口服接种能产生鼠体内对H.felis的保护性免疫学反应。该保护性免疫反应的结果是,与未接种的动物(它们确定受到感染)相比,经接种的动物,在受病原体攻击时,大大减少了受感染的机率。
因此,首先本发明提供在哺乳动物宿主体内引发对螺杆菌感染的保护性免疫反应的方法,该方法包括对哺乳动物(包括人)粘膜表面施以能产生这种保护性免疫反应的、免疫学上有效量的脲酶抗原(优选幽门螺杆菌脲酶)。
第二方面,本发明提供在哺乳动物宿主体内,产生对螺杆菌感染的保护性免疫反应的方法。该方法包括给哺乳动物(包括人)的粘膜表面,施以能产生此种保护性免疫反应的、免疫学上重组有效量的、酶促失活的、作为抗原的脲酶B亚单位,优选重组幽门螺杆菌脲酶B亚单位。
本发明的范围内也包括治疗或预防哺乳动物动物(包括人)的螺杆菌感染,其中,将能产生对螺杆菌感染保护性免疫反应的,免疫学上有效量的脲酶或其亚单位施以患者的粘膜表面。优选的脲酶是幽门螺杆菌脲酶或幽门螺杆菌脲酶B亚单位。并且该脲酶可以单独用药,或与羟基化磷酸钙(例如作为载体颗粒的羟磷灰石)结合给药。此外优选幽门螺杆菌粘膜佐剂、霍乱毒素B亚单位,胞壁酰二肽或其它此类佐剂一起给药。
虽不与任何理论相关,但本发明人相信,以脲酶抗原或其B亚单位对粘膜表面给药,能刺激普通粘膜免疫体系,和或许诱导免疫反应的消化道粘膜中的局部位点,包括在消化分泌物中出现幽门螺杆菌特异性IgA抗体(它能预防螺杆菌感染)。因为在动物模式中,它是一种对人使用的候选疫苗进行临床前试验的常规方式,因此人们相信本发明的方法对人类是有效的,特别是预防和治疗消化道溃疡、胃炎、胃癌以及由于幽门螺杆菌和/或H.heilmanii存在所引起的其它症状。
A-细菌培养及脲酶纯化
用于研究的幽门螺杆菌菌株来源于患十二指肠溃疡的患者,并在BHI琼脂糖平板中再培养至均一。幽门螺杆菌在适当的培养基中培养,典型的是BHI(脑-心浸剂)培养基,包含0.25%酵母提取物和10%胎牛血清及补充了0.4%弯曲杆菌(Campylobacter)选择补体(Skirrow Supplement;Oxoid 69)。将该细菌于瓶中,在37℃微需氧条件下保温过夜,然后将瓶密封,于37℃振荡2-3天产生液体培养物。也可以在含BHI加有0.25%酵母提取物及5%山羊血的琼脂糖平板中,于37℃微需氧条件下培养3天来制备培养物。通过测660nm处BHI溶液的光密度来测定细菌的数量,一个光密度单位相当于108个细菌。琼脂糖平板上的培养物首先再悬浮于154mM NaCl中。
一种目前优选的显示脲酶抗原决定簇的多氨基酸源是经纯化的脲酶。例如,由采用经如下修改的Dunn等人(J.Biol.Chem.265,9464-9469)的一般方法,获得的幽门螺杆菌脲酶。培养之后,将幽门螺杆菌在水中收集,涡流状旋转并再次旋转产生上清液。将含脲酶活性物幽门螺杆菌(由快速脲酶试验测出,见下文)的溶液于CL-6B分级柱上进行色谱分离,将有很浓脲酶活性物的馏份汇集,并透吸过夜,于阴离子交换凝胶上再次进行色谱分离。在增加NaCl的缓冲液中洗脱出各馏份,将收集的含有浓脲酶活性物的馏份单独送至SDS凝胶,并随后以考马斯(Coomassie)染色。相应于分子量约63和28KDa的两个明显区别的谱带被鉴定为脲酶。将含脲酶的馏份汇集起来,得到纯度为95%-99%的经提纯的幽门螺杆菌脲酶。
B-以从幽门螺杆菌提纯的脲酶口服接种
虽然优选采用按上文所述获得的纯化幽门螺杆菌脲酶作为抗原材料,但应当明白能产生对螺杆菌感染的保护性免疫反应的任何脲酶或其亚单位,无论天然产生的或以以重组DNA技术获得的,以及其消化片段,含该片段或完整脲酶的融合蛋白,截短脲酶构件,或存在脲酶抗原决定簇的其它肽或蛋白制剂均能使用(见下文)。因此,采用与幽门螺杆菌脲酶本质上同源的,并能有效产生对螺杆菌Helicobacter交叉保护性免疫反应的脲酶是可能的。此类脲酶的例子是刀豆(jack bean)脲酶,它与幽门螺杆菌脲酶有70%同源性。因此本发明不限于使用完整的脲酶。本发明包括显示脲酶抗原决定簇,并能有效地在宿主中产生对螺杆菌感染保护性免疫学反应的任何多氨基酸制剂的使用。一般说来,具有70-95%与幽门螺杆菌脲酶同源的(例如80-90%同源)脲酶,都可在本发明中作为脲酶抗原使用。
有使用潜力的脲酶制剂来源,仅不完全的列举出来,即包括不同螺杆菌品种的内源脲酶、来自其它细菌、例如Klebsiella Pneumoniae或Proteus mirabilis的脲酶,以及由此类推,包括在该条件下幽门螺杆菌脲酶分享交叉反应抗原决定簇的那些脲酶的任何其它脲酶。上面所述所有生物体的脲酶基因,均代表表达重组脲酶产物为完整蛋白或其部分的有效工具。
有使用潜力的脲酶制剂,仅不完全列举,即包括由纯化脲酶(上面介绍过其来源),使用物理和/或化学裂解法(即CnBr)和/或蛋白水解裂法(使用蛋白酶、例如V8蛋白酶、胰蛋白酶或其它蛋白酶)产生的肽,或化学合成并与脲酶分享交叉反应抗原决定簇的肽。
其它有使用潜力的抗原决定簇的来源包括以脲酶交叉反应所鉴定的,以抗脲酶抗体筛选结果的抗原决定簇。这些肽可以是天然出现的肽或化学合成产生的肽。而且这类肽可以是从重组随机寡核苷酸表达的结果。
有使用潜力的抗原决定簇的另一个来源包括与脲酶相似,作为脲酶抗基因型抗体生殖结果的抗原决定簇。这类在任何免疫适格宿主体内产生的抗基因型抗体,由以抗脲酶抗体接种宿主,以产生针对抗脲酶抗体的抗体为目的而获得,它与脲酶分享结构同源性。
本文讨论的内容主要针对幽门螺杆菌天然产生的脲酶的应用(B部分)。然而,能产生所需保护性免疫反应的,上面介绍的脲酶或其亚单位或构件也是有价值的,它们可以通过本领域已知重组DNA技术来产生。具体制剂的效力,可以由动物模式常规给药、候选疫苗口服给药,并使用基本上与下述方法相似或相同的方法,以病原体进行攻击,来加以测定。
下表1和2及图1-5描述当用纯化幽门螺杆菌脲酶口服接种鼠时所获结果。在该第一个实验中,幽门螺杆菌抗原经口服给鼠给药,按A部分所述将幽门螺杆菌脲酶提纯并与羟磷灰石晶体结合,该羟磷灰石晶体用以作为载体,以提高M细胞结和吸收。加以霍乱毒素(Sigma)作为粘膜佐剂。该实验中、给各组雌性SPF BALB/C六星期龄鼠,于第0、7、14和21天,口服接种30μg提纯的幽门螺杆菌脲酶,结合1mg羟磷灰石,加10μg霍乱毒素佐剂。然后于第28及30天,以108H.felis攻击鼠两次。为了进行比较,给相似的雌性SPF BALB/C六周龄鼠,于第0、7、14及21天,口服接种完整幽门螺杆菌溶胞产物(声处理的)及10μg霍乱毒素。该鼠于第28天和30天,以H.felis攻击。该幽门螺杆菌声处理物系从细胞培养收集的幽门螺杆菌离心沉淀、并于0.9%氯化钠中于悬浮该沉淀,接着进行声处理制备而成。
给雌性SPF BALB/C六周龄鼠于第0、7、14及21天时口服假接种10μg霍乱毒素及1mg羟磷灰石作为空白对照。该鼠于第28及30天以H.felis攻击之。所有的鼠圈养,接种及攻击均平行进行。第35天将所有受试鼠杀死。
C-以幽门螺杆菌重组脲酶亚单位口服接种
根据以前公开的序列[29],按标准方法进行聚合酶链反应(PCR)克隆,获得编码幽门螺杆菌脲酶结构A和B亚单位的基因。将这些基因插入设计用于大肠杆菌中高水平表达,并容易纯化外来基因的载体[即pEV40]中。简单说来,将外来基因插入热抑制启动子的下游,于编码六组氨酸重复单元序列的框架内。该载体上有一amp R基因用于转化子选择。在适当温度条件下、将获得的重组蛋白在N末端补充以六组氨酸,使之能在镍柱上一步亲和提纯。将幽门螺杆菌重组脲酶A和B亚单位二者分别在大肠杆菌中表达并在镍柱上提纯至纯度>95%。
虽然优选采用按上述方法获得的重组幽门螺杆菌脲酶作为抗原利料,但应当了解,也可使用通过重组技术(例如融合蛋白)表达脲酶抗原位点获得的,能产生对螺杆菌感染保护性免疫反应的任何脲酶或脲酶亚单位,来作为抗原材料。因此,采用一个与幽门螺杆菌脲酶基本上同源,能产生对螺杆菌交叉保护性免疫反应的构建的脲酶基因是可能的。此类脲酶的例子是刀豆(jack bean)脲酶,它与幽门螺杆菌脲酶或H.felis脲酶具约70%同源性。因此,本发明不限于使用幽门螺杆菌脲酶基因及其基因产物,也覆盖使用能在宿主体内有效产生对螺杆菌(Helicobacter)感染的保护性免疫反应的任何重组脲酶或其亚单位。一般来说,与幽门螺杆菌脲酶具有70-95%同源性(例如80-90%同源性)的重组脲酶可用作本发明的重组脲酶抗原。
本文主要集中讨论有关由大肠杆菌(C部分)产生的重组幽门螺杆菌脲酶A和B亚单位的应用。但是,上面提到的能产生所需保护性免疫反应的重组脲酶或其亚单位或构建均是有价值的,它们可使用本技术领域已知的其它重组DNA技术及其它真核或原核表达载体来产生。
下面表3、4和5及图6描述当鼠以在大肠杆菌内产生的重组幽门螺杆菌脲酶亚单位口服接种而获得的结果。在该实验中,给鼠口服按上面所述在大肠杆菌中产生并提纯的重组幽门螺杆菌脲酶A或B亚单位,来进行幽门螺杆菌抗原给药,并结合羟磷灰石晶体,该羟磷灰石晶体用作载体并增加M细胞结合和吸取。霍乱毒素(Sigma)用作粘膜佐剂。在该实验中,于第0、8、14及21天,各组雌性SPFBALB/C六周龄鼠,均以30μg重组幽门螺杆菌脲酶A亚单位,结合1mg羟磷灰石加10μg霍乱毒素佐剂口服接种。然后于第32、34及36天以108H.felis三次攻击鼠。为比较的目的,于第0、8、14及21天,给相似的雌性SPF BALB/C六周龄鼠口服接种30μg重组幽门螺杆菌脲酶B亚单位结合羟磷灰石加10μg霍乱毒素。于第32、34和36天以H.felis攻击该鼠三次。给雌性SPF BALB/C六周龄鼠,以10μg霍乱毒素及1mg羟磷灰石于第0、8、14及21天口服假接种作为空白对照。然后于第32、34及36天以H.felis攻击该鼠。所有受试鼠接种及攻击均平行进行。在第48天(攻击后12天)或攻击后1D周将动物杀死。
D-胃活检、血及肠分泌物的分析
从胃取活组织检查并从心脏得到血,移出肠,在PBS缓冲液中以1mM PMSF(Boeringher)洗涤,得到肠分泌物用于ELISA分析。
为评价抗H.felis移出保护作用,根据厂商的说明书,采用Jatrox HP试验(Rohm Pharma)快速评估脲酶活性,将每只动物的胃活检进行筛选,确定H.felis的存在。简单地说,将胃活检浸入0.5ml厂商提供的脲和酚红(pH指示剂)混合物中。从脲中脲酶活性物产生氨和碳酸氢盐,接着通过结肠测定器(colometril)于550nm处测定溶液向较高吸收率处的改变。由分光光度计分析对脲酶活性物进行定量分析。
包括在B部分所述实验中各动物胃活检也可于BHI琼脂糖平板(按上面所述加以补充的)上培养,用于检测H.felis。在微需氧条件下培养3-10天后,通过革兰氏(Gram)染色及脲酶活性测定,可以肯定H.felis的存在。因为在第一组实验中,在胃活检中检测H.felis,脲酶试验和H.felis培养这两种方法之间获得十分明显的相关性(见表3),在C部分所述实验中仅进行胃活检脲酶试验来检测H.felis。由显微镜,使用两种不同的染色法(吖啶橙及甲酚紫),通过两个独立的观察者来证实H.felis检测。
将血样于RT凝聚3小时,收取血清并于-20℃冷冻至分析。将肠分泌物旋转5分钟,于4℃除去细胞碎片并于-20℃冷冻之。按照标准方法,以ELISA分析各动物的血清及肠样品,以评估抗螺杆菌活性。简单说来,以声处理的幽门螺杆菌覆盖96孔平板,接着用5%无脂奶使之饱和。将样品进行1∶1至1∶1000的系列稀释,并于ELISA平板上4℃保温过夜。使用生物素化抗鼠IgG(血清)及抗鼠IgA,然后用抗生蛋白链菌素-辣根过氧化酶来测定抗体水平。
在纯化幽门螺杆菌脲酶接种之后,以H.felis攻击后的结果列于表1-3中及图1-4中,而以重组幽门螺杆菌脲酶A和B亚单位接种后再以H.felis攻击的结果列于表4-6及图5和6中。

从表1和2的结果看出,与使用幽门螺杆菌声处理物或霍乱毒素相比,使用幽门螺杆菌脲酶口服接种所获得的对H.felis攻击,具统计学上明显的保护作用。就表2来说,可以看出,总共10只接种动物,只有3只受到感染,而相比之下,以幽门螺杆菌声处理物接种的动物有6只感染,而以霍乱毒素接种的有9只受到感染。表2表现出有70%的动物对H.felis攻击受到保护,相比之下对于随后经受H.felis攻击以幽门螺杆菌声处理物接种的动物仅33%被保护,而对于随后经受H.felis攻击以霍乱毒素接种的动物仅10%受到保护。换言之,空白对照鼠暴露于H.felis有90%受到病原体感染,而相反的,暴露于H.felis以前28天以幽门螺杆菌(H.Pylori)脲酶给鼠接种,感染率仅为30%。这表明了感染的明显降低(P=0.0198,在Fisher精确试验中与空白对照鼠相比)。当给鼠以幽门螺杆菌(H.Pylori)声处理物口服免疫时,感染率是67%(与空白对照相比差别不明显。使用幽门螺杆菌脲酶所获得的保护作用是未曾予料到的,并且不可能根据使用幽门螺杆菌声处理物所观察到的结果予测到。
有关图1-4,图1是以图形表示用脲酶接种之后,未受到保护的鼠体内的血清(IgG)及肠分泌物(IgA)中抗体试验结果。这些是表1中的第1、4和6号鼠,构成A组。图2表示用脲酶接种之后,受到保护的鼠的抗体反应(B组),即第2、3、5及7-10号鼠。图3和4是有关31-39号鼠所获得的结果,图3(C组)表示以幽门螺杆菌(H.Pylori)声处理物接种之后未受到保护的鼠的抗体反应(第31、32、33、35、36及38号鼠),而图4(D组)表示以幽门螺杆菌(H.Pylori)声处理物接种之后受到保护的鼠(第34、37及39号鼠)的抗体反应。非常有趣的注意到,就图3和4来说,表现出保护作用的鼠体内的IgA抗体反应(而非IgG)比起未受到保护的鼠体内来说要高,这表明保护作用与IgA反应之间的相关性。血清IgG反应没有表现出相关。已知粘膜IgA而非血清IgG抗体,在对抗内脏的细菌感染的保护作用中起作用[30]。
在胃活检中,用脲酶试验与通过培养来检测H.felis之间的相关结果列于表3中。

表3表明在胃活检中进行的脲酶试验的结果和由培养鉴定H.felis二者之间存在非常明显的相关性。因为通过培养较之脲酶试验较少能检测幽门螺杆菌感染,在下面的实验中,脲酶实验由于其灵敏度较好,而优选被用来诊断鼠的H.felis感染。该方法允许以各鼠胃的较大片断重脲酶试验,并进一步提高了脲酶试验的灵敏度。而且,使用最高灵敏度的方法可避免过高评估被试疫苗制剂所获得的保护作用。当阳性培养物用作感染标准时,在B部分所述实验中经脲酶接种之后所诱导的保护作用相当于以脲酶试验及培养结合使用时那么大(p=0.021对p=0.019)。
C部分所述实验根据胃活检脲酶试验所得结果列于表4、5和6中并描述于图6中。

从表4、5和6的结果可看出,与使用重组幽门螺杆菌脲酶A亚单位或霍乱毒素相比,使用重组幽门螺杆菌脲酶B亚单位口服接种获得统计学上明显的对抗H.felis攻击的保护作用。从表4中可看出,在脲酶B亚单位组,攻击后12天总共10只接种鼠中仅有3只发现感染,相比之下,以幽门螺杆菌脲酶A亚单位接种的10只和以霍乱毒素接种的10只均受感染。表4表明,70%动物受到保护免于I.felis攻击,相比之下,经受H.felis攻击后以幽门螺杆菌脲酶A亚单位及以霍乱毒素接种的动物受保护均为0%。换言之以H.felis攻击后空白对照鼠100%受到感染,反之,以重组幽门螺杆菌脲酶B亚单位接种的鼠感染率仅为30%。这表明与空白对照鼠相比感染明显下降(P=0.0031 Fisher精确试验)。
用幽门螺杆菌脲酶所观察到的保护作用完全由脲酶B亚单位免疫作用提供,而A亚单位没有此种效果这一事实,是根据我们以纯化脲酶所作实验未曾予料到的。因此对脲酶的该两结构亚单位在产生保护性免疫反应中的作用进行上述定义是新的。使用重组脲酶B亚单位(它被酶促失活)所获得的保护作用也告诉我们,脲酶的非毒性形式可以用作抗螺杆菌感染的口服疫苗。而且这些结果也有力地证明,活性位点的识别对保护作用并不需要,因为失活脲酶B亚单位非常不可能诱导出将识别和抑制天然脲酶催化位点的抗体。
从表6可以看出,当鼠在攻击后10周杀死时,以脲酶B亚单位接种的动物60%(10只中的6只),而以幽门螺杆菌脲酶A亚单位接种的动物80%(5只中的4只)对H.felis感染具抵抗作用。以脲酶B亚单位接种获得的保护作用持续时间延长,而以脲酶A接种所诱导的保护作用与由脲酶B亚单位诱导的保护作用相比滞后这一事实,从我们以纯化脲酶所作实验,或早期所作其它实验中不可能予料到的。脲酶A亚单位接种所给与的保护作用这一事实,肯定证明了活性位点的识别对保护作用是不需要的。图6总结了以重组脲酶A和B亚单位口服按种后所获得的结果(表5和6所述)。
本发明也提供适于予防螺杆菌感染的疫苗组合物,该组合物包含有效量的脲酶抗原,优选幽门螺杆菌脲酶或重组幽门螺杆菌脲酶亚单位(它能在宿主体内产生对螺杆菌感染的保护性免疫反应)与药物学上可接受的载体和稀释剂相结合。
本发明的疫苗给药量很容易由本领域技术人员来确定。因此,对成年人来说,适合剂量在10μg到100mg之间,例如50μg至50mg。相近的剂量范围对儿童也可用。用于人的载体体系可包括能保护抗原经受胃的酸性环境的肠释放胶囊,并包括作为融合蛋白不溶形式的脲酶抗原。该疫苗可作为一级予防剂对成人和儿童施用,在成功地根除感染宿主体内的幽门螺杆菌之后用作二级予防,或者可作为治疗剂,为在能根除幽门螺杆菌的宿主体内诱导免疫反应。
正如上面指出的,适当的粘膜佐剂是霍乱毒素,可以使用的其它佐剂是胞壁酰二肽或其衍生物,霍乱毒素无毒衍生物(包括其B亚单位),和/或脲酶抗原加霍乱毒素或其B亚单位的连接或基因工程融合物。其它适宜的输送方法包括可生物降解的微胶囊或免疫刺激复合物(ISCOM′S)或脂质体,基因工程减毒活载体,例如病毒或细菌,以及重组(嵌合)类病毒颗粒,例如兰舌病毒(bluetongue)。粘膜佐剂的用量取决于所用粘膜佐剂的类型。例如当粘膜佐剂是霍乱毒素,适宜用量为5μg到50μg,例如10μg至35μg。当以微胶囊形式使用时,用量取决于为达到所需剂量而在微胶囊基质中使用的量。该量的决定是本领域普通技术人员的技能所及。
本发明疫苗的适当载体是肠衣胶囊和聚交酯-乙交酯微球。适宜的稀释剂是0.2N碳酸氢钠和/或盐水。
微粒羟基化磷酸钙(HCP)作为幽门螺杆菌脲酶施用于粘膜表面的载体是特别有用的。确信幽门螺杆菌脲酶-羟化磷酸钙结合物穿过产生多Ig免疫反应的上皮。优选羟化磷酸钙为适于穿过上皮(特别是为此目的特化的细胞[M细胞])的微粒形式。羟化磷酸钙的优选形式是羟基磷灰石,一种市售的结晶羟磷酸钙Ca10(PO4)6(OH)2。
市售羟磷灰石一般由类似厚片的晶体组成,该晶体是正常骨组织中的无机羟磷灰石的化学和物理类似物。因此咽下羟磷灰石是安全的,正如来自用于咽下的磨细的骨粉所具有的营养钙/磷补充剂一样的作用。市售高分解羟磷灰石(购自CalBioChecm)由大小变化很大的晶体组成。长度超过1μm的晶体不大可能由M细胞吸收。因此为用于本发明,可用如声处理等技术将市售羟磷灰石晶体粉碎成小的十分均匀的结晶片。优选羟磷灰石基本上为约0.01-1.0μm的碎片,使用标准技术、可通过电子显微镜或光散射来测量其碎裂程度。
优选的幽门螺杆菌脲酶抗原的给药方式是口服、鼻注、直肠、眼滴。口服给药可以传输到包括肠粘膜在内的其它G、L、粘膜。
本发明的疫苗可以以气雾剂、悬浮液、胶囊和/或栓剂的形式施用于粘膜表面。给药方法对于本领域普通技术人员来说是显而易见的。
本发明进一步包括哺乳动物(包括人)对螺杆菌感染的被动免疫作用。该目的通过给患者粘膜表面施用有效量的脲酶特异性抗体来达到。优选给以有效量的幽门螺杆菌脲酶特异性IgA单克隆抗体。
因为已证明幽门螺杆菌脲酶代表与诱导保护性免疫相关的抗原,因此本发明进一步涉及的方面是使用幽门螺杆菌脲酶作为诊断剂,用来测定已接受基于脲酶的疫苗的个人的免疫反应、或测定某个体是具免疫力的还是易受感染的(因而需要接种)。本发明也包括使用脲酶和脲酶特异性抗体以建立用于诊断螺杆菌免疫力、评估螺杆菌的易感性,定义对疫苗的免疫反应的测试方法及药盒。
实施例
通过参考下述非限制性实施例现对本发作作进一步描述。
a)细菌菌株
H.felis由J.Fox(division of Coperative Medicine,Mass,Institute of Technology,Boston,USA),幽门螺杆菌由患溃疡的病人体内分离出(CHUV,Lausanne,Switzerland)。
b)细菌培养
液体培养-在含有0.25%酵母提取物(Difco)及10%胎牛血清(Inoteeh)补充有0.4%弯曲杆菌(Campylobaeter)选择补体(Oxoid)的BHI(脑-心浸剂,BioMerieux)液体培养基中培养细菌。于37℃、微需氧条件下将该细菌保温过夜并于37℃振荡2-3天。
琼脂糖平板培养-在由加有0.25%酵母提取液和5%羊血的BHI组成的琼脂平板上,于37℃微需氧条件下将细菌培养3天。
定时-由BHI溶液于660nm波长处的光密度来定量测定细菌数(1光密度单位相当于108个细菌)。
c)声处理物的制备
将幽门螺杆菌从31个血琼脂平板上收集到0.15M NaCl中,并于4℃ 1400g下旋转5分钟。将沉淀再悬浮于3ml NaCl中并声处理4分钟。以Bradford检测法估测蛋白质的量(根据厂商的BioRad Kit)。
d)免疫原与羟磷灰石结合
将免疫原(脲酶或其亚单位)与羟磷灰石于4℃培养1小时,每鼠使用30μg免疫原时羟磷灰石为1.0mg。培养结束,加入10μg霍乱毒素,最终体积为200μl PBS。
实施例1
a)提取
将从30个血琼脂平板上收集到的幽门螺杆菌置于冰上的0.15MNaCl中,并于4℃ 1400g下将该溶液旋转5分钟。将所得沉淀再悬浮于20ml水中并涡旋45秒(最大速度)。将提取液于4℃6700g下旋转20分钟。回收上清液并进行蛋白定量测定(见上述定量一节),再以70%硫酸氨沉淀之。
b)脲酶的提纯
在Sepharose CL-6B柱(Pharmacia)上,以PBS(磷酸盐缓冲盐水)作为流动相将溶液进行色谱分离。将22个表现出强脲酶活性的收集馏分汇集在一起,并于4℃,以3升PEB(20mM磷酸缓冲液,PH7)透析过夜,然后在Q Sepharose快速流动柱(Pharmacia)上以PBS作为流动相进行色谱分离。由0-500mM NaCl的梯度液洗脱出各馏分。将10个显示强脲酶活性的收集馏分独立地进行SDS凝胶分析,然后进行考马斯(Coomassie)染色。将代替2个不同谱带,相当于MW-63和-28KDa的6个馏分汇集在一起,这些被认为是纯化的脲酶。
实施例2(也见B部分)
在胃内接种之前,将用于接种研究的鼠以乙醚轻度麻醉。然后,将声处理制剂或纯化脲酶,羟磷灰石和霍乱毒素悬浮于PBS,使用连接于皮下注射器的聚乙烯管,将200μl该液输送到各鼠的胃中。该方法作为口服接种法。
对三种口服接种方案进行评估,叙述如下:
方法B1-以纯化的脲酶接种
在第0、7、14及21天,以30μg纯化的幽门螺杆菌脲酶及1mg羟磷灰石和10μg霍乱毒素,给雌性BALB/c六周龄鼠(20只)口服接种。在第28和30天,以5×107和108来自液体培养的H.felis攻击10只鼠。
方法B2-以螺杆菌的声处理液接种
在第0、7、14及21天,用2mg幽门螺杆菌声处理物溶液给雌性BALB/c六周龄鼠(20只)口服接种。在第28和30天,以5×107及108 H.felis攻击10只鼠。
方法B3-空白对照
在第0、7、14及21天,用1mg羟磷灰石及10μg霍乱毒素给雌性BALB/c六周龄鼠口服接种。在第28和30天,以5×107及108 H.felis攻击该鼠。
在第35天将所有鼠杀死,从胃取活检并取肠分泌物及血。
保护作用及评估
为评估保护作用,采用Jetrox HP试验(Rohm Pharma),根据厂商的说明,对活检进行脲酶活性检查。于550nm波长处进行分光光度测量对脲酶定量分析。该活检也于H.felis存在下培养,并通过革兰氏染色估测。将胃窦活检进行匀浆处理并在0.15M NaCl中稀释(1∶10及1∶1000),铺于血琼脂平板上并于37℃微需氧条件下培养4-10天。
ELISA
以ELISA分析肠分泌物及血,以评估抗体效价。ELISA按下述进行:于37℃按1μg/孔在聚乙烯平板(96孔)上涂复纯化的脲酶2小时。以5%奶粉的PBS 0.1%Tween液于37℃将非特异性位点阻隔30分钟。用PBS 0.1%Tween洗平板一次。血样品以1∶1000稀释度试验,肠分泌物以1∶1试验。将各样品100μl加入到涂复抗原的板上。保温2小时后,用PBS 0.1%Tween洗板3次。除了IgA(以1∶250加入)外,来自山羊的抗鼠生物素化完整抗体及抗鼠IgA、IgG和IgM生物素化物(Amersham)均按1∶500稀释度加入(100μl),并于37℃培养1小时。用PBS 0.1%Tween洗该板3次,并加入100μl按1∶1000稀释的抗生蛋白链菌素辣根过氧化酶以PBS 0.01%Tween液,再于37℃培养30分钟。将该板洗三次,并加入50μl以1∶50稀释的O-苯基-二胺的柠檬酸缓冲液(PH5.0,含1μl/ml30%H2O),于室温下培养20分钟,测量每孔于495nm处的吸收率。
实施例3(也见C部分)
进行胃内接种之前用乙醚将用于免疫接种研究的鼠轻度麻醉。然后,将30μg大肠杆菌中产生的重组幽门螺杆菌脲酶A或B亚单位与羟磷灰石结合,并补充以霍乱毒素悬浮于PBS液中。使用连接于聚乙烯管的皮下注射器,将200μl该液输送于各鼠胃中。该方法作为口服接种法。
评估三种口服接种法,叙述如下:
方法C1-以重组脲酶A亚单位接种
在第0、8、14及21天,以30μg纯化的重组幽门螺杆菌脲酶A亚单位和10mg羟磷灰石及10μg霍乱毒素给雌性BALB/c六周龄鼠(10只)口服接种,于第32、34及36天用来自液体培养的108 H.felis攻击10只鼠。
方法C2-用重组脲酶B亚单位接种
在第0、8、14及21天,以30μg重组幽门螺杆菌脲酶B亚单位和1mg羟磷灰石及10μg霍乱毒素给雌性BALB/c六周龄鼠(10只)口服接种,于第32、34及36天用来自液体培养的108 H.felis攻击10只鼠。
方法C3-空白对照
在第0、8、14及21天,用1mg羟磷灰石和10μg霍乱毒素给雌性BALB/c六周龄鼠口服接种,于第32、34及36天用108 H.felis攻击该鼠。
在第42天或第106天将鼠杀死,从胃中取多份活检。
保护作用及评估
为评估保护作用,采用Jetrox HP试验(Rohm Pharma),根据厂商的说明,对胃体及胃窦活检进行脲酶活性检查。于550nm波长处进行分光光度测量以对脲酶定量分析。总胃体和胃窦OD值相加得到每鼠最终OD值。
